Abstract:Backgrounds: Although worldwide evidence tends to prove that diabetes adversely affects periodontal health, there are insufficient clues indicating whether periodontitis may aggravate metabolic control and systemic inflammation. This study, as a preliminary part of an ongoing research project, aims to clarify the relationship of periodontal parameters with metabolic levels and systemic inflammatory markers in patients with diabetes.Methods: A total of 140 qualified, adult patients with type 2 diabetes and periodontitis were recruited into this study. Periodontal examinations, including a full-mouth assessment of probing depths (PDs), bleeding on probing, gingival recession, and clinical attachment level, were determined. Blood analyses were carried out for glycated hemoglobin (HbA1c), fasting glucose, high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-alpha (TNF-alpha), and lipid profiles. Subjects were divided into three groups according to tertiles of the mean PD and compared.Results: Upon an analysis of covariance, subjects with an increased mean PD had significantly higher levels of HbA1c and hsCRP (P <0.05). No significant difference was found among different groups in the levels of serum TNF-alpha, fasting glucose, and lipid profiles (P >0.05). After controlling for age, gender, body mass index, duration of diabetes mellitus, smoking, regular physical exercise, and alcohol consumption, positive correlations were found between mean PD and HbA1c (r = 0.2272; P= 0.009) and between mean PD and hsCRP (r = 0.2336; P= 0.007). After adjustment for possible confounders, the mean PD emerged as a significant predictor variable for elevated levels of HbA1c and hsCRP (P<0.05).Conclusion: Chronic periodontitis was associated with glycemic metabolic and serum hsCRP levels in patients with type 2 diabetes. Comparison of Caries and Gingival Status in Patients with Type 2 Diabetes and Healthy Children

Abstract:Background: Oral health problems often occur in patients with type 2 diabetes (T2D), and the incidences of dental caries and gingivitis increase as blood glucose (BG) levels rise. This work compared caries and gingival status of patients with T2D and healthy children to improve the understanding and attention of patients with T2D to oral health. Methods: Clinical data of 60 patients with T2D under the age of 10 from May 2020 to September 2022 were retrospectively collected and assigned to the diabetes group. Those 60 healthy children with the same physical examination were collected and assigned to the healthy group. Children in both groups underwent periodontal examination, dental caries examination, and gingival index examination. The prevalence, decayed, missed, and filled teeth (DMFT), caries average (CA), plaque index (PI), gingival crevicular bleeding index (GCBI), attachment loss (AL), and tooth looseness (TL) were observed and compared. Results: The prevalence of dental plaque (DP) (91.67%) and moderate to severe DP (45%) in the diabetes group was much higher based on those in the healthy group (73.33% and 23.33%) [1.25, 95% CI (0.96, 1.63), P < .001]. The prevalence of caries and CA was greatly higher in the diabetes group (75% vs. 21.67%, 2.88 vs. 1.06), and the incidence of gingivitis was higher (63.33% vs. 16.67%) [1.93, 95% CI (1.38, 2.70), P < .001]. Meanwhile, the diabetes group exhibited much higher PI (2.31 ± 0.13), GCBI (2.45 ± 0.28), AL (5.62 ± 0.47 mm), and TL (0.85 ± 0.17 mm) and exhibited obvious difference to those in the healthy group (0.92 ± 0.21, 0.86 ± 0.23, 1.65 ± 0.46mm, 0.36 ± 0.08 mm) [3.46, 95% CI (2.33, 5.15), P < .001]. Conclusion: The prevalence of dental caries and gingivitis in patients with T2D was higher than those in healthy children. Based on these findings, it is suggested that patients with T2D should be educated and encouraged to prioritize their oral health. Regular dental check-ups, proper oral hygiene practices, and preventive measures, such as professional cleanings and fluoride treatments, should be emphasized. Maintaining optimal blood glucose control is also crucial, as it may help reduce the risk and severity of oral health complications associated with diabetes.
Poor glycemic control impairs oral health in children with type 1 diabetes mellitus - a systematic review and meta-analysis

Abstract:Objective: There are more than one million children and adolescents living with type 1 diabetes mellitus, and their number is steadily increasing. Diabetes affects oral health through numerous channels, including hyposalivation, immune suppression, and the inflammatory effect of glycation end-products. However, patients with type 1 diabetes must follow a strict sugar free diet that is proven to be carioprotective. Therefore, the aim of this systematic review and meta-analysis is to investigate whether children with type 1 diabetes have a difference in Decayed, Missing, Filled Teeth index (DMFT), salivary function, and periodontal status than children without diabetes, with an emphasis on glycemic control. Materials and methods: PubMed, Embase and Cochrane libraries were screened for articles, using predefined search keys without any language or date restrictions. Two independent authors performed the selection procedure, extracted data from the eligible articles, carried out a manual search of the reference lists, and assessed the risk of bias using the Newcastle-Ottawa scale. Meta-analysis was performed in R using the random-effects model. Effect sizes were mean differences; subgroup analysis was performed on glycemic control. Results: 33 studies satisfied the eligibility criteria. 22 studies did not show a significant difference regarding the DMFT index between the diabetes and non-diabetes groups; six studies found that children living with diabetes had higher DMFT scores, compared to five studies that found significantly lower scores. Meta-analysis found no statistically significant differences in plaque, gingival, and calculus indexes, however it found significant differences in pooled DMFT indexes, and salivary flow rate. Subgroup analysis on glycemic control using DMFT values found significant differences in children with good and poor glycemic control with results of 0.26 (CI95%=-0.50; 1.03) and 1.46 (CI95%=0.57; 2.35), respectively. Conclusions: Children with poor glycemic control face higher risk of developing caries compared to good control and non-diabetes children. Regular dental check-ups and strict control of glycemic levels are highly advised for children living with type 1 diabetes, further emphasizing the importance of cooperation between dentists and diabetologists.
EVALUATION OF DENTAL AND PERIODONTAL STATUS IN CHILDREN WITH TYPE 1 DIABETES MELLITUS

Abstract:Diabetes mellitus is a well-known predisposing factor for oral diseases. Aim: To evaluate dental caries and periodontal status of children with type 1 diabetes mellitus (T1DM). Materials and methods: Eighty children with type 1 diabetes mellitus (age 10-15 years) have received a comprehensive oral health examination, that included the DMFT index for permanent teeth, plaque index (PI), gingival index (GI), and stimulated salivary flow rate (SSFR). P<0.05 was considered significant. Results: HbA1c values indicated poor metabolic control of diabetes in children, which also adversely affected their oral health (9.45±2.87). The mean DMFT index in children with type 1 diabetes mellitus was notably higher at 6.56±3.56. Similarly, the plaque index (PI) and gingival index (GI) were significantly higher (2.04±0.39/1.18±0.63). In contrast, the stimulated salivary flow rate (SSFR) was observed to be lower (0.86±0.16). There was significant positive correlation between DMFT index and PI (p<0.01). Conclusion: Children with T1DM exhibit poor oral health conditions related to the level of metabolic control. Maintenance of toothbrushing habits and regular dental check-ups recommended to manage and prevent these complications. Additionally, proper management of metabolic control can also help mitigate the adverse effects on oral health.
Oral Microbiota in Children and Adolescents with Type 1 Diabetes Mellitus: Novel Insights into the Pathogenesis of Dental and Periodontal Disease

Abstract:The oral microbiota can be influenced by multiple factors, but only a few studies have focused on the role of glycemic control in determining early alterations of oral microbiota and their association with pathogenesis of both periodontitis and caries. The aim of this study is to evaluate the interplay between bacteria composition, oral hygiene, and glycemic control in a cohort of children with T1D. A total of 89 T1D children were enrolled (62% males, mean age: 12.6 ± 2.2 years). Physical and clinical characteristics, glucometabolic parameters, insulin treatment, and oral hygiene habits data were collected. Microbiological analysis was performed from saliva samples. A high prevalence of cariogenic and periodontopathogens bacteria in our cohort was detected. In particular, in all subjects Actinomyces spp., Aggregatibacter actinomycetemcomitans, Prevotella intermedia, and Lactobacillus spp. were isolated. S. mutans was found in about half of the analyzed sample (49.4%), in particular in patients with imbalance values of glycemic control. Moreover, a higher presence of both S. mutans and Veillonella spp. was detected in subjects with poorer glycemic control, in terms of HbA1c, %TIR and %TAR, even adjusting for age, sex, and hygiene habits as covariates. Virtuous oral hygiene habits, such as frequency of toothbrush changes and professional oral hygiene, negatively correlated with the simultaneous presence of Tannerella forsythia, Treponema denticola, and Porphyromonas gingivalis, red complex bacteria. Our study shows it is crucial to pay attention to glycemic control and regular oral hygiene to prevent the establishment of an oral microbiota predisposing to dental and periodontal pathology in subjects with T1D since childhood.

Abstract:Aim: To examine the oral health of children and adolescents with and without diabetes mellitus. Background: Diabetes mellitus is the most common metabolic disease in childhood and demonstrates an increasing incidence. Many children live with gingivitis as a precursor to periodontitis. If left untreated, it can cause the development of periodontitis. The links between periodontitis and diabetes mellitus are known but have been little studied in the age group of children and adolescents. Materials and Methods: Clinical examination and collection of sulcus fluid from participants aged 5 to 21 years was performed. The following data were collected: demographic variables, caries prevalence, DMF-T, VPI, PUFA, salivary flow rate, HbA1c, PSI, and the concentration of IL-1β, IL-6, MMP-8, and TNF-α. Results: Patients with diabetes mellitus showed a significantly lower salivary flow rate with higher concentrations of MMP-8 and IL-1β. The data indicate that at this age, regular visits to the dentist are of great importance for the promotion of oral health in children and adolescents regardless of diabetes and that patients with diabetes mellitus in particular benefit from prevention, as they belong to the periodontitis risk group. Conclusions: Patients with low salivary flow rates and increased inflammatory mediators are high-risk patients for whom dental preventive measures play a major role.

Abstract:IntroductionThe association between dental caries, salivary alterations, and type-1 diabetes mellitus (D1TM) remains inconclusive in children and adolescents. This systematic review evaluated the caries status and salivary alterations of children and adolescents suffering from D1TM.MethodsMeta-analysis searched from 5 electronic databases was performed considering the decay-missing-filled teeth (DMFT), decay-missing-filled surfaces, and decay-missing-filled primary teeth (dmft) index for caries and the stimulated flow rate, unstimulated flow rate, and buffering capacity for salivary alterations. Quality assessments were performed using the Healthcare Research and Quality Agency.ResultsEleven studies satisfied the eligibility criteria. The meta-analysis revealed that DMF(T) (0.61; 95% confidence interval [CI], 0.14-1.08, P = .01) and dmft (-0.43; 95% CI, -0.71 to -0.15, P = .002) were significantly higher in D1TM group than the non-D1TM group. Stimulated flow rate (20.40; 95% CI, 20.57 to 20.23, P < .00001) was significantly lower in D1TM group than in the non-D1TM group. No significant differences in decay-missing-filled surfaces and the buffering capacity were observed between the D1TM and the non-D1TM groups.ConclusionsThe level of DMF(T) and dmft was higher in children/adolescents with D1TM than that with non-D1TM, and the salivary flow could be considered decreased with D1TM. The strength of the evidence of the studies analyzed, however, was deemed to be moderate and low.

Abstract:OBJECTIVE:To detect the salivary factors related to caries and periodontal disease and to analyze the risk of caries and periodontal disease in children and adolescents with diabetes mellitus. METHODS:The study comprised 30 children with diabetic mellitus, aged 7-15 years old, and 60 healthy age-and gender-matched children. Caries and periodontal indexes were recorded and saliva related factors were analyzed. RESULTS:Caries indexes of diabetes children [permanent teeth: decay missing filling tooth (DMFT) M (Q1,Q3) = 0(0, 4), deciduous teeth: decay missing filling tooth (dmft) M (Q1,Q3) = 0(0, 1)] were not significantly different with those of healthy children [DMFT M (Q1,Q3) = 1(0, 3), dmft M (Q1,Q3) = 0(0, 4)], but plaque index (PLI) (1.25 ± 0.33) and bleeding index (BI) (0.74 ± 0.45) of diabetes children were significantly higher than those of healthy children (PLI was 0.93 ± 0.31,BI was 0.34 ± 0.22) (P < 0.001). Salivary pH of diabetes children (7.68 ± 0.36) was significantly higher than that of healthy children (7.30 ± 0.32) (P < 0.05), and salivary acid buffering capacity had no significant difference between the two groups (P > 0.05). Salivary glucose, immunoglobulin sIgA and sIgG were not significantly different between the two groups (P > 0.05).Salivary lysozyme of diabetes children was significantly higher than that of healthy children (P < 0.05). Total protein was significantly lower in diabetes children than in healthy children (P < 0.05). Salivary lactate dehydrogenase had no significant difference between the two groups (P > 0.05). CONCLUSIONS:Diabetes mellitus can lead to the changes of some salivary factors related to gingivitis in diabetes children. Children and adolescents with diabetes mellitus may have a higher risk of periodontal disease.
Dental, periodontal and salivary conditions in diabetic children associated with metabolic control variables and nutritional plan adherence

Abstract:Aim: Diabetes mellitus is a chronic disease that has manifestations other than alterations in endocrine regulation or in metabolic pathways. Several diseases of the oral cavity have been associated with diabetes mellitus type 1 and 2 in young people according to their evolution. Scarce information exists regarding the role of diabetes and its association with the oral health status in paediatric diabetic patients. The aims of the study were to assess the quality of saliva, saliva acidogenicity, dental caries experience, fluorosis and periodontal status in diabetic patients and to evaluate their relationship with metabolic control variables and nutritional plan adherence. Materials and methods: The study population consisted of 60 paediatric patients with both types of diabetes mellitus. Saliva testing included stimulated flow, pH (using pH indicator strips), buffer capacity and Snyder's Test. DMFT/dmft and dental caries experience were determined on the basis of ICDAS II codes. The periodontal status was assessed by PI and GI and fluorosis by FI. Nutritional plan adherence was established from the subscale "Dietary Control" of the Diabetes Self-Management Profile questionnaire. Medical Data was retrieved from the clinical registers in the Diabetic Clinic. Results: We describe the main characteristics of the oral cavity related variables of our population that might guide the clinical practice in similar settings; we found a dmft/DMFT of 1.71 ± 1.74 and 0.64 ± 1.03, PI of 1.91 ± 0.75, GI of 0.50 ± 0.56 and a fluorosis prevalence of 61%. We identified several correlated variables, which indicate strong associations between the nutritional habits of the patients and co-occurrence of oral cavity physiopathological alterations. Several correlations were found between acidogenic activity of the saliva (Snyder Test) and the percentage of adherence to the nutritional plan and to the dmft index. Furthermore, a significant correlation between the buffering capacity of the saliva and the glycemic control of the participants was found. Neither an association nor a difference among means was found between treatment regime and the plaque index. Conclusion: The results of the present study concluded that there was a significant relationship between diabetes mellitus and an increased prevalence of oral cavity related diseases in the paediatric population. These are also associated with a poor adherence to the nutritional plan.
Oral Health in Children with Obesity or Diabetes Mellitus

Abstract:Oral health status must be considered in the care of children with obesity (OB) and diabetes mellitus (DM). The health of these patients' mouths may have significant effects on their overall health and evolution of their disease. Here we address periodontal disease (PD) and dental caries (DC), since these are two of the most common chronic diseases affecting OB and DM patients. OB plays a plausible role in the development of PD. Both overall OB and central adiposity are associated with increased hazards of gingivitis and its progression to PD. The inflammatory changes of PD might not be limited to the oral cavity, these may also trigger systemic consequences. Patients with type 1 and type 2 diabetes mellitus (T1DM, T2DM) present an increased prevalence of gingivitis and PD. In diabetics PD develops at a younger age than in the healthy population, it also worsens with the prolongation of DM. The progression to PD has been correlated with the metabolic control of the disease as it is more prevalent and more severe in patients with elevated hemoglobin A1c (A1c) levels. PD negatively affects glycemic control and other diabetes related complications and there is a general consensus that treatment of PD can positively influence these negative effects. Additionally, DC is a multifactorial oral disease that is frequently detected in those with OB and DM, although its prevalence in systematic reviews is inconclusive. The associations between gingivitis, PD and DC share similar behaviors, i.e. inadequate oral hygiene habits and unhealthy dietary intake. Insufficient tooth brushing and intake of sugary foods may result in greater detrimental oral effects. Maintaining oral health will prevent oral chronic diseases and ameliorate the consequences of chronic inflammatory processes. Thus, the care of obese and diabetic patients requires a multidisciplinary team with medical and dental health professionals.
Dental caries and salivary alterations in patients with type 2 diabetes: A systematic review and meta-analysis

Abstract:Introduction: The association between dental caries, salivary alterations, and type 2 diabetes (T2D) is inconclusive. This systematic review evaluated the caries status and salivary alterations in adults with T2D. Data: Observational studies (cross-sectional, case-control, or cohort studies) that evaluated the association between type 2 diabetes and dental caries and/or salivary changes were included. Sources: Systematic searches of PubMed/Medline, Web of Science, Embase, and the Cochrane Library for relevant literature published up to January 2024 were performed. Study selection: Ninety-eight studies were selected from the 1981 records obtained after screening the databases. Thirty-nine studies that met the eligibility criteria were included in the qualitative synthesis, and twenty-eight of the included studies were suitable for the quantitative synthesis (meta-analysis). Results: The meta-analysis showed that compared to non-diabetic individuals, type 2 diabetic individuals had higher DMFT (mean difference: 2.27; 95 % CI, 1.31 to 3.22; P < 0.00001). Moreover, type 2 diabetic individuals had lower salivary flow rate (standardized mean difference: -2.62; 95 % CI, -4.38 to -0.86; P = 0.003), salivary pH (standardized mean difference: -1.35; 95 % CI, -1.89 to -0.80; P < 0.00001) and salivary buffering capacity (standardized mean difference: -0.60; 95 % CI, -0.93 to -0.27; P = 0.0004). Whereas DMFT (mean difference: -2.15; 95 % CI, -6.47 to 2.17; P = 0.33) and salivary buffering capacity (standardized mean difference: -0.05; 95 % CI, -0.48 to 0.37; P = 0.80) were not significantly different between individuals with well-controlled diabetes and those with poorly-controlled diabetes. Conclusions: Type 2 diabetes may lead to an increased index of dental caries and a reduction in salivary flow rate, salivary pH, and salivary buffering capacity in adults. Clinical significance: Individuals with type 2 diabetes exhibit an elevated risk of dental caries and salivary alterations that facilitate the progression of dental caries. It is recommended that dentists and endocrinologists focus their attention on the oral health of this population.
Oral Health Behaviour and Predictors of Oral Health Behaviour Among Patients With Diabetes in the Republic of Mauritius

Abstract:Objectives: To investigate the oral care habits and assess the determinants of oral care behaviour among people with diabetes in the Republic of Mauritius. Methods: The present study draws on data collected from 589 dentate persons with diabetes by means of a close-ended questionnaire. Multivariate logistic regression analyses were used to estimate the association of different demographic and clinical factors with recommended dental hygiene practices. Results: The majority of the participants brushed at least twice daily (84.2%), never flossed (88.6%), attended dental clinics on need only (87.1%), and did not monitor their blood glucose levels regularly (69.9%). Neither awareness about the increased risk of periodontal disease and xerostomia nor receiving advice from diabetes care providers was found to be associated with good oral hygiene or increased service utilisation. The experience of oral diseases did not encourage recommended oral health practice, with participants without experience with periodontal disease being 3 times more likely to floss (odds ratio [OR], 2.9; P = .045). Regular dental visits were strongly associated with self-reported type 1 diabetes (OR, 7.8; P = .025). Participants from urban areas were more than twice as likely to visit their dental care provider at least once annually (OR, 2.3; P = .006). Regular dental attendance (OR, 3.7; P = .011) and flossing (OR, 4.5; P = .012) were strongly associated with one another. Conclusion: There is widespread noncompliance with regular flossing and dental service utilisation. Our findings highlight the need for an emphasis on preventive care through the provision of integrated medical and dental interventions to high-risk individuals suffering from both diabetes and chronic periodontitis.
Dental caries and adolescent cardiometabolic health from the National Health and Nutrition Examination Survey (NHANES)

Abstract:Background and aims: To assess the association between dental caries and cardiometabolic risk in adolescents. Methods and results: The analysis included adolescents aged 13-17 years enrolled in the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2018 who completed an Oral Health Examination. Untreated caries was defined as having one or more decayed teeth. Caries experience was assessed by Decayed, Missing, Filled Teeth (DMFT) score. Primary cardiometabolic outcomes included elevated BP (defined as BP 120-129/<80 mmHg) and hypertensive BP (defined as BP ≥ 130/80 mmHg). Secondary cardiometabolic outcomes included obesity, dyslipidemia (defined as any abnormal lipid level), glucose intolerance (measured by HOMA-IR), and microalbuminuria (defined as urine albumin: creatinine ≥30 mg/mg). Adjusted linear and logistic models examined associations using complex survey design procedures. In the sample of 2861 adolescents, 25.6 % (1.3 %) had untreated caries. 55.4 % (1.3 %) had DMFT ≥1. In adjusted regression analyses, untreated caries status was not significantly associated with primary outcomes of elevated BP (OR = 1.04, 95 % CI 0.71, 1.52 p > 0.05), hypertensive BP (OR = 1.72, 95 % CI 0.71, 3.89 p > 0.05), nor secondary cardiometabolic outcomes. No statistically significant associations were found between DMFT score and primary outcomes of elevated BP (OR = 0.01, 95 % CI 0.34, 1.07 p > 0.05), hypertensive BP (OR = 0.91, 95 % CI 0.81, 1.08 p > 0.05), or secondary cardiometabolic outcomes. Conclusion: Although studies in other countries and in adults show associations between caries and cardiometabolic outcomes, this study did not find an association between caries and cardiometabolic markers.

Abstract:In this study, we present three significant artifacts that have the potential to negatively impact the accuracy and precision of film dosimetry measurements made using GAFCHROMIC® EBT radiochromic film when read out with CCD flatbed scanners. Films were scanned using three commonly employed instruments: a Macbeth TD932 spot densitometer, an Epson Expression 1680 CCD array scanner, and a Microtek ScanMaker i900 CCD array scanner. For the two scanners we assessed the variation in optical density (OD) of GAFCHROMIC EBT film with scanning bed position, angular rotation of the film with respect to the scan line direction, and temperature inside the scanner due to repeated scanning. Scanning uniform radiochromic films demonstrated a distinct bowing effect in profiles in the direction of the CCD array with a nonuniformity of up to 17%. Profiles along a direction orthogonal to the CCD array demonstrated a 7% variation. A strong angular dependence was found in measurements made with the flatbed scanners; the effect could not be reproduced with the spot densitometer. An IMRT quality assurance film was scanned twice rotating the film 90° between the scans. For films scanned on the Epson scanner, up to 12% variation was observed in unirradiated EBT films rotated between 0° and 90°, which decreased to approximately 8% for EBT films irradiated to 300cGy. Variations of up to 80% were observed for films scanned with the Microtek scanner. The scanners were found to significantly increase the film temperature with repeated scanning. Film temperature between 18 and 33°C caused OD changes of approximately 7%. Considering these effects, we recommend adherence to a strict scanning protocol that includes: maintaining the orientation of films scanned on flatbed scanners, limiting scanning to the central portion of the scanner bed, and limiting the number of consecutive scans to minimize changes in OD caused by film heating.

Abstract:Background: Regarding past epidemiological studies, there has been disagreement over whether type 1 diabetes (T1DM) is one of the risk factors for dental caries. The purpose of this study was to determine the causative links between genetic susceptibility to T1DM, glycemic traits, and the risk of dental caries using Mendelian randomization (MR) approaches. Methods: Summary-level data were collected on genome-wide association studies (GWAS) of T1DM, fasting glucose (FG), glycated hemoglobin (HbA1c), fasting insulin (FI), and dental caries. MR was performed using the inverse-variance weighting (IVW) method, and sensitivity analyses were conducted using the MR-Egger method, weighted median, weighted mode, replication cohort, and multivariable MR conditioning on potential mediators. Results: The risk of dental caries increased as a result of genetic susceptibility to T1DM [odds ratio (OR) = 1.044; 95% confidence interval (CI) = 1.015–1.074; p = 0.003], with consistent findings in the replication cohort. The relationship between T1DM and dental caries was stable when adjusted for BMI, smoking, alcohol intake, and type 2 diabetes (T2DM) in multivariable MR. However, no significant correlations between the risk of dental caries and FG, HbA1c, or FI were found. Conclusion: These results indicate that T1DM has causal involvement in the genesis of dental caries. Therefore, periodic reinforcement of oral hygiene instructions must be added to the management and early multidisciplinary intervention of T1DM patients, especially among adolescents and teenagers, who are more susceptible to T1DM.

Abstract:Objective: The impact of diabetes on oral diseases remains debatable. We aimed to determine the prevalence of dental caries, gingival health status and knowledge, and oral hygiene level and practice in children with type 1 diabetes mellitus in Riyadh, Saudi Arabia. Methods: The teeth of Sixty children with diabetes (33 males and 27 females; age 11.3 ± 2.2 [7-14] years) were examined for dental caries following the modified World Health Organization criteria. Data on age, sex, medical history, gingival health status and knowledge, and oral hygiene level and practice were collected from parents. Results: In this study, 53 % of the children had dental caries with decayed, missing, and filled primary and permanent teeth scores of 5.6 ± 3.8 and 3.6 ± 3.2, respectively, with similar scores for male and female children. Children with and without caries had similar oral hygiene practices and oral health knowledge but differed in the rates of good or fair oral hygiene status (89.2 vs. 56.6 %; P = 0.024) and normal gingival health status or mild gingivitis (96.5 vs. 81.3 %; P = 0.010). Conclusions: We found that the oral hygiene status and rate of gingivitis differed in children with type 1 diabetes mellitus with or without dental caries.

Abstract:Background: There is emerging evidence linking diabetes with periodontal disease. Diabetes is a well-recognized risk factor for periodontal disease. Conversely, pro-inflammatory molecules released by periodontally-diseased tissues may enter the circulation to induce insulin resistance. While this association has been demonstrated in adults, there is little information regarding periodontal status in obese children with and without type 2 diabetes (T2D). We hypothesized that children with T2D have higher rates of gingivitis, elevated salivary inflammatory markers, and an altered salivary microbiome compared to children without T2D. Methods: Three pediatric cohorts ages 10-19 years were studied: lean (normal weight-C), obese (Ob), and obese with T2D (T2D). Each subject completed an oral health survey, received a clinical oral examination, and provided unstimulated saliva for measurement of inflammatory markers and microbiome analysis. Results: The diabetes group was less likely to have had a dental visit within the last six months. Body mass index (BMI) Z-scores and waist circumference/height ratios were similar between Ob and T2D cohorts. The number of carious lesions and fillings were similar for all three groups. The gingival index was greater in the T2D group compared to the Ob and C groups. Although salivary microbial diversity was minimal between groups, a few differences in bacterial genus composition were noted. Conclusions: Obese children with T2D show a trend toward poorer oral health compared to normal weight and obese children without T2D. This study characterizes the salivary microbiome of children with and without obesity and T2D. This study supports a modest link between T2D and periodontal inflammation in the pediatric population.

Abstract:Dans cette these nous nous interessons a des problemes d’apprentissage automatique dans le cadre de sorties structurees avec une structure sequentielle. D’une part, nous considerons le probleme de l’apprentissage de mesure de similarite pour deux tâches : (i) la detection de rupture dans des signaux multivaries et (ii) le probleme de deformation temporelle entre paires de signaux. Les methodes generalement utilisees pour resoudre ces deux problemes dependent fortement d’une mesure de similarite. Nous apprenons une mesure de similarite a partir de donnees totalement etiquetees. Nous presentons des algorithmes usuels de prediction structure, efficaces pour effectuer l’apprentissage. Nous validons notre approche sur des donnees reelles venant de divers domaines. D’autre part, nous nous interessons au probleme de la faible supervision pour la tâche d’alignement d’un enregistrement audio sur la partition jouee. Nous considerons la partition comme une representation symbolique donnant (i) une information complete sur l’ordre des symboles et (ii) une information approximative sur la forme de l’alignement attendu. Nous apprenons un classifieur pour chaque symbole avec ces informations. Nous developpons une methode d’apprentissage fondee sur l’optimisation d’une fonction convexe. Nous demontrons la validite de l’approche sur des donnees musicales.
The Associations of Cardiometabolic and Dietary Variables with Clinical Periodontitis in Adults with and without Type 2 Diabetes: A Cross-Sectional Study

Abstract:Periodontitis is a commonly occurring inflammatory oral disease affecting a large proportion of global and US adults and is characterized by the destruction of the tooth-supporting apparatus. Its etiology is multifactorial, and type 2 diabetes and diet play critical roles in its remission and progression. However, few studies have addressed nutritional and serum vitamin D status in adults with periodontitis in the presence of diabetes. A cross-sectional study (n = 78), and a sub-set of age- and BMI-matched case-control studies (n = 50), were conducted to examine differences in dietary and cardiometabolic variables, and serum vitamin D in adults with periodontitis with or without diabetes. Participants provided fasting blood samples and 24-h diet recalls on at least two different days. Data on health history, body weight, height, nutritional habits, and clinical features of periodontitis were also collected. The Mann-Whitney U Test (with exact p-value estimation by Monte Carlo simulation) was used to examine differences by diabetes status in continuous and ordinal variables. Results revealed significantly lower serum vitamin D, and dietary intake of fruits, vegetables, dairy, vitamins A and C in adults with periodontitis with vs. without diabetes in the sub-study (all p < 0.05). In the overall sample, adults with diabetes presented with higher caries risk measures and lower numbers of teeth than those without diabetes; plaque and bleeding scores did not differ by diabetes status. Finally, a significant associations of food habits was observed, especially consuming protein-rich foods twice a day with a lower bleeding score, and daily consumption of fried or fast foods with a fewer number of teeth present (all p < 0.05). The present findings show significant dietary and serum vitamin D inadequacies among adults with periodontitis, and diabetes further aggravates the observed malnourishment and oral health.
