Abstract:Background Slow-paced breathing training (6 breaths per minute [BPM]) improves physiological and psychological well-being by inducing relaxation characterized by increased heart rate variability (HRV). However, classic breathing training has a limited target group, and retention rates are very low. Although a gameful approach may help overcome these challenges, it is crucial to enable breathing training in a scalable context (eg, smartphone only) and ensure that they remain effective. However, despite the health benefits, no validated mobile gameful breathing training featuring a biofeedback component based on breathing seems to exist. Objective This study aims to describe the design choices and their implementation in a concrete mobile gameful breathing training app. Furthermore, it aims to deliver an initial validation of the efficacy of the resulting app. Methods Previous work was used to derive informed design choices, which, in turn, were applied to build the gameful breathing training app Breeze. In a pretest (n=3), design weaknesses in Breeze were identified, and Breeze was adjusted accordingly. The app was then evaluated in a pilot study (n=16). To ascertain that the effectiveness was maintained, recordings of breathing rates and HRV-derived measures (eg, root mean square of the successive differences [RMSSDs]) were collected. We compared 3 stages: baseline, standard breathing training deployed on a smartphone, and Breeze. Results Overall, 5 design choices were made: use of cool colors, natural settings, tightly incorporated game elements, game mechanics reflecting physiological measures, and a light narrative and progression model. Breeze was effective, as it resulted in a slow-paced breathing rate of 6 BPM, which, in turn, resulted in significantly increased HRV measures compared with baseline (P<.001 for RMSSD). In general, the app was perceived positively by the participants. However, some criticized the somewhat weaker clarity of the breathing instructions when compared with a standard breathing training app. Conclusions The implemented breathing training app Breeze maintained its efficacy despite the use of game elements. Moreover, the app was positively perceived by participants although there was room for improvement.

Abstract:This study examined the effectiveness of diaphragmatic paced breathing at (0.1Hz) or obtaining biofeedback via a computer-game compared to normal breathing on heart rate variability (HRV). The aim was to determine whether individual differences exist and allow best practice on an individualized basis for relaxation and stress reduction. Six subjects participated in the repeat measures pilot study. HRV data was recorded during every session using HeartMath and analysed using Kubios software. Paced breathing indicated a substantial decrease in heart rate. Maximum change of low frequency power (%) was seen for the balloon condition with lesser change during paced breathing, The most changes between rest and intervention was observed for the very low frequency power (%) with no substantial differences between paced breathing and the balloon intervention. This suggests that the balloon game may more effective when breathing at approximately 0.1Hz and initiates vagal predominance which has been associated with reduced stress.

Abstract:Clinical studies has shown that patients with breathing disorders can have a better standard of living through breathing exercises. To this end, this paper presents an interactive game, Flappy Breath, in which users can learn and practice breathing exercises in a gamified manner. The game is meant to be not only beneficial to users health, but also enjoyable to play and easy to understand. The testing application is a Flappy Bird like game developed for the Android OS. The User inhales and exhales in order to dodge obstacles. This application is targeted on making lung rehabilitation exercises accessible to anyone with an Android device, providing patient breathing data, and a simple interface for exercising. Throughout the applications lifetime, inhaling and exhaling data is saved and can be used to construct a model of the patients breathing cycle. Therefore, the application could provide vital information to aid in patient breathing exercises and lung health.

Abstract:Paced breathing-longer exhalation than inhalation-can show short-term improvement of physiologic responses and affective well-being, though most studies have relied on narrow sample demographics, small samples, and control conditions that fail to address expectancy effects. We addressed these limitations through an app-based experiment where participants were randomly assigned to paced breathing or sham control (hand closure) conditions. We first validated the conditions in an online sample (N = 201; Study 1) and in a lab environment (N = 72; Study 2). In the primary app-based experiment, participants (N = 3,277; Study 3) completed 3 days of baseline assessments that included three check-ins each day in which we obtained heart rate and blood pressure responses using an optic sensor and assessed current stress and emotions. Participants were then randomly assigned to either the paced breathing or hand closure condition for the next 6 days. Relative to baseline days, both conditions were associated with increased positive emotions and perceived coping, and reduced blood pressure. Moreover, the increase in positive emotions and perceived coping was not evident among a comparison sample (N = 2,600) who completed check-ins but did not participate in either of the paced breathing or sham-control conditions. However, their blood pressure declined over time, suggesting that the continual monitoring of one's blood pressure may result in detectable decreases. Our results highlight the importance of designing experiments with appropriately matched control conditions and suggest that changes associated with techniques like paced breathing, in part, may stem from positive incidental features of the technique. Abstract:Although regular meditation practice is linked to numerous mental health and cognitive benefits, it is often difficult for beginners to maintain focus during practice and persevere with the activity over time. To tackle this issue, we externalised the ability of self reporting loss of focus by developing a feedback loop that helps the user track and maintain their concentration levels in a non-invasive manner. We hypothesise that the change in breathing pattern can indicate a loss of concentration and the act of audibly amplifying a person's breathing sounds during meditation can help them regain and maintain focus on their breathing, leading to a more effective meditation session. We present experimental designs and findings towards this end.

Abstract:A key component of stress management and biofeedback training is the use of relaxation exercises, such as slow/deep breathing (6 breaths/minute) in heart coherence exercises (HCEs). Breathing exercises are also increasingly being integrated into smartphones as part of health apps, though their effectiveness in adolescents after acute stress has rarely been validated scientifically. The aim of the current study was to investigate the effectiveness of an app-guided HCE (n = 36) after an acute stress situation (Trier Social Stress Test) compared with natural relaxation (n = 37), among healthy adolescents (aged 11-17 years). Endocrine, autonomic, and psychological stress parameters (cortisol, alpha-amylase, heart rate, heart rate variability, mood) were examined in 73 adolescents (46 female, 27 male; Mage = 13.86, SDage = 1.87). Significant group differences were found in heart rate variability, with higher values in the low frequency band and low-to-high frequency ratio for the HCE condition, possibly indicating improved physiological functions through the stimulation of vagal tone and baroreflex. The use of a general breathing technique (natural and app-guided) also resulted in stronger relaxation reactions in cortisol when controlling for the previous stronger stress reactivity. On the other hand, app-guided slow breathing without a long training may be experienced as more uncomfortable during relaxation. The integration of breathing exercises in health apps for adolescents appears to be useful, offering a helpful and low-threshold coping/relaxation strategy during acute stress situations. Further studies should examine the benefits of app-guided breathing exercises in both psychiatric samples and the general population across a wide age range.

Abstract:To assess the potential effectiveness of playing a simple, casual video game (Flower) in reducing stress relative to participating in a mindfulness-meditation session (body scan) among undergraduate students. Eighty undergraduate student participants (mean age = 19.46 years, SD = 1.43; gender: 48 females, 29 males, 3 preferred not to answer) were assigned to one of two groups in alternating order: one who played Flower and the comparison group who participated in a body scan, each lasting 20 min. Psychological and physiological stress measurements were made before and after each intervention. Self-perceived psychological stress was measured using a modified version of the nine-item Psychological Stress Measure (PSM-9), and physiological stress (heart rate, systolic and diastolic blood pressure) was measured using an electronic blood-pressure cuff. The results were analyzed with a 2 (measurement: pre, post) × 2 (intervention: video game, mindfulness-meditation) mixed model analysis of variance (ANOVA) for each of the four outcome measures. There was a statistically significant reduction (pre- to post-intervention) across all outcome measures. Notably, there was a significant measurement × intervention interaction (p < .001) for the psychological stress measure; participants in the mindfulness-meditation group reported greater stress reduction after the intervention than participants in the video game group. Although these results suggest mindfulness-meditation provides a slight advantage for stress reduction than casual video games, the similarity in reduction across all physiological measures between the two interventions nevertheless suggests casual video games may also be an effective medium. This finding is especially promising given casual video games' accessibility, ease of use, and popularity among students. These results may inform initiatives by colleges and universities to better support students during peak times of stress and especially during the current COVID-19 pandemic.

Abstract:Background: Heart rate variability biofeedback (HRVB) is an established intervention for increasing heart rate variability (HRV) in the clinical context. Using this technique, participants become aware of their HRV through real-time feedback and can self-regulate it. Objective: The aim of this study was 2-fold: first, to develop a serious game that applies the HRVB technique to teach participants to self-regulate HRV and, second, to test the app with participants in a pilot study. Methods: An HRVB app called the FitLab Game was developed for this study. To play the game, users must move the main character up and down the screen, avoiding collisions with obstacles. The wavelength that users must follow to avoid these obstacles is based on the user's basal heart rate and changes in instantaneous heart rate. To test the FitLab Game, a total of 16 participants (mean age 23, SD 0.69 years) were divided into a control group (n=8) and an experimental group (n=8). A 2 × 2 factorial design was used in each session. Participants in the experimental condition were trained in breathing techniques. Results: Changes in the frequency and time domain parameters of HRV and the game's performance features were evaluated. Significant changes in the average RR intervals and root mean square of differences between adjacent RR intervals (RMSSD) were found between the groups (P=.02 and P=.04, respectively). Regarding performance, both groups showed a tendency to increase the evaluated outcomes from baseline to the test condition. Conclusions: The results may indicate that playing different levels leads to an improvement in the game's final score by repeated training. The tendency of changes in HRV may reflect a higher activation of the mental system of attention and control in the experimental group versus the control group. In this context, learning simple, voluntary strategies through a serious game can aid the improvement of self-control and arousal management. The FitLab Game appears to be a promising serious game owing to its ease of use, high engagement, and enjoyability provided by the instantaneous feedback.
AirFlow: designing immersive breathing training games for COPD.

Abstract:Chronic Obstructive Pulmonary Disease (COPD) refers to a collection of lung diseases that result in breathing difficulties. In some cases, the symptoms of COPD can be reduced, by engaging in breathing exercises. Technology can support, and we are developing AirFlow, a suite of interactive computer games, to guide breathing exercises and promote learning. To establish requirements, we interviewed 20 people with COPD, in China, to understand their use of breathing exercises, and learn how technology might fit their lifestyle. The findings informed our design goals. We outline a prototype system, where respiration rate, waveform, and amplitude are captured and used to control a virtual environment. The system will guide users through breathing exercises, and provide training instructions, using a series of games. The immersive environment aims to support a fun and motivating experience, therefore underpinning user confidence.

Abstract:Stress impacts driving-related cognitive functions like attention and decision-making, and may arise in automated vehicles due to non-driving tasks. Unobtrusive relaxation techniques are needed to regulate stress without distracting from driving. Tactile wearables have shown efficacy in stress regulation through respiratory guidance, but individual variations may affect their efficacy. This study assessed slow-breathing tactile guidance under different stress levels on 85 participants. Physiological, behavioral and subjective data were collected. The influence of individual variations (e.g., driving habits and behavior, personality) using logistic regression analysis was explored. Participants could follow the guidance and adjust breathing while driving, but subjective efficacy depended on individual variations linked to different efficiency in using the technique, in relation with its attentional cost. An influence of factors linked to the evaluation of context criticality was also found. The results suggest that considering individual and contextual variations is crucial in designing and using such techniques in demanding driving contexts. In this line some design recommendations and insights for further studies are provided.

Abstract:Mindfulness-based intervention is widely applied for positive change in the general population. It guides people to focus on the unfolding of experience nonjudgmentally through practice to bring cognitive and emotional benefits. However, conventional intervention is hardly accessible for people with limited time and energy. Virtual Reality technology offers a solution for providing automatic and flexible mindfulness intervention. Considering its interactive and immersive nature, VR is an ideal medium for game design, promisingly enhancing people's interest and motivation in practice. However, current studies in VR-based mindfulness intervention still lack explorations in the aspect of gamification. To address the gap, we designed a VR mindfulness serious game with multiple levels and biofeedback-based interaction strategies to help people cultivate mindfulness through gameplay. A randomized controlled experiment was conducted to evaluate the effectiveness and quality of the game. Results showed that the game significantly increased players' mindfulness levels with equivalent effectiveness as traditional approaches. The game also reduced negative emotions and self-reported anxiety and depression levels. Experimental data and user feedback were analyzed for the game's future development.

Abstract:While many methods have been proposed to detect driver stress with high accuracy, few studies have explored how to mitigate stress during driving effectively. This study proposed and evaluated two driver stress intervention methods, i.e., auditory Positive Comments w/o haptic Breathing guidance (BPC and PC). Sixty drivers were randomly assigned to four groups (i.e., no stress, stressful but no intervention-NI, PC, and BPC) and completed a simulated driving task with their physiological, psychological, and behavioral data collected. Driver stress was effectively induced by challenging simulated driving events. Haptic guidance provided by smartwatches efficiently regulated the breathing rate to the target. Engaging in the intervention was associated with increased RMSSD and did not worsen driving performance. Participants perceived moderate to large comfort effects. The complexity of driving scenarios should be considered for choosing interventions. Breathing intervention was less effective when complex maneuvers were required than normal driving. The findings provided implications regarding the design of in-vehicle stress intervention systems for intelligent transportation.

Breathing on the mind: Treating dyspnea and anxiety symptoms with biofeedback in chronic lung disease - A qualitative analysis

Abstract:Rationale: Chronic obstructive pulmonary disease (COPD) is characterized by dysfunctional breathing patterns that contribute to impaired lung function and symptoms of dyspnea, anxiety, and abnormal carbon dioxide (CO2) levels. Objective: The study objective was to measure the acceptability of a new mind-body intervention we developed called Capnography-Assisted, Learned Monitored (CALM) Breathing, implemented before pulmonary rehabilitation. Methods: CALM Breathing is a 4-week (8-session) intervention designed to treat dyspnea and anxiety in adults with COPD by targeting dysfunctional breathing behaviors (guided by end-tidal CO2 levels). CALM Breathing consists of ten core breathing exercises, CO2 biofeedback, and motivational interviewing. Using qualitative methods and semi-structured interviews immediately post-intervention, we evaluated the acceptability and participation process of CALM Breathing. Themes were identified using constant comparative analysis. Results: Sixteen participants were interviewed after receiving CALM Breathing. Three main themes of CALM Breathing were identified: (1) Process of learning self-regulated breathing, (2) Mechanisms of a mind-body intervention, (3) Clinical and implementation outcomes. Conclusions: Positive themes supported the acceptability of CALM Breathing and described participants' process of learning more self-regulated breathing to manage their dyspnea and anxiety. Positive signals from qualitative participant feedback provided support for CALM Breathing as an intervention for COPD, but larger scale efficacy trials are needed.
Contrasting Cognitive, Behavioral, and Physiological Responses to Breathwork vs. Naturalistic Stimuli in Reflective Chamber and VR Headset Environments: Randomized Control Study

Abstract:Background: Virtual reality (VR) and immersive technologies offer mental health benefits, including anxiety reduction, mood enhancement, and cognitive function improvement. Previous research has demonstrated the anxiolytic effects of breathwork and exposure to natural environments, leading to enhanced well-being. Objective: This study examines the effectiveness of rain stimuli and guided breathwork delivered through two distinct systems: MindGym, a reflective chamber, and a traditional VR headset. Methods: Physiological measures, cognitive, and trait and state assessments were collected for 126 participants, randomly assigned to VR (Breathwork), VR (Rain), MindGym (Breathwork), or MindGym (Rain) conditions. Results: Significant improvements were observed across all groups in cognitive performance, anxiety, and mood. The Trail Making Test (RTACC) decreased (ΔM=-30.4, W=5443.00, Z=4.32, P.FDR<.001, RBC=.45), Architex Total Speed improved (ΔM=-76.4, W=6044.00, Z=7.82, P.FDR<.001, RBC=.84), and anxiety (STAI) reduced (ΔM=-3.78, W=5156.50, Z=4.86, P.FDR<.001, RBC=.52). Breathwork conditions showed a greater decrease in breath rate compared to Rain conditions (F(2.48, 208.49)=28.49, P.FDR=.002). Individual differences moderated treatment responses, with MODTAS predicting Awe (b=0.37, SE=0.11, Z=3.28, P.FDR=.004) and Ego Dissolution (b=0.03, SE=0.01, Z=2.98, P.FDR=.015). Openness interacted with stimuli type on anxiety reduction (F(1, 111)=9.58, P.FDR=.038), with higher openness associated with greater anxiety reduction in Breathwork conditions. No significant differences were found between MindGym and VR in effectiveness or immersion. Conclusions: MindGym and VR demonstrated promise as acute anxiolytics, with MindGymgenerated content maintaining effectiveness when translated to VR. This highlights MindGym's potential as a versatile content creation platform for immersive, anxiety-reducing experiences. Individual differences moderated treatment responses, suggesting opportunities for personalized interventions. Future research should explore complex MindGym experiences and adaptation to various delivery systems for accessible, effective anxiety management tools.

Abstract:Breathing is one of the most vital functions for being mentally and emotionally healthy. A growing number of studies confirm that breathing, although unconscious, can be under voluntary control. However, it requires systematic practice to acquire relevant experience and skillfulness to consciously utilize breathing as a tool for self-regulation. After the COVID-19 pandemic, a global discussion has begun about the potential role of emerging technologies in breath-control interventions. Emerging technologies refer to a wide range of advanced technologies that have already entered the race for mental health training. Artificial intelligence, immersive technologies, biofeedback, non-invasive neurofeedback, and other wearable devices provide new, but yet underexplored, opportunities in breathing training. Thus, the current systematic review examines the synergy between emerging technologies and breathing techniques for improving mental and emotional health through the lens of skills development. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology is utilized to respond to the objectives and research questions. The potential benefits, possible risks, ethical concerns, future directions, and implications are also discussed. The results indicated that digitally assisted breathing can improve various aspects of mental health (i.e., attentional control, emotional regulation, mental flexibility, stress management, and self-regulation). A significant finding of this review indicated that the blending of different technologies may maximize training outcomes. Thus, future research should focus on the proper design and evaluation of different digital designs in breathing training to improve health in different populations. This study aspires to provide positive feedback in the discussion about the role of digital technologies in assisting mental and emotional health-promoting interventions among populations with different needs (i.e., employees, students, and people with disabilities).

Abstract:Introduction Medical students have been under immense pressure throughout their studies, impacting their mental health and academic performance. Stress reduction is a fundamental skill that all students require to manage their studies and lives efficiently. Biofeedback devices providing information about physiological states have been shown to aid stress reduction. Methods to reduce stress should be taught to medical students to help them tackle the challenges of medical school. Objectives Our goal was to teach stress reduction methods such as extracurricular activities and paced breathing aided by biofeedback training and its application in simulated healthcare situations to medical students. Methods 15 medical students who completed medical physiology were recruited for an elective course of 7 sessions on practical techniques in stress management. One credit was offered to those who completed the course requirements consisting of participation in sessions and individual biofeedback training. Sessions (classes) consisted of presentations on good sleeping and eating patterns, group simulations of stressful hospital environments, visiting a science centre with interactive displays, an orchestra performance, and nature walks. Before biofeedback training, heart and respiration rates were taken individually by a biofeedback device during the first week of the course. Data was processed using a code created in statistical software. Heartbeats per minute and heart rate variability (HRV) for every 10 seconds were calculated and plotted on a graph. Two measurements were taken with each student: a baseline measurement for 10 minutes and another measurement during controlled breathing paced at 6 breaths per minute for 15 minutes, of which the first 10 minutes were used for calculation and plotting. Students provided narrative feedback in an essay submitted after the course was completed. Results 5 males and 10 females from years 2-5 registered for the elective, and 12 participated in individually scheduled sessions. Heart beats per minute decreased, whereas HRV increased during paced breathing sessions in 83% of them. Most students reported feeling calm and drowsy during the sessions, and 2 students fell asleep by the end. Feedback from 11 students showed that the music session and the science centre visit were the highlights throughout the elective. Improvements recommended were to have a consistent time slot for all sessions and fewer simulations. Conclusions In concert with the literature, biofeedback training seems to be a feasible and effective method for relaxation in medical students. This method could be offered as part of mental health services for students. Data could be used to follow students' progress and identify those requiring extra support. Providing them with avenues to de-stress while emphasizing activities outside medicine could boost their confidence and improve their coping skills. Disclosure of Interest None Declared

Abstract:Designing emotional intelligence training programs requires first testing the effectiveness of techniques targeting its main dimensions. The aim of this study was to investigate the effects of a brief slow-paced breathing (SPB) exercise on psychophysiological variables linked to emotion regulation, namely cardiac vagal activity (CVA), as well as perceived stress intensity, emotional arousal, and emotional valence. A total of 61 participants completed a 5-min SPB exercise and a control condition of a 5-min rest measurement. CVA was indexed with the root mean square of successive differences (RMSSD). Participants were also asked to rate their perceived stress intensity, emotional arousal, and emotional valence. Results showed that CVA was higher during SPB in comparison to the control condition. Contrary to our hypothesis, perceived stress intensity and emotional arousal increased after SPB, and perceived emotional valence was less positive after SPB. This could be explained by experiencing dyspnea (i.e., breathing discomfort), and the need to get acclimatized to SPB. Consequently, we may conclude that although physiological benefits of SPB on CVA are immediate, training may be required in order to perceive psychological benefits.

Abstract:Digital Therapeutics (DTx) are experiencing rapid advancements within mobile and mental healthcare sectors, with their ubiquity and enhanced accessibility setting them apart as uniquely effective solutions. In this evolving context, our research focuses on deep breathing, a vital technique in mental health management, aiming to optimize its application in DTx mobile platforms. Based on well-founded theories, we introduced a gamified and affordance-driven design, facilitating intuitive breath control. To enhance user engagement, we deployed the Mel Frequency Cepstral Coefficient (MFCC)-driven personalized machine learning method for accurate biofeedback visualization. To assess our design, we enlisted 70 participants, segregating them into a control and an intervention group. We evaluated Heart Rate Variability (HRV) metrics and collated user experience feedback. A key finding of our research is the stabilization of the Standard Deviation of the NN Interval (SDNN) within Heart Rate Variability (HRV), which is critical for stress reduction and overall health improvement. Our intervention group observed a pronounced stabilization in SDNN, indicating significant stress alleviation compared to the control group. This finding underscores the practical impact of our DTx solution in managing stress and promoting mental health. Furthermore, in the assessment of our intervention cohort, we observed a significant increase in perceived enjoyment, with a notable 22% higher score and 10.69% increase in positive attitudes toward the application compared to the control group. These metrics underscore our DTx solution's effectiveness in improving user engagement and fostering a positive disposition toward digital therapeutic efficacy. Although current technology poses challenges in seamlessly incorporating machine learning into mobile platforms, our model demonstrated superior effectiveness and user experience compared to existing solutions. We believe this result demonstrates the potential of our user-centric machine learning techniques, such as gamified and affordance-based approaches with MFCC, which could contribute significantly to the field of mobile mental healthcare.
