Abstract:Objectives Researchers continue to seek easier ways to evaluate the quality of bone and screen for osteoporosis and osteopenia. Until recently, radiographic images of various parts of the body, except the distal femur, have been reappraised in the light of dual-energy X-ray absorptiometry (DXA) findings. The incidence of osteoporotic fractures around the knee joint in the elderly continues to increase. The aim of this study was to propose two new radiographic parameters of the distal femur for the assessment of bone quality. Methods Anteroposterior radiographs of the knee and bone mineral density (BMD) and T-scores from DXA scans of 361 healthy patients were prospectively analyzed. The mean cortical bone thickness (CBTavg) and the distal femoral cortex index (DFCI) were the two parameters that were proposed and measured. Intra-and interobserver reliabilities were assessed. Correlations between the BMD and T-score and these parameters were investigated and their value in the diagnosis of osteoporosis and osteopenia was evaluated. Results The DFCI, as a ratio, had higher reliability than the CBTavg. Both showed significant correlation with BMD and T-score. When compared with DFCI, CBTavg showed better correlation and was better for predicting osteoporosis and osteopenia. Conclusion The CBTavg and DFCI are simple and reliable screening tools for the prediction of osteoporosis and osteopenia. The CBTavg is more accurate but the DFCI is easier to use in clinical practice.
Relationship between bone mineral density and body compositions, strength, type of sport competition, vitamin D and birth-related factors in elite Polish track and field athletes: a cross-sectional study

Abstract:Purpose: The purpose of this investigation was to characterize the forearm bone mineral density (BMD) in Elite Competitive Track and Field Athletes. Somatic, body composition, hand grip strength birth variables, and blood vitamin D levels were also analyzed. Methods: In this cross-sectional study, the participants were 30 elite Polish athletes. Somatic measurements, body composition by kinanthropometry, and bioelectrical impedance were taken. Forearm bone parameters were measured by densitometry in two points distal (dis) and proximal (prox). Measurement of hand grip strength (HGS) on a dynamometer was performed. Birth-related factors such as birth weight and length of breastfeeding were collected by direct interview. Vitamin D concentration was assessed by blood test. Results: Low BMD was not observed in the distal part of the forearm. Significantly more frequent low BMD prox were found in endurance athletes (EA) group compared to throwing athletes group (TA) (χ2=10.8; P = .001; Φ= .6). Inadequate vitamin D levels were significantly higher in the EA group compared to TA (χ2=10.8; P = .001; Φ= .6). The main parameters affecting BMD dis and BMC prox of men athletes were HGS (kg) and type of sports competition (Rˆ2 adj= .93; .89). BMC dis was affected by the type of sports competition (Rˆ2 adj= .83). There was also a significant relationship between the T-score prox and the HGS (Rˆ2 adj= .98). In the case of women, the main parameters affecting BMD prox were menarche age and body composition (Rˆ2 adj= .93). The BMC prox was affected by the age of the menarche (Rˆ2 adj= .73). Conclusions: In conclusion, the present study found that the highest BMD of the forearm bones was characterized by men and women athletes training in athletic throws relative to running sports. Screening assessment of bone mineral status should be included in sports diagnostics.

Abstract:Bone metabolism and the trace element content associated with it change at each stage of degenerative disease. The aim of this study was to find out about the role of the analyzed elements in different stages of hip osteoarthritis. Elements associated with oxidative and enzymatic processes were analyzed depending on the changes in the radiological images of the hip joint. Element content analysis was performed by the inductively coupled plasma mass spectrometry analytical technique. The femoral head in severely osteoarthritic hips (KL3–4) compared to mild grade osteoarthritis (KL2) had a greater content of Cu (median 1.04 vs. 0.04), Sr (median 38.71 vs. 29.59), and Zn (median 75.12 vs. 63.21). There were no significant differences in the content of Mo, Cr, and Fe in the femoral head and neck between the groups. The Cu/Fe correlation was negative in the KL2 group (−0.47) and positive in the KL3–4 groups (0.45). Changes in the content and correlation of trace elements in the hip joint explain the changes in metabolism dependent on the severity of degenerative changes.

Interactions between concentrations of chemical elements in human femoral heads

Abstract:Environmental and occupational exposure to various metals has been a major public health concern and the subject of many studies. With the development of industry and transportation, environmental pollution has markedly worsened. As a result, metals are now ubiquitous and are absorbed into the body with food, drinking water, and polluted air. Exposure to these elements leads to numerous health problems, affecting almost every system of the human body, including the skeletal system. Bone is a specific research material that is difficult to obtain, therefore chemical analyses of metal concentrations in this tissue are rarely found in the literature. Nevertheless, bone, due to its long regeneration period, can serve as a biomarker of a long-term metal accumulation resulting from environmental or occupational exposure. Our study was conducted on bone samples harvested from inhabitants of the Upper Silesia region during hip replacement surgery. Femoral heads removed during surgery were sectioned into slices and further subdivided into samples comprising articular cartilage, cortical bone, and trabecular bone. Concentrations of 12 trace elements were measured with an atomic absorption spectrophotometry method. We found significant correlation between concentrations of these metal elements in the samples of cortical bone. This is determined not only by the physiological functions of these metals in hydroxyapatite, but also by the specific mineral structure of the bone tissue.
Lead and calcium content in the human hip joint

Abstract:Concentration of lead in bone, unlike in soft tissues, increases during the lifetime and reflects severity of exposure to this element. The main aim of the study was to determine concentrations of lead and calcium and to find possible relationship between calcium and lead in the tissues of the hip joints obtained from inhabitants of the Upper Silesian Industrial Area. We also attempted to identify factors that might affect this relationship. The samples were harvested intraoperatively during total hip replacement procedures; in most cases, the indication for the surgery was hip osteoarthritis. Concentrations of lead and calcium were measured with a Pye Unicam SP-9 acetylene-oxygen flame atomic absorption spectrometer. The highest mean concentration of lead was found in the cancellous bone from the femoral head, followed by articular cartilage, cortical bone and the intertrochanteric cancellous bone (0.75 μg/g). The smallest concentration was found in the joint capsule (0.19 μg/g). The highest mean concentration of calcium was found in cancellous bone from the femoral head, followed by cancellous bone from the intertrochanteric area, cortical bone, articular cartilage and joint capsule. The concentration of lead showed no correlation with sex. The bone concentration of calcium decreased with age. In the analysed hips, this finding was true in the cortical bone, as well as in the cancellous bone of the intertrochanteric area. Statistically significant correlation between calcium and lead was found only in the hip articular cartilage.
Relationship between Serum Nutritional Factors and Bone Mineral Density: A Mendelian Randomization Study

Abstract:Purpose: Multiple risk factors have been implicated in the development of osteoporosis. This study examined potential associations between serum nutritional factors and bone mineral density (BMD). Methods: Six nutritional factors were selected as exposures. Outcomes included total body BMD (n = 66 945); BMD at the forearm (FA), femoral neck (FN) and lumbar spine (LS) (n = 8143, n = 32 735, and n = 28 498, respectively); estimated heel BMD (HL eBMD) (n = 394 929); and HL eBMD stratified by sex (n = 206 496). A 2-sample Mendelian randomization approach was adopted to estimate the association between serum nutritional factors and BMD. The threshold for adjusted P value was 1.39 x 10(-3). Results: Serum calcium levels were inversely associated with LS BMD (effect = -0.55; 95% CI, -0.86 to -0.24; P = 0.001), whereas serum selenium levels were positively correlated with HL eBMD (effect = 0.22; 95% CI, 0.10 to 0.33; P = 1.70 x 10(-4)). Regarding nominal significance, there was a positive association between serum selenium levels and FA BMD. Nominally significant results were also obtained for serum retinol as well as vitamin E levels and HL eBMD. Moreover, sex-specific effects of serum retinol and vitamin E levels on BMD were observed in men. Conclusion: Serum calcium and selenium levels influence BMD at specific skeletal sites. This implies that these nutritional factors play crucial roles in bone metabolism.
Influence of Environmental Factors and Relationships between Vanadium, Chromium, and Calcium in Human Bone

Abstract:The aim of this study was to investigate the impact of environmental factors on the concentrations of vanadium (V), chromium (Cr), and calcium (Ca) and to examine the synergistic or antagonistic relationships between these metals, in cartilage (C), cortical bone (CB), and spongy bone (SB) samples obtained following hip joint surgery on patients with osteoarthritis in NW Poland. We found significantly higher concentrations of V and Cr in spongy bone in patients who consumed game meat and also those with prosthetic implants. Chromium levels were significantly lower in patients with kidney diseases. The greatest positive correlations were found between spongy bone V and (i) the amount of consumed beer and (ii) seafood diet. Correlation analysis also showed a significant correlation between Cr levels and seafood diet. To a certain extent these results indicate that the concentrations of V, Cr, and Ca in the human hip joint tissues are connected with occupational exposure, kidney diseases, diet containing game meat, sea food, beer, and the presence of implants. Furthermore, we noted new types of interactions in specific parts of the femoral head. Vanadium may contribute to the lower bone Ca levels, especially in the external parts (cartilage and cortical bone).

Abstract:Objective To investigate the effect of bone metabolic markers and bone mineral density (BMD) on bone calcium,phosphorus,and metal elements contents in geriatric patients with intertrochanteric fractures,and determine the relationship between bone mineral elements and osteoporosis.Methods Sixty two geriatric patients suffered from intertrochanteric fractures and received surgical treatment were harvested bone tissues during the operation from September 2015 to September 2016.Energy dispersive X-ray fluorescence (EDX) was used to examine the bone inorganic element contents of calcium,phosphorus,manganese,iron,copper,and zinc in cortical bone zone,cancellous bone zone and interfacing zone between cortical bone and cancellous bone.Partial correlation analysis was performed to definite the correlation between bone metabolic markers,BMD and bone calcium,phosphorus and metal elements contents by controlling the influence of age,sex and BMI.Results The partial correlation analysis suggested that the level of serum 25 hydroxy vitamin D3 [25 (OH) D3] was positively correlated to bone copper and zinc contents in cancellous bone zone (r=0.589,0.494,P=0.002,0.012).Besides,the level of parathyroid hormone (PTH) was positively correlated to bone calcium,iron,and zinc contents (P =0.009,0.011,0.025) whose coefficient r were 0.568,0.554 and 0.498 respectively but negatively correlated to bone phosphorus contents (r=-0.582,P=0.007) in the interfacing zone between cortical bone and cancellous bone.Total hip BMDs may be negatively correlated to contents (r=-0.472,P=0.089).Additionally,the BMDs in the trochanter exhibited significantly positive correlation to bone calcium contents (r=-0.621,P=0.031) but significantly negativly correlated to bone phosphorus contents (r =0.690,P =0.013).Conclusions Sufficient vitamin D3 might be beneficial to absorb zinc and copper,which could improve bone density and increase bone mass.The deceased trochanter BMDs might be correlated to the deceased bone metal trace-elements contents due to the increment of bone calcium contents in cancellous bone zone.

Abstract:Background: Erectile dysfunction (ED) stands out as one of the most prevalent sexual disorders in men, with its incidence progressively escalating with age. As delineated by the International Consultation Committee for Sexual Medicine on Definitions/Epidemiology/Risk Factors for Sexual Dysfunction, the prevalence of ED among men under 40 years is estimated to be within the range of 1–10%. The aim of this study was to determine the relationship between the concentration of bioelements (Zn, Cu, Fe, Cr, Mg, and Mn) in the serum and bone tissue and the concentration of selected hormones in men with and without erectile dysfunction. Materials and methods: The retrospective cohort study included 152 men who underwent total hip arthroplasty for hip osteoarthritis at the Department of Orthopaedic Traumatology and Musculoskeletal Oncology at the Pomeranian Medical University in Szczecin. Certain exclusion criteria were applied to ensure the integrity of the study. These included individuals with diabetes, a history of cancer, alcohol abuse, liver or kidney failure, New York Heart Association (NYHA) class III or IV heart failure, and those taking medications that affect bone metabolism, such as mineral supplements, neuroleptics, chemotherapeutic agents, immunosuppressants, corticosteroids, or antidepressants. Patients with hypogonadism or infertility were excluded from the study. Results: The study showed an association between bioT concentrations and Cu concentrations in both patients with and without erectile dysfunction. A correlation between bioactive testosterone and Cr concentrations was also observed in both groups. Patients with erectile dysfunction showed a relationship between bioT concentration and Zn concentration, TT concentration and Mn concentration, FT concentration and Zn concentration, and E2 concentration and Cr concentration. An analysis of elemental concentrations in bone tissue showed an association between FT and Mg and Mn concentrations, but only in patients with erectile dysfunction. In patients without erectile dysfunction, a correlation was observed between FT and Cu concentrations. A correlation was also observed between bioT concentrations and Mg, Mn, and Zn concentrations, but only in patients with erectile dysfunction. In patients without erectile dysfunction, a correlation was observed between bioT and Cu concentrations. Conclusions: Studying the relationship between the concentration of bioelements (Zn, Cu, Fe, Cr, Mg, and Mn) in the serum and bone tissue and the concentration of selected hormones in men may be important in explaining the etiology of the problem. The study of the concentration of Zn and Cu in bone tissue and serum showed that these two elements, regardless of the place of accumulation, may be related to the concentration of androgens in men.

Serum Zinc Levels and Multiple Health Outcomes: Implications for Zinc-Based Biomaterials

Abstract:Background: Zinc-based biomaterials, including biodegradable metal, nanoparticles, and coatings used in medical implants release zinc ions that may increase the whole-body and serum zinc concentrations. The impact of serum zinc concentrations on major health outcomes can provide insights for device design and clinical transformation of zinc-based biomaterials. Methods: This nationally representative cross-sectional study enrolled participants from the National Health and Nutrition Examination Survey (NHANES, 2011-2014) including 3607 participants. Using unadjusted and multivariate-adjusted logistic regression analyses, two-piecewise linear regression model with a smoothing function and threshold level analysis, we evaluated the associations between elevated serum zinc levels and major health outcomes. Results: Elevated serum zinc levels were significantly associated with an increase in total spine and total femur bone mineral density (BMD). Every 10 mu g/dL increase was associated with a 1.12-fold increase in diabetes mellitus (DM) and 1.23-fold and 1.29-fold increase in cardiovascular diseases (CVD) and coronary heart disease (CHD), in participants with serum zinc levels >= 100 mu g/dL. It had no significant linear or nonlinear associations with risk of fractures, congestive heart failure, heart attack, thyroid disease, arthritis, osteoarthritis, rheumatoid arthritis, dyslipidemia and cancer. Conclusion: Serum zinc levels are significantly associated with increased BMD in the total spine and total femur, and risk of DM, and CVD/CHD among participants with serum zinc levels >= 100 mu g/dL.
Sex-Specific Association of Clinical Parameters and Components of Femoral Bone Quality in Patients Undergoing Total Hip Arthroplasty

Abstract:Poor bone quality is a critical factor associated with an increased risk of complications after total hip arthroplasty (THA). However, no consistent recommendations have yet been established for assessing indicators of bone quality preoperatively. Thus, it remains unclear which preoperatively available and readily accessible parameters are most closely associated with femoral bone quality. Here, we obtained femoral neck specimens from 50 patients undergoing THA. Preoperative Dual-energy X-ray absorptiometry (DXA) scans, pelvic radiographs, and laboratory parameters were analyzed. In the obtained specimens, bone microstructure was assessed using micro-CT and histomorphometry. Additionally, matrix mineralization and osteocyte lacunar morphology were evaluated using quantitative backscattered electron imaging. Our analysis revealed that DXA-derived T-scores correlated with trabecular microstructure. Furthermore, radiographic indices and body mass index correlated differentially with aspects of bone quality in women and men. Contrary to previous observations, no correlation was found between serum vitamin D levels and osteoid indices, nor between clinical parameters and matrix mineralization. Age was strongly associated with the number of mineralized osteocyte lacunae, a factor that appeared to be independent of sex. Taken together, our study demonstrates that no single preoperatively available parameter exhibits a strong and consistent association with femoral bone quality. However, DXA remains a reliable preoperative measure for determining the trabecular microstructure of the femoral neck. In clinical practice, surgeons should adopt an individualized approach to preoperative assessments by considering age, sex, BMI, and radiographic indices to enhance their insight into femoral bone quality, particularly when DXA is unavailable.

Material and Methods: The study comprised 151 men aged 60 to 75 years who were scheduled for hip replacement surgery due to osteoarthritis. The concentrations of elements in the serum and bone tissue were measured using inductively coupled plasma optical emission spectrometry (Mg, Zn, Cu, Cr, Fe, and Pb) and the spectrofluorometric method (Se). Fasting plasma glucose (FPG), high-density cholesterol (HDL-Ch), and triacylglycerols (TAG) were determined. Lipid accumulation product (LAP) and visceral adiposity index (VAI) were calculated. Results: There was no relationship between serum and bone concentrations of bioelements and lead and the BMI index. Bone Mg was significantly higher in men with higher VAI, but no such relation was observed in the serum. Similarly, bone Mg and Zn were higher in patients with higher LAP, which was not observed in the serum. Multivariate logistic regression analysis with adjustment for age was performed. There was a correlation between serum Zn concentration and the cut-off point for VAI. The cut-off point for LAP was related to the bone tissue concentrations of Mg, Zn, and Cu. Conclusion: We found some relationships between the concentrations of selected bioelements and Pb and VAI, LAP, and BMI in bone but not in the serum. VAI positively correlated with bone Mg, while LAP positively correlated with bone Cu, Zn, and Mg. They are responsible for maintaining normal osmotic pressure and electrolyte balance, they regulate various metabolic processes and are the elements of hormones, enzymes, and coenzymes. Magnesium (Mg) deficiency in serum is associated with METS through the concept that metabolic syndrome is conditioned by the formation and development of inflammation. Blood concentrations of Cu and Fe may be positively related to diabetes risk. 2 In turn, a higher dietary intake of Zn may be associated with a reduced risk of METS. 3 The relationship between serum Pb concentration and the diagnosis of obesity is unclear. Studies show different results, eg a positive correlation 4,5 and a negative correlation, 6 with the relationship between type 2 diabetes diagnosis and Pb concentration frequently described as positive. 7,8 Although literature describes a link between macronutrient concentrations and metabolic disorders, the data are often inconclusive, 9–11 which may be related to the multiplicity of indices used to assess fat accumulation and metabolism. Abstract:For a proper assessment of osteoporotic fragility fracture prediction, all aspects regarding bone mineral density, bone texture, geometry and information about strength are necessary, particularly in endocrinological and rheumatological diseases, where bone quality impairment is relevant. Data regarding bone quantity (density) and, partially, bone quality (structure and geometry) are obtained by the gold standard method of dual X-ray absorptiometry (DXA). Data about bone strength are not yet readily available. To evaluate bone resistance to strain, a new DXA-derived index based on the Finite Element Analysis (FEA) of a greyscale of density distribution measured on spine and femoral scan, namely Bone Strain Index (BSI), has recently been developed. Bone Strain Index includes local information on density distribution, bone geometry and loadings and it differs from bone mineral density (BMD) and other variables of bone quality like trabecular bone score (TBS), which are all based on the quantification of bone mass and distribution averaged over the scanned region. This state of the art review illustrates the methodology of BSI calculation, the findings of its in reproducibility and the preliminary data about its capability to predict fragility fracture and to monitor the follow up of the pharmacological treatment for osteoporosis.

Estimation of Percentage Body Fat by Dual-Energy X-Ray Absorptiometry: Evaluation by in Vivo Human Elemental Composition

Abstract:Dual-energy x-ray absorptiometry (DXA) is widely applied for estimating body fat. The percentage of body mass as fat (%fat) is predicted from a DXA-estimated RST value defined as the ratio of soft tissue attenuation at two photon energies (e.g., 40 keV and 70 keV). Theoretically, the RST concept depends on the mass of each major element in the human body. The DXA RST values, however, have never been fully evaluated by measured human elemental composition. The present investigation evaluated the DXA RST value by the total body mass of 11 major elements and the DXA % fat by the five-component (5C) model, respectively. Six elements (i.e. C, N, Na, P, Cl and Ca) were measured by in vivo neutron activation analysis, and potassium (i.e. K) by whole-body (40)K counting in 27 healthy adults. Models were developed for predicting the total body mass of four additional elements (i.e. H, O, Mg and S). The elemental content of soft tissue, after correction for bone mineral elements, was used to predict the RST values. The DXA RST values were strongly associated with the RST values predicted from elemental content (r = 0.976, P < 0.001), although there was a tendency for the elemental-predicted RST to systematically exceed the DXA-measured RST (mean +/- SD, 1.389 +/- 0.024 versus 1.341 +/- 0.024). DXA-estimated % fat was strongly associated with 5C % fat (24.4 +/- 12.0% versus 24.9 +/- 11.1%, r = 0.983, P < 0.001). DXA RST is evaluated by in vivo elemental composition, and the present study supports the underlying physical concept and accuracy of the DXA method for estimating % fat.
The Relationship between Bone Health Parameters, Vitamin D and Iron Status, and Dietary Calcium Intake in Young Males

Abstract:Vitamin D, calcium, and iron are micronutrients crucial for bone health. However, their effect has been studied primarily in the cortical bone, with vitamin D status being assessed mainly from the total 25(OH)D serum fraction. The study aimed to investigate the impact of vitamin D (total and free fraction) and iron status (i.e., serum ferritin or soluble transferrin receptor) and calcium intake (ADOS-Ca questionnaire) on lumbar cortical and trabecular bone. In a cohort of 113 male subjects (76 athletes, 37 non-athletes) aged 15–19, the lumbar spine status (Z-score, bone mineral apparent density (BMAD), and trabecular bone score (TBS)) was determined using dual-energy X-ray absorptiometry (DXA). Relationships between the examined micronutrients and bone health parameters were observed only in athletes. Free 25(OH)D was significantly (p < 0.001) correlated with Z-score and BMAD, while total 25(OH)D (p < 0.001) and iron status (ferritin, Fe stores; p < 0.01) correlated solely with BMAD. Free 25(OH)D and ferritin concentrations were the best determinants of bone status (R2 = 0.330; p < 0.001) and explained 25% and 7% of the BMAD variance, respectively. No relationships were found between the micronutrients and TBS. The results confirmed the positive influence of vitamin D and iron on cortical, but not trabecular, bone status solely in physically active subjects. In athletes, free 25(OH)D seems to be a superior indicator of bone health to a total 25(OH)D fraction.

Investigation of Elemental Distribution in Human Femoral Head by Pixe and Srxrf Microprobe

Abstract:In order to study the distribution and possible degenerative processes inducing the loss of inorganic substances in bone and to provide a scientific basis for the prevention and therapy of osteoporosis, proton induced X-ray emission (PIXE) method is used for the determination of elemental concentrations in femoral heads from five autopsies and seven patients with femoral neck fractures. Synchrotron radiation X-ray fluorescence (SRXRF) microprobe analysis technique is used to scan a slice of the femoral head from its periphery to its center, via cartilage, compact and spongy zones. The specimen preparation and experiment procedure are described in detail. The results show that the concentrations of P, Ca, Fe, Cu, Sr in the control group are higher than those in the patient group, but the concentrations of S, K, Zn, Mn are not significantly different. The quantitative results of elemental distribution, such as Ca, P, K, Fe, Zn, Sr and Pb in bone slice tissue including cartilage, substantial compact and substantial spongy, are investigated. The data obtained show that the concentrations of Ca, P, K, (the major elements of bone composition), are obviously low in both spongy and cartilage zones in the patient group, but there are no remarkable differences in the compact zone. Combined with the correlations between P, K, Zn, Sr and Ca, the loss mechanism of minerals and the physiological functions of some metal elements in bone are also discussed.
Sex-specific associations of urinary mixed-metal concentrations with femoral bone mineral density among older people: an NHANES (2017–2020) analysis

Abstract:Background: Heavy metal exposure is an important cause of reduced bone mineral density (BMD). Epidemiological studies focusing on the effects of mixed heavy metal exposure on BMD in middle-aged and older people are scarce. In single-metal studies, men and women have shown distinct responses of BMD to environmental metal exposure. This study therefore aimed to elucidate the association between mixed heavy metal exposure and BMD and to investigate whether it is sex-specific. Methods: Data from the 2017–2020 National Health and Nutrition Examination Survey were selected for this cross-sectional study. The study used three statistical methods, i.e., linear regression, Bayesian kernel machine regression (BKMR) modeling, and weighted quartiles (WQS) regression, to explore the association between the urinary concentrations of 11 metals (barium, cadmium, cobalt, cesium, manganese, molybdenum, lead, antimony, tin, thallium, and Tungsten), either individually or as a mixture, and total femoral BMD. Results: A total of 1,031 participants were included in this study. Femoral BMD was found to be higher in men than women. A significant negative correlation between the urinary concentrations of the 10 metals and femoral BMD was found in the overall cohort. Further gender sub-stratified analyses showed that in men, urinary metal concentrations were negatively correlated with femoral BMD, with cobalt and barium playing a significant and non-linear role in this effect. In women, although urinary metal concentrations negatively modulated femoral BMD, none of the correlations was statistically significant. Antimony showed sex-specific differences in its effect. Conclusion: The urinary concentrations of 10 mixed heavy metals were negatively correlated with femoral BMD in middle-aged and older participants, and this effect showed gender differences. These findings emphasize the differing role of mixed metal exposure in the process of BMD reduction between the sexes but require further validation by prospective studies.

Abstract:Background: Toxic elements, such as aluminum (Al), arsenic (As), cadmium (Cd), and lead (Pb), are persistent environmental pollutants that can cause adverse effects on the health of exposed individuals. Bone is one of the primary target organs of accumulation and potential damage from toxic elements. Objectives: This study was performed to determine the Al, As, Cd, and Pb concentrations in the femoral cancellous bone, femoral cartilage, anterior cruciate ligament, meniscus, tibial cartilage, tibial cancellous bone and infrapatellar fat pad. Furthermore, the aim of this study was to explore the relationships between toxic element concentrations and related factors such as gender, age, place of residence, hypertension and diabetes, and to determine the correlations among these toxic elements in knee joint structures. Methods: The samples used this study were collected from 51 patients following total knee arthroplasty. The Al, As, Cd, and Pb concentrations were determined using inductively coupled plasma optic emission spectrometry. Results: Significant differences were found in the Al, As, Cd, and Pb concentrations among the knee joint structures. Cd concentration in the tibial cancellous bone in women was significantly higher than in men. Pb concentration in the infrapatellar fat pad of urban patients was significantly higher as compared to rural patients. Al concentrations in the femoral cancellous bone, femoral cartilage, anterior cruciate ligament, meniscus and tibial cartilage were significantly higher in patients living in urban areas than in rural areas. As concentration in the tibial cancellous bone of diabetic patients was significantly higher compared to non-diabetic patients. In addition, significant Spearman's positive correlations were found between Al and Pb in the knee joint structures. Conclusion: The obtained results of the investigated toxic elements may serve as a basis for establishing the reference values of Al, As, Cd, and Pb in the knee joint structures. The results reported in the study provides novel data regarding the relationships between the toxic element concentrations and gender, age, place of residence, hypertension and diabetes in the studied structures of knee joint. Furthermore, new interactions among these toxic elements were noted.
Association between physical function and parameters of hip structural analysis in patients with hip fracture

Abstract:Objectives: The current study sought to investigate whether physical function and activity were associated with hip structural analysis (HSA) parameters on the non-fracture side of patients with hip fractures. Methods: Participants were patients with unilateral hip fracture treated by surgery. HSA of the proximal femur was conducted based on dual-energy x-ray absorptiometry data. HSA parameters in the narrow neck region included cross-sectional area (CSA), cross-sectional moment of inertia (CSMI), section modulus (SM), and buckling ratio (BR). Hierarchical multiple regression analysis was conducted to identify predictors of HSA. Results: Except for the adjustment variables, age, gender and BMI, other variables were extracted. Hierarchical multiple regression analysis (standardised partial regression coefficients) identified movement control during one-leg standing on the non-fractured side (0.15) as factors associated with CSA. Hierarchical multiple regression analysis (standardised partial regression coefficients) identified hand grip (0.12, 0.23) as factors associated with CSMI and SM, respectively. Hierarchical multiple regression analysis (standardised partial regression coefficients) identified presence of steroid (0.23) and cerebrovascular disease (0.19) as factors associated with BR. The coefficients of determination adjusted for degrees of freedom (R2) were 0.545, 0.331, 0.401, and 0.148 for CSA, CSMI, SM, and BR, respectively. Conclusions: Our results indicate that movement control during 1-leg standing and muscle strength may be important for maintaining and improving bone strength.

Abstract:BackgroundDXA is widely used in clinical practise in BMD measurement of osteoporosis. Forearm and axial BMD is the most common site to define in clinical practise. However, the relationship between forearm and axial BMD is not confirmed in RA patients.ObjectivesTo compare the forearm (bone mineral density) BMD between lumbar spine and left hip BMD by dual-energy X-ray abosorptionmetry (DXA), and explore the diagnostic value of the forearm BMD in rheumatoid arthritis (RA) patients.MethodsIn the study, 200 female patients with established RA underwent DXA of the lumber, left hip and non-superiority forearm DXA at the same time. We compared BMD at different sites, and the diagnostic cut-off value of abnormal axial BMD by forearm BMD was explored. Data analysis was performed by independent-sample t test and Pearson’s correlation test. The area under the receiver operating characteristic curve (AUC) and calibration of predictions were assessed. Sensitivity and specificity were calculated to determine the correlation between cases of osteoporosis detected by the axial DXA scan and forearm.Results(1) The mean age of the 200 female patients was (55.9±13.8) years. Twenty (10.0%) patients had the fragility fracture history. Based on their axial DXA data and fracture history, 30 (15.0%) patients had normal BMD (T score≥-1.0), 170 (85.0%) patients had abnormal BMD. The mean forearm BMD was (0.35±0.13) g/cm2, and T score was -2.3±1.9. (2) Compared with abnormal axial BMD group, forearm BMD in normal axial group was significantly decreased [(0.33±0.13) g/cm2 vs (0.44±0.06) g/cm2, t=4.29, P<0.01], forearm T score was also significantly decreased [(-2.6±1.9) vs (-0.71±0.9), t=4.35, P<0.01]. T score of the forearm BMD was positively correlated with axial BMD T score, both at lumber and left hip respectively (r=0.488, 0.543, P<0.01). After correction by age, T score of the forearm remained positively correlated with axial T score by partial correlation analysis (Pearson partial vertebral r=0.46, P<0.001; left hip r=0.502, P<0.001). (3) The area under the ROC curve for diagnosis of lumber and hip osteoporosis were 0.757 (95%CI: 0.638, 0.876) and 0.615(95%CI: 0.481,0.749) separately. (4) The sensitivity and specificity for identifying osteoporosis in lumber were 86.5% and 50.1% respectively when the T-score threshold of forearm was defined as -3.52; however, the sensitivity and the specificity for identifying osteoporosis at left hip were 66.2% and 76.2% when T score threshold of forearm was defined as -2.5.ConclusionOur study has confirmed that DXA measurement performed of forearm analysis is capable of screening osteoporosis defined by axial BMD in female RA patients.References[1] Siris ES, Adler R, Bilezikian J, et al.
