Abstract:A new mode of transfer excitation (TE) process in high-energy ion-atom collisions is analyzed in which two or more electrons in the target system are involved incoherently in the excitation of the projectile ion and electron transfer. This uncorrelated TE mode (UTE) is shown to resolve the long-standing discrepancy in the high-energy tail of the resonant TE (RTE) cross section in the Nb{sup 31+}+H{sub 2} collisions, and explains satisfactorily the recent experimental studies of the system F{sup 6+}+H{sub 2} by Schulz {ital et} {ital al}. (Phys. Rev. Abstract:INTRODUCTION:Non-communicable diseases (NCD) now represent the major burden of adverse health in most countries. It is clear that much of the risk of such conditions begins very early in life, potentially in utero. Given their complex aetiology, an understanding of the origins of NCD requires an in-depth analysis of the interplay between genetic variation and environment, preferably over time. For decades, twin studies have played a key role in understanding such traits. Their strength lies in the ability to disentangle genetic and environmental factors that contribute to a phenotype. This is done by comparing genetically identical monozygotic (MZ) with dizygotic twins, who share on average 50% of genetic variation, or by comparing MZ twins within a pair. This study aims to determine the relative contributions of genes and environment to early-onset intermediate phenotypes related to later adult onset disease (such as growth and neurodevelopment) and to identify specific biomarkers and time points for emergence of phenotypes from infancy, largely independent of underlying genetic factors.METHODS/DESIGN:The Chongqing Longitudinal Twin Study (LoTiS) will recruit 300 women pregnant with twins, enriched for MZ pregnancies, with follow-up to 3 years of age. Data collection will be undertaken at key time points in gestation (×3), at delivery and postnatally (×9). Maternal and infant biospecimens including blood, urine, hair, nails and buccal swabs along with measures such as fetal scans and body measurements will be collected. Additional information from questionnaires and medical records includes pregnancy, diet, sociodemographics, maternal stress, and infant growth and neurodevelopment.ETHICS AND DISSEMINATION:This study has been approved by the Ethics Committee of Chongqing Medical University (record no: 201530) and has been registered with the Chinese Clinical Trial Registry (registry no: ChiCTR-OOC-16008203). Prenatal Naled and Chlorpyrifos Exposure is Associated with Deficits in Infant Motor Function in a Cohort of Chinese Infants.

Abstract:Background: Organophosphate insecticides (OPs) are used worldwide, yet despite nearly ubiquitous exposure in the general population, few have been studied outside the laboratory. Fetal brains undergo rapid growth and development, leaving them susceptible to long-term effects of neurotoxic OPs. The objective here was to investigate the extent to which prenatal exposure to OPs affects infant motor development.Methods: 30 OPs were measured in umbilical cord blood using gas chromatography tandem mass spectrometry in a cohort of Chinese infants. Motor function was assessed at 6-weeks and 9-months using Peabody Developmental Motor Scales 2nd edition (PDMS-2) (n = 199). Outcomes included subtest scores: reflexes, stationary, locomotion, grasping, visual-motor integration (V-M), composite scores: gross (GM), fine (FM), total motor (TM), and standardized motor quotients: gross (GMQ), fine (FMQ), total motor (TMQ).Results: Naled, methamidophos, trichlorfon, chlorpyrifos, and phorate were detected in = 10% of samples. Prenatal naled and chlorpyrifos were associated with decreased 9-month motor function. Scores were 0.55, 0.85, and 0.90 points lower per 1 ng/mL increase in log-naled, for V-M (p = 0.04), FM (p = 0.04), and FMQ (p = 0.08), respectively. For chlorpyrifos, scores were 0.50, 1.98, 0.80, 1.91, 3.49, 2.71, 6.29, 2.56, 2.04, and 2.59 points lower for exposed versus unexposed infants, for reflexes (p = 0.04), locomotion (p = 0.02), grasping (p = 0.05), V-M (p < 0.001), GM (p = 0.007), FM (p = 0.002), TM (p < 0.001), GMQ (p = 0.01), FMQ (p = 0.07), and TMQ (p = 0.008), respectively. Girls appeared to be more sensitive to the negative effects of OPs on 9-month motor function than boys.Conclusions: We found deficits in 9-month motor function in infants with prenatal exposure to naled and chlorpyrifos. Naled is being aerially sprayed to combat mosquitoes carrying Zika virus, yet this is the first nonoccupational human study of its health effects. Delays in early-motor skill acquisition may be detrimental for downstream development and cognition.

Abstract:Prenatal pesticide exposure may adversely affect child neurodevelopment, and this may partly arise from impairing the placenta's vital role in fetal development. In a cohort of pregnant farmworkers from Thailand (N=248), we examined the links between urinary metabolites of pyrethroid pesticides during pregnancy, placental gene expression networks derived from transcriptome sequencing, and newborn neurobehavior assessed using the NICU Network Neurobehavioral Scales (NNNS) at 5 weeks of age. The urinary concentrations of cis-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropane carboxylic acid (cis-DCCA) during pregnancy were found to be significantly positively correlated with the NNNS scores for attention (β = 0.49, p = 0.005), handling (β = 1.04, p = 0.04), and excitability (β = 0.15, p = 0.02). The urinary concentrations of trans-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropane carboxylic acid (trans-DCCA) also showed a significant positive association with attention (β = 0.49, p = 0.01). Focusing on the 21 gene network modules in the placenta identified by Weighted Gene Co-expression Network Analysis (WGCNA), our analysis revealed significant associations between metabolites and nine distinct modules, and between thirteen modules and NNNS, with eight modules showing overlap. Notably, stress was negatively associated with the middleblue module (interferon alpha response) and the salmon module (Myc target). The middleblue module was correlated with attention, arousal, and quality of movement. The analysis also highlighted the first and third trimesters as critical periods for the influence of exposures on placental function, with pyrethroid metabolites measured early in pregnancy significantly negatively associated with the turquoise module (protein secretion), and those measured later in pregnancy having negative associations with modules related to Oxidative Phosphorylation (OXPHOS) and DNA repair. Additionally, the cumulative sum of 3PBA across pregnancy was significantly negatively associated with the lightyellow module (OXPHOS). These findings suggest that prenatal exposure to pyrethroid pesticides may influence neonatal neurobehavior through specific placental mechanisms that impact gene expression and metabolic pathway, and that the effects of environmental pyrethroid exposures on fetal neurodevelopment varies throughout pregnancy. These results offer valuable insights for future risk assessment and intervention strategies.

Abstract:Sir—We read with interest the article by Tan et al. [1], which is a retrospective review of 6 cases of legionnaires disease with bacteremic coinfection. Although coinfection may occur, the diagnosis should be established on firm grounds. In the 6 patients described by the authors, the diagnosis was based on antibody response (3 patients) or detection of Legionella antigen in urine specimens (3 patients). Both methods have shortcomings. The specificity of antibody tests is questionable because of cross-reactions with other organisms [2]. The results of Legionella antigen detection in the urine, assuming that it has 100% specificity (which is questionable), remain positive several months after infection occurs, suggesting possible falsepositive reactions [3]. Abstract:Introduction Preterm (PT) infants are at high likelihood for poor neurodevelopmental outcomes, including autism spectrum disorder (ASD), attention-deficit hyperactivity disorder (ADHD) and other neurodevelopmental disorders (NDDs), which could considerably impair the individuals' functions throughout their whole life. The current cohort study aims to investigate adverse outcomes, especially NDDs, in PT children, and the related early aberrant brain developmental biomarkers. Methods and analysis This is a prospective cohort study in Beijing, China. We plan to recruit 400 PT infants born at <37 weeks of gestational age (GA), and 200 full-term (FT) controls during the neonatal period (40 weeks corrected GA), then follow them up until they reach 6 years of age. This cohort is designed to assess neuropsychological functions, brain development, related environmental risk factors and the incidence of NDDs by using the following measures: (1) social, emotional, cognitive and sensorimotor functions; (2) MRI, electroencephalogram and functional near-infrared spectroscopy; (3) social economic status, maternal mental health and DNA methylation; and (4) symptoms and diagnosis of NDDs. Main data analyses will include comparing the neurodevelopment outcomes and brain developmental trajectories between PT and FT children using linear or logistic regressions and mixed-effects models. Regression analyses and machine learning will be used to identify early biological predictors and environmental risk or protective factors for later NDDs outcomes. Ethics and dissemination Ethical approval has been obtained from the research ethics committee of Peking University Third Hospital (M2021087). This study is under review in the Chinese Clinical Trial Register. The study results from the current cohort will be disseminated and popularised through social media to participating parents, as well as parents who are giving care to PT children.

Neurobehavioral Performance in Preschool Children Exposed Postnatally to Organophosphates in Agricultural Regions, Northern Thailand

Abstract:Evidence of the effects of postnatal exposure to organophosphates (OPs) on children's neurodevelopment remains limited but crucial. This cross-sectional study evaluated exposure to OPs and neurobehavioral performance in 172 preschool children. Urinary dialkyl phosphate (DAP) metabolites, biomarkers for exposure to OPs, were measured. The neurobehavioral assessments included motor skills, memory, and cognitive function, measured using the Purdue pegboard test, digit span test, object memory test, and visual-motor integration. Multiple linear regression models were employed to explore the associations between urinary DAP metabolite levels and neurobehavioral performance, adjusting for potential confounders. Findings revealed that children of farming parents had higher urinary levels of dimethylphosphate (DMP) (Beta = 0.730, 95% CI = 0.138, 1.322, p value = 0.016) and diethylphosphate (DEP) (Beta = 0.668, 95% CI = 0.044, 1.291, p value = 0.036). Additionally, high fruit consumption correlated with increased urinary DEP levels (Beta = 0.398, 95% CI = 0.063, 0.733, p value = 0.020). Critically, elevated urinary DEP was associated with poorer fine motor coordination, affecting performance in the Purdue pegboard test for the dominant hand (Beta = −0.428, 95% CI = −0.661, −0.194, p value < 0.001), the preferred hand (Beta = −0.376, 95% CI = −0.603, −0.149, p value = 0.001), and both hands (Beta = −0.524, 95% CI = −0.773, −0.276, p value < 0.001). These findings highlight the role of parental occupation and diet in children's OP exposure and suggest that OP exposure negatively impacts fine motor coordination. Targeted interventions, such as promoting organic diets, enhancing workplace safety, and ongoing biomonitoring, are vital to reduce neurodevelopmental risks for vulnerable populations.

[The Current Status of End-of-Life Care in ALS: Progress and Personal Experience from the Past Ten Years in the USA].

Abstract:End-of-life (EOL) care and management in ALS is one of the least studied subjects within ALS management. In early 2000, Robert Wood Johnson and the ALS Association assembled an advisory committee to develop recommendations for the promotion of excellence during EOL care. We have reviewed this 10-year-old publication1) and describe the progress we have made in 16 different areas of EOL recommendations. Although there are some areas with little progress over the past 10 years, a few areas, particularly those regarding psychosocial management, symptomatic treatments, and ethical issues of EOL, have made major advancements during this time. the statements and recommendations for promoting excellence in EOL made 10 years ago appear to still be valid. We also review hastening death, a newly recognized issue of EOL, and discuss the potential reasons why Japanese and American patients chose tracheostomy-invasive ventilator at different rates. We conclude that EOL management is still evolving and in progress. Until we have a cure for ALS, every effort is being made to improve EOL care for patients with ALS.
Prospective Investigation of Folic Acid Supplements Before and During Early Pregnancy and Paediatric and Adult Cancers in the Chinese Children and Families Cohort: a Pilot Study in a Sample of Rural and Urban Families

Abstract:Objective To determine the feasibility of long-term prospective follow-up and ascertainment of cancer in offspring and mothers from the 1993-1995 Chinese Community Intervention Program that provided folic acid supplements before and during early pregnancy to reduce neural tube defects. Design Feasibility pilot study for a prospective cohort study. Setting Families residing during 2012-2013 in one rural and one urban county from 21 counties in 3 provinces in China included in the Community Intervention Program campaign. Participants The feasibility study targeted 560 families, including 280 from the rural and 280 from the urban county included in the large original study; about half of mothers in each group had taken and half had not taken folic acid supplements. Intervention The planned new study is observational. Primary and secondary outcome measures Primary: incidence of paediatric cancers in offspring; secondary: other chronic diseases in offspring and chronic diseases in mothers Results Only 3.4% of pilot study families could not be found, 3.9% had moved out of the study area and 8.8% refused to participate. Interviews were completed by 82% of mothers, 79% of fathers and 83% of offspring in the 560 families. Almost all mothers and offspring who were interviewed also participated in anthropometric measurements. We found notable urban-rural differences in sociodemographic and lifestyle characteristics of the parents, but fewer differences among the offspring. In eight catchment area hospitals, we identified a broad range of paediatric cancers diagnosed during 1994-2013, although paediatric brain tumours, lymphomas and rarer cancers were likely under-represented. Conclusions Overall, 20 years after the original Community Intervention Program, the pilot study achieved high levels of follow-up and family member interview participation, and identified substantial numbers of paediatric malignancies during 1994-2013 in catchment area hospitals. Next steps and strategies for overcoming limitations are described.
Prenatal and childhood exposure to organophosphate pesticides and functional brain imaging in young adults

Abstract:Background: Early life exposure to organophosphate (OP) pesticides has been linked with poorer neurodevelopment from infancy to adolescence. In our Center for the Health Assessment of Mothers and Children of Salinas (CHAMACOS) birth cohort, we previously reported that residential proximity to OP use during pregnancy was associated with altered cortical activation using functional near infrared spectroscopy (fNIRS) in a small subset (n = 95) of participants at age 16 years. Methods: We administered fNIRS to 291 CHAMACOS young adults at the 18-year visit. Using covariate-adjusted regression models, we estimated associations of prenatal and childhood urinary dialkylphosphates (DAPs), non-specific OP metabolites, with cortical activation in the frontal, temporal, and parietal regions of the brain during tasks of executive function and semantic language. Results: There were some suggestive associations for prenatal DAPs with altered activation patterns in both the inferior frontal and inferior parietal lobes of the left hemisphere during a task of cognitive flexibility (β per ten-fold increase in DAPs = 3.37; 95% CI: -0.02, 6.77 and β = 3.43; 95% CI: 0.64, 6.22, respectively) and the inferior and superior frontal pole/dorsolateral prefrontal cortex of the right hemisphere during the letter retrieval working memory task (β = -3.10; 95% CI: -6.43, 0.22 and β = -3.67; 95% CI: -7.94, 0.59, respectively). We did not observe alterations in cortical activation with prenatal DAPs during a semantic language task or with childhood DAPs during any task. Discussion: We observed associations of prenatal OP concentrations with mild alterations in cortical activation during tasks of executive function. Associations with childhood exposure were null. This is reasonably consistent with studies of prenatal OPs and neuropsychological measures of attention and executive function found in CHAMACOS and other birth cohorts.
Prenatal and Postnatal Exposure to Organophosphate Pesticides and Child Neurodevelopment in an Agricultural Area of Jiangsu Province, China: a Cohort Study

Abstract:BackgroundChildren's exposure to organophosphorous pesticides attracted global concerns because of their vulnerability and physiological differences compared with adults. Few studies have examined the associations of both prenatal and postnatal organophosphorous exposure with child neurodevelopment in China. Our aim was to investigate the associations of maternal and child organophosphorous urinary metabolite concentrations with child neurodevelopment at age 2 years.MethodsTo assess exposure to organophosphorous pesticides, we measured dimethyl phosphate, diethyl phosphate, and total dialkyl phosphate metabolites in maternal urine collected before delivery and in urinary samples of their children aged 2 years (n=310). Healthy pregnant women who gave birth at local maternity hospitals without serious chronic diseases (eg, diabetes, hypertension, thyroid disease, or a serious pregnancy complication that could affect fetal growth and development) were recruited during June, 2009, to January, 2010. Neurodevelopment of children at age 2 years was assessed using the Gesell Developmental Schedules, and developmental quotients were calculated for four main categories of neurobehavioural functions—namely, motor, adaptive, language, and social functions. Multiple linear regression and logistic regression analysis were used for all children combined and also after stratification by sex. To compare effect estimates for urinary dialkyl phosphates measured in prenatal versus postnatal periods, seemingly unrelated regression estimation was used. All mothers who participated in the study signed an informed consent form and agreed to donate maternal and child urine samples. This study was approved by the health bureau of Sheyang county and the ethics committees of the School of Public Health, Fudan University, Shanghai, China.FindingsNo significant association between child neurodevelopment and urinary organophosphorous exposure was reported using multiple linear regression. Logistic regression analysis showed that elevated prenatal diethyl phosphates (odds ratio [OR] 9·75; p=0·028) and total dialkyl phosphates (9·69; p=0·072) were significantly associated with an increased probability of developmental delay in adaptive area. Elevated postnatal diethyl phosphates and total dialkyl phosphates were significantly associated with an increased probability of developmental delay; specifically, for every 10-times increase in postnatal total dialkyl phosphates, the probability of developmental delay increased by 2·58% (p=0·026) in motor area and 2·45 % (p=0·058) in social area. In relation to child neurodevelopment, the effect coefficients for organophosphorous exposure measured in children's urines were not significantly different from those for maternal metabolites. Boys showed adverse associations of both prenatal and postnatal organophosphorous exposure with developmental delay, but such associations were not reported in girls.InterpretationBoth prenatal and postnatal exposure to organophosphorous pesticides can adversely affect neurodevelopment of infants living in agricultural areas. This study added strength to the accumulating evidence on associations of prenatal and postnatal organophosphorous exposure with infant neurodevelopment. Abstract:ABSTRACT Purpose Arsenic exposure and micronutrient deficiencies may alter immune reactivity to influenza vaccination in pregnant women, transplacental transfer of maternal antibodies to the fetus, and maternal and infant acute morbidity. The Pregnancy, Arsenic, and Immune Response (PAIR) Study is a longitudinal pregnancy and birth cohort designed to assess whether arsenic exposure and micronutrient deficiencies alter maternal or newborn immunity and acute morbidity following maternal seasonal influenza vaccination during pregnancy. Participants We enrolled 784 pregnant women in rural Gaibandha District in northern Bangladesh between October 2018 and March 2019. Women received a quadrivalent seasonal inactivated influenza vaccine at enrollment in the late first or early second trimester between 11 and 17 weeks of gestational age. Follow-up included up to 13 visits between enrollment and three months postpartum as well as weekly telephone surveillance to ascertain influenza-like illness and other acute morbidity symptoms in women and infants. Tube well drinking water and urine specimens were collected to assess arsenic exposure. Of 784 women who enrolled, 736 (93.9%) delivered live births and 551 (70.3%) completed follow-up visit to three months postpartum. Findings to Date Arsenic was ≥0.02 μg/L in 97.9% of water specimens collected from participants at enrollment. The medians (interquartile ranges) of water and urinary arsenic were 5.1 (0.5-25.1) μg/L and 33.1 (19.6-56.5) μg/L, respectively. Water and urinary arsenic were strongly correlated (Spearman’s ρ=0.72) among women with water arsenic ≥ median but weakly correlated (ρ=0.18) among women with water arsenic < median. Future Plans The PAIR Study is well positioned to examine the effects of low-moderate arsenic exposure and micronutrient deficiencies on immune outcomes in women and infants. Registration NCT03930017

Abstract:A simple and useful characterization of many predictive models is in terms of model structure and model parameters. Accordingly, uncertainties in model predictions arise from uncertainties in the values assumed by the model parameters (parameter uncertainty) and the uncertainties and errors associated with the structure of the model (model uncertainty). When assessing uncertainty one is interested in identifying, at some level of confidence, the range of possible and then probable values of the unknown of interest. All sources of uncertainty and variability need to be considered. Although parameter uncertainty assessment has been extensively discussed in the literature, model uncertainty is a relatively new topic of discussion by the scientific community, despite being often the major contributor to the overall uncertainty. This article describes a Bayesian methodology for the assessment of model uncertainties, where models are treated as sources of information on the unknown of interest. The general framework is then specialized for the case where models provide point estimates about a single‐valued unknown, and where information about models are available in form of homogeneous and nonhomogeneous performance data (pairs of experimental observations and model predictions). Several example applications for physical models used in fire risk analysis are also provided.
Pregnancy pesticide exposure and child development in low- and middle-income countries: A prospective analysis of a birth cohort in rural Bangladesh and meta-analysis

Abstract:Background: Despite considerable evidence on a negative association between pregnancy pesticide exposure and child development in high-income countries, evidence from low- and middle-income countries (LMICs) is limited. Therefore, we assessed associations between pregnancy pesticide exposure and child development in rural Bangladesh and summarised existing literature in a systematic review and meta-analysis. Methods: We used data from 284 mother-child pairs participating in a birth cohort established in 2008. Eight urinary pesticide biomarkers were quantified in early pregnancy (mean gestational age 11.6±2.9 weeks) as an index of pesticide exposure. The Bayley Scales of Infant and Toddler Development, Third Edition were administered at 20-40 months of age. Associations between creatinine-adjusted urinary pesticide biomarker concentrations and child development scores were estimated using multivariable generalised linear models. We searched ten databases up to November 2021 to identify prospective studies on pregnancy pesticide exposure and child development conducted in LMICs. We used a random-effects model to pool similar studies, including our original analysis. The systematic review was pre-registered with PROSPERO: CRD42021292919. Results: In the Bangladesh cohort, pregnancy 2-isopropyl-4-methyl-6-hydroxypyrimidine (IMPY) concentrations were inversely associated with motor development (-0.66 points [95% CI -1.23, -0.09]). Pregnancy 3,5,6-trichloro-2-pyridinol (TCPY) concentrations were inversely associated with cognitive development, but the association was small: -0.02 points (-0.04, 0.01). We observed no associations between 4-nitrophenol and 3-phenoxybenzoic acid (3-PBA) concentrations and child development. The systematic review included 13 studies from four LMICs. After pooling our results with one other study, we found consistent evidence that pregnancy 3-PBA concentrations were not associated with cognitive, language, or motor development. Conclusion: Evidence suggests that pregnancy exposure to some organophosphate pesticides is negatively associated with child development. Interventions to reduce in-utero pesticide exposure in LMICs may help protect child development.

Abstract:Purpose Exposures early in life, beginning in utero, have long-term impacts on mental and physical health. The ECHO prenatal and early childhood pathways to health consortium (ECHO-PATHWAYS) was established to examine the independent and combined impact of pregnancy and childhood chemical exposures and psychosocial stressors on child neurodevelopment and airway health, as well as the placental mechanisms underlying these associations. Participants The ECHO-PATHWAYS consortium harmonises extant data from 2684 mother–child dyads in three pregnancy cohort studies (CANDLE [Conditions Affecting Neurocognitive Development and Learning in Early Childhood], TIDES [The Infant Development and Environment Study] and GAPPS [Global Alliance to Prevent Prematurity and Stillbirth]) and collects prospective data under a unified protocol. Study participants are socioeconomically diverse and include a large proportion of Black families (38% Black and 51% White), often under-represented in research. Children are currently 5–15 years old. New data collection includes multimodal assessments of primary outcomes (airway health and neurodevelopment) and exposures (air pollution, phthalates and psychosocial stress) as well as rich covariate characterisation. ECHO-PATHWAYS is compiling extant and new biospecimens in a central biorepository and generating the largest placental transcriptomics data set to date (N=1083). Findings to date Early analyses demonstrate adverse associations of prenatal exposure to air pollution, phthalates and maternal stress with early childhood airway outcomes and neurodevelopment. Placental transcriptomics work suggests that phthalate exposure alters placental gene expression, pointing to mechanistic pathways for the developmental toxicity of phthalates. We also observe associations between prenatal maternal stress and placental corticotropin releasing hormone, a marker of hormonal activation during pregnancy relevant for child health. Other publications describe novel methods for examining exposure mixtures and the development of a national spatiotemporal model of ambient outdoor air pollution. Future plans The first wave of data from the unified protocol (child age 8–9) is nearly complete. Future work will leverage these data to examine the combined impact of early life social and chemical exposures on middle childhood health outcomes and underlying placental mechanisms.

Abstract:Background: Prenatal exposure to organophosphate (OP) pesticides associate with impaired neurodevelopment in humans and animal models. However, much uncertainty exists about the brain structural alterations underlying these associations. The objective of this study was to determine whether maternal OP pesticide metabolite concentrations in urine repeatedly measured during gestation are associated with brain morphology and white matter microstructure in 518 preadolescents aged 9-12 years. Method: Data came from 518 mother-child pairs participating in the Generation R Study, a population-based birth cohort from Rotterdam, the Netherlands. Maternal urine concentrations were determined for 6 dialkylphosphates (DAPs) including 3 dimethyl (DM) and 3 diethyl (DE) alkyl phosphate metabolites, collected at early, mid, and late pregnancy. At child's age 9-12 years, magnetic resonance imaging was performed to obtain T1-weighted images for brain volumes and surface-based cortical thickness and cortical surface area, and diffusion tensor imaging was used to measure white matter microstructure through fractional anisotropy (FA) and mean diffusivity (MD). Linear regression models were fit for the averaged prenatal exposure across pregnancy. Results: DM and DE metabolite concentrations were not associated with brain volumes, cortical thickness, and cortical surface area. However, a 10-fold increase in averaged DM metabolite concentrations across pregnancy was associated with lower FA (B = -1.00, 95%CI = -1.80, -0.20) and higher MD (B = 0.13, 95%CI = 0.04, 0.21). Similar associations were observed for DE concentrations. Conclusions: This study provides the first evidence that OP pesticides may alter normal white matter microstructure in children, which could have consequences for normal neurodevelopment. No associations were observed with structural brain morphology, including brain volumes, cortical thickness, and cortical surface area.

Abstract:S-31C2-3 Background/Aims: Large birth cohort study with long-term follow-up is a powerful and feasible way to assess the impact of environment, genetics, and environment-genetics interaction on different health outcomes of both pregnant women and their offspring, by prospectively collecting extensive data and specimen during various periods from preconception to childhood or even later life. Methods: Peking University Institute of Reproductive and Child Health have conducted 3 large birth cohort studies. Results: First cohort study includes about 230,000 children, whose mother had participated in a previous folic acid intervention study to prevent the occurrence of neural tube defects in China. Women entered the program either at premarital health assessment or at any stage of pregnancy. Second cohort includes 17,000 children, whose mother had participated in a 3-armed randomized controlled trial to assess the impact of iron-folic acid and multi-micronutrient supplementations on mortality, morbidity, and complications during pregnancy, labor, and delivery in China. Women were enrolled before 20 weeks of gestational age, women's venous blood and baby's cord blood were collected, and children born to the enrolled women are being followed up. Third cohort study is now enrolling women, and is planned to enroll 10,000 women before or during early pregnancy, to collect women's venous blood, and follow these enrolled women up to 42 days postpartum. For all 3 cohort studies, maternal and children's data about demographic, relevant obstetric and pregnancy outcomes, folic acid supplements taking, and body growth were collected at the time of entry, subsequent prenatal visits, delivery, or at the follow-up. Conclusion: These cohort studies provide exciting opportunities to explore the cause and prevention measures for various interested health outcomes including the adverse health effects of environmental contaminants.
Maternal methylmercury exposure through rice ingestion and child neurodevelopment in the first three years: a prospective cohort study in rural China

Abstract:Background: Rice is an important dietary source for methylmercury; however, rice does not contain the same beneficial nutrients as fish. Our main objective was to assess associations of prenatal methylmercury exposure through rice ingestion with child neurodevelopment in rural China. Methods: Eligible peripartum women were enrolled (n = 391), provided peripartum hair samples, and children's neurodevelopment was assessed at 12 months (n = 264, 68%) and 36 months (n = 190, 48%) using the Bayley Scales of Infant Development, 2nd Edition, including the Mental Developmental Index (MDI) and the Psychomotor Developmental Index (PDI). Associations between prenatal methylmercury exposure during the third trimester [log2 maternal hair total mercury (THg)] and child's neurodevelopment were assessed using linear mixed models for repeated measures. Results: In adjusted models, a doubling in maternal hair THg corresponded to a 1.3-point decrement in the MDI score [95% confidence interval (CI): - 2.6, - 0.14], and a 1.2-point decrement in the PDI score (95% CI: - 2.6, 0.14). Overall, adverse associations between maternal hair THg and MDI scores attenuated over time. However, associations were robust and stable over time among children whose primary caregiver was their parent(s). During the study follow-up, an increasing proportion of children were raised by grandparents (12 months: 9% versus 36 months: 27%), a trend associated with rural-to-urban parental migration for work. Conclusions: For young children living in rural China, a biomarker of prenatal methylmercury exposure was associated with decrements in cognitive function assessed between 12 and 36 months of age. Changes in the family structure over the study follow-up time interval potentially impacted children's sensitivity to prenatal methylmercury exposure.
