Abstract:With the progress of medical technology, the development of new drugs and the improvement of the therapeutic effect of graft-versus host disease in the last two decades, the outcomes of allogeneic hematopoietic stem cell transplantation (allo-HSCT) have been greatly improved. However, graft failure is still a rare but serious complication of allo-HSCT. HLA incompatibility, virus infection, elderly donor, uncontrolled primary disease, damage of bone marrow hematopoietic microenvironment, ABO blood group incompatibility, T cell depletion, reduced intensity conditioning, and low nucleated cell number are all risk factors for graft failure. In recent years, with the implementation of HLA haplo-identical hematopoietic stem cell transplantation, the role of donor-specific antibodies in graft failure has attracted attention increasingly. This article reviews the recent studies involving the mechanism, risk factors and prevention measures of graft failure in allo-HSCT.

Abstract:Graft-versus-host disease (GVHD) is an immunological condition that affects recipients of hematopoietic stem cell transplantation (HSCT), leading to an exacerbated inflammatory response in the body, potentially impacting various organs and posing a significant health risk. A 44-year-old male patient was initially diagnosed with acute lymphoblastic leukemia and subsequently underwent two HSCT procedures. Around twenty days after the second HSCT, the patient presented with an intraoral clinical manifestation, featuring a painful atypical ulcerative lesion on the dorsum of the tongue.

Abstract:Abstract Abstract 4510 Recently the increasing number of cases of solid malignancies developed in recipients after allogeneic hematopoietic stem cell transplantation (allo-HSCT) has attracted more attention while less donor-derived solid malignancies after allo-HSCT have been found. We describe here a case of donor-derived tongue squamous cell carcinoma (SCC) in a young adult patient who had undergone allogeneic peripheral stem cell transplantation for acute leukemia 2 years ago. The female patient had a history of acute lymphoblastic leukemia (ALL-L2) at 20 years of age in July 2004 and received a successful busulfan and cyclophosphamide (BU/CY2) myeloablative allografting (allogeneic peripheral blood stem cells) from a HLA-matched unrelated male donor in August 2006. Acute graft versus host disease (GVHD) prophylaxis consisted of cyclosporine A 2.5 mg/kg/day, methotrexate 10 mg on day 1,3,6, and mycophenolate mofetil 0.5 g/day. However, on day 12 the patient developed skin grade II acute GVHD. Subsequently, she complicated with intestinal grade II acute GVHD characterized by recurrent diarrhea on day 49. 2 mg/kg/day of methylprednisolone and 4 mg/kg/day of cyclosporine A were given. 7 days later acute GVHD was controlled gradually. After that, grade II chronic GVHD with skin rash and oral mucositis accompanied by leukoplakia occurred after 5 months post engraftment. Prolonged immunosuppressive agents containing prednisolone and cyclosporine A were administered. In April 2008 (20 months post engraftment), the patient presented with symptoms of pain in the tongue which worsened in the following 2 months. A firm mass was seen in the tongue which was highly suspicious for malignancy. The histology of the puncture biopsy showed a well-differentiated SCC. The neoplastic cells were pleomorphic and prominent nucleoli. Cell nests and keratinous pearls could be seen clearly. Then she was treated with mass excision in addition to supraomohyoid neck dissection immediately. At the same time peripheral blood STR analysis showed full donor's chimerism, and bone marrow aspiration as well as minimal residual disease with FACS indicated complete remission of the primary disease. The patient is now under follow-up with no recurrence or metastasis. Further investigation of the tissue specimens by fluorescent in situ hybridization (FISH) (X,Y probes) combined with double-SP immunohistochemical staining (CD45, Pan-cytokerin) revealed the presence of one Y chromosome-specific signal and one × signal within most of the neoplastic cells which was identified as pan-cytokerin (+), CD45(-) cells and this suggested that donor-derived stem cells might transform into malignancies in the recipient by several potential mechanisms (Hu YX et al, Onkologie 2010). Till now about 13 cases developing donor-derived solid malignancies including oral SCC (Janin A et al, Blood 2009; Tomihara K et al, Head Neck 2009), colonic adenoma (Cogle CR et al, Stem cells 2007), breast adenoma (Golfinopoulos V et al, Breast Cancer Res Treat 2009), gastric adenoma (Arai Y et al, AM J Transplant 2006), larynx squamous cell carcinoma (Avital I et al, Stem cells 2007), Kaposi's sarcoma (Avital I et al, Stem cells 2007), lung adenoma (Avital I et al, Stem cells 2007) and glioblastoma multiforme (Avital I et al, Stem cells 2007) have been reported post-HSCT. The presence of donor-derived solid malignancies suggests that human bone marrow-derived stem cells have a role in the carcinogenesis of solid organ cancer especially in the organ with inflammation. But which kind of bone marrow derived stem cells transformed into solid malignancies and its mechanisms need further investigation. Disclosures: No relevant conflicts of interest to declare.

Abstract:Graft-versus-host disease (GVHD) is a significant complication that can arise in patients who have undergone allogeneic hematopoietic stem cell transplantation (AHCT). The oral cavity is affected in more than 80% of GVHD cases. Here, we present two cases of oral GVHD: Case 1: A 58-year-old male patient of African descent had undergone AHCT for acute myeloid leukemia treatment four years prior. During extraoral examination, depigmented areas were noted on the skin. Intraoral examination revealed striae-associated whitish plaques, which were symptomatic and affected the buccal mucosa and tongue for approximately 10 months.

Abstract:BACKGROUND: Graft-versus-host disease (GVHD) is an immune system reaction that occurs in patients with a history of hematopoietic stem cell transplantation (HSCT), in which the grafted donor's cells attack those of the host. The objective of this systematic review was to present a study on oral squamous cell carcinoma (OSSC) that developed from GVHD areas in patients undergoing HSCT.MATERIAL AND METHODS: An electronic search was conducted in the databases PUBMED, WEB OF SCIENCE, SCOPUS, MEDLINE and SCIENCE DIRECT, according to PRISMA guidelines.RESULTS: Of the 1582 results, 23 articles were included, resulting in 81 cases. The most common underlying disease for performing the transplant was Myeloid Leukemia (55.6%). The mean age was 39 years, with a predilection for males (64.2%). The tongue was the site of GVHD that most frequently underwent transformation to SCC (59.3%). The average time between transplantation and the development of GVHD was of approximately of 8 months, while the average period of development between transplantation and the development of OSCC was of approximately of 111 months. The most common treatment to GVHD was cyclosporine associated with corticosteroids.CONCLUSIONS: OSCCs arising from areas of GVHD present a different evolution from conventional oral carcinomas, since they affect younger patients, smoking and alcohol are not important etiological factors and finally because they present good prognosis, but further studies with larger number cases followed are needed to confirm our findings.

Abstract:Hematopoietic stem cell transplantation (HSCT) is the standard treatment for hematopoietic malignancies and certain solid tumors; however, graft-versus-host disease (GVHD) remains a significant complication. Chronic GVHD (cGVHD), which occurs more than 100 days post-transplant, can lead to various oral manifestations that necessitate multidisciplinary management to prevent disease progression and enhance the quality of life. We present the case of a 60-year-old woman who developed dry mouth, oral pain, a brown-yellow tongue, and dental caries seven years after receiving HLA-matched unrelated donor HSCT. Examination revealed a brown-yellow hairy tongue, extensive tooth decay, reduced saliva production, and oral leukoplakia with mild epithelial hyperplasia, along with dysfunction of the salivary and lacrimal glands. Following appropriate treatment, the patient's symptoms resolved without recurrence at the six-month follow-up. This case highlights the importance of professional diagnosis, timely interventions, and regular monitoring in managing oral cGVHD to achieve a favorable prognosis.

This multi-organ disorder frequently affects the oral cavity (45-85% of cGVHD patients). Despite prevalent oral complications, the mechanism remains undefined, contributing to a lack of effective therapy, significant morbidity, compromised nutrition, and reduced quality of life. This study analyzed metabolite and immune mediator shifts in HSCT patients to unravel cGVHD pathogenesis mechanistically.

Abstract:Programmed cell death ligand-1 (PD-L1/CD274) is an immunomodulatory molecule involved in cancer and complications of bone marrow transplantation, such as graft rejection and graft-versus-host disease. The present study was designed to assess the dynamic expression of this molecule after hematopoietic stem cell transplantation in relation to acute graft-versus-host disease. Female BALB/c mice were conditioned with busulfan and cyclophosphamide and transplanted with either syngeneic or allogeneic (male C57BL/6 mice) bone marrow and splenic cells. The expression of PD-L1 was evaluated at different time points employing qPCR, western blot and immunohistochemistry. Allogeneic- but not syngeneic-transplanted animals exhibited a marked up-regulation of PD-L1 expression in the muscle and kidney, but not the liver, at days 5 and 7 post transplantation. In mice transplanted with allogeneic bone marrow cells, the enhanced expression of PD-L1 was associated with high serum levels of IFNγ and TNFα at corresponding intervals. Our findings demonstrate that PD-L1 is differently induced and expressed after allogeneic transplantation than it is after syngeneic transplantation, and that it is in favor of target rather than non-target organs at the early stages of acute graft-versus-host disease. This is the first study to correlate the dynamics of PD-L1 at the gene-, protein- and activity levels with the early development of acute graft-versus-host disease. Our results suggest that the higher expression of PD-L1 in the muscle and kidney (non-target tissues) plays a protective role in skeletal muscle during acute graft-versus-host disease.

Abstract:Allogenic hematopoietic stem cell transplantation(HSCT) is the most effective treatment for malignant hematologic disease,while graft versus host disease(GVHD) is the main complication and leading cause of mortality in patients receiving HSCT.Oral cavity involvement occurs frequently in patients suffering from GVHD,and it may be the primary or even the only site of GVHD.It is essential for oral professionals to be familiar with various oral GVHD.This review discusses the pathogenesis,multifarious clinical manifestation,available evidence for diagnostic,and the progress of treatment in oral GVHD.The management of the oral GVHD seems to be important in the early diagnosis of GVHD.

Abstract:In 2014, a 44-year-old male patient sought specialized care, reporting an asymptomatic "rough spot in the mouth" that had been present for one year. He mentioned experiencing frequent occlusal trauma in the affected region. The patient's medical history included a bone marrow transplant conducted a year prior, and he had been diagnosed with graft-versus-host disease (GVHD) six months after the transplant. Upon intraoral examination, a whitish plaque was observed next to tooth 37, raising the suspicion of leukoplakia.

Abstract:Abnormal change on the tongue is a potential complication after allogeneic hematopoietic stem cell transplantation (allo-HSCT). The exact pathogenesis remains unclear and several risk factors include chemoradiotherapy, infection, graft-versus-host disease, disease relapse, and secondary malignancy. Our case described a 42-year-old woman with B-cell acute lymphoblastic leukemia treated by allo-HSCT 2 months later followed by a rare and atypical tongue neoplasia without oral pain, dysphagia, and dysgeusia. The biopsy was operated which showed granulation tissue with no evidence on typical graft-versus-host disease or malignancy, and no specific infection had been identified. Cyclosporine and mycophenolate mofetil, which were used for immunosuppression after allo-HSCT accompanying with the rapid growth of the tongue neoplasia, was then replaced by sirolimus. One month later, the patient underwent a complete remission unexpectedly.

Abstract:An 18-year-old male was referred to a hospital dental office 26 days after undergoing Allogeneic Hematopoietic Stem Cell Transplantation, reporting lip pain, difficulty ingesting solid food, and restricted mouth opening. Clinical examination revealed dryness, edema, hemorrhagic crusts on the lip vermillion, as well as ulcers on the labial commissures. The initial diagnostic considerations included Acute Graft-Host Disease (aGVHD) and Stevens-Johnson Syndrome (SJS). As the lesions progressed to necrosis on the lips and ulcers on the anterior region of the buccal mucosa, resembling SJS oral manifestations, but less frequently observed in aGVHD, the final diagnosis leaned toward aGVHD.

RTMIPAS : A fast radiative transfer model for the assimilation of infrared limb radiances from MIPAS

Abstract:An RTTOV-type fast radiative transfer model has been developed for emitted clear-sky limb radiances from the Michelson Interferometer for Passive Atmospheric Sounding (MIPAS) onboard ESA's Envisat. MIPAS is an infrared sounder with very high spectral resolution (0.025 cm, Table 1). It is designed to provide information on the thermal structure and chemical composition of the upper troposphere to lower mesosphere. The fast model is referred to as RTMIPAS, and it has been developed to directly assimilate limb radiances in a global variational data assimilation system (see Bormann and Healy in these proceedings).

Abstract:Abstract Allogeneic hematopoietic stem cell transplantation is a curative cancer immunotherapy whose use is limited by the high incidence of post-transplant chronic graft-versus-host disease (cGVHD), an alloimmune disease that manifests in multiple organ systems including the oral cavity where buccal mucosa (BM) and minor salivary glands (MSG) are affected. As little is known about the immune network that drives pathogenesis of cGVHD, therapeutic agents are limited, and this study used a multiparameter approach to query the relationship between salivary gland gene expression, salivary proteins and the immune cell network within oral tissues with regard to oral cGVHD severity. Microarray analysis of labial MSG from oral cGVHD patients revealed significant up-regulation in gene expression pathways involving immune/inflammatory responses (IL-1 and IL-6 signaling, IL-32), response to oxidative stress (superoxide dismutase 2) and wounding/fibrosis (TIMP-1, TGF-β) in patients versus non-sicca controls. Immunohistochemistry revealed that MSG of these patients had increased numbers of activated T-bet+ CD8+ T cells which were directly associated with acinar structure alteration and glandular infiltrates. Flow cytometric analysis also demonstrated high frequency of CXCR3+ cells in cGVHD-affected MSG, consistent with increased levels of CXCL9 and CXCL10 detectable in the whole saliva of oral cGVHD patients. Parallel flow cytometric analyses in a mouse model of cGVHD in the submandibular salivary gland demonstrated an increased percentage of CD103+ DCs, which can cross-present antigens to activate CD8+ T cells. These studies are ongoing, but implicate activation of T cytotoxic (Tc1) cells in the pathogenesis of oral cGVHD.

Abstract:Acute promyelocytic leukaemia (APL) is a form of acute myelogenous leukaemia. APL is characterised by anaemia due to suppression of normal haematopoiesis and infection. Haematopoietic stem cell transplantation (HSCT) is current option for the treatment of haematopoietic malignancies and is proving to be successful. Although HSCT has been effective for the treatment of haematopoietic malignant tumours, chronic graft-versus-host disease (GVHD) but secondary cancers can occur, which is a serious complication and frequently involves the oral cavity and skin. Here, we report the case of tongue cancer occurring 17 years after transplantation in a patient who developed GVHD after haematopoietic stem cell transplantation and APL remission. To the best of our knowledge, this is the first report of secondary oral cancer after HSCT with APL as the primary disease. Abstract:Chronic graft-versus-host disease (cGvHD) is a multisystem disorder that occurs in recipients of allogeneic hematopoietic (alloHCT) stem cell transplants and is characterized by both inflammatory and fibrotic manifestations. It begins with the recognition of host tissues by the non-self (allogeneic) graft and progresses to tissue inflammation, organ dysfunction and fibrosis throughout the body. Oral cavity manifestations of cGVHD include mucosal features, salivary gland dysfunction and fibrosis. This review synthesizes current knowledge on the pathogenesis, diagnosis and management of oral cGVHD, with a focus on emerging trends and novel therapeutics. Data from various clinical studies and expert consensus are integrated to provide a comprehensive overview.

Abstract:As a method to improve the survival rate of patients with hematological malignancies, allogeneic hematopoietic stem cell transplantation (allo-HSCT) has increasingly been used for treatment. However, some potentially serious complications after allo-HSCT, including graft-versus-host disease, graft failure, infection, end-organ toxicity, and secondary malignancies, will determine the success of hematopoietic reconstitution. Here, we describe a case of a patient with p16-positive tongue squamous cell carcinoma (TSCC) following allo-HSCT. A 62-year-old man who had previously received allo-HSCT due to acute lymphocytic leukemia (AML) presented with erosions on the back of the tongue surrounded by multiple white patches, which were compatible with oral chronic graft-versus-host disease (cGVHD). During follow-up, a circular-like erosive lesion appeared on the right dorsal surface of the tongue. Biopsy of this lesion confirmed early invasive TSCC (T2N0M0). Partial glossectomy and tongue reconstruction were performed after cessation of immunosuppressants. Immunohistochemical (IHC) staining was positive for p16 and ki-67, suggesting a probable active human papillomavirus (HPV) infection. Six months after surgery, the patient showed no signs of metastasis or recurrence nor progression of oral GVHD.

Abstract:We studied the ability of a beta-adrenergic agonist (albuterol) to attenuate calcium chelator- and acetylcholine-induced airway constriction in the lung periphery of anesthetized mongrel and Basenji-Greyhound (BG) dogs. A wedged bronchoscope technique was used to measure collateral system resistance before and after challenges with aerosolized Na2EDTA and acetylcholine. Time course of the response to Na2EDTA differed significantly between mongrel and BG dogs. Peak response to challenge with 4% Na2EDTA occurred within 2 min for mongrel dogs and at 5 min for BG dogs. Albuterol (1 microgram/kg iv) significantly attenuated Na2EDTA-induced bronchoconstriction in both groups of animals (P less than 0.01, each group). Albuterol (1 microgram/kg iv) significantly attenuated acetylcholine-induced bronchoconstriction in mongrel (P less than 0.01) but not in BG dogs. We conclude that a qualitative difference exists in the mechanism of Na2EDTA-induced constriction in the lung periphery of BG compared with mongrel dogs. In addition, the lung periphery of BG dogs demonstrates reduced beta-adrenergic sensitivity with respect to a cholinergic challenge compared with mongrels, suggesting enhanced cholinergic inhibition of the beta-adrenergic system.
