Abstract:During the COVID-19 pandemic, renewal equation estimates of time-varying effective reproduction numbers were useful to policymakers in evaluating the need for and impact of mitigation measures. Our objective here is to illustrate the utility of mechanistic expressions for the basic and effective (or intrinsic and realized) reproduction numbers, [Formula: see text] and related quantities derived from a Susceptible-Exposed-Infectious-Removed (SEIR) model including features of COVID-19 that might affect transmission of SARS-CoV-2, including asymptomatic, pre-symptomatic, and symptomatic infections, with which people may be hospitalized. Expressions from homogeneous host population models can be analyzed to determine the effort needed to reduce [Formula: see text] from [Formula: see text] to 1 and contributions of modeled mitigation measures. Our model is stratified by age, 0-4, 5-9, …, 75+ years, and location, the 50 states plus District of Columbia. Expressions from such heterogeneous host population models include subpopulation reproduction numbers, contributions from the above-mentioned infectious states, metapopulation numbers, subpopulation contributions, and equilibrium prevalence. While the population-immunity at which [Formula: see text] has captured the popular imagination, the metapopulation [Formula: see text] could be attained in an infinite number of ways even if only one intervention (e.g., vaccination) were capable of reducing [Formula: see text] However, gradients of expressions derived from heterogeneous host population models,[Formula: see text] can be evaluated to identify optimal allocations of limited resources among subpopulations. We illustrate the utility of such analytical results by simulating two hypothetical vaccination strategies, one uniform and other indicated by [Formula: see text] as well as the actual program estimated from one of the CDC's nationwide seroprevalence surveys conducted from mid-summer 2020 through the end of 2021.
Modelling preventive measures and their effect on generation times in emerging epidemics

Abstract:We present a stochastic epidemic model to study the effect of various preventive measures, such as uniform reduction of contacts and transmission, vaccination, isolation, screening and contact tracing, on a disease outbreak in a homogeneously mixing community. The model is based on an infectivity process, which we define through stochastic contact and infectiousness processes, so that each individual has an independent infectivity profile. In particular, we monitor variations of the reproduction number and of the distribution of generation times. We show that some interventions, i.e. uniform reduction and vaccination, affect the former while leaving the latter unchanged, whereas other interventions, i.e. isolation, screening and contact tracing, affect both quantities. We provide a theoretical analysis of the variation of these quantities, and we show that, in practice, the variation of the generation time distribution can be significant and that it can cause biases in the estimation of reproduction numbers. The framework, because of its general nature, captures the properties of many infectious diseases, but particular emphasis is on COVID-19, for which numerical results are provided.

Abstract:Social contact networks and the way people interact with each other are the key factors that impact on epidemics spreading. However, it is challenging to model the behavior of epidemics based on social contact networks due to their high dynamics. Traditional models such as susceptible-infected-recovered (SIR) model ignore the crowding or protection effect and thus has some unrealistic assumption. In this paper, we consider the crowding or protection effect and develop a novel model called improved SIR model. Then, we use both deterministic and stochastic models to characterize the dynamics of epidemics on social contact networks. The results from both simulations and real data set conclude that the epidemics are more likely to outbreak on social contact networks with higher average degree. We also present some potential immunization strategies, such as random set immunization, dominating set immunization, and high degree set immunization to further prove the conclusion.
Dynamics of COVID-19 models with asymptomatic infections and quarantine measures

Abstract:Considering the propagation characteristics of COVID-19 in different regions, the dynamics analysis and numerical demonstration of long-term and short-term models of COVID-19 are carried out, respectively. The long-term model is devoted to investigate the global stability of COVID-19 model with asymptomatic infections and quarantine measures. By using the limit system of the model and Lyapunov function method, it is shown that the COVID-19-free equilibrium $V^0$ is globally asymptotically stable if the control reproduction number $\mathcal{R}_{c}<1$ and globally attractive if $\mathcal{R}_{c}=1$, which means that COVID-19 will die out; the COVID-19 equilibrium $V^{\ast}$ is globally asymptotically stable if $\mathcal{R}_{c}>1$, which means that COVID-19 will be persistent. In particular, to obtain the local stability of $V^{\ast}$, we use proof by contradiction and the properties of complex modulus with some novel details, and we prove the weak persistence of the system to obtain the global attractivity of $V^{\ast}$. Moreover, the final size of the corresponding short-term model is calculated and the stability of its multiple equilibria is analyzed. Numerical simulations of COVID-19 cases show that quarantine measures and asymptomatic infections have a non-negligible impact on the transmission of COVID-19.

The Effect of Population Size for Pathogen Transmission on Prediction of COVID-19 Pandemic Spread

Abstract:Extreme public health interventions play a critical role in mitigating the local and global prevalence and pandemic potential of COVID-19. Here, we use population size for pathogen transmission to measure the intensity of public health interventions, which is a key characteristic variable for nowcasting and forecasting of the epidemic. By formulating a hidden Markov dynamic system and using nonlinear filtering theory, we have developed a stochastic epidemic dynamic model under public health interventions. The model parameters and states are estimated in time from internationally available public data by combining an unscented filter and an interacting multiple model filter. Moreover, we consider the computability of the population size and provide its selection criterion. We estimate the mean of the basic reproductive number of China and the rest of the globe except China (GEC) to be 2.46 (95% CI: 2.41-2.51) and 3.64 (95% CI: (3.55-3.72), respectively. We infer that the number of latent infections of GEC is about 7.47*10^5 (95% CI: 7.32*10^5-7.62*10^5) as of April 2, 2020. We predict that the peak of infections in hospitals of GEC may reach 3.00*10^6 on the present trajectory, i.e., if the population size for pathogen transmission and epidemic parameters remains unchanged. If the control intensity is strengthened, e.g., 50% reduction or 75% reduction of the population size for pathogen transmission, the peak would decline to 1.84*10^6, 1.27*10^6, respectively.

Abstract:Mathematical and computational models are useful tools for virtual policy experiments on infectious disease control. Most models fail to provide flexible and rapid simulation of various epidemic scenarios for policy assessment. This paper establishes a multi-scale agent-based model to investigate the infectious disease propagation between cities and within a city using the knowledge from person-to-person transmission. In the model, the contact and infection of individuals at the micro scale where an agent represents a person provide insights for the interactions of agents at the meso scale where an agent refers to hundreds of individuals. Four cities with frequent population movements in China are taken as an example and actual data on traffic patterns and demographic parameters are adopted. The scenarios for dynamic propagation of infectious disease with no external measures are compared versus the scenarios with vaccination and non-pharmaceutical interventions. The model predicts that the peak of infections will decline by 67.37% with 80% vaccination rate, compared to a drop of 89.56% when isolation and quarantine measures are also in place. The results highlight the importance of controlling the source of infection by isolation and quarantine throughout the epidemic. We also study the effect when cities implement inconsistent public health interventions, which is common in practical situations. Based on our results, the model can be applied to COVID-19 and other infectious diseases according to the various needs of government agencies.

Abstract:Abstract Background The global spread of the COVID-19 pandemic has become the most fundamental threat to human health. In the absence of vaccines and effective therapeutical solutions, non-pharmaceutic intervention has become a major way for controlling the epidemic. Gentle mitigation interventions are able to slow down the epidemic but not to halt it well. While strict suppression interventions are efficient for controlling the epidemic, long-term measures are likely to have negative impacts on economics and people’s daily live. Hence, dynamically balancing suppression and mitigation interventions plays a fundamental role in manipulating the epidemic curve. Methods We collected data of the number of infections for several countries during the COVID-19 pandemics and found a clear phenomenon of periodic waves of infection. Based on the observation, by connecting the infection level with the medical resources and a tolerance parameter, we propose a mathematical model to understand impacts of combining intervention measures on the epidemic dynamics. Results Depending on the parameters of the medical resources, tolerance level, and the starting time of interventions, the combined intervention measure dynamically changes with the infection level, resulting in a periodic wave of infections controlled below an accepted level. The study reveals that, (a) with an immediate, strict suppression, the numbers of infections and deaths are well controlled with a significant reduction in a very short time period; (b) an appropriate, dynamical combination of suppression and mitigation may find a feasible way in reducing the impacts of epidemic on people’s live and economics. Conclusions While the assumption of interventions deployed with a cycle of period in the model is limited and unrealistic, the phenomenon of periodic waves of infections in reality is captured by our model. These results provide helpful insights for policy-makers to dynamically deploy an appropriate intervention strategy to effectively battle against the COVID-19.

Abstract:Within a short period of time, COVID-19 grew into a world-wide pandemic. Transmission by pre-symptomatic and asymptomatic viral carriers rendered intervention and containment of the disease extremely challenging. Based on reported infection case studies, we construct an epidemiological model that focuses on transmission around the symptom onset. The model is calibrated against incubation period and pairwise transmission statistics during the initial outbreaks of the pandemic outside Wuhan with minimal non-pharmaceutical interventions. Mathematical treatment of the model yields explicit expressions for the size of latent and pre-symptomatic subpopulations during the exponential growth phase, with the local epidemic growth rate as input. We then explore reduction of the basic reproduction number R_0 through specific disease control measures such as contact tracing, testing, social distancing, wearing masks and sheltering in place. When these measures are implemented in combination, their effects on R_0 multiply. We also compare our model behaviour to the first wave of the COVID-19 spreading in various affected regions and highlight generic and less generic features of the pandemic development.
An infectious disease model with asymptomatic transmission and waning immunity

Abstract:Infectious diseases present persistent challenges to global public health, demanding a comprehensive understanding of their dynamics to develop effective prevention and control strategies. The presence of asymptomatic carriers, individuals capable of transmitting pathogens without displaying symptoms, challenges conventional containment approaches focused on symptomatic cases. Waning immunity, the decline in protective response following natural recovery or vaccination, introduces further complexity to disease dynamics. In this paper, we developed a mathematical model to investigate the interplay between these factors, aiming to inform strategies for the management of infectious diseases. We derived the basic reproduction number for the model and showed that the disease would die out when this number falls below 1. We obtained a formula to estimate the relative contributions of asymptomatic and symptomatic transmission to the basic reproduction number, which remains unchanged when vaccination is included in the model. Through computer simulations with parameter values tailored for COVID-19 and sensitivity analysis, we demonstrated that population susceptibility significantly impacts the timing and magnitude of infection peaks. Populations with lower susceptibility experience delayed and less severe outbreaks. Vaccination was shown to play a crucial role in disease control, with an increased vaccination rate, extended immunity, and heightened vaccine efficacy proving pivotal. However, the effectiveness of these strategies hinges on maintaining a low vaccine escape proportion. Taken together, this study underscores the need for multifaceted, adaptable approaches to infectious disease management, highlighting the central role of vaccination in mitigating disease spread. Further research and validation with disease-specific data will enhance parameter estimates, improve model predictions, and inform evidence-based disease control strategies.

Abstract:Almost all models used in analysis of infectious disease outbreaks contain some notion of population size, usually taken as the census population size of the community in question. In many settings, however, the census population is not equivalent to the population likely to be exposed, for example if there are population structures, outbreak controls or other heterogeneities. Although these factors may be taken into account in the model: adding compartments to a compartmental model, variable mixing rates and so on, this makes fitting more challenging, especially if the population complexities are not fully known. In this work we consider the concept of effective population size in outbreak modelling, which we define as the size of the population involved in an outbreak, as an alternative to use of more complex models. Effective population size is an important quantity in genetics for estimation of genetic diversity loss in populations, but it has not been widely applied in epidemiology. Through simulation studies and application to data from outbreaks of COVID-19 in China, we find that simple SIR models with effective population size can provide a good fit to data which are not themselves simple or SIR.

Abstract:Individual heterogeneity (i.e., age and underlying health condition) and contact pattern heterogeneity (i.e., circumstances and frequency of contact) are two crucial aspects of heterogeneous population, which have been demonstrated markedly affect the dynamical process and effectiveness of containing strategy of infectious disease. The former primarily influences susceptibility to the disease, while the latter predominantly controls the spreading process (e.g., "super-spreading" events often occur within households). In this study, we propose a contact data-driven infectious disease spreading model with age structure, underlying health conditions, and containing measures, as well as two types of heterogeneity-based containing strategies for heterogeneous populations. Based on this, a theoretical analysis framework is developed by extending mean-field theory. Using the next-generation matrix method, we estimate the basic reproduction number, finding it to be sensitive to individual heterogeneity, contact pattern heterogeneity, and containing measures. We perform extensive simulations for the spreading of Covid-19 and influenza in 31 cities and find that stronger household infection within contact pattern heterogeneity and the implementation of immunization both lead to a trade-off between the final cumulative infection density (daily infection density peak) and pandemic duration. Individual heterogeneity and contact pattern heterogeneity result in uneven distribution of infection density across different age groups and affect the immunization effects of different immunization strategies. When considering the impact of individual heterogeneity, immunizing only individuals without underlying diseases within each age group is insufficient to achieve optimal control; individuals with underlying diseases should be encouraged to get vaccinated as well. When considering the impact of contact pattern heterogeneity, the results from Anhui indicate that immunizing the 40–59 age group is most effective in the early stages of the epidemic, while overall, immunizing the 20–39 age group is most effective.

Abstract:The correct evaluation of the reproductive number $R$ for COVID-19 -- which characterizes the average number of secondary cases generated by each typical primary case -- is central in the quantification of the potential scope of the pandemic and the selection of an appropriate course of action. In most models, $R$ is modeled as a universal constant for the virus across outbreak clusters and individuals -- effectively averaging out the inherent variability of the transmission process due to varying individual contact rates, population densities, demographics, or temporal factors amongst many. Yet, due to the exponential nature of epidemic growth, the error due to this simplification can be rapidly amplified and lead to inaccurate predictions and/or risk evaluation. From the statistical modeling perspective, the magnitude of the impact of this averaging remains an open question: how can this intrinsic variability be percolated into epidemic models, and how can its impact on uncertainty quantification and predictive scenarios be better quantified? In this paper, we propose to study this question through a Bayesian perspective, creating a bridge between the agent-based and compartmental approaches commonly used in the literature. After deriving a Bayesian model that captures at scale the heterogeneity of a population and environmental conditions, we simulate the spread of the epidemic as well as the impact of different social distancing strategies, and highlight the strong impact of this added variability on the reported results. We base our discussion on both synthetic experiments -- thereby quantifying of the reliability and the magnitude of the effects -- and real COVID-19 data.

[Comparative studies of wavefront aberration and contrast sensitivity after phacoemulsification and intraocular lens implantation].

Abstract:OBJECTIVE To study the difference of the contrast sensitivity and wavefront aberration of two eyes who undergone phacoemulsification and intraocular transplantation respectively. METHODS Sixty-three patients with cataract were included in the study. With the consent of the patients, one eye was implanted SA60AT intraocular lens and another eye was implanted Tennis Z9001 intraocular lens after phacoemulsification. One and three months post-surgery, the best corrected visual acuity was recorded by computer and comprehensive optometry. Contrast sensitivity was analyzed by CSV-1000. The corneal aberration was measured by Option Keratron Scout. The whole ocular aberration was evaluated by Allegretto Waver Analyzer at pupillary diameter 4 mm and 6 mm. RESULTS The best corrected visual acuity and the corneal spherical aberration, corneal coma aberration and RMS had no significant difference between the two groups. After one and three months, the contrast sensitivity of 3, 6, 12, 18c/d had statistic significance between the eye with and without glare through univariate split-plot analysis. The Z9001 contrast sensitivity value was much higher than the SA60AT groups. The results of Zernike polynomials C12, RMS4 and RMSh had statistic significance between the two groups through T-test. The Z9001 group value was much lower than the SA60AT groups. CONCLUSIONS The Z9001 intraocular lens transplantation can reduce total ocular aberration, special the spherical aberration and improve visual acuity.
Spatial Heterogeneity and Infection Patterns on Epidemic Transmission Disclosed by a Combined Contact-Dependent Dynamics and Compartmental Model

Abstract:Epidemics, such as COVID-19, have caused significant harm to human society worldwide. A better understanding of epidemic transmission dynamics can contribute to more efficient prevention and control measures. Compartmental models, which assume homogeneous mixing of the population, have been widely used in the study of epidemic transmission dynamics, while agent-based models rely on a network definition for individuals. In this study, we developed a real-scale contact-dependent dynamic (CDD) model and combined it with the traditional susceptible-exposed-infectious-recovered (SEIR) compartment model. By considering individual random movement and disease spread, our simulations using the CDD-SEIR model reveal that the distribution of agent types in the community exhibits spatial heterogeneity. The estimated basic reproduction number R0 depends on group mobility, increasing logarithmically in strongly heterogeneous cases and saturating in weakly heterogeneous conditions. Notably, R0 is approximately independent of virus virulence when group mobility is low. We also show that transmission through small amounts of long-term contact is possible due to short-term contact patterns. The dependence of R0 on environment and individual movement patterns implies that reduced contact time and vaccination policies can significantly reduce the virus transmission capacity in situations where the virus is highly transmissible (i.e., R0 is relatively large). This work provides new insights into how individual movement patterns affect virus spreading and how to protect people more efficiently.
The Impact of Contact Structure and Mixing on Control Measures and Disease-Induced Herd Immunity in Epidemic Models: A Mean-Field Model Perspective

Abstract:Abstract The contact structure of a population plays an important role in transmission of infection. Many ‘structured models’ capture aspects of the contact pattern through an underlying network or a mixing matrix. An important observation in unstructured models of a disease that confers immunity is that once a fraction $$1-1/{\mathcal {R}}_0$$ 1 - 1 / R 0 has been infected, the residual susceptible population can no longer sustain an epidemic. A recent observation of some structured models is that this threshold can be crossed with a smaller fraction of infected individuals, because the disease acts like a targeted vaccine, preferentially immunising higher-risk individuals who play a greater role in transmission. Therefore, a limited ‘first wave’ may leave behind a residual population that cannot support a second wave once interventions are lifted. In this paper, we set out to investigate this more systematically. While networks offer a flexible framework to model contact patterns explicitly, they suffer from several shortcomings: (i) high-fidelity network models require a large amount of data which can be difficult to harvest, and (ii) very few, if any, theoretical contact network models offer the flexibility to tune different contact network properties within the same framework. Therefore, we opt to systematically analyse a number of well-known mean-field models. These are computationally efficient and provide good flexibility in varying contact network properties such as heterogeneity in the number contacts, clustering and household structure or differentiating between local and global contacts. In particular, we consider the question of herd immunity under several scenarios. When modelling interventions as changes in transmission rates, we confirm that in networks with significant degree heterogeneity, the first wave of the epidemic confers herd immunity with significantly fewer infections than equivalent models with less or no degree heterogeneity. However, if modelling the intervention as a change in the contact network, then this effect may become much more subtle. Indeed, modifying the structure disproportionately can shield highly connected nodes from becoming infected during the first wave and therefore make the second wave more substantial. We strengthen this finding by using an age-structured compartmental model parameterised with real data and comparing lockdown periods implemented either as a global scaling of the mixing matrix or age-specific structural changes. Overall, we find that results regarding (disease-induced) herd immunity levels are strongly dependent on the model, the duration of the lockdown and how the lockdown is implemented in the model.

Abstract:The large proportion of asymptomatic patients is the major cause leading to the COVID-19 pandemic which is still a significant threat to the whole world. A six-dimensional ODE system (SEIAQR epidemical model) is established to study the dynamics of COVID-19 spreading considering infection by exposed, infected, and asymptomatic cases. The basic reproduction number derived from the model is more comprehensive including the contribution from the exposed, infected, and asymptomatic patients. For this more complex six-dimensional ODE system, we investigate the global and local stability of disease-free equilibrium, as well as the endemic equilibrium, whereas most studies overlooked asymptomatic infection or some other virus transmission features. In the sensitivity analysis, the parameters related to the asymptomatic play a significant role not only in the basic reproduction number R0. It is also found that the asymptomatic infection greatly affected the endemic equilibrium. Either in completely eradicating the disease or achieving a more realistic goal to reduce the COVID-19 cases in an endemic equilibrium, the importance of controlling the asymptomatic infection should be emphasized. The three-dimensional phase diagrams demonstrate the convergence point of the COVID-19 spreading under different initial conditions. In particular, massive infections will occur as shown in the phase diagram quantitatively in the case R0>1. Moreover, two four-dimensional contour maps of Rt are given varying with different parameters, which can offer better intuitive instructions on the control of the pandemic by adjusting policy-related parameters.

Abstract:<p>The 2019 novel coronavirus (COVID-19) emerged at the end of 2019 has a great impact on China and all over the world. The transmission mechanism of COVID-19 is still unclear. Except for the initial status and the imported cases, the isolation measures and the medical treatments of the infected patients have essential influences on the spread of COVID-19. In this paper, we establish a mathematical model for COVID-19 transmission involving the interactive effect of various factors for the infected people, including imported cases, isolating rate, diagnostic rate, recovery rate and also the mortality rate. Under the assumption that the random incubation period, the cure period and the diagnosis period are subject to the Weibull distribution, the quantity of daily existing infected people is finally governed by a linear integral-differential equation with convolution kernel. Based on the asymptotic behavior and the quantitative analysis on the model, we rigorously prove that, for limited external input patients, both the quantity of infected patients and its variation ratio will finally tend to zero, if the infected patients are sufficiently isolated or the infection rate is small enough. Finally, numerical performances for the proposed model as well as the comparisons between our simulations and the clinical data of the city Wuhan and Italy are demonstrated, showing the validity of our model with suitably specified model parameters.</p>

A Nested Multi-Scale Model for COVID-19 Viral Infection

Abstract:In this study, we develop and analyze a nested multi-scale model for COVID -19 disease that integrates within-host scale and between-host scale sub-models. First, the well-posedness of the multi-scale model is discussed, followed by the stability analysis of the equilibrium points. The disease-free equilibrium point is shown to be globally asymptotically stable for $R_0 < 1$. When $R_0$ exceeds unity, a unique infected equilibrium exists, and the system is found to undergo a forward (trans-critical) bifurcation at $R_0=1$. Two parameter heat plots are also done to find the parameter combinations for which the equilibrium points are stable. The parameters $\beta, \pi$ and $\Lambda$ are found to be most sensitive to $R_0$. The influence of within-host sub-model parameter on the between-host sub-model variables is numerically illustrated. The spread of infection in a community is shown to be influenced by within-host level sub-model parameters, such as the production of viral particles by infected cells $(\alpha)$, the clearance rate of infected cells by the immune system $(x)$, and the clearance rate of viral particles by the immune system $(y)$. The comparative effectiveness of the three health interventions (antiviral drugs, immunomodulators, and generalized social distancing) for COVID-19 infection was examined using the effective reproductive number $R_E$ as an indicator of the effectiveness of the interventions. The results suggest that a combined strategy of antiviral drugs, immunomodulators and generalized social distancing would be the best strategy to implement to contain the spread of infection in the community.
Adaptive Multi-Factor Quantitative Analysis and Prediction Models: Vaccination, Virus Mutation and Social Isolation on COVID-19

Abstract:Different countries have adopted various control measures for the COVID-19 pandemic in different periods, and as the virus continues to mutate, the progression of the pandemic and preventive measures adopted have varied dynamically over time. Thus, quantitative analysis of the dynamic impact of different factors such as vaccination, mutant virus, social isolation, etc., on transmission and predicting pandemic progress has become a difficult task. To overcome the challenges above and enable governments to formulate reasonable countermeasures against the ongoing COVID-19 pandemic, we integrate several mathematical methods and propose a new adaptive multifactorial and geographically diverse epidemiological model based on a modified version of the classical susceptible-exposed-infectious-recovered (SEIR) model. Based on public datasets, a multi-center study was carried out considering 21 regions. First, a retrospective study was conducted to predict the number of infections over the next 30 days in 13 representative pandemic areas worldwide with an accuracy of 87.53%, confirming the robustness of the proposed model. Second, the impact of three scenarios on COVID-19 was quantified based on the scalability of the model: two different vaccination regimens were analyzed, and it was found that the number of infections would progressively decrease over time after vaccination; variant virus caused a 301.55% increase in infections in the United Kingdom; and 3-tier social lockdown in the United Kingdom reduced the infections by 47.01%. Third, we made short-term prospective predictions for the next 15 and 30 days for six countries with severe COVID-19 transmission and the predicted trend is accurate. Abstract:The global crisis of the COVID-19 pandemic has highlighted the need for mathematical models to inform public health strategies. The present study introduces a novel six-compartment epidemiological model that uniquely incorporates a higher isolation rate for unreported symptomatic cases of COVID-19 compared to reported cases, aiming to enhance prediction accuracy and address the challenge of initial underreporting. Additionally, we employ optimal control theory to assess the cost-effectiveness of interventions and adapt these strategies to specific epidemiological scenarios, such as varying transmission rates and the presence of asymptomatic carriers. By applying this model to COVID-19 data from India (30 January 2020 to 24 November 2020), chosen to capture the initial outbreak and subsequent waves, we calculate a basic reproduction number of 2.147, indicating the high transmissibility of the virus during this period in India. A sensitivity analysis reveals the critical impact of detection rates and isolation measures on disease progression, showing the robustness of our model in estimating the basic reproduction number. Through optimal control simulations, we demonstrate that increasing isolation rates for unreported cases and enhancing detection reduces the spread of COVID-19. Furthermore, our cost-effectiveness analysis establishes that a combined strategy of isolation and treatment is both more effective and economically viable. This research offers novel insights into the efficacy of non-pharmaceutical interventions, providing a tool for strategizing public health interventions and advancing our understanding of infectious disease dynamics.
