Abstract:Sixty-one outbreaks of food-borne botulism involving a total of 122 cases, of which 21 were fatal, were recorded from 1971 to 1984 in Canada. Most occurred in northern Quebec, the Northwest Territories or British Columbia. Of the 122 victims 113 were native people, mostly Inuit. Most of the outbreaks (59%) were caused by raw, parboiled or "fermented" meats from marine mammals; fermented salmon eggs or fish accounted for 23% of the outbreaks. Three outbreaks were attributed to home-preserved foods, and one outbreak was attributed to a commercial product. The causative Clostridium botulinum type was determined in 58 of the outbreaks: the predominant type was E (in 52 outbreaks), followed by B (in 4) and A (in 2). Renewed educational efforts combined with a comprehensive immunization program would significantly improve the control of botulism in high-risk populations.
Automated interpretation of prenatal ultrasound using a predefined acquisition protocol in resource-limited countries

Abstract:In this study, we combine a standardized acquisition protocol with image analysis algorithms to investigate if it is possible to automatically detect maternal risk factors without a trained sonographer. The standardized acquisition protocol can be taught to any health care worker within two hours. This protocol was acquired from 280 pregnant women at St.\ Luke's Catholic Hospital, Wolisso, Ethiopia. A VGG-like network was used to perform a frame classification for each frame within the acquired ultrasound data. This frame classification was used to automatically determine the number of fetuses and the fetal presentation. A U-net was trained to measure the fetal head circumference in all frames in which the VGG-like network detected a fetal head. This head circumference was used to estimate the gestational age. The results show that it possible automatically determine gestational age and determine fetal presentation and the potential to detect twin pregnancies using the standardized acquisition protocol.

Abstract:Loading of HAuCl4 in poly(amido amine) G4 dendrimers having poly(ethylene glycol) (PEG) grafts at all chain ends and subsequent reduction with NaBH4 yielded PEG-modified dendrimers encapsulating gold nanoparticles (Au NPs) of ca. 2 nm diameter. The Au NPs held in the dendrimers were stable in aqueous solutions and dissolved readily, even after freeze-drying. Despite their small particle size, the heat-generating ability of Au NPs held in the dendrimer was comparable to that of widely used Au NPs with ca. 11 nm diameter under visible light irradiation. The observed excellent colloidal stability, high heat-generating ability and their biocompatible surface confirm that the PEG-modified dendrimers encapsulating Au NPs are a promising tool for photothermal therapy and imaging.

Abstract:During pregnancy, ultrasound examination in the second trimester can assess fetal size according to standardized charts. To achieve a reproducible and accurate measurement, a sonographer needs to identify three standard 2D planes of the fetal anatomy (head, abdomen, femur) and manually mark the key anatomical landmarks on the image for accurate biometry and fetal weight estimation. This can be a time-consuming operator-dependent task, especially for a trainee sonographer. Computer-assisted techniques can help in automating the fetal biometry computation process. In this paper, we present a unified automated framework for estimating all measurements needed for the fetal weight assessment. The proposed framework semantically segments the key fetal anatomies using state-of-the-art segmentation models, followed by region fitting and scale recovery for the biometry estimation. We present an ablation study of segmentation algorithms to show their robustness through 4-fold cross-validation on a dataset of 349 ultrasound standard plane images from 42 pregnancies. Moreover, we show that the network with the best segmentation performance tends to be more accurate for biometry estimation. Furthermore, we demonstrate that the error between clinically measured and predicted fetal biometry is lower than the permissible error during routine clinical measurements.

Abstract:A new coupled redox procedure has been developed for the determination of glucose in 4 151 of plasma or serum. The photometric endpoint is formation of the violet.blue Fe2+.5.pyridyl benzodiazepin-2-one chelate, for which the absorbance at 580 nm is linear with glucose concentration from 25 to 300 mg/100 ml. Results obtained by the present technique and those obtained by the o-toluidinemethod and a glucose oxidase method agree well. The mean recovery of glucose added to plasma is 96.6% (range: 92.1-99.6%).

Abstract:Obstetric ultrasound is a fundamental ingredient of modern prenatal care with many applications including accurate dating of a pregnancy, identifying pregnancy-related complications, and diagnosis of fetal abnormalities. However, despite its many benefits, two factors currently prevent wide-scale uptake of this technology for point-of-care clinical decision-making in low- and middle-income country (LMIC) settings. First, there is a steep learning curve for scan proficiency, and second, there has been a lack of easy-to-use, affordable, and portable ultrasound devices. We introduce a framework toward addressing these barriers, enabled by recent advances in machine learning applied to medical imaging. The framework is designed to be realizable as a point-of-care ultrasound (POCUS) solution with an affordable wireless ultrasound probe, a smartphone or tablet, and automated machine-learning-based image processing. Specifically, we propose a machine-learning-based algorithm pipeline designed to automatically estimate the gestational age of a fetus from a short fetal ultrasound scan. We present proof-of-concept evaluation of accuracy of the key image analysis algorithms for automatic head transcerebellar plane detection, automatic transcerebellar diameter measurement, and estimation of gestational age on conventional ultrasound data simulating the POCUS task and discuss next steps toward translation via a first application on clinical ultrasound video from a low-cost ultrasound probe.

Abstract:One of the outstanding facts about the hydrogen-oxygen reaction is the existence of three distinct Explosion limits, designated, in order of increasing pressure, the first, second and third explosion limits. The influence on the third limit of (a) hydrogen-oxygen proportions, (b) additions of nitrogen, carbon dioxide and steam, has been examined: the measurements were made mainly at 586° in a silica vessel coated with potassium chloride (which controls the reaction chains). The results are qualitatively consistent with the theory that the third limit depends upon the (essentially isothermal) branching of reaction chains. An expression based upon the equations OH + H2 = H2O + H, H + O2 = OH + O, O + H2 = OH + H, H + O2 + M = HO2 + M, HO2+ H2 =H2O + OH, together with the assumption that HO2 diffuses to the walls of the vessel and is destroyed, leads to a satisfactory description of the third limit as well as of the other two. The constants characteristic of the various gases participating are in fair agreement with those derived by independent means.
Development and validation of an artificial intelligence assisted prenatal ultrasonography screening system for trainees

Abstract:Objective: Fetal anomaly screening via ultrasonography, which involves capturing and interpreting standard views, is highly challenging for inexperienced operators. We aimed to develop and validate a prenatal-screening artificial intelligence system (PSAIS) for real-time evaluation of the quality of anatomical images, indicating existing and missing structures. Methods: Still ultrasonographic images obtained from fetuses of 18-32 weeks of gestation between 2017 and 2018 were used to develop PSAIS based on YOLOv3 with global (anatomic site) and local (structures) feature extraction that could evaluate the image quality and indicate existing and missing structures in the fetal anatomical images. The performance of the PSAIS in recognizing 19 standard views was evaluated using retrospective real-world fetal scan video validation datasets from four hospitals. We stratified sampled frames (standard, similar-to-standard, and background views at approximately 1:1:1) for experts to blindly verify the results. Results: The PSAIS was trained using 134 696 images and validated using 836 videos with 12 697 images. For internal and external validations, the multiclass macro-average areas under the receiver operating characteristic curve were 0.943 (95% confidence interval [CI], 0.815-1.000) and 0.958 (0.864-1.000); the micro-average areas were 0.974 (0.970-0.979) and 0.973 (0.965-0.981), respectively. For similar-to-standard views, the PSAIS accurately labeled 90.9% (90.0%-91.4%) with key structures and indicated missing structures. Conclusions: An artificial intelligence system developed to assist trainees in fetal anomaly screening demonstrated high agreement with experts in standard view identification.

Abstract:Introduction Undetected high-risk conditions in pregnancy are a leading cause of perinatal mortality in low-income and middle-income countries. A key contributor to adverse perinatal outcomes in these settings is limited access to high-quality screening and timely referral to care. Recently, a low-cost one-dimensional Doppler ultrasound (1-D DUS) device was developed that front-line workers in rural Guatemala used to collect quality maternal and fetal data. Further, we demonstrated with retrospective preliminary data that 1-D DUS signal could be processed using artificial intelligence and deep-learning algorithms to accurately estimate fetal gestational age, intrauterine growth and maternal blood pressure. This protocol describes a prospective observational pregnancy cohort study designed to prospectively evaluate these preliminary findings. Methods and analysis This is a prospective observational cohort study conducted in rural Guatemala. In this study, we will follow pregnant women (N =700) recruited prior to 18 6/7 weeks gestation until their delivery and early postpartum period. During pregnancy, trained nurses will collect data on prenatal risk factors and obstetrical care. Every 4 weeks, the research team will collect maternal weight, blood pressure and 1-D DUS recordings of fetal heart tones. Additionally, we will conduct three serial obstetric ultrasounds to evaluate for fetal growth restriction (FGR), and one postpartum visit to record maternal blood pressure and neonatal weight and length. We will compare the test characteristics (receiver operator curves) of 1-D DUS algorithms developed by deep-learning methods to two-dimensional fetal ultrasound survey and published clinical pre-eclampsia risk prediction algorithms for predicting FGR and pre-eclampsia, respectively. Ethics and dissemination Results of this study will be disseminated at scientific conferences and through peer-reviewed articles. Deidentified data sets will be made available through public repositories. The study has been approved by the institutional ethics committees of Maya Health Alliance and Emory University.

Abstract:Fetal ultrasounds are an essential part of prenatal care and can be used to estimate gestational age (GA). Accurate GA assessment is important for providing appropriate prenatal care throughout pregnancy and identifying complications such as fetal growth disorders. Since derivation of GA from manual fetal biometry measurements (head, abdomen, femur) are operator-dependent and time-consuming, there have been a number of research efforts focused on using artificial intelligence (AI) models to estimate GA using standard biometry images, but there is still room to improve the accuracy and reliability of these AI systems for widescale adoption. To improve GA estimates, without significant change to provider workflows, we leverage AI to interpret standard plane ultrasound images as well as 'fly-to' ultrasound videos, which are 5-10s videos automatically recorded as part of the standard of care before the still image is captured. We developed and validated three AI models: an image model using standard plane images, a video model using fly-to videos, and an ensemble model (combining both image and video). All three were statistically superior to standard fetal biometry-based GA estimates derived by expert sonographers, the ensemble model has the lowest mean absolute error (MAE) compared to the clinical standard fetal biometry (mean difference: -1.51 $\pm$ 3.96 days, 95% CI [-1.9, -1.1]) on a test set that consisted of 404 participants. We showed that our models outperform standard biometry by a more substantial margin on fetuses that were small for GA. Our AI models have the potential to empower trained operators to estimate GA with higher accuracy while reducing the amount of time required and user variability in measurement acquisition.

Abstract:Fetal growth assessment from ultrasound is based on a few biometric measurements that are performed manually and assessed relative to the expected gestational age. Reliable biometry estimation depends on the precise detection of landmarks in standard ultrasound planes. Manual annotation can be time-consuming and operator dependent task, and may results in high measurements variability. Existing methods for automatic fetal biometry rely on initial automatic fetal structure segmentation followed by geometric landmark detection. However, segmentation annotations are time-consuming and may be inaccurate, and landmark detection requires developing measurement-specific geometric methods. This paper describes BiometryNet, an end-to-end landmark regression framework for fetal biometry estimation that overcomes these limitations. It includes a novel Dynamic Orientation Determination (DOD) method for enforcing measurement-specific orientation consistency during network training. DOD reduces variabilities in network training, increases landmark localization accuracy, thus yields accurate and robust biometric measurements. To validate our method, we assembled a dataset of 3,398 ultrasound images from 1,829 subjects acquired in three clinical sites with seven different ultrasound devices. Comparison and cross-validation of three different biometric measurements on two independent datasets shows that BiometryNet is robust and yields accurate measurements whose errors are lower than the clinically permissible errors, outperforming other existing automated biometry estimation methods. Abstract:Objective Automated medical image analysis solutions should closely mimic complete human actions to be useful in clinical practice. However, more often an automated image analysis solution represents only part of a human task, which restricts its practical utility. In the case of ultrasound-based fetal biometry, an automated solution should ideally recognize key fetal structures in freehand video guidance, select a standard plane from a video stream and perform biometry. A complete automated solution should automate all three subactions. Methods In this article, we consider how to automate the complete human action of first-trimester biometry measurement from real-world freehand ultrasound. In the proposed hybrid convolutional neural network (CNN) architecture design, a classification regression-based guidance model detects and tracks fetal anatomical structures (using visual cues) in the ultrasound video. Several high-quality standard planes that contain the mid-sagittal view of the fetus are sampled at multiple time stamps (using a custom-designed confident-frame detector) based on the estimated probability values associated with predicted anatomical structures that define the biometry plane. Automated semantic segmentation is performed on the selected frames to extract fetal anatomical landmarks. A crown–rump length (CRL) estimate is calculated as the mean CRL from these multiple frames. Results Our fully automated method has a high correlation with clinical expert CRL measurement (Pearson's p = 0.92, R -squared [ R 2 ] = 0.84) and a low mean absolute error of 0.834 (weeks) for fetal age estimation on a test data set of 42 videos. Conclusion A novel algorithm for standard plane detection employs a quality detection mechanism defined by clinical standards, ensuring precise biometric measurements.

Abstract:Purpose: Ultrasound is the most commonly used medical imaging modality for diagnosis and screening in clinical practice. Due to its safety profile, noninvasive nature and portability, ultrasound is the primary imaging modality for fetal assessment in pregnancy. Current ultrasound processing methods are either manual or semi-automatic and are therefore laborious, time-consuming and prone to errors, and automation would go a long way in addressing these challenges. Automated identification of placental changes at earlier gestation could facilitate potential therapies for conditions such as fetal growth restriction and pre-eclampsia that are currently detected only at late gestational age, potentially preventing perinatal morbidity and mortality.

Abstract:A semi-automated fetal ultrasound image segmentation system is developed to improve the estimation of fetal weight (EFW). Four standardized fetal parameters are measured by the proposed segmentation system: biparietal diameter, head circumference, abdominal circumference and femur length. Computerized measurements of 215 fetuses are compared with manual measurements in term of fitness analysis and difference analysis. Among 215 cases, computerized measurements of 103 fetuses within 3 days of delivery are utilized in the fetal weight estimation. The EFW based on computerized measurements and manual measurements are compared by using regression analysis, artificial neural network and support vector regression. By using different estimation methods, the computerized measurements decrease the EFW errors about 40–70 g. The lowest mean absolute percentage error of EFW decrease from 6.71% for manual measurements to 4.66% for computerized measurements. The proposed fetal ultrasound image segmentation system can provide more accurate EFW in antepartum examination.

Abstract:The current approach to fetal anomaly screening is based on biometric measurements derived from individually selected ultrasound images. In this paper, we introduce a paradigm shift that attains human-level performance in biometric measurement by aggregating automatically extracted biometrics from every frame across an entire scan, with no need for operator intervention. We use a convolutional neural network to classify each frame of an ultrasound video recording. We then measure fetal biometrics in every frame where appropriate anatomy is visible. We use a Bayesian method to estimate the true value of each biometric from a large number of measurements and probabilistically reject outliers. We performed a retrospective experiment on 1457 recordings (comprising 48 million frames) of 20-week ultrasound scans, estimated fetal biometrics in those scans and compared our estimates to the measurements sonographers took during the scan. Our method achieves human-level performance in estimating fetal biometrics and estimates well-calibrated credible intervals in which the true biometric value is expected to lie.

Abstract:Examination of the umbilical artery with Doppler ultrasonography is performed to investigate blood supply to the fetus through the umbilical cord, which is vital for the monitoring of fetal health. Such examination involves several steps that must be performed correctly: identifying suitable sites on the umbilical artery for the measurement, acquiring the blood flow curve in the form of a Doppler spectrum, and ensuring compliance to a set of quality standards. These steps rely heavily on the operator's skill, and the shortage of experienced sonographers has thus created a demand for machine assistance. In this work, we propose an automatic system to fill the gap. By using a modified Faster R-CNN network, we obtain an algorithm that can suggest locations suitable for Doppler measurement. Meanwhile, we have also developed a method for assessment of the Doppler spectrum's quality. The proposed system is validated on 657 images from a national ultrasound screening database, with results demonstrating its potential as a guidance system.

Abstract:Medical and healthcare institutions have embraced the utilization of fetal monitoring systems to evaluate the condition of fetus throughout pregnancy, with a particular emphasis on the labor and delivery phases. Technological advancements have facilitated the early detection of defects and irregularities in the initial stages of pregnancy, thereby amplifying the possibility of successful interventions. The primary objective of this study is to develop a system capable of detecting abnormalities in the fetus by analyzing ultrasound images obtained during routine ultrasonography sessions within the first 21 weeks of pregnancy. The developed system evaluates crucial parameters such as the dimensions of the head, abdomen, and femur, as well as the fetal weight. Furthermore, the methodology is capable of detecting any structural anomalies that may be present. Notably, this approach has yielded an impressive accuracy rate of 95.238%, thus offering healthcare practitioners a valuable diagnostic aid.
