Abstract:In-depth analysis of medical data through machine learning to achieve disease prediction is beneficial to the early detection and treatment of diseases. However, medical data involves mass patient privacy, and datasets of different medical institutions cannot be directly shared due to privacy protection. So medical data often exists in the form of data islands, which makes it difficult for most existing prediction models to complete disease prediction. In this paper, a digital twin assisted efficient clustering Federated Learning (FL) algorithm for disease prediction is proposed. It can break data islands to predict diseases on the premise of privacy security. Firstly, we design an efficient clustering Federated Learning with Client Selection (FLCS) protocol based on heterogeneity and contribution to improve the training efficiency and prediction accuracy. Secondly, we use digital twin to assist the FLCS protocol to carry out large-scale prediction. In addition, the shapley value introduced in the calculation of client contribution makes the model interpretable and enhances the reliability of prediction results. Finally, the evaluation results show that compared with the common prediction models and FedAvg algorithm, the FLCS protocol assisted by digital twin has better efficiency and accuracy in binary classification prediction.

Abstract:Hematoxylin and Eosin (H&E) staining of whole slide images (WSIs) is considered the gold standard for pathologists and medical practitioners for tumor diagnosis, surgical planning, and post-operative assessment. With the rapid advancement of deep learning technologies, the development of numerous models based on convolutional neural networks and transformer-based models has been applied to the precise segmentation of WSIs. However, due to privacy regulations and the need to protect patient confidentiality, centralized storage and processing of image data are impractical. Training a centralized model directly is challenging to implement in medical settings due to these privacy <a class="link-external link-http" href="http://concerns.This" rel="external noopener nofollow">this http URL</a> paper addresses the dispersed nature and privacy sensitivity of medical image data by employing a federated learning framework, allowing medical institutions to collaboratively learn while protecting patient privacy. Additionally, to address the issue of original data reconstruction through gradient inversion during the federated learning training process, differential privacy introduces noise into the model updates, preventing attackers from inferring the contributions of individual samples, thereby protecting the privacy of the training <a class="link-external link-http" href="http://data.Experimental" rel="external noopener nofollow">this http URL</a> results show that the proposed method, FedDP, minimally impacts model accuracy while effectively safeguarding the privacy of cancer pathology image data, with only a slight decrease in Dice, Jaccard, and Acc indices by 0.55%, 0.63%, and 0.42%, respectively. Abstract:Deep learning and computer vision (CV) can easily recognize pictures that the human doctor would have to spend a lot of time and labor to process. Thus, they have a wide range of applications in the field of ophthalmology, which is highly dependent on fundus images. However, training a robust and highly accurate model requires a large number of labeled high-quality images, which is often difficult for a single institution to provide. A natural solution is to use data from multiple medical centers to train the model. But due to privacy protection, data heterogeneity, and other problems, these data cannot be shared directly. Based on a real case of ophthalmic disease diagnosis with multiple data sources, this paper proposes solutions to the heterogeneity of data amount and features between datasets, establishes a heterogeneous federated learning framework, and applies federated learning to the diagnosis of multiple types of ophthalmic diseases for the first time. The experimental results show that federated learning can greatly improve the diagnostic accuracy of clients with less data, and the accuracy of clients with more data can also be slightly improved. The ablation experiment has demonstrated the effectiveness of the proposed method for handling data heterogeneity. Our research shows that through heterogeneous federated learning, we can use multi-source, non-independent-identical data to build accurate disease diagnosis models, and make clients who cannot obtain effective models get a practical level disease diagnosis model.

Abstract:Federated learning (FL) is a decentralized method enabling hospitals to collaboratively learn a model without sharing private patient data for training. In FL, participant hospitals periodically exchange training results rather than training samples with a central server. However, having access to model parameters or gradients can expose private training data samples. To address this challenge, we adopt secure multiparty computation (SMC) to establish a privacy-preserving federated learning framework. In our proposed method, the hospitals are divided into clusters. After local training, each hospital splits its model weights among other hospitals in the same cluster such that no single hospital can retrieve other hospitals' weights on its own. Then, all hospitals sum up the received weights, sending the results to the central server. Finally, the central server aggregates the results, retrieving the average of models' weights and updating the model without having access to individual hospitals' weights. We conduct experiments on a publicly available repository, The Cancer Genome Atlas (TCGA). We compare the performance of the proposed framework with differential privacy and federated averaging as the baseline. The results reveal that compared to differential privacy, our framework can achieve higher accuracy with no privacy leakage risk at a cost of higher communication overhead.

Abstract:Abstract Several studies underscore the potential of deep learning in identifying complex patterns, leading to diagnostic and prognostic biomarkers. Identifying sufficiently large and diverse datasets, required for training, is a significant challenge in medicine and can rarely be found in individual institutions. Multi-institutional collaborations based on centrally-shared patient data face privacy and ownership challenges. Federated learning is a novel paradigm for data-private multi-institutional collaborations, where model-learning leverages all available data without sharing data between institutions, by distributing the model-training to the data-owners and aggregating their results. We show that federated learning among 10 institutions results in models reaching 99% of the model quality achieved with centralized data, and evaluate generalizability on data from institutions outside the federation. We further investigate the effects of data distribution across collaborating institutions on model quality and learning patterns, indicating that increased access to data through data private multi-institutional collaborations can benefit model quality more than the errors introduced by the collaborative method. Finally, we compare with other collaborative-learning approaches demonstrating the superiority of federated learning, and discuss practical implementation considerations. Clinical adoption of federated learning is expected to lead to models trained on datasets of unprecedented size, hence have a catalytic impact towards precision/personalized medicine.

Abstract:Federated learning(FL) is a new kind of Artificial Intelligence(AI) aimed at data privacy preservation that builds on decentralizing the training data for the deep learning model. This new technique of data security and privacy sheds light on many critical domains with highly sensitive data, including medical image analysis. Developing a strong, scalable, and precise deep learning model has proven to count on a variety of high-quality data from different centers. However, data holders may not willing to share their data considering the restriction of privacy. In this paper, we approach this challenge with a federated learning paradigm. Specifically, we present a case study on the whole slide image classification problem. At each local client center, a multiple-instance learning classifier is developed to conduct whole slide image classification. We introduce a privacy-preserving federated learning framework based on hyper-network to update the global model. Hyper-network is deployed at the global center that produces the weights of the local network conditioned on its input. In this way, hyper-networks can simultaneously learn a family of the local client networks. Instead of communicating raw data with the local client, only model parameters injected with noise are transferred between the local client and the global model. By using a large scale of whole slide images with only slide-level labels, we mensurated our way on two different whole slide image classification problems. The results demonstrate that our proposed federated learning model based on hyper-network can effectively leverage multi-center data to develop a more accurate model which can be used to classify a whole slide image. Its improvements in terms of over the isolated local centers and the commonly used federated averaging baseline are significant. Code will be available.

Abstract:Deep-learning methods for computational pathology require either manual annotation of gigapixel whole-slide images (WSIs) or large datasets of WSIs with slide-level labels and typically suffer from poor domain adaptation and interpretability. Here we report an interpretable weakly supervised deep-learning method for data-efficient WSI processing and learning that only requires slide-level labels. The method, which we named clustering-constrained-attention multiple-instance learning (CLAM), uses attention-based learning to identify subregions of high diagnostic value to accurately classify whole slides and instance-level clustering over the identified representative regions to constrain and refine the feature space. By applying CLAM to the subtyping of renal cell carcinoma and non-small-cell lung cancer as well as the detection of lymph node metastasis, we show that it can be used to localize well-known morphological features on WSIs without the need for spatial labels, that it overperforms standard weakly supervised classification algorithms and that it is adaptable to independent test cohorts, smartphone microscopy and varying tissue content.

FedDBL: Communication and Data Efficient Federated Deep-Broad Learning for Histopathological Tissue Classification

Abstract:Histopathological tissue classification is a fundamental task in computational pathology. Deep learning-based models have achieved superior performance but centralized training with data centralization suffers from the privacy leakage problem. Federated learning (FL) can safeguard privacy by keeping training samples locally, but existing FL-based frameworks require a large number of well-annotated training samples and numerous rounds of communication which hinder their practicability in the real-world clinical scenario. In this paper, we propose a universal and lightweight federated learning framework, named Federated Deep-Broad Learning (FedDBL), to achieve superior classification performance with limited training samples and only one-round communication. By simply associating a pre-trained deep learning feature extractor, a fast and lightweight broad learning inference system and a classical federated aggregation approach, FedDBL can dramatically reduce data dependency and improve communication efficiency. Five-fold cross-validation demonstrates that FedDBL greatly outperforms the competitors with only one-round communication and limited training samples, while it even achieves comparable performance with the ones under multiple-round communications. Furthermore, due to the lightweight design and one-round communication, FedDBL reduces the communication burden from 4.6GB to only 276.5KB per client using the ResNet-50 backbone at 50-round training. Since no data or deep model sharing across different clients, the privacy issue is well-solved and the model security is guaranteed with no model inversion attack risk. Abstract:The development of decision support systems for pathology and their deployment in clinical practice have been hindered by the need for large manually annotated datasets. To overcome this problem, we present a multiple instance learning-based deep learning system that uses only the reported diagnoses as labels for training, thereby avoiding expensive and time-consuming pixel-wise manual annotations. We evaluated this framework at scale on a dataset of 44,732 whole slide images from 15,187 patients without any form of data curation. Tests on prostate cancer, basal cell carcinoma and breast cancer metastases to axillary lymph nodes resulted in areas under the curve above 0.98 for all cancer types. Its clinical application would allow pathologists to exclude 65–75% of slides while retaining 100% sensitivity. Our results show that this system has the ability to train accurate classification models at unprecedented scale, laying the foundation for the deployment of computational decision support systems in clinical practice.

Abstract:Healthcare is one of the foremost applications of machine learning (ML). Traditionally, ML models are trained by central servers, which aggregate data from various distributed devices to forecast the results for newly generated data. This is a major concern as models can access sensitive user information, which raises privacy concerns. A federated learning (FL) approach can help address this issue: A global model sends its copy to all clients who train these copies, and the clients send the updates (weights) back to it. Over time, the global model improves and becomes more accurate. Data privacy is protected during training, as it is conducted locally on the clients' devices. However, the global model is susceptible to data poisoning. We develop a privacy-preserving FL technique for a skin cancer dataset and show that the model is prone to data poisoning attacks. Ten clients train the model, but one of them intentionally introduces flipped labels as an attack. This reduces the accuracy of the global model. As the percentage of label flipping increases, there is a noticeable decrease in accuracy. We use a stochastic gradient descent optimization algorithm to find the most optimal accuracy for the model. Although FL can protect user privacy for healthcare diagnostics, it is also vulnerable to data poisoning, which must be addressed.

Abstract:Computer-aided diagnosis (CAD) can help pathologists improve diagnostic accuracy together with consistency and repeatability for cancers. However, the CAD models trained with the histopathological images only from a single center (hospital) generally suffer from the generalization problem due to the straining inconsistencies among different centers. In this work, we propose a pseudo-data based self-supervised federated learning (FL) framework, named SSL-FT-BT, to improve both the diagnostic accuracy and generalization of CAD models. Specifically, the pseudo histopathological images are generated from each center, which contain both inherent and specific properties corresponding to the real images in this center, but do not include the privacy information. These pseudo images are then shared in the central server for self-supervised learning (SSL) to pre-train the backbone of global mode. A multi-task SSL is then designed to effectively learn both the center-specific information and common inherent representation according to the data characteristics. Moreover, a novel Barlow Twins based FL (FL-BT) algorithm is proposed to improve the local training for the CAD models in each center by conducting model contrastive learning, which benefits the optimization of the global model in the FL procedure. The experimental results on four public histopathological image datasets indicate the effectiveness of the proposed SSL-FL-BT on both diagnostic accuracy and generalization.

Federated attention consistent learning models for prostate cancer diagnosis and Gleason grading

Abstract:Artificial intelligence (AI) holds significant promise in transforming medical imaging, enhancing diagnostics, and refining treatment strategies. However, the reliance on extensive multicenter datasets for training AI models poses challenges due to privacy concerns. Federated learning provides a solution by facilitating collaborative model training across multiple centers without sharing raw data. This study introduces a federated attention-consistent learning (FACL) framework to address challenges associated with large-scale pathological images and data heterogeneity. FACL enhances model generalization by maximizing attention consistency between local clients and the server model. To ensure privacy and validate robustness, we incorporated differential privacy by introducing noise during parameter transfer. We assessed the effectiveness of FACL in cancer diagnosis and Gleason grading tasks using 19,461 whole-slide images of prostate cancer from multiple centers. In the diagnosis task, FACL achieved an area under the curve (AUC) of 0.9718, outperforming seven centers with an average AUC of 0.9499 when categories are relatively balanced. For the Gleason grading task, FACL attained a Kappa score of 0.8463, surpassing the average Kappa score of 0.7379 from six centers. In conclusion, FACL offers a robust, accurate, and cost-effective AI training model for prostate cancer pathology while maintaining effective data safeguards.

Continual Domain Incremental Learning for Privacy-aware Digital Pathology

Abstract:In recent years, there has been remarkable progress in the field of digital pathology, driven by the ability to model complex tissue patterns using advanced deep-learning algorithms. However, the robustness of these models is often severely compromised in the presence of data shifts (e.g., different stains, organs, centers, etc.). Alternatively, continual learning (CL) techniques aim to reduce the forgetting of past data when learning new data with distributional shift conditions. Specifically, rehearsal-based CL techniques, which store some past data in a buffer and then replay it with new data, have proven effective in medical image analysis tasks. However, privacy concerns arise as these approaches store past data, prompting the development of our novel Generative Latent Replay-based CL (GLRCL) approach. GLRCL captures the previous distribution through Gaussian Mixture Models instead of storing past samples, which are then utilized to generate features and perform latent replay with new data. We systematically evaluate our proposed framework under different shift conditions in histopathology data, including stain and organ shift. Our approach significantly outperforms popular buffer-free CL approaches and performs similarly to rehearsal-based CL approaches that require large buffers causing serious privacy violations.

The Whole Pathological Slide Classification via Weakly Supervised Learning

Abstract:Due to its superior efficiency in utilizing annotations and addressing gigapixel-sized images, multiple instance learning (MIL) has shown great promise as a framework for whole slide image (WSI) classification in digital pathology diagnosis. However, existing methods tend to focus on advanced aggregators with different structures, often overlooking the intrinsic features of H\&E pathological slides. To address this limitation, we introduced two pathological priors: nuclear heterogeneity of diseased cells and spatial correlation of pathological tiles. Leveraging the former, we proposed a data augmentation method that utilizes stain separation during extractor training via a contrastive learning strategy to obtain instance-level representations. We then described the spatial relationships between the tiles using an adjacency matrix. By integrating these two views, we designed a multi-instance framework for analyzing H\&E-stained tissue images based on pathological inductive bias, encompassing feature extraction, filtering, and aggregation. Extensive experiments on the Camelyon16 breast dataset and TCGA-NSCLC Lung dataset demonstrate that our proposed framework can effectively handle tasks related to cancer detection and differentiation of subtypes, outperforming state-of-the-art medical image classification methods based on MIL. A Generalized Deep Learning Framework for Whole-Slide Image Segmentation and Analysis

Abstract:Histopathology tissue analysis is considered the gold standard in cancer diagnosis and prognosis. Given the large size of these images and the increase in the number of potential cancer cases, an automated solution as an aid to histopathologists is highly desirable. In the recent past, deep learning-based techniques have provided state of the art results in a wide variety of image analysis tasks, including analysis of digitized slides. However, the size of images and variability in histopathology tasks makes it a challenge to develop an integrated framework for histopathology image analysis. We propose a deep learning-based framework for histopathology tissue analysis. We demonstrate the generalizability of our framework, including training and inference, on several open-source datasets, which include CAMELYON (breast cancer metastases), DigestPath (colon cancer), and PAIP (liver cancer) datasets. We discuss multiple types of uncertainties pertaining to data and model, namely aleatoric and epistemic, respectively. Simultaneously, we demonstrate our model generalization across different data distribution by evaluating some samples on TCGA data. On CAMELYON16 test data (n=139) for the task of lesion detection, the FROC score achieved was 0.86 and in the CAMELYON17 test-data (n=500) for the task of pN-staging the Cohen's kappa score achieved was 0.9090 (third in the open leaderboard). On DigestPath test data (n=212) for the task of tumor segmentation, a Dice score of 0.782 was achieved (fourth in the challenge). On PAIP test data (n=40) for the task of viable tumor segmentation, a Jaccard Index of 0.75 (third in the challenge) was achieved, and for viable tumor burden, a score of 0.633 was achieved (second in the challenge). Slideflow: Deep Learning for Digital Histopathology with Real-Time Whole-Slide Visualization

Abstract:Deep learning methods have emerged as powerful tools for analyzing histopathological images, but current methods are often specialized for specific domains and software environments, and few open-source options exist for deploying models in an interactive interface. Experimenting with different deep learning approaches typically requires switching software libraries and reprocessing data, reducing the feasibility and practicality of experimenting with new architectures. We developed a flexible deep learning library for histopathology called Slideflow, a package which supports a broad array of deep learning methods for digital pathology and includes a fast whole-slide interface for deploying trained models. Slideflow includes unique tools for whole-slide image data processing, efficient stain normalization and augmentation, weakly-supervised whole-slide classification, uncertainty quantification, feature generation, feature space analysis, and explainability. Whole-slide image processing is highly optimized, enabling whole-slide tile extraction at 40X magnification in 2.5 seconds per slide. The framework-agnostic data processing pipeline enables rapid experimentation with new methods built with either Tensorflow or PyTorch, and the graphical user interface supports real-time visualization of slides, predictions, heatmaps, and feature space characteristics on a variety of hardware devices, including ARM-based devices such as the Raspberry Pi.

Abstract:Introduction: Because tiotropium has demonstrated clinical benefits as a long-term maintenance treatment for chronic obstructive pulmonary disease (COPD), the number of patent applications of new chemical entities with antimuscarinic activity has significantly increased, along with the number of compounds that have reached clinical development. Areas covered: This review summarizes the current status of long-acting muscarinic antagonists (LAMA) in clinical development for COPD, and the associated patent literature since 2006, with a focus on new chemical entities. Expert opinion: In recent years, companies have taken different approaches to obtain compounds with high potency, long duration of action and minimal systemic exposure. Several strategies for minimizing adverse effects due to systemic exposure have been identified (quaternization, higher rate of plasma hydrolysis and degradation, increased plasma protein binding). Abstract:Artificial Intelligence (AI) has great potential to improve health outcomes by training systems on vast digitized clinical datasets. Computational Pathology, with its massive amounts of microscopy image data and impact on diagnostics and biomarkers, is at the forefront of this development. Gigapixel pathology slides pose a unique challenge due to their enormous size and are usually divided into tens of thousands of smaller tiles for analysis. This results in a discontinuity in the machine learning process by separating the training of tile-level encoders from slide-level aggregators and the need to adopt weakly supervised learning strategies. Training models from entire pathology slides end-to-end has been largely unexplored due to its computational challenges. To overcome this problem, we propose a novel approach to jointly train both a tile encoder and a slide-aggregator fully in memory and end-to-end at high-resolution, bridging the gap between input and slide-level supervision. While more computationally expensive, detailed quantitative validation shows promise for large-scale pre-training and fine-tuning of pathology foundation models.
