Abstract:ObjectivesTo assess the impact on image quality and dose reduction of a new deep learning image reconstruction (DLIR) algorithm compared with a hybrid iterative reconstruction (IR) algorithm.MethodsData acquisitions were performed at seven dose levels (CTDIvol : 15/10/7.5/5/2.5/1/0.5 mGy) using a standard phantom designed for image quality assessment. Raw data were reconstructed using the filtered back projection (FBP), two levels of IR (ASiR-V50% (AV50); ASiR-V100% (AV100)), and three levels of DLIR (TrueFidelity™ low, medium, high). Noise power spectrum (NPS) and task-based transfer function (TTF) were computed. Detectability index (d′) was computed to model a large mass in the liver, a small calcification, and a small subtle lesion with low contrast.ResultsNPS peaks were higher with AV50 than with all DLIR levels and only higher with DLIR-H than with AV100. The average NPS spatial frequencies were higher with DLIR than with IR. For all DLIR levels, TTF50% obtained with DLIR was higher than that with IR. d′ was higher with DLIR than with AV50 but lower with DLIR-L and DLIR-M than with AV100. d′ values were higher with DLIR-H than with AV100 for the small low-contrast lesion (10 ± 4%) and in the same range for the other simulated lesions.ConclusionsNew DLIR algorithm reduced noise and improved spatial resolution and detectability without changing the noise texture. Images obtained with DLIR seem to indicate a greater potential for dose optimization than those with hybrid IR.Key Points• This study assessed the impact on image quality and radiation dose of a new deep learning image reconstruction (DLIR) algorithm as compared with hybrid iterative reconstruction (IR) algorithm.• The new DLIR algorithm reduced noise and improved spatial resolution and detectability without perceived alteration of the texture, commonly reported with IR.• As compared with IR, DLIR seems to open further possibility of dose optimization.

Performance evaluation of deep learning image reconstruction algorithm for dual-energy spectral CT imaging: A phantom study

Abstract:OBJECTIVES: To evaluate the performance of deep learning image reconstruction (DLIR) algorithm in dual-energy spectral CT (DEsCT) as a function of radiation dose and image energy level, in comparison with filtered-back-projection (FBP) and adaptive statistical iterative reconstruction-V (ASIR-V) algorithms. METHODS: An ACR464 phantom was scanned with DEsCT at four dose levels (3.5 mGy, 5 mGy, 7.5 mGy, and 10 mGy). Virtual monochromatic images were reconstructed at five energy levels (40 keV, 50 keV, 68 keV, 74 keV, and 140 keV) using FBP, 50% and 100% ASIR-V, DLIR at low (DLIR-L), medium (DLIR-M), and high (DLIR-H) settings. The noise power spectrum (NPS), task-based transfer function (TTF) and detectability index (d’) were computed and compared among reconstructions. RESULTS: NPS area and noise increased as keV decreased, with DLIR having slower increase than FBP and ASIR-V, and DLIR-H having the lowest values. DLIR had the best 40 keV/140 keV noise ratio at various energy levels, DLIR showed higher TTF (50%) than ASIR-V for all materials, especially for the soft tissue-like polystyrene insert, and DLIR-M and DLIR-H provided higher d’ than DLIR-L, ASIR-V and FBP in all dose and energy levels. As keV increases, d’ increased for acrylic insert, and d’ of the 50 keV DLIR-M and DLIR-H images at 3.5 mGy (7.39 and 8.79, respectively) were higher than that (7.20) of the 50 keV ASIR-V50% images at 10 mGy. CONCLUSIONS: DLIR provides better noise containment for low keV images in DEsCT and higher TTF(50%) for the polystyrene insert over ASIR-V. DLIR-H has the lowest image noise and highest detectability in all dose and energy levels. DEsCT 50 keV images with DLIR-M and DLIR-H show potential for 65% dose reduction over ASIR-V50% withhigher d’.

Application of deep learning image reconstruction in low-dose chest CT scan

Abstract:Objective: Deep learning image reconstruction (DLIR) is a new reconstruction method for maintaining image quality at reduced radiation dose. The purpose of this study was to compare image quality of reduced-dose DLIR images with the standard-dose adaptive statistical iterative reconstruction (ASIR-V) images in chest CT. Methods: Our prospective study included 48 adult patients (30 women and 18 men, mean age ±SD, 49.8 ± 14.3 years) who underwent both the standard-dose CT (SDCT) and low-dose CT (LDCT) on a GE Revolution CT scanner. All patients gave written informed consent. All scans were reconstructed with ASIR-V40%. Additionally, LDCT scans were reconstructed with DLIR with high-setting (DLIR-H) and medium-setting (DLIR-M). Image noise and contrast-noise-ratio (CNR) of thoracic aorta with different reconstruction modes were measured and compared. Results: LDCT reduced radiation dose by 96% compared with SDCT (CTDIvol: 0.54mGy vs 12.46mGy). In LDCT, DLIR significantly reduced image noise compared with the state-of-the-art ASIR-V40% with DLIR-H provided the lowest image noise and highest image quality score. In addition, the image noise, CNR of aorta and overall image quality of the low-dose DLIR-H images did not have significant difference compared with the SDCT ASIR-V40% images (all p > 0.05). Conclusion: DLIR significantly reduces image noise in LDCT chest scans and provides similar image quality as the SDCT ASIR-V images at 4% of the radiation dose. Advances in knowledge: DLIR uses high-quality FBP data to train deep neural networks to learn how to distinguish between signal and noise, and effectively suppresses noise without affecting anatomical and pathological structures. It opens a new era of CT image reconstruction. DLIR significantly reduces image noise and improves image quality compared with ASIR-V40% under same radiation dose condition. DLIR-H achieves similar image quality at 4% radiation dose as ASIR-V40% at standard-dose level in non-contrast chest CT.

Abstract:Deep learning image reconstruction (DLIR) algorithms employ convolutional neural networks (CNNs) for CT image reconstruction to produce CT images with a very low noise level, even at a low radiation dose. The aim of this study was to assess whether the DLIR algorithm reduces the CT effective dose (ED) and improves CT image quality in comparison with filtered back projection (FBP) and iterative reconstruction (IR) algorithms in intensive care unit (ICU) patients. We identified all consecutive patients referred to the ICU of a single hospital who underwent at least two consecutive chest and/or abdominal contrast-enhanced CT scans within a time period of 30 days using DLIR and subsequently the FBP or IR algorithm (Advanced Modeled Iterative Reconstruction [ADMIRE] model-based algorithm or Adaptive Iterative Dose Reduction 3D [AIDR 3D] hybrid algorithm) for CT image reconstruction. The radiation ED, noise level, and signal-to-noise ratio (SNR) were compared between the different CT scanners. The non-parametric Wilcoxon test was used for statistical comparison. Statistical significance was set at p < 0.05. A total of 83 patients (mean age, 59 ± 15 years [standard deviation]; 56 men) were included. DLIR vs. FBP reduced the ED (18.45 ± 13.16 mSv vs. 22.06 ± 9.55 mSv, p < 0.05), while DLIR vs. FBP and vs. ADMIRE and AIDR 3D IR algorithms reduced image noise (8.45 ± 3.24 vs. 14.85 ± 2.73 vs. 14.77 ± 32.77 and 11.17 ± 32.77, p < 0.05) and increased the SNR (11.53 ± 9.28 vs. 3.99 ± 1.23 vs. 5.84 ± 2.74 and 3.58 ± 2.74, p < 0.05). CT scanners employing DLIR improved the SNR compared to CT scanners using FBP or IR algorithms in ICU patients despite maintaining a reduced ED.

Abstract:OBJECTIVES To explore the feasibility of achieving diagnostic images in low-dose abdominal CT using a Deep Learning Image Reconstruction (DLIR) algorithm. METHODS Prospectively enrolled 47 patients requiring contrast-enhanced abdominal CT scans. The late-arterial phase scan was added and acquired using lower-dose mode (tube current range, 175-545 mA; 80 kVp for patients with BMI ≤24 kg/m2 and 100 kVp for patients with BMI > 24 kg/m2) and reconstructed with DLIR at medium setting (DLIR-M) and high setting (DLIR-H), ASIR-V at 0% (FBP), 40% and 80% strength. Both the quantitative measurement and qualitative analysis of the five types of reconstruction methods were compared. In addition, radiation dose and image quality between the early-arterial phase ASIR-V images using standard-dose and the late-arterial phase DLIR images using low-dose were compared. RESULTS For the late-arterial phase, all five reconstructions had similar CT value (P > 0.05). DLIR-H, DLIR-M and ASIR-V80% images significantly reduced the image noise and improved the image contrast noise ratio, compared with the standard ASIR-V40% images (P < 0.05). ASIR-V80% images had undesirable image characteristics with obvious "waxy" artifacts, while DLIR-H images maintained high spatial resolution and had the highest subjective image quality. Compared with the early-arterial scans, the late-arterial phase scans significantly reduced the radiation dose (P < 0.05), while the DLIR-H images exhibited lower image noise and good display of the specific image details of lesions. CONCLUSIONS DLIR algorithm improves image quality under low-dose scan condition and may be used to reduce the radiation dose without adversely affecting the image quality.

Abstract:ObjectivesTo compare the effect of deep-learning image reconstruction (DLIR) and adaptive statistical iterative reconstruction-Veo (ASiR-V) for upper abdominal-enhanced CT. The foci of this study were to validate whether the image effect of DLIR would be better than that of ASiR-V under the same dose and whether the image effect of DLIR under low dose would be not lower than that of ASiR-V under routine dose, and the two algorithms were studied from different layer thickness.MethodsA total of 17 patients with hepatic lesions undergoing upper abdominal-enhanced CT examination were randomly divided into two groups: low dose (LD, n = 7) with 150 mAs and ultralow dose (ULD, n = 10) with 100 mAs; routine dose (RD) scans were subsequently performed in both groups (n = 17). Images were reconstructed with DLIR with low, medium, and high strength (DL-L, -M, and -H, respectively) and ASiR-V with standard 30% and 70% (ASiR-V 30% and 70%) on thin (0.625 mm) and thick (5 mm) slices. Noise and contrast-to-noise ratio (CNR) measurements were performed. Two radiologists, blinded to examination details, scored four categories of image quality while comparing reconstructions for vascular significance, image noise and texture, lesion conspicuity and lesion diagnosis confidence.ResultsUnder the same dose, DLIR had better performance in noise, CNR and image quality scores than ASiR-V did, DL-H performed the best. Under different radiation doses, ULD-DL-M did not differ significantly from RD-ASiR-V 30% in noise and CNR (P > 0.05), but had lower image quality scores (P < 0.05). Noise, CNR and image quality scores of the LD-DL-H were comparable to those of RD-ASiR-V 30% (P > 0.05).ConclusionDLIR can significantly improve the image quality of enhanced abdominal CT under the same dose. By decreasing the radiation dose to 150mAs, DL-H showed comparable image quality to that of RD-ASiR-V 30%.

Abstract:ObjectivesTo evaluate the usefulness of deep learning image reconstruction (DLIR) to improve the image quality of dual-energy computed tomography (DECT) of the abdomen, compared to hybrid iterative reconstruction (IR).MethodsThis study included 40 patients who underwent contrast-enhanced DECT of the abdomen. Virtual monochromatic 40-, 50-, and 70-keV and iodine density images were reconstructed using three reconstruction algorithms, including hybrid IR (ASiR-V50%) and DLIR (TrueFidelity) at medium- and high-strength level (DLIR-M and DLIR-H, respectively). The standard deviation of attenuation in liver parenchyma was measured as image noise. The contrast-to-noise ratio (CNR) for the portal vein on portal venous phase CT was calculated. The vessel conspicuity and overall image quality were graded on a 5-point scale ranging from 1 (poor) to 5 (excellent). The comparative scale of lesion conspicuity in 47 abdominal solid lesions was evaluated on a 5-point scale ranging from 0 (best) to −4 (markedly inferior).ResultsThe image noise of virtual monochromatic 40-, 50 -, and 70-keV and iodine density images was significantly decreased by DLIR compared to hybrid IR (p < 0.0001). The CNR was significantly higher in DLIR-H and DLIR-M than in hybrid IR (p < 0.0001). The vessel conspicuity and overall image quality scores were also significantly greater in DLIR-H and DLIR-M than in hybrid IR (p < 0.05). The lesion conspicuity scores for DLIR-M and DLIR-H were significantly higher than those for hybrid IR in the virtual monochromatic image of all energy levels (p ≤ 0.001).ConclusionsDLIR improves vessel conspicuity, CNR, and lesion conspicuity of virtual monochromatic and iodine density images in abdominal contrast-enhanced DECT, compared to hybrid IR.Key Points• Deep learning image reconstruction (DLIR) is useful for reducing image noise and improving the CNR of visual monochromatic 40-, 50-, and 70-keV images in dual-energy CT.• DLIR can improve lesion conspicuity of abdominal solid lesions on virtual monochromatic images compared to hybrid iterative reconstruction.• DLIR can also be applied to iodine density maps and significantly improves their image quality.

Abstract:Objectives: To assess the impact of a new artificial intelligence deep-learning reconstruction (Precise Image; AI-DLR) algorithm on image quality against a hybrid iterative reconstruction (IR) algorithm in abdominal CT for different clinical indications. Methods: Acquisitions on phantoms were performed at 5 dose levels (CTDIvol: 13/11/9/6/1.8 mGy). Raw data were reconstructed using level 4 of iDose4 (i4) and 3 levels of AI-DLR (Smoother/Smooth/Standard). Noise power spectrum (NPS), task-based transfer function (TTF) and detectability index (d') were computed: d' modelled detection of a liver metastasis (LM) and hepatocellular carcinoma at portal (HCCp) and arterial (HCCa) phases. Image quality was subjectively assessed on an anthropomorphic phantom by 2 radiologists. Results: From Standard to Smoother levels, noise magnitude and average NPS spatial frequency decreased and the detectability (d') of all simulated lesions increased. For both inserts, TTF values were similar for all three AI-DLR levels from 13 to 6 mGy but decreased from Standard to Smoother levels at 1.8 mGy. Compared to the i4 used in clinical practice, d' values were higher using the Smoother and Smooth levels and close for the Standard level. For all dose levels, except at 1.8 mGy, radiologists considered images satisfactory for clinical use for the 3 levels of AI-DLR, but rated images too smooth using the Smoother level. Conclusion: Use of the Smooth and Smoother levels of AI-DLR reduces the image noise and improves the detectability of lesions and spatial resolution for standard and low-dose levels. Using the Smooth level is apparently the best compromise between the lowest dose level and adequate image quality. Abstract:This study aimed to compare the performance of deep learning image reconstruction (DLIR) and adaptive statistical iterative reconstruction-Veo (ASIR-V) in improving image quality and diagnostic performance using virtual monochromatic spectral images in abdominal dual-energy computed tomography (DECT). Sixty-two patients [mean age ± standard deviation (SD): 56 years ± 13; 30 men] who underwent abdominal DECT were prospectively included in this study. The 70-keV DECT images in the portal phase were reconstructed at 5-mm and 1.25-mm slice thicknesses with 40% ASIR-V (ASIR-V40%) and at 1.25-mm slice with deep learning image reconstruction at medium (DLIR-M) and high (DLIR-H) levels and then compared. Computed tomography (CT) attenuation, SD values, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) were measured in the liver, spleen, erector spinae, and intramuscular fat. The lesions in each reconstruction group at 1.25-mm slice thickness were counted. The image quality and diagnostic confidence were subjectively evaluated by two radiologists using a 5-point scale. For the 1.25-mm images, DLIR-M and DLIR-H had lower SD, higher SNR and CNR, and better subjective image quality compared with ASIR-V40%; DLIR-H performed the best (all P values < 0.001). Furthermore, the 1.25-mm DLIR-H images had similar SD, SNR, and CNR values as the 5-mm ASIR-V40% images (all P > 0.05). Three image groups had similar lesion detection rates, but DLIR groups exhibited higher confidence in diagnosing lesions. Compared with ASIR-V40% at 70 keV, 70-keV DECT with DLIR-H further reduced image noise and improved image quality. Additionally, it improved diagnostic confidence while ensuring a consistent lesion detection rate of liver lesions.

Abstract:Background Recently, computed tomography (CT) manufacturers have developed deep‐learning‐based reconstruction algorithms to compensate for the limitations of iterative reconstruction (IR) algorithms, such as image smoothing and the spatial resolution's dependence on contrast and dose levels. Purpose To assess the impact of an artificial intelligence deep‐learning reconstruction (AI‐DLR) algorithm on image quality and dose reduction compared with a hybrid IR algorithm in chest CT for different clinical indications. Methods Acquisitions on the CT American College of Radiology (ACR) 464 and CT Torso CTU‐41 phantoms were performed at five dose levels (CTDIvol: 9.5/7.5/6/2.5/0.4 mGy) used for chest CT conditions. Raw data were reconstructed using filtered backprojection, two levels of IR (iDose4 levels 4 (i4) and 7 (i7)), and five levels of AI‐DLR (Precise Image; Smoother, Smooth, Standard, Sharp, Sharper). Noise power spectrum (NPS), task‐based transfer function, and detectability index (d′) were computed: d′‐modeled detection of a soft tissue mediastinal nodule (low‐contrast soft tissue chest nodule within the mediastinum [LCN]), ground‐glass opacity (GGO), or high‐contrast pulmonary (HCP) lesion. The subjective image quality of chest anthropomorphic phantom images was independently evaluated by two radiologists. They assessed image noise, image smoothing, contrast between vessels and fat in the mediastinum for mediastinal images, visual border detection between bronchus and lung parenchyma for parenchymal images, and overall image quality using a commonly used four‐ or five‐point scale. Results From Standard to Smoother levels, on average, the noise magnitude decreased (for all dose levels: −66.3% ± 0.5% for mediastinal images and −63.1% ± 0.1% for parenchymal images), the average NPS spatial frequency decreased (for all dose levels: −35.3% ± 2.2% for mediastinal images and −13.3% ± 2.2% for parenchymal images), and the detectability (d′) of the three lesions increased. The opposite pattern was found from Standard to Sharper levels. From Smoother to Sharper levels, the spatial resolution increased for the low‐contrast polyethylene insert and the opposite for the high‐contrast air insert. Compared to the i4 used in clinical practice, d′ values were higher using Smoother (mean for all dose levels: 338.7% ± 29.4%), Smooth (103.4% ± 11.2%), and Standard (34.1% ± 6.6%) levels for the LCN on mediastinal images and Smoother (169.5% ± 53.2% for GGO and 136.9% ± 1.6% for HCP) and Smooth (36.4% ± 22.1% and 24.1% ± 0.9%, respectively) levels for parenchymal images. Radiologists considered the images satisfactory for clinical use at these levels, but adaptation to the dose level of the protocol is required. Conclusion With AI‐DLR, the smoothest levels reduced the noise and improved the detectability of chest lesions but increased the image smoothing. The opposite was found with the sharpest levels. The choice of level depends on the dose level and type of image: mediastinal or parenchymal.

Abstract:Objective: Few studies have explored the clinical feasibility of using deep-learning reconstruction to reduce the radiation dose of CT. We aimed to compare the image quality and lung nodule detectability between chest CT using a quarter of the low dose (QLD) reconstructed with vendor-agnostic deep-learning image reconstruction (DLIR) and conventional low-dose (LD) CT reconstructed with iterative reconstruction (IR). Materials and methods: We retrospectively collected 100 patients (median age, 61 years [IQR, 53-70 years]) who received LDCT using a dual-source scanner, where total radiation was split into a 1:3 ratio. QLD CT was generated using a quarter dose and reconstructed with DLIR (QLD-DLIR), while LDCT images were generated using a full dose and reconstructed with IR (LD-IR). Three thoracic radiologists reviewed subjective noise, spatial resolution, and overall image quality, and image noise was measured in five areas. The radiologists were also asked to detect all Lung-RADS category 3 or 4 nodules, and their performance was evaluated using area under the jackknife free-response receiver operating characteristic curve (AUFROC). Results: The median effective dose was 0.16 (IQR, 0.14-0.18) mSv for QLD CT and 0.65 (IQR, 0.57-0.71) mSv for LDCT. The radiologists' evaluations showed no significant differences in subjective noise (QLD-DLIR vs. LD-IR, lung-window setting; 3.23 ± 0.19 vs. 3.27 ± 0.22; P = .11), spatial resolution (3.14 ± 0.28 vs. 3.16 ± 0.27; P = .12), and overall image quality (3.14 ± 0.21 vs. 3.17 ± 0.17; P = .15). QLD-DLIR demonstrated lower measured noise than LD-IR in most areas (P < .001 for all). No significant difference was found between QLD-DLIR and LD-IR for the sensitivity (76.4% vs. 72.2%; P = .35) or the AUFROCs (0.77 vs. 0.78; P = .68) in detecting Lung-RADS category 3 or 4 nodules. Under a noninferiority limit of -0.1, QLD-DLIR showed noninferior detection performance (95% CI for AUFROC difference, -0.04 to 0.06). Conclusion: QLD-DLIR images showed comparable image quality and noninferior nodule detectability relative to LD-IR images.
Deep Learning Versus Iterative Reconstruction for CT Pulmonary Angiography in the Emergency Setting: Improved Image Quality and Reduced Radiation Dose

Abstract:To compare image quality and the radiation dose of computed tomography pulmonary angiography (CTPA) subjected to the first deep learning-based image reconstruction (DLR) (50%) algorithm, with images subjected to the hybrid-iterative reconstruction (IR) technique (50%). One hundred forty patients who underwent CTPA for suspected pulmonary embolism (PE) between 2018 and 2019 were retrospectively reviewed. Image quality was assessed quantitatively (image noise, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR)) and qualitatively (on a 5-point scale). Radiation dose parameters (CT dose index, CTDIvol; and dose-length product, DLP) were also recorded. Ninety-three patients were finally analyzed, 48 with hybrid-IR and 45 with DLR images. The image noise was significantly lower and the SNR (24.4 ± 5.9 vs. 20.7 ± 6.1) and CNR (21.8 ± 5.8 vs. 18.6 ± 6.0) were significantly higher on DLR than hybrid-IR images (p < 0.01). DLR images received a significantly higher score than hybrid-IR images for image quality, with both soft (4.4 ± 0.7 vs. 3.8 ± 0.8) and lung (4.1 ± 0.7 vs. 3.6 ± 0.9) filters (p < 0.01). No difference in diagnostic confidence level for PE between both techniques was found. CTDIvol (4.8 ± 1.4 vs. 4.0 ± 1.2 mGy) and DLP (157.9 ± 44.9 vs. 130.8 ± 41.2 mGy∙cm) were lower on DLR than hybrid-IR images. DLR both significantly improved the image quality and reduced the radiation dose of CTPA examinations as compared to the hybrid-IR technique.

Abstract:Background: Numerous computed tomography (CT) image reconstruction algorithms have been developed to improve image quality, and high-quality renal CT images are crucial to clinical diagnosis. This study evaluated the image quality and lesion visibility of deep learning-based image reconstruction (DLIR) compared with adaptive statistical iterative reconstruction-Veo (ASiR-V) in contrast-enhanced renal CT at different reconstruction strengths and doses. Methods: From January 2020 to May 2021, we prospectively included 101 patients who underwent renal contrast-enhanced CT scanning (69 at 120 kV; 32 at 80 kV). All image data were reconstructed with ASiR-V (30% and 70%) and DLIR at low, medium, and high reconstruction strengths (DLIR-L, DLIR-M, and DLIR-H, respectively). The CT number, noise, noise reduction rate (NRR), signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), overall image quality, and the proportion of acceptable images were compared. Lesions of DLIR groups were evaluated, and the conspicuity-to-noise ratio (C/N) was calculated. Results: Quantitative values (noise, SNR, CNR, and NRR) significantly differed between all reconstructions at 120 and 80 kV (P<0.001) and between each reconstruction, except ASiR-V 70% vs. DLIR-M. At 120 kV, the overall image quality and the proportion of acceptable images significantly differed between all reconstructions (P<0.001) and between each reconstruction, except ASiR-V 30% vs. DLIR-L and ASiR-V 70% vs. DLIR-M. At 80 kV, the overall image quality significantly differed between all reconstructions (P<0.001) and between each reconstruction, except between ASiR-V 30%, ASiR-V 70%, and DLIR-L. Quantitative and qualitative values were highest in DLIR-H, while the values were close in DLIR-H (80 kV) vs. ASiR-V 70% (120 kV) and DLIR-M (80 kV) vs. ASiR-V 30% (120 kV). The lesion conspicuity and noise significantly differed in DLIR at 120 kV and 80 kV (P<0.001). C/N significantly differed in DLIR at 120 kV (P<0.001) but not at 80 kV. DLIR-L and DLIR-M exhibited much-improved lesion display (C/N >1), and DLIR-H exhibited much-improved noise (C/N <1) at 120 kV. Conclusions: DLIR significantly improved the image quality and lesion visibility of renal CT compared with ASiR-V, even at a low dose.

Abstract:This study aimed to evaluate the value of the deep learning image reconstruction (DLIR) algorithm (GE Healthcare's TrueFidelityTM) in improving the image quality of low-dose computed tomography (LDCT) of the chest. First, we retrospectively extracted raw data of chest LDCT from 50 patients and reconstructed them by using model-based adaptive statistical iterative reconstruction-Veo at 50% (ASIR-V 50%) and DLIR at medium and high strengths (DLIR-M and DLIR-H). Three sets of images were obtained. Next, two radiographers measured the mean CT value/image signal and standard deviation (SD) in Hounsfield units at the region of interest (ROI) and calculated the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR). Two radiologists subjectively evaluated the image quality using a 5-point Likert scale. The differences between the groups of data were analyzed through a repeated measures ANOVA or the Friedman test. Last, our result show that the three reconstructions did not differ significantly in signal (p > 0.05) but had significant differences in noise, SNR, and CNR (p < 0.001). The subjective scores significantly differed among the three reconstruction modalities in soft tissue (p 0.05). DLIR-H had the best noise reduction ability and improved SNR and CNR without distorting the image texture, followed by DLIR-M and ASIR-V 50%. In summary, DLIR can provide a higher image quality at the same dose, enhancing the physicians' diagnostic confidence and improving the diagnostic efficacy of LDCT for lung cancer screening.

Abstract:Background: The most recent advances in Computed Tomography (CT) image reconstruction technology are Deep learning image reconstruction (DLIR) algorithms. Due to drawbacks in Iterative reconstruction (IR) techniques such as negative image texture and nonlinear spatial resolutions, DLIRs are gradually replacing them. However, the potential use of DLIR in Head and Chest CT has to be examined further. Hence, the purpose of the study is to review the influence of DLIR on Radiation dose (RD), Image noise (IN), and outcomes of the studies compared with IR and FBP in Head and Chest CT examinations. Methods: We performed a detailed search in PubMed, Scopus, Web of Science, Cochrane Library, and Embase to find the articles reported using DLIR for Head and Chest CT examinations between 2017 to 2023. Data were retrieved from the short-listed studies using Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines. Results: Out of 196 articles searched, 15 articles were included. A total of 1292 sample size was included. 14 articles were rated as high and 1 article as moderate quality. All studies compared DLIR to IR techniques. 5 studies compared DLIR with IR and FBP. The review showed that DLIR improved IQ, and reduced RD and IN for CT Head and Chest examinations. Conclusions: DLIR algorithm have demonstrated a noted enhancement in IQ with reduced IN for CT Head and Chest examinations at lower dose compared with IR and FBP. DLIR showed potential for enhancing patient care by reducing radiation risks and increasing diagnostic accuracy.
Dose reduction potential of vendor-agnostic deep learning model in comparison with deep learning-based image reconstruction algorithm on CT: a phantom study

Abstract:Objectives: To compare the dose reduction potential (DRP) of a vendor-agnostic deep learning model (DLM, ClariCT.AI) with that of a vendor-specific deep learning-based image reconstruction algorithm (DLR, TrueFidelity™). Methods: Computed tomography (CT) images of a multi-sized image quality phantom (Mercury v4.0) were acquired under six radiation dose levels (0.48/0.97/1.93/3.87/7.74/15.47 mGy) and were reconstructed using filtered back projection (FBP) and three strength levels of the DLR (low/medium/high). The FBP images were denoised using the DLM. For all DLM and DLR images, the detectability index (d') (a task-based detection performance metric) was obtained, under various combinations of three target sizes (10/5/1 mm), five inlets (CT value difference with the background; -895/50/90/335/1000 HU), five phantom diameters (36/31/26/21/16 cm), and six radiation dose levels. Dose reduction potential (DRP) measures the dose reduction made by using DLM or DLR, while yielding d' equivalent to that of FBP at full dose. Results: The DRPs of the DLM, DLR-low, DLR-medium, and DLR-high were 86% (81-88%), 60% (46-67%), 76% (60-81%), and 87% (78-92%), respectively. For 10-mm targets, the DRP of the DLM (87%) was higher than that of all DLR algorithms (58-86%). However, for smaller targets (5 mm/1 mm), the DRPs of the DLR-high (89/88%) were greater than those of the DLM (87/84%). Conclusion: The dose reduction potential of the vendor-agnostic DLM was shown to be comparable to that of the vendor-specific DLR at high strength and superior to those of the DLRs at medium and low strengths. Key points: • DRP of the vendor-agnostic model was comparable to that of high-strength vendor-specific model and superior to those of medium- and low-strength models. • Under various radiation dose levels, the deep learning model shows higher detectability indexes compared to FBP.
Deep Learning-Based Reconstruction Improves the Image Quality of Low-Dose CT Colonography

Abstract:Rationale and Objectives To evaluate the image quality of low-dose CT colonography (CTC) using deep learning-based reconstruction (DLR) compared to iterative reconstruction (IR). Materials and Methods Adults included in the study were divided into four groups according to body mass index (BMI). Routine-dose (RD: 120 kVp) CTC images were reconstructed with IR (RD-IR); low-dose (LD: 100 kVp) images were reconstructed with IR (LD-IR) and DLR (LD-DLR). The subjective image quality was rated on a 5-point scale by two radiologists independently. The parameters for objective image quality included noise, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR). The Friedman test was used to compare the image quality among RD-IR, LD-IR and LD-DLR. The Kruskal Wallis test was used to compare the results among different BMI groups. Results A total of 270 volunteers (mean age: 47.94 years ± 11.57; 115 men) were included. The effective dose of low-dose CTC was decreased by approximately 83.18% (5.18 mSv ± 0.86 vs. 0.86 mSv ± 0.05, P < 0.001). The subjective image quality score of LD-DLR was superior to that of LD-IR (3.61 ± 0.56 vs. 2.70 ± 0.51, P < 0.001) and on par with the RD- IR's (3.61 ± 0.56 vs. 3.74 ± 0.52, P = 0.486). LD-DLR exhibited the lowest noise, and the maximum SNR and CNR compared to RD-IR and LD-IR (all P < 0.001). No statistical difference was found in the noise of LD-DLR images between different BMI groups (all P > 0.05). Conclusion Compared to IR, DLR provided low-dose CTC with superior image quality at an average radiation dose of 0.86 mSv, which may be promising in future colorectal cancer screening.

Abstract:Abstract Background To evaluate the performance of a Deep Learning Image Reconstruction (DLIR) algorithm in pediatric head CT for improving image quality and lesion detection with 0.625 mm thin-slice images. Methods Low-dose axial head CT scans of 50 children with 120 kV, 0.8 s rotation and age-dependent 150–220 mA tube current were selected. Images were reconstructed at 5 mm and 0.625 mm slice thickness using Filtered back projection (FBP), Adaptive statistical iterative reconstruction-v at 50% strength (50%ASIR-V) (as reference standard), 100%ASIR-V and DLIR-high (DL-H). The CT attenuation and standard deviation values of the gray and white matters in the basal ganglia were measured. The clarity of sulci/cisterns, boundary between white and gray matters, and overall image quality was subjectively evaluated. The number of lesions in each reconstruction group was counted. Results The 5 mm FBP, 50%ASIR-V, 100%ASIR-V and DL-H images had a subjective score of 2.25 ± 0.44, 3.05 ± 0.23, 2.87 ± 0.39 and 3.64 ± 0.49 in a 5-point scale, respectively with DL-H having the lowest image noise of white matter at 2.00 ± 0.34 HU; For the 0.625 mm images, only DL-H images met the diagnostic requirement. The 0.625 mm DL-H images had similar image noise (3.11 ± 0.58 HU) of the white matter and overall image quality score (3.04 ± 0.33) as the 5 mm 50% ASIR-V images (3.16 ± 0.60 HU and 3.05 ± 0.23). Sixty-five lesions were recognized in 5 mm 50%ASIR-V images and 69 were detected in 0.625 mm DL-H images. Conclusion DL-H improves the head CT image quality for children compared with ASIR-V images. The 0.625 mm DL-H images improve lesion detection and produce similar image noise as the 5 mm 50%ASIR-V images, indicating a potential 85% dose reduction if current image quality and slice thickness are desired.

Abstract:Deep learning CT reconstruction (DLR) has become increasingly popular as a method for improving image quality and reducing radiation exposure. Due to their nonlinear nature, these algorithms result in resolution and noise performance which are object-dependent. Therefore, traditional CT phantoms, which lack realistic tissue morphology, have become inadequate for assessing clinical imaging performance. We propose to utilize 3D-printed PixelPrint phantoms, which exhibit lifelike attenuation profiles, textures, and structures, as a better tool for evaluating DLR performance. In this study, we evaluate a DLR algorithm (Precise Image (PI), Philips Healthcare) using a custom PixelPrint lung phantom and perform head-to-head comparisons between DLR, iterative reconstruction, and filtered back projection (FBP) with scans acquired at a broad range of radiation exposures (CTDIvol: 0.5, 1, 2, 4, 6, 9, 12, 15, 19, and 20 mGy). We compared the performance of each resultant image using noise, peak signal to noise ratio (PSNR), structural similarity index (SSIM), feature-based similarity index (FSIM), information theoretic-based statistic similarity measure (ISSM) and universal image quality index (UIQ). Iterative reconstruction at 9 mGy matches the image quality of FBP at 12 mGy (diagnostic reference level) for all metrics, demonstrating a dose reduction capability of 25%. Meanwhile, DLR matches the image quality of diagnostic reference level FBP images at doses between 4 - 9 mGy, demonstrating dose reduction capabilities between 25% and 67%. This study shows that DLR allows for reduced radiation dose compared to both FBP and iterative reconstruction without compromising image quality. Furthermore, PixelPrint phantoms offer more realistic testing conditions compared to traditional phantoms in the evaluation of novel CT technologies. This, in turn, promotes the translation of new technologies, such as DLR, into clinical practice.

Abstract:Background ECG-gated cardiac CT is now widely used in infants with congenital heart disease (CHD). Deep Learning Image Reconstruction (DLIR) could improve image quality while minimizing the radiation dose. Objectives To define the potential dose reduction using DLIR with an anthropomorphic phantom. Method An anthropomorphic pediatric phantom was scanned with an ECG-gated cardiac CT at four dose levels. Images were reconstructed with an iterative and a deep-learning reconstruction algorithm (ASIR-V and DLIR). Detectability of high-contrast vessels were computed using a mathematical observer. Discrimination between two vessels was assessed by measuring the CT spatial resolution. The potential dose reduction while keeping a similar level of image quality was assessed. Results DLIR-H enhances detectability by 2.4% and discrimination performances by 20.9% in comparison with ASIR-V 50. To maintain a similar level of detection, the dose could be reduced by 64% using high-strength DLIR in comparison with ASIR-V50. Conclusion DLIR offers the potential for a substantial dose reduction while preserving image quality compared to ASIR-V.
