Abstract:Ingested foreign bodies occasionally migrate to the paraglottic space. The external transcervical approach is almost always required to extract completely embedded foreign bodies. We report a case of an ingested fishbone embedded in the paraglottic space, which was successfully removed through transcervical exploration of the paraglottic space via the posterolateral approach. The posterolateral approach is safe and effective for the removal of foreign bodies completely embedded in the paraglottic space.

Abstract:Foreign body ingestion is a common problem encountered at gastroenterology clinics and emergency rooms which can cause serious complications. Usually, foreign bodies are directly visible with flexible endoscopes and can be readily removed. However, when foreign bodies migrate into the deeper tissue of the esophagus, surgery is typically required. There is currently no consensus regarding the best treatment. In this report, we present two cases in which fish bones embedded in the submucosal and muscularis propria of the esophagus were successfully removed via endoscopic submucosal dissection (ESD). Both patients were discharged without any complications.

Abstract:Objective: To explore clinical features, diagnosis, localization, and therapeutic strategy of migratory pharyngeal and cervical esophageal foreign bodies. Methods: A total 23 cases of pharyngeal and cervical esophageal migratory foreign bodies were admitted between January 2015 and December 2021. There were 14 females and 9 males with the age ranged from 35 to 82 (55.0±12.7)years. In all the cases, esophageal CT was taken to confirm the esophageal foreign body. Multiplanar reconstruction (MPR) was performed to locate the foreign body from the horizontal, coronal and sagittal dimensions as well as the corrected reconstructed MPR. According to the location of the foreign body, appropriate surgical method was selected.The symptoms, complications, types of foreign body, positioning, surgical methods, and relevant information were recorded.Data were analyzed using the descriptive method and SPSS 25.0 software. Results: The clinical symptoms of 23 migrating esophageal foreign bodies included pharyngodynia (20/23), foreign body sensation (6/23), hoarsenss (1/23), difficulty in turning neck(1/23), difficulty in opening mouth (1/23), fever (7/23), poor appetite (1/23), and abdominal pain (1/23). The foreign bodies included 19 fish bones, 2 wires, 1 embroidery needle and 1 chicken bone. There were 9 cases (39.1%) of foreign bodies located in extraluminal cervical esophagus, 2 cases (8.7%) of foreign bodies located in the muscular layer of the cervical esophagus and 12 cases (52.2%) of foreign bodies located in pharynx. Twenty-one cases of foreign bodies were removed by cervical lateral incision, in which 11 were removed by cervical lateral incision directly, 10 by the second lateral cervical incision after the foreign bodies were accurately located by MPR and/or corrected MPR, 1 foreign body was removed by incision of the pharyngeal mucosa under suspension laryngoscope, 1 foreign body was removed by tracheoscopy. Compared with patients with intraluminal foreign bodies (n=308) treated in the same period, intake of fishbone [19 (19/23) vs. 133 (82.6% (43.2%, 133/308), OR=7.31] and first visit was more than 24 hours [20(87.0%, 20/23) vs. 77(25.0%, 77/308),OR=17.2] were the significant risk factors of migratory esophageal foreign bodies. Conclusions: MPR and the corrected MPR can accurately locate the migrating pharyngeal and cervical esophageal foreign bodies, by providing more intuitive imaging evidence for doctors, which provide imaging basis for formulation of surgical programs. Foreign bodies in pharyngeal and cervical esophagus need to be treated as soon as possible, otherwise they are easy to migrate, leading lead to serious complications.
Entrapment of a Metal Foreign Body in the Cervical Spinal Canal During Surgical Procedure

Abstract:RATIONALE:Retention of foreign objects in spinal canal usually results from penetrating spinal trauma or failed internal instruments. However, entrapment of a foreign body in cervical spinal canal during surgery is rare, and whether such an object may cause neurological complications remains unknown in literature.PATIENT CONCERNS:A 50-year-old man underwent C5 corpectomy and instrumentation surgery due to cervical myelopathy. During the surgery, the cutting edge of a Kerrison rongeur was broken and the metal tip was retained behind C4 vertebra.DIAGNOSIS:Retention of foreign body in the cervical spinal canal.INTERVENTIONS:To remove the metal object, multiple strategies were tried but all failed. As such a metal object was thought to be dangerous to the spinal cord, a remedy C4 corpectomy was performed to remove it. Accidentally, however, the metal fragment further migrated to C2/3 canal. At last, the metal fragment had to be retained in the cervical spinal canal.OUTCOMES:At 2-year follow-up, the metal fragment remained in situ and no delayed complications occurred.LESSONS:We reported a rare case of metal object retention in cervical spinal canal due to rongeur fatigue fractures. Under certain circumstances, retention of a small foreign object in spinal canal may not lead to neurological complications. If failed to remove an entrapped foreign body, it may be safe to leave it in the spinal canal for further observation.

Abstract:BACKGROUND A fish spike stuck in the throat is a common ear, nose, and throat (ENT) emergency. However, it is very rare for a fish spike to reach the thyroid tissue through the throat, which is very dangerous and can lead to pharyngeal fistula, cervical abscess, mediastinal abscess, and thyroid abscess. Proper and timely management can help reduce complications, especially in elderly patients. CASE SUMMARY In the case presented here, the causative factor was dentures, but improper management aggravated the condition. In the case presented here, an elderly woman with a history of accidentally swallowing fish bones for 20 d had a sensation of foreign bodies in her throat. Eventually, computed tomography (CT) of the neck showed that the left side of the thyroid gland had a dense shadow in the form of a stripe. CONCLUSION If a fishbone foreign body is not visible during endoscopic examination but the patient has significant symptoms, the surgeon should be aware that the fishbone may be lodged in the thyroid. To avoid a misdiagnosis, ultrasound, CT, and other tests can be used to clarify the diagnosis. T The first step in treating a fish bone in the thyroid gland is to determine the position of the foreign body and the extent of the infection, and to develop a personalized surgical plan for its removal. At the same time, scientific information should be made available to the general public so that people know that if a fish bone is accidentally lodged, they should not force it to be swallowed or be spit out by inducing vomiting, which are incorrect methods and may aggravate the condition or even cause it to migrate outside the cavity, leading to serious complications, as in this reported case.

Delayed Endoscopic Removal of Sharp Foreign Body in the Esophagus Increased Clinical Complications

Abstract:Foreign bodies impaction in the esophagus is a common clinical emergency. The aim of this study was to investigate the clinical features of foreign body ingestion, and to analyze the risk factors of complications during the endoscopic procedure. From 18 general hospitals in Zhejiang Province in China, 595 patients who underwent gastroscopic removal of ingested foreign bodies were prospectively recruited. Patient characteristics, clinical features, foreign body features, clinical outcomes, and complications were documented. The most common types of foreign body in the esophagus were sharp objects (75.9%), including fish bones (34.0%), chicken bones (22.1%), and fruit nuclei (17.1%). The remaining types were non-sharp objects (24.1%), including food bolus (14.6%). Most objects were lodged in the proximal esophagus (75.9%). Foreign body-related complications occurred in 63 patients (10.5%), including hemorrhage (5.0%), perforation and infection (5.5%). The complication rate was increased by 4.04- and 8.48-fold when endoscopic retrieval was performed after impacted for over 24 and 48hours, respectively, after impaction, as compared with within 12hours. Logistic regression analysis revealed that the patients with sharp objects developed more complications than those with non-sharp ones (odds ratio, 2.85; 95% confidence interval, 1.08-7.50; P=.034). However, complications were unrelated with the location in the esophagus or length of foreign bodies (P>.05). Sharp objects were the most frequently ingested foreign bodies in the esophagus in China. The prevalence of complications was increased in the patients with long foreign body retention time (>24 hours) and sharp objects. Sharp foreign bodies in the esophagus are recommended to be removed within 24hours.
The Role Of Endovascular Stent Graft In The Successful Management Of Esophageal Foreign Body With Dangerous Vascular Complications

Abstract:The esophageal foreign body with dangerous vascular complications is a rare but usually fatal disease, including aortoesophageal fistula (AEF), arterio-esophageal fistula, penetrating aortoesophageal injury, artery dissection etc. Although surgical intervention is mandatory to these entities, there is little consensus about the optimal management of these conditions due to rare cases reported. With advent of the endovascular surgical strategy, the notion of treatment has been changed, and the successful cases reported had increased. Herein, we report three cases with dangerous vascular complications. First case is a 31-year-old man accidentally swallowing a pill with an oblong plastic packaging one day before admission and clinical manifestation of retrosternal pain and worsening dysphagia, with the diagnose of potential AEF. The second case is a 58-year-old woman swallowing a duck bone accidentally five days before admission and clinical manifestation of mild odynophagia, with the diagnose of aortic intramural hematoma. The third case is a 46-year-old-man swallowing a fish bone accidentally one day before admission and clinical manifestation of mild odynophagia and abnormal foreign body sensation, with the diagnose of one penetrating aortoesophageal injury. They were all successfully managed by combination of an emergent endovascular stent graft and endoscopic technique, and uneventful with follow-up of 14 months, 19 months and 23 months respectively.

Abstract:Fishbones are the most commonly ingested foreign bodies that cause gastrointestinal tract penetration. However, fishbones embedded in the gastrointestinal tract that lead to foreign body granulomas that mimic submucosal tumors are rare. Herein, we describe a 56-year-old woman who presented with a 20-day-history of upper abdominal pain. Endoscopy revealed an elevated lesion in the gastric antrum. An abdominal computed tomography scan showed a mass in the gastric antrum and a linear calcified lesion in the mass. An endoscopic ultrasonography examination revealed a 3.9 cm x 2.2 cm, irregular, hypoechoic mass with indistinct margins in the muscularis propria layer. The patient was initially diagnosed as having a submucosal tumor, and subsequent surgical resection showed that the lesion was a foreign body granuloma caused by an embedded fishbone. Our case indicated that the differential diagnosis of a foreign body granuloma should be considered in cases of elevated lesions in the gastrointestinal tract. Risk Factors of Difficult Pharyngeal Accidental Fishbones Ingestion

Abstract:OBJECTIVE:Accidental pharyngeal fishbone ingestion is a common complaint in ear, nose, and throat clinics. Approximately two-thirds of the accidentally ingested fishbones can be removed using tongue depressors and indirect laryngoscopy. However, the remaining third is challenging to identify and remove using these methods. These difficult fishbones require identification and removal via more advanced approaches. Video-guided laryngoscope is used to deal with difficult fishbones in our center. This study aimed to explore the risk factors for difficult fishbones.METHODS:A prospective study was performed at a teaching hospital on 2080 patients. Univariate and multivariate analyses were performed to identify the risk factors.RESULTS:The common fishbone locations were the tonsils (39.8%; defined as STEP-I), tongue base (37.1%), vallecula (13.3%; STEP-II), and hypopharynx (9.8%; STEP-III). With increasing STEP level, the ratio of difficult fishbones correspondingly increased (Z = 13.919, P < .001), and the proportions were 21.1%, 41.9%, and 70% in STEP-I, II, and III, respectively. In particular, fishbones in STEP-III (vs STEP-I) had a higher risk of difficult fishbones (odds ratio [OR]: 11.573, 95% CI: 7.987-16.769). Complaints of neck pain (yes vs no), foreign body sensation (yes vs no), and shorter length of fishbones always had a lower risk of difficult fishbones (OR: 0.455, 95% CI: 0.367-0.564; OR: 0.284, 95% CI: 0.191-0.422; OR: 0.727, 95% CI: 0.622-0.85). Missing teeth (yes vs no), swallowing behavior after fishbone ingestion (yes vs no), and male patients (vs female) had a higher risk of difficult fishbones (OR: 1.9, 95% CI: 1.47-2.456; OR: 1.631, 95% CI: 1.293-2.059; OR: 1.278, 95% CI: 1.047-1.56).CONCLUSIONS:Neck pain, foreign body sensation, fishbone length, patient age and sex, tooth status, and swallowing behavior after fishbone ingestion are independent risk factors for difficult fishbones.

Abstract:The orbit is susceptible to traumatic injuries, and surgical management of transorbital penetrating injury with foreign body retention is challenging due to the associated risks of nervous and vascular injury. Application of multiple imaging modalities is helpful for the preoperative surgical planning, and technical advances in percutaneous endoscopy would render the surgical approach even less invasive.

Abstract:Fishbone ingestion has been reported multiple times previously as a cause of oesophageal perforation. This is a surgical emergency that needs to be identified early to ensure immediate medical attention. This report presents the case of a 70-year-old patient with laryngeal perforation and the migration of a Chrysophrys auratus (Australasian snapper) fishbone to the C5 vertebral body. It is hypothesized that the fishbone migrated from the larynx to the visceral fascia and prevertebral fascia before lodging in between the intramuscular substance of the longus coli muscle. Multiple imaging modalities were used to identify and locate the foreign body, including flexible nasopharyngoscopy, esophagogastroduodenoscopy, and a computed tomography (CT) scan of the neck. The exploration of the neck was done by the ENT team and the orthopaedic spine team via the left anterior cervical approach at the level of the C5 and C6 vertebral bodies. The foreign body was identified (15 mm fishbone) at the left lateral of the C5 body, lodged between the intramuscular substance of the longus coli muscle, and was successfully removed.
Migrating fish bone piercing the common carotid artery, thyroid gland and causing deep neck abscess

Abstract:Foreign body (FB) ingestion is very common in Malaysian population. The most commonly ingested FB is fish bone. Common presenting symptoms include FB sensation, odynophagia and or sharp pricking pain during swallowing. A careful history and physical examination is very important. Despite negative laryngoscopy and rigid esophagoscopy, persistent symptoms warrants further radiographic imaging studies. The FB can migrate extraluminally and involve other important adjacent structures of the neck and along the digestive tract. We report 3 cases of extraluminal migration of fish bone and their complications, which were successfully managed. One case with vascular complication which involve common carotid artery and the other two cases with neck abscess formation involving thyroid gland, retropharyngeal and parapharyngeal abscess.

Abstract:RATIONALE:Accidental ingestion of a foreign body is common in daily life. But the hepatic migration of perforated foreign body is rather rare.PATIENT CONCERNS:A 37-year-old man presented with a history of vague epigastric discomfort for about 2 months.DIAGNOSIS:A diagnosis of the foreign body induced hepatic inflammatory mass was made based on abdominal computed tomographic scan and upper gastrointestinal endoscopy.INTERVENTIONS:The patient underwent laparoscopic laparotomy. During the operation, inflammatory signs were seen in the lesser omentum and segment 3 of liver. B- Ultrasound guided excision of the mass (in segment 3) was performed. Dissecting the specimen revealed a fish bone measuring 1.7 cm in length.OUTCOMES:The patient recovered uneventfully and was discharged on day 5 after surgery.LESSONS:This study shows the usefulness of endoscopy for final diagnosis and treatment in foreign body ingestion. Early diagnosis and decisive treatment in time are lifesaving for patients with this potentially lethal condition.
A Combination of Flexible and Rigid Bronchoscopy in the Successful Removal of a Residual Fish Bone from a Peripheral Bronchus: A Case Report

Abstract:Although rigid bronchoscopy remains the gold standard for the management of foreign body (FB) inhalation, sometimes it still misses residual FBs. Inhalation of sharp FBs by infants is an uncommon but hazardous occurrence, which presents a significant challenge and demands expertise in therapeutic bronchoscopy. Particularly, residual sharp FBs in the peripheral tracheobronchial tree may pose challenging management problems for bronchoscopists. Herein, we describe the case of 1-year-old girl, who presented with persistent atelectasis in the left lower lobe for 20 days without responding to antibiotic therapy after removal of fish bone by rigid bronchoscopy at local hospital. Flexible bronchoscopy at our department showed a residual fish bone in the outer basal segment of the left lower lobe. A combined flexible and rigid bronchoscopy was then applied, and a fish bone measuring 1.5 cm in length was extracted on multiple attempts without any complications. Thus, our reports demonstrated that removal of challenging residual sharp FBs in the distal airways is possible with the aid of combined flexible and rigid bronchoscopy by an experienced multidisciplinary team. Additionally, a physician should pay special attention to abnormal chest images after removal of FBs.
Successful Treatment of Fish Bone Perforation of the Duodenum with Hepatic Abscess Formation and Literature Review

Abstract:Symptomatic gastrointestinal foreign bodies are a common clinical event. Some patients can develop complications, such as gastrointestinal obstruction, perforation etc., and rarely the secondary hepatic abscess can also occur. A 45-year-old male was admitted to hospital with abdominal pain for 1 month and high fever for 6 days. He was diagnosed with hepatic abscess formation due to perforation of the digestive tract by a foreign body. This diagnosis was confirmed when the patient underwent surgery and the foreign body was identified as a fish bone. The fish bone was removed by laparotomy and the patient recovered uneventfully. Based on this case and the currently available literature, we recommend that endoscopic or surgical treatment should be initiated as soon as possible after detection of symptomatic gastrointestinal foreign bodies. Such intervention is important in reducing the risk of perforation of the digestive tract, as well as hepatic abscess formation.
Application of Computer Navigation Technology in Removal of Foreign Bodies in Oral and Maxillofacial Region

Abstract:Objective:To explore the clinical application of computer aided navigation in removal of foreign bodies in the oral and maxillofacial regions and evaluate the clinical effect.Methods:From January 2014 to December 2016,5 patients with foreign bodies in oral and maxillofacial regions were treated at Nanjing Stomatological Hospital.Preoperative CT or CBCT scanning were undertaken for all patients and the scanning data were imported to computer aided navigation system as digital imaging and communication in medicine (DICOM) for 3D reconstruction.The data were used for preoperative planning and intra-operative navigation.The clinical effect was evaluated by observing the accuracy of intra-operative navigation,the operation time,and the postoperative condition of patients.Results:With the computer aided navigation,a total of 28 foreign bodies were found in 5 cases,including 5 glass fragments,16 splinters,and 7 metals.One metal fragments could not be accurately located due to the loose of navigation reference frame.After re-fixed the reference frame,all foreign bodies were removed by minimally invasive access without any complications,the accuracy of computer aided navigation was less than 1mm,and the operating time was shorten.Conclusion:Computer aided navigation technology not only can accurately locate the foreign body in maxillofacial region,but also reduce the operation time and the postoperative complications,increase the accuracy and safety of surgery.Computer aided navigation had significant clinical values in the removal of foreign bodies in the oral and maxillofacial regions.
More than chest pain: A case of oesophageal foreign body ingestion

Abstract:Background: Physicians often focus on possible cardiac causes in patients presenting with chest pain. However, this case highlights a patient who presented with chest pain caused by ingestion of a foreign body after an uneventful meal eaten an hour prior to presentation. The fishbone was discovered after imaging. The article aims to raise awareness regarding the potential origins of chest pain, highlighting that it may stem from non-cardiac conditions. Methods: The methodology employed in this study involved conducting a case study that meticulously examined the repercussions and management strategies associated with foreign body ingestion. Results: The case report delineates the scenario of a 27-year-old male patient who inadvertently ingested a fishbone during a routine meal. It details the swift decline in clinical status, the meticulous diagnostic procedures employed, the subsequent management strategies implemented and the ultimate discharge of the patient in a stable condition. Conclusion: This case highlights the importance of comprehensive history taking and considering a wide range of causes of chest pain when evaluating a patient. The foreign body ingested with the resulting cardiac complications could have been fatal if not promptly diagnosed.Contribution: This study contributed to advancing awareness surrounding foreign body ingestion, shedding light on potential complications and offering valuable insights into effective management strategies.
Liver abscess secondary to fishbone ingestion: case report and review of the literature

Abstract:Abstract We report a rare silent migration of a fishbone into the liver and review the relevant literature. A 56-year-old man presented with a 2-day history of dull epigastric pain and raised inflammatory markers. Computerized tomography scan revealed a 4-cm abscess in the left lobe of the liver, with a linear radio-dense foreign body within the collection. At laparoscopy the hepatogastric fistula was disconnected. The fishbone was retrieved from the liver. Gastrostomy was closed with an omental patch. The patient had an uneventful recovery. Fifty-two cases of liver abscess secondary to enterohepatic fishbone migration were reported with over two-thirds presenting with a left-lobe abscess. There was marked variability in the management of liver abscess in the setting of fishbone migration-summarized in table. We believe that laparoscopic drainage of the abscess and extraction of the foreign body offer control of the source of sepsis and diminishes recurrence, whilst having a low-risk profile.
Minimally invasive removal of a foreign body in the pancreas using digital intelligent technology: a case report

Abstract:Pancreatitis caused by a fish bone penetrating the posterior wall of the stomach and entering the pancreas is rare. We herein report a case involving a woman in her late 30s with an approximately 1-month history of recurrent upper abdominal pain. Initial evaluation at another hospital failed to identify the cause but raised suspicion of pancreatic cancer. Computed tomography, magnetic resonance imaging, and a detailed consultation led us to suspect that the patient's pain had been caused by inadvertent ingestion of a fish bone. We used three-dimensional visualization technology to determine the location of the fish bone and informed the patient of the lesion and surgical plan through a simulated surgical demonstration. During surgery, we applied augmented reality navigation technology to remove the fish bone by a minimally invasive approach. The patient was discharged on postoperative day 3. She was followed up by telephone 24 hours after discharge. Outpatient follow-up was performed 1 week after discharge and on day 30. The patient recovered well and developed no complications. This case shows that digital medical technology can be applied in patients undergoing surgical removal of a pancreatic foreign body. Such technology assists with preoperative evaluation, patient education, and intraoperative trauma reduction.

Abstract:The accidental ingestion of fish bones is common. However, the complication of migration wherein the fish bone penetrated through the esophagus and into the right upper lobe is extremely rare. In this case, we present a rare case of recurrent pneumonia caused by a migrated esophageal foreign body. The case had been misdiagnosed for 5 years. A right upper lobectomy was performed; a fish bone was found in the lung parenchyma.
