Abstract:Background: This study presents the legal obligations set for the main stakeholders in the establishment and implementation of electronic health (e-health) records in Iran and investigates the fulfillment of these obligations. Methods: This qualitative study was conducted using document analysis. The research population comprised all the documents, laws, reports, and policies of the establishment and utilization of electronic health records in Iran. Sampling was purposive, and all the internal documents of Iran were included until the end of May 2022. A data extraction form was used to collect the data, and Scott's criterion was used to select the documents. Descriptive analysis was used to analyze the data. Results: Based on the documents, 56% of the sentences were implemented, and 22% were not enforced according to the assigned tasks. The status of the execution of tasks by the main players was also extracted. The most important of these included the drafting of the electronic health security and privacy regulations, not providing the conditions for receiving electronic copies in the e-records, creating internal automation for the Ministry of Health and Medical Education instead of the e-health records system, instability of the integrated portal for exchanging health information and disconnection of the majority of service centers, eliminating the extension of the validity of paper booklets, operationalizing the online entitlement system, using the first phase of the electronic prescription project, not inserting nursing notes in the records, recording financial and administrative information instead of clinical ones, refusal of numerous doctors to write electronic prescriptions, using an alternative method of two-step verification instead of electronic signature, the progress of coding and drug authenticity, and the non-implementation of medical equipment coding. Conclusions: Reviewing the status of the enforcement of approvals also revealed that the format of actions since 2018 has ended with a positive result. The main achievement so far has been electronic prescription, while it should be noted that prescription is only part of the electronic health record project.

Abstract:Background: Referral system is among the key elements of primary health care that leads to enhanced efficiency, reduced costs, reduced waiting time, and patients' enhanced access to more specialized services. The present study was aimed at analysing the policies of the electronic referral (e-Referral) system in Iran. Methods: This qualitative study was conducted based on Walt and Gilson's policy triangle and Kingdon's models. Data were collected through document analysis and 51 semi-structured interviews with managers at various levels, experts, family physicians, specialist physicians, and patients. Document analysis was performed by content analysis method, and interview analysis was performed through framework analysis method in Atlas.ti 8. Results: The e-Referral system was launched with the aim of equitable access to services and to benefit from better management of health resources. Valid scientific evidences were used to formulate policies. Numerous meetings were held with domestic and foreign stakeholders at the provincial, city, and rural levels. The implementation of the programme followed a bottom-up approach, and the main obstacles to the implementation of the programme included the change of senior managers of the health system and their not being fully aware of the importance of the programme, inadequate allocation of financial resources, and unwillingness of some patients to follow the referral system. Conclusions: The policy triangle framework, while explaining the components of the e-Referral system programme, revealed the obstacles to the proper implementation of the programme. In order to ensure that the programme goes on continuously and successfully, it is essential to create the necessary determination and commitment on the part of the minister of health and medical education and senior managers of the health system, earmark resources for the programme, improve resource allocation with insurance management, reform the payment system, plan to raise public awareness, and attract community participation.

Abstract:Background and aims: Every individual is entitled to the right to health, and public health policies are the direct result of this principle. Both from a humanism perspective and due to the inherent requirements of a universal public health system, maintaining health has priority over treatment. Accordingly, any system aiming to promote public health must adjust its decisions, policies, and activities so as to continuously and progressively improve the quality of health services. The present research analyzes the role of Public health policies and intellectual trends in developing Iran's health system. The results can be used to outline the future of primary healthcare at different levels of health system. Methods: Data were gathered through a review of the literature as well as relevant documents and round table discussions. Data was analyzed using content analysis. Results: Clarification of health policies and distinguishing between key and ideal choices allows for more rigorous pursuit of the fundamental goals of the health system. Adherence to the guiding principles of a comprehensive health system, implementation of an all-inclusive network of health services, issues related to training human resources, the role of the government and the Ministry of Health in providing health services, and the basic needs of the health system are the most important factors that require more attention for further development of healthcare at different levels of Iran's health system. Conclusion: By analyzing the experiences of Iran's health system and by understanding the status quo, the strength and weakness of the system can be identified and a systematic approach can be taken to develop and implement new and better policies. Health issues are interrelated and are even affected by social conditions. Increasing the number of physicians, having extra funds, and building new hospitals with more beds are not as effective as promoting the concept of prevention in solving the problems that people faces today. The health system must be viewed as a whole made up of various intertwined parts. Its goals must be clarified, its requirements, resources, and limitations must be specified, and a systematic approach must be taken to solve the problems of the health system by taking into account all the contributing factors.

Abstract:Background and aims: Evidence-based policy documents that are well developed by senior civil servants and are timely available can reduce the barriers to evidence utilization by health policy makers. This study examined the barriers and facilitators in developing evidence-based health policy documents from the perspective of their producers in a developing country. Methods: In a qualitative study with a framework analysis approach, we conducted semi-structured interviews using purposive and snowball sampling. A qualitative analysis software (MAXQDA-10) was used to apply the codes and manage the data. This study was theory-based and the results were compared to exploratory studies about the factors influencing evidence-based health policymaking. Results: 18 codes and three main themes of behavioral, normative, and control beliefs were identified. Factors that influence the development of evidence-based policy documents were identified by the participants: behavioral beliefs included quality of policy documents, use of resources, knowledge and innovation, being time-consuming and contextualization; normative beliefs included policy authorities, policymakers, policy administrators, and co-workers; and control beliefs included recruitment policy, performance management, empowerment, management stability, physical environment, access to evidence, policy making process, and effect of other factors. Conclusion: Most of the cited barriers to the development of evidence-based policy were related to control beliefs, i.e. barriers at the organizational and health system levels. This study identified the factors that influence the development of evidence-based policy documents based on the components of the theory of planned behavior.But in exploratory studies on evidence utilization by health policymakers, the identified factors were only related to control behaviors. This suggests that the theoretical approach may be preferable to the exploratory approach in identifying the barriers and facilitators of a behavior.

Abstract:While many advances have been made in surgical robotics technology, even for surgeons with relatively high levels of robotic surgery experience, many tasks take less time to perform manually. Although there are other benefits to surgical robotics that may outweigh task completion time, relatively lower efficiency will hinder the adoption of this technology. This study focused on two interface parameters: Control-Display Gain (CDG, i.e., the amount of robot movement resulting from a given robot controller movement) and the optical Zoom level that defines the working field of view. Results from a study with 10 participants suggest that CDG is a promising interface parameter for optimizing movement time in robot-assisted surgical tasks. The results have implications for the development and implementation of intelligent surgeon-robot interface technology and hold the potential to greatly improve the efficiency of robotic-assisted surgery techniques.

Abstract:Background Electronic prescriptions represent a fundamental shift in service delivery, healthcare management, and associated costs, offering numerous advantages. However, akin to other electronic systems, they also present challenges. This study aimed to investigate patients’ understanding of the challenges associated with electronic prescriptions in Iran. Methods This study used a qualitative research design, utilizing individual and semi-structured interviews with patients referred to selected pharmacies across all 11 districts of Shiraz City. The data were analyzed using MAXQDA software (version 10), and descriptive statistics for demographic data were calculated using SPSS version 19. Results The study revealed that the participants generally demonstrated a certain level of familiarity with electronic prescribing systems. However, it was evident that many were unaware of the potential implications of such technology for their relationships with healthcare providers. This underscores the urgent need for patient understanding in the context of the electronic prescription system. While patients were relatively familiar with the functionality of electronic prescribing systems, they lacked a comprehensive understanding of how using these systems could affect their interactions with healthcare providers. Conclusion Patients are significant beneficiaries of the electronic prescribing system. By addressing their needs and concerns, they can develop a positive attitude toward this system. Their active engagement can pave the way for the system’s ease of use, increase its acceptance, and ultimately enhance the quality of healthcare services.

Intermediates in the melting transitions of aluminum nanoclusters.

Abstract:The author uses heat capacity data for aluminum cluster ions, Aln+, obtained in the laboratory of Breaux et al. [Phys. Rev. Lett. 94, 17340 (2005)] to determine whether or not intermediate species are present in the transition from the solidlike form of the clusters present at low temperatures to the liquidlike form present at high temperatures. He gives a general method on how to test for the presence of such intermediates and how to calculate their probabilities and thermodynamics as a function of temperature. In addition he uses energy distribution functions, using the maximum-entropy method that he developed previously, to substantiate the presence or absence of intermediates. As examples of the method he treats n=53 and n=79 clusters both of which exhibit marked maxima in the temperature dependence of their heat capacity curves, indicating strong order-disorder transitions. He find that in the melting transition n=53 clusters have no intermediates while the melting of n=79 clusters is dominated by intermediate species.

Abstract:Introduction: The COVID-19 pandemic has had a profound impact on Iran and numerous other nations, resulting in a surge in infections and fatalities. In response to the COVID-19 crisis, a range of policies and initiatives were enacted, including the deployment of telehealth services. This study aims to outline the requirements for establishing an all-encompassing platform capable of delivering telemedicine services in Iran.Material and Methods: This cross-sectional study was carried out using a researcher-made electronic questionnaire during the period of July to August 2022. All experts in the field of medical informatics and health information systems based in three provinces (Kermanshah, Kerman, and West Azerbaijan) were contacted to fill out the questionnaire, 15 participants completed and returned the questionnaire. Data were analyzed by SPSS using descriptive statistics.Results: The requirements for the design and implementation of the systems could be divided into internal (technical and infrastructural, security-legal, and environmental), and external categories (technical and infrastructural, and security-legal). The majority of internal and external requirements were related to technical and infrastructure aspects, accounting for 83% and 95%, respectively.Conclusion: Telemedicine development tools are available to enhance healthcare services in Iran, but there is a need to strengthen the infrastructure and technical equipment to enable the utilization of this technology for improving therapeutic and educational objectives within the healthcare system.

Abstract:Abstract Background Equity, efficiency, sustainability, acceptability to clients and providers, and quality are the cornerstones of universal health coverage (UHC). No country has a single way to achieve efficient UHC. In this study, we documented the Iranian health insurance reforms, focusing on how and why certain policies were introduced and implemented, and which challenges remain to keep a sustainable UHC. Methods This retrospective policy analysis used three sources of data: a comprehensive and chronological scoping review of literature, interviews with Iran health insurance policy actors and stakeholders, and a review of published and unpublished official documents and local media. All data were analysed using thematic content analysis. Results Health insurance reforms, especially health transformation plan (HTP) in 2014, helped to progress towards UHC and health equity by expanding population coverage, a benefits package, and enhancing financial protection. However, several challenges can jeopardize sustaining this progress. There is a lack of suitable mechanisms to collect contributions from those without a regular income. The compulsory health insurance coverage law is not implemented in full. A substantial gap between private and public medical tariffs leads to high out-of-pocket health expenditure. Moreover, controlling the total health care expenditures is not the main priority to make keeping UHC more sustainable. Conclusion To achieve UHC in Iran, the Ministry of Health and Medical Education and health insurance schemes should devise and follow the policies to control health care expenditures. Working mechanisms should be implemented to extend free health insurance coverage for those in need. More studies are needed to evaluate the impact of health insurance reforms in terms of health equity, sustainability, coverage, and access.
Health Information Management System for Elderly Health Sector: A Qualitative Study in Iran

Abstract:BACKGROUND: There are increasing change and development of information in healthcare systems. Given the increase in aging population, managers are in need of true and timely information when making decision.OBJECTIVES: The aim of this study was to investigate the current status of the health information management system for the elderly health sector in Iran.MATERIALS AND METHODS: This qualitative study was conducted in two steps. In the first step, required documents for administrative managers were collected using the data gathering form and observed and reviewed by the researcher. In the second step, using an interview guide, the required information was gathered through interviewing experts and faculty members. The convenience, purposeful and snowball sampling methods were applied to select interviewees and the sampling continued until reaching the data saturation point. Finally, notes and interviews were transcribed and content analysis was used to analyze them.RESULTS: The results of the study showed that there was a health information management system for the elderly health sector in Iran. However, in all primary health care centers the documentation of data was done manually; the data flow was not automated; and the analysis and reporting of data are also manually. Eventually, decision makers are provided with delayed information.CONCLUSIONS: It is suggested that the steward of health in Iran, the ministry of health, develops an appropriate infrastructure and finally puts a high priority on the implementation of the health information management system for elderly health sector in Iran.

Abstract:Abstract Background This research analyzed the Sixth Five-Year Economic, Social, and Cultural Development Plan of the Islamic Republic of Iran (6NPD) to shed light on how the plan addresses the Universal Health Coverage (UHC). Methods This research was a qualitative study. We systematically analyzed ‘Secs. 14 -Health, Insurance, Health & Women, and Family’ in the 6NPD. Through a content analysis, we converted this section into meaning units and coded them. Coding was guided through the conceptual framework ‘Six Building Blocks of Health System’ and the key principles of UHC. Results Six themes and twenty-one subthemes were identified. The subthemes of financing include a fair and secured process of resource pooling, payment methods, revenue generation for the health sector, and a definition of a basic benefits package. The subthemes of governance and leadership consist of social insurance policies’ integration, compliance of providers, a designation of the Ministry of Health and Medical Education (MoHME) as the regulator and the steward of health resources, a payer-provider split, and stakeholders’ participation. The subthemes of health workforce emphasizes balancing the quality and quantity of the health workforce with populations’ health needs and the health system’s requirements. The subthemes of health information system s consist of the electronic health records for Iranians, information systems for organization and delivery functions, and information systems for the financing function. The subthemes of the organization and delivery consider improving effectiveness and efficiency of healthcare delivery, strengthening the family physician program and referral system, and extending the pre-hospital emergency system. Lastly, access to medicine focuses on the design and implementation of an essential drug list and drug systems for approving the coverage and provision of generic medicine. Conclusions The 6NPD introduced policies for strengthening the 6 building blocks of the health system. It introduced policies to improve financing particularly resource pooling and the sustainability of financial resources. As mandated by 6NPD, centering the health system’s governance/leadership in MoHME may exacerbate the existing conflict of interests and provoke various arguments, which impede the enforcement of rules and regulation. The 6NPD is a step forward in terms of improving financial protection, yet several other policies need to be made to adequately meet the requirement of UHC regarding equity and effective coverage.

Abstract:Background: As the strategies proposed for oral health improvement in developed countries are not adapted for developing ones, this study aimed to identify the challenges of oral health policy implementation in Iran as a low-income developing country. Methods: This qualitative study was conducted in 2019 in Iran as a middle-eastern developing country. The study population consisted of experts who had experience in oral health and were willing to participate in the study. Snowball sampling was used to select 12 participants for semi-structured interviews and saturation was achieved after 16 interviews. Guba and Lincoln criteria including credibility, transferability, confirmability and dependability were used to determine reliability and transparency, and finally a five-step framework analysis method was used to analyze the data. Results: The analysis of the interviews resulted in identification of 7 main themes that were categorized into 5 problems of policy implementation as proposed by the Matus framework. The main themes of executive and preventive challenges to implement oral health policies were categorized as organizational problems, the main themes of educational and resource challenges were situated as material problems, and the main themes of insurance, policy making and trusteeship challenges were considered as legal, policymaking and perspective. Conclusion: The implementation of oral health policies has faced some challenges. It seems that the national coverage of oral health and integration of these services in prevention and serious attention to the private sector can be considered as the most important strategies for achieving improved oral health in Iran.
The Potential Use of Digital Health in Iran: A Systematic Mapping Review

Abstract:Introduction: Digital health technologies are transforming healthcare delivery globally. The purpose of the current study was to identify and map the current status of digital health applications in Iran through providing graphical/tabular classifications on the studies conducted in this field.Material and Methods: Following PRISMA guidelines, relevant English-language papers on digital health in Iran published from 2012 until 2023 in online scientific databases, including PubMed, Scopus, Web of Science and IEEE Xplore were screened. A total of 97 papers were selected for data extraction data including digital heath technologies, medical fields, application areas and users.Results: The number of publications has grown considerably since 2016. The most common digital health technologies were artificial intelligence including machine learning (34%), mobile health (25%) and telehealth (16%). These were mostly applied in infections (16%), nutrition/metabolism disorders (13%), mental health (20%) and cancers (12%). The key application areas were education (21%), therapy (16%) and diagnosis (15%). The primary users were patients (45%) and healthcare professionals (42%).Conclusion: Digital health studies in Iran are continuously evolving. The activities are focused on a few technologies like artificial intelligence, with applications in diverse medical subfields for objectives like education and diagnosis. These results help identify research gaps and future directions for advancing digital health in Iran.

Abstract:Background and aims: This study aimed to evaluate the basis in which the decisions about pivotal matters in Iranian health system are made and how these decisions are in accordance with the best available evidence. Methods: To conduct this qualitative study, in the first step, four main issues with a high degree of importance, novelty and significant impact on health system were selected as follows: outsourcing clinical and Para-clinical services and health sectors to the private sector, global payment system to the hospitals and listing or delisting of pharmaceuticals from insurance coverage. In the next step, along with reviewing reports, documents and papers, semi-structured interviews were conducted with policymakers, experts and officials involved in the formulation and implementation of the selected issues. In the following step, a comprehensive search was carried out to find the research studies on the mentioned topics (fundamental evidence of policy-making). In the final stage, the results of the interviews and documentary analysis were compared with the best acquired evidence from the research studies. As a result, analytical model about decision making and interpretation method of evidence informed policy making in the health system were extracted. Results: In Iranian health system, evidence production is limited and the produced evidence is moderately applied in decision-making. Most decisions are made subconsciously, intuitively, fast, with low cognitive load and based on experiences. In recent years, due to the increasing number of researchers, awareness about modern methods of research and using evidence in decision-making is increasing gradually in the health system. Conclusion: Developing the importance and role of using evidence in making better decisions and policies for health managers and policy-makers and localizing global evidence are proper ways to enhance the use of evidence-informed decision-making approach in Iranian health system.

Abstract:In summary, then, we can learn three useful things from small clinical trials. First, we can learn when to challenge conventional but untested therapeutic wisdom. Second, because the patient number in any trial is the number of events, rather than the number of study patients, some small trials are so definitively positive that they are sufficient to identify the best therapy. And third, small trials, even when individually inconclusive, can serve as the basis for convincingly conclusive overviews and meta-analyses that carry, and deserve, greater credibility than a single large trial of similar size to their sum.

This study aimed to prepare a framework for new financial management system in Iran health sector.MATERIALS AND METHODS: This qualitative study was conducted by content analysis approach and 15 key informant participants selected through purposive sampling consisted of three minsters of Ministry of Health and Medical Education (MOHME), three vice-chancellors in the Center of Resource Development and Management, Two members of the Health Assembly of the Islamic Consultative, four Medical Sciences university's presidents, and three managers in Budgeting and performance Monitoring Center of MOHME in 2017. Data were collected through semi-structured interviews and they were analyzed using Atlas T6 software.RESULTS: Six main themes were emerged as follow: "legal reform," "removing barriers to set up accrual accounting," "cost price calculation," "operational planning and budgeting," "human resources' organization, recruitment, and moderation," and "financial system output utilization (management accounting techniques) as the base for evidence-based policymaking and decision-making practices."CONCLUSION: Any efforts for improving the current situation and reducing of mentioned limitations could be useful in providing required space for future phases of reforms and calculation of unit cost, operational budgeting, and management of cost and productivity. This can be achieved through an integrated system of recording and producing standard and accrual financial information. Furthermore, changing the accounting process and the financial system that complies with one single encoding in the country is a key issue.

Abstract:Background: This study aims to analyze the public-private partnership (PPP) policy in primary health care (PHC), focusing on the experience of the East Azerbaijan Province (EAP) of Iran. Methods: This research is a qualitative study. Data were gathered using interviews with stakeholders and document analysis and analyzed through content analysis. Results: Participants considered political and economic support as the most important underlying factors. Improving system efficiency was the main goal of this policy. Most stakeholders were supporters of the plan, and there was no major opponent. Implementing the health evolution plan (HEP) was an opportunity to design this policy. Participants considered the lack of provision of infrastructure as the main weakness, changing the role of the public sector as the main strength, and promoting social justice as the main achievement of policy. The results of the quantitative data review showed that following the implementation of this policy, health indicators have been improved. Conclusions: Based on the results of this study, the PPP model in EAP is a new and successful experience in PHC in Iran. Supporting and developing this policy may improve the quality and quantity of providing care.
Exploring Barriers to the Development of Home Health Care in Iran: A Qualitative Study

Abstract:Changes in society’s needs have increased individuals’ desire to be independent and receive health care at home. Home health care (HHC) is one of the main components of realizing these ambitions. However, there is a serious concern about the state of development of the HHC industry in Iran and the quality of its services. This study aims to explore the barriers to the development of HHC in Iran. This qualitative study with a conventional content analysis approach was conducted in Tabriz, Iran, from October 2019 to March 2020. Twenty-one key stakeholders including nurses, home health directors, physicians, policy-makers, patients, and their families participated in the study. Participants were selected using purposive sampling. Data were collected using a focus group discussion (FGD) and 18 semi-structured in-depth interviews and analyzed using Graneheim and Lundman’s techniques. Data collection continued until saturation was reached. Three main themes emerged from the analysis: quality management challenges, insufficient infrastructure and policy, and political issues. The findings show that applying standard strategies to monitor the quality of services, paying attention to infrastructure, and having a clear framework for policy-making are beneficial for the development of the HHC industry. Health managers can monitor and upgrade the quality of HHC services by defining standards and assessment indicators, as well as providing comprehensive data. It is suggested that legislators consider the principles of community-based accountability, justice, and accessibility their top priority. Moreover, policymakers need to develop a roadmap to reform policies to create the conditions for the development of this industry.
