Abstract:Background and Aims: Current cardiovascular disease (CVD) prevention is based on diagnosis and treatment of specific disease. Little is known for high risk people with CVD at the community level. In rural China, health records of all residents were established after the recent health reforms. This study aims to describe the characters of the rural population with high CVD risk regarding their clinical indicators, disease patterns, drug treatment and adherence.Methods and Results: 17042 (87%) of all the 19500 rural residents in the two townships had valid health records in 2009. We employed a validated tool, the Asian Equation, to screen 8182 (48%) resident health records of those aged between 40-75 years in 2010. Those who were identified with a CVD risk of 20% or higher were selected for a face-to-face questionnaire survey regarding their diagnosed disease and drug treatment. 453 individuals were identified as high risk of CVD, with an average age of 53 years, 62% males, 50% smoking rate and average systolic blood pressure of 161 mmHg. 386 (85%) participated in the survey, while 294 (76%) were diagnosed with and 88 (23%) were suspects of CVD, hypertension, diabetes or hyperlipidaemia. 75 (19%) took drug regularly and 125 (32%) either stopped treatment or missed drugs. The most often used drugs were calcium channel blockers (20%). Only 2% used aspirins and 0.8% used statins. The median costs of drugs were 17 RMB (USD2.66) per month.Conclusion: The majority of the high risk population in our setting of rural China had already been diagnosed with a CVD related disease, but very few took any drugs, and less still took highly effective drugs to prevent CVD. A holistic strategy focused on population with high risk CVD and based on the current China public health reform is suggested in the context of primary care.

Abstract:Irreversible thermoinactivation of immobilized glucose isomerase from Streptomyces olivochromogenes has been mechanistically investigated at the pH‐optimum of enzymatic activity (pH 8.0). Ligands (high fructose corn syrup and the competitive inhibitor xylitol) greatly stabilize the immobilized enzyme at high temperatures. At 90°C in the presence of 2M xylitol, irreversible inactivation of immobilized glucose isomerase is caused by deamidation of its asparagine/glutamine residues. On the basis of the data obtained, it appears that the time‐dependent decay of glucose isomerase activity in industrial bioreactors is brought about by oxidation of the enzyme's cysteine residue and/or heat‐induced deleterious reactions with high fructose corn syrup or its impurities.

Abstract:Introduction: Consumer wearable devices with health and wellness features are increasingly common and may enhance prevention and management of cardiovascular disease. However, the characteristics and attitudes of wearable device users versus non-users are poorly understood. Methods: Wearable Activity Tracking for Comprehensive Healthcare-Integrated Technology (WATCH-IT) was a prospective study of adults aged [≥]18 years receiving longitudinal primary or ambulatory cardiovascular care at one of eleven hospitals within the Mass General Brigham multi-institutional healthcare system between January 2010-July 2021. We invited patients, including wearable users and non-users, to participate via an electronic patient portal. Participants were asked to complete a 20-question survey regarding perceptions and use of consumer wearable devices. Responses were linked to electronic health record data. Multivariable logistic regression was used to identify factors associated with device use. Results: Among 280,834 individuals receiving longitudinal primary or cardiovascular care, 65,842 did not have an active electronic portal or opted out of research contact. Of the 214,992 individuals sent a survey link, 11,121 responded (5.2%), comprising the WATCH-IT patient sample. Most respondents (55.8%) reported current use of a wearable device, and most non-users (95.3%) reported they would use a wearable device if provided at no cost. Although most users (70.2%) had not shared device data with their doctor previously, the majority believed it would be very (20.4%) or moderately (34.4%) important to share device-related health information with providers. In multivariable models, older age (odds ratio [OR] 0.80 per 10-year increase, 95% CI 0.77-0.82), male sex (0.87, 95% CI 0.80-0.95), and heart failure (0.75, 95% CI 0.63-0.89) were associated with lower odds of wearable device use, whereas higher median zip code income (1.08 per 1-quartile increase, 95% CI 1.04-1.12) and care in a cardiovascular medicine clinic (1.17, 95% CI 1.05-1.30) were associated with greater odds of device use. Nearly all respondents (98%) stated they would share device data with researchers studying health outcomes. Conclusions: Within an electronically assembled cohort of patients in primary and cardiovascular medicine clinics with linkage to detailed health records, wearable device use is common. Most users perceive value in wearable data. Our platform may enable future study of the relationships between wearable technology and resource utilization, clinical outcomes, and health disparities.

Abstract:Background Consumer-grade wearable devices enable detailed recordings of heart rate and step counts in free-living conditions. Recent studies have shown that summary statistics from these wearable recordings have potential uses for longitudinal monitoring of health and disease states. However, the relationship between higher resolution physiological dynamics from wearables and known markers of health and disease remains largely uncharacterized. Objective We aimed to derive high-resolution digital phenotypes from observational wearable recordings and to examine their associations with modifiable and inherent markers of cardiometabolic disease risk. Methods We introduced a principled framework to extract interpretable high-resolution phenotypes from wearable data recorded in free-living conditions. The proposed framework standardizes the handling of data irregularities; encodes contextual information regarding the underlying physiological state at any given time; and generates a set of 66 minimally redundant features across active, sedentary, and sleep states. We applied our approach to a multimodal data set, from the SingHEART study (NCT02791152), which comprises heart rate and step count time series from wearables, clinical screening profiles, and whole genome sequences from 692 healthy volunteers. We used machine learning to model nonlinear relationships between the high-resolution phenotypes on the one hand and clinical or genomic risk markers for blood pressure, lipid, weight and sugar abnormalities on the other. For each risk type, we performed model comparisons based on Brier scores to assess the predictive value of high-resolution features over and beyond typical baselines. We also qualitatively characterized the wearable phenotypes for participants who had actualized clinical events. Results We found that the high-resolution features have higher predictive value than typical baselines for clinical markers of cardiometabolic disease risk: the best models based on high-resolution features had 17.9% and 7.36% improvement in Brier score over baselines based on age and gender and resting heart rate, respectively (P<.001 in each case). Furthermore, heart rate dynamics from different activity states contain distinct information (maximum absolute correlation coefficient of 0.15). Heart rate dynamics in sedentary states are most predictive of lipid abnormalities and obesity, whereas patterns in active states are most predictive of blood pressure abnormalities (P<.001). Moreover, in comparison with standard measures, higher resolution patterns in wearable heart rate recordings are better able to represent subtle physiological dynamics related to genomic risk for cardiometabolic disease (improvement of 11.9%-22.0% in Brier scores; P<.001). Finally, illustrative case studies reveal connections between these high-resolution phenotypes and actualized clinical events, even for borderline profiles lacking apparent cardiometabolic risk markers. Conclusions High-resolution digital phenotypes recorded by consumer wearables in free-living states have the potential to enhance the prediction of cardiometabolic disease risk and could enable more proactive and personalized health management.

Abstract:A new splitting method designed for the numerical solutions of stochastic delay Hopfield neural networks is introduced and analysed. Under Lipschitz and linear growth conditions, this split-step θ-Milstein method is proved to have a strong convergence of order 1 in mean-square sense, which is higher than that of existing split-step θ-method. Further, mean-square stability of the proposed method is investigated. Numerical experiments and comparisons with existing methods illustrate the computational efficiency of our method.

Abstract:Risk prediction tools are crucial for population-based management of cardiovascular disease (CVD). However, most prediction models are currently used to assess long-term risk instead of the risk of short-term CVD onset. We developed a Dynamic Risk-based Early wArning Monitoring (DREAM) system using large-scale, real-time electronic health record data from 2010 to 2020 from the CHinese Electronic health Records Research in Yinzhou study. The dynamic risk scores were derived from a 1:5 matched nested case–control set comprising 70,470 individuals (11,745 CVD events) and then validated in a cohort of 81,205 individuals (5950 CVD events). The individuals were Chinese adults aged 40–79 years without a history of CVD at baseline. Eleven predictors related to vital signs, laboratory tests, and health service utilization were selected to establish the dynamic scores. The proposed scores were significantly associated with the subsequent CVD onset (adjusted odds ratio, 1.21; 95% confidence interval, 1.20–1.23). The area under the receiver operating characteristic curves (AUCs) was 0.6010 (0.5929–0.6092) and 0.6021 (0.5937–0.6105) for the long-term 10-year CVD risk <10% and ≥10% groups in the derivation set, respectively. In the long-term 10-year CVD risk ≥10% group in the validation set, the change in AUC in addition to the long-term risk was 0.0235 (0.0155–0.0315). By increasing the risk threshold from 7 to 16 points, the proportion of true subsequent CVD cases among those given alerts increased from 40.61% to 85.31%. In terms of management efficiency, the number needed to manage per CVD case ranged from 2.46 to 1.17 using the risk scores. With the increasing popularity and integration of EHR systems with wearable technology, the DREAM scores can be incorporated into an early-warning system and applied in dynamic, real-time, EHR-based, automated management to support healthcare decision making for individuals, general practitioners, and policymakers.

Abstract:BACKGROUND: Cardiovascular disease (CVD) is a harmful disease that poses a serious threat to human life. By effectively controlling its risk factors, the occurrence and development of CVD can be reduced, and people's health status and quality of life can be improved.METHODS: A total of 36,215 participants were collected from participants of the Early Screening and Comprehensive Intervention Program for High Risk Population of Cardiovascular Disease in Tianjin on July 31, 2017. We analyzed the relationship between CVD risk and personal information, personal and family medical history, biochemical index, and physical fitness index using Pearson's chi-squared test with and without Yates's correction for continuity, and Fisher's exact test. CVD risk-related factors were examined through logistic regression and decision tree analysis.RESULTS: A personal history of hypertension and apoplexy had a contingency coefficient with CVD risk of more than 0.3. A higher risk of CVD was also found to be associated with biochemical markers of cholesterol, low-density lipoprotein cholesterol, and blood sugar. Logistic regression analysis revealed 12 indicators to be influencing factors of CVD, including age, systolic blood pressure (SBP), diastolic blood pressure (DBP), and the number of people aged >90 in the family. Hypertension, SBP, BMI, cholesterol, and blood glucose were associated with five or more other indicators.CONCLUSIONS: The prevalence of CVD risk factors in Tianjin residents is relatively high. Family disease history and individual physical fitness indicators need to be taken into account during CVD screening and intervention, to reduce the risk of CVD.

Abstract:Long-term use of digital devices is critical for successful clinical or research use, but digital health studies are challenged by a rapid drop-off in participation. A nested e-cohort (eFHS) is embedded in the Framingham Heart Study and uses three system components: a new smartphone app, a digital blood pressure (BP) cuff, and a smartwatch. This study aims to identify factors associated with the use of individual eFHS system components over 1-year. Among 1948 eFHS enrollees, we examine participants who returned surveys within 90 days ( n = 1918), and those who chose to use the smartwatch ( n = 1243) and BP cuff ( n = 1115). For each component, we investigate the same set of candidate predictors for usage and use generalized linear mixed models to select predictors ( P < 0.1, P value from Z test statistic), adjusting for age, sex, and time (app use: 3-month period, device use: weekly). A multivariable model with the predictors selected from initial testing is used to identify factors associated with use of components ( P < 0.05, P value from Z test statistic) adjusting for age, sex, and time. In multivariable models, older age is associated with higher use of all system components. Female sex and higher education levels are associated with higher completion of app-based surveys whereas higher scores for depressive symptoms, and lower than excellent self-rated health are associated with lower use of the smartwatch over the 12-month follow-up. Our findings show that sociodemographic and health related factors are significantly associated with long-term use of digital devices. Future research is needed to test interventional strategies focusing on these factors to evaluate improvement in long-term engagement.

Abstract:Aims:Existing electronic health records (EHRs) often consist of abundant but irregular longitudinal measurements of risk factors. In this study, we aim to leverage such data to improve the risk prediction of atherosclerotic cardiovascular disease (ASCVD) by applying machine learning (ML) algorithms, which can allow automatic screening of the population. Methods and results:A total of 215 744 Chinese adults aged between 40 and 79 without a history of cardiovascular disease were included (6081 cases) from an EHR-based longitudinal cohort study. To allow interpretability of the model, the predictors of demographic characteristics, medication treatment, and repeatedly measured records of lipids, glycaemia, obesity, blood pressure, and renal function were used. The primary outcome was ASCVD, defined as non-fatal acute myocardial infarction, coronary heart disease death, or fatal and non-fatal stroke. The eXtreme Gradient boosting (XGBoost) algorithm and Least Absolute Shrinkage and Selection Operator (LASSO) regression models were derived to predict the 5-year ASCVD risk. In the validation set, compared with the refitted Chinese guideline-recommended Cox model (i.e. the China-PAR), the XGBoost model had a significantly higher C-statistic of 0.792, (the differences in the C-statistics: 0.011, 0.006-0.017, P < 0.001), with similar results reported for LASSO regression (the differences in the C-statistics: 0.008, 0.005-0.011, P < 0.001). The XGBoost model demonstrated the best calibration performance (men: Dx = 0.598, P = 0.75; women: Dx = 1.867, P = 0.08). Moreover, the risk distribution of the ML algorithms differed from that of the conventional model. The net reclassification improvement rates of XGBoost and LASSO over the Cox model were 3.9% (1.4-6.4%) and 2.8% (0.7-4.9%), respectively. Conclusion:Machine learning algorithms with irregular, repeated real-world data could improve cardiovascular risk prediction. They demonstrated significantly better performance for reclassification to identify the high-risk population correctly.

Abstract:Abstract Background Cardiovascular risk, e.g. obstructive sleep apnea (OSA), hypertension, frequent ectopic, are susceptible to atrial fibrillation (AF). Smartwearable-based detection for OSA, ectopics, AF and blood pressure have been tested in mobile health technology supported screening and integrated care for atrial fibrillation (mAFA II programme). Objective The present study aimed to investigate the AF occurrence after the cardiovascular risk identified with wearables. Method The risk factors which were monitored by wearables are as follows: i) OSA: more than 80% monitoring measures with 15 < apnea-hypopnea index (AHI) < 30 during sleep (intermediate risk)or with AHI ≥ 30 (high risk); ii) ectopics: the proportion of the premature beats among total monitored heart beats: 0.5%-10% among over heart beats (intermediate risk) or >10% premature beat (high risk); iii) High-risk blood pressure (BP): average 24-hour BP >130/80 mm Hg, reverse-dippers of BP, and pulse pressure over 60mmHg. Subjects aged over 18 years old, who used smart watch/wristband (Huawei Technologies Co., Ltd., Shenzhen, China) across China were enrolled into present study analysis between October 26, 2018 to Dec 14, 2022. The first detection AF episode were analyzed since the subjects were identified with high risk for AF, that was, intermediate- or high risk-OSA, high risk-blood pressure, intermediate- or high risk-ectopic by the wearables. Results There were 4.37 million subjects (78.6% male, mean age, SD, 37 ± 13 years) involving in the screening programme of mAFA II extension cohort. Among these, 611495 intermediate-risk ectopic, 5037 high-risk ectopic, 43777 intermediate-risk OSA, 22886 high-risk OSA, and 25892 high-risk blood pressure were identified (Figure 1). The rates of progressed into AF from identified cardiovascular risk were as follows: 0.97% (5936/611495) of 611495 intermediate-risk ectopic, 4.80% (242/5037) of high-risk ectopic, 1.09% (478/43777) of intermediate-risk OSA, 1.51% (345/22886) of high-risk OSA, and 1.74% (450/25892) of high-risk blood pressure, respectively. Subjects identified with high-risk blood pressure were much likely to be early monitored with AF episode than other risk, with time to first AF (median, interquartile) of 10 (3-56) days (p<0.05). However, the above cardiovascular risk was possibly progressed into AF episode during five months. Conclusion Smartwearables facilitated the detection of risk factors, OSA, hypertension, frequent ectopic, which might progressed into AF during five months, calling for the effective proactive management on cardiovascular risk to reduce AF burden.Subjects involving in risk screening pro

Association of Wearable Device Use With Pulse Rate and Health Care Use in Adults With Atrial Fibrillation

Abstract:Importance: Increasingly, individuals with atrial fibrillation (AF) use wearable devices (hereafter wearables) that measure pulse rate and detect arrhythmia. The associations of wearables with health outcomes and health care use are unknown.Objective: To characterize patients with AF who use wearables and compare pulse rate and health care use between individuals who use wearables and those who do not.Design, Setting, and Participants: This retrospective, propensity-matched cohort study included 90 days of follow-up of patients in a tertiary care, academic health system. Included patients were adults with at least 1 AF-specific International Statistical Classification of Diseases and Related Health Problems, Tenth Revision (ICD-10) code from 2017 through 2019. Electronic medical records were reviewed to identify 125 individuals who used wearables and had adequate pulse-rate follow-up who were then matched using propensity scores 4 to 1 with 500 individuals who did not use wearables. Data were analyzed from June 2020 through February 2021.Exposure: Using commercially available wearables with pulse rate or rhythm evaluation capabilities.Main Outcomes and Measures: Mean pulse rates from measures taken in the clinic or hospital and a composite health care use score were recorded. The composite outcome included evaluation and management, ablation, cardioversion, telephone encounters, and number of rate or rhythm control medication orders.Results: Among 16 320 patients with AF included in the analysis, 348 patients used wearables and 15 972 individuals did not use wearables. Prior to matching, patients using wearables were younger (mean [SD] age, 64.0 [13.0] years vs 70.0 [13.8] years; P < .001) and healthier (mean [SD] CHA2DS2-VASc [congestive heart failure, hypertension, age ≥ 65 years or 65-74 years, diabetes, prior stroke/transient ischemic attack, vascular disease, sex] score, 3.6 [2.0] vs 4.4 [2.0]; P < .001) compared with individuals not using wearables, with similar gender distribution (148 [42.5%] women vs 6722 women [42.1%]; P = .91). After matching, mean pulse rate was similar between 125 patients using wearables and 500 patients not using wearables (75.01 [95% CI, 72.74-77.27] vs 75.79 [95% CI, 74.68-76.90] beats per minute [bpm]; P = .54), whereas mean composite use score was higher among individuals using wearables (3.55 [95% CI, 3.31-3.80] vs 3.27 [95% CI, 3.14-3.40]; P = .04). Among measures in the composite outcome, there was a significant difference in use of ablation, occurring in 22 individuals who used wearables (17.6%) vs 37 individuals who did not use wearables (7.4%) (P = .001). In the regression analyses, mean composite use score was 0.28 points (95% CI, 0.01 to 0.56 points) higher among individuals using wearables compared with those not using wearables and mean pulse was similar, with a -0.79 bpm (95% CI -3.28 to 1.71 bpm) difference between the groups.Conclusions and Relevance: This study found that follow-up health care use among individuals with AF was increased among those who used wearables compared with those with similar pulse rates who did not use wearables. Given the increasing use of wearables by patients with AF, prospective, randomized, long-term evaluation of the associations of wearable technology with health outcomes and health care use is needed.

However, national information about the prevalence and treatment of high cardiovascular disease (CVD) risk is limited.</p></section><section><span>Objective:</span><p>To assess the prevalence and treatment of high CVD risk as well as variations in risk across population subgroups.</p></section><section><span>Design:</span><p>National project of CVD screening and management.</p></section><section><span>Setting:</span><p>141 county-level regions in all 31 provinces of China.</p></section><section><span>Participants:</span><p>Local residents aged 35 to 75 years.</p></section><section><span>Measurements:</span><p>Rates of high CVD risk were assessed both in the overall study population and by age, sex, body mass index, geographic region, and socioeconomic status. Multivariable mixed models were fitted to assess the associations between individual characteristics and high CVD risk. Statin and aspirin use was evaluated among persons at high risk for CVD.</p></section><section><span>Results:</span><p>Among 1 680 126 participants, 9.5% (95% CI, 9.5% to 9.6%) had high risk for CVD. Mixed models identified persons who were of Han ethnicity, had medical insurance, were currently using alcohol, or were obese as more likely to be at high risk for CVD. Of those with high CVD risk, only 0.6% (CI, 0.5% to 0.6%) and 2.4% (CI, 2.3% to 2.5%) reported using statins and aspirin, respectively. Among persons with high CVD risk and hypertension, 31.8% were receiving antihypertensive medications.</p></section><section><span>Limitation:</span><p>Samples were not nationally representative.</p></section><section><span>Conclusion:</span><p>Of the 1.7 million participants, 1 in 10 had a high risk for CVD; among those at high risk, fewer than 3% were receiving statins or aspirin. Abstract:Background: Numerous studies have suggested that the relationship between cardiovascular disease (CVD) risk and the usage of mobile health (mHealth) technology may vary depending on the total number of CVD risk factors present. However, whether higher CVD risk is associated with a greater likelihood of engaging in specific mHealth use among US adults is currently unknown. Objective: We aim to assess the associations between the composite CVD risk and each component of mHealth use among US adults regardless of whether they have a history of CVD or not. Methods: This study used cross-sectional data from the 2017 to 2020 Health Information National Trends Survey. The exposure was CVD risk (diabetes, hypertension, smoking, physical inactivity, and overweight or obesity). We defined low, moderate, and high CVD risk as having 0-1, 2-3, and 4-5 CVD risk factors, respectively. The outcome variables of interest were each component of mHealth use, including using mHealth to make health decisions, track health progress, share health information, and discuss health decisions with health providers. We used multivariable logistic regression models to examine the association between CVD risk and mHealth use adjusted for demographic factors. Results: We included 10,531 adults, with a mean age of 54 (SD 16.2) years. Among the included participants, 50.2% were men, 65.4% were non-Hispanic White, 41.9% used mHealth to make health decisions, 50.8% used mHealth to track health progress toward a health-related goal, 18.3% used mHealth to share health information with health providers, and 37.7% used mHealth to discuss health decisions with health providers (all are weighted percentages). Adults with moderate CVD risk were more likely to use mHealth to share health information with health providers (adjusted odds ratio 1.49, 95% CI 1.24-1.80) and discuss health decisions with health providers (1.22, 95% CI 1.04-1.44) compared to those with low CVD risk. Similarly, having high CVD risk was associated with higher odds of using mHealth to share health information with health providers (2.61, 95% CI 1.93-3.54) and discuss health decisions with health providers (1.56, 95% CI 1.17-2.10) compared to those with low CVD risk. Upon stratifying by age and gender, we observed age and gender disparities in the relationship between CVD risk and the usage of mHealth to discuss health decisions with health providers. Conclusions: Adults with a greater number of CVD risk factors were more likely to use mHealth to share health information with health providers and discuss health decisions with health providers. These findings suggest a promising avenue for enhancing health care communication and advancing both primary and secondary prevention efforts related to managing CVD risk factors through the effective usage of mHealth technology.

Abstract:Background: Identifying high-risk individuals is crucial for preventing cardiovascular diseases (CVDs). Currently, risk assessment is mostly performed by physicians. Mobile health apps could help decouple the determination of risk from medical resources by allowing unrestricted self-assessment. The respective test results need to be interpretable for laypersons. Objective: Together with a patient organization, we aimed to design a digital risk calculator that allows people to individually assess and optimize their CVD risk. The risk calculator was integrated into the mobile health app HerzFit, which provides the respective background information. Methods: To cover a broad spectrum of individuals for both primary and secondary prevention, we integrated the respective scores (Framingham 10-year CVD, Systematic Coronary Risk Evaluation 2, Systematic Coronary Risk Evaluation 2 in Older Persons, and Secondary Manifestations Of Arterial Disease) into a single risk calculator that was recalibrated for the German population. In primary prevention, an individual's heart age is estimated, which gives the user an easy-to-understand metric for assessing cardiac health. For secondary prevention, the risk of recurrence was assessed. In addition, a comparison of expected to mean and optimal risk levels was determined. The risk calculator is available free of charge. Data safety is ensured by processing the data locally on the users' smartphones. Results: Offering a risk calculator to the general population requires the use of multiple instruments, as each provides only a limited spectrum in terms of age and risk distribution. The integration of 4 internationally recommended scores allows risk calculation in individuals aged 30 to 90 years with and without CVD. Such integration requires recalibration and harmonization to provide consistent and plausible estimates. In the first 14 months after the launch, the HerzFit calculator was downloaded more than 96,000 times, indicating great demand. Public information campaigns proved effective in publicizing the risk calculator and contributed significantly to download numbers. Conclusions: The HerzFit calculator provides CVD risk assessment for the general population. The public demonstrated great demand for such a risk calculator as it was downloaded up to 10,000 times per month, depending on campaigns creating awareness for the instrument.
Association of Physical Activity from Wearable Devices and Chronic Disease Risk: Insights from the All of Us Research Program

Abstract:Background: Physical activity is widely recognized as a key modifiable factor for reducing the risk of chronic diseases. Wearable devices such as Fitbit offer a unique opportunity to objectively measure physical activity metrics, providing insights into the association between different types of physical activity and chronic disease risk. Objective: This study aims to examine the association between physical activity metrics derived from Fitbit devices and the incidence of various chronic diseases among participants from the All of Us Research Program. Methods: Physical activity metrics included daily steps, elevation gain, and activity durations at different intensities (e.g., very active, lightly active, and sedentary). Cox proportional hazards models and multiple regression models were used to assess the relationship between these metrics and the incidence of chronic diseases represented by Phecodes. Age, sex, and body mass index (BMI) were included as covariates. Results: A total of 15,538 participants provided Fitbit activity data, of which 9,320 also had electronic health records (EHR). Increased daily step count, elevation gain, and very active minutes were significantly associated with a reduced risk of several chronic conditions, including obesity, Type 2 diabetes, and major depressive disorder. Conversely, increased sedentary time was linked to higher risks for conditions such as obesity, Type 2 diabetes, and essential hypertension. Multiple regression analyses confirmed these associations. Conclusion: Our findings highlight the beneficial effects of increased physical activity, particularly daily steps and elevation gain, on reducing the risk of chronic diseases. Conversely, sedentary behavior remains a significant risk factor for the development of several conditions. These insights may inform personalized activity recommendations aimed at reducing disease burden and improving population health outcomes.

Abstract:Abstract BackgroundDigital health has become a heated topic today and smart homes have received much attention as an important area of digital health. However, most of the existing studies have focused on discussing the impact of smart homes on people or the attitudes of older people towards smart homes. Only few studies have focused on relationship between health-related risks and use of smart homes.AimsTo investigate the association between health-related risks and the use of smart homes, provide new recommendations to promote the implementation of digital health strategies and achieve health for all.MethodsWe used data from 11,031 participants aged 18 and above. The population was clustered based on five health-related risk factors: perceived social support, family health, health literacy, media use, and chronic diseases self-behavioral management. A total of 23 smart homes were categorized into three sub-categories: entertainment smart home, functional smart home, and health smart home. We analyzed demographic characteristics and utilization rate of smart home across different cluster.ResultsThe participants were clustered into three groups: low risk, meddle risk, and high risk. The utilization rate of smart home was the most popular in the low risk group (total smart home: 86.97%; entertainment smart home: 61.07%, functional smart home: 77.42%, and health smart home: 75.33%; p < 0.001). For entertainment smart home, smart TV had the highest utilization rate (low risk: 45.73%; middle risk: 43.52%, high risk: 33.38%, p<0.001). For functional smart home, smart washing machine (low risk: 37.66%, middle risk: 35.11%, high risk: 26.49%; p<0.001) and smart air conditioner (low risk: 35.95%, middle risk: 29.13%, high risk: 24.61%) were higher than other of this category. For health smart home, sports bracelet has the highest utilization rate (low risk: 37.29%, middle risk: 24.49%, high risk: 22.83%).ConclusionHealth-related risks are an important factor affecting the use of smart homes. Joint efforts of government and product manufacturers are needed to broaden the smart home market and promote the implementation of digital health strategies.

Abstract:PurposeRapid developments in information technology have enabled wearable devices to be applied in the health field. In elderly adults, wearable devices aid in data collection and exerts a positive effect on their social capital. This study evaluated the changes in these two parameters among elderly adults using wearable devices, and analyzed the effect of these devices on their daily lives.MethodsWe selected 18 elderly people using wearable devices, between February and May 2017. The data collected by the wearable devices included the number of steps taken, sleep duration, blood pressure, heart rate, respiratory rate, fatigue, and mood of the wearers. Using a questionnaire and the trajectory equifinality model, we interviewed and surveyed elderly adults in order to understand their health status and social capital.ResultsThe health of the participants was generally good, and most were able to achieve > 8000 steps per day (p < 0.05). Mild and moderate fatigue symptoms were noted in elderly adults for 90% of the study period (p < 0.05). The number of steps, blood pressure, and heart rate changed significantly within a month. From the commencement of using the wearable devices, a steady increase was noted in the monthly number of steps. Interviews suggested that the elderly adults perceived wearable devices as having the potential to improve health and social capital.ConclusionsBy using wearable devices, the participants had a better understanding of their own health, and were willing to take health-boosting measures. The participants were also more willing to increase their social capital and expand their social network.

Abstract:Background Consumer wearable devices such as wristbands and smartwatches have potential application value in communicable disease surveillance. Objective We investigated the ability of wearable devices to monitor COVID-19 patients of varying severity. Methods COVID-19 patients with mobile phones supporting wearable device applications were selected from Dalian Sixth People Hospital. Physiological parameters from the wearable devices and electronic questionnaires were collected from the device wearing until 14 days post-discharge. Clinical information during hospitalization was also recorded. Based on imaging data, the patients were categorized into the milder group without pneumonia and the more severe group with pneumonia. We plotted the curves of the physiological parameters of the two groups to compare the differences and changes. Results Ninety-eight patients were included in the analysis. The mean age was 39.6 ± 10.5 years, including 45 males (45.9%). There were 24 asymptomatic patients, 10 mild patients, 60 moderate patients, and 4 severe patients. Compared with the milder group, the more severe group had higher heart rate-related parameters, while the heart rate variability (HRV) was the opposite. In the more severe group, the heart rate-related parameters showed a downward trend from 0 to 7 days after the fever resolution. Among them, the resting heart rate and sleep heart rate decreased on the 25th day after the onset and were close to the milder group 1 week after discharge. Conclusions Consumer wearable devices have the potential to monitor respiratory infections. Heart rate-related parameters obtained from these devices can be sensitive indicators of COVID-19 severity and correlate with disease evolution. Trial registration ClinicalTrials.gov NCT04459637.

Abstract:BACKGROUND: Wearable devices are gaining increasing market attention; however, the monitoring accuracy and consistency of the devices remains unknown.OBJECTIVE: The purpose of this study was to assess the consistency of the monitoring measurements of the latest wearable devices in the state of normal activities to provide advice to the industry and support to consumers in making purchasing choices.METHODS: Ten pieces of representative wearable devices (2 smart watches, 4 smart bracelets of Chinese brands or foreign brands, and 4 mobile phone apps) were selected, and 5 subjects were employed to simultaneously use all the devices and the apps. From these devices, intact health monitoring data were acquired for 5 consecutive days and analyzed on the degree of differences and the relationships of the monitoring measurements by the different devices.RESULTS: The daily measurements by the different devices fluctuated greatly, and the coefficient of variation (CV) fluctuated in the range of 2-38% for the number of steps, 5-30% for distance, 19-112% for activity duration, .1-17% for total energy expenditure (EE), 22-100% for activity EE, 2-44% for sleep duration, and 35-117% for deep sleep duration. After integrating the measurement data of 25 days among the devices, the measurements of the number of steps (intraclass correlation coefficient, ICC=.89) and distance (ICC=.84) displayed excellent consistencies, followed by those of activity duration (ICC=.59) and the total EE (ICC=.59) and activity EE (ICC=.57). However, the measurements for sleep duration (ICC=.30) and deep sleep duration (ICC=.27) were poor. For most devices, there was a strong correlation between the number of steps and distance measurements (R2>.95), and for some devices, there was a strong correlation between activity duration measurements and EE measurements (R2>.7). A strong correlation was observed in the measurements of steps, distance and EE from smart watches and mobile phones of the same brand, Apple or Samsung (r>.88).CONCLUSIONS: Although wearable devices are developing rapidly, the current mainstream devices are only reliable in measuring the number of steps and distance, which can be used as health assessment indicators. However, the measurement consistencies of activity duration, EE, sleep quality, and so on, are still inadequate, which require further investigation and improved algorithms.
