Abstract:The Banana Bunchy Top Virus (BBTV) is one of the most economically important vector-borne banana diseases throughout the Asia-Pacific Basin and presents a significant challenge to the agricultural sector. Current models of BBTV are largely deterministic, limited by an incomplete understanding of interactions in complex natural systems, and the appropriate identification of parameters. A stochastic network-based Susceptible-Infected-Susceptible model has been created which simulates the spread of BBTV across the subsections of a banana plantation, parameterising nodal recovery, neighbouring and distant infectivity across summer and winter. Findings from posterior results achieved through Markov Chain Monte Carlo approach to approximate Bayesian computation suggest seasonality in all parameters, which are influenced by correlated changes in inspection accuracy, temperatures and aphid activity. This paper demonstrates how the model may be used for monitoring and forecasting of various disease management strategies to support policy-level decision making.The Banana Bunchy Top Virus (BBTV) poses one of the greatest threats to the food security of developing nations and the banana industry throughout the Asia-Pacific Basin. Decision-makers face significant challenges in mitigating BBTV spread in banana plantations due to the vector-borne spread of this disease, which is significantly influenced by a vast array of external environmental factors that are unique to each plantation. We propose a flexible network-based model that describes the spread of BBTV in a real banana plantation through a random process while accounting for individual plantation characteristics and utilise a principled methodology for estimating model parameters. Our models can be used to quantify the effects of seasonal changes and plantation configuration on BBTV spread and can be used to predict high-risk areas in this plantation. We believe that our model might be used by decision-makers to evaluate the effectiveness of current disease management strategies and explore opportunities for improvements.

Abstract:Malawi has local banana germplasm preferred by its population. However, the epidemics of banana bunchy top disease, caused by the banana bunchy top virus (BBTV), is wiping out the most preferred germplasm and limiting its plantation. On the other hand, there needs to be more knowledge on viruses present in Malawi on banana crops. Therefore, a survey was conducted country-wide to characterize banana germplasm and evaluate the presence, incidence and prevalence of banana viruses. The survey covered four country zones, combined farmer's structured questionnaire and plant sampling. PCR products from infected germplasm were sequenced and aligned for each detected virus to build a phylogenetic tree. BBTV, BanMMV and BSV species were detected in Malawi. Malawi's BBTV isolates belonged to the Pacific Indian Ocean group only and BanMMV isolates clustered to three sub-branches. All the BSV species present in Malawi belonged to clade 1. Among the genetic groups of Musa, the identified banana germplasm belonged to AA, AAA, AB, AAB, and ABB groups with some germplasms unique compared to already genotyped germplasms. The ABB group was dominant in Malawi and was often infected by BSV species (probably originating from endogenous viral sequences), while BBTV more often infected the AAA group. Banana propagule sharing was the primary source of banana planting materials with a higher risk of spreading virus diseases. The survey underlined the need to set up a banana seed industry and policies that promote farmers' access to virus-tested planting materials, which will consequently prevent future virus epidemics.

Abstract:Summary This paper describes the results of an international study to investigate the effect of a reasonable amount of dietary fish on platelet aggregation in platelet-rich plasma (PRP) induced by collagen and thrombin. In Maastricht, Tiomsø, and Zeist two groups of healthy male volunteers were given a daily dietary supplement consisting of 135 g of canned mackerel paste (experimental group, n = 40) or meat paste (control group, n = 42) for a 6-week period. Compliance, calculated on the basis of the urinary excretion of lithium, added to the supplements, was about 80%. Platelet number in PRP decreased significantly in the fish group. Collagen-induced platelet aggregation in PRP differed widely between the three centres despite the attempt to use exactly the same conditions. Nonetheless, aggregation decreased significantly in the fish group. The mackerel effect on thrombin-induced aggregation was inconsistent.
Climate-Related Risk Modeling of Banana Xanthomonas Wilt Disease Incidence in the Cropland Area of Rwanda

Abstract:Banana Xanthomonas wilt (BXW) is a major threat to banana production in Rwanda, causing up to 100% yield loss. There are no biological or chemical control measures, and little is known about the potential direction and magnitude of its spread; hence, cultural control efforts are reactive rather than proactive. In this study, we assessed BXW risk under current and projected climates to guide early warning and control by applying the maximum entropy (Maxent) model on 1,022 georeferenced BXW datapoints and 20 environmental variables. We evaluated the significance of variables and mapped potential risk under current and future climates to assess spatial dynamics of the disease distribution. BXW occurrence was reliably predicted (mean validation AUC values ranging from 0.79 to 0.85). Precipitation of the coldest quarter, average maximum monthly temperature, annual precipitation, and elevation were the strongest predictors, which were responsible for 22.1, 13, 12.6, and 9.4% of the observed incidence variability, respectively, while mean temperature of the coldest quarter had the highest gain in isolation. Furthermore, the most susceptible regions (western, northern, and southern Rwanda) were characterized by elevation (1,350 to 2,000 m), annual precipitation (900 to 1,700 mm), and average temperature (14 to 20°C), among other variables, suggesting that a consistent, rainy, and warm climate is more favorable for BXW spread. Under the future climate, the risk was predicted to increase and spread to other regions. We conclude that climate change will likely exacerbate BXW-related losses of banana land area and yield under the influence of temperature and moisture. Our findings support evidence-based targeting of extension service delivery to farmers and national early warning for timely action.

Abstract:Banana Xanthomonas wilt (BXW) is a destructive disease caused by Xanthomonas vasicola pv. musacearum (Xvm) bacterium that indiscriminately infects all banana varieties grown in East and Central Africa (ECA). In this region, BXW was first reported in 2001 in Uganda and was projected to eliminate >90% of Uganda’s bananas worth US$4 billion if not controlled in less than 10 years. Lack of basic information led to application of control approaches that were based on similarity of BXW symptoms to those of Moko disease of bananas. The approaches were however, unsuccessful and in 7-9 years, BXW had covered six countries and threatened to wipe out the banana industry in ECA. However, BXW has to-date been tamed, mainly due to relentless and systematic deployment of carefully crafted and packaged cultural control practices based on epidemiological information generated within target banana cropping ecosystems. In Uganda, the initial “top-down” communication approaches reached >85% banana farming communities but did not mobilize the communities enough into action; hence, only 30% impact in controlling BXW was registered. In contrast, participatory approaches mobilised farming communities into action and effectively controlled BXW at field and community levels to near eradication. The approaches effectively controlled BXW in Uganda, and consequently, in eastern Kenya, northern Tanzania, Rwanda, Burundi and DRC. This paper reviews step-wise processes leading to success over the 2 decades and identifies critical research gaps. Deployment of resistant genotypes is urgently needed as a significant addition to the BXW management tool-box to create BXW-free banana cropping systems in ECA.

Abstract:Pentalonia nigronervosa Coquerel ( Hemiptera : Aphididae ) is the vector of the Banana Bunchy Top Virus (BBTV), the most serious viral disease of banana ( Musa spp.) in the world. Before acquiring the virus, the vector is more attracted to infected banana plants in response to the increased emissions of volatile organic compounds (VOCs). Here, we test the hypothesis that BBTV acquisition directly modifies the preference of P. nigronervosa for infected banana plants, and that the change in preference results from the alteration of the organs linked to the VOC detection or to the behaviour of the vector. We found that the preference of P. nigronervosa for infected banana plants reverses after virus acquisition in dessert banana, while it remains similar between healthy and infected banana plants before and after the acquisition of BBTV. At the same time, aphids reared on infected bananas had smaller forewing areas and hind tibia length than aphids reared on healthy bananas, although the number of secondary rhinaria on the antennae was lower on dessert banana-reared aphids than plantain-reared aphids, this was not affected by the infection status of the aphid. These results support the "vector manipulation hypothesis—VMH" of pathogens to promote their spread. They have implications for the BBTV management.

Abstract:Banana Xanthomonas wilt (BXW) is one of the most important diseases threatening banana production in Africa south of the Sahara (SSA). In this study, we examine the potential impacts of BXW on banana production, demand, and food security in SSA, if the disease spread across all banana-producing countries in the region. The analysis is based on a multidisciplinary approach that combines a mathematical model of field-level BXW spread over time with a dynamic global partial equilibrium economic model. Since BXW control relies exclusively on management, we analyze three scenarios of BXW spread that are constructed around assumptions about the level of policy response to the disease, and about how this response may affect the adoption of appropriate management practices by farmers to control BXW. Modeling results suggest that if the disease is left uncontrolled, banana production in SSA within 10 years can decrease by as much as 55%, compared to a BXW-free baseline scenario, resulting in economic losses of around 25 billion USD. At the same time, the population at risk of hunger in countries that highly depend on bananas as a staple food is projected to increase by more than 4.6%. Even a limited policy response to BXW can reduce infections and mitigate some of the production, economic, and food security consequences. BXW impacts are almost completely negated when farmers have good knowledge of the disease and fully adopt the appropriate management practices. This result highlights the need for policy frameworks which rely on sustained and coordinated efforts by public and private stakeholders, within and across SSA countries and at different geographical scales. It also aims to raise awareness and promote the adoption of such practices, while also considering local peculiarities and socioeconomic conditions.

Abstract:Abstract The rice yellow mottle virus (RYMV) is a model in plant virus molecular epidemiology, with the reconstruction of historical introduction routes at the scale of the African continent. However, information on patterns of viral prevalence and viral diversity over multiple years at a local scale remains scarce, in spite of potential implications for crop protection. Here, we describe a 5-year (2015–9) monitoring of RYMV prevalence in six sites from western Burkina Faso (geographic areas of Bama, Banzon, and Karfiguela). It confirmed one irrigated site as a disease hotspot and also found one rainfed lowland (RL) site with occasional high prevalence levels. Within the studied fields, a pattern of disease aggregation was evidenced at a 5-m distance, as expected for a mechanically transmitted virus. Next, we monitored RYMV genetic diversity in the irrigated disease hotspot site, revealing a high viral diversity, with the current coexistence of various distinct genetic groups at the site scale (ca. 520 ha) and also within various specific fields (25 m side). One genetic lineage, named S1bzn, is the most recently emerged group and increased in frequency over the studied period (from 20 per cent or less in 2015–6 to more than 65 per cent in 2019). Its genome results from a recombination between two other lineages (S1wa and S1ca). Finally, experimental work revealed that three rice varieties commonly cultivated in Burkina Faso were not different in terms of resistance level, and we also found no significant effect of RYMV genetic groups on symptom expression and viral load. We found, however, that infection outcome depended on the specific RYMV isolate, with two isolates from the lineage S1bzn accumulating at the highest level at early infections. Overall, this study documents a case of high viral prevalence, high viral diversity, and co-occurrence of divergent genetic lineages at a small geographic scale. A recently emerged lineage, which comprises viral isolates inducing severe symptoms and high accumulation under controlled conditions, could be recently rising through natural selection. Following up the monitoring of RYMV diversity is required to confirm this trend and further understand the factors driving the local maintenance of viral diversity.

Abstract:As global demand for bananas rises, the threat of banana bunchy top disease (BBTD), caused by banana bunchy top virus (BBTV), poses a significant constraint to sustainable banana production. To circumvent this, it is critical to fast-track the development of BBTV-resistant banana cultivars. In a dataset of differentially expressed genes between BBTV-resistant and susceptible bananas, six RLK genes involved in plant defense against pathogens were exclusively upregulated in the BBTV-resistant wild Musa balbisiana compared to the BBTV-susceptible Musa acuminata cv. ’Lakatan’. Here, two genic variants of RLKs, single nucleotide polymorphisms (SNPs) and simple sequence repeats (SSR), were identified in 10 banana accessions representing major genomic groupings. Nine gene-specific markers were developed using three marker systems: SSR, allele-specific PCR, and tetra primer amplification refractory mutation system PCR markers. Six of these were polymorphic and successfully discriminated 117 Musa genotypes at the NPGRL-IPB ex situ germplasm, with an average polymorphism information content of 0.36 for SNP markers and 0.49 for SSR marker, both of which are moderately informative. These markers facilitated the construction of a dendrogram using the Unweighted Neighbor-Joining (UNJ) method with 1000 bootstrap iterations, identifying four major clades: M. acuminata spp. and other Musa spp. (I), wild M. balbisiana (BBw) and other accessions with high-proportion of B-genome (II & III), hybrid bananas (triploids and tetraploids) (IV), which revealed the genetic diversity and relationships among Musa accessions. Ultimately, this genomic resource will aid in positioning the Philippines as a key player in banana disease resistance breeding towards a food-secure banana industry.

Abstract:This paper looks into the MaxEnt model in a trial to comprehend the ecological and environmental conditions that propagate and drive the spread of Ebola Virus Disease in Africa. We use the MaxEnt model to assess risk determinants associated with the occurrence and distribution of EVD, taking into account non-correlated variables such as neighborhood mean temperature, rainfall, and human population density. Our findings indicate that among the factors that significantly shape the geographical distribution of EVD risk are human population density, annual rainfall, temperature variability, and seasonality. The model used is both reliable and accurate (the average value for training AUC was 0.987); it can be used as a valuable approach for the prediction of infectious disease outbreaks. High-risk areas are primarily identified in the western and central regions of Africa, with some of the others in the east also vulnerable. This further calls for specified public health interventions and enhanced surveillance in specified hotspots, contributing to global efforts to predict and mitigate risks associated with EVD outbreaks more adequately. The findings further support that it remains imperative to conduct additional research, including socio-economic and cultural variables, to enhance the understanding of how environmental factors contribute to the emergence and transmission of Ebola.

Abstract:Banana bunchy top disease (BBTD) is one of the world's most destructive viral diseases of banana and plantain, causing up to 100% yield loss in severe cases. The disease is vectored by banana aphids (Pentalonia nigronervosa) and carried long distances through the movement of infected plant materials. The banana aphids harboring banana bunchy top virus (BBTV) present in banana producing regions are the sole vector and the most efficient method of transmitting the virus to the healthy plants. Controlling the spread of BBTD has been very challenging since no known banana germplasm is immune to BBTV. The disease can be managed with the use of virus‐free planting material and roguing. However, once BBTD is established in the field, it is very difficult to eradicate or manage it. Therefore, a more sustainable way of controlling the disease is developing host plant resistance against the virus and the vector. Biotechnological strategies via RNA interference (RNAi) could be used to target the banana aphid as well as BBTV to reduce virus‐associated yield losses of banana and plantain, which feed over 500 million people around the world. This review discusses the status of BBTD and perspectives on effective RNAi technologies for controlling BBTV and the vector, banana aphid, transmitting the virus as sustainable management of the disease.

Abstract:Understanding how weather and landscape shape the fine-scale distribution and diversity of malaria vectors is crucial for efficient and locally tailored vector control. This study examines the meteorological and landscape determinants of (i) the spatiotemporal distribution (presence and abundance) of the major malaria vectors in the rural region of Korhogo (northern Côte d'Ivoire) and (ii) the differences in vector probability of presence, abundance, and diversity observed between that area and another rural West African region located 300 km away in Diébougou, Burkina Faso. We monitored Anopheles human-biting activity in 28 villages of the Korhogo health district for 18 months (2016 to 2018), and extracted fine-scale environmental variables (meteorological and landscape) from high-resolution satellite imagery. We used a state-of-the-art statistical modeling framework to associate these data and identify environmental determinants of the presence and abundance of malaria vectors in the area. We then compared the results of this analysis with those of a similar, previously published study conducted in the Diébougou area. The spatiotemporal distribution of malaria vectors in the Korhogo area was highly heterogeneous and appeared to be strongly determined and constrained by meteorological conditions. Rice paddies, temporary sites filled by rainfall, rivers and riparian forests appeared to be the larval habitats of Anopheles mosquitoes. As in Diébougou, meteorological conditions (temperatures, rainfall) appeared to significantly affect all developmental stages of the mosquitoes. Additionally, ligneous savannas were associated with lower abundance of malaria vectors. Anopheles species diversity was lower in Korhogo compared to Diébougou, while biting rates were much higher. Our results suggest that these differences may be due to the more anthropized nature of the Korhogo region in comparison to Diébougou (less forested areas, more agricultural land), supporting the hypothesis of higher malaria vector densities and lower mosquito diversity in more anthropized landscapes in rural West Africa. This study offers valuable insights into the landscape and meteorological determinants of the spatiotemporal distribution of malaria vectors in the Korhogo region and, more broadly, in rural west-Africa. The results emphasize the adverse effects of the ongoing landscape anthropization process in the sub-region, including deforestation and agricultural development, on malaria vector control.

Abstract:BackgroundThe United Nations forecasts that by 2050, more than 60% of the African population will live in cities. Thus, urban malaria is considered an important emerging health problem in that continent. Remote sensing (RS) and geographic information systems (GIS) are useful tools for addressing the challenge of assessing, understanding and spatially focusing malaria control activities. The objectives of the present study were to use high spatial resolution SPOT (Satellite Pour l'Observation de la Terre) satellite images to identify some urban environmental factors in Dakar associated with Anopheles arabiensis densities, to assess the persistence of these associations and to describe spatial changes in at-risk environments using a decadal time scale.MethodsTwo SPOT images from the 1996 and 2007 rainy seasons in Dakar were processed to extract environmental factors, using supervised classification of land use and land cover, and a calculation of NDVI (Normalized Difference Vegetation Index) and distance to vegetation. Linear regressions were fitted to identify the ecological factors associated with An. arabiensis aggressiveness measured in 1994-97 in the South and centre districts of Dakar. Risk maps for populated areas were computed and compared for 1996 and 2007 using the results of the statistical models.ResultsAlmost 60% of the variability in anopheline aggressiveness measured in 1994-97 was explained with only one variable: the built-up area in a 300-m radius buffer around the catching points. This association remained stable between 1996 and 2007. Risk maps were drawn by inverting the statistical association. The total increase of the built-up areas in Dakar was about 30% between 1996 and 2007. In proportion to the total population of the city, the population at high risk for malaria fell from 32% to 20%, whereas the low-risk population rose from 29 to 41%.ConclusionsEnvironmental data retrieved from high spatial resolution SPOT satellite images were associated with An. arabiensis densities in Dakar urban setting, which allowed to generate malaria transmission risk maps. The evolution of the risk was quantified, and the results indicated there are benefits of urbanization in Dakar, since the proportion of the low risk population increased while urbanization progressed.

Abstract:Malaria remains endemic in tropical areas, especially in Africa. For the evaluation of new tools and to further ourunderstanding of host-parasite interactions, knowing the environmental risk of transmission-even at a very local scale-isessential. The aim of this study was to assess how malaria transmission is influenced and can be predicted by local climaticand environmental factors. As the entomological part of a cohort study of 650 newborn babies in nine villages in the ToriBossito district of Southern Benin between June 2007 and February 2010, human landing catches were performed to assessthe density of malaria vectors and transmission intensity. Climatic factors as well as household characteristics were recordedthroughout the study. Statistical correlations between Anopheles density and environmental and climatic factors weretested using a three-level Poisson mixed regression model. The results showed both temporal variations in vector density(related to season and rainfall), and spatial variations at the level of both village and house. These spatial variations could belargely explained by factors associated with the house's immediate surroundings, namely soil type, vegetation index andthe proximity of a watercourse. Based on these results, a predictive regression model was developed using a leave-one-outmethod, to predict the spatiotemporal variability of malaria transmission in the nine villages. This study points up theimportance of local environmental factors in malaria transmission and describes a model to predict the transmission risk ofindividual children, based on environmental and behavioral characteristics.

Abstract:Abstract Several initiatives by the Government of Uganda, Research Institutes and CGIAR centers have promoted the use of tissue culture (TC) banana technology as an effective means of providing clean planting material to reduce the spread of Banana Xanthomonas wilt (BXW) but its uptake is still low. We examine factors that constrain uptake of tissue culture banana planting materials in central Uganda by considering the cultural context of banana cultivation. Data were collected using eight focus group discussions involving 64 banana farmers and 10 key informant interviews and subjected to thematic analysis. Results showed that banana cultivars in the study communities were important for food, cultural practices and medicine. Cultivars supplied through TC were based on commercial considerations focusing on market value and household income and insufficient attention was given to their cultural importance. Farmers regard banana from TC planting material to be incompatible with their tastes and preferences for traditional food and drinks, culture and medicine. Furthermore, the plantlets are perceived as complicated to use, and farmers report requiring more knowledge and information on how to plant and maintain the plantlets on-farm. In these aspects, TC planting material does not align with cultural values linked to societal welfare. Future efforts aimed at controlling pests and diseases would benefit from more location-specific and holistic approaches that integrate cultural dimensions alongside planting material hygiene, quality and vigor.

Abstract:Cameroon and Ethiopia face humanitarian crises that disrupt the production of key food security crops like banana and plantain, cassava, potato, and sweetpotato. In this study, we address seed degeneration in these crops, the reduction in planting material quality due to the accumulation of pathogens and pests. We consider the context of humanitarian crises, with the goal of developing an ongoing 'national plant health risk analysis', in this case to inform decisions about mitigating seed degeneration. We analyzed cropland density maps and applied network centrality metrics to identify locations that are candidates for disease and pest management and surveillance in Cameroon and Ethiopia. We used expert knowledge elicitation and global trade data to map pathogen and pest movement risks and characterize trade of crop-specific commodities and planting materials. We identified specific locations that may be important for pathogen and pest spread in in Cameroon and Ethiopia, given their roles in the cropland network, and the reported presence of pathogens and pests. This national plant health risk analysis can inform regional and national surveillance and management strategies and provide a baseline for more in-depth studies in Cameroon and Ethiopia. We discuss strategies for ongoing improvement of a national plant health risk analysis, to inform decision-making in the humanitarian sector for designing on-the-ground actions to avoid unintentional spread of pathogens and pests during agricultural recovery interventions.

Dilution Effect In Bovine Tuberculosis: Risk Factors For Regional Disease Occurrence In Africa

Abstract:Changes in host diversity have been postulated to influence the risk of infectious diseases, including both dilution and amplification effects. The dilution effect refers to a negative relationship between biodiversity and disease risk, whereas the amplification effect occurs when biodiversity increases disease risk. We tested these effects with an influential disease, bovine tuberculosis (BTB), which is widespread in many countries, causing severe economic losses. Based on the BTB outbreak data in cattle from 2005 to 2010, we also tested, using generalized linear mixed models, which other factors were associated with the regional BTB presence in cattle in Africa. The inter-dependencies of predictors and their correlations with BTB presence were examined using path analysis. Our results suggested a dilution effect, where increased mammal species richness was associated with reduced probability of BTB presence after adjustment for cattle density. In addition, our results also suggested that areas with BTB infection in the preceding year, higher cattle density and larger percentage of area occupied by African buffalo were more likely to report BTB outbreaks. Climatic variables only indirectly influenced the risk of BTB presence through their effects on cattle density and wildlife distribution. Since most studies investigating the role of wildlife species on BTB transmission only involve single-species analysis, more efforts are needed to better understand the effect of the structure of wildlife communities on BTB dynamics.

Abstract:The unexpected Ebola virus outbreak in West Africa in 2014 involving the Zaire ebolavirus made clear that other regions outside Central Africa, its previously documented niche, were at risk of future epidemics. The complex transmission cycle and a lack of epidemiological data make mapping areas at risk of the disease challenging. We used a Geographic Information System-based multicriteria evaluation (GIS-MCE), a knowledge-based approach, to identify areas suitable for Ebola virus spillover to humans in regions of Guinea, Congo and Gabon where Ebola viruses already emerged. We identified environmental, climatic and anthropogenic risk factors and potential hosts from a literature review. Geographical data layers, representing risk factors, were combined to produce suitability maps of Ebola virus spillover at the landscape scale. Our maps show high spatial and temporal variability in the suitability for Ebola virus spillover at a fine regional scale. Reported spillover events fell in areas of intermediate to high suitability in our maps, and a sensitivity analysis showed that the maps produced were robust. There are still important gaps in our knowledge about what factors are associated with the risk of Ebola virus spillover. As more information becomes available, maps produced using the GIS-MCE approach can be easily updated to improve surveillance and the prevention of future outbreaks.

We discuss strategies for ongoing improvement of a national plant health risk analysis, to inform decision-making in the humanitarian sector for designing on-the-ground actions to avoid unintentional spread of pathogens and pests during agricultural recovery interventions.

Abstract:664 Background: Cumulative sensory neurotoxicity of l-OHP used in standard 1st-line FOLFOX / CapeOX +/- BV therapies in mCRC results in impairment of patient's quality of life (QoL) and discontinuation of the treatment. CONcePT trial revealed that intermittent l-OHP use in BV plus FOLFOX7 is safe, keeping efficacy comparing with BV plus FOLFOX7 in which l-OHP is administered until time to treatment failure. But it is unknown that intermittent l-OHP administration is also safe and effective in Japanese pts with mCRC Methods: This trial aims to enroll 65 of Japanese pts with mCRC. Either BV+FOLFOX or BV+CapeOX is selected by decision of physician in charge. In BV+FOLFOX arm, pts receive 6 cycles of BV plus mFOLFOX6 (5 mg/kg of BV, 85 mg/m2 of l-OHP, 200 mg/m2 of I-LV, 400 mg/m2 of 5-FU by rapid IV infusion on day1, and 2,400 mg/m2 of 5-FU 46 hr by continuous IV infusion as a 2 weeks course) and 6 cycles of BV plus sLV5FU2. In BV+CapeOX arm, pts recieve 4 cycles of BV plus CapeOX (7.5 mg/kg of BV, 120 mg/m2 of l-OHP and capecitabine 1,000mg/m2 twice daily, day1 to 14 as a 3weeks course) and 4 cycles of BV+capecitabine. The primary endpoint is progression free survival (PFS) and secondary endpoints are tumor response (RR), overall survival (OS), time to treatment failure (TTF) and frequency of neurotoxicity. RESULTS According the protocol, interim analysis about efficacy and safety was performed at the time the 13th pts was received the treatment for 7 months. Median progression free survival is 290+ days (95% CI 119 - 558 days) and PFS of 9 cases were more than 7 months, which is threshold value. No serious adverse effects were observed in 13 cases. The efficacy and safety evaluation committee recommended continuing the study and the current accrual is 47 cases. CONCLUSIONS This is the interim report of T-CORE0901 trial and the study remains open to accrual with projected enrollment completion in 2012.
