Abstract:As infectious disease outbreaks emerge, public health agencies often enact vaccination and social distancing measures to slow transmission. Their success depends on not only strategies and resources, but also public adherence. Individual willingness to take precautions may be influenced by global factors, such as news media, or local factors, such as infected family members or friends. Here, we compare three modes of epidemiological decision-making in the midst of a growing outbreak using network-based mathematical models that capture plausible heterogeneity in human contact patterns. Individuals decide whether to adopt a recommended intervention based on overall disease prevalence, the proportion of social contacts infected, or the number of social contacts infected. While all strategies can substantially mitigate transmission, vaccinating (or self isolating) based on the number of infected acquaintances is expected to prevent the most infections while requiring the fewest intervention resources. Unlike the other strategies, it has a substantial <em>herd effect</em>, providing indirect protection to a large fraction of the population.

Abstract:<p>In the outbreak of infectious diseases such as COVID-19, social media channels are important tools for the public to obtain information and form their opinions on infection risk, which can affect their disease prevention behaviors and the consequent disease transmission processes. However, there has been a lack of theoretical investigation into how social media and human behaviors jointly affect the spread of infectious diseases. In this study, we develop an agent-based modeling framework that couples (1) a general opinion dynamics model that describes how individuals form their opinions on epidemic risk with various information sources, (2) a behavioral adoption model that simulates the adoption of disease prevention behaviors, and (3) an epidemiological SEIR model that simulates the spread of diseases in a host population. Through simulating the spread of a coronavirus-like disease in a hypothetical residential area, the modeling results show that social media can make a community more sensitive to external drivers. Social media can increase the public's awareness of infection risk, which is beneficial for epidemic containment, when high-quality epidemic information exists at the early stage of pandemics. However, fabricated and fake news on social media, after a "latent period", can lead to a significant increase in infection rate. The modeling results provide scientific evidence for the intricate interplay between social media and human behaviors in epidemic dynamics and control, and highlight the importance of public education to promote behavioral changes and the need to correct misinformation and fake news on social media in a timely manner.</p>

The advantage of self-protecting interventions in mitigating epidemic circulation at the community level

Abstract:Protecting interventions of many types (both pharmaceutical and non-pharmaceutical) can be deployed against the spreading of a communicable disease, as the worldwide COVID-19 pandemic has dramatically shown. Here we investigate in detail the effects at the population level of interventions that provide an asymmetric protection between the people involved in a single interaction. Masks of different filtration types, either protecting mainly the wearer or the contacts of the wearer, are a prominent example of these interventions. By means of analytical calculations and extensive simulations of simple epidemic models on networks, we show that interventions protecting more efficiently the adopter (e.g the mask wearer) are more effective than interventions protecting primarily the contacts of the adopter in reducing the prevalence of the disease and the number of concurrently infected individuals ("flattening the curve"). This observation is backed up by the study of a more realistic epidemic model on an empirical network representing the patterns of contacts in the city of Portland. Our results point out that promoting wearer-protecting face masks and other self-protecting interventions, though deemed selfish and inefficient, can actually be a better strategy to efficiently curtail pandemic spreading.

Abstract:The outbreak of an infectious disease in a human population can lead to individuals responding with preventive measures in an attempt to avoid getting infected. This leads to changes in contact patterns. However, as we show in this paper, rational behaviour at the individual level, such as social distancing from infectious contacts, may not always be beneficial for the population as a whole. We use epidemic network models to demonstrate the potential negative consequences at the population level. We take into account the social structure of the population through several network models. As the epidemic evolves, susceptible individuals may distance themselves from their infectious contacts. Some individuals replace their lost social connections by seeking new ties. We show that social distancing can worsen the disease outcome both in the initial phase of an outbreak and the final epidemic size. Moreover, the same negative effect can arise in real-world networks. Our results suggest that one needs to be careful when targeting behavioural changes as they could potentially worsen the epidemic outcome. Furthermore, network structure crucially influences the way that individual-level measures impact the epidemic at the population level. These findings highlight the importance of careful analysis of preventive measures in epidemic models.
Mass behaviour change amid COVID-19: How public health information and social norms explain the transformation

Abstract:COVID-19 is a severe and ongoing threat globally, with the spread disrupting lives and society. Despite the developments of vaccines, the key measure to reduce the transmission of variants has stemmed from mass changes to personal behaviours. COVID-19 pandemic offers a unique context, where the protection behaviours enacted by an individual are necessary to keep the community safe. A social psychological perspective can be used to understand the reasons for adherence to policies and determine what other factors can shape preventive behaviours. To resolve this, in partnership with health consumers we use an online survey, with the findings substantiating preventive behaviours are positively related to COVID-19 information access and descriptive norms. Additionally, findings demonstrate the mediating role of injunctive norms on preventive behaviour suggesting that policy makers can influence decision-making by promoting health information that provides guidance on acceptable behaviours, but also demonstrates subsequent success. The integrity of the model is substantiated by partial least squares (PLS) testing.
Efficient local behavioral change strategies to reduce the spread of epidemics in networks

Abstract:It has recently become established that the spread of infectious diseases between humans is affected not only by the pathogen itself but also by changes in behavior as the population becomes aware of the epidemic; for example, social distancing. It is also well known that community structure (the existence of relatively densely connected groups of vertices) in contact networks influences the spread of disease. We propose a set of local strategies for social distancing, based on community structure, that can be employed in the event of an epidemic to reduce the epidemic size. Unlike most social distancing methods, ours do not require individuals to know the disease state (infected or susceptible, etc.) of others, and we do not make the unrealistic assumption that the structure of the entire contact network is known. Instead, the recommended behavior change is based only on an individual's local view of the network. Each individual avoids contact with a fraction of his/her contacts, using knowledge of his/her local network to decide which contacts should be avoided. If the behavior change occurs only when an individual becomes ill or aware of the disease, these strategies can substantially reduce epidemic size with a relatively small cost, measured by the number of contacts avoided.

Abstract:We show that precise knowledge of epidemic transmission parameters is not required to build an informative model of the spread of disease. We propose a detailed model of the topology of the contact network under various external control regimes and demonstrate that this is sufficient to capture the salient dynamical characteristics and to inform decisions. Contact between individuals in the community is characterised by a contact graph, the structure of that contact graph is selected to mimic community control measures. Our model of city-level transmission of an infectious agent (SEIR model) characterises spread via a (a) scale-free contact network (no control); (b) a random graph (elimination of mass gatherings); and (c) small world lattice (partial to full lockdown -- "social" distancing). This model exhibits good qualitative agreement between simulation and data from the 2020 pandemic spread of coronavirus. Estimates of the relevant rate parameters of the SEIR model are obtained and we demonstrate the robustness of our model predictions under uncertainty of those estimates. The social context and utility of this work is identified, contributing to a highly effective pandemic response in Western Australia.

Abstract:The course of an epidemic is not only shaped by infection transmission over face-to-face contacts, but also by preventive behaviour caused by risk perception and social interactions. This study explores the dynamics of coupled awareness and biological infection spread within a two-layer multiplex network framework. One layer embodies face-to-face contacts, with a biological infection transmission following a simple contagion model, the SIR process. Awareness, modelled by the linear threshold model, a complex contagion, spreads over a social layer and induces behaviour that lowers the chance of a biological infection occurring. It may be provoked by the presence of either aware or infectious neighbours. We introduce a novel model combining these influences through a convex combination, creating a continuum between pure social contagion and local risk perception. Simulation of the model shows distinct effects arising from the awareness sources. Also, for convex combinations where both input sources are of importance, awareness cascades that are not attributable to only one of these sources, emerge. Under these conditions, the combination of a small-world face-to-face and a scale-free social layer, but not vice versa, make that the extent of the infections decreases with increasing transmission probability.

Abstract:Epidemic models often heavily simplify the dynamics of human-to-human contacts, but the resulting bias in outbreak dynamics – and hence requirements for control measures – remains unclear. Even if high-resolution temporal contact data were routinely used for modelling, the role of this temporal network structure towards outbreak control is not well characterised. We address this by assessing dynamic networks across varied social settings in three ways. Firstly, we characterised the distribution of retained contacts over consecutive timesteps by developing a novel metric, the “retention index”, which accounts for the change in the number of contacts over consecutive timesteps on a normalised scale with the extremes representing fully static and fully dynamic networks. Secondly, we described the repetition of contacts over the days by estimating the frequency of contact pairs occurring over the study duration. Thirdly, we distinguish the difference between ‘superspreader’ and infectious individuals driving ‘superspreading events’ by estimating the connectivity of an individual (i.e. individual has high connectivity in a timestep if he accounts for 80% of the contacts in the timestep) and the frequency of exhibiting high connectivity. Using 11 networks from 5 settings studied over 3–10 days, we estimated that more than 80% of the individuals in most settings were highly connected for only short periods. This suggests a challenge to identify ‘superspreaders’, and more individuals would need to be targeted as part of outbreak interventions to achieve the same reduction in transmission as predicted from a static network. Taking into account repeated contacts over multiple days, we estimated simple resource planning models might overestimate the number of contacts made by an infector by 20%–70%. In workplaces and schools, contacts in the same department accounted for most of the retained contacts. Hence, outbreak control measures would be better off targeting specific sub-populations in these settings to reduce transmission. In contrast, no obvious type of contact dominated the retained contacts in hospitals, so reducing the risk of disease introduction is critical to avoid disrupting the interdependent work functions. This study identified key epidemiological properties of temporal networks that potentially shape outbreak dynamics and illustrated the need for incorporating such properties in outbreak simulations.

Abstract:Social distancing and isolation have been introduced widely to counter the COVID-19 pandemic. However, more moderate contact reduction policies become desirable owing to adverse social, psychological, and economic consequences of a complete or near-complete lockdown. Adopting a social network approach, we evaluate the effectiveness of three targeted distancing strategies designed to 'keep the curve flat' and aid compliance in a post-lockdown world. These are limiting interaction to a few repeated contacts, seeking similarity across contacts, and strengthening communities via triadic strategies. We simulate stochastic infection curves that incorporate core elements from infection models, ideal-type social network models, and statistical relational event models. We demonstrate that strategic reduction of contact can strongly increase the efficiency of social distancing measures, introducing the possibility of allowing some social contact while keeping risks low. This approach provides nuanced insights to policy makers for effective social distancing that can mitigate negative consequences of social isolation.

Apparent structural changes in contact patterns during COVID-19 were driven by survey design and long-term demographic trends

Abstract:Social contact patterns are key drivers of infectious disease transmission. During the COVID-19 pandemic, differences between pre-COVID and COVID-era contact rates were widely attributed to non-pharmaceutical interventions such as lockdowns. However, the factors that drive changes in the distribution of contacts between different subpopulations remain poorly understood. Here, we present a clustering analysis of 33 contact matrices generated from surveys conducted before and during the COVID-19 pandemic, and analyse key features distinguishing their topological structures. While we expected to identify aspects of pandemic scenarios responsible for these features, our analysis demonstrates that they can be explained by differences in study design and long-term demographic trends. Our results caution against using survey data from different studies in counterfactual analysis of epidemic mitigation strategies. Doing so risks attributing differences stemming from methodological choices or long-term changes to the short-term effects of interventions.

Abstract:The effectiveness of social distancing and other public health interventions for containing the COVID-19 pandemic has been demonstrated. However, whether and how Internet use behaviours can lead to enhanced self-protection and reduced transmission when considered in conjunction with behavioural interventions remains unclear. This study investigated the strength of effective Internet behaviours and its interaction with global public health interventions for controlling the COVID-19 pandemic. We conducted an econometric analysis of multisource infection and policy information, Internet behaviour, and meteorological information from worldwide in a 3-month period. People's Internet behaviours may contribute crucially to pandemic containment. Furthermore, they may help enhance the effects of public health interventions, particularly behavioural interventions. We discussed plausible mechanisms through which Internet behaviours reduce epidemic spread independently or in tandem with behavioural interventions. Further investigation into the heterogeneity of the interventions demonstrates Internet behaviour's significance in heightening the effects of difficult-to-implement, primitive crisis orientation, and specific objectives of interventions. Governments should recognise the importance of the Internet and leverage it in managing social crises. Our findings serve as a reference for the formulation of global public health policy. Specifically, the insights provided herein can facilitate the implementation of strategies for containing ongoing secondary outbreaks of COVID-19 or outbreaks of other emergent infectious diseases.
Effectiveness of communication on epidemic personal protection with community residents via new media during COVID-19 outbreak: Data from China

Abstract:Personal protection measures are the key to COVID-19 epidemic control and prevention in the community. Newer forms of media present useful options for such information transmission. By presenting primary data from 420 residents in mainland China, this exploratory study aims to explore the communication mechanism which utilizes new media for disseminating epidemic-related information in the community during the COVID-19 outbreak, and to assess its effectiveness by identifying effects of this communication on perceptions and behaviours accordingly. Results show China-originated mobile social media such as WeChat, rather than FB or TW are predominantly used and preferred compared with the traditional media, on which residents are more reliant for information, and through which both types of general and specialized purpose information take significant effects on individuals’ perceptions and behaviors toward personal protection. Particularly, the specialized purpose information is more likely to influence individuals’ personal protection behaviours which are also significantly affected by their perceptions. It reveals that information content or new media content can lead to changes in personal protection perceptions and/or behaviours of residents. This article constructs the mechanism framework of COVID-19 epidemic crisis communication on personal protection. Implications and suggestions are then given for more relevant effective community communication strategy making accordingly. Modeling of Network Based Digital Contact Tracing and Testing Strategies for the COVID-19 Pandemic

Abstract:With more than 1.7 million COVID-19 deaths, identifying effective measures to prevent COVID-19 is a top priority. We developed a mathematical model to simulate the COVID-19 pandemic with digital contact tracing and testing strategies. The model uses a real-world social network generated from a high-resolution contact data set of 180 students. This model incorporates infectivity variations, test sensitivities, incubation period, and asymptomatic cases. We present a method to extend the weighted temporal social network and present simulations on a network of 5000 students. The purpose of this work is to investigate optimal quarantine rules and testing strategies with digital contact tracing. The results show that the traditional strategy of quarantining direct contacts reduces infections by less than 20% without sufficient testing. Periodic testing every 2 weeks without contact tracing reduces infections by less than 3%. A variety of strategies are discussed including testing second and third degree contacts and the pre-exposure notification system, which acts as a social radar warning users how far they are from COVID-19. The most effective strategy discussed in this work was combined the pre-exposure notification system with testing second and third degree contacts. This strategy reduces infections by 18.3% when 30% of the population uses the app, 45.2% when 50% of the population uses the app, 72.1% when 70% of the population uses the app, and 86.8% when 95% of the population uses the app. When simulating the model on an extended network of 5000 students, the results are similar with the contact tracing app reducing infections by up to 79%.
The Network Limits of Infectious Disease Control via Occupation-Based Targeting

Abstract:Policymakers commonly employ non-pharmaceutical interventions to manage the scale and severity of pandemics. Of non-pharmaceutical interventions, social distancing policies -- designed to reduce person-to-person pathogenic spread -- have risen to recent prominence. In particular, stay-at-home policies of the sort widely implemented around the globe in response to the COVID-19 pandemic have proven to be markedly effective at slowing pandemic growth. However, such blunt policy instruments, while effective, produce numerous unintended consequences, including potentially dramatic reductions in economic productivity. Here we develop methods to investigate the potential to simultaneously contain pandemic spread while also minimizing economic disruptions. We do so by incorporating both occupational and network information contained within an urban environment, information that is commonly excluded from typical pandemic control policy design. The results of our method suggest that large gains in both economic productivity and pandemic control might be had by the incorporation and consideration of simple-to-measure characteristics of the occupational contact network. However we find evidence that more sophisticated, and more privacy invasive, measures of this network do not drastically increase performance.

Characterising the role of human behaviour in the effectiveness of contact-tracing applications

Abstract:Albeit numerous countries relied on contact-tracing (CT) applications as an epidemic control measure against the COVID-19 pandemic, the debate around their effectiveness is still open. Most studies indicate that very high levels of adoption are required to stop disease progression, placing the main interest of policymakers in promoting app adherence. However, other factors of human behaviour, like delays in adherence or heterogeneous compliance, are often disregarded. To characterise the impact of human behaviour on the effectiveness of CT apps we propose a multilayer network model reflecting the co-evolution of an epidemic outbreak and the app adoption dynamics over a synthetic population generated from survey data. The model was initialised to produce epidemic outbreaks resembling the first wave of the COVID-19 pandemic and was used to explore the impact of different changes in behavioural features in peak incidence and maximal prevalence. The results corroborate the relevance of the number of users for the effectiveness of CT apps but also highlight the need for early adoption and, at least, moderate levels of compliance, which are factors often not considered by most policymakers. The insight obtained was used to identify a bottleneck in the implementation of several apps, such as the Spanish CT app, where we hypothesise that a simplification of the reporting system could result in increased effectiveness through a rise in the levels of compliance.

Abstract:Implementing a lockdown for disease mitigation is a balancing act: Non-pharmaceutical interventions can reduce disease transmission significantly, but interventions also have considerable societal costs. Therefore, decision-makers need near real-time information to calibrate the level of restrictions. We fielded daily surveys in Denmark during the second wave of the COVID-19 pandemic to monitor public response to the announced lockdown. A key question asked respondents to state their number of close contacts within the past 24 hours. Here, we establish a link between survey data, mobility data, and hospitalizations via epidemic modelling. Using Bayesian analysis, we then evaluate the usefulness of survey responses as a tool to monitor the effects of lockdown and then compare the predictive performance to that of mobility data. We find that, unlike mobility, self-reported contacts decreased significantly in all regions before the nation-wide implementation of non-pharmaceutical interventions and improved predicting future hospitalizations compared to mobility data. A detailed analysis of contact types indicates that contact with friends and strangers outperforms contact with colleagues and family members (outside the household) on the same prediction task. Representative surveys thus qualify as a reliable, non-privacy invasive monitoring tool to track the implementation of non-pharmaceutical interventions and study potential transmission paths.

Abstract:Non-pharmaceutical interventions (NPIs) are essential for the effective prevention and control of the COVID-19 pandemic. However, the scenarios for disease transmission are complicated and varied, and it remains unclear how real-world networks respond to the changes in NPIs. Here, we propose a multi-layer network combining structurally fixed social contact networks with a time-varying mobility network, select the COVID-19 outbreak in two metropolitans in China as case studies, and assess the effectiveness of NPIs. Human mobility, both in relatively fixed places and in urban commuting, is considered. Enclosed places are simulated by three different types of social contact networks, while urban commuting is represented by a time-varying commute network. We provide a composite framework that captures the heterogeneity and time variation of the real world and enables us to simulate large populations with low computational costs. We give out a thorough evaluation of the effectiveness of NPIs (i.e., work from home, school closure, close-off management, public transit limitation, quarantine, and mask use) under certain vaccine coverage varying with implementation timing and intensity. Our results highlight the strong correlation between the NPI pattern and the epidemic mitigation effect and suggest important operational strategies for epidemic control.
Modeling the Impact of Social Distancing and Targeted Vaccination on the Spread of COVID-19 through a Real City-Scale Contact Network

Abstract:We use mobile device data to construct empirical interpersonal physical contact networks in the city of Portland, Oregon, both before and after social distancing measures were enacted during the COVID-19 pandemic. These networks reveal how social distancing measures and the public's reaction to the incipient pandemic affected the connectivity patterns within the city. We find that as the pandemic developed there was a substantial decrease in the number of individuals with many contacts. We further study the impact of these different network topologies on the spread of COVID-19 by simulating an SEIR epidemic model over these networks, and find that the reduced connectivity greatly suppressed the epidemic. We then investigate how the epidemic responds when part of the population is vaccinated, and we compare two vaccination distribution strategies, both with and without social distancing. Our main result is that the heavy-tailed degree distribution of the contact networks causes a targeted vaccination strategy that prioritizes high-contact individuals to reduce the number of cases far more effectively than a strategy that vaccinates individuals at random. Combining both targeted vaccination and social distancing leads to the greatest reduction in cases, and we also find that the marginal benefit of a targeted strategy as compared to a random strategy exceeds the marginal benefit of social distancing for reducing the number of cases. These results have important implications for ongoing vaccine distribution efforts worldwide.

Abstract:While several non-pharmacological measures have been implemented for a few months in an effort to slow the coronavirus disease (COVID-19) pandemic in the United States, the disease remains a danger in a number of counties as restrictions are lifted to revive the economy. Making a trade-off between economic recovery and infection control is a major challenge confronting many hard-hit counties. Understanding the transmission process and quantifying the costs of local policies are essential to the task of tackling this challenge. Here, we investigate the dynamic contact patterns of the populations from anonymized, geo-localized mobility data and census and demographic data to create data-driven, agent-based contact networks. We then simulate the epidemic spread with a time-varying contagion model in ten large metropolitan counties in the United States and evaluate a combination of mobility reduction, mask use, and reopening policies. We find that our model captures the spatial-temporal and heterogeneous case trajectory within various counties based on dynamic population behaviors. Our results show that a decision-making tool that considers both economic cost and infection outcomes of policies can be informative in making decisions of local containment strategies for optimal balancing of economic slowdown and virus spread.
