Abstract:Recently, we developed liposome-encapsulated hemoglobin (LEH), a novel cellular hemoglobin-based oxygen carrier. We hypothesized that the LEH effectively suppresses scavenging of nitrogen oxides by sequestering hemoglobin, thereby being useful for resuscitation from hemorrhagic shock, especially in prehospital settings where blood transfusion is not available. However, putting a catheter into the peripheral vessels is sometimes difficult in prehospital resuscitation, because these vessels collapse in patients with hemorrhagic shock. The intraosseous route does not collapse under such conditions. We here studied the resuscitation of severe hypohemoglobinemia following massive hemorrhage using intraosseous (intrafemur) transfusion with LEH in mice. First, we examined the effect of intravenous transfusion with LEH on the resuscitation of mice with fatal hypohemoglobinemia that was made with progressive hemodilution by blood exchanges. Despite a success in initial resuscitation without scavenging of NO2− or NO3−, LEH transfusion did not significantly improve mouse survival 72 h later as compared with red blood cell (RBC) transfusion. In other experiments, hypohemoglobinemic mice were also made with blood withdrawal and intraosseous infusion with 5% albumin. Thereafter, the mice were rescued with intraosseous transfusion of LEH or RBCs. Unlike intravenous transfusion, intraosseous transfusion with LEH (but not such transfusion with RBCs) significantly increased mouse survival without scavenging of NO2− or NO3−, presumably because LEH vesicles were much smaller than RBCs, thereby effectively flowing into the circulation from the femur. Thus, intraosseous transfusion with LEH may be a candidate strategy for efficient prehospital resuscitation from hemorrhagic shock.
The Surgical Timing of Brachial Plexus Compression Secondary to Traumatized Subclavicular Vessels Rupture

Abstract:Objective To investigate the mechanism ,clinical classification ,diagnosis and treatment in patients with brachial plexus compression caused by traumatized subclavicular vessels rupture. Methods Brachial plexus compression caused by traumatized subclavicular vessels rupture in 5 cases were involved. For 4 cases, compression of brachial plexus was due to local hematoma following vessel rupture, in which cases epineurolysis was done after hematoma clearance and repair and litigation of rupture of the vessels. For 1 case, brachial plexus root injury was complicated by thrombus of subclavian artery. Both thrombectomy and nerve transfer were performed. Phrenic nerve and accessory nerve was transferred to muslcutaneous and suprascapular nerve respectively. Results Over an average period of 13 months follow - up, assessment criteria described by GU YuDong was done with a result of excellent 2 cases ,good 1 case, fair 2 case. Conclusions Once the diagnosis was made, operation be performed immediately thus satisfactory outcome can be obtained. Prognosis was related with surgical time, the region and size of vessel rupture.
Axillary artery lesions from humeral neck fracture: A study in relation to repair.

Abstract:Whether axillary artery injuries associated with proximal humeral fractures must be repaired is uncertain. The present study reports three cases treated with various approaches. In case 1, the left humeral surgical neck was broken, the radial pulse disappeared and the arm temperature was significantly reduced. Computerized tomography angiography (CTA) revealed a 3-cm filling deficiency in the axillary artery. The injured artery was reconstructed with a segment of the greater saphenous vein after the fracture was reduced and fixed with a plate. In case 2; the axillary artery was compressed by the broken humeral segment, which caused the right hand to become cool. The fracture was fixed but the axillary artery embolism was not treated. In case 3, the humeral neck was broken with injury to the brachial plexus, although the patient's hand remained warm. Digital subtraction angiography (DSA). revealed that the axillary artery was injured badly but there was extensive collateral circulation. The proximal humeral fracture was reduced and fixed without artery reconstruction. The three cases all resulted in a good outcome with the bone healed and limb alive. No ischemic necrosis occurred. However, the neurapraxia did not dissappear completely. Axillary artery injury resulting from humeral neck fracture is a rare but disabling traumatic event. Early diagnosis based on signs of acute ischemia of the arm enables early treatment and a favourable outcome. An angiogram is the best way to diagnose the artery injury and evaluate the condition of the collateral circulation. The injured artery in a cold arm should be repaired, while in a warm arm reconstruction is unnecessary due to rich collateral circulation.
Axillary artery injury secondary to displaced proximal humeral fractures: a report of two cases

Abstract:We report 2 cases of axillary artery injury secondary to low-energy proximal humeral fractures. In case 1, early diagnosis based on signs of acute ischaemia of the arm enabled early treatment and a favourable outcome. In case 2, there were no signs of ischaemia or neurological deficit, resulting in delayed diagnosis and increased severity of the injury. The patient developed a false aneurysm and sepsis and eventually died. A high index of suspicion is necessary for diagnosing an axillary artery injury. We recommend that all patients with proximal humeral fractures with severe medial displacement of the shaft and a bone spike should routinely undergo Doppler ultrasound scanning to rule out vascular injuries and the presence of a false aneurysm.

Abstract:Aims Vascular compromise due to arterial injury is a rare but serious complication of a proximal humeral fracture. The aims of this study were to report its incidence in a large urban population, and to identify clinical and radiological factors which are associated with this complication. We also evaluated the results of the use of our protocol for the management of these injuries. Methods A total of 3,497 adult patients with a proximal humeral fracture were managed between January 2015 and December 2022 in a single tertiary trauma centre. Their mean age was 66.7 years (18 to 103) and 2,510 (72%) were female. We compared the demographic data, clinical features, and configuration of those whose fracture was complicated by vascular compromise with those of the remaining patients. The incidence of vascular compromise was calculated from national population data, and predictive factors for its occurrence were investigated using univariate analysis. Results A total of 18 patients (0.5%) had a proximal humeral fracture and clinical evidence of vascular compromise, giving an annual incidence of 0.29 per 100,000 of the population. Their mean age was 68.7 years (45 to 92) and ten (56%) were female. Evidence of a mixed pattern neurological deficit (brachial plexus palsy) (odds ratio (OR) 380.6 (95% CI 85.9 to 1,685.8); p < 0.001), complete separation of the proximal shaft from the humeral head with medial displacement (OR 39.5 (95% CI 14.0 to 111.8); p < 0.001), and a fracture-dislocation (OR 5.0 (95% CI 1.6 to 15.3); p = 0.015) were all associated with an increased risk of associated vascular compromise. A policy of reduction and fixation of the fracture prior to vascular surgical intervention had favourable outcomes without vascular sequelae. Conclusion The classic signs of distal ischaemia are often absent in patients with proximal injuries to major vessels. We were able to identify specific clinical and radiological ‘red flags’ which, particularly when present in combination, should increase the suspicion of a fracture with an associated vascular injury, and facilitate early diagnosis and appropriate combined orthopaedic and vascular intervention. Acute Post-Traumatic Brachial Plexus Compression

Abstract:ObjectiveThe injury mechanisms, clinical manifestation, diagnosis, treatment and prognosis of acute brachial plexus compression after trauma were presented.Methods12 acute trauma cases were involved in this series. Symptoms and signs included swelling, hematoma, tenderness and deformity of the clavicle shoulder region, presence of pulsation and vascular murmur, and impaired sensory and motor function of the territory innervated by lower trunk of the brachial plexus. 7 cases had rib fracture, clavicle fracture or multiple fractures. Injuries of the subclavicle vessels (vessel rupture, pseudoaneurysm, artero venous fistula) were found in 9 cases. Hematoma was seen in 3 cases. Diagnosis of acute brachial plexus compression secondary to trauma was diagnosed and surgical exploration was carried out in all cases.ResultsIn 3 cases, pseudoaneurysm was excised and the subclavicular artery was repaired by direct anastomosis or vein graft. Repair of the ruptured vessels was done in 4 cases, while another 2 were managed with vessel ligation. Removal of hematoma was done in 3 cases. 8 cases underwent open reduction and internal fixation of the fractured bones. All of 12 cases had neurolysis of the brachial plexus. Postoperatively the patients were followed for an average of 26 months. The patients had strong pulsation at the radial artery of the affected side. All fractures were healed. Assessment of the upper limb function using the standard issued by the Hand Surgery Society of Chinese Medical Association graded 11 cases as excellent and 1 case as fair.ConclusionBrachial plexus entrapment secondary to trauma has an acute onset, usually occurring within 2 to 3 hours or 1 to 2 days after the injury. In addition to brachial plexus neuropathy, fractures of the shoulder girdle and subclavicle vessel injuries are often present. Early surgical intervention can lead to satisfactory results.

Abstract:Iatrogenic vascular injuries are rare but potentially serious complications that can occur during orthopedic procedures involving the proximal humerus. We present a case report of a patient who sustained a brachial artery injury during a long Proximal Humeral Interlocking System (PHILOS) plating procedure for a proximal humeral fracture. A 62-year-old female patient with a left humerus proximal 1/3 shaft fracture underwent open reduction and internal fixation with a long PHILOS plate. During the procedure, difficulty was encountered in achieving adequate plate positioning due to osteoporotic bone and fracture comminution. Upon insertion of a distal second last screw, brisk brachial artery bleeding was encountered. Immediate hemostasis measures were taken, and a plastic surgeon was consulted. The brachial artery injury was identified and repaired with a cephalic vein graft harvested and flushed. Postoperatively, the patient developed median nerve neuropraxia. This case highlights the risk of iatrogenic brachial artery injury during left humerus proximal 1/3 shaft fracture fixation, especially in cases with technical challenges due to osteoporotic bone or comminution. Prompt recognition, involvement of vascular surgery, and appropriate management are crucial in mitigating potential devastating consequences. Associated neurological complications, such as nerve injuries, can also occur and should be monitored. Meticulous surgical technique, anatomical awareness, and vigilant monitoring are essential to minimize the risk of vascular and neurological complications during these procedures. Iatrogenic brachial artery injury is a rare but potentially serious complication of humerus proximal 1/3 shaft fracture. Early recognition, multidisciplinary involvement, and appropriate management strategies are crucial in optimizing patient outcomes and preventing long-term morbidity.
Management of Proximal Humerus Fracture With Concurrent Axillary Artery Injury Using a Saphenous Vein Graft and Reverse Shoulder Prosthesis: A Case Report

Abstract:This case report details the management of a 60-year-old male who sustained a complex proximal humerus fracture with an axillary artery injury following a fall. Initial assessment revealed a fractured humeral head and complete occlusion of the axillary artery, which was repaired using a saphenous vein graft. Subsequently, the patient underwent reverse shoulder arthroplasty (RTSA) after the limb stabilized. The staged approach, prioritizing urgent vascular repair followed by delayed orthopedic intervention, proved effective. The patient achieved satisfactory functional recovery with an improved range of motion and no pain. This case highlights the importance of prompt diagnosis and treatment of vascular injuries in complex proximal humeral fractures and supports the use of saphenous vein grafting and RTSA as effective management strategies.

Abstract:In the course of examining afferents to ventromedial hypothalamic (VMH) neurons using horseradish peroxidase (HRP), we needed to know how close to an iontophoretic deposit site neurons could be proved to be retrogradely labeled. In evaluating cells near but clearly outside HRP deposit sites visualized after a 24‐hour survival period, for example, neurons which had been filled with HRP by somal or dendritic uptake could not be treated as retrogradely labeled and thus would not add to studies of intrahypothalamic connections.
Humeral shaft fracture with brachial artery injury

Abstract:The management of ten patients with fracture of the humeral diaphysis and concomitant injury to the brachial artery was analyzed retrospectively for examination of the factors contributing to preservation of limb viability and function. Of ten treated fractures, eight were rigidly stabilized with either a plate or an external fixation device. Seven of the vascular injuries required an interpositional graft. Three arteries were repaired with an end-to-end anastomosis. Two of these repairs failed in patients whose fractures were not rigidly stabilized, and one of these patients required amputation. Fasciotomy was performed in five cases. Seven patients retained normal or functional extremities despite ligation of the brachial artery in two individuals. Preoperative arteriography was of equivocal value, and it doubled the patient's transit time from the emergency room to surgery. The use of a temporary intraluminal vascular shunt to perfuse the limb at surgery and the semi-invasive rigid stability afforded by contemporary external fixators are important advances in the management of these complex injuries. In civilian practice, the prioritized guidelines the authors suggest for care of combined major skeletal and vascular trauma to the arm are: resuscitation of the patient; arteriogram, which is nonessential in open injuries but helpful in closed trauma; intraoperative perfusion of the limb with a temporary shunt; rigid bony stabilization; wound debridement; vascular repair with an autogenous vein graft; neurorrhaphy; and assessment of the need for fasciotomy.

Abstract:The upper limb has a complex anatomy comprised of many nerve and vascular structures, making humeral shaft fractures extremely important. Injury to the humeral shaft commonly occurs due to trauma and affects younger male or older female patients. The radial nerve travels along the spiral groove of the humerus, placing it at an increased risk of damage in humeral shaft fractures. If injured, there are a variety of classifications of radial nerve injury, different indications for exploration, and treatment methods that orthopedic surgeons have available in treating these injuries. This review aims to discuss the etiology of humeral shaft fracture-associated radial nerve palsy, tools for diagnosis, and treatment.

Abstract:Traumatic pseudoaneurysm of the axillary artery combined with brachial plexus injury is extremely rare. The factors that influence the symptoms and functional recovery related to this condition are unclear. Nine patients who had sustained this trauma were surgically treated at our unit between June 1999 and November 2010. The cause of trauma, symptoms, signs and examinations of neurological and vascular deficits, and the surgical findings of the involved nerves and vessels were recorded in detail. The functional recovery of vessels and nerves, as well as the extent of pain, were evaluated, respectively. The average length of patient follow-up was 4.5 years (range, 24 months to 11.3 years). After vessel repair, whether by endovascular or operative treatment, the distending, constant, and pulsating pain was relieved in all patients. Furthermore, examination of the radial artery pulse on the repaired side appeared normal at last follow-up. All patients showed satisfactory sensory recovery, with motor recovery rated as good in five patients and fair in four patients. The symptom characteristics varied with the location of the damage to the axillary artery. Ultrasound examination and computed tomography angiography are useful to evaluate vascular injury and provide valuable information for operative planning. Surgical exploration is an effective therapy with results related to the nerve injury condition of the brachial plexus.

Radial Nerve Palsy after Humeral Fracture: To Explore or Not to Explore? - A Case Report

Abstract:The incidence of radial nerve injuries varies from 2-17% and humeral fracture is the most common cause in young adults. The treatment of radial nerve damage, especially in cases of fracture, remains a challenge. The authors report a case of radial nerve palsy related to humeral shaft fracture and shall revise the literature about their treatment. Whether fracture or associated radial nerve injury, were treated conservatively, and the patient referred, of course, to Physiatry, to start intensive functional recovery of the affected upper limb. At 5-6 months after fracture, patient maintains radial sensory and motor deficits, and it was proposed surgical exploration of the radial nerve, which was scheduled, because a week before the alleged scheduled surgery, he demonstrated a recovery, and has fully function of the radial nerve after 2 weeks. Humeral shaft fractures are treated mostly conservatively, the radial nerve usually suffers neuropraxia and their function returns spontaneously after fracture healing, as happened in this case. The nerve exploration is reserved for cases in which there is no recovery of nerve function 3-4 months or a little more, after the fracture consolidation. Thus, nerve routine operation would subject many patients to unnecessary complications of surgery.

Abstract:BACKGROUND: Ulnar nerve injury subsequent to a fracture of the distal radius is extremely rare compared to median nerve injury. Treatment of ulnar nerve injury after closed distal radial fracture is controversial. Reasonable surgical planning and careful postoperative management can improve the prognosis of patients.CASE SUMMARY: We report two cases of ulnar nerve injury subsequent to fracture of the distal radius. Both patients were admitted to hospital. Both patients had persistent ulnar nerve compression syndromes. The first patient achieved rapid recovery by early nerve decompression surgery, while the second patient had no recovery at 2-3 mo after injury and had more severe symptoms. At 10 wk after injury, the second patient agreed to nerve decompression surgery. The second patient finally achieved a successful outcome after nerve decompression and neurolysis, although she still has residual symptoms.CONCLUSION: For patients with ulnar nerve compression syndrome related to acute wrist fracture, if symptoms persist and signs of recovery are not observed, early release is necessary to prevent permanent neurological damage.

Primary Radial Nerve Lesions in Humerus Shaft Fractures—Revision or Wait and See

Abstract:Background: This study investigates the surgical state-of-the-art procedure for humeral shaft fractures with primary radial nerve palsy based on its own case series in relation to the current and established literature. Methods: Retrospective review of treated cases between January 2018 and December 2022 describing radial nerve palsy after humerus shaft fractures, radiological fracture classification, intraoperative findings, surgical procedure, patient follow-up and functional outcome. Results: A total of 804 patients (463 women and 341 men) with humerus shaft fractures were identified. A total of 33 patients showed symptomatic lesions of the radial nerve (4.1%). The primary lesion was identified in 17 patients (2.1%). A broad and inhomogeneous distribution of fractures according to the AO classification was found. According to the operative reports, the distraction of the radial nerve was found eleven times, bony interposed three times and soft tissue constricted/compressed three times. In every case the radial nerve was surgically explored, there was no case of complete traumatic nerve transection. Four intramedullary nails and thirteen locking plates were used for osteosynthesis. Complete recovery of nerve function was seen in 12 cases within 1 to 36 months. Three patients still showed mild hypesthesia in the thumb area after 18 months. Two patients were lost during follow-up. Conclusions: With this study, we support the strategy of early nerve exploration and plate osteosynthesis in humeral fractures with primary radial nerve palsy when there is a clear indication for surgical fracture stabilisation. In addition, early exploration appears sensible in the case of palsies in open fractures and secondary palsy following surgery without nerve exposure as well as in the case of diagnostically recognisable nerve damage. Late nerve exploration is recommended if there are no definite signs of recovery after 6 months. An initial wait-and-see strategy with clinical observation seems reasonable for primary radial nerve palsies without indication for surgical fracture stabilisation.

Abstract:Background: Radial nerve palsy (RNP) associated with humeral shaft fracture is a common injury pattern in trauma patients. The management of RNP associated with humeral fractures in high-energy trauma is controversial and poses a challenge to surgeons treating it.Purpose: Based on a review of our experience over the past 15 years, we determined the prognostic factors of radial nerve recovery after humeral fractures, evaluated the diagnostic role of nerve conduction studies and electromyography (EMG), and compared the outcomes of different treatment strategies.Materials and Methods: The data of 26 patients having RNP associated with humeral shaft fractures over a 15-year period were collected for a retrospective review. For statistical analysis, the patients were divided into groups on the basis of their recovery from RNP and the treatment strategies used.Results: The incidence of RNP associated with humeral fractures was 2.05%. In total, 91.3% of patients with primary RNP in this series experienced high-energy trauma. Spontaneous recovery was observed in 9 of 26 patients (34.6%). Radial nerve lesions were found in 7 of 8 patients with high-energy trauma. The severity of humeral shaft fractures was found to be a significant prognostic factor for spontaneous recovery from RNP. The rate of spontaneous recovery was significantly higher in the AO Foundation and Orthopaedic Trauma Association Type A humeral shaft fractures (P = 0.028) and lower in Type C fractures (P = 0.055). The median time to detect initial radial nerve recovery using EMG was 34 and 75 days after injury (P = 0.033). In high-energy trauma, tendon transfers were associated with more predictable outcomes than nerve reconstruction (favorable functional recovery: 100% for tendon transfers vs. 25% for nerve reconstruction, P = 0.007). Moreover, tendon transfers were associated with a shorter overall treatment duration (median treatment duration: 190 days for tendon transfers vs. 422 days for nerve reconstruction, P = 0.007).Conclusion: The prognosis of RNP associated with humeral shaft fractures in high-energy trauma is less favorable, with a low rate of spontaneous recovery. EMG is helpful for the early detection of initial nerve recovery. The outcomes of tendon transfers in high-energy trauma are predictable and the treatment duration is shorter. First-intention tendon transfer is a reasonable treatment strategy in patients with difficult nerve exploration, lower requirement for functional recovery, and lower compliance with treatment.
Is there an association between arteriovenous fistulas and axillary artery aneurysms? A report of two cases

Abstract:Introduction: Axillary artery aneurysms are rare vascular conditions that can present with various clinical manifestations, including neurological deficits and vascular compromise. While the underlying pathophysiology remains complex and multifactorial, potential associations with trauma, arteriovenous fistula formation, and atherosclerosis have been reported. Presentation of case: Two male patients, aged 33 and 38, with a history of kidney transplantation and previous arteriovenous fistula (AVF) presented with symptoms of upper limb ischemia and neurological compromise. Imaging revealed large axillary artery aneurysms. Open surgical repair was performed for both cases. Two weeks after discharge, one patient showed good pronation and supination with mildly limited extension. The other patient's wrist drop gradually improved with physiotherapy. Discussion: Multifactorial pathophysiology encompassed altered blood flow dynamics, inflammation, and the underlying vascular pathology. Chief complaints and prior vascular interventions contributed. Open surgical repair was preferred to endovascular approaches, achieving favorable outcomes. Conclusion: Axillary artery aneurysms in patients with a history of AVF are rare but potentially serious complications. The cases highlight the complexity of axillary artery aneurysms and the need for careful evaluation and surgical intervention This strategy is crucial to prevent potential complications and optimize patient outcomes. Further research is needed to elucidate the precise pathophysiology and the potential association between AVF and the subsequent development of axillary artery aneurysms. Increasing awareness among surgeons could enable earlier detection of aneurysms, thereby reducing the risk of complications.
[Treating Neer Two- and Three-Part of Proximal Humeral Fractures Through Anterolateral Acromial Approach and Deltopectoral Approach].

Abstract:OBJECTIVE:To compare clinical results of treating Neer two- and three-part of proximal humeral fractures between anterolateral acromial approach and deltopectoral approach.METHODS:From January 2009 to December 2012, 49 patients with Neer two- and three-part of proximal humeral fractures were treated with locked plate fixation. In anterolateral acromial approach group, there were 22 patients including 9 males and 13 females with an average of (63.2±7.6) years old, while 27 patients in deltopectoral approach including 12 males and 15 females with an average of (62.9±7.0) years old. Operative time, blood loss during operation, fracture healing time and complications were observed and compared, postoperative Constant-Murley scoring and VAS scoring were applied for evaluate function of shoulder joint and pain at 3 months, 1 and 2 years respectively.RESULTS:All patients were followed up from 24 to 41 months with an average of 34.5 months. Operative time, blood loss, fracture healing time in anterolateral acromial approach group was (68.20±7.04) min, (151.30±20.57) ml, (10.88±4.90) weeks respectively, and better than that of in deltopectoral approach group which was (75.81±13.70) min, (242.10±37.25) ml and (13.60±2.45) weeks. Three months after operation, Constant-Murley scoring and VAS score in anterolateral acromial approach group was 88.32±5.45, 0.41±0.63 and better that of in deltopectoral approach group which was 63.53±8.31, 1.65±1.02. There was no significant differences between two groups in Constant-Murley scoring and VAS score at 1 and 2 years after operation. Each group has one case occurred loss of length humerus head height, and there was 1 case with subacromial impingement, 1 case with bolt loose and 2 cases with delayed union in deltopectoral approach. No axillary nerve injury, humeral head necrosis and breakage of internal fixation occurred both of two groups.CONCLUSION:Both of anterolateral acromial approach and deltopectoral approach are effective in treating Neer two- and three-part of proximal humeral fractures, and can obtain excellent outcomes. Moreover, anterolateral acromial approach has advantage of less trauma, less blood loss, shorter operative time, rapid recovery of shoulder joint function and fracture.
