Association of Long-Term Sleep Habits and Hypertension: a Cross-Sectional Study in Chinese Adults

Abstract:Poor sleep has been associated with risk of hypertension, but previous studies were limited by treating one or two sleep factors as predictor. Our previous study has developed a sleep factor questionnaire (SFQ) to comprehensively assess wide range of sleep characteristics including sleep duration, sleep quality, light at night (LAN) exposure, night/shift work, daytime napping, and frequency of nighttime waking, insomnia, and snoring. In this cross-sectional study we used the SFQ to evaluated the associations between these sleep domains and hypertension. Comparing with the subjects who slept 7–8 h, subjects with sleep duration 6–7 h (OR = 1.42, 95% CI: 1.01, 2.00), 8–9 h (OR = 1.60, 95% CI: 1.07, 2.40), and over 9 h (OR = 2.39, 95% CI: 1.60, 3.58) had an increased prevalence of hypertension. The associations were significant among individuals under 45 years sleeping 6–7 h and those over 45 years sleeping over 8 h. The OR of prevalent hypertension was 0.80 (95% CI: 0.62, 0.97) for habitual daytime napping compared with never napping, and the association was also significant among individuals over 45 years old. Moreover, among subjects with sleep duration 7–8 h, habitual daytime napping may decrease prevalent hypertension (OR = 0.635, 95% CI: 0.437, 0.924). In conclusion, our data suggested a positive association between both short (6–7 h) and long sleep duration and hypertension, and a preventive pattern for habitual daytime napping among over 45 years old and hypertension. Moreover, interaction analysis indicated that habitual midday nap may decrease prevalent hypertension among subjects with sleep duration 7–8 h.

Abstract:Abstract Although sleep duration has been identified as a significant factor in risk for hypertension, there is limited data on the relationship between sleep duration and hypertension control. This study examined the association between habitual sleep duration and hypertension control in United States of America (USA) adults with hypertension. A total of 5,163 adults from the National Health and Nutrition Examination Survey (2015 – 2018) were analyzed. Survey-weighted multivariable logistic regression models were fit to examine the association between habitual sleep duration (coded as <6, 6 - 9 hours) and hypertension control (BP <130/80mmHg versus ≥130/80mmHg), adjusted for sociodemographic, sleep and health characteristics. In the fully adjusted model, habitual sleep duration of <6 hours/main sleep period was associated with reduced odds of hypertension control (OR = 0.66, 95% CI: 0.46 – 0.95, P = 0.027) when compared to 7 – 9 hours. No significant differences were noted in hypertension control between the reference group (7 - 9 hours) and the 6 - 9 hours groups. These findings suggest that measures to support adequate habitual sleep duration may help improve hypertension control in adults who habitually sleep for <6 hours/day.

Abstract:STUDY OBJECTIVES The objective of this study was to evaluate the cross-sectional relationship between sleep duration and hypertension in a large, nationally-representative dataset that spans 10 years. This analysis may provide detailed information with high resolution about how sleep duration is related to hypertension and how this differs by demographic group. METHODS Data were aggregated from the 2013 Behavioral Risk Factor Surveillance System (n = 433,386) and the combined 2007-2016 National Health Interview Surveys (n = 295,331). These data were collected by the Centers for Disease Control and Prevention from nationally-representative samples. Surveys were combined, and survey-specific weights were used in all analyses. Sleep duration was assessed with the item, "On average, how many hours of sleep do you get in a 24-hour period?" in both surveys. Hypertension was assessed as self-reported history. Covariates were assessed identically in both datasets and included, age (in 5-year groupings), sex, race/ethnicity, and employment status. RESULTS In adjusted analyses, compared to 7 hours, increased risk of hypertension was seen among those sleeping ≤ 4 hours (odds ratio [OR] = 1.86, P < .0005), 5 hours (OR = 1.56, P < .0005), 6 hours (OR = 1.27, P < .0005), 9 hours (OR = 1.19, P < .0005), and ≥ 10 hours (OR = 1.41, P < .0005). When stratified by age, sex, and race/ethnicity groups, short sleep was associated with increased risk for all age groups < 70 years, and long sleep (≥ 10 hours only) was associated with risk for all except < 24 years and > 74 years. Findings for short sleep were relatively consistent across all race/ethnicities, although findings for long sleep were less pronounced among Black/African-American and Other/Multiracial groups. A significant sleep by 3-way sleep × age × sex interaction (P < .0005) suggests that the relationship depends on both age and sex. For both men and women, the OR of having hypertension associated with short sleep decreases with increasing age, but there is a higher association between short sleep and hypertension for women, throughout the adult lifespan. CONCLUSIONS Both short and long sleep duration are associated with increased hypertension risk across most age groups. The influence of covariates is stronger upon long sleep relationships. Relationships with short sleep were stronger among younger adults and women.

In the subpopulation of adults who were on antihypertensive medication, those with a sleep duration of < 6 hours had lower odds of BP control than those with a sleep duration of 7– 9 hours (OR = 0.53, 95% CI: 0.36– 0.77, P = 0.002). No significant differences were noted in all adults with hypertension and in the subpopulation of those on antihypertensive medication in BP control between the reference sleep duration group (7– 9 hours) and the 6 - 9 hours groups. There were no significant differences across age groups or gender in the relationship between habitual sleep duration and BP control. Conclusion: Sleep duration of < 6 hours is associated with reduced odds of hypertension control. These significant findings indicate that interventions to support adequate habitual sleep duration may be a promising addition to the current hypertension management guidelines. Considering Psychosocial Factors when Investigating Blood Pressure in Patients with Short Sleep Duration: A Propensity Score Matched Analysis

Abstract:Few studies have considered psychosocial characteristics when investigating the associations between sleep duration and blood pressure (BP). In this study, we took propensity score matching (PSM) to adjust for psychosocial characteristics when comparing BP between individuals with short sleep duration and those with normal sleep duration. A total of 429 participants were included. 72 participants with sleep duration ≤6 h and 65 participants with sleep duration >6 h were matched after PSM. We compared office BP, 24-hour BP, and prevalence of hypertension in the populations before and after PSM, respectively. In the unmatched population, participants with sleep duration ≤6 h were observed with higher office diastolic BP (DBP) and 24-h systolic BP (SBP)/DBP (all P<0.05). In the matched populations, the differences between the two groups (sleep duration ≤6 h vs. sleep duration >6 h) in office DBP (88.4 ± 10.9 vs. 82.5 ± 11.1 mm Hg; P=0.002), 24-h SBP (134.7 ± 12.0 vs. 129.3 ± 11.6 mm Hg; P=0.009), and 24-h DBP (83.4 ± 9.9 vs. 78.1 ± 10.1 mm Hg; P=0.002) become more significant. Participants with sleep duration ≤6 h only show higher prevalence of hypertension based on 24-h BP data, while analysis after PSM further revealed that these with sleep duration ≤6 h presented about 20% higher prevalence of elevated BP up to office diagnosed hypertension threshold. Therefore, psychosocial characteristics accompanied with short sleep duration should be fully valued in individuals at risks for elevated BP. This trial is registered with NCT03866226.

Abstract:Objective:The purpose of research is to investigate the associations of sleep factors separately and jointly with risk of hypertension.Methods:The National Health and Nutrition Examination Surveys (NHANES) is a nationally representative survey. Participants aged over 20 years with complete and credible data from the NHANES 2015-2016 and 2017-2018 waves were included. Hypertension was assessed based on self-report medical diagnoses, or antihypertensive medication use, or systolic blood pressure >= 140 mmHg and/or diastolic blood pressure >= 90 mmHg. Sleep information (sleep duration, trouble sleeping, daytime sleepiness, self-reported snoring and sleep-related breathing issue) was obtained from household interview.Results:Of 7426 participants, the mean (standard deviation) age was 48.0 (17.3) years, 3845 (51.8%) were females. The prevalence of hypertension was 32.8%, and lower in those with 7-9 h sleep, no trouble sleeping, no excessive daytime sleepiness, no snoring or sleep apnea symptoms, decreased as the quantity of healthy sleep factors increased. The self-reported short sleep (odds ratio [OR]: 1.25, 95% confidence interval [CI]: 1.02-1.54, P = 0.032), trouble sleeping (OR: 1.53, 95% CI: 1.20 to 1.95, P = 0.001), excessive daytime sleepiness (OR: 1.17, 95% CI: 1.01-1.35, P = 0.041) and sleep apnea symptoms (OR: 1.33, 95% CI: 1.10-1.61, P = 0.004) were associated with 25%, 53%, 17% and 33% increased risk of hypertension, respectively. Participants with a poor sleep pattern was associated with higher hypertension risk (OR: 2.47, 95% CI: 1.90-3.22, P < 0.001).Conclusion:Sleep behaviors were cross-sectionally associated with a considerably higher hypertension risk.

Abstract:Background: Suboptimal sleep is a risk factor for hypertension (HTN) and is one of the eight risk factors for cardiovascular disease (CVD). However, the association between sleep disturbances and HTN is unclear. Objective: To examine the relationship between sleep disturbances and HTN among adults in the U.S. We hypothesized that higher sleep disturbance scores are associated with HTN. Methods: We used a cross-sectional design to examine the 2013-2018 National Health Interview Survey. We computed a sleep disturbance score, composite of five components of suboptimal sleep; sleep duration, trouble falling asleep, staying asleep, sleep medication use, and restful. The sleep disturbance score ranges from 0-10 high scores reflect worse sleep disturbance and were categorized into quartiles. Survey-weighted multinomial logistic regression models were fitted, adjusting for covariates. Results: A total of 190,217 participants were included; 31% had HTN, mean age was 47.2 (±0.09 SE) years, and 49% were male. Mean sleep disturbance score was 2.7; mild sleep disturbance was observed in 21%, moderate in 33%, severe in 13%, and very severe in 33%. In the fully adjusted model, the odds of moderate (adjusted odds ratio (aOR): 1.30; 95% Confidence Intervals, 95%CI: [1.15,1.47]) and severe sleep disturbances (aOR: 1.42; [1.30,1.56]) were 30% and 42%, respectively, higher in persons with HTN compared to those without HTN (Table). Conclusion: Adults with self-reported HTN are more likely to have higher sleep disturbances, hence, HTN may play a larger role in sleep outcomes. When screening for HTN, it is also important to screen for sleep patterns that may have an adverse effect on health outcomes.

Abstract:Background Poor sleep quality pose significant public health challenges globally. The objective of this study is to investigate the association between sleep quality, duration and the risk of hypertension. Methods 284,250 adults from the UK Biobank (UKB) and 6,104 participants from the English Longitudinal Study of Ageing (ELSA) were analyzed. The exposures of interest were sleep quality and duration. Sleep quality was evaluated based on the number of healthy sleep behaviors (UKB cohort) or questionnaire adapted from the Jenkins Sleep Problems Scale (ELSA cohort). Participants were divided into three groups based on the assessment of sleep quality. Long sleep is characterized by a duration exceeding 8 hours, medium sleep ranges from 7 to 8 hours and short sleep denotes less than 7 hours of sleep. The primary endpoint was incident hypertension. Results In UKB cohort, participants with poor sleep quality showed significantly higher risk of hypertension compared those with healthy sleep quality (hazard ratio [HR] 1.276, confidence interval [CI] 1.21, 1.346). Short sleep duration (≤7h) was associated with an elevated hypertension risk in participants regardless of different sleep quality, and both short(≤7h) and prolonged sleep time (≥9h) were associated with an increased hypertension risk in the poor sleep quality group. Several factors were independently associated with a lower risk of hypertension, including appropriate sleep duration (7-8 hours per day), absence of frequent insomnia, no self-reported snoring(P<0.001). Associations between sleep quality and an increased risk of hypertension remained significant regardless of genetic susceptibility to hypertension (P for interaction= 0.067). Conclusions Poor sleep quality is associated with an elevated risk of hypertension. Adjusting sleep behavior to enhance sleep quality within a recommended range could be a promising strategy for mitigating the risk of hypertension.

Abstract:Elucidation of the association between short sleep duration and elevated blood pressure has implications for assessing and managing hypertension in adults.OBJECTIVE: To assess the relationship between sleep duration and blood pressure, and its role in the etiology of hypertension.METHODS: On a systematic search from MEDLINE, EMBASE, CINAHL, PEDro, PsychINFO and grey literature were included articles with participants over 18 years, reported sleep duration, measured blood pressure or diagnosed hypertension, and the relationship between sleep duration and blood pressure was analyzed.RESULTS: Of 2522 articles initially identified, 11 studies met the inclusion criteria. Sample sizes ranged from 505 to 8860 (aged ≥ 20-98 years). Five studies (aged ≥ 58-60 years) determined that sleep duration and blood pressure were unrelated. In younger adults, five studies reported an association between short sleep duration and hypertension before adjustment for confounding variables; only the findings from one study remained significant after adjustment. Two studies supported a sex association; women who sleep less than 5-6 h nightly are at greater risk of developing hypertension.CONCLUSION: Sleep duration and blood pressure are associated in both women and adults under 60 years. Controlled studies are needed to elucidate confounding factors and the degree to which sleep profiles could augment diagnosis of hypertension and sleep recommendations to prevent or manage hypertension.

Abstract:Objective: To explore the association between sleep duration, sleep quality and the prevalence of hypertension in the elderly aged 65 years and above. Methods: This study was conducted among the elderly in communities in Yiwu, China from April to July, 2019, and participants were recruited through physical examination in the hospital. Face-to-face interview was performed to obtain basic information. Sleep duration and sleep quality were evaluated by Pittsburgh Sleep Quality Index (PSQI). Associations between sleep duration, sleep quality and hypertension were evaluated by multivariate logistic regression analysis. Results: A total of 3 169 elderly persons, aged ≥65 years old, were included in the study. The overall prevalence of hypertension was 50.8%. The elderly with very poor sleep quality and short sleep duration accounted for 22.4% and 28.5%, respectively. After adjusting for demographic characteristics, socioeconomic status, lifestyle and health status, the OR of hypertension for the elderly with very poor sleep quality was 1.42 (95%CI: 1.12-1.80) compared with those with very good sleep quality. Compared with the elderly with sleep duration of 6-7 h a night, the OR of hypertension for those with sleep duration <6 h was 1.37 (95%CI: 1.15-1.65). As the sleep quality decreased, the risk for hypertension increased. An U-shaped association was found between sleep duration and risk of hypertension. Subgroup analyses showed that this association existed in both men and women, but only significant in the elderly aged <75 years. Conclusion: Poor sleep quality and short sleep duration were associated with risk for hypertension in the elderly.
Association between Sleep Duration and Hypertension among Adults in Southwest China

Abstract:Objective: This study aimed to evaluate the association between sleep duration and hypertension among adults in southwest China.Methods: Baseline variables were collected from a representative sample of 20,053 adults aged 23-98 years in southwest China who received physical examinations from January 2019 to December 2020. All participants were categorized into either a hypertension group or a non-hypertension group. Sleep duration was classified as short (<6 h/day), normal (6-8 h/day),or long (>8 h/day). Baseline variables were compared between individuals with and without hypertension by rank-sum tests for two independent samples or χ2 tests for nonparametric data. Multivariate logistic regression analysis was performed to evaluate the association between sleep duration and hypertension.Results: The overall incidence of hypertension was 51.2%. Unadjusted analysis showed that the risk of hypertension was higher in individuals with short (<6h/day) or long (>8h/day) sleep durations compared with those with a normal (6-8 h/day) sleep duration. The risk of hypertension was significantly increased by 30.1% in participants with a long (>8h/day) sleep duration compared with those with a normal (6-8h/day) sleep duration (OR = 1.301, P < 0.010, 95%CI = 1.149-1.475). The risk of hypertension was also increased by 1.1% in participants with a short (<6h/day) sleep duration compared with participants with a normal (6-8h/day) sleep duration, but the difference was not significant (OR = 1.011, P = 0.849, 95%CI = 0.905-1.129). After fully adjusting for confounding factors (model 4), the risk of hypertension was increased significantly (by 25%) in individuals with a short (<6h/day) sleep duration (OR = 1.25, P = 0.02, 95%CI = 1.036-1.508) but not in those with a long (>8h/day) sleep duration (17.5% increase) compared with participants with a normal (6-8h/day) sleep duration (OR = 1.175, P = 0.144, 95%CI = 0.946-1.460).Conclusion: The results of this study indicate that a short (<6h/day) sleep duration is related to an increased risk of hypertension, suggesting that sleep helps to protect against hypertension.

Abstract:Background: Sleep is a naturally occurring period of lower blood pressure (BP), recommended by the AHA as an essential lifestyle feature to prevent cardiovascular disease (CVD). While hypertension (HTN) disproportionately affects Black adults in the United States (US), differences in sleep duration among Black ethnic subgroups including African American, African immigrant, and Afro-Caribbean adults with hypertension are unknown. Objective: To examine differences in sleep duration among Black ethnic subgroups and Non-Hispanic (NH) White persons with HTN in the US. Methods: Using cross-sectional data from the 2004-2018 National Health Interview Survey, sleep duration was defined by participants’ self-reported average number of hours of sleep per day. Survey-weightedgeneralized linear models were used to examine the differences between sleep duration among the Black ethnic subgroups with hypertension compared to their NH White counterparts. Results: Of 120,511 adults (17% Black, 51% female with mean age 57±.08 [SE] years) mean(±SE) sleep duration was 7.2 (0.01) hours. Unadjusted, African immigrants had the shortest mean sleep duration (6.8 hours) compared to NH White persons (7.2 hours). In the fully adjusted model, compared to NH White persons, all adults in the Black Ethnic Subgroups had significantly lower sleep durations compared to White persons, with Afro-Caribbean persons having the lowest sleep duration ( Table ). Conclusion: Sleep duration varied across ethnic subgroups of Black adults with self-reported HTN. Whether interventions targeting these differences could improve hypertension control represents an important subject for subsequent research.

Abstract:Studies show a link between short sleep duration and hypertension. The mechanistic associations between sleep duration and blood pressure (BP) are less clear. We studied the association between sleep duration, blood pressure, metabolic profile as well as obstructive sleep apnoea in participants with suspected or confirmed hypertension undergoing 24 h ambulatory blood pressure measurement (ABPM). We included 177 participants (mean age 57 ± 11 years, 67 women) referred for ABPM, with available exact time-points for bedtimes during the recording. Daytime and night-time BP values were calculated accordingly. Biochemistry included triglycerides (TG), high- and low-density lipoproteins (HDL, LDL) and total cholesterol. The HDL/TG ratio was calculated as a marker of glucose intolerance. Sleep apnoea was quantified by simultaneous ambulatory polygraphy. Participants were stratified into those with short (n = 16) /normal (n = 139) /long (n = 22) sleep (< = 6 h, > 6 < = 9 h and > 9 h). There were no differences in age, body mass index (BMI) or apnoea hypopnea index (AHI) between groups. Mean office BP was 146 ± 19/89 ± 11 mmHg and mean 24 h ABPM 133 ± 14/80 ± 8 mmHg. By standard ABPM definitions 81, 86, and 77% in the short/normal/long sleep group were hypertensive, and 56, 56 and 73% were on antihypertensive medication. The long sleep group had lower 24 h diastolic BP than the short and normal sleep groups (p < 0.05 by ANOVA). TG levels and TG/HDL ratio were higher in the long sleep group than in the short sleep group (both p < 0.05 by ANOVA; not adjusted for statin treatment). No differences were found in HDL, LDL, or total cholesterol between groups. In a multiple linear regression model, longer sleep duration, older age, and female sex were all independently associated with lower 24 h ABPM diastolic BP (all p < 0.01; full model R2 = 0.25, p < 0.001) while BMI, AHI, TG, and the TG/HDL ratio were not related. Longer sleep duration was independently associated with lower ABPM 24 h diastolic BP together with older age and female sex. The observation with increased TG and TG/HDL ratio in the long sleep group warrants further study.

Abstract:Objective Hypertension is a major risk factor and cause of many non-communicable diseases in China. While there have been studies on various diet and lifestyle risk factors, we do not know whether sleep duration has an association to blood pressure in southwest China. This predictor is useful in low-resource rural settings. We examined the association between sleep duration and hypertension in southwest China. Design Population-based cross-sectional study. Setting This study was part of the baseline survey of a large ongoing prospective cohort study, the China Kadoorie Biobank. Participants were enrolled in 15 townships of Pengzhou city in Sichuan province during 2004–2008. Participants 55 687 participants aged 30–79 years were included. Sleep duration was assessed by a self-reported questionnaire. Main outcome measures Hypertension was defined as systolic blood pressure ≥140 mm Hg and/or diastolic blood pressure ≥90 mm Hg, or prior physician-diagnosed hypertension in hospitals at the township (community) level or above. Results The prevalence of hypertension was 25.17%. The percentages of subjects with sleep durations of <6, 6, 7, 8 and ≥9 hours were 17.20%, 16.14%, 20.04%, 31.95% and 14.67%, respectively. In multivariable-adjusted analyses, the increased ORs of having hypertension were across those who reported ≥9 hours of sleep (men: 1.16, 95% CI 1.04 to 1.30; women: 1.19, 95% CI 1.08 to 1.32; general population: 1.17, 95% CI 1.08 to 1.26). The odds of hypertension was relatively flat until around 6.81 hours of sleep duration and then started to increase rapidly afterwards in subjects and a J-shaped pattern was observed. There was a U-shaped relationship between sleep duration and hypertension in females. Conclusion Long sleep duration was significantly associated with hypertension and a J-shaped pattern was observed among rural adults in southwest China, independent of potential confounders. However, this association was not obvious between short sleep duration and hypertension.

[A single rectal diclofenac dose reduces post-haemorrhoidectomy urine retention: results of a randomised, controlled clinical study].

Abstract:INTRODUCTION Post-surgical urinary retention requiring a catheter has a mean incidence of 15% (1% to 52%) in the post-operative period after anal surgery. The primary objective of this study was to assess the efficacy of topical rectal Diclofenac in reducing post-surgical haemorrhoidectomy urinary retention. Its impact on the reduced need for post-surgery analgesia has also been assessed as a secondary objective. PATIENTS AND METHOD The 157 patients intervened for symptomatic haemorrhoids were randomised into two groups: Group with 100 mg diclofenac anal suppository after surgery; and Control group without diclofenac. Haemorrhoidectomy was performed with diathermy, without pedicle ligatures and with hyperbaric spinal anaesthesia. No anal dressings were used and the use of drips was restricted during the post-operative period. Metamizole and diclofenac were used as post-surgical intravenous analgesia in all patients. Meperidine was chosen as rescue analgesia. The descriptive variables evaluated in the patient series were: age, sex, grade and number of haemorrhoid groups, and post-surgical complications. The response variables were: need or not for a urethral catheter, post-surgical pain (Visual Analogue Scale) VAS and rescue analgesia requirements. RESULTS The 86 males (54.6%) and 71 females (45.4%), with a mean age of 54 years (26-82 years) were randomised into 2 groups: 73 (46.5%) in the diclofenac group and 84 (53.5% in the control group. Of the haemorrhoidectomies performed, 95 were open and 63 were closed. There were no differences as regards the distribution by, age, sex, haemorrhoid grade or in the number of haemorrhoid groups extirpated. Only 1 (1.3%) patient in the diclofenac group and 8 (9.5%) in the control group (p = 0.028) had urine retention. Rescue analgesia was required by 9 (12.3%) patients in the study group compared to 20 (33.3%) in the control group (p = 0.002). The assessment of the magnitude of therapeutic effect of diclofenac gave an RRR of 85.6% (95% CI, 12.3-98.2), RAR, 8.2% (95% CI, 1.3-15) and an NNT of 13 (95% CI, 7-77). CONCLUSIONS The placing of a 100 mg diclofenac suppository at the end of a haemorrhoidectomy reduces urinary retention and rescue analgesia requirements.

Abstract:Hypertension, Ahead of Print. BACKGROUND:Irregularities in sleep duration and sleep timing have emerged as potential risk factors for hypertension. This study examined associations between irregularity in sleep duration and timing with hypertension in a large, global sample over multiple months.METHODS:Data from 12 287 adults, who used an under-mattress device to monitor sleep duration and timing and also provided blood pressure recordings on ≥5 separate occasions, were analyzed. Sleep duration irregularity was assessed as the SD in total sleep time across the ≈9-month recording period. Sleep timing irregularity was assessed as SDs in sleep onset time, sleep midpoint, and sleep offset time. Logistic regressions were conducted to investigate associations between sleep irregularity and hypertension, defined as median systolic blood pressure ≥140 mm Hg or median diastolic blood pressure ≥90 mm Hg.RESULTS:Participants were middle-aged (mean±SD, 50±12 years), mostly men (88%) and overweight (body mass index, 28±6 kg/m−2). Sleep duration irregularity was consistently associated with an ≈9% to 17% increase in hypertension independently of the total sleep time. A ≈34-minute increase in sleep onset time irregularity was associated with a 32% increase in hypertension (1.32 [1.20–1.45]). A 32-minute increase in sleep midpoint irregularity was associated with an 18% increase in hypertension (1.18 [1.09–1.29]), while a 43-minute increase in sleep offset time irregularity was associated with an 8.9% increase in hypertension (1.09 [1.001–1.18]).CONCLUSIONS:These findings support that sleep irregularity, both in duration and timing, is a risk marker for poor cardiovascular health. Further mechanistic investigations of temporal relationships between day-to-day fluctuations in sleep duration and timing, next-day blood pressure, and other cardiovascular outcomes are warranted.

Abstract:Objectives Previous studies have reported that sleep duration might increase the risk of hypertension. However, the results have been conflicting. We investigated whether sleep duration is independently associated with hypertension. We aimed to assess the relationship between sleep duration and hypertension in a population-based cross-sectional study. Methods In this study we used multistage stratified cluster sampling. A total of 19 407 adults aged 18–79 years were enrolled in the study. The participants were divided into three groups ( 8 hours/day) according to self-reported sleep duration. Hypertension was defined as systolic blood pressure ≥140 mm Hg or diastolic blood pressure ≥90 mm Hg or the use of anti-hypertensive medications. Univariate and multivariate logistic regressions were performed to determine the association between hypertension and sleep duration adjusted for sociodemographic, body mass index, and lifestyle covariates. Results The overall prevalence of hypertension was 32.6%. Among participants aged 18–44 years, individuals sleeping less than 7 hours per day had a higher risk of hypertension (OR=1.24, 95% CI: 1.05 to 1.46), compared with those who slept 7–8 hours per day. There were no significant associations between sleep duration and hypertension in the total sample, among middle-aged adults (45–59 years) or older adults (60–79 years). Conclusions Our study demonstrates that short sleep duration was significantly associated with hypertension among people aged 18–44 years in northeast China.

Abstract:Aim: Sleep duration has been suggested to be associated with hypertension (HTN). However, evidence of the nature of the relationship and its direction has been inconsistent. Therefore, we performed a meta-analysis to assess the relationship between sleep duration and risk of HTN incidence, and to distinguish more susceptible populations. Methods: PubMed, Embase, Scopus, Web of Science, and ProQuest were searched from January 2000 to May 2023 for cohort studies comparing short and long sleep durations with 7-8 hours of sleep for the risk of HTN incidence. Random-effect model (the DerSimonian-Laird method) was applied to pool risk ratios (RR) and 95% confidence interval (CI). Results: We included sixteen studies ranging from 2.4 to 18 years of follow-up duration evaluating HTN incidence in 1,044,035 people. Short sleep duration was significantly associated with a higher risk of developing HTN (HR: 1.07, 95% CI: 1.06-1.09). The association was stronger when the sleep duration was less than 5 hours (HR: 1.11, 95% CI: 1.08-1.14). In contrast to males, females (HR: 1.07, 95% CI: 1.04-1.09) were more vulnerable to developing HTN due to short sleep duration. No significant difference between different follow-up durations and age subgroups was observed. Long sleep duration was not associated with an increased incidence of HTN. Conclusion: Short sleep duration was associated with higher risk of HTN incidence, however, there was no association between long sleep duration and incidence of HTN. These findings highlight the importance of implementing target-specific preventive and interventional strategies for vulnerable populations with short sleep duration to reduce the risk of HTN.

Abstract:Background: Previous studies demonstrated conflicting results about the association of sleep duration and hypertension. Given the potential relationship between sleep quality and hypertension, this study aimed to investigate the interaction of self-reported sleep duration and sleep quality on hypertension prevalence in adult Chinese males. Methods: We undertook a cross-sectional analysis of 4144 male subjects. Sleep duration were measured by self-reported average sleep time during the past month. Sleep quality was evaluated using the standard Pittsburgh Sleep Quality Index. Hypertension was defined as blood pressure level >= 140/90 mm Hg or current antihypertensive treatment. The association between hypertension prevalence, sleep duration, and sleep quality was analyzed using logistic regression after adjusting for basic cardiovascular characteristics. Results: Sleep duration shorter than 8 hours was found to be associated with increased hypertension, with odds ratios and 95% confidence intervals (CIs) of 1.25 (95% CI, 1.03-1.52) for 7 hours, 1.41 (95% CI, 1.14-1.73) for 6 hours, and 2.38 (95% CI, 1.81-3.11) for <6 hours. Using very good sleep quality as the reference, good, poor, and very poor sleep quality were associated with hypertension, with odds ratios of 1.20 (95% CI, 1.01-1.42), 1.67 (95% CI, 1.32-2.11), and 2.32 (95% CI, 1.67-3.21), respectively. More importantly, further investigation of the association of different combinations of sleep duration and quality in relation to hypertension indicated an additive interaction. Conclusions: There is an additive interaction of poor sleep quality and short sleep duration on hypertension prevalence. More comprehensive measurement of sleep should be performed in future studies.
