Abstract:Purpose: We have demonstrated that the depth of unbalanced interocular suppression can be quantified by balancing the interocular luminance differences required when both eyes are viewing simultaneously. In this study, we aimed to investigate the applicability of this method in children with intermittent exotropia (IXT), offering a quantitative assessment of interocular suppression in individuals with binocular imbalance. Additionally, we evaluated its association with the clinical characteristics of IXT. Methods: Interocular suppression in IXT was quantitatively measured using a polarizer and neutral-density (ND) filters. The density of the ND filter was adjusted incrementally from 0.3ND to 3ND, with a step size of 0.3ND (a total of 10 levels). Our prospective study involved 46 patients with IXT (mean age: 10.12 ± 4.89 years; mean ± SD) and 24 normal observers (mean age: 7.88 ± 1.83 years). Results: The suppression test exhibited good test–retest reliability, supported by statistical analysis. We observed more pronounced interocular suppression in individuals with IXT compared to controls. Notably, the magnitude of suppression during distant and near viewing significantly differed in IXT (1.55 ± 0.93 vs. 0.57 ± 0.64; Z = 4.764, p < 0.001). Furthermore, we identified a positive correlation between interocular suppression and data obtained from the Worth-4-Dot test. Additionally, interocular suppression showed a significant association with distance control scores. Conclusion: Our novel test offers a convenient and reliable means to quantify interocular suppression in patients with IXT. The quantitative assessment of interocular suppression provides a sensitive tool to evaluate the clinical characteristics of IXT.

Abstract:Interocular suppression was quantified by the interocular luminance difference that was needed when the two eyes were balanced in discriminating a black-white stripe formed butterfly stimulus, which was dichoptically presented through polarized glasses. Stronger interocular suppression was found in amblyopes than that in controls at both the near (33 cm, 0.95 ± 1.00 vs. 0.14 ± 0.18, p < .001) and far (5 m, 2.18 ± 0.97 vs. 0.24 ± 0.16, p < .001) viewing distances. The interocular suppression in amblyopes was significantly correlated with the interocular visual acuity difference, the visual acuity of amblyopic eye, the Worth-4-Dot test, and the stereo acuity at both the near and far distances (for all cases, p < .001). Our new test enables convenient and robust measurements of interocular suppression in children with amblyopia. The measured interocular suppression is in agreement with other clinical measures.

Abstract:Purpose:We investigate the orientation tuning of interocular suppression using a dichoptic masking paradigm in adult controls and amblyopes.Methods:Fourteen adults with anisometropic or mixed amblyopia and 10 control adults participated in our study. Contrast sensitivity was measured by presenting a target Gabor in the tested eye and mean luminance in the untested eye (monocular) and by presenting a target in the tested eye and a bandpass oriented filtered noise in the other eye (masked). Interocular suppression was defined as the thresholds difference between the monocular and masked conditions for each eye. Interocular suppression was measured under parallel and orthogonal suppression configurations. The peak spatial frequency of the target and mask was 0.25 c/d in experiment 1 (low), 1.31 c/d in experiment 2 (mid), and 6.87 c/d in experiment 3 (high).Results:The masking suppression induced by the amblyopic eye was less strong than that induced by the fellow eye. The suppression from the fellow eye was similar to that observed in the controls. Interocular suppression under parallel configuration was less strong than under orthogonal configuration in amblyopes at low and mid spatial frequency, but not at high spatial frequency.Conclusions:We demonstrate that the abnormal interocular masking in amblyopia displays the expected characteristic of orientation selectivity expected of normal controls at low and mid spatial frequency, but not at high spatial frequency. The dichoptic masking imbalance between the eyes of amblyopes results in a net suppression of the amblyopic eye during binocular viewing, modeling clinical suppression.

Abstract:Objective: To investigate the time and postoperative binocular vision of strabismus surgery for children with intermittent exotropia (X(T)). Methods: A retrospective investigation was conducted in 80 child patients with intermittent exotropia. Pre- and postoperative angles of deviation fixating at near (33 cm) and distant targets (6 m) were measured with the prolonged alternate cover testing. The binocular function was assessed with synoptophore. Twenty-one patients took the postoperative synoptophore exercise. Results: (1) A week after surgery, 96.2% of the 80 patients had binocular normotopia, while a year after surgery, 91.3% of the 80 patients had binocular normotopia; (2) Preoperatively, 58 patients had near stereoacuity, while postoperatively, 72 patients achieved near stereoacuity (P < 0.05); (3) Preoperatively, 64 patients had Grade I for the synoptophore evaluation and postoperatively, 76 patients achieved Grade I. Meanwhile, 55 patients had Grade II preoperatively and 72 achieved Grade II postoperatively. For Grade III, there were 49 patients preoperatively and 64 patients postoperatively (P < 0.05); (4) Patients of 5-8 years old had a significantly better recovery rate of binocular vision than those of 9-18 years old (P < 0.05); (5) Patients taking postoperative synoptophore exercise had a better binocular vision than those taking no exercise (P < 0.05). Conclusions: (1) Strabismus surgery can help to preserve or restore the binocular vision for intermittent exotropia; (2) Receiving the surgery at young ages may develop better postoperative binocular vision; (3) The postoperative synoptophore exercise can help to restore the binocular vision.
The role of diagnostic monocular occlusion test in basic type intermittent exotropia

Abstract:To assess the response to monocular occlusion test in basic type intermittent exotropia (IXT) and to evaluate the surgical outcomes of titrated surgery based on the test's result. Medical records were retrospectively reviewed for patients who underwent bilateral lateral rectus recession for basic type IXT. Patients were categorized into two groups: those who underwent a preoperative diagnostic monocular occlusion test (occlusion group) and those who did not (no occlusion group). In the occlusion group, patients exhibiting a change in deviation angle of ≥ 5 prism diopters (PD), either at distance or near fixation following occlusion therapy, were classified as responders, and augmented surgery was performed for patients with an increased deviation. A total of 215 patients were included in this study, with 79 patients (36.7%) in the no occlusion group and 136 patients (63.3%) in the occlusion group. In the occlusion group, while the mean distance deviation did not significantly change, the mean near deviation significantly increased from 27.2 ± 7.7 PD to 32.5 ± 8.6 PD after diagnostic monocular occlusion ( p < 0.001). 58% (58%) of patients showed an increase in near deviation angle of 5 PD or more. Patients with good fusional control at near fixation were more likely to show a change in their deviation after the occlusion (odds ratio = 1.722, p = 0.028). The success rate of the strabismus surgery was significantly higher, and recurrence rate was significantly lower in the occlusion group compared to the no occlusion group ( p = 0.025 and p = 0.030), while overcorrection rate was not significantly different between the two groups ( p = 1.000). Over half of the patients with basic type IXT demonstrated a significant increase in near deviation angle after diagnostic monocular occlusion. Diagnostic monocular occlusion may be useful for revealing the maximum deviation angle at near fixation and may help determine the optimal surgical dosage in basic type IXT.

Abstract:ABSTRACT The maintenance of visual acuity during active and passive body motion is ensured by gaze-stabilizing reflexes that aim at minimizing retinal image slip. For the optokinetic reflex (OKR), large-field visual motion of the surround forms the essential stimulus that activates eye movements. Properties of the moving visual world influence cognitive motion perception and the estimation of visual image velocity. Therefore, the performance of brainstem-mediated visuo-motor behaviors might also depend on image scene characteristics. Employing semi-intact preparations of mid-larval stages of Xenopus laevis tadpoles, we studied the influence of contrast polarity, intensity, contour shape and different motion stimulus patterns on the performance of the OKR and multi-unit optic nerve discharge during motion of a large-field visual scene. At high contrast intensities, the OKR amplitude was significantly larger for visual scenes with a positive contrast (bright dots on a dark background) compared with those with a negative contrast. This effect persisted for luminance-matched pairs of stimuli, and was independent of contour shape. The relative biases of OKR performance along with the independence of the responses from contour shape were closely matched by the optic nerve discharge evoked by the same visual stimuli. However, the multi-unit activity of retinal ganglion cells in response to a small single moving vertical edge was strongly influenced by the light intensity in the vertical neighborhood. This suggests that the underlying mechanism of OKR biases related to contrast polarity directly derives from visual motion-processing properties of the retinal circuitry.

Abstract:Objective: This study aims to investigate the monocular and binocular accommodative amplitude (AMP) and accommodative function (AF) in children with different types of intermittent exotropia (IXT). Methods: A total of 40 children with IXT were enrolled in the study. Monocular and binocular AMP and AF were measured using the modified approach method and the ±2D flip method, and the differences between the fixing and non-fixing eyes of non-strabismic children and children with different types of IXT were compared. Results: The AMP of the fixing eyes of children with IXT was lower than that of their non-fixing eyes ( p = 0.007). Conversely, the AF was higher in the fixing eyes than in the non-fixing eyes ( p < 0.001). The AMPs of each group of children with IXT were lower than those of the control group, while the AMP of the group with convergence insufficiency was lower than that of the other two groups with IXT. In addition, the AF of the group with convergence insufficiency was lower than that of the group with basic exotropia and the control group ( p < 0.05). Conclusion: There is a difference in accommodation between the fixing and non-fixing eyes of children with IXT, and the degree of variation depends on the type of IXT. Moreover, the binocular accommodative function of children with IXT is lower than that of non-strabismic children.

Abstract:Purpose: To determine binocular summation of surgically treated intermittent exotropia (IXT) patients by measuring the contrast threshold. Methods: We recruited 38 surgically treated IXT patients aged 8–24 years and 20 age-matched healthy controls. All participants had normal or corrected-to-normal visual acuity (Snellen ≥ 20/20) in both eyes. The IXT patients had undergone the surgery at least a year prior to the study. Twenty-one of them obtained good alignment and 17 experienced a recurrence of exotropia. We measured the observers' monocular and binocular contrast sensitivities (CS) at six spatial frequencies (1.5, 3, 6, 12, 18, 24 cycles/degree) as an index of visual information processing at the threshold level. Binocular summation was evaluated against a baseline model of simple probability summation based on the CS at each spatial frequency and the area under the log contrast sensitivity function (AULCSF). Results: The exo-deviation of IXTs with good alignment was −6.38 ± 3.61 prism diopters (pd) at 33 cm and −5.14 ± 4.07 pd at 5 m. For the patients with recurrence, it was −23.47 ± 5.53 pd and −21.12 ± 4.28 pd, respectively. There was no significant difference in the binocular summation ratio (BSR) between the surgically treated IXT patients, including those with good alignment and recurrence, and normal controls at each spatial frequency [ F (2,55) = 0.416, P = 0.662] and AULCSF [ F (2,55) = 0.469, P = 0.628]. In addition, the BSR was not associated with stereopsis ( r = −0.151, P = 0.365). Conclusion: Our findings of normal contrast sensitivity binocular summation ratio in IXT after surgical treatment suggest that the ability of the visual cortex in processing binocular information is intact at the contrast threshold level.

Binocular Visual Deficits at Low to High Spatial Frequency in Intermittent Exotropia After Surgery

Abstract:Purpose: To investigate binocular visual deficits at low to high spatial frequencies in patients with intermittent exotropia (IXT) after surgical correction, using the binocular orientation combination task. Methods: Thirteen patients whose IXT has been aligned surgically (17 ± 4.8 years old; 7 females) and 13 normal individuals (21.8 ± 2.5 years old; 6 females) were recruited. All participants had normal or corrected-to-normal visual acuity. The IXT patients had undergone surgery at least one month prior to the study and achieved successful eye alignment post-surgery. We measured participants' balance points (BPs), defined as the interocular contrast ratio (nondominant eye/dominant eye) when both eyes contributed equally to binocular combination, using the binocular orientation combination task at three spatial frequencies (0.5, 4.0, and 8.0 cycles/degree). The absolute values of log10(BP) (i.e., |logBP|) and the area under of the |logBP| versus spatial frequency curve were used to quantify the extent of binocular imbalance. Results: Surgery aligned the eye position of patients with IXT, with a postoperative exodeviation of -4.92 ± 4.29 prism diopters at distance. Participants' |logBP| values showed significant differences between groups, F(1,24) = 9.175, P = 0.006, and across spatial frequencies, F(2,48) = 7.127, P = 0.002. However, the interaction between group and spatial frequency was not significant, F(2,48) = 0.379, P = 0.687. Conclusions: Patients whose IXT has been alighted surgically experience binocular imbalance across a wide range of spatial frequencies, with greater binocular imbalance occurring at high spatial frequencies than low spatial frequencies.

Abstract:Purpose: Intermittent exotropia (IXT) is the most common form of strabismus. Surgery can potentially improve binocular function in patients with IXT. We aimed to evaluate binocular function using a novel parameter-binocular summation ratio (BSR), measured using quantitative contrast sensitivity function (CSF) in patients with IXT before and after surgery.Methods: Prospective study of 63 patients with IXT and 41 healthy controls were consecutively enrolled and underwent quantitative CSF testing binocularly and monocularly. BSR was calculated by dividing the CSF of the binocular value by the better monocular value. Forty-eight patients with IXT underwent strabismus surgery. BSR, stereoacuity, fusion ability, and strabismus questionnaires were assessed pre-operatively and 2 months postoperatively.Results: Sixty-three patients with IXT (median age = 9 years) compared with 41 healthy controls showed a worse mean BSR based on all CSF metrics at baseline (the area under the log CSF [AULCSF], spatial frequency [SF] cutoff, and contrast sensitivity at 1.0-18.0 cpd SF). All 48 patients with IXT showed successful alignment after surgery, and there were significant improvements in BSR based on the AULCSF, SF cutoff, and contrast sensitivity at 6.0, 12.0, and 18.0 cpd SF, respectively. The distance stereoacuity and fusion ability also improved after surgery, and a better BSR was associated with better stereoacuity and fusion. For strabismus questionnaires, the psychosocial subscale scores improved postoperatively, whereas the functional subscale scores did not change.Conclusions: BSR based on quantitative CSF can characterize binocular function across a range of spatial frequencies and can be used as a supplemental measurement for monitoring binocularity in patients with IXT in clinical settings.

Abstract:Introduction Intermittent exotropia (IXT) is the most common type of divergent squint. IXT is primarily a cortical neurologic dysfunction disorder, occurring as a result of the insufficient maintenance of sensory and motor fusion. Recent reports have demonstrated the relationship between IXT and visual cortical impairment. We planned to assess blood oxygen level-dependent (BOLD)-functional magnetic resonance imaging (fMRI) in patients with IXT during the preoperative and postoperative follow-ups to evaluate the functional changes in the visual cortex. Methods and analysis A total of 90 Chinese subjects will be recruited, and their ages will be between 18 and 40 years old. The subjects will include the surgical treatment (ST) group (45 subjects with IXT who will undergo surgery) and the HC group (45 age-matched, sex-matched and education-matched healthy volunteers). The assessments will include the following aspects: fMRI and general ophthalmic examinations, optometry measurements and strabismus-related tests, such as the ocular deviation, binocular vision test and Newcastle Control Score (NCS). Each subject will complete the resting-state BOLD-fMRI, and the sequences will include echo planar imaging (EPI) pulse and 3-dimensional brain volume (3D-BRAVO) to acquire high-resolution images. The follow-up schedule will be 6 and 12 months after the surgery. The primary outcome will be determined by cortex changes in BOLD-fMRI in the ST group before and after surgery. We will also compare the HC group with the preoperative subjects in the ST group. The secondary outcomes will be changes in strabismus-related examinations, such as binocular visual function and NCS. Ethics and dissemination Ethical approval was obtained from the Medical Ethics Committee of Beijing Tongren Hospital. We plan to publish the results of this study in a peer-reviewed journal article. Abstract:Background: Intermittent exotropia (IXT) is the most common type of exotropia in China. Surgery is usually required to align the eye deviation to maintain or obtain better binocular visual function. However, there is a high rate of exodrift or recurrence in surgically treated patients. Orthoptic therapy is sometimes recommended for IXT patients after surgery. However, there is a lack of high-quality randomized controlled trials to prove that orthoptic therapy could be an effective supplement to surgical treatment for IXT patients. The main purpose of this study is to test the clinical effectiveness of orthoptic therapy in long-term stabilization of postoperative IXT patient. This report describes the design and methodology of the Intermittent Exotropia Postoperative Treatment Clinical Trial, which is the first large-sample, blank-controlled, randomized clinical trial. Methods: A total of 136 IXT patients (aged 7 to 17 years) will be enrolled and assigned to the orthoptic therapy group or blank control group according to a simple randomization scheme. Patients in the orthoptic therapy group will receive at least 2 months of orthoptic therapy, such as anti-suppression, vergence, and accommodation training. Patients in the blank control group will receive only refractive correction. All enrolled patients will need regular follow-up observation until 24 months after surgery. The primary outcome will be the proportion of participants meeting suboptimal surgical outcomes in this 24-month follow-up, which is defined as (1) exodeviation of 10 prism diopters (PD) at distance or near using the simultaneous prism and cover test (SPCT) or (2) loss of 2 or more octaves of stereoacuity from baseline, at any masked follow-up visit examination. The secondary outcomes will be the exodeviation at distance and near using the simultaneous prism and alternate cover test (PACT), magnitude of fusional convergence, stereoacuity, and accommodation. Measurements will be taken at baseline and at the 6-, 12-, 18-, and 24-month follow-ups. Discussion: To the best of our knowledge, this will be the first prospective, randomized controlled study of orthoptic training in IXT patients after surgery. The aim of this work is to confirm the efficacy of orthoptic therapy in reducing the proportion of recurrence among IXT patients after surgery and improving binocular vision function. Disrupted Neural Signals in Patients with Concomitant Exotropia

Abstract:Purpose Decreased binocular and oculomotor function in strabismics has recently been considered as cortical in origin. This study aimed to investigate functional abnormalities using a frequency-specific neuroimaging method in patients with concomitant exotropia (XT), and to demonstrate the clinical implications. Methods Resting-state functional magnetic resonance imaging data were collected in 26 XT patients and 26 matched controls. To evaluate the local spontaneous neural activity, the amplitude of low frequency fluctuations (ALFF) was calculated in the typical frequency band (0.01-0.08 Hz) as well as five narrowly-defined frequency bands (slow-6: 0-0.01 Hz, slow-5: 0.01-0.027 Hz, slow-4: 0.027-0.073 Hz, slow-3: 0.073-0.167 Hz, and slow-2: 0.167-0.25 Hz), respectively. Results Patients with XT showed decreased ALFF in the bilateral parieto-occipital sulcus (POS), and increased ALFF in the bilateral thalamus within the typical frequency band. Frequency-dependent ALFF alterations were found in the higher visual areas such as the right lateral occipital complex (LOC). Furthermore, ALFF in the right LOC in the slow-5 band was positively correlated with fusion control score (r = 0.70,p < 0.0001) and binocular function score (r = 0.67,p = 0.0002). Regression analyses showed that early age of onset remained the only significant explanatory factor for ALFF reduction in the right POS in the typically-measured frequency band (also referred to as the typical frequency band) (Odds ratio, 0.038; 95% confidence interval, 0.001 to 0.075). Conclusions Our findings provide spatial information regarding the functionally disrupted regions in XT. Moreover, the frequency-dependent ALLF alteration in the right LOC might reflect a potential plastic capacity in binocular function, which could be a potential objective index for evaluating disease severity.
Objective evaluation of visual fatigue in patients with intermittent exotropia

Abstract:The purpose of this study was to evaluate the degree of visual fatigue in patients with intermittent exotropia (IXT) using the binocular fusion maintenance (BFM) test.Fourteen patients with IXT (32.1 ± 16.4 years) and 15 age-matched healthy volunteers (31.2 ± 9.3 years) participated in the study. BFM was assessed by measuring the transmittance of liquid crystals placed in front of the subject's nondominant eye at the instance when binocular fusion was broken and vergence eye movement was induced. A questionnaire on subjective symptoms was administered to the subjects before and after the visual task. The visual task consisted of a reciprocal movement between 67 and 40 cm.The change [post–pre] of BFM was significantly lower in the IXT group (−0.185 ± 0.187) than in the control group (−0.030 ± 0.070) (P = 0.010). The change of total subjective eye symptom score was significantly greater in the IXT group (2.28 ± 1.43) than in the control group (0.93 ± 1.27) (P = 0.018). The reduction in BFM rate with increasing total subjective eye symptom score was significantly greater in the IXT group (−0.106 ± 0.017) than in the control group (−0.030 ± 0.013) (P = 0.006).The present findings objectively showed that patients with IXT are at a greater risk of visual fatigue in comparison with healthy individuals.

Abstract:Purpose To evaluate the short‐term (1 week after completion of treatment) effect of office‐based vergence and anti‐suppression therapy (OBVAT) on binocular vision and accommodative function when compared to observation alone in children with small‐to‐moderate angle intermittent exotropia (IXT). Methods In this single‐masked (examiner masked), two‐arm, single‐centre randomised clinical trial, 40 participants, 6 to <18 years of age with untreated IXT were randomly assigned to OBVAT or observation alone. Participants assigned to therapy received 16 weeks of OBVAT (60 min per visit, once per week) with home reinforcement (15 min per day, 5 days a week). Therapy included vergence, accommodation and anti‐suppression techniques. This paper reports the results of clinical measures of binocular vision and accommodation, including fusional vergence, near point of convergence, vergence facility, amplitude of accommodation, the Worth 4‐dot test and the Fusion Maintenance Score (FMS). Results Treatment group differences at the primary outcome visit significantly favoured the OBVAT group (n = 20) over the observation group (n = 16) in negative fusional vergence break point at both distance (p ≤ 0.001; r = 0.58) and near (adjusted mean difference: 12.3 Δ; 95% CI: 5.3–19.3 Δ; p = 0.001; partial eta squared: 0.28), positive fusional vergence break point at both distance (p = 0.009; r = 0.43) and near (adjusted mean difference: 20.6 Δ; 95% CI: 11.8–29.4 Δ; p ≤ 0.001; partial eta squared: 0.41), vergence facility (adjusted mean difference: 5.6 cpm; 95% CI: 1.9–9.4 cpm; p = 0.005; partial eta squared: 0.22) and the FMS (p = 0.007; r = 0.44). Conclusions In this randomised clinical trial of participants aged 6 to <18 years with IXT, those in the OBVAT group had a significantly better fusional vergence range, vergence facility, near Worth 4‐dot test result and FMS than the observation group at the 17‐week visit. The improvement in these areas further support the effectiveness of OBVAT for improving IXT.

Abstract:Objective: To investigate if the concordance between sighting dominance and fixation preference depends on stereoacuity in children with intermittent exotropia (IXT). Methods: A total of 160 children (aged 7.24 +/- 2.14 years, range 4-13 years) with the basic type of IXT at distance participated in the study. Binocular fusion and vergence were evaluated with synoptophore. Stereoacuity was assessed using the Titmus stereo test. The hole-in-the-card test was used to determine sighting dominance, while the eye of fixation preference was determined by the cover-uncover test. The chi-squared test was used to evaluate whether a distribution was different from the chance distribution. The Kappa value was computed to quantify the concordance between fixation preference and sighting dominance. Results: The mean deviations were 19 +/- 4.58 prism diopters (PD) and 18.9 +/- 4.47 PD for at distance and near, respectively. The mean amplitude of divergence was 5.34 +/- 1.89 PD, and the mean amplitude of convergence was 14.08 +/- 4.96 PD. Subjects were categorized as having either good (40-60 seconds of arc, n = 41), moderate (80-140 seconds of arc, n = 46), poor (>= 200 seconds of arc, n = 45), or having no measurable stereoacuity (n = 28). The concordance between sighting dominance and fixation preference was high in subjects with good (Kappa = 0.858) or moderate (kappa = 0.812) stereoacuity, but it decreased quickly in subjects with poor stereoacuity (kappa = 0.496) or no stereopsis (kappa = 0.563). Conclusions: In pediatric patients with IXT, the concordance between sighting dominance and fixation preference depends on stereoacuity. The results from these two tests become increasingly incongruent as stereoacuity deteriorated.

Abstract:Objective:To evaluate the accommodative facility and accommodative response in patients with intermittent exotropia.Methods:Thirty-five patients with basic intermittent exotropia and 24 normal controls were recruited for a case control study at Eye Hospital,Wenzhou Medical University from October 2016 to January 2017.Binocular and monocular accommodative facility were tested using a ± 2.00 diopter (D) flip.Accommodative responses under binocular and monocular viewing conditions at 40 cm were tested by an open-field autorefractor WAM-5500.Data were analyzed using t-tests.Results:The binocular accommodative facility of intermittent exotropia was 8.8 ± 4.1 cycles per minute (cpm),which was significantly lower than the normal control,10.9± 3.1 cpm (t=-2.165,P=0.035).The fixating eye of intermittent exotropia had higher monocular accommodative facility than the non-fixating eye (t=4.657,P＜0.001).Under binocular viewing conditions,the fixating eye of the intermittent exotropia had a higheraccommodative response than the fixating eye of the normal control (t=-2.163,P=0.035).The binocular accommodative response was also higher than the accommodative response under monocular viewing conditions (t=3.801,P=0.001).Conclusions:Patients with intermittent exotropia have low binocular accommodative facility.They also have asymmetric monocular accommodative facility and asymmetric accommodative responses between the fixating eye and the deviating eye.Patients with intermittent exotropia have a higher accommodative response under binocular viewing conditions than under monocular viewing conditions.

Abstract:When tracking with the eyes an object moving against a textured background, the background retinal image moves in the opposite direction to the smooth pursuit eye movement. Optokinetic responses, such as optokinetic nystagmus (OKN) or ocular tracking, to this reafferent signal must be suppressed to sustain pursuit of the object-of-interest. We varied the contrast of a brief background motion to tell apart two plausible accounts of the suppression of optokinesis during pursuit; a visuomotor gain modulation account, which predicts that ocular tracking of background motion is suppressed in the same proportion at irrespective of contrast, and a sensory attenuation account, which predicts that larger contrasts are needed to elicit the same response. Unexpectedly, neither account fits ocular tracking in the reafferent signal direction. The combination of contrast-dependent gating, with maximal suppression observed with higher contrasts, and visuomotor gain modulation, provides a good fit for most observers' data. Contrast-dependent gating promotes visuomotor stability in response to most salient signals, as a likely adaptation to the statistics of the environment.

Abstract:Intermittent exotropia (IXT) is characterizedby an intermittent outward deviation of the eyes. Yet, the neural substrates associated with IXT are not fully understood. This study investigated brain structure and spontaneous functional activity changes in children with IXT. All participants underwent detailed ophthalmological examinations and multimodal magnetic resonance imaging (MRI) scanning. During functional scanning, binocular visual stimuli were presented to subjects to determine brain areas involved in visual and oculomotor processing. Regions of interest(ROI) were subsequently selected based on functional activation to investigate brain structural and spontaneous functional differences between IXT children and healthy controls (HCs) using small volume correction (SVC). Reduced gray matter density (GMD) was found in the right frontal eye field (FEF) and bilateral inferior parietal lobe (IPL) in IXT children compared with HCs. Besides, reduced fractional amplitude of low-frequency fluctuations (fALFF) values were observed in the left lingual gyrus, right inferior occipital gyrus (IOG), bilateral IPL, and bilateral cerebellum in the IXT children compared to the HCs. IXT children with worse eye position control ability exhibited lower GMD and fALFF values in these areas. Finally, resting state functional connectivity (RSFC) was reduced in frontoparietal oculomotor processing areas in IXT children compared to HCs. In addition, increased cortical thickness was found in the right visual areas and bilateral IPL. These results showed that IXT-related structural and functional brain abnormalities occurred in childhood and may be related to underlying neuropathological mechanisms.
