Abstract:High myopia esotropia is a kind of special strabismus whose pathology is still uncertain. In this paper, the finite element analysis method is applied to examine some physiological hypotheses by modeling the movement of the eyeball and morbid extraocular muscles. The model stems from clinical data. Theoretical analyses are also performed to examine the effect of oblique pulley. Finally, postoperative eyes are simulated to examine the clinical operation criterion. The results show that the distorted initial eye position and weakness of relative muscles may be the main etiology.
Towards Understanding the Aetiology of High Myopic Strabismus Using Mechanical Analysis and Finite Element Modelling

Abstract:It has been widely accepted that the pathology of high myopic esotropia, a special form of strabismus, is still not fully understood. In this study, the mechanical analysis and finite element analysis (FEA) of the oculomotor system was based on clinical MRI data and applied to examine the physiological hypotheses of extra-ocular muscle obliquity and deformation respectively. Our mechanical analysis indicated that the muscular obliquity is not the dominated cause of high myopic strabismus. Next, by simulating the effect of different forces applied to the cross section of each extra-ocular rectus muscles, the corresponding eyeball rotations were quantified on normal eyes, and high myopic eyes with and without strabismus. The model suggests that the limitation of rotation in high myopic strabismic eyes is mainly caused by the extra-ocular muscle deformation instead of, but related with, its obliquity, providing a better understanding of the aetiology of high myopic strabismus. To the best of our knowledge, this is the first mechanical and FEA model developed from clinical data to investigate the aetiology of high myopic strabismus, providing important tools for future studies.
Scleral Cross-Linking Using Riboflavin UVA Irradiation for the Prevention of Myopia Progression in a Guinea Pig Model: Blocked Axial Extension and Altered Scleral Microstructure

Abstract:PurposeTo develop methods of collagen cross-linking (CXL) in the sclera for the treatment of progressive myopia and to investigate the biomechanical and histological changes that occur in as a result.MethodsTwenty 14-day-old guinea pigs were divided into 3 groups: the cross-linking group (CL, n = 8), non cross-linking group (NCL, n = 8), and control group (n = 4). The scleras of the right eyes of the guinea pigs in the CL group were surgically exposed and riboflavin was dropped onto the irradiation zone for 20 seconds prior to ultraviolet-A (UVA) irradiation. The same procedure was conducted on the NCL group but without UVA irradiation. No procedure was conducted on the control group. The right eyes of the guinea pigs in the CL and NCL groups were then fitted with -10.00DS optics for six weeks. Retinoscopy and the axial lengths (AXL) were measured at baseline, and at the second, fourth and sixth weeks post-treatment in all three groups. All animal subjects were euthanized after the sixth week and then biomechanical and histopathological examinations of the scleras were conducted.ResultsThe mean AXL of the NCL group was longer than both the control and CL groups at six weeks (P = 0.001). The mean refractive error in the NCL group was statistically significantly more negative than both the control and the CL groups at six weeks (P = 0.001). The scleral collagen fiber arrangements of the CL and control groups were denser and more regularly distributed than the NCL group. Ultimate stress of the sclera was lowest in the NCL group, followed by the CL then the control group (P<0.05). Ultimate strain (%) of the sclera was lowest in the CL group followed by the NCL and then the control group (P<0.05).ConclusionOur study demonstrates that scleral CXL using riboflavin UVA irradiation effectively prevents the progression of myopia by increasing scleral biomechanical strength in a guinea pig model.
Effects of riboflavin/ultraviolet-A(UVA) scleral crosslinking on the mechanical behavior of the scleral fibroblasts of lens-induced myopia Guinea pigs

Abstract:Myopia is becoming increasingly severe, and studies have shown that the cellular mechanics of scleral fibroblasts are altered following myopia. Scleral UVA-Riboflavin Collagen Crosslinking（sCXL） is a promising treatment for myopia prevention and control of axial growth. Understanding the mechanical properties of scleral fibroblasts is crucial, as it influences the cellular response and limits the extent of molecular deformation triggered. Thus, our study aimed to investigate the effect of mechanical properties of scleral fibroblasts in a lens-induced myopic guinea pig model following sCXL. For this purpose, we performed the 0.1% riboflavin/UVA scleral crosslinking (365 nm,3 mW/cm2,30 min) in the right eyes of guinea pigs in Group CXL. In Group LIM, the right eyes were only administrated negative lens for 6 weeks. No treatment was performed in both eyes of the guinea pigs in group Control. The scleral fibroblasts were isolated and cultured from the scleral tissue at the cross-linking area in Group CXL and the corresponding area in Group LIM and control. The curve of the length of microtubules inhaled by cells under negative pressure was measured by a microaspiration-based isolation technique, and the equilibrium Young's modulus and apparent viscosity of scleral fibroblasts were calculated by formula fitting. The equilibrium Young's modulus of scleral fibroblasts in group CXL was significantly lower than that in the LIM group (P < 0.01, two-sample t-test between pairs）, and there was no significant difference between groups CXL and control. The results show that sCXL can effectively moderate the phenomenon that scleral fibroblasts are not easy to deform after myopia. The apparent viscosity modulus in the CXL group was higher than the groups' control and LIM. Taken together, our data demonstrate the biomechanics of the scleral fibroblasts altered after Riboflavin/UVA scleral collagen cross-linking in a lens-induced myopia model.
The effectiveness and safety of modified posterior scleral reinforcement in Chinese children with high myopia

Abstract:Abstract Background To evaluate the effectiveness and safety of modified posterior scleral reinforcement in controlling the progression of myopia in patients with high myopia and provide reference information on surgical treatment for these patients.Methods The study included 32 patients (55 eyes) with high myopia who were treated with modified posterior scleral reinforcement and were followed up at 3 months, 6 months, 1 year and 2 years after the operation.The axial length, refractive error, best corrected visual acuity (BCVA) and fundus colour photographs after the operation were compared with those before the operation, and whether there were any complications was recorded.Results The axial length of the eye increased by 0.07 mm, 0.12 mm, 0.31 mm and 0.19 mm at 3 months, 6 months, 1 year and 2 years after the operation, respectively. The refractive error increased by 0.05 D, 0.28 D, 0.53 D and 0.50 D at 3 months, 6 months, 1 year and 2 years after the operation, respectively, and the differences were not significant (P > 0.05). There were no statistically significant changes in BCVA, but there was an upward trend. No severe complications occurred.Conclusion Modified posterior scleral reinforcement can effectively delay the worsening of the ocular axis and refractive error, without severe complications in the short term.

Abstract:In recent decades, the prevalence of myopia has been on the rise globally, attributed to changes in living environments and lifestyles. This increase in myopia has become a significant public health concern. High myopia can result in thinning of the sclera and localized ectasia of the posterior sclera, which is the primary risk factor for various eye diseases and significantly impacts patients' quality of life. Therefore, it is essential to explore effective prevention strategies and programs for individuals with myopia. Collagen serves as the principal molecule in the extracellular matrix (ECM) of scleral tissue, consisting of irregular collagen fibrils. Collagen plays a crucial role in myopia progression and control. During the development of myopia, the sclera undergoes a thinning process which is primarily influenced by collagen expression decreased and remodeled, thus leading to a decrease in its biomechanical properties. Improving collagen expression and promoting collagen crosslinking can slow down the progression of myopia. In light of the above, improving collagen expression or enhancing the mechanical properties of collagen fibers via medication or surgery represents a promising approach to control myopia.
Effects of Postweaning Protein Malnutrition on Intestinal Inositol Phosphatase Activities in Normally Weaned and Neonatally Undernourished Rats

Abstract:Postweaning protein malnutrition imposed on normally weaned or neonatally undernourished rats fed a low-protein diet induced retardation of body and small intestinal growth. A sparing effect on intestinal growth as compared to body growth was observed during protein malnutrition. Postweaning protein malnutrition in normally weaned rats resulted in a significant elevation of specific activities of inositol triphosphatase and phytase in duodenum and jejunum without affecting the activity in ileum. On the other hand, protein malnutrition imposed on neonatally undernourished rats resulted in a significant decrease of enzyme activities in small intestinel segments. These results suggest altered activity of intestinal inositol phosphatase in post weaning protein malnutrition with the direction of effects dependent on the neonatal nutritional status.

Abstract:Background: This study aims to assess the efficacy of the scleral collagen cross-linking method using glyceraldehyde solution for prevention of lens-induced axial elongation in New Zealand rabbits and investigate the biochemical and microstructural changes that occur. Methods: The right eyes of New Zealand rabbits aged seven weeks were randomly divided into three groups: the cross-linking group (n = 6), non-crosslinking group (n = 5), and untreated control group (n = 5). Eyes in cross-linking and non-crosslinking groups were treated with a -8.00 Diopter spherical lens over the course of two weeks. The cross-linking effects were achieved by a sub-Tenon's injection of 0.15 ml 0.5 M glyceraldehyde to eyes in the CL group. Ocular parameters were measured on the 1st, 7(th,) and 14th days. Biomechanical testing, light and electronic microscopy were used. Results: Following the cross-linking treatment, eyes in the cross-linking group had a shorter axial length compared to those in the non-crosslinking group (p = 0.006). Collagen fibrils larger than 240 nm were observed in the scleral stroma of cross-linking group, which were absent in the scleral stroma of the non-crosslinking and untreated control group. The mean ultimate stress and Young's modulus was significantly greater in the cross-linking group compared to those in the non-crosslinking and untreated control group (p < 0.05). No histological damage observed in the retina or choroid. Conclusions: This study demonstrates that lens-induced axial elongation in rabbits can be effectively blocked by cross-linking using glyceraldehyde, with anatomical and mechanical modification and no deleterious effects.
Whole globe inflation testing of exogenously crosslinked sclera using genipin and methylglyoxal

Abstract:Exogenous collagen cross-linking has been investigated as method of reinforcing scleral biomechanics, with the goal of counteracting scleral weakening that occurs at the onset of myopia. This study uses whole globe inflation testing to investigate the biomechanical effect of treating posterior sclera with the collagen cross-linking agents methylglyoxal and genipin. Pairs of porcine eyes were treated in four ways. Three groups involved 1% methylglyoxal: two-hour (Group I) or thirty-minute (Group II) incubation of the whole globe, and thirty-minute incubation of only the posterior sclera of the intact eye (Group III). Group IV consisted of a thirty-minute incubation of the posterior sclera in 1% genipin. Following treatment, each eye was subjected to inflation testing under physiological pressure levels (0-150 mmHg); four strain markers on the posterior pole were tracked, providing displacement measurements in two directions. Results were used to derive load versus deformation behavior and to calculate stiffness at 0.25% strain (toe stiffness) and at peak strain (peak stiffness). Toe stiffness of Group I was 4.8 and 1.3 times greater than controls (sagittal and transverse directions, respectively: 5.23 ± 0.39 vs. 0.90 ± 0.08 mHg, P < 0.001; and 3.41 ± 0.19 vs. 1.51 ± 0.22 mHg, P < 0.01; values in mean ± SE). Group II was 7.4 and 4.3 times stiffer than controls (sagittal and transverse directions, respectively: 5.26 ± 0.49 vs. 0.63 ± 0.10 mHg, P < 0.02; and 3.44 ± 0.44 vs. 0.65 ± 0.07 mHg, P < 0.003). Group III was 3.6 and 3.4 times stiffer than controls (sagittal and transverse directions, respectively: 5.21 ± 0.39 vs. 1.13 ± 0.31 mHg, P < 0.01; and 4.94 ± 1.48 vs. 1.13 ± 0.25, P < 0.01), while Group IV was 8.2 and 2.8 times stiffer than controls (sagittal and transverse: 12.36 ± 1.96 vs. 1.35 ± 0.14 mHg, P < 0.01; and 12.45 ± 1.34 vs. 3.27 ± 0.50 mHg, P < 0.05). In all groups, there was no significant difference in peak stiffness after scleral cross-linking (SXL). At low strain, the posterior sclera was stiffer in both measured directions following methylglyoxal and genipin treatments, however at peak strain the treated sclera was not stiffer. Additionally, the saturation level of scleral stiffening by methylglyoxal can be reached within thirty minutes of treatment.
[Long-Term Studies on Clinical Therapeutic Efficiency of Posterior Scleral Reinforcement Surgery].

Abstract:OBJECTIVE:To assess the safety and efficiency of posterior scleral reinforcement surgery by analyzing data collected for more than ten years after the surgery.METHODS:Sixteen patients (32 eyes) with high myopia who underwent scleral reinforcement were followed up for more than ten years (10 to 13 years).RESULTS:Best corrected visual acuity was stable (t = 0.26, P > 0.05). The average axial elongation was 0.056 mm per year postoperation after surgery. The ratio of choroidal neovascularization in the operation group was 3.1% (1/32).CONCLUSIONS:Posterior scleral reinforcement is a safe and effective. This procedure can prevent the development of choroidal neovascularization in high myopia. A control study with a larger group of cases as well as related basic is still required.
Efficacy and safety of scleral crosslinking using poly(ethylene glycol)ether tetrasuccinimidyl glutarate for form-deprivation myopia progression in rabbits

Abstract:Myopia is becoming increasingly prevalent worldwide at an alarming rate. However, no effective treatment is available for inhibiting myopia progression. Materials chemistry advancements have made it possible to regulate mechanical properties and rate of degradation with good compatibility by developing newly crosslinking systems such as the branched polyethylene glycol (PEG) systems. Herein, we presented a PEG molecule with N-hydroxysuccinimide (NHS) ester functional groups at the chain ends as a macromolecular crosslinking agent for the treatment of myopia. We found that the scleral collagen crosslinked with the four-armed star-shaped PEG molecule with NHS ester functional group (4S-PEG) showed better biomechanical properties, increased thermal stability and higher resistance to degradation. 4S-PEG exhibited relatively low cytotoxicity for human fetal scleral fibroblasts. The retrobulbar injection of 4S-PEG at a relatively low concentration (2.5 mM) showed good effective control of the progression of form-deprivation myopia in rabbits. There were no signs of adverse effect or damage by repeated injections with 4S-PEG in rabbits. The results of this work demonstrate that 4S-PEG can serve as a robust macromolecular crosslinking agent and is expected to have promise for application in the treatment of the progression of myopia.
Iontophoresis-assisted accelerated riboflavin/ultraviolet A scleral cross-linking: A potential treatment for pathologic myopia

Abstract:Scleral collagen cross-linking is one of the most promising treatments to control the pathologic process of myopia. However, the exact procedure and its impact on animal models of myopia are still to be explored. We modified the scleral riboflavin/ultraviolet A (UVA) cross-linking procedure with an iontophoresis-assisted drug delivery system and an accelerated UVA irradiation (10 mW/cm2, 9 min) and applied this treatment to an animal model of myopia. Ninety-six New Zealand White rabbits developed relatively stable myopia by visual deprivation and then underwent the modified scleral cross-linking surgery. All the statistics and sample collection were obtained from 4 postoperative time points (1-day, 10-day, 1-month and 3-month groups). We found that the ultimate stress, Young's modulus and physiological Young's modulus of treated myopia sclera were significantly increased and maintained in 4 groups. The abnormal elongation of the myopic eye was effectively controlled 1 month after the treatment and even almost halted 3 months after the treatment. The histochemical assay revealed no notable post-surgery damage or apoptosis in the retina and choroid. Vigorous collagen synthesis was observed in scleral fibroblasts of the treated samples but were rarely observed in the untreated ones under electron microscopy. Furthermore, the remarkable difference in collagen gene expression and protein content between treated and untreated samples also indicated that an alteration in collagen metabolism may be triggered by the treatment. The effectiveness and safety exploration suggested that the modified scleral cross-linking procedure may be a potential method to control the pathologic process of myopia.
Genipin-Crosslinked Donor Sclera for Posterior Scleral Contraction/Reinforcement to Fight Progressive Myopia.

Abstract:PurposeMyopia has become a global public health problem, particularly in East Asia where myopic retinopathy has become one of the leading causes of blindness and visual impairment in the elderly population. The purpose of this study was to evaluate the efficacy of posterior scleral contraction/reinforcement (PSCR) surgery on controlling the progressive elongation of axial length of highly myopic eyes in young patients.MethodsThis is a prospective self-controlled interventional case series. Forty young patients (<18-years old) with progressive high myopia received PSCR with a genipin-crosslinked donor scleral strip for one eye and the fellow eye served as concurrent control without surgery. The main outcome measurement was the change of axial length over 2 to 3 years of follow-up.ResultsImmediately after the surgery, axial length was shortened and subsequently increased by 0.32 mm over the follow-up period. In contrast, axial length of the fellow eyes increased by 0.82 mm over the same period (P < 0.001, paired t-test). PSCR delayed axial elongation in eyes with or without staphyloma. No significant change of visual acuity, cornea refractive power, or retina thickness was noted between the surgery and fellow eyes. None of the patients lost visual acuity compared with the baseline. The procedure was well tolerated with only temporary corneal refractive axis shifts that recovered by the 6-month postsurgical visit.ConclusionsPSCR with genipin-crosslinked sclera is safe and effective to restrain eye globe elongation in young patients within a 2- to 3-year follow-up period.
The efficacy of modified posterior scleral reinforcement with round scleral patches in Chinese children with high myopia

Abstract:Purpose To observe the efficacy of modified Snyder-Thompson posterior scleral reinforcement with round scleral patches in Chinese children with high myopia. Methods This retrospective study included 46 Chinese children with high myopia (72 eyes) who underwent modified Snyder-Thompson posterior scleral reinforcement (PSR) with round scleral patches and 43 Chinese children with high myopia (67 eyes) who wore only spectacles as a control group. Both groups attended a follow-up at 3 years. Axial length (AL), spherical equivalent (SE), best-corrected visual acuity (BCVA), and full ocular assessment results were evaluated at the initial and final visits. Complications were recorded. Results AL had increased by 0.29 ± 0.33 mm in the PSR group and 0.82 ± 0.33 mm in the control group ( P < 0.0001) at the final follow-up. The change in the SE was 0.31 ± 0.81 D in the PSR group and 2.25 ± 1.02 D in the control group ( P < 0.0001). A decrease of 0.02 ± 0.11 LogMAR was found in the control group, and a change of 0.22 ± 0.35 LogMAR was found in the PSR group. No serious complications due to PSR surgery occurred. Conclusions Modified Snyder-Thompson PSR surgery with round scleral patches can effectively limit the progression of axial elongation in Chinese children with high myopia. The operation is safe, causes little damage, and can be customized.

Abstract:There are irreversible eyeball structural changes in high myopic patients. These changes include axial length, corneal radius, anterior chamber depth, fundus degeneration, macula thickness, etc. There is a close relationship between the damage degree of visual function and these changes. The incidence of complications, such as vitreous opacity, posterior vitreous detachment, cataract, glaucoma, posterior staphyloma and retina detachment, is also highly related to the myopia diopter. More and more researches have indicated that the myopia diopter and the level of visual function are affected by multiple factors. It is promising to detect all of these changes by different kinds of methods, and to assess visual function through these changes. By clarifying these changes, it is also useful to distinguish traumatic damage from disease to provide evidence for forensic assessment of eye injuries.
Scleral Ultrastructure And Biomechanical Changes In Rabbits After Negative Lens Application

Abstract:AIM: To address the microstructure and biomechanical changes of the sclera of rabbits after negative lens application by spectacle frame apparatus.METHODS: Five New Zealand rabbits of seven weeks post-natal were treated with -8 D lens monocularly over the course of two weeks. Refractive errors and axial length (AXL) were measured at the 1st, 7th and 14th days of the induction period. Ultrastructure of sclera was determined with electron microscopy. Biomechanical properties were tested by an Instron 5565 universal testing machine.RESULTS: Lens-induced (LI) eyes elongated more rapidly compared with fellow eyes with AXL values of 15.56 +/- 0.14 and 15.21 +/- 0.14 mm (P<0.01). Fibril diameter was significantly smaller in the LI eyes compared with control ones in the inner, middle, and outer layers (inner layer, 63.533 vs 76.467 nm; middle layer, 92.647 vs 123.984 nm; outer layer, 86.999 vs 134.257 nm, P<0.01, respectively). In comparison with control eyes, macrophage-like cells that engulfed fibroblasts, dilated endoplasmic reticulum, and vacuoles in fibroblasts were observed in the inner and middle stroma in the LI eyes. Ultimate stress and Young's modulus were lower in the LI eyes compared with those in the control eyes.CONCLUSION: Negative lens application alters eye growth, and results in axial elongation with changes in scleral ultrastructural and mechanical properties.
Collagen crosslinking of human and porcine sclera

Abstract:PURPOSE: To develop methods of collagen crosslinking the sclera to increase its biomechanical strength for the treatment of progressive myopia.SETTING: Department of Ophthalmology, Technical University of Dresden, Dresden, Germany.METHODS: Sagitally oriented scleral strips of 4.0 mm x 8.0 mm were prepared from 5 human postmortem eyes and 50 porcine cadaver eyes and treated with various crosslinking methods including physical crosslinking by combined riboflavin-ultraviolet A (UVA) or rose bengal/white-light irradiation and chemical crosslinking by incubation with glucose, ribose, glyceraldehyde, and glutaraldehyde solutions. Parallel scleral strips from the same eye were used as untreated controls. After crosslinking, stress-strain measurements of the treated and control scleras were performed using a microcomputer-controlled biomaterial tester.RESULTS: A statistically significant increase in scleral rigidity was found after crosslinking with riboflavin-UVA, with a rise in stress in treated porcine (157%) and human (29%) sclera, and after treatment with glyceraldehyde, with a rise in stress in treated porcine (487%) and human (34%) sclera, and with glutaraldehyde, with a rise in stress in treated porcine (817%) and human sclera (122%) at 8% strain. The other crosslinking methods proved ineffective. The untreated human sclera had a 4-fold higher stiffness than porcine sclera.CONCLUSIONS: Collagen crosslinking induced by riboflavin-UVA, glyceraldehyde, and glutaraldehyde led to a significant increase in biomechanical strength in human and porcine sclera. Using these methods, collagen crosslinking might become a treatment possibility for progressive myopia. Future animal and clinical studies must determine the best application methods and the long-term effects of increased crosslinking on scleral rigidity and prevent potential toxicity or serious side effects.

Cytokine Expression and Biomechanical Characteristics after Posterior Scleral Reinforcement Using Demineralized Bone Matrix and Allogeneic Sclera

Abstract:Background: Myopia is associated with scleral weakness and thinness, leading to visual impairment. Currently, posterior scleral reinforcement (PSR) remains the primary treatment for this condition. However, clinical practice commonly faces challenges such as insufficient donor availability and inadequate strength of allogeneic sclera materials. Therefore, in this study, we evaluated the cytokine expression and biomechanical characteristics of two types of scleral reinforcement materials (demineralized bone matrix (DBM) and allogeneic sclera) to identify the optimal material for PSR. Methods: Seventy-two two-week-old New Zealand rabbits were utilized in this study. Each rabbit eye was assigned to either an experimental group or an untreated group (no surgical intervention), which were further divided into DBM, allogenic sclera, and control groups (surgery without implantation). Samples were analyzed during different postoperative periods including the inflammatory response period at week 2, angiogenesis period at week 4, collagen formation period at week 12, and connective tissue proliferation period at week 24. Refractive power and axial length of the experimental eyes were measured at 2, 4, 12,and 24 weeks postoperatively while implanted slices with attached sclera from the DBM and Sclera group experimental eyes were collected. The same area of sclera was obtained from the sham group for immunohistochemical analysis and western blot detection to analyze levels of bFGF (Basic Fibroblast Growth Factor), CTGF (Connective Tissue Growth Factor), TGF-β (Transforming Growth Factor β),and Collagen I along with respective elasticity modulus and ultimate strength of the implant slice taken. Results: There were no significant differences (P > .05) in axial length and refractive power between the DBM and allogenic groups before 24 weeks, while a significant difference (P < .05) was observed compared to the control group. The levels of bFGF, CTGF, and TGF-β in the DBM and sclera groups were significantly higher than those in the control group (P < .05). After 24 weeks, histological analyses revealed a strong connection between the implants and sclera with collagen formation. The elasticity modulus and ultimate strength of both DBM and scleral groups were significantly higher than those of the control group (P < .05). Furthermore, the DBM group exhibited a higher elastic modulus and ultimate strength compared to the scleral group (P < .05). The synthesis of collagen can be effectively promoted by bFGF, CTGF, and TGF-β, leading to increased elastic modulus and ultimate strength which helps prevent posterior scleral expansion, thereby controlling further axial growth delay complications occurrence. Conclusion: The cytokine expression profile along with biomechanical characteristics make DBM an ideal material for posterior scleral reinforcement due to its low antigenicity, excellent biocompatibility without obvious postoperative rejection reaction as well as its ability to closely associate with autologous sclera making it widely available from various sources.
Crosslinking Enzyme Lysyl Oxidase Modulates Scleral Remodeling in Form-Deprivation Myopia

Abstract:Purpose: Scleral remodeling causes the excessive ocular elongation that underlies myopia. Lysyl oxidase (LOX), a copper-containing amine oxidase, can catalyze collagen and elastin crosslinking. The purpose of this study was to investigate the role of LOX in scleral remodeling in form-deprivation myopia (FDM). Methods: Seventy-five guinea pigs were randomly divided into five groups as follows: a normal control group, an FDM group, an FDM plus β-aminopropionitrile (BAPN) group, an FDM plus TGF-β1 (TGF-β1) group, and an FDM plus vehicle group. A translucent diffuser was used to induce FDM, and intravitreal injection was used to administer BAPN, TGF-β1 or vehicle. The scleral LOX and collagen gene and protein levels and the posterior scleral ultrastructure and biomechanics were measured. Results: In the FDM group, both the scleral LOX and collagen gene and protein levels were significantly lower than those in the control eyes. The collagen fibril diameters were significantly decreased in the FDM group compared with the diameters in the control group. A significant decrease in LOX gene and protein expression was observed after BAPN injection, and an increase was observed after TGF-β1 treatment compared with the levels in the FDM group. Additionally, the scleral collagen fibrils were significantly decreased in the BAPN-treated eyes but increased in the TGF-β1-treated eyes compared with the FDM eyes. The ultimate stress and Young's modulus of the sclera were lowest in the BAPN group, followed by the FDM group and the TGF-β1 group. The ultimate strain (%) of the sclera was lowest in the TGF-β1 group, followed by the FDM group and the BAPN group. Conclusion: LOX expression was significantly lowered in myopic sclera. Modulating LOX expression induced a change in both the scleral collagen fibril diameter and the scleral biomechanics. Therefore, LOX may play a key role in the myopia scleral remodeling procedure.

Abstract:Purpose: To investigate the differences in corneal biomechanical parameter stress-strain index (SSI) among different degrees of myopic eyes in Chinese individuals and to analyze the relevant factors of the SSI. Methods: This study analyzed the right eyes of 240 participants (240 eyes) aged 18-34 years. The participants were divided into low-, moderate-, high-, and ultra-high myopia groups according to their spherical equivalent (SE), with 60 eyes included in each group. Spherical, cylinder, and SE were measured via automatically integrated optometry. Intraocular pressure (IOP) was measured using a non-contact tonometer. AL was measured using an IOLMaster device. Corneal curvature and central corneal thickness (CCT) were measured using a Pentacam. SSI and biomechanical corrected IOP (bIOP) were measured via corneal visualization Scheimpflug technology (Corvis ST). The statistical analyses included one-sample Kolmogorov-Smirnov tests and normal distribution histogram methods, Levene variance homogeneity tests, Pearson's correlation analyses, multiple linear stepwise regression analyses, one-way ANOVA, and LSD t-tests. Results: The mean (+/- SD) age of the 240 participants was (24.97 +/- 4.16) years. The SSI was positively correlated with spherical, cylinder, SE, CCT, IOP, and bIOP and negatively correlated with K1 and AL (r = 0.475, 0.371, 0.497, 0.169, 0.291, 0.144, -0.154, and -0.464, respectively; all p < 0.05), but were not correlated with age, K2, or Km (all p > 0.05). Multiple linear regression analysis performed with SSI as the dependent variable, and spherical, cylinder, K1, CCT, and IOP as independent variables produced the following regression equation: SSI = 0.989 + 0.017 spherical + 0.042 cylinder +0.018 IOP (R (2) = 0.402, F = 31.518, p < 0.001). The SSI values in the low-, moderate-, high-, and ultra-high myopia groups were 0.945 +/- 0.135, 0.940 +/- 0.128, 0.874 +/- 0.110, and 0.771 +/- 0.104, respectively. The values decreased sequentially, and the differences between pairs were statistically significant (all p < 0.05), except for that between the low- and moderate-myopia groups (p > 0.05). Conclusion: SSI decreased with increasing myopia and astigmatism in the Chinese participants. The SSI was significantly lower in high and ultra-high myopia, especially ultra-high myopia. These findings indicate that increased corneal elasticity may be related to the pathogenesis of high and ultra-high myopia.
