Abstract:The paper undertakes a textual and documentary analysis of the Advanced Therapy Medicinal Products Regulation, which was passed into law in the European Union (EU) in 2007. This law is significant for the development of regenerative medicine in the EU and globally. Regulatory texts such as this one provide significant material for addressing key concerns in recent theorising about innovative technologies in socio-legal studies, innovation studies, and science and technology studies. These concerns include expectations about future technologies and economic sector-building. By revisiting philosopher J.L. Austin’s well-known work on How to Do Things with Words, this paper deploys his concepts of performative utterances to inform its analysis. Pursuing Austin’s and later commentators’ analysis of performatives in language use, and drawing on Lindsay Prior’s application of actor-network theory to documents, the analysis shows two different types of performativity at work in and through the document. These are termed ‘generative’ and ‘enactive’ performativity. The ‘enactive’ type includes ‘legislative’ and ‘social’ forms. In addition, a more conventional content analysis reveals a range of actions, both legislative and discursive, in the regulatory document. The analysis shows a tension between standardisation and imprecision in the conceptual detailing of the document. Legislative texts produced through established politico-legal conventions are a special class of document that should be accorded a more prominent place in understanding the role of political governance processes in shaping emergent technoscientific fields and sectors.

Donor-derived Hypouricemia in Irrelevant Recipients Caused by Kidney Transplantation.

Abstract:Background: Hereditary renal hypouricemia (HRH) is a genetically heterogenetic disease. Patients with HRH are almost asymptomatic; but some may experience exercise-induced acute kidney injury ( EAKI) and nephrolithiasis which may bring concerns regarding the risk-benefit ratio as marginal kidney donors. This study examined the pathogenic mutations of hypouricemia in two recipients after receiving kidney transplantation, providing preliminary evidence for the mechanism of hypouricemia. Methods: Two participants underwent detailed biochemical examinations. DNA and RNA were extracted from transplant specimens for sequencing. The whole-genome sequencing and polymerase chain reaction (PCR) amplification were performed to confirm the pathogenic genes. Functional effects of mutant proteins were verified by bioinformatics analysis. RNA-sequencing (RNA-seq) was used to study the transcriptome of hypouricemia. Results: Both of the recipients had the low serum uric acid (UA) (45-65 mu mol/l), high fraction excretion of UA (44% and 75%) and an increase in the UA clearance (35.9 and 73.3 mL/min) with a functioning graft. The sequencing analyses revealed 7 kinds of potential mutational genes in this case, two novel mutations p.R89H and p.L181V in SLC22A12 gene which were revealed by bioinformatics could be pathogenic in nature. Conclusions: Two novel mutations of SLC22A12 were identified. Preliminary functional analysis revealed a potential deleterious effect of these mutations in the grafts derived from the donor and sequencing analysis expand the molecular mechanisms of renal hypouricemia.
Sister chromatid exchange (SCE) analysis in 106 control individuals.

Abstract:This study was aimed at obtaining baseline data on the degree of variability of sister chromatid exchange (SCE) values in 106 individuals. There were highly significant differences between individuals and between replicate serum cultures within individuals in SCE frequencies. Females had significantly higher SCE frequencies than males, and cigarette smoking significantly increased such values after correction for sex. On the basis of this study, the following recommendations are proposed for future studies monitoring SCE frequencies in control and exposed populations: 1) subjects should be matched by sex and smoking status; 2) 50 cells per individual with 25 from 2 replicate cultures is a suitable sample size; 3) either the original data or the means should be transformed to logarithms for analysis; 4) the dispersion test should be carried out and the H statistic (variance/mean) should be reported; 5) differences between control and exposed groups can be analyzed either by analysis of variance or nonparametric methods; 6) the sample size should be based on considerations of the significance level required and interindividual variability; 7) concurrent controls are advisable when using statistics based on background incidences; and 8) quality control methods should be applied to routine collection of background control data.

Abstract:Background: The relationship between chronic hepatitis B (CHB) infection and natural killer (NK) cell dysfunction is well-established, but the specific role of HBV viral antigens in driving NK cell impairment in patients with CHB remains unclear. This study investigates the modulatory effects of hepatitis B virus subviral particles (HBVsvp, a representative model for HBsAg) on the phenotypic regulation (activating and inhibitory receptors), cytokine production and cytotoxic potential of peripheral blood mononuclear cell-derived natural killer cells (PBMCs-derived NK cell), which contributes to NK cell dysfunction in CHB infection, potentially serving as an effective HBV immune evasion strategy by the virus. Methods: NK cells were isolated from peripheral blood of patients with CHB (n=5) and healthy individuals (n=5), stimulated with HBVsvp. Subsequent flow cytometric characterization involved assessing changes in activating (NKp46 and NKG2D) and inhibitory (CD94) receptors expression, quantifying TNF-α and IFN- γ cytokine secretion, and evaluating the cytotoxic response against HepG2.2.15 cells with subsequent HBVsvp quantification. Results: In CHB patients, in vitro exposure of PBMCs-derived NK cell with HBVsvp (represent HBsAg model) significantly reduced NK cell-activating receptors expression (P = 0.022), increased expression of CD94 + NK cells (p = 0.029), accompanied with a reduced TNF-α - IFN-γ cytokine levels, and impaired cytotoxic capacity (evidenced by increased cell proliferation and elevated HBVsvp levels in co-cultures with HepG2.2.15 cells in a time-dependent), relative to healthy donors. Conclusion: These findings suggest that HBVsvp may induce dysfunctional NK cell responses characterized by phenotypic imbalance with subsequent reduction in cytokine and cytotoxic levels, indicating HBVsvp immunosuppressive effect that compromises antiviral defense in CHB patients. These data enhance our understanding of NK cell interactions with HBsAg and highlight the potential for targeting CD94 inhibitory receptors to restore NK cell function as an immunotherapeutic approach. Further clinical research is needed to validate these observations and establish their utility as reliable biomarkers.

Abstract:Ubiquitin‐specific protease 53 (USP53) is essential for formation of cellular tight junctions and variations in this gene disrupt the tight junctions, resulting in cholestasis. We describe the clinical manifestations and outcomes of patients with USP53 mutations from the Indian progressive familial intrahepatic cholestasis registry. All 29 patients who harbored mutations in the USP53 gene either in the homozygous, compound heterozygous, or heterozygous state and presented with cholestasis were included. USP53 variants related to cholestasis had good outcomes, with native liver survival in 82.7%, whereas 17.3% required liver transplantation. Jaundice developed in 93% and within 3 months of age in 48.8%. Jaundice resolved in 21 (72.4%). Pruritus 76% at a median age of 7 months (severe in 10/22, 45% and refractory to medical therapy in 4, 18.1%). Majority of them (82.7%) had biallelic mutations. Protein‐truncating mutations were present in 20 (69%) and missense mutations in 9 (31%). No correlation was found between the genotype and the outcome.

Elucidation of the chemical nature of adsorbed species for Pt(111) in H2SO4 solutions by thermodynamic analysis.

Abstract:The nature of the adsorbed species for Pt(111) in sulfuric acid solutions has been elucidated by a careful thermodynamic analysis of the effect of pH on charge density data. This analysis takes advantage of the fact that, for solutions of constant total sulfate + bisulfate concentration, an increase of pH would increase the sulfate concentration, at the expense of decreasing the bisulfate concentration. As a result, sulfate adsorption would be shifted toward lower potentials, while bisulfate adsorption would follow the opposite trend. In the present work, coulostatic data for Pt(111) in (0.2 - x) M Me(2)SO(4) + x M H(2)SO(4) (Me: Li, Na; x: 10(-4) - 0.2) and (0.1 - x) M KClO(4) + x M HClO(4) + 10(-3) M K(2)SO(4) (x: 10(-4) - 0.1) solutions are carefully analyzed. It is concluded that sulfate rather than bisulfate adsorption takes place at potentials higher than the potential of zero charge. This result agrees with the fact that similar FTIRRAS bands for adsorbed sulfate species are observed for pH 0.8-3.5 in (0.2 - x) M K(2)SO(4) + x M H(2)SO(4) solutions.

Abstract:Hereditary cholestatic liver disease caused by a class of autosomal gene mutations results in jaundice, which involves the abnormality of the synthesis, secretion, and other disorders of bile acids metabolism. Due to the existence of a variety of gene mutations, the clinical manifestations of children are also diverse. There is no unified standard for diagnosis and single detection method, which seriously hinders the development of clinical treatment. Therefore, the mutated genes of hereditary intrahepatic cholestasis were systematically described in this review.

Abstract:Hepatitis C virus (HCV) infection is a major cause of chronic liver disease, leading to liver steatosis, fibrosis, and hepatocellular carcinoma (HCC). Despite the accumulation of clinical data showing the impact of amino acid substitutions at positions 70 (R70Q/H) and/or 91 (L91M) in the HCV core protein in progressive liver diseases, including HCC, the underlying mechanisms have not been elucidated. We analyzed 72 liver biopsy specimens from patients with chronic HCV genotype 1b (HCV-1b) infection prior to antiviral treatment. Levels of 8-hydroxy-2'-deoxyguanosine (8-OHdG) and nuclear factor erythroid 2-related factor 2 (NRF2) in the nucleus were quantified using liver tissue immunohistochemistry. The effects of amino acid substitutions in the HCV core region on hepatocellular oxidative stress were investigated using wild-type or double-mutant (R70Q/H+L91M) HCV-1b core transfection and stable expression in human hepatoma HuH-7 cells. Overall, 24, 19, 11, and 18 patients had the wild-type, R70Q/H, L91M, and R70Q/H+L91M genotypes, respectively, in the HCV core. A significantly higher accumulation of hepatocellular 8-OHdG and a lower NRF2/8-OHdG ratio were observed in patients with R70Q/H+L91M than in those with the wild-type disease. Increased levels of intracellular superoxide and hydrogen peroxide in the cytoplasm and mitochondria, mRNA expression of enzymes generating oxidative stress, and nuclear expression of nicotinamide adenine dinucleotide phosphate oxidase 4 were augmented in cells treated with R70Q+L91M. HCV core proteins harboring either or both substitutions of R70Q/H or L91M enhanced hepatocellular oxidative stress in vivo and in vitro. These amino acid substitutions may affect HCC development by enhancing hepatic oxidative stress in patients with chronic HCV-1b infection.

Abstract:Bartonella henselae is a zoonotic pathogen whose clinical presentation depends on the patient's immune status. While immunocompetent hosts often present with cat scratch disease, immunocompromised hosts including solid organ transplant recipients can present with other infectious syndromes. Bartonellosis is a rare infection in liver transplant recipients. We present a case of Bartonella hepatitis and sepsis in a female liver transplant recipient with prior contact with cats.

Abstract:Objective To explore the hepatic pathology of a patient with erythropoietic protoporphyria ( EPP ). Methods Percutaneous liver biopsy was performed with a Chiba needle in a 31year-old man suffering from EPP. The sample was fixed in 10% formalin solution, and the paraffin-embedded section was stained with H-E, PAS, etc. Unstained paraffin-errbedded and H-E stained paraffin embedded sections were examined under polarization microscope. Ultrathin sections were examined in a transmission electron microscope. Results In H-E stained sections, deposits of dark reddish brown pigment were seen in the hepatocytes, Kupffer cells, portal macrophages and plugs in the lumen of bile canaliculi and ducts. Under light microscope, such deposits, with rare exception, exhigoited strikin g birefringence with tie unique shape of "Maltese cross". Non-membrane limited compact masses of crystals were straight or slightly curved and their dimensions (40-640 nm in length and 6-22 nm in width ) were different under tie transmission electron microscope. Conclusion Microscolpy, especially polarization microscopy, provides a highly sensitive and specific technique for the diagnosis of EPP.
Differences in the patterns and outcomes of enhanced viral replication between hepatitis C virus and hepatitis B virus in patients with hepatocellular carcinoma during transarterial chemolipiodolization

Abstract:BACKGROUND/AIMS: Enhanced replication of hepatitis C virus (HCV) is well described in the setting of moderate to severe immunosuppression. The aims of this retrospective study were to determine the incidence of enhanced HCV replication in hepatocellular carcinoma (HCC) patients undergoing transarterial chemolipiodolization (TACL) and to identify the factors associated with enhanced replication of HCV. The clinical pattern of enhanced HCV replication was compared with hepatitis B virus (HBV) reactivation during TACL.METHODS: This study enrolled 49 anti-HCV-seropositive patients who were diagnosed with HCC between January 2005 and December 2010 and who underwent TACL using epirubicin and/or cisplatin with consecutive HCV RNA copies checked. For comparison, 46 hepatitis B surface antigen(1)-positive patients with HCC who were treated with TACL were also enrolled. The frequency, associated factors, and clinical outcomes of enhanced HCV replication were analyzed and compared with those of HBV reactivation during TACL.RESULTS: Enhanced replication of HCV occurred in 13 (26.5%) of the 49 anti-HCV-seropositive patients during TACL. Of these 13 patients, 4 developed hepatitis, but none of the subjects developed decompensation due to the hepatitis. No significant clinical factors for enhanced HCV replication during TACL were found. Compared with HBV reactivation, the frequency of hepatitis attributed to enhanced HCV replication was significantly lower than that for HBV reactivation (8.2% vs. 23.9%, P=0.036).CONCLUSIONS: TACL can enhance HCV replication; however, the likelihood of hepatitis and decompensation stemming from enhanced HCV replication was lower than that for HBV reactivation in patients undergoing TACL.
Functional Comparison of Two Full-length Genomes of Hepatitis B Virus Strains Isolated from Two Chronic Hepatitis B Patients

Abstract:For studying the characterisitcs of gemomic structures, replication and antigen expression of hepatitis B virus (HBV) strains, the full-length HBV genomes from sera of two chronic hepatitis B patients with high level of serum HBV DNA (10 14 and 10 13 copies/ml respectively) were separately cloned. The full-length genomes of the two HBV strains were sequenced and used to transfect HepG2 cells separately. The HBsAg expressed in the supernatant of transfected HepG2 cells was similar between the two strains, but the level of HBeAg of strain #56 was three times higher than that of strain #2-18 Replication efficiency of HBV strain #56 was significantly higher than that of strain #2-18 detected by Southern blot and hybridization anlaysis of intracelluar viral particles and extracellular replicative intermediates. Both genomes were 3215 nucleotides in length, belonged to genotype B (adw subtype), and were of 98.7% homology. The 42 diverse nucleotides distributed in the C, Pre-S1, Pre-S2, P and X genes and in Sp I, SpII, Xp and ENH I regulatory regions. Since HBV strain #56 had a point mutation at Pre-S2 initial codon (from ATG to GTG), the major protein translated from Pre-S1 contained partial amino acids of Pre-S2, therefore serum sample #56 and the supernatant of transfected HepG2 cells reacted positively with the Pre-S2 antibodies.

Abstract:OBJECTIVE:To investigate the evolution of HCV quasispecies in persistent and self-limited infection patients.METHODS:Peripheral blood HCV preserved for 10 years from 8 naive patients were analyzed. HCV core and E2 genes were cloned and sequenced. Genetic distances of different isolates at different time were analyzed by phylip software.RESULTS:The genetic distances of HCV quasispecies were higher in persistent infection patients than that of self-limited patients. In each patient the genetic distance of HCV E2 region isolates were higher than that of HCV C region which has some relationship between each other. In the 4 persistent patients there exist no differences between within-group and between-group genetic distance in HCV C region and also exist no differences in HCV E2 region in 2 of the 4 persistent patients.CONCLUSIONS:The diversity degree of HCV quasispecies may affect the outcome of hepatitis C. Research on HCV C and E2 region to study HCV quasispecies may obtain different results. The distribution of HCV quasispecies may be stable in hepatitis C natural history.

Abstract:Cholangitis lenta (CL) is a rare histopathological diagnosis characterized by dilated bile ductules containing inspissated bile, often associated with high bilirubin levels. Most cases have been observed in patients with clinical evidence of sepsis or a history of liver transplantation. Giant cell hepatitis (GCH) almost exclusively presents in children and is characterized by inflammation and large multinucleated hepatocytes in the hepatic parenchyma. Both conditions are diagnosed via liver biopsy. We present a case of an elderly man with decompensated cirrhosis secondary to autoimmune hepatitis (AIH) who was found to have biopsy-proven CL and GCH.

Expression of NKp46 and NKG2A in Peripheral Blood NK Cells of Patients with Chronic Hepatitis B

Abstract:Objective To determine the frequency of NK cells and expression of their receptors,including NKG2A and NKp46,in peripheral blood of patients with chronic hepatitis B.Methods Peripheral blood mononuclear cells(PBMC)were separated from patients and healthy controls.Frequency of NK cells and expression of their receptors were detected by flow cytometry.Results The number of NK cells in peripheral blood of chronic hepatitis B patients was less at immunoclearence phase(10.91%±3.93%,n=21)than at immunotolerance phase(15.65%±5.58%,n=15,P=0.024)and than that of healthy controls(14.23%±7.37%,n=25,P=0.011).In contrast,the expression level of NKp46 was significantly higher in patients at immunoclearence phase(45.45%±14.00%,n=19)than at immunotolerant phase(23.35%±6.56%,n=8,P0.001)and than that in healthy controls(31.69%±13.07%,n=20,P=0.001).Moreover,the NKG2A expression level was higher in patients at immunoclearence phase(35.62%±13.20%,n=19)than at immunotolerant phase(25.57%±5.56%,n=8,P=0.038)and than that in healthy controls(26.80%±10.49%,n=20,P=0.047).Conclusion The frequency of NK cells in peripheral blood of patients at immunoclearence phase is decreased whereas the expression of NKp46 and NKG2A is up-regulated,which is related to viral immunoclearence.
