Abstract:Although there is increasing evidence suggesting that there may be subtle abnormalities in idiopathic generalized epilepsy (IGE) patients using modern neuroimaging techniques, most of these previous studies focused on the brain grey matter, leaving the underlying white matter abnormalities in IGE largely unknown, which baffles the treatment as well as the understanding of IGE. In this work, we adopted multiple methods from different levels based on diffusion tensor imaging (DTI) to analyze the white matter abnormalities in 14 young male IGE patients with generalized tonic-clonic seizures (GTCS) only, comparing with 29 age-matched male healthy controls. First, we performed a voxel-based analysis (VBA) of the fractional anisotropy (FA) images derived from DTI. Second, we used a tract-based spatial statistics (TBSS) method to explore the alterations within the white matter skeleton of the patients. Third, we adopted region-of-interest (ROI) analyses based on the findings of VBA and TBSS to further confirm abnormal brain regions in the patients. At last, considering the convergent evidences we found by VBA, TBSS and ROI analyses, a subsequent probabilistic fiber tractography study was performed to investigate the abnormal white matter connectivity in the patients. Significantly decreased FA values were consistently observed in the cerebellum of patients, providing fresh evidence and new clues for the important role of cerebellum in IGE with GTCS.
Altered White-Matter Functional Network in Children with Idiopathic Generalized Epilepsy

Abstract:Rationale and Objectives The white matter (WM) functional network changes offers insights into the potential pathological mechanisms of certain diseases, the alterations of WM functional network in idiopathic generalized epilepsy (IGE) remain unclear. We aimed to explore the topological characteristics changes of WM functional network in childhood IGE using resting-state functional Magnetic resonance imaging (MRI) and T 1 -weighted images. Methods A total of 84 children (42 IGE and 42 matched healthy controls) were included in this study. Functional and structural MRI data were acquired to construct a WM functional network. Group differences in the global and regional topological characteristics were assessed by graph theory and the correlations with clinical and neuropsychological scores were analyzed. A support vector machine algorithm model was employed to classify individuals with IGE using WM functional connectivity as features, and the model's accuracy was evaluated using leave-one-out cross-validation. Results In IGE group, at the network level, the WM functional network exhibited increased assortativity; at the nodal level, 17 nodes presented nodal disturbances in WM functional network, and nodal disturbances of 11 nodes were correlated with cognitive performance scores, disease duration and age of onset. The classification model achieved the 72.6% accuracy, 0.746 area under the curve, 69.1% sensitivity, 76.2% specificity. Conclusion Our study demonstrated that the WM functional network topological properties changes in childhood IGE, which were associated with cognitive function, and WM functional network may help clinical classification for childhood IGE. These findings provide novel information for understanding the pathogenesis of IGE and suggest that the WM function network might be qualified as potential biomarkers.

Abstract:The aim of this study was to investigate brain structure and corresponding static and dynamic functional connectivity (sFC & dFC) abnormalities in untreated, first-episode pediatric idiopathic generalized epilepsy (IGE), with the goal of better understanding the underlying pathological mechanisms of IGE. Thirty-one children with IGE and 31 age-matched healthy controls (HC) were recruited. Structural magnetic resonance imaging (sMRI) data were acquired, and voxel-based morphometry (VBM) analysis were performed to reveal abnormal gray matter volume (GMV). Moreover, sFC and dFC analyses were conducted using the brain areas exhibiting abnormal GMV as seed regions to explore abnormal functional couplings. Compared to HC, the IGE group exhibited increased GMV in left middle cingulate cortex (MCC) and right parahippocampus (ParaHipp). In addition, the analyses of dFC and sFC with MCC and ParaHipp as seeds revealed more extensive functional connectivity (FC) changes in dFC. Notably, the structurally and functionally abnormal brain areas were primarily localized in the default mode network (DMN). However, our study did not find any significant associations between these altered neuroimaging measurements and clinical outcomes. This study uncovered microstructural changes as well as corresponding sFC and dFC changes in patients with new-onset, untreated pediatric IGE. The affected brain regions were primarily located within the DMN, highlighting the DMN's crucial role in the development of pediatric IGE.

Diffusion Tensor Imaging Study on Pediatric Bipolar Mania

Abstract:Objective: The aim of the present study was to investigate brain white matter(WM) changes in pediatric bipolar disorder mania(PBD-mania) using diffusion tensor imaging technique. Methods: Twenty-one children and adolescents with bipolar mania(male/female: 8/13) aged 10-18 years and 18 age-and sex-matched healthy controls were included in the present study. Diffusion tensor imaging(DTI) data were obtained on a Siemens 3.0 Tesla scanner. Voxel-based analyses(VBA) were conducted to compare the WM fractional anisotropy(FA) values between patients and controls. Correlations of the MRI data of each survived area with clinical characteristics in PBD patients were further analyzed. Results:As compared with the control group, PBD-mania patients showed decreased FA values in the right anterior cingulate(AC)and the left thalamus. No region of increased FA value was observed in PBD-mania. There was no significant correlation of FA values of the two survived areas with the illness duration, onset age, number of episodes, or mania score measured as Young mania rating scale(YMRS) in PBD mania patients. Conclusion: The present findings suggested that microstructural abnormality of the anterior cingulate and thalamus might contribute to the pathophysiology of PBD-mania.
Application of Diffusion Tensor Imaging to Idiopathic Generalized Epilepsy

Abstract:Objective Patients with idiopathic generalized epilepsy (IGE) has no abnormal presentation in routine magnetic resonance imaging (MRI). In this article, we explored the neural pathological mechanism of IGE by observing micro-structural abnormalities of the brain using diffusion tensor imaging (DTI). Methods Thirty IGE patients and 30 healthy adult controls underwent conventional MRI and DTI with the GE signal 1.5T MR scanner. The mean diffusivity (MD) and fractional anisotropy (FA) maps were compared by voxel-based analysis, and so were the areas of the brain with significant changes of the adjusted MD and FA (P0.01) between the two groups. Results Compared with the controls, the IGE patients showed decreased MD in the bilateral thalamus, temporal lobe, insula, occipital lobe and midbrain, and increased MD in the mesial prefrontal lobe. The regions showing increased FA included bilateral thalamus and posterior cingulate gyrus; while the decreased FA was mainly in the bilateral mesial prefrontal lobe and temporal lobe. Conclusion DTI is a sensitive imaging technique, which can reveal the changes in brain microstructural connectivity, and help the clinical diagnosis of IGE and investigation of its pathophysiological mechanism.

Abstract:In frontal lobe epilepsy (FLE), interictal discharges and seizures are more likely to spread to contralateral hemisphere and become secondarily generalized. The aim of this study was to assess white matter (WM) integrity in children with FLE using diffusion tensor imaging (DTI). Children with FLE and normal MRI, and healthy controls with no neurological or psychiatric disorders underwent DTI on 3T MRI. Whole brain fractional anisotropy (FA) and mean diffusivity (MD) maps were compared between right and left FLE with controls. 43 children with FLE, consisting of 28 left and 15 right FLE, and 44 healthy controls were recruited. Patients with left FLE had significant FA reductions in left (p=0.002) and right (p=0.003 and p=0.034) superior longitudinal fasciculi (SLF), genu/body (p=0.0002) and splenium (p=0.011) of corpus callosum. Patients with right FLE had significant FA reductions in left (p=0.016) and right (p=0.033) SLF, genu (p=0.001) and body of corpus callosum (p=0.001 and p=0.008), and significant MD elevation in right thalamus (p=0.032). There was no significant association between FA or MD and clinical seizure parameters. The abnormal WM both ipsilateral and contralateral to seizure focus may be due to seizure activity or abnormal brain development.

Abstract:The epilepsies are commonly accompanied by widespread abnormalities in cerebral white matter. ENIGMA-Epilepsy is a large quantitative brain imaging consortium, aggregating data to investigate patterns of neuroimaging abnormalities in common epilepsy syndromes, including temporal lobe epilepsy, extratemporal epilepsy, and genetic generalized epilepsy. Our goal was to rank the most robust white matter microstructural differences across and within syndromes in a multicentre sample of adult epilepsy patients. Diffusion-weighted MRI data were analysed from 1069 healthy controls and 1249 patients: temporal lobe epilepsy with hippocampal sclerosis (n = 599), temporal lobe epilepsy with normal MRI (n = 275), genetic generalized epilepsy (n = 182) and non-lesional extratemporal epilepsy (n = 193). A harmonized protocol using tract-based spatial statistics was used to derive skeletonized maps of fractional anisotropy and mean diffusivity for each participant, and fibre tracts were segmented using a diffusion MRI atlas. Data were harmonized to correct for scanner-specific variations in diffusion measures using a batch-effect correction tool (ComBat). Analyses of covariance, adjusting for age and sex, examined differences between each epilepsy syndrome and controls for each white matter tract (Bonferroni corrected at P < 0.001). Across 'all epilepsies' lower fractional anisotropy was observed in most fibre tracts with small to medium effect sizes, especially in the corpus callosum, cingulum and external capsule. There were also less robust increases in mean diffusivity. Syndrome-specific fractional anisotropy and mean diffusivity differences were most pronounced in patients with hippocampal sclerosis in the ipsilateral parahippocampal cingulum and external capsule, with smaller effects across most other tracts. Individuals with temporal lobe epilepsy and normal MRI showed a similar pattern of greater ipsilateral than contralateral abnormalities, but less marked than those in patients with hippocampal sclerosis. Patients with generalized and extratemporal epilepsies had pronounced reductions in fractional anisotropy in the corpus callosum, corona radiata and external capsule, and increased mean diffusivity of the anterior corona radiata. Earlier age of seizure onset and longer disease duration were associated with a greater extent of diffusion abnormalities in patients with hippocampal sclerosis. We demonstrate microstructural abnormalities across major association, commissural, and projection fibres in a large multicentre study of epilepsy. Overall, patients with epilepsy showed white matter abnormalities in the corpus callosum, cingulum and external capsule, with differing severity across epilepsy syndromes. These data further define the spectrum of white matter abnormalities in common epilepsy syndromes, yielding more detailed insights into pathological substrates that may explain cognitive and psychiatric co-morbidities and be used to guide biomarker studies of treatment outcomes and/or genetic research.
Microstructural white matter abnormality and frontal cognitive dysfunctions in juvenile myoclonic epilepsy

Abstract:PURPOSE: Previous neuroimaging studies provide growing evidence that patients with juvenile myoclonic epilepsy (JME) have both structural and functional abnormalities of the thalamus and frontal lobe gray matter. However, limited data are available regarding the issue of white matter (WM) involvement, making the microstructural WM changes in JME largely unknown. In the present study we investigated changes of WM integrity in patients with JME, and their relationships with cognitive functions and epilepsy-specific clinical factors.METHODS: We performed diffusion tensor imaging (DTI) and neuropsychological assessment in 25 patients with JME and 30 control subjects matched for age, gender, and education level. Between-group comparisons of fractional anisotropy (FA) and mean diffusivity (MD) were carried out in a whole-brain voxel-wise manner by using tract-based spatial statistics (TBSS). In addition, both FA and MD were correlated with cognitive performance and epilepsy-specific clinical variables to investigate the influence of these clinical and cognitive factors on WM integrity changes.KEY FINDINGS: Neuropsychological evaluation revealed that patients with JME had poorer performance than control subjects on most of the frontal function tests. TBSS demonstrated that, compared to controls, patients with JME had significantly reduced FA and increased MD in bilateral anterior and superior corona radiata, genu and body of corpus callosum, and multiple frontal WM tracts. Disease severity, as assessed by the number of generalized tonic-clonic seizures in given years, was negatively correlated with FA and positively correlated with MD extracted from regions of significant differences between patients and controls in TBSS.SIGNIFICANCE: Our findings of widespread disturbance of microstructural WM integrity in the frontal lobe and corpus callosum that interconnects frontal cortices could further support the pathophysiologic hypothesis of thalamofrontal network abnormality in JME. These WM abnormalities may implicate frontal cognitive dysfunctions and disease progression in JME.

White Matter Hyperintensities on Brain Magnetic Resonance Imaging in People with Epilepsy: A Hospital‐Based Study

Abstract:AimsWe aim to explore whether people with epilepsy have increased white matter hyperintensities (WMHs).MethodsEligible patients were categorized into newly diagnosed epilepsy (NE) and chronic epilepsy (CE); the latter were subdivided to those treated with enzyme-inducing antiepileptic drugs (EIAEDs) with or without non-enzyme-inducing antiepileptic drugs (NEIAEDs) and those with NEIAEDs only. WMHs were measured using age-related white matter changes (ARWMC) scale and compared between patients and healthy control group. Higher scores indicate greater WMH changes. The strengths of associations were estimated as incidence rate ratios (IRRs) with 95% confidence interval (CI).ResultsA total of 217 patients were included in the analysis, of whom 67 had NE, 45 had CE treated with NEIAEDs, and 105 had CE treated with EIAEDs. Age was positively associated with ARWMC score (IRR per year, 1.03; 95%CI, 1.03-1.04, P < 0.001). Compared with the healthy control group (n = 23), all patient groups had higher ARWMC score (P < 0.05). The difference was greatest in patients receiving EIAEDs (IRR, 2.13; 95%CI, 1.22-3.70, P = 0.007).ConclusionsWMHs tended to be observed in people with epilepsy, especially in those treated with EIAEDs. People with epilepsy with white matter changes should be evaluated for stroke risk, particularly if they are receiving EIAEDs.
Abnormal Anterior Cingulum Integrity in First Episode, Early-Onset Schizophrenia: A Diffusion Tensor Imaging Study

Abstract:Adolescents with early-onset schizophrenia (EOS) provide a unique opportunity to examine regional and disease-specific early abnormal brain development in schizophrenia. There is currently a general agreement that clinical symptoms and cognitive dysfunctions of schizophrenia may arise from a failure of adequate communication between different brain regions. This disturbed connectivity may be related to alterations in the numbers, distribution, and ultrastructural integrity of oligodendrocytes of white matter. Diffusion tensor imaging provides a relatively new approach for quantifying possible connectivity of white matter in vivo. In this study, thirty-eight adolescents with early-onset schizophrenia (EOS; onset of psychotic symptoms by age 18years) and 38 age- and gender-matched healthy volunteers received diffusion tensor imaging examinations. Fractional anisotropy images were compared between groups in the white matter using a voxelwise analysis after inter-subject registration to standard Montreal Neurological Institute (MNI) space. Adolescents with EOS demonstrated significantly reduced fractional anisotropy in right anterior cingulum compared to healthy volunteers. Increased fractional anisotropy was not observed in any brain region in patients compared to controls. ROI analysis also reported a trend-level negative correlation between mean regional FA in right anterior cingulum and PANSS positive symptom score. These findings indicate that white matter abnormalities in the anterior cingulum are associated with adolescent EOS.

Abstract:Background: Simple febrile seizures (SFS) and epilepsy are common seizures in childhood. However, the mechanism underlying SFS is uncertain, and the presence of obvious variances in white matter (WM) integrity and glymphatic function between SFS and epilepsy remain unclear. Therefore, this study aimed to investigate the differences in WM integrity and glymphatic function between SFS and epilepsy. Material and Methods: We retrospectively included 26 children with SFS, 33 children with epilepsy, and 28 controls aged 6–60 months who underwent magnetic resonance imaging (MRI). Tract-based spatial statistics (TBSS) were used to compare the diffusion tensor imaging (DTI) metrics of WM among the above-mentioned groups. T2-weighted imaging (T2WI) was used to segment the visible Virchow-Robin space (VRS) through a custom-designed automated method. VRS counts and volume were quantified and compared among the SFS, epilepsy, and control groups. Correlations of the VRS metrics and seizure duration and VRS metrics and the time interval between seizure onset and MRI scan were also investigated. Results: In comparison with controls, children with SFS showed no significant changes in fractional anisotropy (FA), axial diffusivity (AD), or radial diffusivity (RD) in the WM ( P > 0.05). Decreased FA, unchanged AD, and increased RD were observed in the epilepsy group in comparison with the SFS and control groups ( P < 0.05). Meanwhile, VRS counts were higher in the SFS and epilepsy groups than in the control group (VRS_SFS, 442.42 ± 74.58, VRS_epilepsy, 629.94 ± 106.55, VRS_control, 354.14 ± 106.58; P < 0.001), and similar results were found for VRS volume (VRS_SFS, 6,228.18 ± 570.74 mm 3 , VRS_epilepsy, 9,684.84 ± 7,292.66mm 3 , VRS_control, 4,007.22 ± 118.86 mm 3 ; P < 0.001). However, VRS metrics were lower in the SFS group than in the epilepsy group ( P < 0.001). In both SFS and epilepsy, VRS metrics positively correlated with seizure duration and negatively correlated with the course after seizure onset. Conclusion: SFS may not be associated with WM microstructural disruption; however, epilepsy is related to WM alterations. Seizures are associated with glymphatic dysfunction in either SFS or epilepsy.

Abstract:PurposeIn brain development, Myelination is the characteristic feature of white matter maturation, which plays an important role in efficient information transmitting. The white matter abnormality has been reported to be associated with self-limited epilepsy with centrotemporal spikes (SeLECTS). This study aimed to detect the altered white matter region in the SeLECTS patients by the combination of diffusion tensor imaging (DTI) and quantitative susceptibility mapping (QSM) technique.Methods27 children with SeLECTS and 23 age- and gender-matched healthy children were enrolled. All participants were scanned with 3.0-T MRI to acquire the structure, diffusion and susceptibility-weighted data. The susceptibility and diffusion weighted data were processed to obtain quantitative susceptibility map and fraction anisotropy (FA) map. Then voxel-wise tract-based spatial statistics (TBSS) were used to analyze quantitative susceptibility and FA data.ResultsBoth DTI and QSM revealed extensive white matter alterations in the frontal, parietal, and temporal lobes in SeLECTS patients. The overlapped region of DTI and QSM analyses was located in the fiber tracts of the corona radiata. The FA values in this overlapped region were negatively correlated with the magnetic susceptibility values.ConclusionOur results suggest that TBSS-based QSM can be employed as a novel approach for characterizing alterations in white matter in SeLECTS. And the combination of QSM and DTI can provide a more comprehensive evaluation of white matter integrity by utilizing different biophysical features.

Abstract:Objective This study aimed to investigate the differences of white matter alterations between partial seizures(PS) in medial temporal lobe epilepsy with unilateral hippocampal sclerosis (mTLE-HS) and secondary generalized tonic-clonic seizures (sGTCS) by using diffusion tensor imaging (DTI) technique,then to explore the two seizures'differentstructure bases.Methods All of DTI images were obtained on a MRI scanner (3.0 Tesla).Thirty unilateral mTLE-HSpatients,including 14 PS patients and 16 sGTCS patients,and twenty age,gender and education-matched healthy controlsubjects were recruited.The white matter in the same side of lesion was defined as " ipsilateral" white matter.The inter-group differences of white matter among 3 groups were performed as well as the differences between the white matter of ipsi-lateral and contralateral hemispheres.P ＜ 0.05 was considered as statistically significant.Results Compared with thehealthy controls,the PS patients showed decreased FA and increased MD in ipsilateral inferior longitudinal fasciculus,unci-nate fasciculus.We also observed extensive alterations of white matter in sGTCS patients,including decreased FA and in-creased MD in bilateral anterior thalamic radiation(ATR),corticospinal tract(CT),cingulate cortex(CC),corpus callosum,inferior fronto-occipital fasciculus (IFOF),inferior longitudinal fasciculus (ILF),superior longitudinal fasciculus (SLF).Compared with PS patients,sGTCS patients showed that MD in bilateral ATR,ipsilateral CT,CC (hippocampus),IFPF andcontralateral SLF,forceps minor was increased.Compared with contralateral white matter respectively,the ipsilateral ATR,CC (hippocampus),IFOF,ILF,uncinate fasciculus(UF),SLF(temporal part) in sGTCS patients and ILF,UF in PS patients showed significant white matter alterations (P ＜ 0.05).Conclusion The sGTCS patients with unilateral mTLE-HS were more extensive than PS patients in cerebral white matter damage,and the ipsilateral hemisphere were more extensive than the contralateral hemisphere.The detection of these white matter structures provides an radiological basis for the different clinical manifestations of mTLE-HS.

Abstract:Objective To investigate the influence factors,such as epilepsy,preterm and history of hypoxic ischemic encephalopathy(HIE)effect on the white matter maturation in young children.Materials and Methods Sixty-seven children(ages ranged from 6 to 117 months with a mean of 51 months)underwent cranial MR examination.The DTI data were analyzed using tract-based spatial statistics(TBSS)to extract fractional anisotropy(FA)values from multiple white matter fibers.Eleven regions of interest were defined,including projection,association and commissural fibers.Analysis of variance was used to detect the differences of FA among the fiber tracts.The severity of epilepsy was scored on the basis of developmental evaluation.Linear regression analysis of the tract-specific measurements was conducted to identify age-related trends,the effect of epilepsy,preterm and history of HIE on FA values for each tract.Results There were statistically significant differences in FA among the different regions(F=162.676,P=0.000).Significant age-related trends were observed in FA values of the majority of fiber tracts(P<0.05),except retrolenticular part of the internal capsule(RLIC)and splenium of corpus callosum.History of HIE was found to show a significant inverse correlation with FA in some regions,including cerebral peduncle,posterior thalamic radiation,superior longitudinal fasciculus and RLIC(r=-0.212,P=0.036;r=-0.326,P=0.005;r=-0.269,P=0.021;r=-0.29,P=0.017,respectively,P<0.05).Epilepsy and preterm birth showed no relationship with FA values in any of the fiber tracts.Conclusion TBSS can be successfully applied in the analysis of pediatric DTI data.History of HIE is a definite factor leading to white matter abnormalities in children.

Abstract:Background and purpose: The present study was designed to map alterations in brain whitematter in photosensitive epilepsy (PSE) by applying tract-based spatial statistics (TBSS) analysis. Methods: Diffusion tensor-imaging (DTI) data from MRI brain scans were collected from eight PSE patients and 16 gender- and age-matched non-epileptic controls using a SIEMENS Trio 3.0-Testa scanner. For the white-matter analysis, DTI scans were processed using FSL software (http://www.fmrib.ox.ac.uk/fsl/index.html). Fractional anisotropy (FA) values in the PSE and control groups were compared using TBSS analysis corrected for multiple comparisons using threshold-free cluster enhancement.Results: Compared with the control subjects, the corpus callosum of PSE patients had significantly lower FA values.Conclusion: Our DTI study indicates that white-matter in the corpus callosum was abnormal in PSE patients, and that DTI methods can serve as useful non-invasive tools to evaluate whitematter changes in PSE patients. Brain structural changes in preschool children with MRI-negative epilepsy

Abstract:Purpose: To investigate abnormalities in cortical and subcortical structures of the brain in preschool children with MRI-negative epilepsy. Methods: Cortical thickness, cortical mean curvature, cortical surface area, cortical volume, and volumes of subcortical structures were measured using Freesurfer software in preschool children with epilepsy and age-matched controls. Results: Findings showed cortical thickening in the left fusiform gyrus, left middle temporal gyrus, right suborbital sulcus, and right gyrus rectus, and cortical thinning mainly in the parietal lobe of preschool children with epilepsy compared to controls. The difference in cortical thickness in the left superior parietal lobule remained after correction for multiple comparisons and was negatively correlated with duration of epilepsy. Cortical mean curvature, surface area, and volume were mainly altered in the frontal and temporal lobes. Changes in mean curvature in the right pericallosal sulcus were positively correlated with age at seizure onset, and changes in mean curvature in the left intraparietal sulcus and transverse parietal sulcus were positively correlated with frequency of seizures. There were no significant differences in the volumes of the subcortical structures. Conclusion: Changes in preschool children with epilepsy occur in the cortical rather than subcortical structures of the brain. These findings further our understanding of the effects of epilepsy in preschool children and will inform management of epilepsy in this patient population.
Differences between Tuberous Sclerosis Complex Patients with and without Epilepsy: The Results of a Quantitative Diffusion Tensor Imaging Study

Abstract:Introduction: Tuberous sclerosis complex (TSC) is a neurocutaneous disease with a high incidence of epilepsy and damaging effects on cognitive development. To understand the mechanisms leading to abnormal cognitive development, diffusion tensor imaging (DTI) techniques have begun to be used in recent years. The present study is the first to investigate differences in the microstructure and integrity of white matter tracts in adult patients with TSC and with and without epilepsy. Method: A total of 37 patients with TSC (18 with epilepsy, median age 36 years; 19 without epilepsy, median age 35 years) without intellectual disability and autism spectrum disorder were included in the study. The control group (median age 34 years) comprised 37 individuals without psychiatric or neurodevelopmental disorders and neurological and cardiovascular diseases, diabetes, or addictions. A magnetic resonance imaging (MRI) DTI sequence was applied. Results: There were differences in the average values of DTI parameters between patients with TSC and epilepsy and patients with TSC but without epilepsy in five white matter bands. When comparing the average values of DTI parameters between patients with TSC and epilepsy and healthy controls, we found differences in 15 of 20 analysed white matter fibres. White matter tracts in patients with TSC and epilepsy had more abnormalities than in patients with TSC but without epilepsy. The former group presented abnormalities in longer white matter fibres, especially in the left hemisphere. However, the latter group presented abnormalities in more medial and shorter white matter fibres. Conclusion: This DTI study documents the changes in the brain white matter of patients with TSC associated with the presence of epilepsy.

Abstract:The structural connection patterns of the human brain are the underlying bases for functional connectivity. Although abnormal functional connectivity has been uncovered in childhood absence epilepsy (CAE) in previous electroencephalography and functional magnetic resonance imaging studies, little is known regarding the structural connectivity in CAE. We hypothesized that the structural connectivity would be disrupted in response to the decreased brain function in CAE.Diffusion tensor imaging tractography was utilized to map the white matter (WM) structural network, composed of 90 cortical and sub-cortical regions, in 18 CAE and 18 age- and gender-matched healthy controls. Graph theoretical methods were applied to investigate the alterations in the topological and nodal properties of the networks in these patients.Both the CAE and the controls showed small-world properties in their WM networks. However, the network connection strength, absolute clustering coefficient, and global/local efficiency were significantly decreased, but characteristic path length was significantly increased in the CAE compared with the controls. Significantly decreased WM connections, nodal properties, and impaired sub-networks were found in the sub-cortical structures, orbitofrontal area, and limbic cortex in the CAE. Moreover, network connection strength, local efficiency, and nodal features in some regions were significantly negatively correlated with the duration of epilepsy.The present study demonstrated, for the first time, the disrupted topological organization of WM networks in CAE. The decreased connectivity and efficiency in the orbitofrontal and sub-cortical regions may serve as anatomical evidence to support the functional abnormalities related to the epileptic discharges observed in CAE. Moreover, the orbitofrontal sub-network may play a key role in CAE. These findings open up new avenues toward the understanding of absence epilepsy.
Correlations Between Structural Brain Abnormalities, Cognition and Electroclinical Characteristics in Patients With Juvenile Myoclonic Epilepsy

Abstract:Objective: To explore the structural brain abnormality and its relationship with neuropsychological disorders and electroclinical characteristics in juvenile myoclonic epilepsy (JME) patients. Methods: Sixty-seven patients diagnosed with JME and 56 healthy controls were enrolled. All subjects underwent MRI using T1-weighted 3D brain structural images with 1 mm thickness. Voxel-based morphometry (VBM) and surface-based morphometry (SBM) analyses were performed. They also underwent a series of neuropsychological tests to assess cognitive function. The correlation analyses were conducted between structural changes, neuropsychological outcomes, and electroclinical features. Results: The gray matter concentration (GMC) was decreased in the bilateral pre-central and post-central gyrus, right anterior cingulate gyrus, left posterior orbital region, bilateral occipital regions, bilateral hippocampus and bilateral caudate nucleus in the JME groups (corrected P < 0.05). The evaluation of gray matter volume (GMV) showed significant decrease respectively in bilateral pre-central and post-central gyrus, left paracentral lobule, left orbital gyrus, left amygdala, left basal ganglia and left thalamus of JME patients ( P < 0.05). The cortex thicknesses of the right inferior temporal gyrus, right insular gyrus, and right cingulate gyrus had negative correlations with the disease duration significantly. At the same time, the whole-brain white matter volume was positively associated with the course of the disease ( P < 0.05). Patients with persistent abnormal EEG discharges had significantly less whole-brain gray matter volume than JME patients with normal EEG ( P = 0.03). Correlation analyses and linear regression analyses showed that, in addition to the gray matter volumes of frontal and parietal lobe, the temporal lobe, as well as the basal ganglia and thalamus, were also significantly correlated with neuropsychological tests' results ( P < 0.05). Conclusion: The JME patients showed subtle structural abnormalities in multiple brain regions that were not only limited to the frontal lobe but also included the thalamus, basal ganglia, parietal lobe, temporal lobe and some occipital cortex, with significant involvement of the primary somatosensory cortex and primary motor cortex. And we significantly demonstrated a correlation between structural abnormalities and cognitive impairment. In addition, the course of disease and abnormal discharges had a specific negative correlation with the structural changes.
