Abstract:While the usage of digital ocular fundus image has been widespread in ophthalmology practice, the interpretation of the image has been still on the hands of the ophthalmologists which are quite costly. We explored a robust deep learning system that detects three major ocular diseases: diabetic retinopathy (DR), glaucoma (GLC), and age-related macular degeneration (AMD). The proposed method is composed of two steps. First, an initial quality evaluation in the classification system is proposed to filter out poor-quality images to enhance its performance, a technique that has not been explored previously. Second, the transfer learning technique is used with various convolutional neural networks (CNN) models that automatically learn a thousand features in the digital retinal image, and are based on those features for diagnosing eye diseases. Comparison performance of many models is conducted to find the optimal model which fits with fundus classification. Among the different CNN models, DenseNet-201 outperforms others with an area under the receiver operating characteristic curve of 0.99. Furthermore, the corresponding specificities for healthy, DR, GLC, and AMD patients are found to be 89.52%, 96.69%, 89.58%, and 100%, respectively. These results demonstrate that the proposed method can reduce the time-consumption by automatically diagnosing multiple eye diseases using computer-aided assistance tools.
Deep Learning for the Detection of Multiple Fundus Diseases Using Ultra-widefield Images

Abstract:Introduction: To design and evaluate a deep learning model based on ultra-widefield images (UWFIs) that can detect several common fundus diseases. Methods: Based on 4574 UWFIs, a deep learning model was trained and validated that can identify normal fundus and eight common fundus diseases, namely referable diabetic retinopathy, retinal vein occlusion, pathologic myopia, retinal detachment, retinitis pigmentosa, age-related macular degeneration, vitreous opacity, and optic neuropathy. The model was tested on three test sets with data volumes of 465, 979, and 525. The performance of the three deep learning networks, EfficientNet-B7, DenseNet, and ResNet-101, was evaluated on the internal test set. Additionally, we compared the performance of the deep learning model with that of doctors in a tertiary referral hospital. Results: Compared to the other two deep learning models, EfficientNet-B7 achieved the best performance. The area under the receiver operating characteristic curves of the EfficientNet-B7 model on the internal test set, external test set A and external test set B were 0.9708 (0.8772, 0.9849) to 1.0000 (1.0000, 1.0000), 0.9683 (0.8829, 0.9770) to 1.0000 (0.9975, 1.0000), and 0.8919 (0.7150, 0.9055) to 0.9977 (0.9165, 1.0000), respectively. On a data set of 100 images, the total accuracy of the deep learning model was 93.00%, the average accuracy of three ophthalmologists who had been working for 2 years and three ophthalmologists who had been working in fundus imaging for more than 5 years was 88.00% and 94.00%, respectively. Conclusion: High performance was achieved on all three test sets using our UWFI multidisease classification model with a small sample size and fast model inference. The performance of the artificial intelligence model was comparable to that of a physician with 2-5 years of experience in fundus diseases at a tertiary referral hospital. The model is expected to be used as an effective aid for fundus disease screening.

Abstract:Deep learning and computer vision (CV) can easily recognize pictures that the human doctor would have to spend a lot of time and labor to process. Thus, they have a wide range of applications in the field of ophthalmology, which is highly dependent on fundus images. However, training a robust and highly accurate model requires a large number of labeled high-quality images, which is often difficult for a single institution to provide. A natural solution is to use data from multiple medical centers to train the model. But due to privacy protection, data heterogeneity, and other problems, these data cannot be shared directly. Based on a real case of ophthalmic disease diagnosis with multiple data sources, this paper proposes solutions to the heterogeneity of data amount and features between datasets, establishes a heterogeneous federated learning framework, and applies federated learning to the diagnosis of multiple types of ophthalmic diseases for the first time. The experimental results show that federated learning can greatly improve the diagnostic accuracy of clients with less data, and the accuracy of clients with more data can also be slightly improved. The ablation experiment has demonstrated the effectiveness of the proposed method for handling data heterogeneity. Our research shows that through heterogeneous federated learning, we can use multi-source, non-independent-identical data to build accurate disease diagnosis models, and make clients who cannot obtain effective models get a practical level disease diagnosis model.
Retinal Disease Diagnosis Using Deep Learning on Ultra-Wide-Field Fundus Images

Abstract:Ultra-wide-field fundus imaging (UFI) provides comprehensive visualization of crucial eye components, including the optic disk, fovea, and macula. This in-depth view facilitates doctors in accurately diagnosing diseases and recommending suitable treatments. This study investigated the application of various deep learning models for detecting eye diseases using UFI. We developed an automated system that processes and enhances a dataset of 4697 images. Our approach involves brightness and contrast enhancement, followed by applying feature extraction, data augmentation and image classification, integrated with convolutional neural networks. These networks utilize layer-wise feature extraction and transfer learning from pre-trained models to accurately represent and analyze medical images. Among the five evaluated models, including ResNet152, Vision Transformer, InceptionResNetV2, RegNet and ConVNext, ResNet152 is the most effective, achieving a testing area under the curve (AUC) score of 96.47% (with a 95% confidence interval (CI) of 0.931–0.974). Additionally, the paper presents visualizations of the model's predictions, including confidence scores and heatmaps that highlight the model's focal points—particularly where lesions due to damage are evident. By streamlining the diagnosis process and providing intricate prediction details without human intervention, our system serves as a pivotal tool for ophthalmologists. This research underscores the compatibility and potential of utilizing ultra-wide-field images in conjunction with deep learning.

Abstract:Abstract To demonstrate the identification of corneal diseases using a novel deep learning algorithm. A novel hierarchical deep learning network, which is composed of a family of multi-task multi-label learning classifiers representing different levels of eye diseases derived from a predefined hierarchical eye disease taxonomy was designed. Next, we proposed a multi-level eye disease-guided loss function to learn the fine-grained variability of eye diseases features. The proposed algorithm was trained end-to-end directly using 5,325 ocular surface images from a retrospective dataset. Finally, the algorithm’s performance was tested against 10 ophthalmologists in a prospective clinic-based dataset with 510 outpatients newly enrolled with diseases of infectious keratitis, non-infectious keratitis, corneal dystrophy or degeneration, and corneal neoplasm. The area under the ROC curve of the algorithm for each corneal disease type was over 0.910 and in general it had sensitivity and specificity similar to or better than the average values of all ophthalmologists. Confusion matrices revealed similarities in misclassification between human experts and the algorithm. In addition, our algorithm outperformed over all four previous reported methods in identified corneal diseases. The proposed algorithm may be useful for computer-assisted corneal disease diagnosis.

Abstract:Artificial intelligence (AI) has lately witnessed an age of tremendous expansion across several industries, including healthcare. In recent years, substantial advancements in AI, notably in machine learning (ML) and deep learning (DL), have had a considerable influence on traditional diagnostic and treatment procedures. Although DL has been widely used in image identification, voice recognition, and natural language processing, it is now beginning to influence healthcare. The principal imaging techniques for early detection and treatment of eye problems are now fundus digital photography and optical coherence tomography (OCT). The study predicts the presence of multiple eye diseases in the human eye. Multiple images of eye diseases like diabetic macular edema (DME) and choroidal neovascularization (CNV), DRUSEN, GLAUCOMA, NORMAL, and CATARACTS have been used to train and validate AI-based intelligence learning approaches. This study proposed multiple transfer learning models for the prediction of eye diseases. The deep transfer learning approaches used in the study are Basic CNN, Deep CNN, AlexNet 2, Xception, Inception V3, ResNet 50, and DenseNet121. The simulation results verify that the ResNet50 attained 98.9% validation accuracy and outperformed all the other approaches. Also, the Xception model performed well and achieved 98.4% accuracy. The training and validation loss achieved by the Xception model is 0.15 and 0.05, respectively. The best root mean squared error achieved by the Xception model is 0.22. Thus, the research aims to enhance clinical decision-making that has a variety of applications for ophthalmologists.

Abstract:In ophthalmology, early fundus screening is an economic and effective way to prevent blindness caused by ophthalmic diseases. Clinically, due to the lack of medical resources, manual diagnosis is time-consuming and may delay the condition. With the development of deep learning, some researches on ophthalmic diseases have achieved good results, however, most of them are just based on one disease. During fundus screening, ophthalmologists usually give diagnoses of multi-disease on binocular fundus image, so we release a dataset with 8 diseases to meet the real medical scene, which contains 10,000 fundus images from both eyes of 5,000 patients. We did some benchmark experiments on it through some state-of-the-art deep neural networks. We found simply increasing the scale of network cannot bring good results for multi-disease classification, and a well-structured feature fusion method combines characteristics of multi-disease is needed. Through this work, we hope to advance the research of related fields.

Abstract:With the popularization of computer-aided diagnosis (CAD) technologies, more and more deep learning methods are developed to facilitate the detection of ophthalmic diseases. In this article, the deep learning-based detections for some common eye diseases, including cataract, glaucoma, and age-related macular degeneration (AMD), are analyzed. Generally speaking, morphological change in retina reveals the presence of eye disease. Then, while using some existing deep learning methods to achieve this analysis task, the satisfactory performance may not be given, since fundus images usually suffer from the impact of data imbalance and outliers. It is, therefore, expected that with the exploration of effective and robust deep learning algorithms, the detection performance could be further improved. Here, we propose a deep learning model combined with a novel mixture loss function to automatically detect eye diseases, through the analysis of retinal fundus color images. Specifically, given the good generalization and robustness of focal loss and correntropy-induced loss functions in addressing complex dataset with class imbalance and outliers, we present a mixture of those two losses in deep neural network model to improve the recognition performance of classifier for biomedical data. The proposed model is evaluated on a real-life ophthalmic dataset. Meanwhile, the performance of deep learning model with our proposed loss function is compared with the baseline models, while adopting accuracy, sensitivity, specificity, Kappa, and area under the receiver operating characteristic curve (AUC) as the evaluation metrics. The experimental results verify the effectiveness and robustness of the proposed algorithm.

Abstract:Some common physical diseases, such as diabetes and hypertension, require comprehensive diagnosis with ophthalmology in clinical practice. However, the huge number of patients has led to insufficient medical supply in many underdeveloped areas. Therefore, this work proposes a novel deep learning framework based on binocular fundus image pairs for the detection of 8 diseases related to the eyes and body. For the first time, the framework improves its basic capabilities by pre-training external fundus datasets and then uses the Multi-scale Dilated Convolution (MDC) module to fuse binocular features to realize the binocular diagnosis. To verify the effectiveness of the method, 4 different convolutional neural network (CNN) architectures are selected as the backbone and tested on the multi-label classification benchmark. Through extensive experimental comparisons, it is found that the innovations proposed in this paper improve the performance of all CNN architectures and achieve state-of-the-art results in the field.
Development of a deep learning model to distinguish the cause of optic disc atrophy using retinal fundus photography

Abstract:The differential diagnosis for optic atrophy can be challenging and requires expensive, time-consuming ancillary testing to determine the cause. While Leber's hereditary optic neuropathy (LHON) and optic neuritis (ON) are both clinically significant causes for optic atrophy, both relatively rare in the general population, contributing to limitations in obtaining large imaging datasets. This study therefore aims to develop a deep learning (DL) model based on small datasets that could distinguish the cause of optic disc atrophy using only fundus photography. We retrospectively reviewed fundus photographs of 120 normal eyes, 30 eyes (15 patients) with genetically-confirmed LHON, and 30 eyes (26 patients) with ON. Images were split into a training dataset and a test dataset and used for model training with ResNet-18. To visualize the critical regions in retinal photographs that are highly associated with disease prediction, Gradient-Weighted Class Activation Map (Grad-CAM) was used to generate image-level attention heat maps and to enhance the interpretability of the DL system. In the 3-class classification of normal, LHON, and ON, the area under the receiver operating characteristic curve (AUROC) was 1.0 for normal, 0.988 for LHON, and 0.990 for ON, clearly differentiating each class from the others with an overall total accuracy of 0.93. Specifically, when distinguishing between normal and disease cases, the precision, recall, and F1 scores were perfect at 1.0. Furthermore, in the differentiation of LHON from other conditions, ON from others, and between LHON and ON, we consistently observed precision, recall, and F1 scores of 0.8. The model performance was maintained until only 10% of the pixel values of the image, identified as important by Grad-CAM, were preserved and the rest were masked, followed by retraining and evaluation.

Abstract:It is difficult for clinicians or less-experienced ophthalmologists to detect early eye-related diseases. By hand, eye disease diagnosis is labor-intensive, prone to mistakes, and challenging because of the variety of ocular diseases such as glaucoma (GA), diabetic retinopathy (DR), cataract (CT), and normal eye-related diseases (NL). An automated ocular disease detection system with computer-aided diagnosis (CAD) tools is required to recognize eye-related diseases. Nowadays, deep learning (DL) algorithms enhance the classification results of retinograph images. To address these issues, we developed an intelligent detection system based on retinal fundus images. To create this system, we used ODIR and RFMiD datasets, which included various retinographics of distinct classes of the fundus, using cutting-edge image classification algorithms like ensemble-based transfer learning. In this paper, we suggest a three-step hybrid ensemble model that combines a classifier, a feature extractor, and a feature selector. The original image features are first extracted using a pre-trained AlexNet model with an enhanced structure. The improved AlexNet (iAlexNet) architecture with attention and dense layers offers enhanced feature extraction, task adaptability, interpretability, and potential accuracy benefits compared to other transfer learning architectures, making it particularly suited for tasks like retinograph classification. The extracted features are then selected using the ReliefF method, and then the most crucial elements are chosen to minimize the feature dimension. Finally, an XgBoost classifier offers classification outcomes based on the desired features. These classifications represent different ocular illnesses. We utilized data augmentation techniques to control class imbalance issues. The deep-ocular model, based mainly on the AlexNet-ReliefF-XgBoost model, achieves an accuracy of 95.13%. The results indicate the proposed ensemble model can assist dermatologists in making early decisions for the diagnosing and screening of eye-related diseases.

Abstract:There are many kinds of fundus diseases, and early diagnosis is the key to prevent severe visual impairment. In this paper, we propose a deep learning model to automatically detect multiple fundus diseases which includes three parts. The first part is the pre-processing of the data including data screening, black border cropping, data augmentation, and normalization. The second part is a feature extraction network named DSRA-CNN based on Xception architecture, which combines the function blocks of DS block, DSR block, and SE block, among which DS block is composed of depthwise separable convolutions to reduce the amount of calculation, DSR block is composed of DS blocks and residual connections to strengthen the utilization of image features, SE block is used to screen the characteristic information. Lastly, based on the extracted features, we design a classifier to realize the classification of eight different fundus diseases. Experiments are performed on the ODIR dataset and the results of the accuracy, precision, F1 value, and kappa score of the DSRA-CNN network proposed in this paper are respectively 87.90%, 88.50%, 88.16%, and 86.17%. Compared with the original Xception network and existing advanced Convolutional Neural Networks(CNNs), our proposed network performs better.

Abstract:Fundus diseases are major causes of visual impairment and blindness worldwide, especially in underdeveloped regions, where the shortage of ophthalmologists hinders timely diagnosis. AI-assisted fundus image analysis has several advantages, such as high accuracy, reduced workload, and improved accessibility, but it requires a large amount of expert-annotated data to build reliable models. To address this dilemma, we propose a general self-supervised machine learning framework that can handle diverse fundus diseases from unlabeled fundus images. Our method's AUC surpasses existing supervised approaches by 15.7%, and even exceeds performance of a single human expert. Furthermore, our model adapts well to various datasets from different regions, races, and heterogeneous image sources or qualities from multiple cameras or devices. Our method offers a label-free general framework to diagnose fundus diseases, which could potentially benefit telehealth programs for early screening of people at risk of vision loss.

Abstract:Ultra-widefield (UWF) imaging is a promising modality that captures a larger retinal field of view compared with traditional fundus photography. Previous studies have shown that deep learning models are effective for detecting retinal disease in UWF images, but primarily considered individual diseases under less-than-realistic conditions (excluding images with other diseases, artefacts, comorbidities or borderline cases; and balancing healthy and diseased images) and did not systematically investigate which regions of the UWF images are relevant for disease detection. Here we first improve on the state of the field by proposing a deep learning model that can recognize multiple retinal diseases under more realistic conditions than what has previously been considered. We then use global explainability methods to identify which regions of the UWF images the model generally attends to. Our model performs very well, separating between healthy and diseased retinas with an area under the receiver operating characteristic curve (AUC) of 0.9196 (±0.0001) on an internal test set, and an AUC of 0.9848 (±0.0004) on a challenging, external test set. When diagnosing specific diseases, the model attends to regions where we would expect those diseases to occur. We further identify the posterior pole as the most important region in a purely data-driven fashion. Surprisingly, 10% of the image around the posterior pole is sufficient for achieving comparable performance across all labels to having the full images available.

Abstract:Deep learning has been successfully applied to a variety of image classification tasks. There has been keen interest to apply deep learning in the medical domain, particularly specialties that heavily utilize imaging, such as ophthalmology. One issue that may hinder application of deep learning to the medical domain is the vast amount of data necessary to train deep neural networks (DNNs). Because of regulatory and privacy issues associated with medicine, and the generally proprietary nature of data in medical domains, obtaining large datasets to train DNNs is a challenge, particularly in the ophthalmology domain. Transfer learning is a technique developed to address the issue of applying DNNs for domains with limited data. Prior reports on transfer learning have examined custom networks to fully train or used a particular DNN for transfer learning. However, to the best of my knowledge, no work has systematically examined a suite of DNNs for transfer learning for classification of diabetic retinopathy, diabetic macular edema, and two key features of age-related macular degeneration. This work attempts to investigate transfer learning for classification of these ophthalmic conditions. Part I gives a condensed overview of neural networks and the DNNs under evaluation. Part II gives the reader the necessary background concerning diabetic retinopathy and prior work on classification using retinal fundus photographs. The methodology and results of transfer learning for diabetic retinopathy classification are presented, showing that transfer learning towards this domain is feasible, with promising accuracy. Part III gives an overview of diabetic macular edema, choroidal neovascularization and drusen (features associated with age-related macular degeneration), and presents results for transfer learning evaluation using optical coherence tomography to classify these entities.

Abstract:Diabetic retinopathy (DR), retinal vein occlusion (RVO), and age-related macular degeneration (AMD) pose significant global health challenges, often resulting in vision impairment and blindness. Automatic detection of these conditions is crucial, particularly in underserved rural areas with limited access to ophthalmic services. Despite remarkable advancements in artificial intelligence, especially convolutional neural networks (CNNs), their complexity can make interpretation difficult. In this study, we curated a dataset consisting of 15,089 color fundus photographs (CFPs) obtained from 8110 patients who underwent fundus fluorescein angiography (FFA) examination. The primary objective was to construct integrated models that merge CNNs with an attention mechanism. These models were designed for a hierarchical multilabel classification task, focusing on the detection of DR, RVO, AMD, and other fundus conditions. Furthermore, our approach extended to the detailed classification of DR, RVO, and AMD according to their respective subclasses. We employed a methodology that entails the translation of diagnostic information obtained from FFA results into CFPs. Our investigation focused on evaluating the models' ability to achieve precise diagnoses solely based on CFPs. Remarkably, our models showcased improvements across diverse fundus conditions, with the ConvNeXt-base + attention model standing out for its exceptional performance. The ConvNeXt-base + attention model achieved remarkable metrics, including an area under the receiver operating characteristic curve (AUC) of 0.943, a referable F1 score of 0.870, and a Cohen's kappa of 0.778 for DR detection. For RVO, it attained an AUC of 0.960, a referable F1 score of 0.854, and a Cohen's kappa of 0.819. Furthermore, in AMD detection, the model achieved an AUC of 0.959, an F1 score of 0.727, and a Cohen's kappa of 0.686. Impressively, the model demonstrated proficiency in subclassifying RVO and AMD, showcasing commendable sensitivity and specificity. Moreover, our models enhanced interpretability by visualizing attention weights on fundus images, aiding in the identification of disease findings. These outcomes underscore the substantial impact of our models in advancing the detection of DR, RVO, and AMD, offering the potential for improved patient outcomes and positively influencing the healthcare landscape.

Deep Learning Evaluation of Glaucoma Detection Using Fundus Photographs in Highly Myopic Populations

Abstract:Objectives: This study aimed to use deep learning to identify glaucoma and normal eyes in groups with high myopia using fundus photographs. Methods: Patients who visited Tri-Services General Hospital from 1 November 2018 to 31 October 2022 were retrospectively reviewed. Patients with high myopia (spherical equivalent refraction of ≤−6.0 D) were included in the current analysis. Meanwhile, patients with pathological myopia were excluded. The participants were then divided into the high myopia group and high myopia glaucoma group. We used two classification models with the convolutional block attention module (CBAM), an attention mechanism module that enhances the performance of convolutional neural networks (CNNs), to investigate glaucoma cases. The learning data of this experiment were evaluated through fivefold cross-validation. The images were categorized into training, validation, and test sets in a ratio of 6:2:2. Grad-CAM visual visualization improved the interpretability of the CNN results. The performance indicators for evaluating the model include the area under the receiver operating characteristic curve (AUC), sensitivity, and specificity. Results: A total of 3088 fundus photographs were used for the deep-learning model, including 1540 and 1548 fundus photographs for the high myopia glaucoma and high myopia groups, respectively. The average refractive power of the high myopia glaucoma group and the high myopia group were −8.83 ± 2.9 D and −8.73 ± 2.6 D, respectively (p = 0.30). Based on a fivefold cross-validation assessment, the ConvNeXt_Base+CBAM architecture had the best performance, with an AUC of 0.894, accuracy of 82.16%, sensitivity of 81.04%, specificity of 83.27%, and F1 score of 81.92%. Conclusions: Glaucoma in individuals with high myopia was identified from their fundus photographs.

Abstract:Glaucoma is the leading cause of preventable, irreversible blindness world-wide. The disease can remain asymptomatic until severe, and an estimated 50%-90% of people with glaucoma remain undiagnosed. Glaucoma screening is recommended for early detection and treatment. A cost-effective tool to detect glaucoma could expand screening access to a much larger patient population, but such a tool is currently unavailable. We trained a deep learning algorithm using a retrospective dataset of 86,618 images, assessed for glaucomatous optic nerve head features and referable glaucomatous optic neuropathy (GON). The algorithm was validated using 3 datasets. For referable GON, the algorithm had an AUC of 0.945 (95% CI, 0.929-0.960) in dataset A (1205 images, 1 image/patient; 18.1% referable), images adjudicated by panels of Glaucoma Specialists (GSs); 0.855 (95% CI, 0.841-0.870) in dataset B (9642 images, 1 image/patient; 9.2% referable), images from Atlanta Veterans Affairs Eye Clinic diabetic teleretinal screening program; and 0.881 (95% CI, 0.838-0.918) in dataset C (346 images, 1 image/patient; 81.7% referable), images from Dr. Shroff's Charity Eye Hospital's glaucoma clinic. The algorithm showed significantly higher sensitivity than 7 of 10 graders not involved in determining the reference standard, including 2 of 3 GSs, and showed higher specificity than 3 graders, while remaining comparable to others. For both GSs and the algorithm, the most crucial features related to referable GON were: presence of vertical cup-to-disc ratio of 0.7 or more, neuroretinal rim notching, retinal nerve fiber layer defect, and bared circumlinear vessels. An algorithm trained on fundus images alone can detect referable GON with higher sensitivity than and comparable specificity to eye care providers. The algorithm maintained good performance on an independent dataset with diagnoses based on a full glaucoma workup.

Abstract:Purpose: To develop a visual function-based deep learning system (DLS) using fundus images to screen for visually impaired cataracts. Materials and methods: A total of 8,395 fundus images (5,245 subjects) with corresponding visual function parameters collected from three clinical centers were used to develop and evaluate a DLS for classifying non-cataracts, mild cataracts, and visually impaired cataracts. Three deep learning algorithms (DenseNet121, Inception V3, and ResNet50) were leveraged to train models to obtain the best one for the system. The performance of the system was evaluated using the area under the receiver operating characteristic curve (AUC), sensitivity, and specificity. Results: The AUC of the best algorithm (DenseNet121) on the internal test dataset and the two external test datasets were 0.998 (95% CI, 0.996–0.999) to 0.999 (95% CI, 0.998–1.000),0.938 (95% CI, 0.924–0.951) to 0.966 (95% CI, 0.946–0.983) and 0.937 (95% CI, 0.918–0.953) to 0.977 (95% CI, 0.962–0.989), respectively. In the comparison between the system and cataract specialists, better performance was observed in the system for detecting visually impaired cataracts ( p < 0.05). Conclusion: Our study shows the potential of a function-focused screening tool to identify visually impaired cataracts from fundus images, enabling timely patient referral to tertiary eye hospitals.
