Abstract:Abstract Background Type 2 Diabetes Mellitus (T2DM) has been associated with lower levels of cognitive function in older adults. However, evidence on the potential role of T2DM management in age‐related cognitive function is limited. To explore the prospective association of T2DM and its management with cognitive function among middle‐aged and older Chinese adults. Method This study included 7,230 participants without baseline brain damage, mental retardation, or memory‐related diseases from the China Health and Retirement Longitudinal Study (CHARLS) in 2011‐2012. Fasting plasma glucose and self‐reported information on T2DM diagnosis and treatment was assessed at baseline. Participants were then categorized into normoglycemia, impaired fasting glucose (IFG), and T2DM (including untreated T2DM and treated T2DM) groups. Cognitive function, including episodic memory and executive function, was assessed by the modified Telephone Interview for Cognitive Status (TICS) every two years until 2015. We used the generalized estimating equation (GEE) model to examine the association of baseline T2DM status and cognitive function in succeeding years. Result Compared to those with normoglycemia, participants with T2DM tended to have lower level of overall cognitive function (β = ‐0.23, 95% CI: ‐0.43, ‐0.02) after controlling for demographic variables, lifestyles, follow‐up time, and major clinical factors. In particular, the strong association was mainly observed for those with untreated T2DM (β = ‐0.31, 95% CI: ‐0.54, ‐0.07), especially in the domain of executive function (β = ‐0.22, 95% CI: ‐0.39, ‐0.06) and among those ≥60 years (β = ‐0.58, 95% CI: ‐0.91, ‐0.23). In general, individuals with IFG and treated T2DM had similar levels of cognitive function with normoglycemia participants. Conclusion Our findings supported the detrimental role of T2DM especially untreated T2DM in cognitive function among middle‐aged and older adults. Screening and early treatment for T2DM are warranted for maintaining better cognitive function in later life.

Abstract:Background: The brain and kidney have similar microvascular structure, which makes them susceptible to certain common pathophysiological processes. In this study, we examined several indicators of kidney injury/function associated with cognitive function in older diabetic patients in the hope of finding effective markers for detecting cognitive impairment (CI). Methods: A total of 2209 older participants (aged ≥60 years) from the 2011-2014 National Health and Nutrition Examination Survey (NHANES) were analyzed for the association between diabetes and CI using a multiple linear regression analysis model. Using the same approach, we also analyzed the relationship between indicators of kidney injury/function and cognitive function (Animal Fluency Test, Digit Symbol Substitution Test) in the diabetic population. Results: Diabetes was associated with CI. In age-adjusted model, older diabetics performed significantly poorer on tests of cognitive function compared to normoglycaemic individuals (1.145 points lower on the Animal Fluency Test ( P = 0.005) and 7.868 points reduced on the Digit Symbol Substitution Test ( P < 0.001)). In diabetics, we found elevated serum creatinine (SCr) (especially at SCr≥300uM) was associated with lower scores on cognitive function tests after strict adjustment for potential influences on cognitive function. While, albumin/creatinine ratio (ACR) was only associated with Digit Symbol Substitution score (DSS) not Animal Fluency score (AFS), and estimated glomerular filtration rate (eGFR) was only associated with CI (AFS and DSS) at the end-stage renal disease. Conclusion: SCr, as a sensitive indicator of kidney injury, was significantly associated with CI and can potentially be used as an effective marker for screening CI in older diabetics.

Abstract:Background: Diabetes mellitus type 2 is considered as one of the leading causes of illness and mortality through over the globe. Diabetic retinopathy, neuropathy, and nephropathy are all effectively screened on a regular basis. Recent research have shown that cognitive deterioration can occur in patients with diabetes and that it can go unnoticed for a long time, implying that routine screening is necessary. Methodology: An observational cross sectional study was conducted among 158 patients with complaint of T2DM aged between 60-79 years of age were found with cognitive impairment on the basis of MMSE score in a tertiary care centre. Detailed history along with laboratory and biochemical data were taken from patients after taking written informed consent and approval of Institutional Ethical committee through the pre-structured questionnaire. Results: Mild cognitive impairment was noted in 88 (55.69%) type 2 diabetes mellitus patients and Normal cognitive function in 70 (44.30%). Those with MCI had higher HbA1c (6.57 ± 1.27 vs. 6.13 ± 1.22), higher FBS (148.34 ± 18.61 vs. 145.25 ± 16.31), PPBS (173.91 ± 42.64 vs. 167.47 ± 38.15) and TNF-α (79.32 ± 8.74 vs. 72.98 ± 6.76), which were statistically significant. The cognitive domains of executive function, naming, attention, language, and memory showed a statistically significant difference between those with Mild cognitive impairment and Normal cognitive function. There were no differences in the mean age, duration of disease, and education level between the groups. Conclusion: The significant prevalence of Mild cognitive impairment in type 2 diabetes patients emphasizes the value of routine screening of cognitive functions. Further research into the link between cognitive impairment and poor blood glucose control is needed to see if improving blood glucose control can assist in enhancing cognitive functions.

Abstract:Objectives . To investigate the risk factors for cognitive impairment in Chinese type 2 diabetes mellitus (T2DM) patients of advanced age and to identify effective biomarkers of mild cognitive impairment (MCI) in these patients. Methods . Chinese T2DM patients ( n = 120 ) aged 50–70 years were divided into groups with impaired (mild, moderate, and severe) and normal cognitive function based on Montreal Cognitive Assessment and Mini-Mental State Examination scores. Data regarding demographic characteristics, clinical features of diabetes, biochemical markers, and metabolomics were collected. Results . Age, educational level, duration of diabetes, fasting blood glucose (FBG), HbA1c, total cholesterol (TC), triglyceride (TG), and 24-hour urine protein were significantly associated with cognitive impairment in T2DM patients of advanced age. The severity of fundus retinopathy and the incidence of macrovascular disease also differed significantly among the groups ( P < 0.05 ). Metabolomics analysis suggested that increased levels of glutamate (Glu), phenylalanine (Phe), tyrosine (Tyr), proline (Pro), and homocysteine (Hcy) and a decreased level of glutamine (Gln) were significantly associated with cognitive impairment in the T2DM patients ( P < 0.05 ). Receiver operating characteristic curve analysis demonstrated that Glu, Gln, Phe, and Pro levels were significant predictors of cognitive impairment in the T2DM patients. Conclusions . Age, educational level, duration of diabetes, and the levels of FBG, HbA1c, TC, TG, and 24-hour urine protein were considered as independent risk factors for cognitive impairment in older T2DM patients. Macrovascular and microvascular diseases also were closely associated with cognitive impairment in these patients. Together, Glu and Gln levels may represent a good predictive biomarker for the early diagnosis of cognitive impairment in T2DM patients.

Abstract:Background Chronic subclinical inflammation (inflammaging) and changes in the predominance of type, destruction and endocrine function of adipose tissue are related in aging. Both contribute to the pathogenesis of chronic noncommunicable diseases, such as diabetes and cognitive decline. Considering that age-related cognitive decline is characteristic of physiological aging, and that Type 2 Diabetes Mellitus (DM2) can accentuate the decline in cognitive function and is a risk factor for the development of dementia, it is of interest to study the relationships of cognitive function and anthropometric markers and analyze its difference in older adults with and without diabetes. Objectives To investigate differences in cognitive function and anthropometric indices of older adults with and without DM2. Methods Sixty-four older adults participated (women = 62), including 20 participants with diabetes (69.32 ± 4.48 years old, 8.3 ± 4.0 years of schooling) and 44 participants without diabetes (67.91 ± 5.40 years of age, 9.0 ± 4.5 years of schooling). The groups were matched by age, education, and physical activity. All participants underwent cognitive (Mini-Mental State Examination - MMSE) and anthropometric assessment, including Body Mass Index (BMI), Waist Circumference (WC), Hip Circumference (HC), Waist-Height Ratio (WHR), Waist-Hip Ratio (WHR), Body Adiposity Index (IAC) and Conicity Index (C Index). Based on the analysis of normality (Shapiro-Wilk) the Student's t test and the Mann-Whitney U test were performed for non-parametric variables. The significance level was set at p<0.05. Results All participants had normal cognitive performance, considering the cut-off point adjusted for education. Despite the cognitive performance within the normal range, the older adults with DM2 showed lower cognitive performance (26.92 ± 2.26 points) in the MMSE assessment when compared to participants without diabetes (28.09 ± 1.56 points; p < 0.03). No significant differences were found between participants with and without diabetes, respectively, in: BMI (30.09 ± 5.41; 28.46 ± 4.97; p < 0.722); WC (99.62 ± 12.43 cm; 94.56 ± 11.54 cm; p < 0.560); HC (103.83 ± 11.56; 100.16 ± 13.03; p < 0.252); WHR (0.66 ± 0.10; 0.63 ± 0.07; p < 0.078); WHR (0.96 ± 0.05; 0.95 ± 0.72; p < 0.412); IAC (37.51 ± 8.68; 36.52 ± 7.48; p < 0.426); C index (1.35 ± 0.09; 1.33 ± 0.11; p < 0.663). Conclusion Cognitively healthy older adults with DM2 showed lower cognitive performance compared to participants without DM2, even without differences in anthropometric markers. Implications To recognize the influence of DM2 in accelerating age-related cognitive decline is important for the inclusion of preventive cognitive stimulation strategies for the healthier aging of older adults with diabetes

Abstract:OBJECTIVE: The incidence of cognitive impairment is increasing with age; however, little is known about the role of hyperglycemia in cognitive impairment. This study focuses on investigating the relationship between diabetes mellitus type 2 and cognitive impairment.METHODS: 60 diabetic patients, amongst whom, 30 had a well-controlled diabetes status and the other 30 had not. These patients were compared to 60 non-diabetic controls whose age, sex and educational class matched with the individuals of the first group. Patients with important risk factors for cognitive disorders (renal failure, major depressive disorders and psychoactive drug users, cerebrovascular accident history, etc.) were not included in the study. Modified Mini Mental Status examination (mMMSE) was done for all patients by a blinded expert examiner.RESULTS: Subjects with diabetes (n=60) had lower MMSE score than those without diabetes (P<.01). Diabetes was also associated with increased odds of cognitive decline as determined by MMSE scores (odds ratio=1.9; CI=95%, 1.01-3.6). A significant correlation between duration of disease and cognitive dysfunction was observed, P=0.001. Also, the same correlation was found for quality of diabetes control, P=0.002.CONCLUSION: Diabetes mellitus is associated with lower levels of cognitive function.

Abstract:AIM: Patients with diabetes have shown lower performance in tests of cognitive function especially those testing memory, executive functions, and psychomotor efficiency. They also have an elevated risk of both vascular dementia and Alzheimer's disease. Cognitive impairment may have consequence on treatment compliance.METHODS AND RESULTS: This article provides indication for holding an interview, and reports a few screening bedside tests to detect a cognitive impairment. Some neuropsychological tests useful for characterizing the cognitive profile of a patient are described, as well as the main cognitive profiles expected in patients with diabetes and cognitive decline.CONCLUSION: A systematic assessment of cognition with a rapid interview and screening tests in patients with diabetes, especially the oldest, with a long history of diabetes, co-morbidities, or with unexplained poor metabolic control would be a good clinical practice. Patients with cognitive decline may be referred to memory clinics for identifying the cause of the decline and contribute to provide appropriate medical and medicosocial management.

Abstract:Objective:To study the cognitive function of middle-age and old patients with type 2 diabetes mellitus(T2DM) and related factors. Methods:Questionnaires were used to investigate 155 patients with T2DM to study the cognitive function and the possible infected factors of memory and non-memory. Results:Cognitive function of the patients with T2DM was not only associated with age(r = -0.366),sex(r = -0.374),educational level(r = 0.574) and incomes(r = 0.268),but also with course of diabetes disease(r = -0.247),the levels of blood glucose(r = -0.269),HBA1C(r = -0.235) and systolic pressure(r = -0.280). The patients with T2DM combined hypertension displayed significant difference to the controls in calculating(P = 0.006) and repeating sentence(P = 0.006) in MMSE,the right numbers of point pictures(P = 0.001) and the expended time of point pictures(P = 0.016). Conclusion:The impairment of cognitive function of middle-age and old patients with T2DM was concerned with age,sex,educational level,incomes,and the levels of blood glucose,combined hypertension and the level of hypertension. Control the level of blood glucose and hypertension effectively should prevent and mitigate the deterioration of cognition of T2DM.
An Association of Cognitive Impairment with Diabetes and Retinopathy in End Stage Renal Disease Patients under Peritoneal Dialysis

Abstract:BackgroundDiabetes and retinopathy have been considered as risk factors of cognitive impairment (CI) in previous studies. We investigated both of these two factors and their relationship with global and specific cognitive functions in end stage renal disease patients under peritoneal dialysis (PD).MethodsIn this multicenter cross-sectional study, 424 clinically stable patients were enrolled from 5 PD units, who performed PD for at least three months and completed fundoscopy examination if they had diabetes. Global cognitive function was measured using the Modified Mini-Mental State Examination (3MS), Trail-Making Test forms A and B for executive function, and subtests of the Battery for the Assessment of Neuropsychological Status for immediate and delayed memory, visuospatial skills, and language ability.ResultsPD Patients with DM and Retinopathy had significantly higher prevalence of CI, executive dysfunction, impaired immediate memory and visuospatial skill, compared with patients in non-DM group. By multivariate logistic regression analyses, DM and retinopathy rather than DM only were significantly associated with increased risk for CI, executive dysfunction, impaired immediate memory and visuospatial skill, odds ratios(ORs) and 95% confidence intervals were 2.09[1.11,3.92], 2.89[1.55,5.37], 2.16 [1.15,4.06] and 2.37[1.32,4.22], respectively (all P < 0.05).ConclusionsDiabetic PD patients with retinopathy were at two times risk for overall cognitive impairment, executive dysfunction, impaired immediate memory and visuospatial skill as compared to non-diabetic PD patients.

Abstract:Purpose of review: To review the connection between type 2 diabetes and cognitive dysfunction, including its epidemiology, potential mechanisms of pathophysiology, risk factors, possible prevention, and treatment considerations. Recent findings: Diabetes is a risk factor for mild cognitive decline, in addition to Alzheimer's disease and vascular dementia. Duration of diabetes, concomitant vascular or associated co-morbidities, hyper- and hypoglycemia may lead to worsening cognitive dysfunction. Unfortunately, there is a lack of evidence-based guidance on the prevention of cognitive dysfunction in the diabetes population. Studies of diabetes medications, including metformin, glucagon-like peptide-1 (GLP-1) receptor agonists, and sodium-glucose cotransporter-2 inhibitors (SGLT2) have shown some benefit with cardiovascular morbidity and may affect cognition. In the absence of clearly defined preventive tools, diabetes practice guidelines recommend annual cognitive screening as standard of care in adults with diabetes aged 65 years or older. Summary: People living with diabetes are at risk for significant decline in cognitive function. Epidemiology and risk factors are well defined. Prevention and treatment strategies are limited and require further study.
Association between cognitive dysfunction and diabetes in patients over 65 years old: a cross-sectional study using propensity score matching

Abstract:Objectives: To investigate the association between diabetes and cognitive dysfunction in the elderly population, and examine the impact of cognitive dysfunction on level of activities of daily living (ADL) in patients with diabetes. Methods: Data analysis was conducted on 2,951 individuals aged over 65 years from the Chinese Longitudinal Healthy Longevity Survey cohort. Propensity score matching was utilized to mitigate selection bias. Multivariate binary logistic regression was performed to analyse the association between diabetes and cognitive dysfunction in the study subjects. In addition, the relationship between ADL and cognitive function in patients with diabetes was analysed using the Wilcoxon rank-sum test. Results: A significant association (p = 0.017) was found between diabetes and the occurrence of cognitive dysfunction in older adults. Subgroup analyses revealed that diabetes patients with cognitive dysfunction exhibited a worse ADL dependence compared with those without cognitive dysfunction (p < 0.001). Conclusion: These findings indicate that diabetes is associated with cognitive dysfunction in older adults. Meanwhile, there is an association between cognitive impairment and ADL level in subjects with diabetes. As such, healthcare professionals should pay close attention to the occurrence of cognitive dysfunction and ADL decline during diagnosis and treatment, and proactive prevention and intervention strategies should be implemented.

Abstract:Objective The aim of this study was to investigate risk factors for cognitive impairment (CI) and explore the relationship between obesity and cognition in hospitalised middle-aged patients with type 2 diabetes (T2DM). Methods Subjects were divided into normal cognitive function (NCF) (n=320) and CI (n=204) groups based on the results of the Montreal Cognitive Assessment (MoCA) and Mini-Mental State Examination (MMSE). The risk factors for CI were determined by logistic regression analysis and generalised linear modelling. The associations between obesity parameters (body mass index (BMI) and waist circumference (WC)) and cognitive ability were studied with the use of linear regression analysis, piecewise regression modelling and interaction analysis. The receiver operating characteristic curve analysis was used to examine the diagnostic value of influencing factors for cc Results The prevalence of CI was 38.9% in hospitalised middle-aged T2DM patients (median age, 58 years). Age, WC, hypoglycaemic episode within past 3 months and cerebrovascular disease (CVD) were identified as independent risk factors for CI, while the independent protective factors were education, diabetic dietary pattern, overweight and obesity. BMI was a protective factor for the MoCA score within a certain range, whereas WC was a risk factor for the MMSE and MoCA scores. The area under the curve for the combination of BMI and WC was 0.754 (p<0.001). Conclusion Age, education, diabetic dietary pattern, WC, overweight, obesity, hypoglycaemic episode in 3 months and CVD may be potential influencing factors for the occurrence of CI in hospitalised middle-aged population with T2DM. The combination of BMI and WC may represent a good predictor for early screening of CI in this population. Nevertheless, more relevant prospective studies are still needed.

Methods: This cross-sectional study included 240 patients with T2DM. A questionnaire was used to collect demographic and disease-related data on patients, and the Self-rating Depression Scale (SDS), Diabetes Self-care Scale (DSCS), and Social Support Rating Scale (SSRS) were used to assess patients' depression status, self-management behavior, and social support, respectively. The Chinese version of the Montreal Cognitive Assessment (MoCA) was used to evaluate cognitive function, with a score < 26 set as the threshold for cognitive impairment. Results: The prevalence of cognitive dysfunction in middle-aged and elderly patients with T2DM was 52.5%. Multivariate logistic regression analysis showed that older age, a history of hypoglycemia within 1 month, and depression were independent risk factors for cognitive impairment. Education for > 12 years, urban living, and a higher total score on the DSCS were independent protective factors against cognitive impairment. Conclusion: T2DM patients with high risk of cognitive impairment can be identified early from the bio-psycho-social perspective. Patients with T2DM who are older, less educated, living in rural areas, have hypoglycemia history, and have poor self-management of diabetes are at increased risk of cognitive impairment. Closer monitoring of patients with hypoglycemia, early detection of depression, and improving patients' self-management capacity can prevent cognitive impairment in middle-aged and elderly patients with T2DM. It is estimated that about 9.3% (463 million) of the adult population worldwide suffers from diabetes, with T2DM accounting for 90% of cases. 1 T2DM is often associated with organ damage and dysfunction including cardiovascular disease, blindness, nerve damage, and renal failure that can lead to disability or death. 2 Recent studies have shown that T2DM increases the risk of cognitive impairment in middle-aged and elderly people. 3,4 About 10–20% of patients with T2DM experience mild cognitive impairment (MCI). 5,6 T2DM also accelerates the progression of MCI to dementia, 7,8 which can diminish the quality of life and prognosis of patients. Cognitive impairment is one of the serious complications of T2DM, but it is often easy to be ignored. There are no standardized recommendations on the prevention and treatment of T2DM cognitive impairment. In addition, the impairment of cognitive function is difficult to reverse. 9 Therefore, the early prevention of cognitive impairment is particularly important. Taking measures to delay the progress of cognitive impairment in MCI stage is the key measure to prevent its transformation to dementia. The mechanism of type 2 diabetes affecting cognitive function is not yet clear. However, many studies have reported different risk factors, including long duration of diabetes, obesity, 10 physical inactivity, age, 11 depression, 12 lack of social support, 13,14 unstable blood glucose level, 15,16 elevated level of inflammatory markers, 17 and other diagnostic markers. 18 Some studies have reported the protective effects of being married and living in the city 19 on cognitive function in type 2 diabetic patients. Relationship Between Self-Care Behavior and Cognitive Function in Hospitalized Adult Patients with Type 2 Diabetes: A Cross-Sectional Study

Abstract:Purpose: To investigate the relationship between diabetes self-care behavior and cognitive function of hospitalized young and middle-aged Chinese patients with type 2 diabetes mellitus (T2DM). Patients and Methods: In this cross-sectional study, young and middle-aged T2DM patients (age range, 35-65 years) were recruited at 4 tertiary hospitals between July 2016 and January 2017. Data pertaining to self-care behavior and cognitive function were collected using two questionnaires (the Summary of Diabetes Self-care Activities [SDSCA] and the Montreal Cognitive Assessment [MoCA], respectively). Multivariate linear regression analysis was performed to assess the correlation between cognitive function and self-care activities. Results: A total of 140 patients with diabetes were enrolled (mean age, 53.79 +/- 7.96 years). The mean duration of T2DM was 10.83 +/- 6.76 years. Regarding SDSCA performance, the mean scores for foot care and blood glucose monitoring were 2.20 +/- 2.57 and 1.98 +/- 2.45, respectively, which were the worst; scores for exercise (4.01 +/- 2.58) and diet (3.16 +/- 1.89) were better, while scores for medication administration (5.26 +/- 2.79) were the best. The prevalence of cognitive impairment was 37.9% (53 patients). After variables adjustment, delayed recall showed a significant correlation with blood sugar monitoring behavior (B =0.224, P=0.019); visual space and executive function (B=0.255, P=0.009) and abstraction (B=-0.337, P=0.001) showed a correlation with foot care behavior. Conclusion: Cognitive ability affects the self-care behavior of patients with T2DM. Assessment of cognitive function may help inform patient education interventions to improve the self-care behavior of these patients.

Abstract:The present review integrates findings of published studies that have evaluated the cognitive function of treated and untreated type 2 diabetic patients and provides a detailed overview of the neuropsychological assessments conducted. Cognitive deficits are observed in older people with glucose intolerance or untreated diabetes but these deficits appear to be attenuated by treatments that improve glycemic control. Cognitive decrements in treated type 2 diabetic patients are most consistently observed on measures of verbal memory (35% of the measures) and processing speed (45% of the measures) while preserved function is observed on measures of visuospatial, attention, semantic and language function. Some studies suggest that deficits in cognitive functions are associated with poorer glycemic control. A number of other factors, such as depression, cardiovascular and cerebrovascular disease, increase these deficits. We conclude that, in diabetic patients who achieve and maintain good glycemic control, type 2 diabetes only has a small impact on cognitive functions before the age of 70 years. However, early onset of type 2 diabetes, poor glycemic control and the presence of micro- and macrovascular disease may interact to produce early cognitive deficits. In older adults (70 years and over), diabetes likely interacts with other dementing processes such as vascular disease and Alzheimer's disease to hasten cognitive decline.

Abstract:Diabetes mellitus (DM) and chronic kidney disease (CKD) are common in middle aged and older adult individuals. DM may accelerate the aging process, and the age-related declines in the estimated glomerular filtration rate (eGFR) can pose a challenge to diagnosing diabetic kidney disease (DKD) using standard diagnostic criteria especially with the absence of severe albuminuria among older adults. In the presence of CKD and DM, older adult patients may need multidisciplinary care due to susceptibility to various health issues, e.g., cognitive decline, auditory or visual impairment, various comorbidities, complex medical regimens, and increased sensitivity to medication adverse effects. As a result, it can be challenging to apply recent therapeutic advancements for the general population to older adults. We review the evidence that the benefits from these newer therapies apply equally to older and younger patients with CKD and diabetes type 2 and propose a comprehensive management. This framework will address nonpharmacological measures and pharmacological management with renin angiotensin system inhibitors (RASi), sodium glucose co-transporter 2 inhibitors (SGLT2i), non-steroidal mineralocorticoids receptor antagonists (MRAs), and glucagon like peptide 1 receptor agonists (GLP1-RAs).

Abstract:Background: Early-stage chronic kidney disease has been suggested to be correlated with cognitive decline, but the association has rarely been explored in the oldest old. Subjects and methods: This prospective study included 284 Chinese participants aged 80 years or older with serum creatinine levels,150 mu mol/L. The median follow-up time was 3.3 years, and 247 (87.0%) participants provided valid data at their last visit. Kidney function was evaluated by measuring the estimated glomerular filtration rate (eGFR) at baseline, and cognitive function was evaluated using the Mini-Mental State Examination (MMSE) at both baseline and annual visits. A reliable decrease in the MMSE score over the follow-up period was observed based on a Reliable Change Index of 1.645 (equivalent to a 90% confidence interval [CI]), which was used to define cognitive decline. Poisson regression models were built to analyze the association between baseline kidney function and cognitive decline. Results: A total of 18 (7.3%) cases of incident cognitive decline were observed during the follow-up period. After adjusting for potential confounders, the relative risk of developing cognitive decline was 4.03 (95% CI 1.09-13.81) among participants with an eGFR of 30-59 mL/min/1.73 m(2) compared to participants with an eGFR of >= 60 mL/min/1.73 m(2). Conclusion: Early-stage chronic kidney disease was correlated with cognitive decline in an oldest-old Chinese population.

Tasks related to diabetes self-management involve multiple cognitive skills and processes, such as memory, attention, planning, and calculating. Impaired cognitive function can threaten the patient's ability to perform self-monitoring. The objectives of the study were: to assess cognitive deficits and the level of self-care in elderly patients with diabetes, to identify correlations between cognitive deficits and self-care, and to determine which variables influence self-care behaviors and cognitive deficits. Methods: The study involved 169 patients with type 2 DM. Standardized tools were used: Mini-mental State Examination (MMSE) to assess cognitive function and the Self-Care of Diabetes Inventory (SCODI) to assess the level of self-care. Socioclinical data were taken from the hospital records. Results: 56.8% of patients had cognitive impairment (MMSE≤ 26). In the comparative analysis, patients with cognitive impairment had significantly lower results in all domains of the SCODI: self-care maintenance (72.9 vs 75), self-care monitoring (53.1 vs 56.3), self-care management (71.9 vs 84.4), self-care confidence (79.5 vs 86.4). Correlation analysis showed that the MMSE score correlates significantly and positively (p˂0.05; r˃0) with all SCODI subscales, and the higher the MMSE score the higher the level of self-care (A: r=0.252, B: r=0.244, C: r=0.019, D: r=0.28). Conclusion: In this elderly type 2 diabetes population, and using only one test to verify the cognitive function, self-care management was worse in terms of self-care management (blood glucose control). Cognitive function components are independent determinants of self-care in patients with type 2 diabetes. Recall is an independent predictor of self-care maintenance, and writing a predictor of self-care monitoring. Keywords: adherence, diabetes, cognitive impairment, self-care
