Abstract:Covid-19 vaccination has posed crucial challenges to policymakers and health administrations worldwide. In addition to the pressure posed by the pandemic, government administration has to strive against vaccine hesitancy, which seems to be considerably higher concerning previous vaccination rollouts. To increase the vaccination protection of the population, Ohio announced a monetary incentive as a lottery for those who decided to vaccinate. This first example was followed by 18 other states, with varying results. In this paper, we want to evaluate the effect of such policies within the potential outcome framework, using the penalized synthetic control method. We treat with a panel dataset and estimate causal effects at a disaggregated level in the context of staggered treatment adoption. We focused on policy outcomes at the county, state, and supra-state levels, highlighting differences between counties with different social characteristics and time frames for policy introduction. We also studied the nature of the treatment effect to see whether the impact of these monetary incentives was permanent or only temporary, accelerating the vaccination of citizens who would have been vaccinated in any case.

Abstract:Booster vaccination remains a key strategy to address the ongoing threat of coronavirus disease 2019 (COVID-19). However, take-up has been slow. By the fall of 2022, less than 50% of eligible US residents had received a booster dose. It is a central tenet in health economics that incentives or penalties are necessary to reach optimal vaccination rates. Six rigorous real-world studies provide evidence that COVID-19 vaccine lotteries cost-effectively raised vaccination rates at an estimated cost of $49 to $82 per additional dose. The 5 studies that found no impact of lotteries used statistical methods that underestimated the impact: They were statistically underpowered to detect a small yet cost-effective impact and did not adequately address selection bias. Vaccine lotteries are cost-effective because they not only provide financial incentives but also influence the public via nonfinancial channels: They garner media attention, tap into social networks, combat procrastination, and signal the importance of sustaining high vaccination rates. In fact, vaccine lotteries are likely to be more effective for booster vaccination than for initial doses because barriers to vaccination are higher. The ongoing threat of COVID-19 presents a unique opportunity to develop and implement innovative, evidence-based public health policies like vaccine lotteries to address current challenges.

Abstract:We evaluated whether the Massachusetts COVID-19 vaccine lottery increased vaccine uptake. We analyzed data from the Centers for Disease Control and Prevention COVID-19 Vaccine Tracker to identify total number of adults aged 18 to 64 who received at least first dose of the COVID-19 vaccine or who were fully vaccinated in Massachusetts, Connecticut, Rhode Island, New Jersey, and Vermont during the study period of March 6 –July 31, 2021. Each of the five states contributed 148 days of a daily report on cumulative number of vaccinated people, comprising 740 state-days as the total sample size. We conducted multivariable, state-day level difference-in-differences (DID) regression using a negative binomial regression model that compared the change in outcomes for Massachusetts to those of four geographically adjacent comparison states without the lotteries, before and after the Massachusetts vaccine lottery announcement (June 15, 2021). Our analyses controlled for key state-level characteristics obtained from the American Community Survey as well as day fixed-effects to capture secular trends in the outcomes. Massachusetts COVID-19 vaccine lottery was not associated with a significant increase in the number of adults aged 18 to 64 who were fully vaccinated or received at least one dose of the vaccine, compared with other states [Full dose, incidence rate ratio (IRR): 1.04, 95% confidence interval (CI): 0.97 to 1.11, P > 0.05; At least one dose, IRR: 0.99, 95% CI: 0.93 to 1.06, P > 0.05]. There was insufficient evidence to conclude that Massachusetts COVID-19 vaccine lottery was associated with increased number of adult COVID-19 vaccinations.

Abstract:Experts debate whether COVID-19 vaccine mandates or financial incentives will reduce, rather than increase, interest in vaccination. Among 3,698 unvaccinated U.S. residents, we conducted a randomized, controlled survey-embedded experiment to estimate the absolute and relative psychological effects of vaccine policies specifying: mandates by employers or airlines, bars, and restaurants; lotteries for $1 million, $200,000, or $100,000; guaranteed cash for $1000, $200, or $100; and $1,000 as either a tax credit or penalty. Vaccine intention -the study outcome- predicts uptake and provides insight into the psychological mechanism that is most proximal to behavior (i.e., vaccination). Compared to controls, those who learned about the $1,000 cash reward policy were 17.1 (±5.3)% more likely to want vaccination. Employer mandates are more promising than other mandate policies (8.6 [+/- 7.4]% vs. 1.4 [+/- 6.0]%). The full results suggest that neither mandates nor financial incentives are likely to have counterproductive psychological effects. These policies are not mutually exclusive and, if implemented well, they may increase vaccine uptake.

Abstract:Some people did not get the COVID-19 vaccine even though it was offered at no cost. A monetary incentive might lead people to vaccinate, although existing studies have provided different findings about this effect. We investigate how monetary incentives differ according to individual characteristics. Using panel data with online experiments, we found that (1) subsidies reduced vaccine intention but increased it after controlling heterogeneity; (2) the stronger the social image against the vaccination, the lower the monetary incentive; and (3) persistently unvaccinated people would intend to be vaccinated only if a large subsidy was provided.

Abstract:Recent evidence suggests that vaccination hesitancy is too high in many countries to sustainably contain COVID-19. Using a factorial survey experiment administered to 20,500 online respondents in Germany, we assess the effectiveness of three strategies to increase vaccine uptake, namely, providing freedoms, financial remuneration, and vaccination at local doctors. Our results suggest that all three strategies can increase vaccination uptake on the order of two to three percentage points (PP) overall and five PP among the undecided. The combined effects could be as high as 13 PP for this group. The returns from different strategies vary across age groups, however, with older cohorts more responsive to local access and younger cohorts most responsive to enhanced freedoms for vaccinated citizens.

Abstract:Using the behavioral economics framework, the current study assessed the effectiveness of compliance gaining techniques and social normative influences to reduce vaccine hesitancy and increase vaccination behavior among college students. A total of 1,283 students responded to a cross-sectional survey assessing the influence of compliance gaining techniques and normative pressures upon vaccine attitudes and behavior. Findings suggest that being female, a person of color, and politically liberal were associated with increased likelihood of vaccination behavior. Likelihood of getting vaccinated was influenced by previous influenza vaccine behavior and parents' vaccination status, suggesting the importance of parental social norms. Compliance gaining techniques may have strengthened attitudes for unvaccinated students but were less successful at motivating actual vaccine behavior.

Abstract:Many Americans fail to get life-saving vaccines each year, and the availability of a vaccine for COVID-19 makes the challenge of encouraging vaccination more urgent than ever. We present a large field experiment (N = 47,306) testing 19 nudges delivered to patients via text message and designed to boost adoption of the influenza vaccine. Our findings suggest that text messages sent prior to a primary care visit can boost vaccination rates by an average of 5%. Overall, interventions performed better when they were 1) framed as reminders to get flu shots that were already reserved for the patient and 2) congruent with the sort of communications patients expected to receive from their healthcare provider (i.e., not surprising, casual, or interactive). The best-performing intervention in our study reminded patients twice to get their flu shot at their upcoming doctor's appointment and indicated it was reserved for them. This successful script could be used as a template for campaigns to encourage the adoption of life-saving vaccines, including against COVID-19.

Using an ecological dataset of lottery ticket sales aggregated by postcode, we test two corollaries of the rollover effect. Abstract:While COVID-19 vaccines are generally available, not all people receive vaccines. To reach herd immunity, most of a population must be vaccinated. It is, thus, important to identify factors influencing people's vaccination preferences, as knowledge of these preferences allows for governments and health programs to increase their vaccine coverage more effectively. Fortunately, vaccination data were collected by U.S. Census Bureau in partnership with the CDC via the Household Pulse Survey (HPS) for Americans. This study presents the first analysis of the 24 vaccination datasets collected by the HPS from January 2021 to May 2022 for 250 million respondents of different ages, genders, sexual orientations, races, education statuses, marital statuses, household sizes, household income levels, and resources used for spending needs, and with different reasons for not receiving or planning to receive a vaccine. Statistical analysis techniques, including an analysis of variance (ANOVA), Tukey multiple comparisons test, and hierarchical clustering (HC), were implemented to analyze the HPS vaccination data in the R language. It was found that sexual orientation, gender, age, and education had statistically significant influences on the vaccination rates. In particular, the gay/lesbian group showed a higher vaccination rate than the straight group; the transgender group had a lower vaccination rate than either the female or the male groups; older respondents showed greater preference for vaccination; respondents with higher education levels also preferred vaccination. As for the other factors that were not significant enough to influence vaccinations in the ANOVA, notable trends were found. Asian Americans had higher vaccination rates than other races; respondents from larger household sizes had a lower chance of getting vaccinated; the unmarried group showed the lowed vaccination rate in the marital category; the respondents depending on borrowed money from the Supplemental Nutrition Assistance Program (SNAP) showed a lower vaccination rate than people with regular incomes. Concerns regarding the side-effects and the safety of the vaccines were the two major reasons for vaccination hesitance at the beginning of the pandemic, while having no trust in the vaccines and no trust in the government became more common in the later stage of the pandemic. The findings in this study can be used by governments or organizations to improve their vaccination campaigns or methods of combating future pandemics.

Abstract:Multiple COVID-19 vaccines were proven to be safe and effective in curbing severe illness, but despite vaccine availability, vaccination rates were relatively low in the United States (U.S.). To better understand factors associated with low COVID-19 vaccine uptake in the U.S., our study provides a comprehensive, data-driven population-level statistical analysis at the county level. We find that political affiliation, as determined by the proportion of votes received by the Republican candidate in the 2020 presidential election, has the strongest association with our response variable, the percent of the population that received no COVID-19 vaccine. The next strongest association was median household income, which has a negative association. The percentage of Black people and the average number of vehicles per household are positively associated with the percent unvaccinated. In contrast, COVID-19 infection rate, percentage of Latinx people, postsecondary education percentage, median age, and prior non-COVID-19 childhood vaccination coverage are negatively associated with percent unvaccinated. Unlike previous studies, we do not find significant relationships between cable TV news viewership or Twitter misinformation variables with COVID-19 vaccine uptake. These results shed light on some factors that may impact vaccination choice in the U.S. and can be used to target specific populations for educational outreach and vaccine campaign strategies in efforts to increase vaccination uptake.

Abstract:Objectives. To assess multilevel factors associated with variation in COVID-19 vaccination rates in a US network of community health centers. Methods. Using multilevel logistic regression with electronic health record data from ADVANCE (Accelerating Data Value Across a National Community Health Center Network; January 1, 2022–December 31, 2022), we assessed associations between health care delivery site–level (n = 1219) and patient-level (n = 1 864 007) characteristics and COVID-19 primary vaccine series uptake. Results. A total of 1 337 440 patients completed the COVID-19 primary vaccine series. Health care delivery site characteristics were significantly associated with lower series completion rates, including being located in non-Medicaid expansion states and isolated or rural communities and serving fewer patients. Patient characteristics associated with significantly lower likelihood of completing the vaccine series included being Black/African American or American Indian/Alaska Native (vs White), younger age, lower income, being uninsured or publicly insured (vs using private insurance), and having fewer visits. Conclusions. Both health care delivery site– and patient-level factors were significantly associated with lower COVID-19 vaccine uptake. What Drives Demand for State-Run Lotteries? Evidence and Welfare Implications

Abstract:Abstract We use natural experiments embedded in state-run lotteries and a new nationally representative survey to provide reduced-form and structural estimates of risk preferences and behavioural biases in lottery demand, and to explore the implications for optimal lottery design. We find that sales respond more to the expected value of the jackpot than to price but are unresponsive to variation in the second prize—a pattern that is consistent with probability weighting but is inconsistent with standard parameterizations. In the survey, we find that lottery spending decreases modestly with income and is strongly associated with measures of innumeracy, poor statistical reasoning, and other proxies for behavioural bias, which also decline with income. Regression predictions suggest that Americans would spend 43% less on lotteries if they were unbiased, while the remaining lottery demand is due to other factors such as anticipatory utility or entertainment value. We use these empirical moments to estimate a model of socially optimal lottery design. In the model, current multi-state lottery designs increase welfare but may harm heavy spenders.

Abstract:In parts of the United States, COVID-19 vaccination rates remained low until late in Fall 2021 owing to both limited vaccine access and hesitancy. With colliding epidemics of RSV, flu, and COVID-19 in the winter, the retrospective evaluation of vaccine incentive policies is needed to inform future routine immunization campaigns. The Massachusetts companion program is one example of a policy that could boost vaccine uptake among older populations. Our regression discontinuity analysis suggests that the program was associated with an increase of up to 22 percentage points in the proportion of individuals aged 75 and older who have been fully vaccinated. Going forward, similar intervention strategies could be invaluable in scenarios where household contacts pose the greatest risk of transmission or where social ties can strongly influence individual decision-making.

Bandwagoning, free-riding and heterogeneity in influenza vaccine decisions: An online experiment

Abstract:'Nudge'-based social norms messages conveying high population influenza vaccination coverage levels can encourage vaccination due to bandwagoning effects but also discourage vaccination due to free-riding effects on low risk of infection, making their impact on vaccination uptake ambiguous. We develop a theoretical framework to capture heterogeneity around vaccination behaviors, and empirically measure the causal effects of different messages about vaccination coverage rates on four self-reported and behavioral vaccination intention measures. In an online experiment, N = 1365 UK adults are randomly assigned to one of seven treatment groups with different messages about their social environment's coverage rate (varied between 10% and 95%), or a control group with no message. We find that treated groups have significantly greater vaccination intention than the control. Treatment effects increase with the coverage rate up to a 75% level, consistent with a bandwagoning effect. For coverage rates above 75%, the treatment effects, albeit still positive, stop increasing and remain flat (or even decline). Our results suggest that, at higher coverage rates, free-riding behavior may partially crowd out bandwagoning effects of coverage rate messages. We also find significant heterogeneity of these effects depending on the individual perceptions of risks of infection and of the coverage rates.

Abstract:Abstract While efficacious vaccines have been developed to inoculate against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2; also known as COVID-19), public vaccine hesitancy could still undermine efforts to combat the pandemic. Employing a survey of 1096 adult Americans recruited via the Lucid platform, we examined the relationships between vaccine attributes, proposed policy interventions such as financial incentives, and misinformation on public vaccination preferences. Higher degrees of vaccine efficacy significantly increased individuals’ willingness to receive a COVID-19 vaccine, while a high incidence of minor side effects, a co-pay, and Emergency Use Authorization to fast-track the vaccine decreased willingness. The vaccine manufacturer had no influence on public willingness to vaccinate. We also found no evidence that belief in misinformation about COVID-19 treatments was positively associated with vaccine hesitancy. The findings have implications for public health strategies intending to increase levels of community vaccination.

Abstract:We assess whether the COVID-19 vaccine induces COVID-19 risky behavior (e.g., going to bars and restaurants) and thus reduces vaccine efficacy. A key empirical challenge is the endogeneity bias when comparing risk-taking by vaccination status since people choose whether to get vaccinated. To address this bias, we exploit rich survey panel data on individuals followed before and after vaccine availability over fourteen months in an event study fixed effects model with individual, time, sector, and county-by-time fixed effects and inverse propensity weights. We find evidence that vaccinated persons, regardless of the timing of vaccination, increase their risk-taking activities. The evidence is consistent with the "lulling effect". While vaccine availability may reduce the risk of contracting COVID-19, it also contributes to further spread of the virus by incentivizing risk-taking in the short term.

A Study Examining the Impact of County-Level Demographic, Socioeconomic, and Political Affiliation Characteristics on COVID-19 Vaccination Patterns in Indiana

Abstract:The COVID-19 vaccination campaign resulted in uneven vaccine uptake throughout the United States, particularly in rural areas, areas with socially and economically disadvantaged groups, and populations that exhibited vaccine hesitancy behaviors. This study examines how county-level sociodemographic and political affiliation characteristics differentially affected patterns of COVID-19 vaccinations in the state of Indiana every month in 2021. We linked county-level demographics from the 2016-2020 American Community Survey Five-Year Estimates and the Indiana Elections Results Database with county-level COVID-19 vaccination counts from the Indiana State Department of Health. We then created twelve monthly linear regression models to assess which variables were consistently being selected, based on the Akaike Information Criterion (AIC) and adjusted R-squared values. The vaccination models showed a positive association with proportions of Bachelor's degree-holding residents, of 40-59 year-old residents, proportions of Democratic-voting residents, and a negative association with uninsured and unemployed residents, persons living below the poverty line, residents without access to the Internet, and persons of Other Race. Overall, after April, the variables selected were consistent, with the model's high adjusted R2 values for COVID-19 cumulative vaccinations demonstrating that the county sociodemographic and political affiliation characteristics can explain most of the variation in vaccinations. Linking county-level sociodemographic and political affiliation characteristics with Indiana's COVID-19 vaccinations revealed inherent inequalities in vaccine coverage among different sociodemographic groups. Increased vaccine uptake could be improved in the future through targeted messaging, which provides culturally relevant advertising campaigns for groups less likely to receive a vaccine, and increasing access to vaccines for rural, under-resourced, and underserved populations.

Methods: This study linked data from REACH-US (Race-Related Experiences Associated with COVID-19 and Health in the United States), a nationally representative, online survey of 5475 adults living in the U.S (January-March 2021) to county-level barriers in the COVID-19 Vaccine Coverage Index. County-level vaccination barriers were measured using the COVID-19 Vaccine Coverage Index. Participants reported why they would or would not receive the COVID-19 vaccine in an open-ended item and their responses were coded using thematic analysis. Descriptive statistics and chi-square tests assessed whether reasons for COVID-19 vaccination intentions varied by county-level barriers and whether these distributions varied across racial/ethnic groups. Results: Thematic analysis revealed twelve themes in participants' reasons why they would or would not receive the COVID-19 vaccine. Themes of societal responsibility (9.8% versus 7.7%), desire to return to normal (8.1% versus 4.7%), and trust in science/healthcare/government (7.7% versus 5.1%) were more frequently reported in counties with low/medium barriers (versus high/very high) (p-values < 0.05). Concerns of COVID-19 vaccine side effects/safety/development (25.3% versus 27.9%) and concerns of access/costs/availability/convenience (1.9% versus 3.6%) were less frequently reported in counties with low/medium barriers (versus high/very high) (p-values < 0.05). Trends in the prevalence of these themes varied across racial/ethnic groups (p-values < 0.05). Conclusions: Future pandemic responses should consider potential ways county-level barriers shape reasons for COVID-19 vaccination.
