Abstract:Purpose: To evaluate the prediction error (PE) in pediatric traumatic cataract surgery involving primary or secondary intraocular lens implantation (IOL) and the factors affecting it. Methods: Retrospective data of unilateral traumatic cataract eyes of children aged ≤16 years were collected between February 2019 and March 2022 at a tertiary eye care hospital. Absolute PE was calculated by deducting the target refraction from the observed refraction at 6 weeks postsurgery following suture removal. Simulated PE was calculated in eyes with corneal scar replacing the affected eye keratometry (K) with the K of the fellow eye and standard K (44D) and was then compared with absolute PE. Results: Fifty children with a mean age of 9.5 years (2-16 years) were included. Mean absolute PE was 1.63 ± 1.8D (0 to 9D). Absolute PE was not affected by the age at surgery, method of biometry, duration of injury, type of cataract surgery, position of IOL, and preoperative keratometry. The absolute PE was affected by axial length (AXL) being <1.5D in AXL of 22.73 ± 0.84 mm, <1.5-2D in AXL of 22.07 ± 0.61 mm, and >2D in AXL of 22.01 ± 0.74 mm ( P = 0.039) in univariate analysis. In multivariate analysis, none of the factors affected the absolute PE. In 34 eyes with corneal scar, higher variability in PE was observed. The standard K resulted in greater simulated PE as compared to the affected eye average K in eyes with scar involving the visual axis. Conclusion: Absolute PE following pediatric traumatic cataract was studied. It was higher in shorter AXLs. In corneal scar involving the visual axis, using the fellow eye K yielded lesser simulated PE as compared to standard K.
Surgical challenges of posterior optic capture in pediatric cataract surgery

Abstract:Purpose: The efficacy of posterior optic capture (POC) in reducing posterior capsule opacification (PCO) in pediatric cataract is well recognized. The purpose of this paper was to identify the surgical challenges when attempting this technique and highlight the etiquettes to follow when performing this maneuver. Methods: Prospective observational noncomparative case series. Children diagnosed with congenital or developmental cataracts undergoing cataract surgery and primary IOL implantation with posterior optic capture (and no anterior vitrectomy) from June 2017 to April 2022 at a tertiary care referral institute were included. Records of all intraoperative findings and postoperative complications until the last follow-up were noted. Results: Posterior optic capture was attempted in 53 eyes of 49 children aged 2.4 ± 1.98 years. The mean follow-up of the patients was 16.5 ± 14.2 months (range 6 months-5 years). Successful POC could be performed in 46 eyes (86.8%). Two eyes developed posterior capsular opacification at the last follow-up. In eyes where POC could not be performed, five of these (83%) were children below 12 months of age with half of them having a preexisting posterior capsular defect. Conclusion: Posterior optic capture is technically challenging with a steep learning curve that can be mastered over time. Adequate relative sizing of the anterior and posterior capsulorhexis is important. Caution is advised when using this technique in infants and in cases with posterior capsular defects.
Intraoperative OCT for the Assessment of Posterior Capsular Integrity in Pediatric Cataract Surgery.

Abstract:PURPOSE:To study the morphology of the posterior lens cortex and posterior capsules (PCs) in pediatric patients with posterior lens opacities using intraoperative optical coherence tomography (iOCT).SETTING:Zhongshan Ophthalmic Center, Sun Yat-sen University, Guangzhou, China.DESIGN:Prospective observational study.METHODS:Pediatric patients with posterior lens opacities were imaged using iOCT during cataract surgery. The morphology of the posterior lens cortex and PC, along with the common patterns to indicate PC integrity, was assessed. Moreover, PC rent during surgery was observed.RESULTS:A total of 62 eyes from 53 patients were included. The mean age of patients was 3.8 years. 4 morphological variants of posterior lens opacity were observed: type I (34/62 [54.8%]) with an intact PC; type II (20/62 [32.3%]) with an intact PC, which protruded into the anterior vitreous; type III (3/62 [4.8%]) with a deficient PC and an inability to delineate the PC; and type IV (5/62 [8.1%]) with dense opacity and an inability to characterize the posterior cortex and PC. Phacoemulsification could be performed in types I and II. In types III and IV, manual nucleus removal was performed instead of phacoemulsification. 3 cases (100%) of type III PC dehiscence developed during surgery, whereas no cases developed PC dehiscence of other types.CONCLUSIONS:The morphology of the PC and posterior lens cortex in pediatric posterior lens opacities could be categorized, and PC integrity could be assessed using iOCT, which was useful to guide surgical strategies and increase safety in pre-existing PC dehiscence in pediatric cataract surgery.

Abstract:Purpose: To assess the possibility of anterior segment optical coherence tomography (AS-OCT)-based preoperative evaluation of the lens in order to prevent and predict intraoperative complications of posterior subcapsular cataract (PSC) surgery. Methods: This prospective study included 512 eyes diagnosed with PSC. AS-OCT was performed using Line, Cross Line and 3D Cornea scans to visualize the posterior capsule. The posterior capsule and opacities in the subcortical and cortical lens, their relationship and the state of the retrolenticular space were assessed. The study sample was divided into three groups while taking into account the revealed morphological changes in the lens. Groups 1, 2, and 3 comprised 312, 185 and 15 eyes, respectively, with each group characterized by a specific type (1, 2 or 3) of morphological AS-OCT changes in the PSC. Surgery consisted of ultrasound phacoemulsification with intraocular lens implantation. Well-known measures related to cataract surgery stages were performed, if required, to preserve the integrity of the posterior capsule, while taking into account the type of PSC changes. We preoperatively determined the eyes at risk for intraoperative posterior capsular rupture (PCR) and detection of posterior capsular plaque (PCP), and compared this data with the postoperative data on the state of the posterior capsule. Results: The PSC cases with an expected rate of intraoperative complications of 0 to 10% were classified as those with a low risk, whereas the rest, with a high risk of complications. Only eyes with type 2 or type 3 PSC changes were expected to have a high risk of intraoperative PCP, and only eyes with type 3 PSC changes, a high risk of PCR. In groups 1, 2 and 3, the rates of intraoperative PCP were 0%, 100% and 46.7%, respectively, and the rates of intraoperative PCR, 0%, 0% and 53.3%, respectively. There was a significant positive correlation between preoperative OCT-based morphology of the lens and intraoperative complications (r = 0.88, p ≤0.001). Sensitivity and specificity for the method of AS-OCT-based evaluation of risks of intraoperative complications in PSC surgery were 98.8% and 96.5%, respectively. Conclusion: AS-OCT allows evaluating preoperatively posterior lens opacification morphology and posterior capsular changes, determining the risks of complications, and performing surgical planning for PSC. Key messages: What is Known? Complications (posterior capsule (PC) rupture with or without vitreous loss and residual PC plaque) are common in, and affect the expected outcome of, posterior subcapsular cataract (PSC) surgery. What is new? PC plaque is most likely in eyes with preoperative type 2 changes in the PSC, whereas eyes with preoperative type 3 changes are likely to show PC rupture or residual PC plaque. AS-OCT enables an experienced surgeon to predict the risks of intraoperative complications in, and perform surgical planning for, PSC surgery.

Abstract:Background: This study aimed to evaluate the factors influencing final visual acuity in pediatric traumatic cataracts. Methods: Data of patients who presented with traumatic cataracts were reviewed retrospectively. We evaluated age at trauma; gender, trauma type, cause, and zone; duration between the time of trauma and cataract surgery; surgical method used; time, location, and type of intraocular lens (IOL) implantation; initial and final best corrected visual acuity (BCVA); amblyopia rate; and complications. Results: In all, 61 eyes of 59 patients aged < 16 years with cataracts after trauma were included. The mean age of the children was 7.2 ± 3.9 years. Primary IOL implantation was performed in 70.9% of eyes. The BCVA was 0.7 LogMAR or better in 5.9% of the 49 eyes in which the visual acuity could be measured at the time of trauma and in 69.1% of 55 eyes in which it could be measured after treatment. Evaluation of factors potentially influencing the final visual acuity revealed that eyes that had undergone posterior capsulotomy (PC) and anterior vitrectomy (AV) during cataract surgery had significantly better final visual acuity compared with eyes that did not undergo these procedures. Conclusions: In children with posttraumatic cataracts, final visual acuity was not affected by patient age and gender; trauma type, cause, and zone; duration between the time of trauma and cataract surgery; surgical method used; and time, location, and type of intraocular lens (IOL) implantation. Improvements in the final BCVA could be seen only by PC + AV combined with lens aspiration with or without IOL implantation. However, this approach of amblyopia treatment needs to be confirmed by more comprehensive and prospective studies.
Primary versus secondary intraocular lens implantation in traumatic cataract after open-globe injury in pediatric patients

Abstract:Purpose: To compare the visual and refractive outcomes and complications of primary and secondary intraocular lens (IOL) implantation after open-globe injury in pediatric patients. Setting: Sadguru Netra Chikitsalaya, Madhya Pradesh, India. Design: Retrospective case series. Methods: Patients had primary cataract extraction with IOL implantation with globe repair (Group A) or repair followed by secondary IOL implantation (Group B). Preoperative and surgery data, visual outcomes, and postoperative complications were recorded. Results: The study included 139 patients. Thirty (49%) of 61 patients in Group A and 47 (60%) of 78 patients in Group B achieved a corrected distance visual acuity (CDVA) of 20/40 or better. In both groups, a central corneal scar and amblyopia were the main reasons for not achieving a CDVA better than 20/40. The mean spherical equivalent (SE) was 1.81 diopters (D) in Group A and 1.55 D in Group B. Forty patients (66%) in Group A and in 60 patients (77%) in Group B achieved a desirable refractive outcome (SE <2.00 D). A large wound was the greatest risk factor for not obtaining desirable refractive outcomes. Fibrinous uveitis was the most common postoperative complication in both groups. Pupillary optic capture and IOL decentration were significantly more common in Group A than in Group B (P = .02), while strabismus was significantly more common in Group B (P = .04). Conclusions: Both approaches had satisfactory and comparable visual and refractive outcomes. However, primary IOL implantation can be considered in cases with small peripheral corneal lacerations with the goal of providing early visual rehabilitation and avoiding repeat general anesthesia.

Abstract:Question What are the visual acuity outcomes and adverse events after surgery for traumatic cataract in children?Findings In this cohort study of 72 patients undergoing surgery for traumatic cataract, substantial vision loss due to trauma occurred by 15 months after surgery, with a median visual acuity of 20/63 in eyes with pseudophakia; strabismus was diagnosed in 23 cases (cumulative proportion, 43%, with exotropia in 14 [61%]) and glaucoma was diagnosed in 4 cases (cumulative proportion, 6%). In eyes with pseudophakia, visual axis opacification was diagnosed in 3 of 30 (cumulative proportion, 11%) with and 18 of 27 (cumulative proportion, 77%) without primary posterior capsulotomy.Meaning In this study, within 15 months after lensectomy for traumatic cataract in children, substantial ocular morbidity including permanent vision loss was common, suggesting the need for monitoring for development of strabismus and visual axis opacification.Importance The Pediatric Eye Disease Investigator Group Cataract Registry provides a multicenter assessment of visual outcomes and complications after lensectomy for traumatic pediatric cataract.Objective To report visual acuity (VA) and the cumulative proportion with strabismus, glaucoma, and other ocular complications by 15 months after lensectomy for traumatic cataract among children younger than 13 years at the time of surgery.Design, Setting, and Participants From June 18, 2012, to July 8, 2015, 1266 eyes of 994 children from 33 pediatric eye care practices seen within 45 days after lensectomy were enrolled in a multicenter, prospective observational registry. Of these, 74 eyes of 72 participants undergoing lensectomy for traumatic cataract were included in a cohort study. Follow-up was completed by November 2, 2015, and data were analyzed from March 20, 2018, to July 7, 2020.Exposures Lensectomy after ocular trauma.Main Outcomes and Measures Best-corrected VA from 9 to 15 months after lensectomy for traumatic cataract (for those 3 years or older) and the cumulative proportion with strabismus, glaucoma, and other ocular complications by 15 months.Results Of 994 participants in the registry, 84 (8%) had traumatic cataract. The median age at lensectomy for 72 participants examined within 15 months after surgery was 7.3 (range, 0.1-12.6) years; 46 (64%) were boys. An intraocular lens was placed in 57 of 74 eyes (77%). In children 3 years or older at outcome, the median best-corrected VA was 20/250 (range, 20/20 to worse than 20/800) in 6 eyes with aphakia and 20/63 (range, 20/20 to 20/200) in 26 eyes with pseudophakia. Postoperative visual axis opacification was reported in 18 of 27 eyes with pseudophakia without primary posterior capsulotomy (15-month cumulative proportion, 77%; 95% CI, 58%-92%). The cumulative proportion with strabismus was 43% (95% CI, 31%-58%) in 64 participants with ocular alignment data; exotropia was present in 14 of 23 participants (61%). The cumulative proportion with glaucoma was 6% (95% CI, 2%-16%).Conclusions and Relevance Trauma was not a common cause of pediatric cataract requiring surgery. For children with traumatic cataract, substantial ocular morbidity including permanent vision loss was found, and long-term eye and vision monitoring are needed for glaucoma, strabismus, and capsular opacification.

Cataract surgery following penetrating keratoplasty in children

Abstract:Background: Following penetrating keratoplasty, cataract surgery warrants certain modifications to ensure maximum survival of the graft. Purpose: To emphasize the intraoperative challenges and surgical manipulations to be followed. Synopsis: The surgeon makes a superior sclerocorneal tunnel avoiding the graft host junction. Dispersive viscoelastic is used. Continuous curvilinear capsulorhexis is done. Cataractous lens aspirated with a low flow rate. The intraocular lens is placed in the bag. Superior peripheral iridectomy and primary posterior capsulorhexis are done. The wound closed with two interrupted 10-0 nylon sutures. Graft host junction integrity is maintained. Highlights: Ensure 1. Good intraoperative corneal visibility, 2. Avoid graft host junction for main port incision 3. Generous dispersive viscoelastic use/soft shell technique to protect the corneal endothelium, 4. Avoid phaco energy in case of soft cataracts/low phaco energy and flow rates, 5. Phaco probe to be meticulously oriented away from corneal endothelium, 6. Primary posterior capsulorhexis to be done as in any pediatric cataract surgery, 7. Presence of Posterior Staphyloma in Congenital Cataract Children

Abstract:Purpose: To investigate the prevalence of posterior staphyloma (PS) in congenital cataract children and its role in predicting postoperative axial elongation. Materials and Methods: Preoperative prevalence of PS in 520 congenital cataract patients was reviewed and compared with that of the healthy eyes of 300 unilateral traumatic cataract children after 1:1 propensity score matching. Then, 32 pseudophakic children with preoperative PS and 48 age-matched pseudophakic controls without preoperative PS were followed up after the surgery, to compare their axial growth rates and refractive changes. Results: Congenital cataract was significantly associated with the presence of PS (OR: 14.88, P = .009) after propensity score matching. Even in congenital cataract eyes with axial length <26 mm, 5% were identified with PS on B-scan: <= 22 mm: 3%, 22-24 mm: 5% and 24-26 mm: 13%. Eyes with preoperative PS exhibited faster postoperative axial growth than those without, especially in bilateral cases or in children undergoing surgery before 8 years old (<= 4 years: 0.53 +/- 0.33 vs 0.30 +/- 0.21 mm/y P = .028; 4-8 years: 0.37 +/- 0.26 vs 0.23 +/- 0.15 mm/y P = .044). Myopic shift after surgery was also more significant in children with preoperative PS than in those without (-1.10 +/- 0.50 vs -0.60 +/- 0.47D/y, P < .001). Conclusions: Congenital cataract is a risk factor for PS. Preoperative PS in pediatric cataract eyes may be an indicator of excessive postoperative axial elongation, especially in bilateral cases or in cases undergoing cataract surgery at a younger age. Our findings may also promote better clinical decision-making in intraocular lens power selection for pediatric population.

Abstract:PURPOSE:To compare the ocular trauma score (OTS) and the paediatric penetrating ocular trauma score (POTS) as prognostic model for visual outcome in paediatric traumatic cataract cases after penetrating eye injuries.METHODS:All children younger than 16 years with unilateral traumatic cataract following penetrating trauma between 2007 and 2012 were retrospectively reviewed. Univariate chi-square analysis was conducted to identify the variables associated with profound visual loss. The area under the receiver-operating characteristic curves (AUROC) was used to assess the predictive ability of the two models.RESULTS:The study group comprised 65 boys and 37 girls. The variables associated with profound visual loss were: a relative afferent papillary defect (RAPD) (P<0.001), poor initial vision (P=0.01), vitreous haemorrhage (P<0.001), retinal detachment (P<0.001), posterior penetrating site (P<0.001), hyphema (P<0.001), no intraocular len implantation (P<0.001) and endophthalmitis (P=0.001). OTS could not be calculated in 21 patients (20.6%) without clinical data on initial visual acuity and RAPD. For the patients with complete clinical data, POTS was similar to OTS in predicting poor vision (AUROC 0.904 vs 0.924) and in predicting good vision (AUROC 0.766 vs 0.736). For all the samples, POTS was a robust predictor of poor vision (AUROC 0.910) and had a moderate ability to predict good vision (AUROC 0.764).CONCLUSION:OTS has high ability to predict visual outcome for paediatric traumatic cataract following penetrating ocular trauma. POTS is also a reliable prognostic model for very young child without initial vision or RAPD, but is only for penetrating eye injuries.

Abstract:Purpose: To describe the in vivo morphologic characteristics of the trabecular meshwork (TM), Schlemm canal (SC), and iridocorneal angle in pediatric patients with normal eyes, glaucoma, and cataract. Design: Prospective cohort study. Study population: A total of 41 children (70 eyes) were enrolled, comprising 28 normal eyes, 19 eyes with glaucoma, and 26 eyes with cataract (15 pre-, 11 post-lensectomy). Average age was 2.8 ± 3.0 years at imaging. Methods: Pediatric patients undergoing clinically indicated examination under anesthesia underwent overhead-mounted optical coherence tomography (OCT) imaging of the outflow pathway. Images were then evaluated for abnormalities in the outflow pathway. Main outcome measure: Presence of thickened TM, SC patency, any iridocorneal angle malformation, and episcleral vessel patency in normal eyes vs all other eyes. Results: Thickened TM tissue was found in no normal eyes, 47.4% of eyes with glaucoma, and 53.8% of eyes with cataracts. A patent SC was demonstrated in all normal eyes, 32% with glaucoma, and 65.4% with cataracts. Iridocorneal angle abnormality was identified in no normal eyes, 94.7% with glaucoma, and 69.2% with cataracts. Prevalence of iridocorneal angle abnormality was similar in cataract eyes pre- and post-lensectomy. All but one eye with glaucoma secondary to anterior segment dysgenesis had patent episcleral vessels. Intraocular pressure (IOP) at imaging was lower in eyes with than without a patent SC (13.9±3.8 mm Hg vs 28.2±11.7 mm Hg, P < .001). IOP was lower in eyes with normal iridocorneal angles compared to those with angle malformation (13.9±3.2 mm Hg vs 22.6±12.1 mm Hg, P < .001). There was no significant difference in IOP between eyes with normal TM thickness compared to those with thickened TM (17.4±8.2 mm Hg vs 17.4±9.5 mm Hg, p = 0.943). Conclusion: Eyes of children with glaucoma demonstrated thickened TM, decreased patency of SC, and abnormalities of the iridocorneal angle on OCT imaging, compared with normal controls. These abnormal findings also appear in many pediatric eyes with cataracts, even before lensectomy. Future study is warranted to evaluate the role these structural differences may play in the development of glaucoma following cataract surgery and the significant variation in how individual glaucoma eyes respond to angle surgery.
Refractive and visual outcomes after surgery for pediatric traumatic cataract

Abstract:Purpose: To describe refractive and visual outcomes of pediatric traumatic cataract requiring surgery and evaluate the factors influencing success. Setting: Hospital for Sick Children, Toronto, Ontario, Canada. Design: Retrospective case series. Methods: Charts of children having lensectomy for traumatic cataract between January 1, 2000, and June 30, 2015, were reviewed for demographic information, visual and refractive outcomes, complications, and surgical details. Results: One hundred six children (mean age 7.6 years ± 3.9 [SD]) were included. The median follow-up was 41 months (range 3 to 155 months). Seventy-nine children had open-globe injuries and 27 had closed-globe injuries. Patients with open-globe injuries were younger than those with closed-globe injuries (mean age 6.9 versus 10.4 years; P < .05). The final corrected distance visual acuity (CDVA) was 20/40 or better in 47 children. In the 94 children who had intraocular lens placement, 54% with open-globe injuries and 55% with closed-globe injuries achieved a mean absolute prediction error of 1.0 diopter or less in the early postoperative period. Open-globe injuries and amblyopia were associated with worse visual outcomes (odds ratio [OR], 2.8 and P = .03 versus OR, 2.4 and P = .04) and refractive outcomes (OR, 3.1 and P = .02 versus OR, 3.8 and P = .04). Age younger than 5 years was associated with worse refractive outcomes (OR, 2.88; P = .02). Conclusions: Children requiring surgery for traumatic cataract can have good visual and refractive outcomes. Those with open-globe and those with closed-globe injuries both had good early postoperative refractive accuracy. Sixty-three percent of children with closed-globe injuries attained a CDVA of 20/40 or better at the final follow-up.

Abstract:Cataract is the leading cause of childhood blindness in developing countries. Early detection and treatment of childhood cataracts can reduce the burden of blindness in the nation. Often the etiology of pediatric cataract is idiopathic; however, genetics play a role in the development of congenital cataract. According to epidemiologists, one-fourth of cases of congenital cataracts are hereditary. Gene responsible for the development of cataract is identified using gene mapping, which helps to prevent future blindness in the family. Cataracts can also present with systemic disease, microphthalmia, microcornea, and aniridia. The presentation of cataracts varies in individuals, some are symptomatic while others are asymptomatic. Parents after noticing strabismus and leukocoria bring their children to an ophthalmologist. Early diagnosis can restore visual function in cases of congenital cataract. In young babies, the type of cataract is determined using slit-lamp examination and examination under anesthesia in OR. Most cases of pediatric cataracts are accidental findings during routine checkups. On direct ophthalmoscopy, red reflex is not appreciated in cases of cataracts. Advancing technology changes the ophthalmologist's approach to pediatric cataract surgery, improving postoperative refractory function. In children, minor incision surgery was preferred to heal early. An appropriate choice of intraocular lens (IOL) should be made for implantation in a child's eye to avoid postimplanted complications. Inflammation and amblyopia affect the outcome of treatment. Complications of cataract surgery include posterior capsule opacification, glaucoma, inflammation, and uveitis.

Abstract:Objective To evaluate the safety and efficacy of a 23-gauge transconjunctival sutureless vitrectomy system for the management of traumatic cataract in children and to determine the short-term outcome and complications.Methods This retrospective study involved a consecutive series of 32 children (32 eyes) who suffered from traumatic cataract from March 2009 to September 2011.Each eye underwent anterior capsulotomy,lentectomy,posterior capsulotomy,and anterior vitrectomy through a superior limbal incision using a 23-gauge vitrectomy cutter.A limbal port incision was used to introduce an infusion micro cannula to maintain the anterior chamber with balance salt solution.The superior limbal incision was enlarged to 2.75 mm by a diamond knife.An intraocular lens (IOL) was implanted in the bag or sulcus.The enlarged incision was closed with one or two 10-0 nylon suture.Refraction correction with spectacles and amblyopic training began 1 week postoperatively.The mean follow-up period was 17.1±8.2 months.Results All the surgical procedures were successfully performed in all 32 eyes.No intraoperative complication was noted.The mean postoperative intraocular pressure (IOP) was 12.9±2.4 mmHg.Postoperative inflammation was mild.No fibrin reactions,vitreous escape or incarceration were noted in the anterior chamber.Neither significant aqueous flare nor aqueous cell was observed 1 week after surgery.Five eyes (15.6％) had mild posterior synechiae.All eyes had primary IOL implantation.IOL was implanted in the bag in 22 eyes (68.7％) and in the sulcus in 10 eyes (31.3％).The mean axial length of the 32 eyes was 22.36±1.03 mm at the time of operation and the mean IOL power was +22.97±1.89 D.Four eyes (12.5％) developed visual axis opacification (VAO) and required capsulotomy by YAG laser.IOL pigmentation was noted in 5 eyes (15.6％).No cases of ocular hypertension,glaucoma,choroidal detachment or retinal detachment were observed.The mean logMAR best corrected visual acuity (BCVA) was 2.37±1.16 preoperatively and 0.39±0.48 at the last follow-up,BCVA was significantly improved (t=10.916,P＜0.01).BCVA was improved in 29 eyes postoperatively.The predictors of good visual outcomes (BCVA≥20/40) included metal as the object causing injury,cataract operation within 3 month after injury and good amblyopic treatment (Fisher=O.028,0.047,0.000,respectively.P＜0.05).Conclusion Management of traumatic cataract in children via an anterior approach using the 23-gauge transconjunctival sutureless vitrectomy system appears safe and effective.Advantages include a more precise capsulotomy and more sufficient lensectomy and anterior vitrectomy,stable intraoperative IOP,and reduced surgical trauma and inflammation.
Audit of 1000 consecutive cases of sutureless cataract surgery in children above two years of age

Abstract:Purpose: To study the safety of sutureless cataract surgery and risk factors for wound leak of clear corneal incision in children affected with congenital or developmental cataract. Methods: It is a retrospective, noncomparative interventional case study involving children in the age group of 2 to 16 years, who underwent cataract surgery with intraocular lens implantation with the minimum follow up of 1 month. Results: Out of 1000 eyes studied, lens matter aspiration with intraocular lens implantation with or without primary posterior capsulorhexis and anterior vitrectomy was done in 609 and 391 eyes, respectively. Incisions of 943 eyes did not require sutures while sutures were applied for wound leak in 57 eyes on the table and in 5 eyes on postoperative day 1. Risk of wound leak for suture application was found to be greater in patients; having age less than 5 years (P < 0.0001), surgeries performed by junior surgeons (P < 0.0003), wound problems (P < 0.0001), intraocular lens (IOL) related factors (P < 0.0001), use of iris hooks (P < 0.0001), and anterior capsulorhexis extension (P < 0.0001). On the first postoperative day, anterior chamber reaction (P < 0.0001) and fibrinous membrane (P = 0.0007) were significantly more in the sutured group. Incidence of postoperative complications was 0.98 per sutured eyes (59 complications in 60 eyes). One patient developed endophthalmitis after suture removal. Conclusion: Sutureless clear corneal incision in pediatric patients undergoing cataract surgery can achieve stable wound. However, after hydro closure, every wound should be checked for water tightness and the leaky wound should be sutured.
Primary Posterior Capsulorhexis with Anterior Vitrectomy in Preventing Posterior Capsule Opacification in Pediatric Cataract Microsurgery

Abstract:Our study was performed to investigate the operative effects of pediatric cataract surgery with anterior vitrectomy. The study comprised of 38 pediatric patients aged 2–5 years and involved a total of 60 eyes with congenital cataracts. The study was done between March 2004 and December 2005. Posterior capsulorhexis with anterior vitrectomy was performed on 34 eyes of 20 children, and cataract extraction was performed on 26 eyes of 18 children. The percentages of various complications and rate of neodymium YAG (Nd:YAG) laser capsulotomy were followed up for 12–36 months. There were significant differences in the percentages of posterior capsule opacification (PCO) and rate of neodymium YAG (Nd:YAG) capsulotomy between the anterior vitrectomy group and cataract extraction group ( P < 0.01). The anterior vitrectomy group was associated with less posterior capsular opacification (11.8%) than the cataract extraction group (76.9%). The YAG capsulotomy rate was 2.9% for the anterior vitrectomy group, and 57.7% for the cataract extraction group. After operation the fibrous exudates in anterior chamber and the intraocular lens subluxation or capture were 5.9% and 2.9% for the anterior vitrectomy group while for the cataract extraction group they were 23.1% and 15.4%; the difference was statistically significant ( P < 0.05). The results suggest that primary posterior capsulorhexis with anterior vitrectomy in pediatric cataract surgery who are between 2 and 5 years old is necessary and effective procedure with low PCO rate. © 2008 Wiley‐Liss, Inc. Capsular Outcomes after Pediatric Cataract Surgery Without Intraocular Lens Implantation

Abstract:The objective of this study was to investigate capsular outcomes 12 months after pediatric cataract surgery without intraocular lens implantation via qualitative classification and quantitative measurement. This study is a cross-sectional study that was approved by the institutional review board of Zhongshan Ophthalmic Center of Sun Yat-sen University in Guangzhou, China. Digital coaxial retro-illumination photographs of 329 aphakic pediatric eyes were obtained 12 months after pediatric cataract surgery without intraocular lens implantation. Capsule digital coaxial retro-illumination photographs were divided as follows: anterior capsule opening area (ACOA), posterior capsule opening area (PCOA), and posterior capsule opening opacity (PCOO). Capsular outcomes were qualitatively classified into 3 types based on the PCOO: Type I—capsule with mild opacification but no invasion into the capsule opening; Type II—capsule with moderate opacification accompanied by contraction of the ACOA and invasion to the occluding part of the PCOA; and Type III—capsule with severe opacification accompanied by total occlusion of the PCOA. Software was developed to quantitatively measure the ACOA, PCOA, and PCOO using standardized DCRPs. The relationships between the accurate intraoperative anterior and posterior capsulorhexis sizes and the qualitative capsular types were statistically analyzed. The DCRPs of 315 aphakic eyes (95.8%) of 191 children were included. Capsular outcomes were classified into 3 types: Type I—120 eyes (38.1%); Type II—157 eyes (49.8%); Type III—38 eyes (12.1%). The scores of the capsular outcomes were negatively correlated with intraoperative anterior capsulorhexis size (R = −0.572, P < 0.001), but no significant correlation with intraoperative posterior capsulorhexis size (R = −0.16, P = 0.122) was observed. The ACOA significantly decreased from Type I to Type II to Type III, the PCOA increased in size from Type I to Type II, and the PCOO increased from Type II to Type III (all P < 0.05). Capsular outcomes after pediatric cataract surgery can be qualitatively classified and quantitatively measured by acquisition, division, definition, and user-friendly software analyses of high-quality digital coaxial retro-illumination photographs.
