Abstract:The ageing of the population is reshaping a large part of the economic and social order, with pervasive and transversal consequences that reflect on production, consumption, labour, and especially on welfare. To govern a development of this kind, it is essential to have an innovative approach, capable of stimulating targeted and systematic policies, and capable of facing the structural change of ageing, transforming it from a burden to a resource for our society. Since the beginning of the new millennium, there has been a gradual attention to the phenomenon of demographic ageing at an international level, which has prompted the European Union to proclaim 2012 as the European Year for Active Ageing and Solidarity between Generations. Recommendations from the international organizations to individual countries on adopting active ageing policies led to the creation of the Active Ageing Index (AAI). Istat has extensively contributed to the AAI interacting with its international partners (UNECE and the European Commission).There is no doubt that AAI is a useful tool, both for measuring achieved progress in the area of active ageing, and for promoting the implementation and the evaluation of policies aimed at improving any potential aspect that does not show satisfactory results. AAI consists of four dimensions and 22 indicators. Using nationally representative data, it was first disseminated in Italy as the product of an experiment intended to provide overall and individual domain data on a subnational level placing Istat at the forefront among European statistical institutes.
Active Ageing in Italy: A Systematic Review of National and Regional Policies

Abstract:Active ageing is defined as the process of optimizing opportunities for health, participation and security in order to enhance quality of life as people age. The design of active ageing policies intersects with different overarching societal challenges, especially ageing populations, social rights and sustainability. However, there are no previous attempts to review active ageing policies in the light of these challenges and the international policy objectives and targets that are guiding the international community. The aim of this study is to systematically identify, review and analyse all national and regional policies on active ageing adopted in Italy, by applying a conceptual framework derived from main international policy initiatives in the three areas. The research was conducted in two stages. First, a case study analysis was carried out per each relevant national institution and regional government. Standardised interviews were combined with policy document search, selection and analysis. Second, we performed a policy analysis in the light of a conceptual framework adopted. This latter was composed by nine policy domains, selected and integrated from principles and objectives of three overarching international frameworks on ageing—i.e., the Regional Implementation Strategy (RIS) commitments of the Madrid International Plan of Action on Ageing (MIPAA), social rights—i.e., the European Pillar of Social Rights and sustainability—i.e., the Sustainable Development Goals (SDGs) of the Agenda 2030 for Sustainable Development. Results pointed out that out of the identified nine policy fields, the major intervention areas by Italian policy makers concerned labour market participation, life-long learning, social and economic inequalities, health and well-being. Less attention had been given to issues such as gender and equal opportunities and sustainable cities. This systematic policy review is a milestone for understanding how active ageing policies contribute to address major societal challenges and what domains need further policy development.

Identifying Active Ageing Policy Needs at the Meso-Level

Abstract:Active ageing (AA) is recognised as a concept leading to positive benefits for individuals and society; however, its operationalization at the policy level presents challenges. In particular, while AA implies the interplay between macro, meso, and micro levels, the topic of AA policy needs at the meso level is underresearched. In order to increase knowledge in this area, the present study investigated a total of 154 organisations through a self-administered questionnaire in the Italian Marche region. Of these organisations, 111 reported having implemented 192 AA initiatives. The main policy needs identified concern requests for help in planning and sustainability, as well as economic and human resources. The presence of needs at the policy level was associated with the specific AA domain of volunteering and civic engagement and seemed to be present when initiatives dealt with the reduction of inequalities in terms of generational differences and differences in terms of digital skills. To appropriately manage policy needs, those results call for policy action in order to involve representatives of meso-level organisations through co-decisional policy tools and to promote intergenerational programmes in general and in the field of digitalization in particular, given its benefits for both generations.

Abstract:Abstract Born in Europe as a concept aiming to counteract new demographic and societal challenges, active aging has progressively become a key pillar of an extended welfare state for aging populations in many high-income countries. Needs, interests, and preferences of new aging cohorts are changing, becoming more diverse and requiring a better understanding and greater attention by policy makers, beyond mere social welfare programmes for those with social, economic or health needs. Active aging policies aim at improving individuals’ quality of life by optimizing opportunities for health, participation, and security (WHO 2002), hence unlocking the potential of older people as active citizens in the community and the society. Since the focus is on a multidimensional concept of quality of life, active aging works at the intersection of labour, social, educational, family, infrastructure, and many other policy areas. However, there may be gaps and discrepancies between the concept in itself and its application at the policy level. The purpose of this symposium is to present and discuss how different post-industrial societies are advancing and implementing active aging policies, in the context of overarching societal challenges and competing needs. In this respect, the symposium focuses on four countries representing different traditional welfare state models: Canada, Italy, Poland, and the United Kingdom. These four case studies bring analyses of active aging policies at national and/or regional level, providing a picture of how such policies have been designed, how they evolved and what they have achieved in recent years.

Abstract:An aging population and the low involvement of elderlies in social activities makes the measurement of active aging an important research question, since it gives an insight into the potential of the elderly. Policy agendas in many countries stress the need for active aging in terms of improved health and a greater degree of autonomy. This paper aims to investigate the potential and limits of international comparison for setting policy goals concerning older adults. We use World Health Organization (WHO) micro data from the Study of Global Ageing and Adult Health (SAGE) to measure active aging in five countries demonstrating similar macroeconomic outcomes, namely Russia, India, China, South Africa and Mexico. Following the WHO concept of active aging, we select indicators for three components of active aging (health, participation in social activities, and security) and aggregate them into three sub-indices, which become the outcome index of active aging. Our findings show significant variation in the proportion of actively aging individuals in selected countries (from 89% in China to 44% in South Africa) and in the sub-indices of health, participation and security. We compare our results with macro-level data and conclude that the active aging index could be a useful tool for measuring the proportion of actively aging individuals and understanding the challenges for policy makers in each country. However, one should understand the limits of micro-level data analysis and interpret the results carefully. We also argue that for international comparisons, active aging indexes should be studied with respect to the activity of non-elderlies in each country.

Evaluation of age-specific causes of death in the context of the Italian longevity transition

Abstract:In many low-mortality countries, life expectancy at birth increased steadily over the last century. In particular, both Italian females and males benefited from faster improvements in mortality compared to other high-income countries, especially from the 1960s, leading to an exceptional increase in life expectancy. However, Italy has not become the leader in longevity. Here, we investigate life expectancy trends in Italy during the period 1960–2015 for both sexes. Additionally, we contribute to the existing literature by complementing life expectancy with an indicator of dispersion in ages at death, also known as lifespan inequality. Lifespan inequality underlies heterogeneity over age in populating health improvements and is a marker of uncertainty in the timing of death. We further quantify the contributions of different age groups and causes of death to recent trends in life expectancy and lifespan inequality. Our findings highlight the contributions of cardiovascular diseases and neoplasms to the recent increase in life expectancy but not necessarily to the decrease in lifespan inequality. Our results also uncover a more recent challenge across Italy: worsening mortality from infectious diseases and mortality at older age.

Abstract:Abstract Poland's relatively young population in the past, is aging rapidly, which provokes a growing interest in the realisation of the older population's potential within the framework of the concept of active ageing. From 2012, when the first Governmental Program on Social Participation (ASOS 2012-2013) was introduced, the active aging framework remains one of the dominant strategies in developing and implementing social policies for the older generation. In the current paper, the focus is made on the employment and social participation of older citizens policies, showing the considerable gap in the prioritisation and hence in the outcomes, highlighted both by Active Ageing Index (AAI) indicators and more in-depth analysis. We will discuss the perceived risks and pitfalls of the current long-term ageing policy approach in Poland and provide recommendations for improvement.

Abstract:Background/Objectives Older people with functional limitations find it difficult to age in place alone, without cohabiting with relatives. In light of this, this paper aimed to investigate possible gender differences in this respect among seniors living in Italy. Methods: The study presents findings from the IN-AGE ("Inclusive ageing in place") study carried out in 2019 in this country assessing the ability of seniors aged 65 years and over to carry out basic and instrumental activities of daily living (ADLs and IADLs), in addition to two mobility limitations (going up/down the stairs and bending to pick up an object) and sensory limitations (hearing and eyesight). Qualitative/semi-structured interviews were administered to 120 older people living in three Italian regions (Lombardy, Marche, and Calabria). Quantitative and qualitative analyses were performed by differentiating between genders and among activities carried out autonomously, with help, or not performed (i.e., the senior is "not able"). Possible sources of support were also explored. Results: The main results revealed that cleaning the house, shopping, bathing/showering, and washing the laundry are particularly difficult, with men reporting greater difficulties than women. Moreover, for both genders, the family—especially children—represents the main source of help, in addition to public and private services, but the results differ between males and females. Conclusions: These results can offer insights for policymakers in the development of adequate gender-sensitive policies.

Abstract:Abstract This article examines the evolution of inequalities in life expectancy at 65 and all-cause mortality by socio-economic position (SEP) among Italian workers. Period life tables and negative binomial regression models are used to estimate longevity inequalities. The empirical assessment is carried out on two administrative datasets, one covering the entire population of private sector workers for the years 1990–2019 and the other covering the entire workforce in Turin for the years 1981–2019. We find that social inequalities in life expectancy and all-cause mortality have increased among Italian workers in recent decades. Several robustness checks confirm this worrying trend, especially for men. Longevity has improved for workers in the middle and high SEPs, while it has hardly changed for workers in the lowest SEP. We show that the estimated life expectancy differentials translate into a loss of pension wealth for the most disadvantaged categories, which also increases over time.

Abstract:Alternative pension schemes, and early retirement provisions in particular, can produce different effects on retirement behavior, with significant economic consequences. This paper presents new evidence on the effect of different seniority pension reforms, considering the evolution of an agent-based economy in Italy, with heterogeneous workers whose retirement age depends on expected lifetime incomes. Using dynamic aging methods, we examine behavioral changes along proposed pension reform paths. Our model — calibrated to replicate the main demographic and economic features and retirement dynamics of the Italian economy — is used to estimate the age of retirement, total pension expenditures, pension benefits and the trend of inequality and poverty among pensioners under different policy scenarios. More precisely, we compare the current state of affairs (B) with a reform proposed by the Italian Welfare Minister (M) and with an early introduction of a mixed regime for seniority pensions (A) according to two limiting "retirement behavioral rules." Under the individual rationality hypothesis, M produces slightly higher savings with minor redistributive effects; although it leads to an increase in income concentration, M mitigates poverty problems after 2008. The reform is more effective under family-bounded rationality, but it leads to permanent and more significant increases in income concentration and aggravates the diffusion and intensity of poverty.

Abstract:This study elaborates on a typology of demographic change and tests this definition at the lowest granular level (LAU2, municipality) with official data. This typology distinguishes between fragile and resilient municipalities based on population dynamics (in terms of duration and intensity) over 1991–2021. This study's second aim is to elaborate a spatial autoregressive econometric model to evaluate to what extent and in which direction the rate of participation of potential beneficiaries of the Rural Development Programmes (RDPs) of 2014–2020 is affected by demographic change and other explanatory variables. Regression models compare the results of the OLS (aspatial) and spatial autoregressive models (SAR) of four types of participation rates (all RDP schemes; all LEADER schemes; sectoral schemes of RDP and LEADER; non-sectoral schemes of RDPs and LEADER). This comparison makes it possible to understand the differences between centralised and decentralised management and between sectoral and broader rural-targeted schemes. The results of the models appear attractive in interpreting the role of RDP instruments in different regions and local areas. First, the rate of participation is strongly dependent on macro-regional differences. Regarding the demographic factors at the local level, this study highlights that demographic fragility does not necessarily hamper the use of RDP measures. Conversely, the participation rate in RDP policy schemes seems particularly significant in very fragile areas, whereas significance has yet to be proved in other demographic typologies. This result holds particularly true for the policy uptake of non-sectoral schemes. Furthermore, LEADER decentralised interventions fit the fragile areas more than resilient and vital ones due to the territorially targeted approach followed by the Local Action Groups.

Abstract:Haematomas and recurrent haemarthroses are a common problem in haemophilia patients from early age. Early diagnosis is critical in preventing haemophilic arthritis, and recent years have seen excellent advances in musculoskeletal ultrasound as a diagnostic tool in soft tissue lesions. In this study, we compared the results of ultrasound imaging for the diagnosis of musculoskeletal injuries in haemophilia patients with scores obtained using magnetic resonance (MRI) scans. A total of 61 haemophilia patients aged 4–82 years were included in this study. Both knees and ankles of each patient were assessed using the Gilbert (clinical assessment) and Pettersson scores (X‐ray assessment). Patients with severe haemophilia (n = 30) were examined using ultrasound and MRI (Denver scoring system). Results obtained with ultrasound and MRI in severe patients were correlated using the Pearson test. In patients with severe haemophilia, normal joints were similarly assessed with MRI and ultrasound (κ = 1.000). By component of joint assessment, haemarthrosis was similarly diagnosed with both techniques in all joints (κ = 1.000). A good positive correlation was found between these techniques in detecting and locating synovial hyperplasia (κ = 0.839–1.000, knees and ankles respectively), and erosion of margins (κ = 0.850–1.000). The presence of bone cysts or cartilage loss was better detected with MRI (κ = 0.643–0.552 for knees and ankles, and κ = 0.643–0.462 respectively). Ultrasound is useful in detecting joint bleeds, synovial hyperplasia and joint erosions, with results comparable to those of MRI. A quick and affordable technique, ultrasound imaging may be useful for monitoring joint bleeds and structure normalization and maintenance in routine practice.
An ecological study on health inequalities in the city of Bologna (Emilia-Romagna Region, Northern Italy): bridging knowledge and action

Abstract:Background: since January 2017, a multidisciplinary research group, involving the Local Health Authority, the Municipality, and the University of Bologna, carried out a city-wide action-research project on health inequalities consisting of an ecological study over the years 2011-2015 based on indicators that are routinely available within health and social services. Objectives: to document existing geographical inequalities in health outcomes and use of healthcare services in the city of Bologna (Emilia-Romagna Region, Northern Italy), with the aim to suggest policy action to tackle them. Design: the results of the first phase of the above-mentioned project were reported: five related to the social determinants of health (exposure) and five related to the social determinants of ill-health (outcomes). For each municipal statistical area, the distribution of the exposures as well as rates and Bayesian Relative Risks of the outcomes were plotted on the city map. To evaluate the association between social determinants and health outcomes, Spearman correlation coefficients were estimated. Setting and participants: residents in the city of Bologna aged >=18 years in the period 2011-2015, grouped into 90 statistical areas. Results: a North-South divide was apparent for most of the socioeconomic and ill-health indicators, with a high concentration of adverse outcomes in the North-Western part of the city. Adherence to cancer screening represented an exception, being greater in the areas with higher proportion of unfavourable health outcomes. An inverse association between education level and health outcomes was found. Low family income was weakly to moderately correlated with health outcomes. Proportion of residents in council houses and of the teenage foreign population showed a moderate to strong association with all outcomes, but mortality and screening adherence. Conclusions: an ecological analysis based on data that are routinely collected by local health and social institutions can be effective in revealing the geographical patterns of health inequalities. When accompanied by strategic choices aimed at bridging knowledge and action, this approach may facilitate the direct engagement of local actors towards health equity.
Modelling geographical variations in fertility and population density of Italian and foreign populations at the local scale: a spatial Durbin approach for Italy (2002–2018)

Abstract:Studies on fertility determinants have frequently pointed to the role that socio-economic, cultural and institutional factors play in shaping reproductive behaviours. Yet, little is known about these determinants at an ecological level, although it is widely recognised that demographic dynamics strongly interact with ecosystems. This research responds to the need to enhance the knowledge on variations in fertility across space with an analysis of the relationship between fertility and population density of Italians and foreigners in Italy at the municipal level for the period 2002–2018. Using global and local autocorrelation measures and a spatial Durbin model, we show that there is a negative association between the fertility and population density of the Italian population, while the density of foreigners is correlated with higher fertility. This second result poses new insights on the relationship between space and fertility. Moreover, we find that the features of neighbouring areas, measured by population density, contribute significantly to explaining spatial fertility variation, confirming the importance of the study of spatial diffusion in demographic processes.
A Gender Equality Index for the Italian Regions

Abstract:A gender gap indicator is constructed for the Italian regions, with the aim of highlighting the geographical differences underlying Italy’s backwardness at European and international level. The indicator, which adapts the Gender Equality Index developed by Plantenga et al. (2009) for 25 European countries, considers four dimensions: work (counting both employment and unemployment), income, political and economic representation and use of time. The indicator can be interpreted as a gauge of the progress made towards gender equality. A limited number of regions, led by Piedmont and Emilia Romagna, are approximately half-way down the path, while a larger group is positioned around the Italian average, i.e. one-third down the path. By contrast, all the southern regions (except Sardinia) lag far behind. Overall, the value of the indicator in 2010 does not differ significantly compared with 2005.

Intergenerational Programs Involving Adolescents, Institutionalized Elderly, and Older Volunteers: Results from a Pilot Research-Action in Italy

Abstract:Changes in traditional family structures, public policy arrangements, and new family care patterns are reducing opportunities for interaction between younger and older people in Europe and in Italy, especially when the latter live in residential care facilities. This may bolster age-related stereotypes in both generations and end up with affecting older people’s self-confidence, devaluing their emotional and relational capabilities. “ Let’s Re-Generate ” is a pilot research-action project—based on an intergenerational program carried out in a nursing home in Central Italy—which aimed at prompting institutionalized older people and adolescents in the community to enhance intergenerational ties through various shared activities. Results from focus groups with 25 teenagers and in-depth interviews with 16 older residents and 16 older volunteers provide an in-depth insight on the positive impact of intergenerational programs, which foster the interaction between different ages, help overcome age-related stereotypes, and improve older people’s mental well-being and older volunteers’ generativity. The main recommendation emerging from this study is that intergenerational activities should be integrated in the daily routine of nursing homes, acting as useful tools for fostering older residents’ capability of reacting to dependency and social isolation.

Abstract:<span>Gender equality represents a central topic of our society, and its study is gaining increasing attention in the international panorama. During the last 20 years, various indicators aiming at measuring gender equality have been proposed, but there are no systematic experiences of indicators tailored for a subnational analysis. We propose a regionalization of the most complete and detailed gender equality indicator, the gender equality index (GEI) of the European Institute on Gender Equality, choosing Italy as a case study. The results show how a regionalized approach to gender equality is necessary to set priorities and target regional policy actions.</span>

Monitoring Gender Gaps via LinkedIn Advertising Estimates: the case study of Italy

Abstract:Women remain underrepresented in the labour market. Although significant advancements are being made to increase female participation in the workforce, the gender gap is still far from being bridged. We contribute to the growing literature on gender inequalities in the labour market, evaluating the potential of the LinkedIn estimates to monitor the evolution of the gender gaps sustainably, complementing the official data sources. In particular, assessing the labour market patterns at a subnational level in Italy. Our findings show that the LinkedIn estimates accurately capture the gender disparities in Italy regarding sociodemographic attributes such as gender, age, geographic location, seniority, and industry category. At the same time, we assess data biases such as the digitalisation gap, which impacts the representativity of the workforce in an imbalanced manner, confirming that women are under-represented in Southern Italy. Additionally to confirming the gender disparities to the official census, LinkedIn estimates are a valuable tool to provide dynamic insights; we showed an immigration flow of highly skilled women, predominantly from the South. Digital surveillance of gender inequalities with detailed and timely data is particularly significant to enable policymakers to tailor impactful campaigns.

Abstract:The approach to analysing population ageing and its impacts on the economy has evolved in recent years. There is increasing interest in the development and use of products and services related to gerontechnology as well as other social innovations that may be considered as central parts of the ‘‘silver economy.’’ However, the concept of silver economy is still being formed and requires detailed research. This article proposes a typology of models of the silver economy in the European Union (EU) at the national and regional levels. This typology was created by comparing the Active Ageing Index to the typology of varieties and cultures of capitalism and typology of the welfare states. Practical recommendations for institutions of the EU and directions for further research are discussed.

Abstract:This study aims to tackle the potential effects of population ageing on regional growth dynamics in the EU. We collected information at a NUTS2 level for EU countries from the years 2000 to 2020 to understand the extent to which the ageing of EU society might impact the EU convergence process. Our results emphasize that the ageing of EU society does not seem to harm the possibility of regional economic growth if we think that immigrants can fill openings in local labour markets (left by retired persons), as well as new jobs stemming from new services needed by retired persons (mostly in health and daily care). Our outcomes also stress the relevance of considering the spatial dimension of the analysis to detect the place-based dimension of this dynamic and, hence, the need for tailored policies.
