Abstract:We argue that frequent sampling of the fraction of infected people (either by random testing or by analysis of sewage water), is central to managing the COVID-19 pandemic because it both measures in real time the key variable controlled by restrictive measures, and anticipates the load on the healthcare system due to progression of the disease. Knowledge of random testing outcomes will (i) significantly improve the predictability of the pandemic, (ii) allow informed and optimized decisions on how to modify restrictive measures, with much shorter delay times than the present ones, and (iii) enable the real-time assessment of the efficiency of new means to reduce transmission rates. Here we suggest, irrespective of the size of a suitably homogeneous population, a conservative estimate of 15000 for the number of randomly tested people per day which will suffice to obtain reliable data about the current fraction of infections and its evolution in time, thus enabling close to real-time assessment of the quantitative effect of restrictive measures. Still higher testing capacity permits detection of geographical differences in spreading rates. Furthermore and most importantly, with daily sampling in place, a reboot could be attempted while the fraction of infected people is still an order of magnitude higher than the level required for a relaxation of restrictions with testing focused on symptomatic individuals. This is demonstrated by considering a feedback and control model of mitigation where the feed-back is derived from noisy sampling data.

Abstract:Covid-19 is raging a devastating trail with the highest mortality-to-infected ratio ever for a pandemic. Lack of vaccine and therapeutic has rendered social exclusion through lockdown as the singular mode of containment. Harnessing the predictive powers of Machine Learning within a 6 dimensional infection kinetic model, depicting interactive evolution of 6 infection stages - healthy susceptible ($H$), predisposed comorbid susceptible ($P$), infected ($I$), recovered ($R$), herd immunized ($V$) and mortality ($D$) - the model, PHIRVD, provides the first accurate mortality prediction of 18 countries at varying stages of strategic lockdown, up to 30 days beyond last data training. PHIRVD establishes mortality-to-infection ratio as the correct pandemic descriptor, substituting reproduction number, and highlights the importance of early and prolonged but strategic lockdown to contain secondary relapse. Significance Statement: 1. Accurate prediction of the day-by-day mortality profiles of 18 countries, 30 days beyond the last data of data training. 2. Precise quantification of the impact of early-vs-later lockdown impositions. 3. Accurate prediction of secondary relapse timelines/ 4. Establishment of mortality-to-infected ratio as the correct pandemic descriptor substituting the popular choice of reproduction number, a proven failure in predicting future infection kinetics and secondary surge. The outcomes have potential to redefine healthy policy landscape, particularly in light of secondary relapse and possible future SARS-COV/Ebola group incursion.

Abstract:The COVID-19 pandemic has underscored the necessity of implementing non-pharmaceutical interventions such as lockdowns to mitigate the spread of infectious diseases. This study aims to model the impact of lockdown measures on the progression of an epidemic. Using a combination of compartmental models, specifically a novel delay model, we analyze the effects of varying lockdown intensities and durations on disease transmission dynamics. The results highlight that timely and stringent lockdowns can significantly reduce the peak number of infections and delay the epidemic's peak, thereby alleviating pressure on healthcare systems. Moreover, our models demonstrate the importance of appropriate lifting of lockdowns to prevent a resurgence of cases. Analytical and numerical results reveal critical thresholds for lockdown efficacy from the epidemiological point of view, which depend on such factors as the basic reproduction number (R0), disease duration, and immunity waning. In the case of a single outbreak with permanent immunity, we analytically determine the optimal proportion of isolated people which minimizes the total number of infected. While in the case of temporary immunity, numerical simulations show that the infectious cases decrease with respect to the proportion of isolated people during lockdowns; as we increase the proportion of isolated people, we have to increase the duration of lockdowns to obtain periodic outbreaks. Further, we assess the influence of epidemic with or without lockdown on the economy and evaluate its economical efficacy by means of the level of population wealth. The percentage of productive individuals among isolated people influences the wealth state of the population during lockdowns. The latter increases with the rise of the former for fixed epidemic parameters. This research provides valuable insights for policymakers in designing effective lockdown strategies to control future epidemics.

Abstract:As a common strategy of contagious disease containment, lockdowns will inevitably weaken the economy. The ongoing COVID-19 pandemic underscores the trade-off arising from public health and economic cost. An optimal lockdown policy to resolve this trade-off is highly desired. Here we propose a mathematical framework of pandemic control through an optimal stabilizing non-uniform lockdown, where our goal is to reduce the economic activity as little as possible while decreasing the number of infected individuals at a prescribed rate. This framework allows us to efficiently compute the optimal stabilizing lockdown policy for general epidemic spread models, including both the classical SIS/SIR/SEIR models and a new model of COVID-19 transmissions. We demonstrate the power of this framework by analyzing publicly available data of inter-county travel frequencies to analyze a model of COVID-19 spread in the 62 counties of New York State. We find that an optimal stabilizing lockdown based on epidemic status in April 2020 would have reduced economic activity more stringently outside of New York City compared to within it, even though the epidemic was much more prevalent in New York City at that point. Such a counterintuitive result highlights the intricacies of pandemic control and sheds light on future lockdown policy design.

Abstract:The cross-regional spread of epidemics, such as COVID-19, poses significant challenges due to the spillover of false-negative individuals resulting from incubation periods, detection errors, and individual irrationality. This study develops a stylized model to address the trade-offs faced by the planner in designing optimal lockdown policies: curbing the cross-regional spread of epidemics while balancing economic costs and ensuring long-term sustainability. The model integrates a queuing network to calculate the influx of false-negative cases, which more accurately reflects real-world scenarios and captures the complexity of regional interactions during an outbreak. Subsequently, a SIR network is used to estimate the spread of infections. Unlike similar studies, our approach focuses specifically on the cross-regional dynamics of epidemic spread and the formulation of optimal lockdown policies that consider both public health and economic impacts. By optimizing the lockdown threshold, the model aims to minimize the total costs associated with lockdown implementation and infection spread. Our theoretical and numerical results underscore the crucial role of timely nucleic acid testing in reducing infection rates and highlight the delicate balance between public health benefits and economic sustainability. These findings provide valuable insights for developing sustainable epidemic management strategies.

Abstract:Abstract Objective To analyse the impact and repercussions of the surge in healthcare demand in response to the COVID-19 pandemic, assess the potential effectiveness of various infection/disease control measures, and make projections on the best approach to exit from the current lockdown. Design A four-compartment model was constructed for SARS-CoV-2 infection based on the Wuhan data and validated with data collected in Italy, the UK, and the US. The model captures the effectiveness of various disease suppression measures in three modifiable factors: (a) the per capita contact rate (β) that can be lowered by means of social distancing, (b) infection probability upon contacting infectious individuals that can be lowered by wearing facemasks, personal hygiene, etc., and (c) the population of infectious individuals in contact with the susceptible population, which can be lowered by quarantine. The model was used to make projections on the best approach to exit from the current lockdown. Results The model was applied to evaluate the epidemiological data and hospital burden in Italy, the UK, and the US. The control measures were identified as the key drivers for the observed epidemiological data through sensitivity analyses. Analysing the different lockdown exit strategies showed that a lockdown exit strategy with a combination of social separation/general facemask use may work, but this needs to be supported by intense monitoring which would allow re-introduction/tightening of the control measures if the number of new infected subjects increases again. Conclusions and relevance Governments should act early in a swift and decisive manner for containment policies. Any lockdown exit will need to be monitored closely, with regards to the potential of lockdown reimplementation. This mathematical model provides a framework for major pandemics in the future.

Abstract:This paper repurposes the classic insight from network theory that long-distance connections drive disease propagation into a strategy for controlling a second wave of Covid-19. We simulate a scenario in which a lockdown is first imposed on a population and then partly lifted while long-range transmission is kept at a minimum. Simulated spreading patterns resemble contemporary distributions of Covid-19 across nations, regions, and provinces, providing some model validation. Results suggest that the proposed strategy may significantly flatten a second wave. We also find that post-lockdown flare-ups remain local longer, aiding geographical containment. Public policy may target long ties by heavily focusing medical testing and mobility tracking efforts on traffic and transport. This policy can be communicated to the general public as a simple and reasonable principle: Stay nearby or get checked.

Abstract:We study the effectiveness of tracking and testing in mitigating or suppressing epidemic outbreaks, in combination with or as an alternative to quarantines and global lockdowns. We study these intervention methods on a network-based SEIR model, augmented with an additional probability to model symptomatic, asymptomatic and pre-symptomatic cases. Our focus is on the basic trade-offs between economic costs and human lives lost, and how these trade-offs change under different lockdown, quarantine, tracking and testing policies. Our main findings are as follows: (i) Tests combined with patient quarantines reduce both economic costs and mortality, but require a large-scale testing capacity to achieve a significant improvement; (ii) Tracking significantly reduces both economic costs and mortality; (iii) Tracking combined with a limited number of tests can achieve containment without lockdowns; (iv) If there is a small flow of new incoming infections, dynamic "On-Off" lockdowns are more efficient than fixed lockdowns. Our simulation results underline the extreme effectiveness of tracking and testing policies in reducing both economic costs and mortality and their potential to contain epidemic outbreaks without imposing social distancing restrictions. This highlights the difficult social question of trading-off these gains with the privacy loss that tracking necessarily entails.

Abstract:Testing for infection with SARS-CoV-2 is an important intervention in reducing onwards transmission of COVID-19, particularly when combined with the isolation and contact-tracing of positive cases. Many countries with the capacity to do so have made use of lab-processed Polymerase Chain Reaction (PCR) testing targeted at individuals with symptoms and the contacts of confirmed cases. Alternatively, Lateral Flow Tests (LFTs) are able to deliver a result quickly, without lab-processing and at a relatively low cost. Their adoption can support regular mass asymptomatic testing, allowing earlier detection of infection and isolation of infectious individuals. In this paper we extend and apply the agent-based epidemic modelling framework Covasim to explore the impact of regular asymptomatic testing on the peak and total number of infections in an emerging COVID-19 wave. We explore testing with LFTs at different frequency levels within a population with high levels of immunity and with background symptomatic PCR testing, case isolation and contact tracing for testing. The effectiveness of regular asymptomatic testing was compared with `lockdown' interventions seeking to reduce the number of non-household contacts across the whole population through measures such as mandating working from home and restrictions on gatherings. Since regular asymptomatic testing requires only those with a positive result to reduce contact, while lockdown measures require the whole population to reduce contact, any policy decision that seeks to trade off harms from infection against other harms will not automatically favour one over the other. Our results demonstrate that, where such a trade off is being made, at moderate rates of early exponential growth regular asymptomatic testing has the potential to achieve significant infection control without the wider harms associated with additional lockdown measures.

Abstract:We are currently facing a highly critical case of a world-wide pandemic. The novel coronavirus (SARS-CoV-2, a.k.a. COVID-19) has proved to be extremely contagious and the original outbreak from Asia has now spread to all continents. This situation will fruitfully profit from the study in regards of the spread of the virus, assessing effective countermeasures to weight the impact of the adopted strategies. The standard Susceptible-Infectious-Recovered (SIR) model is a very successful and widely used mathematical model for predicting the spread of an epidemic. We adopt the SIR model on a random network and extend the model to include control strategies {\em lockdown} and {\em testing} -- two often employed mitigation strategies. The ability of these strategies in controlling the pandemic spread is investigated by varying the effectiveness with which they are implemented. The possibility of a second outbreak is evaluated in detail after the mitigation strategies are withdrawn. We notice that, in any case, a sudden interruption of such mitigation strategies will likely induce a resurgence of a second outbreak, whose peak will be correlated to the number of susceptible individuals. In fact, we find that a population will remain vulnerable to the infection until the herd immunity is achieved. We also test our model with real statistics and information on the epidemic spread in South Korea, Germany, and New York and find a remarkable agreement with the simulation data.
Data-driven Simulation and Optimization for Covid-19 Exit Strategies

Abstract:The rapid spread of the Coronavirus SARS-2 is a major challenge that led almost all governments worldwide to take drastic measures to respond to the tragedy. Chief among those measures is the massive lockdown of entire countries and cities, which beyond its global economic impact has created some deep social and psychological tensions within populations. While the adopted mitigation measures (including the lockdown) have generally proven useful, policymakers are now facing a critical question: how and when to lift the mitigation measures? A carefully-planned exit strategy is indeed necessary to recover from the pandemic without risking a new outbreak. Classically, exit strategies rely on mathematical modeling to predict the effect of public health interventions. Such models are unfortunately known to be sensitive to some key parameters, which are usually set based on <a class="link-external link-http" href="http://rules-of-thumb.In" rel="external noopener nofollow">this http URL</a> this paper, we propose to augment epidemiological forecasting with actual data-driven models that will learn to fine-tune predictions for different contexts (e.g., per country). We have therefore built a pandemic simulation and forecasting toolkit that combines a deep learning estimation of the epidemiological parameters of the disease in order to predict the cases and deaths, and a genetic algorithm component searching for optimal trade-offs/policies between constraints and objectives set by decision-makers. Replaying pandemic evolution in various countries, we experimentally show that our approach yields predictions with much lower error rates than pure epidemiological models in 75% of the cases and achieves a 95% R2 score when the learning is transferred and tested on unseen countries. When used for forecasting, this approach provides actionable insights into the impact of individual measures and strategies.

Abstract:The number of coronavirus disease 2019 (COVID-19) cases continues to surge, overwhelming healthcare systems and causing excess mortality in many countries. Testing of infectious populations remains a key strategy to contain the COVID-19 outbreak, delay the exponential spread of the disease, and flatten the epidemic curve. Using the Omicron variant outbreak as a background, this study aimed to evaluate the effectiveness of testing strategies with different test combinations and frequencies, analyze the factors associated with testing effectiveness, and optimize testing strategies based on these influencing factors. We developed a stochastic, agent-based, discrete-time susceptible–latent–infectious–recovered model simulating a community to estimate the association between three levels of testing strategies and COVID-19 transmission. Antigen testing and its combination strategies were more efficient than polymerase chain reaction (PCR)-related strategies. Antigen testing also showed better performance in reducing the demand for hospital beds and intensive care unit beds. The delay in the turnaround time of test results had a more significant impact on the efficiency of the testing strategy compared to the detection limit of viral load and detection-related contacts. The main advantage of antigen testing strategies is the short turnaround time, which is also a critical factor to be optimized to improve PCR strategies. After modifying the turnaround time, the strategies with less frequent testing were comparable to daily testing. The choice of testing strategy requires consideration of containment goals, test efficacy, community prevalence, and economic factors. This study provides evidence for the selection and optimization of testing strategies in the post-pandemic era and provides guidance for optimizing healthcare resources.

Abstract:Abstract Motivated by COVID-19, we develop and analyze a simple stochastic model for the spread of disease in human population. We track how the number of infected and critically ill people develops over time in order to estimate the demand that is imposed on the hospital system. To keep this demand under control, we consider a class of simple policies for slowing down and reopening society and we compare their efficiency in mitigating the spread of the virus from several different points of view. We find that in order to avoid overwhelming of the hospital system, a policy must impose a harsh lockdown or it must react swiftly (or both). While reacting swiftly is universally beneficial, being harsh pays off only when the country is patient about reopening and when the neighboring countries coordinate their mitigation efforts. Our work highlights the importance of acting decisively when closing down and the importance of patience and coordination between neighboring countries when reopening.

Abstract:Controlling the COVID-19 pandemic is an urgent global challenge. The rapid geographic spread of SARS-CoV-2 directly reflects the social structure. Before effective vaccines and treatments are widely available, we have to rely on alternative, non-pharmaceutical interventions, including frequency testing, contact tracing, social distancing, mask wearing, and hand-washing, as public health practises to slow down the spread of the disease. However frequent testing is the key in the absence of any alternative. We propose a network approach to determine the optimal low resources setting oriented pool testing strategies that identifies infected individuals in a small number of tests and few rounds of testing, at low prevalence of the virus. We simulate stochastic infection curves on societies under quarantine. Allowing some social interaction is possible to keep the COVID-19 curve flat. However, similar results can be strategically obtained searching and isolating infected persons to preserve a healthier social structure. Here, we analyze which are the best strategies to contain the virus applying an algorithm that combine samples and testing them in groups [1]. A relevant parameter to keep infection curves flat using this algorithm is the dairy frequency of testing at zones where a high infection rate is reported. On the other hand, the algorithm efficiency is low for random search of infected people.

Abstract:Coronavirus disease 2019 (COVID-19) is currently under a global pandemic trend. The efficiency of containment measures and epidemic tendency of typical countries should be assessed. In this study, the efficiency of prevention and control measures in China, Italy, Iran, South Korea, and Japan was assessed, and the COVID-19 epidemic tendency among these countries was compared. Results showed that the effective reproduction number(Re) in Wuhan, China increased almost exponentially, reaching a maximum of 3.98 before a lockdown and rapidly decreased to below 1 due to containment and mitigation strategies of the Chinese government. The Re in Italy declined at a slower pace than that in China after the implementation of prevention and control measures. The Re in Iran showed a certain decline after the establishment of a national epidemic control command, and an evident stationary phase occurred because the best window period for the prevention and control of the epidemic was missed. The epidemic in Japan and South Korea reoccurred several times with the Re fluctuating greatly. The epidemic has hardly rebounded in China due to the implementation of prevention and control strategies and the effective enforcement of policies. Other countries suffering from the epidemic could learn from the Chinese experience in containing COVID-19.

Abstract:Objectives This study quantified how the efficiency of testing and contact tracing impacts the spread of COVID-19. The average time interval between infection and quarantine, whether asymptomatic cases are tested or not, and initial delays to beginning a testing and tracing programme were investigated. Setting We developed a novel individual-level network model, called CoTECT (Testing Efficiency and Contact Tracing model for COVID-19), using key parameters from recent studies to quantify the impacts of testing and tracing efficiency. The model distinguishes infection from confirmation by integrating a ‘T’ compartment, which represents infections confirmed by testing and quarantine. The compartments of presymptomatic (E), asymptomatic (I), symptomatic (Is), and death with (F) or without (f) test confirmation were also included in the model. Three scenarios were evaluated in a closed population of 3000 individuals to mimic community-level dynamics. Real-world data from four Nordic countries were also analysed. Primary and secondary outcome measures Simulation result: total/peak daily infections and confirmed cases, total deaths (confirmed/unconfirmed by testing), fatalities and the case fatality rate. Real-world analysis: confirmed cases and deaths per million people. Results (1) Shortening the duration between Is and T from 12 to 4 days reduces infections by 85.2% and deaths by 88.8%. (2) Testing and tracing regardless of symptoms reduce infections by 35.7% and deaths by 46.2% compared with testing only symptomatic cases. (3) Reducing the delay to implementing a testing and tracing programme from 50 to 10 days reduces infections by 35.2% and deaths by 44.6%. These results were robust to sensitivity analysis. An analysis of real-world data showed that tests per case early in the pandemic are critical for reducing confirmed cases and the fatality rate. Conclusions Reducing testing delays will help to contain outbreaks. These results provide policymakers with quantitative evidence of efficiency as a critical value in developing testing and contact tracing strategies.

Abstract:In the absence of neither an effective treatment or vaccine and with an incomplete understanding of the epidemiological cycle, Govt. has implemented a nationwide lockdown to reduce COVID-19 transmission in India. To study the effect of social distancing measure, we considered a new mathematical model on COVID-19 that incorporates lockdown effect. By validating our model to the data on notified cases from five different states and overall India, we estimated several epidemiologically important parameters as well as the basic reproduction number ($R_{0}$). Combining the mechanistic mathematical model with different statistical forecast models, we projected notified cases in the six locations for the period May 17, 2020, till May 31, 2020. A global sensitivity analysis is carried out to determine the correlation of two epidemiologically measurable parameters on the lockdown effect and also on $R_{0}$. Our result suggests that lockdown will be effective in those locations where a higher percentage of symptomatic infection exists in the population. Furthermore, a large scale COVID-19 mass testing is required to reduce community infection. Ensemble model forecast suggested a high rise in the COVID-19 notified cases in most of the locations in the coming days. Furthermore, the trend of the effective reproduction number ($R_{t}$) during the projection period indicates if the lockdown measures are completely removed after May 17, 2020, a high spike in notified cases may be seen in those locations. Finally, combining our results, we provided an effective lockdown policy to reduce future COVID-19 transmission in India.

Abstract:The pandemic caused by the novel Coronavirus SARS-CoV2 has been responsible for life threatening health complications, and extreme pressure on healthcare systems. While preventive and definite curative medical interventions are yet to arrive, Non-Pharmaceutical Interventions (NPIs) like physical isolation, quarantine and drastic social measures imposed by governing agencies are effective in arresting the spread of infections in a population. In densely populated countries like India, lockdown interventions are partially effective due to social and administrative complexities. Using detailed demographic data, we present an agent based model to imitate the behavior of the population and its mobility features, even under intervention. We demonstrate the effectiveness of contact tracing policies and how our model efficiently relates to empirical findings on testing efficiency. We also present various lockdown intervention strategies for mitigation - using the bare number of infections, the effective reproduction rate, as well as using reinforcement learning. Our analysis can help assess the socio-economic consequences of such interventions, and provide useful ideas and insights to policy makers for better decision making.

Effectiveness of Testing, Tracing, Social Distancing and Hygiene in Tackling Covid-19 in India: A System Dynamics Model

Abstract:We present a System Dynamics (SD) model of the Covid-19 pandemic spread in India. The detailed age-structured compartment-based model endogenously captures various disease transmission pathways, expanding significantly from the standard SEIR model. The model is customized for India by using the appropriate population pyramid, contact rate matrices, external arrivals (as per actual data), and a few other calibrated fractions based on the reported cases of Covid-19 in India. Also, we have explicitly modeled, using independent time-variant levers, the effects of testing, contact tracing, isolating Covid-positive patients, quarantining, use of mask/better hygiene practices, social distancing through contact rate reductions at distinct zones of home(H), work(W), school(S) and other(O) locations. Simulation results show that, even after an extended lock-down, some non-trivial number of infections (even asymptomatic) will be left and the pandemic will resurface. Only tools that work against the pandemic is high rate of testing of those who show Covid-19 like symptoms, isolating them if they are positive and contact tracing all contacts of positive patients and quarantining them, in combination with use of face masks and personal hygiene. A wide range of combination of effectiveness of contact tracing, isolation, quarantining and personal hygiene measures help minimize the pandemic impact and some imperfections in implementation of one measure can be compensated by better implementation of other measures.

Abstract:With more than 1.7 million COVID-19 deaths, identifying effective measures to prevent COVID-19 is a top priority. We developed a mathematical model to simulate the COVID-19 pandemic with digital contact tracing and testing strategies. The model uses a real-world social network generated from a high-resolution contact data set of 180 students. This model incorporates infectivity variations, test sensitivities, incubation period, and asymptomatic cases. We present a method to extend the weighted temporal social network and present simulations on a network of 5000 students. The purpose of this work is to investigate optimal quarantine rules and testing strategies with digital contact tracing. The results show that the traditional strategy of quarantining direct contacts reduces infections by less than 20% without sufficient testing. Periodic testing every 2 weeks without contact tracing reduces infections by less than 3%. A variety of strategies are discussed including testing second and third degree contacts and the pre-exposure notification system, which acts as a social radar warning users how far they are from COVID-19. The most effective strategy discussed in this work was combined the pre-exposure notification system with testing second and third degree contacts. This strategy reduces infections by 18.3% when 30% of the population uses the app, 45.2% when 50% of the population uses the app, 72.1% when 70% of the population uses the app, and 86.8% when 95% of the population uses the app. When simulating the model on an extended network of 5000 students, the results are similar with the contact tracing app reducing infections by up to 79%.
