Abstract:Introduction: Obstructive sleep apnea (OSA) is a respiratory disorder that contributes to the onset of cardiovascular disease. Repeated obstructive apneas produce a pattern of intermittent hypoxia that results in a sustained state of hypercoagulability. Therefore, patients with OSA are predisposed to develop venous thromboembolic disease (VTE). Given the emerging evidence suggesting an interaction between OSA and VTE, it becomes essential to determine the prevalence of OSA in patients with VTE. Methodology: We conducted a systematic review and meta-analysis including studies that provide data on the prevalence of OSA in patients with acute VTE. The search was conducted through Pubmed, Cochrane and Embase electronic databases. A random-effects model was used to estimate the overall prevalence of OSA in the VTE population. Results: From 3289 articles, 7 studies were elegible including 2784 patients with VTE, of whom 409 had OSA. Polysomnography with an apnea-hypopnea index (AHI) ≥ 5 was the most common diagnostic method. The prevalence of OSA ranged from 26.67% (1:3.7) to 82.36% (1:1.2); overall prevalence: 67% (95% CI 51; 81); the prevalence using OSA criteria according to AHI ≥15 ranged from 26.67% (1:3.7) to 63.24% (1:1.6); overall prevalence 35% (95% CI 13:61). Heterogeneity between studies was high. Conclusions: OSA seems to be positioned as a pathology with a high prevalence among patients with VTE. More studies are needed to provide clear, robust and precise data.

Abstract:Abstract Introduction Morbidity and mortality in patients with untreated obesity hypoventilation syndrome (OHS) is significant. The vast majority of OHS patients have concomitant obstructive sleep apnea (OSA), known to be associated with worse cardiovascular outcomes following bariatric surgery. However, there is limited data on the post-bariatric surgery outcomes of patients with OHS. Methods We queried TriNetX Analytics, a federated health record and claims-derived database of >115 million patients across 5 countries. We established two cohorts of patients with OSA (ICD-10-CM G47.33) who underwent laparoscopic bariatric surgery (CPT 1007387) between 5/1/2008-5/1/2023: one with superimposed OHS (E66.2) and one without. We examined the 30-day incidence of deep vein thrombosis [DVT (I82)], pulmonary embolism [PE (I26)], stroke [CVA (I63)], and myocardial infarction [MI (I21)]. We then re-examined these outcomes following extensive in-platform propensity score matching (PSM) to account for demographic and comorbidity antecedents. Results A total of 66,085 bariatric surgery patients were isolated, all in the US. Of these, 26,392 (40%) were diagnosed with OSA prior to surgery, of which 1,176 (1.8%) also carried a diagnosis of OHS. Prior to PSM, the OSA+OHS cohort experienced a significant excess incidence of all outcomes besides CVA when compared to the cohort with OSA alone: DVT risk was 3.9 vs 1.1% (OR 3.5, CI 2.6-4.4), PE risk 2.6 vs 0.9% (OR 3.0, CI 2.1-4.5), composite DVT/PE risk 5.4 vs 1.8% (OR 3.2, CI 2.4-4.1), CVA risk 0.9 vs 0.6%, MI risk 1.4 vs 0.4% (OR 3.3, CI 2.0-5.5), composite CVA/MI risk 2.0 vs 1.0% (OR 2.0, CI. 1.3-3.2). Following PSM, which generated two like sub-cohorts of 1,167 each, significant risk excess was retained only for DVT incidence: DVT risk was 3.8 vs 2.0% (OR 1.9, CI 1.2-3.2), but not for PE risk 2.4 vs 2.1%, DVT/PE risk 5.1 vs 3.7%, CVA risk 0.9 vs 1.2%, MI risk 1.5 vs 1.5%, and CVA/MI risk 2.0 vs 2.4%. Conclusion Our findings suggest that patients with OSA and OHS undergoing bariatric surgery experience similar rates of adverse cardiovascular events in the first postoperative month as those with OSA alone. The exception is DVT risk, which appears greater in the OHS-OSA cohort despite matching. Support (if any)

Association of Incident Obstructive Sleep Apnoea with Outcomes in a Large Cohort of US Veterans

Abstract:Rationale There is a paucity of large cohort studies examining the association of obstructive sleep apnoea (OSA) with clinical outcomes including all-cause mortality, coronary heart disease (CHD), strokes and chronic kidney disease (CKD).Objectives We hypothesised that a diagnosis of incident OSA is associated with higher risks of these adverse clinical outcomes.Methods, measurements In a nationally representative cohort of over 3 million (n=3 079 514) US veterans (93% male) with baseline estimated glomerular filtration rate (eGFR)=60 mL/min/1.73 m(2), we examined the association between the diagnosis of incident OSA, treated and untreated with CPAP, and: (1) all-cause mortality, (2) incident CHD, (3) incident strokes, (4) incident CKD defined as eGFR< 60 mL/min/1.73 m(2), and (5) slopes of eGFR.Main results Compared with OSA-negative patients, untreated and treated OSA was associated with 86% higher mortality risk, (adjusted HR and 95% CI 1.86 (1.81 to 1.91) and 35% (1.35 (1.21 to 1.51)), respectively. Similarly, untreated and treated OSA was associated with 3.5 times (3.54 (3.40 to 3.69)) and 3 times (3.06 (2.62 to 3.56)) higher risk of incident CHD; 3.5 times higher risk of incident strokes (3.48 (3.28 to 3.64) and 3.50 (2.92 to 4.19)) for untreated and treated OSA, respectively. The risk of incident CKD was also significantly higher in untreated (2.27 (2.19 to 2.36)) and treated (2.79 (2.48 to 3.13)) patients with OSA. The median (IQR) of the eGFR slope was -0.41 (-2.01 to 0.99), -0.61 (-2.69 to 0.93) and -0.87 (-3.00 to 0.70) mL/min/1.73 m(2) in OSA-negative patients, untreated OSA-positive patients and treated OSA-positive patients, respectively.Conclusions In this large and contemporary cohort of more than 3 million US veterans, a diagnosis of incident OSA was associated with higher mortality, incident CHD, stroke and CKD and with faster kidney function decline.

Abstract:Abstract Introduction Obesity is a well-known risk factor for obstructive sleep apnea (OSA), gaining interest due to the global rise in obesity and novel obesity drugs. The rationale behind this lies in the role of obesity in the pathophysiology of OSA. However, many OSA patients are non-obese, and the association between obesity and OSA may vary by age and sex. Past population studies largely reported the prevalence of OSA according to different obesity categories. In this study, we sought to determine the prevalence of obesity across different levels of OSA severity, age, and sex. Methods The analytic sample included individuals with OSA from the Sleep Heart Health Study (SHHS). OSA severity was quantified using the apnea-hypopnea index (AHI4; apneas plus hypopneas with at least 4% oxygen desaturation per hour). Individuals were stratified by the severity of OSA (mild: 5≤AHI4< 15; moderate: 15≤AHI4< 30; severe: AHI4≥30 events/hour), body mass index (BMI), age, and sex. Results A total of 2950 of 5749 participants in SHHS had OSA (AHI4≥5 events/hour; 1729/788/433 mild/moderate/severe). Of these, 40.6% were female; mean±SD age was 65.3±10.7 years; and median [interquartile range] BMI was 28.8 [25.8; 32.3] kg/m2. For those with any OSA, 60.6% were non-obese (BMI< 30 kg/m2) and 39.4% were obese (BMI≥30 kg/m2). Across OSA severities, 34.1% of mild, 42.6% of moderate and 54.3% of severe OSA were obese. A greater proportion of younger adults (< 65 years) with OSA were obese (48.3%) than older adults (≥65 years) with OSA (30.9%). In older adults 26.7% of mild, 34.5% of moderate and 40.3%of severe OSA were obese. Finally, a greater proportion of females with OSA were obese (43.6%) than males with OSA (36.5%). Conclusion The majority of middle-aged/older adults with OSA are non-obese. The association between obesity and OSA varies by age and sex, with a higher prevalence of obesity in younger adults and in females with OSA. Additional analyses of data from the Multi-Ethnic Study of Atherosclerosis, the Osteoporotic Fractures in Men study, and the HypnoLaus study are being conducted to establish the association between OSA and obesity within other community-based samples. Abstract:Background: The effect of OSA on the severity and prognosis of PE remains unclear. Aims and Objectives: To investigate the association between OSA and severity as well as clinical outcomes of patients with acute PE. Methods: A prospective study of patients who underwent type I polysomnography 2 months after acute stable PE. The simplified Pulmonary Embolism Severity Index (sPESI) was used to evaluate acute PE severity. Associations between indexes of OSA severity and risk of belonging to sPESI≥1 group were investigated, after adjustments for confounders. PAP therapy was administered to all patients with OSA. Cox regression analysis was used to examine the relationship between OSA severity and long-term cardiovascular (CV) events. Results: Of the 57 patients with acute PE (mean age 63 years), 60% had OSA (AHI≥5). OSA patients had a significantly higher rate of sPESI≥1 (77% vs. 57%, p=0.003). All indexes of OSA severity (AHI, ODI, mean and lowest SaO2 and TST90) were independently associated with increased risk of severe PE (sPESI≥1) (all p<0.05). During an average follow-up of 22 months, 12% of patients experienced CV complications, with reduced PAP use (<6 hours/day) as a significant predictor (OR: 1.59; 95% CI 1.19–1.86; p=0.002). Conclusion: OSA is a common disease in patients with acute PE and is associated with increased OSA severity, while reduced adherence to OSA treatment is associated with an increased risk of cardiovascular complications. The implementation of routine OSA screening and management could potentially contribute to risk stratification and improve outcomes among acute PE patients.

Abstract:The identification of which patients with obstructive sleep apnea (OSA) are more likely to develop cardiovascular disease (CVD) remains a challenge. OSA causes oxidative stress (OS) which may contribute to CVD pathogenesis. Therefore, OS markers could be useful in risk-stratifying cardiovascular (CV) risk in OSA patients. The purpose of this pilot study was to assess whether three OS marker levels could be associated with incident CVD in suspected OSA patients. Morning plasma levels of 8-isoprostane, 8-hydroxy-2′-deoxyguanosine (8-OHdG) and superoxide dismutase (SOD) were measured in patients with suspected OSA referred for a polysomnogram (PSG). A composite outcome of CV events was defined by linkage with provincial administrative health databases. Cox proportional hazards models were used to assess the relationship between the levels of OS markers and events. 352 patients were included (mean age of 51.4 years, 68% male, median apnea hypopnea index of 16/h). Thirty-one first CV events occurred over an 8-year follow-up. In univariate or fully adjusted models, none of the OS markers were significantly associated with incident CV events (hazard ratio in adjusted models of: 1.25 (95% CI 0.56–2.80, p = 0.59), 1.15 (0.52–2.57, p = 0.73), 0.77 (0.37–1.61, p = 0.48), for 8-OHdG, 8-isoprostane and SOD; however, confidence intervals were wide. In this small preliminary study, oxidative stress markers were not significantly associated with risk of CV events. However, moderate associations between these markers and risk of CV events are possible and should be the focus of future larger studies.

Abstract:Background: In older adults, obstructive sleep apnea (OSA) has been associated with several cardiovascular complications. Whether young patients diagnosed with OSA also are at higher risk of developing subsequent cardiovascular disease is uncertain. We aimed to estimate the risk of developing an incident cardiovascular event among young patients diagnosed with OSA. Methods and results: We linked nationwide Danish health registries to identify a cohort of patients aged ≤50 years with OSA using data from 2010 through 2018. Cases without OSA from the general population were matched as controls (1:5). The main outcome was any cardiovascular event (including hypertension, diabetes, atrial fibrillation, ischemic heart disease, ischemic stroke, heart failure, and venous thromboembolism). All-cause mortality was a secondary outcome. The study included 20 240 patients aged ≤50 years with OSA (19.6% female; mean±SD age 39.9±7.7 years) and 80 314 controls. After 5-year follow-up, 31.8% of the patients with OSA developed any cardiovascular event compared with 16.5% of the controls, with a corresponding relative risk (RR) of 1.96 (95% CI, 1.90-2.02). At 5-year follow-up, 27.3% of patients with OSA developed incident hypertension compared with 15.0% of the controls (RR, 1.84 [95% CI, 1.78-1.90]). Incident diabetes occurred in 6.8% of the patients with OSA and 1.4% of controls (RR, 5.05 [95% CI, 4.60-5.54]). Conclusions: Similar to older adults, young adults with OSA demonstrate increased risk of developing cardiovascular events. To prevent cardiovascular disease progression, accumulation of cardiovascular risk factors, and mortality, risk stratification and prevention strategies should be considered for these patients.

Abstract:Background: Obstructive sleep apnoea (OSA) involves especially obese patients and causes systemic organ damage. Little is known about the impact of body fat distribution. Objective: To evaluate the impact of the visceral adipose tissue (VAT) on respiratory and bone variables. Methods: In our prospective study, 49 untreated mild-to-severe OSA patients underwent dual-energy X-ray absorptiometry. Data about polygraphy were also collected. According to the recent reference values for European adults, patients were divided by the sex-related threshold of the VAT index into two categories: VAT index within limits (nVAT) and increased (iVAT). Results: 63% of patients were iVAT. Compared with nVAT, iVAT patients had a higher prevalence of arterial hypertension (52% vs 22%) and diabetes (32% vs 6%) and higher values of nocturnal desaturation (median 4.6% vs 3.7%; p=0.002). Patients with iVAT had, in comparison to nVAT, lower values of the lumbar spine trabecular bone score (TBS) L1-L4 (mean 1.24 vs 1.39; p<0.001), T-score (mean -1.82 vs –0.52; p<0.001) and Z-score (mean –0.35 vs 0.75; p=0.002). Furthermore, altered values of the TBS Z-score were associated with vertebral fractures. Finally, in two linear regression models, adjusted for age. sex and severity of OSA, VAT index predicted nocturnal desaturation (β 2.26; 95% CI 0.45 to 4.07; t 2.52; p=0.016) and TBS lumbar spine L1-L4 (β –0.43; 95% CI –0.54 to –0.31; t -7.33; p<0.001). Conclusion: In OSA patients, VAT impacts nocturnal desaturation and alters the bone microarchitecture. Some considerations should be made on the role of VAT as a metabolically active tissue.

Abstract:Overlap syndrome (OVS) is a distinct clinical entity that seems to result in potential cardiovascular consequences. We aimed to estimate the prevalence and risk factors for OVS in OSA patients and analyze clinical and PSG characteristics associated with OVS. In this cross-sectional study, 2616 patients evaluated for OSA underwent type-1 polysomnography (PSG). They were grouped as pure OSA (AHI > 15/h) and OVS patients. Demographics, PSG data, pulmonary function tests and arterial blood gases (ABGs) were compared between groups after adjustments for confounders. OSA was diagnosed in 2108 out of 2616 patients. Of those, 398 (19%) had OVS. Independent predictors of OVS were older age [OR: 5.386 (4.153–6.987)], current/former smoking [OR: 11.577 (7.232–18.532)], BMI [OR: 2.901 (2.082–4.044)] and ABG measurements [PaCO2 ≥ 45 OR: 4.648 (3.078–7.019), PO2 [OR: 0.934 (0.920–0.949)], HCO3− [OR: 1.196 (1.133–1.263), all p < 0.001]. OVS was also associated with prevalent hypertension [OR: 1.345 (1.030–1.758), p = 0.03] and cardiovascular disease [OR: 1.617 (1.229–2.126), p < 0.001], depressive symptoms [OR: 1.741 (1.230–2.465), p = 0.002] and nocturia [OR: 1.944 (1.378–2.742), p < 0.001], as well as with indices of OSA severity. Disturbances in sleep architecture were more prominent in OVS expressed by lower %N3 and REM% and higher arousal index. Our data suggest that OVS is prevalent among OSA patients, with distinct clinical and PSG characteristics. These characteristics could be utilized as predictive factors for early identification and further evaluation of these patients towards desirable patient-reported outcomes.

Abstract:BACKGROUND: Obstructive sleep apnea (OSA) is a known independent risk factor for a multiple cardiovascular morbidities and mortality. The association of OSA and ventricular arrhythmias is less well understood. The aim of this analysis is to study the relationship between OSA and ventricular tachyarrhythmias.HYPOTHESIS: OSA is associated with increased ventricular arrhythmias.METHODS: Data from the national inpatient sample (NIS) 2012 to 2014, were reviewed. Discharges associated with OSA were identified as the target population using the relevant ICD-9-CM codes. The primary outcome was a diagnosis of ventricular tachycardia (VT) in the OSA population. Secondary outcomes include the rate of ventricular fibrillation (VF) and cardiac arrest. Multivariable analyses were performed to examine the association of VT with multiple potential confounding clinical variables.RESULTS: Of 18 013 878 health encounters, 943 978 subjects (5.24%) had a diagnosis of OSA. VT and VF were more prevalent among patients with OSA compared to those without a diagnosis of OSA (2.24% vs 1.16%; P < 0.001 and 0.3% vs 0.2%; P < 0.001, respectively). Odds ratio for cardiac arrest in OSA group was not statistically significant (1, 95% confidence interval 0.97-1.02, P < 0.76). In unadjusted analyses, all examined comorbidities were significantly more common in those with OSA, including diabetes mellitus, hypertension, chronic kidney disease, acute coronary syndrome, and heart failure.CONCLUSION: OSA is associated with increased rates of ventricular tachyarrhythmia.

Abstract:Obstructive sleep apnea (OSA) may affect cardiovascular risk. Prior studies examining this issue have often been limited to patients in clinical settings or older adults. Given the high prevalence of obstructive sleep apnea in middle-aged adults and the influence of duration of OSA on cardiovascular risk, examining subclinical cardiovascular in middle aged-adults would further elucidate the direct relationship between OSA and cardiovascular disease. We examined the association between risk of OSA and subclinical cardiovascular disease indicators among 914 young-to-middle aged adults who responded to the Berlin Questionnaire assessment of OSA risk in 2010, and had measures of carotid intima-media thickness (IMT) and left ventricular (LV) geometry. Carotid IMT was measured using standard procedures and categorized into quartiles. Indices of LV geometry were assessed by M-mode echocardiography and classified into normal, concentric remodeling, eccentric hypertrophy, and concentric hypertrophy by integrating gender and race specific relative wall thickness and LV mass index. High-risk for OSA was determined using Berlin Questionnaire score as the primary outcome. Secondary outcomes included habitual snoring and excessive sleepiness. Of those included in the analysis, mean (SD) age was 43.1 (4.5) yrs; 42.1% were male and 31.7% were Black. A total of 235 (25.7%) participants had elevated Berlin scores indicating high-risk for OSA. Mean (SD) of carotid IMT was 0.66 (0.15) mm, and 87 (9.5%) and 161 (17.6%) participants had eccentric and concentric LV hypertrophy, respectively. In log-linear regression models adjusted for age, race, sex, education, current smoking, regular alcohol consumption, total cholesterol, high-density lipoprotein cholesterol, and type-2 diabetes, participants at high-risk of OSA were 1.35 (95% CI: 1.07 - 1.71) times more likely to be in the highest quartile of IMT (≥0.74 mm), were 1.68 (1.09 - 2.58) times as likely to have concentric hypertrophy, and 1.64 (0.95 - 2.82) times as likely to have eccentric hypertrophy, compared to those at low-risk of OSA. Similar positive associations were observed for habitual snoring but not for excessive sleepiness. After further adjusting for current obesity status in the models, the association of OSA risk with IMT remained consistent while the associations with LV hypertrophy was attenuated. There was no significant effect modification by race or sex. In summary, being at high risk OSA was associated with substantially higher risk of subclinical cardiovascular disease in this biracial, semi-rural, community-based population of young-to-middle age adults.

Abstract:Abstract Introduction There is lack of clarity of sleep disordered breathing (SDB)--including the role of nocturnal hypoxia and confounding influence of obesity--on the clinical course of human coronavirus disease 2019 (COVID-19). We postulate that SDB portends increased risk of adverse COVID-19 clinical outcomes even after accounting for confounding factors. Methods A retrospective cohort analysis of COVID-19 and sleep laboratory observational registries March-November 2020 within the Cleveland Clinic health system was performed. Ordinal logistic regression assessed the association of SDB indices and World Health Organization (WHO)-7 COVID-19 clinical outcome (hospitalization, use of supplemental oxygen, non-invasive ventilation, mechanical ventilation/ECMO and death) in an unadjusted model and adjusted for age, sex, race, body mass index(BMI,kg/m2),diabetes mellitus, hypertension, coronary artery disease, heart failure, asthma, chronic obstructive pulmonary disease (COPD), cancer and smoking using SAS software. Results Of 19,449 (32%) patients positive for SARS-CoV-2,2,290 (6%) had an available sleep study. The analytic sample included 1788 of which 1,484(64%) had an apnea hypopnea index (AHI, 3–4% hypopnea oxygen desaturation)≥5. The median duration from sleep study to COVID test was 5.8 years (IQR:3.3–9.0). Age was 56.5±14.4 years,50.4% female,28% African American with BMI=35.9±8.9kg/m2. Nine percent of patients were hospitalized,10% with supplemental oxygen,6% used non-invasive ventilation,2% required ECMO or mechanical ventilation and 2% died. For every AHI increase of 5, the odds of a higher WHO-7 level increased 2% (OR=1.02,95%CI1.01-1.04,p=0.005),but the association was mitigated in the adjusted model (OR=1.00,95%CI:0.98,1.02,p=0.80). Per 5% increase in time spent with SaO2<90%, the odds of a higher WHO-7 level increased 10% (OR=1.10,95%CI1.06-1.13,p=<0.001) persisting in the adjusted model(OR=1.06,95%CI:1.02–1.10,p=0.002). For every decrease of 5% mean SaO2, the odds of a higher level WHO-7 increased 56% (OR=0.56,95%CI:0.46–0.67,p<0.001) persisting in the adjusted model(OR=0.72,95%CI:0.58–0.89,p=0.003). Conclusion Even after adjustment for obesity, underlying cardiopulmonary disease and smoking, sleep-related hypoxemia was a potential key pathophysiologic mechanism associated with increased morbidity and mortality in COVID-19. Elucidation of sleep-related hypoxemia as a risk stratification measure, particularly given the silent hypoxia inherent to early COVID-19, is critical for future investigation, as is the role of sleep-related hypoxia reversal as a target to improve COVID-19 outcomes. Abstract:Background Obstructive sleep apnea (OSA) is frequently seen with atrial fibrillation (AF) and is associated with increased cardiovascular morbidity, including hypertension, congestive heart failure, ischemic heart disease, and stroke. However, the impact of OSA on in-hospital outcomes of patients with AF is unclear. Methodology All patients aged ≥18 admitted primarily for AF between January 2016 and December 2017 were identified in the National Inpatient Sample database. They were then categorized into those with OSA and those without OSA. The primary outcome was in-hospital mortality. Unadjusted and adjusted analysis was performed on appropriate variables of interest. Results Of 156,521 primary AF hospitalizations, 15% of the patients had OSA. Baseline characteristics revealed no race disparity between the two groups. However, compared to those without OSA, the OSA group was younger and had a significantly higher proportion of males, obesity, heart failure, hypertension, chronic obstructive pulmonary disease, diabetes, and hyperlipidemia. Long-term anticoagulation and inpatient cardioversion were also higher in the OSA group. Following propensity matching, inpatient mortality was similar between the two groups [0.54% in OSA vs. 0.51% in non-OSA; adjusted odds ratio = 1.06 (95% confidence interval = 0.82-1.35)]. Similarly, OSA was not significantly associated with acute kidney injury, cardiac arrest, gastrointestinal bleed, acute stroke, or length of stay. However, the OSA group was less anemic and required fewer in-hospital blood transfusions. Conclusions Although OSA is highly prevalent in AF patients, inpatient mortality and cardiovascular outcomes such as cardiac arrest, stroke, or major bleeding were similar in AF patients with or without concomitant OSA with no significant differences in length of stay.
The Associations of Platelet Activation and Coagulation Parameters with Obstructive Sleep Apnoea: A Large-Scale Observational Study.

Abstract:Objectives Obstructive sleep apnoea (OSA) is associated with an increased risk of cardiovascular disease, with alterations in coagulability suspected as the mediating factor. This study explored blood coagulability and breathing-related parameters during sleep in patients with OSA. Design Cross-sectional observational study. Setting. Shanghai Sixth People's Hospital. Participants. 903 patients diagnosed by standard polysomnography. Main Outcome and Measures. The relationships between coagulation markers and OSA were evaluated using Pearson's correlation, binary logistic regression, and restricted cubic spline (RCS) analyses. Results The platelet distribution width (PDW) and activated partial thromboplastin time (APTT) decreased significantly with increasing OSA severity (both p < 0.001). PDW was positively associated with the apnoea-hypopnea index (AHI), oxygen desaturation index (ODI), and microarousal index (MAI) (ß = 0.136, p < 0.001; ß = 0.155, p < 0.001; and ß = 0.091, p = 0.008, respectively). APTT was negatively correlated with AHI (ß = −0.128, p < 0.001) and ODI (ß = −0.123, p = 0.001). PDW was negatively correlated with percentage of sleep time with oxygen saturation below 90%(CT90) (ß = −0.092, p = 0.009). The minimum arterial oxygen saturation (SaO2) correlated with PDW (ß = −0.098, p = 0.004), APTT (ß = 0.088, p = 0.013), and prothrombin time (PT) (ß = 0.106, p = 0.0003). ODI was risk factors for PDW abnormalities (odds ratio (OR) = 1.009, p = 0.009) after model adjustment. In the RCS, a nonlinear dose-effect relationship was demonstrated between OSA and the risk of PDW and APTT abnormalities. Conclusion Our study revealed nonlinear relationships between PDW and APTT, and AHI and ODI, in OSA, with AHI and ODI increasing the risk of an abnormal PDW and thus also the cardiovascular risk. Abstract:Abstract Introduction Sleep disordered breathing (SDB), including obstructive sleep apnea (OSA) and obesity-associated sleep hypoventilation (OASH), has well-characterized adverse effects on the cardiovascular system and increases morbidity and mortality. Long-term impact on cardiovascular outcomes post-bariatric surgery, however, remains unclear. We hypothesize that patients with SDB have increased frequency of major adverse cardiovascular events (MACE) post-bariatric surgery than those without. Methods Patients undergoing polysomnography (PSG) prior to bariatric surgery at The Cleveland Clinic from 2011–2018 were retrospectively examined and followed up from date of last surgery to 2019, including the perioperative period. Primary predictors include moderate-severe OSA, i.e. apnea hypopnea index(AHI)>15, and OASH, i.e. body mass index (BMI)≥30kg/m2 and either end-tidal CO2≥45mmHg or serum bicarbonate≥27mEq/L. MACE (coronary artery events, cerebrovascular events, heart failure or atrial fibrillation)-free probability was compared using hazard ratios estimated from Cox proportional hazards models on four groups: OASH with moderate-severe OSA (N=492), OASH-only (N=442), moderate-severe OSA-only (N=203), and a reference group without OASH or moderate-severe OSA (N=243). Multivariable Cox proportional hazards models adjusting for age, sex, BMI were fit on MACE survival. Analysis was performed based on an overall significance level of 0.05, using SAS software (version 9.4, Cary, NC). Results The sample comprised 1380 patients: age: 43.5±12 years, BMI: 49±9 kg/m2, 17.7% male, 63.7% White. Risk of MACE across the groups bordered significance (p=0.051). Compared to the reference group, the OASH with moderate-severe OSA group had higher risk of MACE (HR2.53, 95%CI:1.07–6.00,p=0.035). Patients with moderate-severe OSA had higher risk of MACE than those with AHI<15 (HR1.94, 95%CI:1.20–3.13,p=0.007). Patients with severe OSA had higher risk of MACE than those AHI<30 (HR2.01, 95%CI:1.28–3.14,p=0.002). For every 5-unit AHI increase, risk of MACE increased by 6% (HR1.056, 95%CI:1.029–1.084,p<0.001) with slight reduction in point estimates in adjusted models. Conclusion Preliminary data from this largest-to-date sample of systematically phenotyped patients with SDB undergoing bariatric surgery show significant differences in risk of MACE and MACE-free survival mitigated after consideration of obesity. The Association of Obstructive Sleep Apnea With Breast Cancer Incidence and Mortality: A Systematic Review and Meta-analysis

Abstract:Purpose: Emerging evidence from animal models suggests that intermittent hypoxia due to obstructive sleep apnea (OSA) is a risk factor for breast cancer. Despite their biological plausibility, human epidemiological studies have reported conflicting results. Therefore, we conducted a meta-analysis to delineate this relationship. Methods: We searched the PubMed, Embase, Scopus, and Cochrane Library databases for eligible studies from inception until June 6, 2021. Two reviewers selected randomized trials or observational studies reporting the association between OSA and breast cancer incidence compared with those without OSA. Two reviewers extracted relevant data and assessed the quality of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) framework and Newcastle-Ottawa Scale (NOS). We pooled the maximally covariate-adjusted hazard ratios (HRs) using a random-effects inverse variance-weighted meta-analysis and performed pre-specified subgroup analyses. Results: We included six studies out of 1,707 records, comprising a combined cohort of 5,165,200 patients. All studies used the International Classification of Diseases codes to classify OSA and breast cancer. OSA patients had a 36% increased breast cancer risk (HR, 1.36; 95% confidence interval [CI], 1.03-1.80; N = 6, I² = 96%) compared to those without OSA. Most studies adjusted for confounders, such as age, sex, obesity, diabetes mellitus, alcohol use, and hypertension. Subgroup analyses for studies with (1) multivariate adjustment and (2) at least five years of follow-up yielded HRs of 1.35 (95% CI, 0.98-1.87; N = 5, I² = 96%) and 1.57 (95% CI, 1.14-2.18; N = 4; I² = 90%), respectively. One Mendelian randomization study suggested a causal relationship, with a two-fold increase in the odds of breast cancer in patients with OSA. Conclusion: This meta-analysis suggested that OSA is a risk factor for breast cancer. Future studies should explore the dose-response relationship between OSA and breast cancer, and whether treatment may mitigate breast cancer risk or progression.
Altered Cardiovascular Variability in Obstructive Sleep Apnea

Abstract:Background —Altered cardiovascular variability is a prognostic indicator for cardiovascular events. Patients with obstructive sleep apnea (OSA) are at an increased risk for cardiovascular disease. We tested the hypothesis that OSA is accompanied by alterations in cardiovascular variability, even in the absence of overt cardiovascular disease. Methods and Results —Spectral analysis of variability of muscle sympathetic nerve activity, RR interval, and blood pressure were obtained during undisturbed supine rest in 15 patients with moderate-to-severe OSA, 18 patients with mild OSA, and 16 healthy control subjects in whom sleep disordered breathing was excluded by complete overnight polysomnography. Patients with OSA were newly diagnosed, never treated for OSA, and free of any other known diseases. Patients with moderate-to-severe OSA had shorter RR intervals (793±27 ms) and increased sympathetic burst frequency (49±4 bursts/min) compared with control subjects (947±42 ms; 24±3 bursts/min; P =0.008 and P <0.001, respectively). In these patients, total variance of RR was reduced ( P =0.01) and spectral analysis of RR variability showed an increase in low frequency normalized units, a decrease in high frequency normalized units, and an increase in the ratio of low to high frequency (all P <0.05). Even though blood pressure was similar to that of the control subjects, blood pressure variance in patients with moderate-to-severe OSA was more than double the variance in control subjects ( P =0.01). Patients with mild OSA also had a reduction in RR variance ( P =0.02) in the absence of any significant difference in absolute RR interval. For all patients with OSA, linear regression showed a positive correlation ( r =0.40; P =0.02) between sleep apnea severity and blood pressure variance. Conclusions —Cardiovascular variability is altered in patients with OSA. This alteration is evident even in the absence of hypertension, heart failure, or other disease states and may be linked to the severity of OSA. Abnormalities in cardiovascular variability may be implicated in the subsequent development of overt cardiovascular disease in patients with OSA.

Abstract:BACKGROUND High plasma homocysteine is associated with premature coronary artery disease in men, but the threshold concentration defining this risk and its importance in women and the elderly are unknown. Furthermore, although low B vitamin status increases homocysteine, the link between these vitamins and coronary disease is unclear. METHODS AND RESULTS We compared 304 patients with coronary disease with 231 control subjects. Risk factors and concentrations of plasma homocysteine, folate, vitamin B12, and pyridoxal 5'-phosphate were documented. A homocysteine concentration of 14 mumol/L conferred an odds ratio of coronary disease of 4.8 (P < .001), and 5-mumol/L increments across the range of homocysteine conferred an odds ratio of 2.4 (P < .001). Odds ratios of 3.5 in women and of 2.9 in those 65 years or older were seen (P < .05). Homocysteine correlated negatively with all vitamins. Low pyridoxal 5'-phosphate (< 20 nmol/L) was seen in 10% of patients but in only 2% of control subjects (P < .01), yielding an odds ratio of coronary disease adjusted for all risk factors, including high homocysteine, of 4.3 (P < .05). CONCLUSIONS Within the range currently considered to be normal, the risk for coronary disease rises with increasing plasma homocysteine regardless of age and sex, with no threshold effect. In addition to a link with homocysteine, low pyridoxal-5'-phosphate confers an independent risk for coronary artery disease.
