Effect of immersive virtual reality on pain in different dental procedures in children: A pilot study

Abstract:Background: Implementing effective pain management is important to increase patient compliance during paediatric dental procedures. Aim: This pilot study was conducted to evaluate the effectiveness of virtual reality (VR) on pain perception in dental procedures in children. Material and methods: Fifty-four children aged between 5 and 12 years were included. Patients scheduled to receive dental procedures not requiring local anesthesia (eg, fluoride therapy) were assigned to Group A, and patients scheduled to receive painful dental procedures requiring local anesthesia (eg, pulp therapy, teeth extraction) were assigned to Group B. Patients in each group were randomly assigned and were equally likely to either receive VR during their dental procedure, or treatment as usual (without VR). Visual analog scale (VAS), Wong-Baker FACES rating scale, and the 'Face, Legs, Activity, Cry, Consolability' scale (FLACC scale) were used to assess the pain levels during dental procedures. Results: Patients receiving painful dental procedures requiring local anesthesia reported significant reductions in pain intensity/worst pain during the dental procedure on all subjective and behavioral pain measures of pain intensity with the use of VR distraction technique (P < .05 on each). Patients receiving non-painful dental procedures showed the predicted pattern but no significant reduction in worst pain during VR. Conclusion: The use of VR was found to be an effective distraction tool to ease pain and anxiety in the tested dental procedures, for children receiving painful dental procedures.

Abstract:BACKGROUND: This study evaluated the efficacy of virtual reality (VR) distraction and counter-stimulation (CS) on dental anxiety and pain perception to local anesthesia in children.METHODS: A prospective, randomized, single-blinded interventional clinical trial with a parallel design was used. Seventy children 7-11 years old who required local anesthesia (LA) for pulp therapy or tooth extraction were recruited and allocated to two groups with equal distribution based on the intervention. Group CS (n = 35) received CS and Group VR (n = 35) received VR distraction with ANTVR glasses. Anxiety levels (using pulse rate) were evaluated before, during, and after administration of local anesthesia, while pain perception was assessed immediately after the injection. Wong-Baker faces pain-rating scale (WBFPS), visual analog scale (VAS), and Venham's clinical anxiety rating scale (VCARS) were used for pain evaluation. Student's t-test was used to test the mean difference between groups, and repeated measures ANOVA was used to test the mean difference of pulse rates.RESULTS: Significant differences in mean pulse rates were observed in both groups, while children in the VR group had a higher reduction (P < 0.05), and the mean VCARS scores were significant in the VR group (P < 0.05). Mean WBFPS scores showed less pain perception to LA needle prick in the CS group while the same change was observed in the VR group with VAS scores.CONCLUSIONS: VR distraction is better than CS for reducing anxiety to injection in children undergoing extraction and pulpectomy.
Comparative Evaluation of the Efficacy of Virtual Reality Distraction, Audio Distraction and Tell-show-do Techniques in Reducing the Anxiety Level of Pediatric Dental Patients: An In Vivo Study

Abstract:Objectives and methods: Children, in the age-group of 6 to 8 years, with moderate level of anxiety, (measured with M-DAS), requiring inferior alveolar nerve block (IANB) for mandibular tooth extraction were selected. Informed consent was obtained from parents. Selected children were allocated randomly into 3 groups virtual reality (VR) distraction group, audio distraction group and Tell-show-do (TSD) group. Pre- and post- distraction anxiety level of children was measured subjectively with facial image scale and objectively with pulse rate and oxygen saturation (measured with pulse oxymeter).Results: There was a statistically significant decrease in pulse rate after distraction (with a p < 0.01) in all the three groups. Postdistraction pulse rate was lowest in the VR distraction group when compared to other groups. While comparing postdistraction pulse rate values in the three groups, the difference in pulse rate between TSD and VR distraction group was significant. But while comparing the pulse rate between audio distraction and TSD group (p = 0.06), and audio distraction and VR distraction group (p = 0.24) was not statistically significant. Oxygen saturation increased in all the three groups which was statistically significant (p < 0.01). There was a statistically significant difference in the postdistraction oxygen (O2) saturation levels between TSD and audio distraction groups (p < 0.05) with more O2 saturation in audio distraction group. But while comparing the audio distraction with VR group and TSD with VR group, the difference was not significant. Facial image scale (FIS) scores decreased in all the three groups (p < 0.01).Conclusion: The overall results revealed by all the parameters indicate that children were most relaxed in VR group, followed by audio group and were least relaxed in TSD group during dental visits. Hence VR distraction can be considered as a useful technique for behavior management of pediatric patients during a conventional dental treatment.How to cite this article: SG G, George S, S A, et al. Abstract:Objective: The purpose of this study was to assess the effects of virtual reality (VR) in reducing pain and anxiety levels in children. The study also compared active and passive distraction methods using VR during the delivery of inferior alveolar nerve blocks (IANBs) in dental procedures in children. Material and methods: The study comprised 45 preschool patients, aged between 4 and 6 years, with no prior dental anesthetic experience. The participants were randomly assigned to three groups based on the sort of management style: Group A used the tell-show-do technique, Group B engaged in passive distraction by watching cartoons using a VR headset, and Group C participated in active distraction by playing games using a controller with the VR headset. Pain and anxiety were evaluated using physiological measurements, namely by analyzing the variations in blood pressure, heart rate, and oxygen saturation before and after the administration of IANB. Psychological assessments were conducted using the Wong-Baker faces scale, Modified Dental Anxiety scale questionnaires, and Revised Face, Legs, Activity, Cry and Consolability scale after administering IANB. Results: The physiological outcomes revealed no statistically significant differences in blood pressure and oxygen saturation. However, there was a statistically significant increase in the heart rate in group A compared with groups B and C. In terms of psychological measurements, groups B and C exhibited a significant improvement in pain experience and a decrease in anxiety. Conclusion: This study concluded that VR reduced pain and anxiety levels in its passive and active forms.

Abstract:DOI: 10.1002/anie.201503620 Following publication of this Communication, Dr. Boy Cornils (Hofheim, Germany) kindly informed the authors about the missing references for the retro-hydroformylation reaction. In the late 1960s, Orlicek[1] and inventors from Ruhrchemie AG[2] reported that cracking of isobutyraldehyde into propylene, carbon monoxide, and hydrogen was possible. The work was also published in academic literature in 1972.[3] In this reference, the two authors reported about the cracking of isobutyraldehyde, an undesired sideproduct of the oxo-process, to recover propylene and syngas using rhodium on alumina as a catalyst. Dr. Cornils comments that in 1972, his group confirmed the data in a months-andmonths run on a pilot plant scale. He says “it worked but not economically enough for industrial scale application.” In addition, in 1968, Tsuji et al. reported the formation of nonene as a major product in their attempt for decarbonylation of decanal with a heterogeneous palladium catalyst.[4]

Abstract:Abstract Background Different distraction techniques have been used in dentistry and have shown great results in managing anxious pediatric patients specially during local anesthesia administration. One of the recently invented techniques is virtual reality. The purpose of the study was to evaluate the effect of virtual reality distraction on anxiety and pain during buccal infiltration anesthesia in pediatric patients. Methods Healthy, cooperative 6- to 12-year-old children requiring buccal infiltration anesthesia were randomly assigned to a test or control group. In the test group, local anesthesia was administered while the subjects were watching a cartoon video using virtual reality goggles. Subjects in the control group watched a cartoon video on a screen during the administration of local anesthesia. To assess anxiety in both groups, heart rate was recorded using a pulse oximeter at five time points: (1) once the subject sets on the dental chair as a baseline; (2) when video is on; (3) at topical anesthesia application; (4) during needle insertion; (5) after the administration of local anesthesia. The face, legs, activity, cry, consolability (FLACC) behavioral pain assessment scale and the Wong–Baker FACES pain rating scale were used to assess pain. Results A total of 50 subjects were included with a mean age of 8.4 ± 1.46 years. Twenty-nine (58.0%) of the subjects were females. The mean heart rate at all time points except baseline was significantly higher among the test group compared to the control group. Multiple regression analysis showed that younger subjects and females had higher mean FLACC behavioral pain assessment scale scores ( P = 0.034 and P = 0.004, respectively) regardless of the distraction technique used. Younger subjects and subjects with higher baseline heart rate reported higher mean Wong–Baker FACES pain rating scale score ( P = 0.031 and P = 0.010, respectively), controlling for all other variables. Conclusion Female subjects and the younger age group were more likely to report higher pain scores during local anesthesia administration regardless of the type of distraction used. Effectiveness of Virtual Reality Distraction on Pain Perception and Fear among Children with Cancer Undergoing IV Cannulation

Abstract:Abstract Background: Cancer children are subjected to multiple cannulations during hospitalization. Pain and fear are most common during invasive procedures that are performed, especially IV cannulation, which can lead to worry. Exactly 25% of children are reported to have a fear of needle-related procedures. To examine the impact of virtual reality (VR) distraction on pain perception and fear in children with cancer undergoing IV cannulation. Materials and Methods: Using a quasi-experimental design, 80 cancer children with IV cannulation between ages 7–18 were recruited to the intervention and control group ( N = 40 + 40) using the purposive sampling technique. The virtual reality distraction device was used 10 min before the peripheral IV cannulation procedure and remained in place until the procedure was completed in the intervention group, and no device was used in the control group. Wong–Baker Faces Pain Rating Scale and Children’s Fear Scale were used to assess the pain and fear of children and their mothers. Result: Pain perception was found to be significantly lower in the VR distraction group among both children and mothers (1.82 ± 0.18, 8.01 ± 3.21) 95% confidence interval (CI) [0.82, 2.16] compared to the control group, respectively ( P = 0.001**). Fear was found to be significantly lower in the VR distraction group among both children and mothers (0.81 ± 0.71, 3.01 ± 1.42) 95% CI [0.46, 0.91]. A strong positive correlation was found between children’s and mothers’ pain perception ( r = 0.91, P = 0.001**) and fear ( r = 0.84, P = 0.001**). Conclusion: VR distraction can be used as an effective device in pain and fear management among children undergoing IV cannulation.

Abstract:Objective: The aim of this study was to evaluate the influence of using virtual reality (VR) eyeglasses on severity of pain and anxiety during pulp therapy in pediatric patients considering childhood anxiety-related disorders as an important confounding factor in the dental setting. Materials and methods: A total of 40 children aged between 4 and 8 years with no previous anxiety disorder were randomly divided into two groups. The study consisted of three consecutive treatment sessions. The two groups received pulp therapy with and without VR eyeglasses in a randomized single-blind-controlled crossover fashion. Then, at the end of each session, the patients' pain severity was assessed using Wong-Baker FACES Pain Rating Scale and state anxiety was measured by Faces version of the Modified Child Dental Anxiety Scale [MCDAS]. Changes in pulse oximeter and heart rate were recorded in every 10 min. The values obtained were tabulated and subjected to statistical analysis. Results: The study shows a significant decrease in pain perception (P < 0.001) and state anxiety scores (P < 0.001) with the use of VR eyeglasses during dental treatment. Conclusion: The results of this study provide an initial encouraging for the use of VR device during dental treatment by pediatric dentists, but additional empirical research is required.

Abstract:BACKGROUND: Dental anxiety and anxiety-related avoidance of dental care creates significant problems for the patients and dental practitioners. Administration of local anesthesia is one of the procedures which results in increased anxiety level among patients.AIM: The aim of this study was to study the effectiveness of virtual reality (VR) as a distraction tool during the administration of local anesthesia in patients undergoing a dental procedure.MATERIALS AND METHODS: Fifty patients were randomized into two groups, namely, control group (n = 25) and study group (n = 25). Preoperative, intra-operative, and postoperative pulse rate and oxygen saturation were measured using pulse oximeter for all the patients. Local anesthesia was administered without the use of VR for the control group and with the VR for the study group.STATISTICAL ANALYSIS: The data were evaluated using the Statistical Package for the Social Sciences 20. Student's t-test was used to compare the variables.RESULTS: Statistically significant results were obtained for preoperative and postoperative oxygen saturation, intraoperative pulse rate, and postoperative visual analog scale pain scale.CONCLUSION: The findings of the current study indicate that immersive VR is an effective distraction tool to alleviate the anxiety of the patient.

Abstract:Aims: This study assessed the effect of immersive virtual reality (IVR) on anxiety and intraoperative pain in adult patients undergoing dental extractions with local anesthesia. Methods: In a single-blind, randomized clinical trial from September 2022 to December 2023 at a private dental clinic, 190 patients with dental anxiety were randomly assigned to either an IVR or a control group. Primary outcomes—dental anxiety and perioperative pain—were measured using the State-Trait Anxiety Inventory (STAI), Modified Dental Anxiety Scale (MDAS), and Visual Analogue Scale (VAS) before and after the procedure. Secondary outcomes included heart rate (HR), diastolic blood pressure (DBP), and systolic blood pressure (SBP), recorded at various stages. Results: The IVR group showed significant reductions in total anxiety, state anxiety, and MDAS scores compared to the control group (p < 0.001). Pain intensity was also lower in the IVR group (p = 0.03). Additionally, heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP) were consistently lower in the IVR group across different stages. Post anesthesia, the IVR group showed notably lower mean SBP and DBP values (p < 0.001). After surgery, the IVR group also showed lower HR (p = 0.01), SBP (p < 0.001), and DBP (p < 0.001) compared to the control group. Conclusions: IVR significantly reduced STAI, STAI-S, and MDAS scores and decreased intraoperative pain compared to the control group.

Abstract:Background and Objectives. Dental fear and anxiety are common causes of dental care avoidance especially in pediatric dentistry. Distraction is among the safest and cheapest techniques for behavioral guidance in children. Virtual reality (VR) can be used as a means of distraction. This study aimed to assess the effect of distraction by VR on the levels of pain and anxiety in 6–8-year-old patients requiring mandibular primary molar pulpotomy. Materials and Methods. This crossover clinical trial was conducted with eligible 6–8-year-old children requiring bilateral mandibular molar pulpotomy. At the first treatment visit, pulpotomy was performed for 15 children using VR glasses distraction while the other 15 children received a pulpotomy without any VR glasses; this trend was reversed at the second session and pulpotomy was performed for the contralateral tooth. Pulse rate (PR) and Modified Child Dental Anxiety Scale (MCDAS) measured the anxiety levels. Wong–Baker Faces Pain Scale (WBFP) assessed the pain perception before and after the intervention. Data were analyzed by Statistical Package for the Social Sciences version 25 using the Mann–Whitney and χ2 tests. Results. The mean PR was not significantly different between the two groups. However, the test group showed significantly lower scores of MCDAS (P value = 0.02) and WBFP (P value = 0.001) compared with the control group. Conclusion. The present results suggest that VR headsets can decrease the level of pain and anxiety of patients during primary mandibular pulpotomy. This trial is registered with IRCT20200315046782N1.

Abstract:Context: Dental environment is one of the most anxiety-provoking stimuli for children, which makes patient management a constant challenge in both typically growing and intellectually disabled children. Distraction is a nonpharmacological technique to manage anxiety in children during dental treatment.Aim: This study compares the effect of audio and virtual reality (VR) distraction on the dental anxiety of healthy and mild intellectually disabled children.Methods: Forty children aged 6-14 years were divided into two groups (Group I: children with mild intellectual disability and Group II: healthy children). Groups I and II were further subdivided into two subgroups of 10 children each, based on the distraction technique given to them during the first appointment. After 1 month, we carried out a cross-over of the subgroups. Anxiety levels were measured using physiological and observational parameters at three-time intervals.Statistical Analysis: Performed using paired t-test for the intergroup comparison and Wilcoxon Signed Rank test for the intra-group comparison.Results: A decrease in the pulse rate, improvement in oxygen saturation levels, and lower Venham's anxiety rating scores were observed on introducing audio and VR distraction in all the subgroups. The inter-group comparison revealed that audio and VR were more effective in healthy children than children with mild intellectual disabilities.Conclusions: Audio and VR distraction techniques can be employed successfully to reduce anxiety in children with mild intellectual disability and healthy children undergoing dental restorative treatment.

Abstract:Background: Dental anxiety is a frequent reason for reluctance in young children, leading to challenges in delivering effective dental treatment due to their uncooperative behavior. Aim: The aim of this study was to evaluate and compare the efficacy of different behaviour management techniques while administrating injectable LA with minimum pain perception and anxiety in children. Material and methods: One hundred and twenty children, aged 4 to 10 years, were equally and randomly assigned to four groups. In Group I, local anesthesia was administered while using wireless headphones. For Group II, local anesthesia was administered using a mobile phone with earpieces. Group III received local anesthesia while incorporating the 3D virtual reality eyewear method and Group IV received local anesthesia while utilizing the stress ball technique. Pain and anxiety assessments were conducted using various scales and Physiological parameters such as pulse rate and oxygen saturation levels were recorded and the resultant data were systematically tabulated for subsequent statistical analysis. Results: Virtual reality group showed highly significant result in terms of lowering anxiety and pain scores compared to audio, audio visual, stress ball groups (p<0.001). Virtual group (5.10) also displays significantly highest behavior scores than the audio (1.70), audio visual group (3.96) and Stress Ball groups (2.66). Conclusions: The virtual reality group emerged as the most effective method in alleviating anxiety and pain experienced by pediatric dental patients. Abstract:A BSTRACT Aim: In order to manage pediatric dental patients, this study set out to assess the effectiveness of two distraction techniques: audio and virtual reality (VR) with tell, show, and do technique (TSD). Materials and Methods: Forty-five children aged 6–10 years were selected for the present randomized control trial. They were split into three groups at random. Group 1 control group: tell-show-do, Group 2: audio distraction, and Group 3: VR distraction. Both pretreatment and postoperative anxiety levels were measured by modified Venham’s anxiety rating scale (MVARS). Results: In each of the three groups, there was a statistically significant difference between the child’s pre- and post-treatment anxiety levels. VR distraction group experienced the greatest reduction in anxiety. Conclusion: All three methods were similarly successful at lowering anxiety while virtual reality distraction outperformed compared to other techniques.

Abstract:Aim: To assess the effectiveness of distraction with virtual reality (VR) eyewear along with modified tell-show-do (MTSD) on anxiety levels of 6-11-year-old children with hearing impairment (HI) during the noninvasive dental procedure.Materials and methods: The randomized controlled trial included 40 children with HI aged 6-11 years requiring oral prophylaxis. The children were randomly allocated into two groups. Oral prophylaxis was carried out in both groups, where in group I (VR + MTSD, n = 20), distraction with VR eyewear was performed along with MTSD, and in group II (MTSD, n = 20), MTSD alone was used. Pre and postoperative anxiety levels were assessed using facial image scale (FIS) (subjective) and pulse rate (PR) (objective) measures. Paired t-test and unpaired t-test were used to analyze the obtained data.Results: Postoperative (post-op) PR readings in the VR + MTSD group were reduced by 6.95, whereas it was increased in the MTSD group by 8.55, and the difference was statistically significant (p = 0.001). Post-op FIS scores were found to be reduced in the VR + MTSD group by 2.15, whereas it was increased in the MTSD group by 0.10, and the difference was statistically significant (p = 0.033).Conclusion: Distraction using VR eyewear along with MTSD is effective in reducing anxiety levels in 6-11-year-old children with HI during noninvasive dental procedures.Clinical significance: Hearing-impaired children are usually anxious about the unknown and have unmet oral health needs due to communication barriers. Abstract:Objective: The current study investigated the virtual reality distraction technique as an indicator of pain and anxiety reduction in children with an age between 6 to 9 years during short invasive dental procedure using salivary cortisol level as an indicator for measuring level of anxiety Method: This randomized clinical trial was conducted in a dental hospital of Lahore in the department of Pediatric dentistry. Data was collected from 30 patients which were divided in two groups. One group was containing patients who had screen time of less than 4hours whereas the other group contained patients who had screen time of more than 4hours in a day. Data was entered in SPSS version 25.0. Frequencies and percentages were calculated as descriptive analysis whereas comparison among both groups was made using Chi-Square test. Results: The results of chi-square test revealed that there is a significant difference in the pain perception of patients using virtual reality distraction technique during dental treatment (X2=4.701, P=.030). The results of paired sample t test revealed a significant difference between patients who has less than 4 hours screen time and patients who has more than 4 hours screen time in terms of salivary cortisol level (P=.001). Conclusion: The application of virtual reality distraction technique for the dental treatment of children is more effective among those who have more screen time as compared to the children who have less screen time Keywords: Virtual reality distraction, Pain, Anxiety, Dental treatment

Local anesthesia is integral to the practice of painless dentistry but the pain of injection itself is deterrent to successful administration of local anesthesia and can be a most anxiety-provoking procedure. Distraction as a behavior management technique is successfully known to reduce pain and manage children's dental behavior by diverting their attention away from painful stimuli during invasive dental procedure. This study aimed to compare the pain associated with local anesthetic injection delivered with and without the use of distraction as a behavior management technique in 6–8-year-old children. Materials and Methods: In this randomized, clinical, in vivo study with a split-mouth design we compared the pain of 30 children (6–8-year-old), requiring dental treatment necessitating the use of local anesthesia bilaterally in either of their maxillary and mandibular arches. Treatment was done in two visits, 1 week apart. Children were randomly assigned to receive the distraction (iPad) at one visit while no distraction in other visits. Two different pain assessment scales were used: Wong-Baker faces pain scale (FPS) and FPS-Revised. The Chi-square test was used for statistical analysis. P ≤ 0.05 was considered to be statistical significance. Results: Children who received local anesthesia with audiovisual distraction had lower pain rating scores than those who received local anesthesia with no distraction. Conclusion: Audiovisual distraction significantly reduces pain associated with injection of local anesthesia.

Abstract:The creating different kinds of the phononic crystals and acoustic metamaterials, which has advanced ultrasound properties, have generated considerable recent research interest. The new kind acoustic materials, allows to studying the new effects of the ultrasound interaction and developing different types of the surface acoustic wave (SAW) devices for electronic industry. We represented the possibilities to creating of the controlled SAW devices on the base 36° rotated Y-cut, X-propagating LiTaO3 single crystal with the volume electroinduced periodical domain structure. We are focusing on the theoretical and experimental investigations of volume induced and controlled domain structures.
Effects of an Immersive Virtual Reality Intervention on Pain and Anxiety Among Pediatric Patients Undergoing Venipuncture: A Randomized Clinical Trial

Abstract:Importance: Venipuncture is one of the most painful and distressing procedures experienced by pediatric patients. Emerging evidence suggests that providing procedural information and distraction using immersive virtual reality (IVR) may reduce pain and anxiety among children undergoing needle-related procedures. Objectives: To examine the effects of IVR on reducing the pain, anxiety, and stress experienced by pediatric patients undergoing venipuncture. Design, setting, and participants: This 2-group randomized clinical trial recruited pediatric patients aged 4 to 12 years undergoing venipuncture from a public hospital in Hong Kong between January 2019 and January 2020. Data were analyzed from March to May 2022. Interventions: Participants were randomly allocated to an intervention (an age-appropriate IVR intervention offering distraction and procedural information) or a control (standard care only) group. Main outcomes and measures: The primary outcome was child-reported pain. Secondary outcomes included child-reported anxiety, heart rate, salivary cortisol, length of procedure, and satisfaction of health care professionals with the procedure (rated on a 40 point scale, with higher scores indicating greater satisfaction). Outcomes were assessed 10 minutes before, during, immediately after, and 30 minutes after the procedure. Results: A total of 149 pediatric patients were recruited, with 86 female patients (57.7%) and 66 patients (44.3%) diagnosed with fever. Compared with the 74 participants in the control group (mean [SD] age, 7.21 [2.49] years), the 75 participants in the IVR group (mean [SD] age, 7.21 [2.43] years) reported significantly less pain (β = -0.78; 95% CI, -1.21 to -0.35; P < .001) and anxiety (β = -0.41; 95% CI, -0.76 to -0.05; P = .03) immediately after the intervention. Health care professional satisfaction in the IVR group (mean [SD] score, 34.5 [4.5]) was significantly higher than that in the control group (mean [SD] score, 32.9 [4.0]; P = .03). Moreover, the length of venipuncture procedure in the IVR group (mean [SD] duration, 4.43 [3.47] minutes) was significantly shorter than that in the control group (mean [SD] duration, 6.56 [7.39] minutes; P = .03). Conclusions and relevance: In this randomized clinical trial, integrating procedural information and distraction in an IVR intervention for pediatric patients undergoing venipuncture significantly improved pain and anxiety in the IVR group compared with the control group. The results shed light on the global trends of research on IVR and its clinical development as an intervention for other painful and stressful medical procedures.
