Abstract:IntroductionSalt intake is very high in China, with ≈80% being added by the consumers. It is difficult to reduce salt in such settings. Our previous study (School-based Education programme to reduce Salt(School-EduSalt)) demonstrated that educating schoolchildren, who then instructed their families to reduce the amount of salt used at home, is effective in lowering salt intake in both children and adults. Our team also developed an app called ‘KnowSalt’, which could help individuals to estimate their salt intake and the major sources of salt in the diet. Building on School-EduSalt and KnowSalt, we propose to develop a new app (AppSalt) focusing on salt reduction through education, target setting, monitoring, evaluation, decision support and management to achieve a progressive lower salt intake for long term. To evaluate the effectiveness of the AppSalt programme, we will carry out a cluster randomised controlled trial.Methods and analysisWe will recruit 54 primary schools from urban and rural areas of three provinces in China. A total of 594 children aged 8–9 years and 1188 adult family members will be randomly selected for evaluation. After baseline assessment, schools will be randomly allocated to either the intervention or control group. Children in the intervention group will be taught, with support of AppSalt, about salt reduction and assigned homework to get the whole family involved in the activities to reduce salt consumption. The duration of the intervention is two school terms (ie, 1 year). The primary outcome is the difference between the intervention and control group in the change of salt intake as measured by 24-hour urinary sodium.Ethics and disseminationThe study has been approved by Queen Mary Research Ethics Committee and Peking University Health Science Centre IRB. Process Evaluation of an Application-Based Salt Reduction Intervention in School Children and Their Families (AppSalt) in China: A Mixed-Methods Study

Abstract:Background Salt reduction is a cost-effective, and rather challenging public health strategy for controlling chronic diseases. The AppSalt program is a school-based multi-component mobile health (mhealth) salt reduction program designed to tackle the high salt intake in China. This mixed-methods process evaluation was conducted to investigate the implementation of this program across sites, identify factors associated with the implementation, and collect evidence to optimize the intervention design for future scale-up. Methods Mixed methods were used sequentially to collect data regarding five process evaluation dimensions: fidelity, dose delivered, dose received, reach, and context. Quantitative data were collected during the intervention process. Participation rate of intervention activities was calculated and compared across cities. The quantitative data was used for the selection of representative intervention participants for the qualitative interviews. Qualitative data were collected in face-to-face semi-structured interviews with purposively selected students ( n = 33), adult family members ( n = 33), teachers ( n = 9), heads of schools ( n = 9), key informants from local health, and education departments ( n = 8). Thematic analysis technique was applied to analyze the interview transcripts using NVivo. The qualitative data were triangulated with the quantitative data during the interpretation phase. Results The total number of families recruited for the intervention was 1,124. The overall retention rate of the AppSalt program was 97%. The intervention was implemented to a high level of fidelity against the protocol. About 80% of intervention participants completed all the app-based salt reduction courses, with a significant difference across the three cities (Shijiazhuang: 95%; Luzhou: 73%; Yueyang: 64%). The smartphone app in this program was perceived as a feasible and engaging health education tool by most intervention participants and key stakeholders. Through the interviews with participants and key stakeholders, we identified some barriers to implementing this program at primary schools, including the left-behind children who usually live with their grandparents and have limited access of smartphones; perceived adverse effects of smartphones on children (e.g., eyesight damage); and overlooked health education curriculum at Chinese primary schools. Conclusion This process evaluation demonstrated the feasibility and acceptability of using smartphone applications delivered through the education system to engage families in China to reduce excessive salt intake. Abstract:Objective To determine whether an education programme targeted at schoolchildren could lower salt intake in children and their families. Design Cluster randomised controlled trial, with schools randomly assigned to either the intervention or control group. Setting 28 primary schools in urban Changzhi, northern China. Participants 279 children in grade 5 of primary school, with mean age of 10.1; 553 adult family members (mean age 43.8). Intervention Children in the intervention group were educated on the harmful effects of salt and how to reduce salt intake within the schools’ usual health education lessons. Children then delivered the salt reduction message to their families. The intervention lasted for one school term (about 3.5 months). Main outcome measures The primary outcome was the difference between the groups in the change in salt intake (as measured by 24 hour urinary sodium excretion) from baseline to the end of the trial. The secondary outcome was the difference between the two groups in the change in blood pressure. Results At baseline, the mean salt intake in children was 7.3 (SE 0.3) g/day in the intervention group and 6.8 (SE 0.3) g/day in the control group. In adult family members the salt intakes were 12.6 (SE 0.4) and 11.3 (SE 0.4) g/day, respectively. During the study there was a reduction in salt intake in the intervention group, whereas in the control group salt intake increased. The mean effect on salt intake for intervention versus control group was −1.9 g/day (95% confidence interval −2.6 to −1.3 g/day; P<0.001) in children and −2.9 g/day (−3.7 to −2.2 g/day; P<0.001) in adults. The mean effect on systolic blood pressure was −0.8 mm Hg (−3.0 to 1.5 mm Hg; P=0.51) in children and −2.3 mm Hg (−4.5 to −0.04 mm Hg; P<0.05) in adults. Conclusions An education programme delivered to primary school children as part of the usual curriculum is effective in lowering salt intake in children and their families. This offers a novel and important approach to reducing salt intake in a population in which most of the salt in the diet is added by consumers. Trial registration ClinicalTrials.gov NCT01821144.

Abstract:Objective: To determine the effects of salt reduction interventions designed for home cooks and family members. Design: Cluster randomised controlled trial. Setting: Six provinces in northern, central, and southern China from 15 October 2018 to 30 December 2019. Participants: 60 communities from six provinces (10 communities from each province) were randomised; each community comprised 26 people (two people from each of 13 families). Interventions: Participants in the intervention group received 12 month interventions, including supportive environment building for salt reduction, six education sessions on salt reduction, and salt intake monitoring by seven day weighed record of salt and salty condiments. The control group did not receive any of the interventions. Main outcome measure: Difference between the two groups in change in salt intake measured by 24 hour urinary sodium during the 12 month follow-up. Results: 1576 participants (775 (49.2%) men; mean age 55.8 (standard deviation 10.8) years) from 788 families (one home cook and one other adult in each family) completed the baseline assessment. After baseline assessment, 30 communities with 786 participants were allocated to the intervention group and 30 communities with 790 participants to the control group. During the trial, 157 (10%) participants were lost to follow-up, and the remaining 706 participants in the intervention group and 713 participants in the control group completed the follow-up assessment. During the 12 month follow-up, the urinary sodium excretion decreased from 4368.7 (standard deviation 1880.3) mg per 24 hours to 3977.0 (1688.8) mg per 24 hours in the intervention group and from 4418.7 (1973.7) mg per 24 hours to 4330.9 (1859.8) mg per 24 hours in the control group. Compared with the control group, adjusted mixed linear model analysis showed that the 24 hour urinary sodium excretion in the intervention group was reduced by 336.8 (95% confidence interval 127.9 to 545.7) mg per 24 hours (P=0.002); the systolic and diastolic blood pressures were reduced by 2.0 (0.4 to 3.5) (P=0.01) and 1.1 (0.1 to 2.0) mm Hg (P=0.03), respectively; and the knowledge, attitude, and behaviours in the intervention group improved significantly. Conclusions: The community based salt reduction package targeting home cooks and family members was effective in lowering salt intake and blood pressure. This intervention has the potential to be widely applied in China and other countries where home cooking remains a major source of salt intake. Abstract:The School-based Education Program to Reduce Salt Intake in Children and Their Families study was a cluster randomized control trial among grade five students in 28 primary schools and their families in Changzhi, China. It achieved a significant effect in lowering systolic blood pressure (SBP) in all family adults by 2.3 mmHg and in elderlies (aged > = 60 years) by 9.5 mmHg. The aim of this study was to assess the cost-effectiveness of this salt reduction program.Costs of the intervention were assessed using an ingredients approach to identify resource use. A trial-based incremental cost-effectiveness ratio (ICER) was estimated based on the observed effectiveness in lowering SBP. A Markov model was used to estimate the long-term cost-effectiveness of the intervention, and then based on population data, extrapolated to a scenario where the program is scaled up nationwide. Findings were presented in terms of an incremental cost per quality-adjusted life year (QALY). The perspective was that of the health sector.The intervention cost Int$19.04 per family and yielded an ICER of Int$2.74 (90% CI: 1.17-12.30) per mmHg reduction of SBP in all participants (combining children and adult participants together) compared with control group. If scaled up nationwide for 10 years and assumed deterioration in treatment effect of 50% over this period, it would reach 165 million families and estimated to avert 42,720 acute myocardial infarction deaths and 107,512 stroke deaths in China. This would represent a gain of 635,816 QALYs over 10-year time frame, translating into Int$1,358 per QALY gained.Based on WHO-CHOICE criteria, our analysis demonstrated that the proposed salt reduction strategy is highly cost-effective, and if scaled up nationwide, the benefits could be substantial.ClinicalTrials.gov NCT01821144.

If scaled up nationwide for 10 years, it would reach 93.75 million families and would avert 22,914 acute myocardial infarction deaths and 61,506 stroke deaths in China. This would represent a gain of 380,694 QALYs and a saving of US$1.0 billion in direct medical cost over a 10-year time frame, translating into US$1,711 per QALY gained. This estimation is much lower than the cost-effective threshold of cost per QALY of $50,000, and lower than the WHO-CHOICE highly cost-effective threshold of once the country's annual gross domestic product per capita ($7,595 for China in 2014). Conclusions: Our analysis demonstrated that the proposed salt reduction strategy is highly cost-effective, and if scaled up nationwide, the benefits would be substantial.

Abstract:We determined whether a town-level comprehensive intervention program could lower the salt intake of a population. The parallel, cluster randomized controlled trial was carried out between October 2018 and January 2020 in 48 towns from 12 counties across 6 provinces in China. All participants were asked to complete the 24 h urine collections, anthropometric measurements and questionnaires at the baseline and one-year post-intervention survey. A total of 2693 participants aged 18 to 75 years were recruited at the baseline. A total of 1347 individuals in 24 towns were allocated to the intervention group and the others were allocated to the control group. Valid information from 2335 respondents was collected in the follow-up survey. The 24-h urinary sodium excretion was 189.7 mmol/24 h for the intervention group and 196.1 mmol/24 h for the control group at baseline. At a one-year follow-up, the mean effect of salt intake did not show a significant change (p = 0.31) in the intervention group compared to the control group. However, the mean result of potassium excretion in the intervention group increased by 2.18 mmol/24 h (85.03 mg/24 h) (p = 0.004) and systolic blood pressure decreased by 2.95 mmHg (p < 0.001). The salt-related knowledge and attitude toward salt reduction improved significantly in the intervention group (p < 0.05). A longer period of intervention and follow-up assessment might be needed to evaluate the long-term effectiveness of the program on salt reduction.

Abstract:Objective: A previous 12-month randomised controlled trial (RCT) demonstrated the effectiveness of an application-based education program, conducted through schoolchildren, in reducing the salt intake and systolic blood pressure of their adult family members. This study aimed to assess whether the effect at 12 months persisted at 24 months. Design and method: As a follow-up survey of a cluster RCT with 54 schools randomly assigned to either the intervention or control group, all participants who completed the baseline survey were contacted again at 24 months, following a 12-month non-intervention period after the RCT. The primary outcome was the difference in salt intake change between the intervention and control groups at 24 months versus baseline and 12-month follow-up, measured by the mean two consecutive 24-hour urinary sodium excretions. The secondary outcome was the difference in blood pressure change. Results: At 24 months, 542 children and 974 adults were followed up, accounting for 91.6% and 82.3% of the participants, respectively. The mean salt intake in adults at baseline, 12 months and 24 months was 10.0 g/day (standard deviation 3.5), 8.9 g/day (standard deviation 3.2) and 9.4 g/day (standard deviation 3.5) in the intervention group and 10.0 g/day (standard deviation 3.6), 9.8 g/day (standard deviation 3.9) and 9.8 g/day (standard deviation 3.9) in the control group. The mean difference in salt intake change in adults between the intervention and control groups after adjusting for confounding factors is -0.38 g/day at 24 months versus baseline (95% confidence interval -0.81 to 0.05, P=0.086), and 0.45 g/day at 24 months versus 12 months (95% confidence interval 0.01 to 0.85, P=0.046). The mean difference in systolic blood pressure change after adjusting for confounding factors is -2.19 mm Hg (95% confidence interval -3.63 to -0.76, P=0.003) at 24 months versus baseline and -0.40 mm Hg (95% confidence interval -1.86 to 1.07, P=0.596) at 24 months versus 12 months. Conclusions: The effect of the education program on adults’ salt intake faded, but the systolic blood pressure lowering effect remained 12 months after the RCT. Continuous salt reduction education is recommended to maintain the effect.

Abstract:Objectives Healthy behaviour changes, such as reducing salt intake, are important to prevent lifestyle-related diseases. Social environment is a major challenge to achieve such behaviours, but the explicit mechanisms remain largely unknown. We investigated whether social networks of children were associated with their behaviours to reduce salt intake. Design An ancillary study of a school-based cluster randomised controlled trial to reduce salt intake in children and their families (School-EduSalt), in which salt intake of children was significantly reduced by 25%. Setting 14 primary schools in urban Changzhi, northern China. Participants 603 children aged 10-12 years in the intervention arm. Primary and secondary outcome measures We developed a score assessing salt-reduction behaviours (SRB score) of children based on self-administered questionnaires. The SRB score was validated by the changes in salt intake measured by 24-hour urine collection in a random sample of 135 children. A 1-unit increase in SRB score was associated with a 0.31 g/day greater reduction in salt intake during the trial (95% CI 0.06 to 0.57, p=0.016). Results Children from families with more family members not supporting salt reduction had significantly lower SRB scores (p<0.0001). Children from a class with a smaller size and from a class with more friendship connections, as well as children having more friends within the class all showed higher SRB scores (all p<0.05). Children whose school teachers attended the intervention programme more frequently also had higher SRB scores (p=0.043). Conclusion Social networks were associated with the behaviours to reduce salt intake in children. Future salt-reduction programmes may benefit from strategies that actively engage families and teachers, and strategies that enhance interconnectivity among peers.
Effect of Salt Reduction on Iodine Status in Children and Their Families in Northern China: a Sub-Study of a Cluster-Randomised Controlled Trial

Abstract:BackgroundSalt reduction is a very cost-effective measure to reduce the risk of cardiovascular disease. Meanwhile, salt has been used as a vehicle for iodine fortification to prevent iodine deficiency. It is therefore important to monitor iodine status after salt intake is reduced.MethodsThis is a prespecified sub-study of the School-EduSalt trial (School-based Education Programme to Reduce Salt), a cluster-randomised controlled trial in Changzhi, northern China, where universal salt iodisation is mandatory. Primary schools were randomly assigned (1:1) to either the intervention or the control group with stratification by the location of schools (ie, urban or suburban) and the size of the class. The randomisation was done using a computer-generated random number system by an independent statistician who was blinded to the identity of the schools. The randomisation took place after written consents had been obtained and the baseline assessments had completed. Therefore, the participants, the school teachers, and the local investigators who undertook participant recruitment and data collection, were unaware of the allocation until the point prior to the commencement of the intervention. Children were eligible for inclusion if they ate homemade meals for at least 3 days a week and if their home was less than 3 km from the school. From each child's family we also enrolled two adults who shared the same meals with the child. Children in the intervention group were educated on how to reduce salt. They then delivered the message to their families. The duration was one school term (about 3·5 months). Urinary iodine was measured by ammonium persulfate digestion with spectrophotometric detection of the Sandell-Kolthoff reaction. The primary outcome was iodine intake as measured by repeat 24 h urinary iodine excretion. All participants who entered the School-EduSalt trial were included in this sub-study. The analysis was carried out on intention-to-treat basis. We used linear mixed models with adjustment of clustering and potential confounding factors. Logarithmic transformed iodine was used in the analysis. This trial is registered with ClinicalTrials.gov, number NCT01821144.FindingsWe enrolled 28 primary schools: 279 children (mean age 10·1 years [SD 0·5] and 553 adults (43·8 years [12·2]). At baseline, the mean salt intake was 7·0 g/d (SD 2.5) for children and 11·7 g/d (4·4) for adults, and the median iodine intake was 165·1 μg/d (IQR 122·6–216·7) for children and 280·7 μg/d (IQR 205·1–380·9) for adults. During the study both salt and iodine intake decreased in the intervention group compared with the control group. The mean effect on salt for intervention versus control was –1·9 g/day (95% CI –2·6 to –1·3; p<0·0001) in children and –2·9 g/day (–3·7 to –2·2; p<0·0001) in adults. The mean effect on iodine was –19% (–29% to –8%; p=0·002) in children and –11% (–20% to –1%; p=0·03) in adults.InterpretationA 25% reduction in salt intake was associated with a significant reduction in iodine consumption in northern China where salt is iodised. Despite this, iodine intake was still adequate. Children were educated about the harmful effects of salt on health and how to reduce salt intake during their usual health education lessons. Children then instructed their families to reduce salt consumption. The intervention duration was one school term (≈3.5 months). Results: At baseline, the mean salt intake was 7.0 ± 2.5 g/d in children and 11.7 ± 4.4 g/d in adults and the median iodine intake was 165.1 μg/d (IQR: 122.6–216.7) and 280.7 μg/d (IQR:205.1–380.9) in children and adults respectively. At the end of the study, both salt and iodine decreased in the intervention compared with control group. The mean effect on iodine was −19.3% (95%CI: −29.4% to −7.7%) in children and -11.4% (95%CI: −20.3% to −1.5%) in adults. Conclusions: Our study, for the first time, has assessed salt and iodine intake by repeat 24 h urine collections. The School-EduSalt programme successfully reduced salt intake by ≈25% in both children and adults. Despite there being an accompany reduction in iodine, the median iodine intake was still adequate and well above the estimated average requirement for both children and adults. Our findings indicate that the WHO's salt target of 30% reduction by 2025 can be achieved without compromising iodine status.

Abstract:Background: Salt intake in China was high and a series of salt reduction measures were accordingly carried out recently. Our study aimed to assess the long-term effect of a scale-up community randomized controlled trial (RCT); Methods: Individuals between the ages of 18 and 75, from six provinces in China, were recruited and randomized into control (n = 1347) and intervention (n = 1346) groups. A one-year salt reduction intervention was first implemented in the intervention group, followed by a two-year scale-up intervention in both groups. The 24 h urine sample, anthropometric measurement, and knowledge, attitude, and practice (KAP) of salt reduction, as well as lifestyle information, were collected at baseline, after one-year RCT (mid-term evaluation, n = 2456), and two-year scale-up intervention (terminal evaluation, n = 2267); Results: Both control (351.82 mg/24 h, p < 0.001) and intervention (192.84 mg/24 h, p = 0.006) groups showed a decrease in 24 h urinary sodium excretion from baseline to terminal evaluation. Except for an increase in 24 h urinary potassium excretion (85.03 mg/24 h, p = 0.004) and a decrease in systolic blood pressure (SBP) (2.95 mm Hg, p < 0.001) in the intervention group at the mid-term assessment, no statistically significant differences in other indicators were found between two groups. The KAP of salt reduction in two groups was gradually improved; Conclusions: After one-year RCT two-year scale-up, all participants showed a decreasing trend in 24 h urinary sodium excretion and an increase in salt reduction KAP. The community salt reduction intervention package has the potential for broader application across other regions in China.

Abstract:Introduction Salt intake in China is high, and most of it comes from that added by consumers. Nevertheless, recent years have seen a rapid increase in the frequency at which people eat out. The aim of this study is to evaluate the effectiveness of interventions designed for salt reduction in restaurants through a randomised controlled trial in China. Methods and analysis As a randomised controlled trial with restaurants as study subjects, we recruited 192 restaurants from 12 counties of 6 provinces in China. After the baseline survey, restaurants were randomly assigned to intervention or control group. Using social cognitive theory, comprehensive intervention activities were designed to encourage salt reduction in all restaurant foods, and at the same time, to encourage consumers to choose lower salt options when eating out. The interventions will be conducted only in restaurants of the intervention group during the first year. The follow-up assessment will be conducted at the end of the trial. The primary outcome is the change in the average salt content of the five best-selling dishes of the restaurant, as measured by laboratory tests. Secondary outcomes include differences in the monthly use of salt and salty condiments between intervention and control restaurants, and the knowledge, attitude and practice on salt among restaurant consumers. Ethics and dissemination The study was reviewed and approved by the Review Board of the National Institute for Nutrition and Health, Chinese Center for Disease Control and Prevention and Queen Mary Research Ethics Committee. Results will be disseminated through presentations, publications and social media. Trial registration number ChiCTR1800019694; Pre-results.

Abstract:Objective: To identify the social determinants shaping children's behaviour on salt reduction from a network perspective. Design and method: School-EduSalt is a cluster-randomized controlled trial carried out in 28 primary schools in northern China involving 279 families (279 children aged 10.1 ± 0.5yrs and 553 adults). A 3.5-month education on salt reduction was delivered to the 14 schools (classes) in the intervention group, of which children and their families achieved a significant reduction in salt intake as measured by 24-hour urine sodium. Information on ego-centric family networks was collected for the 279 children while an additional post-trial survey was conducted for all the children in the 28 recruited classes (n = 1273) to obtain information on children's peer networks and behaviour change. Mixed models and exponential random graph (ERG) models was used in network analysis. Network was visualized using NetDraw. Results: The reduction of salt intake in children closely correlated with that of their adult family members (r = 0.4, P < 0.001). Children whose father held a supportive attitude had a 0.9 g/d greater reduction in salt intake (P = 0.04), as were children whose families attended more frequently (P = 0.02) in the intervention activities after adjusting for age, sex, BMI and baseline salt intake. Moreover, children who were embedded in a network with larger density (i.e. better interconnected) and those who had a greater degree centrality (i.e. more “popular”) had significantly higher score on salt reduction with adjustment of age, sex and class size (P = 0.048 and P = 0.03, respectively). Conclusions: Both family and peer networks significantly influence children's behaviour on salt reduction from multiple different aspects. Our study is the first to have explored the effect of social network on behaviour of salt reduction and demonstrated the importance of targeting the macro networks individuals embedded in. Our study offers novel insight into the social determinants of behaviour change and provides evidence to optimize future salt-reduction strategies.
Changes of urinary sodium and potassium in children and their families after health education among school children

Abstract:Objective The aim of this study was to determine whether an education programme targeted at schoolchildren could lower salt intake in children and their families.Methods Every class was chosen randomly from 28 primary schools'per grade five in Changzhi urban area and divided into intervention group and control group randomly.Then 10 students and their adult family members wererandomly selected as research objects.The students in intervention group were educated on the harmful effects of salt and how to reduce salt intake within the schools' usual health education lessons.Children then delivered the salt reduction message to their families.24h-urine sodium,potassium was measured in the two groups' students and their parents before and after health education.Results After health education,in the intervention group adults' 24h-urine sodium decreased 35.03±73.60mmol compared with that before the intervention.In the matched group,adults' 24h-urine sodium increased 15.30±72.91mmol and the difference was statistically significant (t=7.726,P＜0.05).In the intervention group,adults' the ratio of Na/K decreased 1.28±9..49 compared with that before the intervention.In the matched group,adults' Na/K ratio increased 0.28 ±2.06 and the difference was statistically significant (t =6.435,P＜0.05).After health education,the intervention children'24h-urine sodium decreased 8.43±24.16g compared with that before the intervention.In the matched group,children'24h-urine sodium increased 11.12±27.47g and the difference was statistically significant (t=6.212,P＜0.05).In the intervention group,children'Na/K ratio increased 2.18±9.49 compared with that before the intervention.In the matched group,children'Na/K ratio increased 2.73±1.87 and the difference was statistically significant (t=2.028,P＜0.05).Conclusion An education program delivered to primary school children was effective in lowering salt intake in children and their families.

DESIGN:A substudy of a cluster randomised controlled trial, with schools randomly assigned to either the intervention or the control group. SETTING:28 primary schools in Changzhi, northern China. PARTICIPANTS:279 children in grade 5 of primary school (mean age: 10.1); 553 adults (age: 43.8). INTERVENTION:Children were educated about the harmful effects of salt and how to reduce salt intake using the schools' usual health education lessons. Children then delivered the message to their families. The duration was 1 school term (≈3.5 months). MAIN OUTCOME MEASURE:Difference between the intervention and control groups in the change of iodine intake as measured by repeat 24 hour urinary iodine from baseline to the end of the trial. RESULTS:At baseline, the mean salt intake was 7.0±2.5 g/day in children and 11.7±4.4 g/day in adults and the median iodine intake was 165.1 μg/day (IQR: 122.6-216.7) and 280.7 μg/day (IQR: 205.1-380.9) in children and adults, respectively. At the end of the study, salt and iodine decreased in the intervention compared with control group. The mean effect on salt for intervention versus control was -1.9 g/day (95% CI -2.6 to -1.3) in children and -2.9 g/day (95% CI -3.7 to -2.2) in adults. The mean effect on iodine was -19.3% (95% CI -29.4% to -7.7%) in children and -11.4% (95% CI -20.3% to -1.5%) in adults. CONCLUSIONS:With ≈25% reduction in salt intake, there was a significant reduction in iodine consumption in northern China where salt is iodised. Abstract:Average sodium intake and stroke mortality in northern China are both among the highest in the world. An effective, low-cost strategy to reduce sodium intake in this population is urgently needed.We sought to determine the effects of a community-based sodium reduction program on salt consumption in rural northern China.This study was a cluster-randomized trial done over 18 months in 120 townships (one village from each township) from five provinces. Sixty control villages were compared to 60 intervention villages that were given access to a reduced-sodium, added-potassium salt substitute in conjunction with a community-based health education program focusing on sodium reduction. The primary outcome was the difference in 24-hour urinary sodium excretion between randomized groups.Among 1,903 people with valid 24-hour urine collections, mean urinary sodium excretion in intervention compared with control villages was reduced by 5.5% (-14mmol/day, 95% confidence interval -26 to -1; p = 0.03), potassium excretion was increased by 16% (+7mmol/day, +4 to +10; p<0.001), and sodium to potassium ratio declined by 15% (-0.9, -1.2 to -0.5; p<0.001). Mean blood pressure differences were -1.1 mm Hg systolic (-3.3 to +1.1; p = 0.33) and -0.7 mm Hg diastolic (-2.2 to +0.8, p = 0.35) and the difference in the proportion with hypertension was -1.3% (-5.1 to 2.5, p = 0.56).There were clear differences in population sodium and potassium intake between villages that were most likely a consequence of increased use of salt substitute. The absence of effects on blood pressure reflects the moderate changes in sodium and potassium intake achieved.Clinicaltrials.gov identifier: NCT01259700.

Abstract:Rationale and objective Salt reduction remains a global challenge and different salt reduction strategies have been studied in China. This study is to systematically evaluate evidence from randomised controlled trials (RCT) in China and inform the effective salt reduction strategies. Design Systematic review and meta-analysis. Data sources MEDLINE, Embase, Cochrane Central Register of Controlled Trials, Wanfang Data and the China National Knowledge Infrastructure databases through October 2019. Eligibility criteria RCTs conducted in China with at least 4 weeks' duration of study and blood pressure (BP) reported. Data extraction and synthesis Data were screened, extracted and appraised by two independent reviewers. The quality of study was assessed using a modified Cochrane Collaboration's risk of bias tool. The primary outcome was the difference in BP change from baseline to the end of study between interventions and control. The effects were pooled using a random effects model and associated factors were explored by a meta-regression. Results We identified 24 studies involving 10 448 participants, including 8 studies on health education (4583 participants), 2 studies on salt restriction diet (162 participants), 1 study on salt restriction spoon (50 participants) and 13 studies on salt substitute (5653 participants). Six studies on salt substitute and three studies on health education were identified with high quality. Pooled results from the six studies showed that salt substitutes significantly reduced systolic BP (-5.7 mm Hg; 95% CI-8.5 to-2.8) and diastolic BP (-2.0 mm Hg; 95% CI-3.5 to-0.4). The School-EduSalt study showed that the school-based health education significantly reduced systolic BP among parents (-2.3 mm Hg; 95% CI -4.5 to-0.04). Conclusions Among four salt reduction strategies studied in China with RCT design, only salt substitute was proved effective in lowering BP by the pooled effect from multiple studies with high quality. More well-designed studies are warranted for other strategies.
