Abstract:An Implantable Cardioverter Defibrillator (ICD) is a medical device used for the detection of potentially fatal cardiac arrhythmia and their treatment through the delivery of electrical shocks intended to restore normal heart rhythm. An ICD reprogramming attack seeks to alter the device's parameters to induce unnecessary shocks and, even more egregious, prevent required therapy. In this paper, we present a formal approach for the synthesis of ICD reprogramming attacks that are both effective, i.e., lead to fundamental changes in the required therapy, and stealthy, i.e., involve minimal changes to the nominal ICD parameters. We focus on the discrimination algorithm underlying Boston Scientific devices (one of the principal ICD manufacturers) and formulate the synthesis problem as one of multi-objective optimization. Our solution technique is based on an Optimization Modulo Theories encoding of the problem and allows us to derive device parameters that are optimal with respect to the effectiveness-stealthiness tradeoff (i.e., lie along the corresponding Pareto front). To the best of our knowledge, our work is the first to derive systematic ICD reprogramming attacks designed to maximize therapy disruption while minimizing detection. To evaluate our technique, we employ an extensive dataset of synthetic EGMs (cardiac signals), each generated with a prescribed arrhythmia, allowing us to synthesize attacks tailored to the victim's cardiac condition. Our approach readily generalizes to unseen signals, representing the unknown EGM of the victim patient.

Abstract:Modern cyber-physical systems (CPSs) use the Internet of Things (IoT) to collect and exchange data efficiently, monitor device/sensor level interaction effectively, and adopt new standards effortlessly. Machine learning (ML) models are growingly used in the controllers of these IoT-enabled CPSs for pattern identification, state estimation, prediction, and anomaly detection. However, sophisticated adversaries can launch various attacks on the communication network and the hardware/firmware to introduce corrupted sensor measurements to manipulate the ML-based CPSs and create critical physical hazards. Hence, analyzing the threat space of a CPS is essential to understand the system's strength and identify the most vital resources to protect. However, existing studies have not proposed any verifiable solution for the threat analysis of ML-based CPSs. This paper presents a novel framework that uses efficient mechanisms to extract constraints from ML-based decision models and perform a formal threat analysis to identify potential false data injection (FDI) attack paths and corresponding effects on an IoT-enabled ML-based CPS. Our framework can provide us with all possible attack vectors, each representing a set of sensor measurements to be altered for a CPS given a specific set of attack attributes. The attack vectors enable us to assess the system's resiliency, thus providing insight to enhance the system's robustness. We consider an internet of medical things-enabled safety-critical CPS naming smart healthcare system (SHS) as the reference case. We validate our framework on a real SHS dataset, proving our framework's success in revealing feasible FDI attack paths. Our evaluation using synthetic and two real SHS datasets also affirms the tool's efficacy in the threat analysis of ML-based CPSs.

Abstract:The increasing availability of healthcare data requires accurate analysis of disease diagnosis, progression, and realtime monitoring to provide improved treatments to the patients. In this context, Machine Learning (ML) models are used to extract valuable features and insights from high-dimensional and heterogeneous healthcare data to detect different diseases and patient activities in a Smart Healthcare System (SHS). However, recent researches show that ML models used in different application domains are vulnerable to adversarial attacks. In this paper, we introduce a new type of adversarial attacks to exploit the ML classifiers used in a SHS. We consider an adversary who has partial knowledge of data distribution, SHS model, and ML algorithm to perform both targeted and untargeted attacks. Employing these adversarial capabilities, we manipulate medical device readings to alter patient status (disease-affected, normal condition, activities, etc.) in the outcome of the SHS. Our attack utilizes five different adversarial ML algorithms (HopSkipJump, Fast Gradient Method, Crafting Decision Tree, Carlini & Wagner, Zeroth Order Optimization) to perform different malicious activities (e.g., data poisoning, misclassify outputs, etc.) on a SHS. Moreover, based on the training and testing phase capabilities of an adversary, we perform white box and black box attacks on a SHS. We evaluate the performance of our work in different SHS settings and medical devices. Our extensive evaluation shows that our proposed adversarial attack can significantly degrade the performance of a ML-based SHS in detecting diseases and normal activities of the patients correctly, which eventually leads to erroneous treatment.

Systematically Assessing the Security Risks of AI/ML-enabled Connected Healthcare Systems

Abstract:The adoption of machine-learning-enabled systems in the healthcare domain is on the rise. While the use of ML in healthcare has several benefits, it also expands the threat surface of medical systems. We show that the use of ML in medical systems, particularly connected systems that involve interfacing the ML engine with multiple peripheral devices, has security risks that might cause life-threatening damage to a patient's health in case of adversarial interventions. These new risks arise due to security vulnerabilities in the peripheral devices and communication channels. We present a case study where we demonstrate an attack on an ML-enabled blood glucose monitoring system by introducing adversarial data points during inference. We show that an adversary can achieve this by exploiting a known vulnerability in the Bluetooth communication channel connecting the glucose meter with the ML-enabled app. We further show that state-of-the-art risk assessment techniques are not adequate for identifying and assessing these new risks. Our study highlights the need for novel risk analysis methods for analyzing the security of AI-enabled connected health devices.

Abstract:A Cardiac Implantable Medical device (IMD) is a device, which is surgically implanted into a patient's body, and wirelessly configured using an external programmer by prescribing physicians and doctors. A set of lethal attacks targeting these devices can be conducted due to the use of vulnerable wireless communication and security protocols, and the lack of security protection mechanisms deployed on IMDs. In this paper, we propose a system for postmortem analysis of lethal attack scenarios targeting cardiac IMDs. Such a system reconciles in the same framework conclusions derived by technical investigators and deductions generated by pathologists. An inference system integrating a library of medical rules is used to automatically infer potential medical scenarios that could have led to the death of a patient. A Model Checking based formal technique allowing the reconstruction of potential technical attack scenarios on the IMD, starting from the collected evidence, is also proposed. A correlation between the results obtained by the two techniques allows to prove whether a potential attack scenario is the source of the patient's death.

Security Defense Strategy for Intelligent Medical Diagnosis Systems (IMDS)

Abstract:Aims: The Intelligence Medical Diagnosis System (IMDS) has been targeted by the cyber terrorists, who aim to destroy the Critical National Infrastructure (CNI). This paper is motivated by the most recent incidents happened worldwide and have resulted in the compromise of diagnosis results. This study was undertaken to show how the IMDS could be attacked and diagnosis results compromised and present a set of cyber defense strategies to prevent against such attacks. Methods and results: This study used the ECGs data from the PhysioNet/Computing in Cardiology (CinC) Challenge 2017. We fed the data into our IMDS and launched a series of ethical hacking, which is specifically tailored to target IMDS. We proposed a set of cyber security strategies to prevent such compromise. We tested the effectiveness of our cyber defense strategies using an experiment. The results showed that the strategies were effective in protecting the IMDS diagnosis results from being compromised. Conclusions: This study provides novel insights into the protection of IMDS and concludes that our cyber defense strategies can protect IMDS from being compromised by Brute Force and SQL Injection attacks.

Abstract:Cardiovascular diseases remain one of the main threats to human health, significantly affecting the quality and life expectancy. Effective and prompt recognition of these diseases is crucial. This research aims to develop an effective novel hybrid method for automatically detecting dangerous arrhythmias based on cardiac patients' short electrocardiogram (ECG) fragments. This study suggests using a continuous wavelet transform (CWT) to convert ECG signals into images (scalograms) and examining the task of categorizing short 2-s segments of ECG signals into four groups of dangerous arrhythmias that are shockable, including ventricular flutter (C1), ventricular fibrillation (C2), ventricular tachycardia torsade de pointes (C3), and high-rate ventricular tachycardia (C4). We propose developing a novel hybrid neural network with a deep learning architecture to classify dangerous arrhythmias. This work utilizes actual electrocardiogram (ECG) data obtained from the PhysioNet database, alongside artificially generated ECG data produced by the Synthetic Minority Over-sampling Technique (SMOTE) approach, to address the issue of imbalanced class distribution for obtaining an accuracy-trained model. Experimental results demonstrate that the proposed approach achieves high accuracy, sensitivity, specificity, precision, and an F1-score of 97.75%, 97.75%, 99.25%, 97.75%, and 97.75%, respectively, in classifying all the four shockable classes of arrhythmias and are superior to traditional methods. Our work possesses significant clinical value in real-life scenarios since it has the potential to significantly enhance the diagnosis and treatment of life-threatening arrhythmias in individuals with cardiac disease. Furthermore, our model also has demonstrated adaptability and generality for two other datasets.

Abstract:Background Remote monitoring (RM) has become the standard of care for follow up of patients with cardiac implantable electronic devices (CIED). Data deluge from RM of CIEDs is one of the major challenges faced by the cardiac device clinics. This study explores the role of a novel artificial intelligence (AI) algorithm in reducing the data deluge from CIED. Methods: The study included remote transmissions from CIEDs being monitored at 78 clinics across USA from Jan 2023 to April 2023. De-identified data was obtained from the data repository of Octagos Health. The transmissions were initially processed by AI algorithm and were either dismissed, forwarded to the device technician (DT) for review or forwarded directly to the device clinics. The main reasons for dismissals were redundant data or clinically non-actionable and non billable information. After initial triage by AI algorithm, the DT served as the second checkpoint who either dismissed or forwarded the transmissions to the device clinic with and without alerts. Results: A total of 42,534 patients with CIEDs were included in this study. There were 19,895 (46.8%) patients with pacemakers, 8,858 (20.8%) with ILRs, 7,012 (16.5%) with ICDs, 5,028 (11.8%) with CRT-D and 1,741 (4.1%) with CRT-P. Over a period of 4 months of RM, 273,435 transmissions were received. Of these, 57,345 (21%) transmissions were dismissed by AI and 118,546 (43.4%) were dismissed by the device technician team. As such only 97,544 (35.7%) transmissions were sent to the clinicians, of which 37,627 (13.8%) had alerts while 59,917 (21.9%) were routine transmissions. For quality control, about 15% (11,324) transmissions were randomly verified by EP for accuracy and were found to be 99% (11,193/11,324) accurate. Conclusion In our study, the use of AI algorithm significantly decreased data deluge by appropriate triaging of RM data from CIEDs. This has a potential to streamline the workflow of cardiac device clinics and improving efficiency and patient care.

Personalized Deep Learning for Ventricular Arrhythmias Detection on Medical IoT Systems

Abstract:Life-threatening ventricular arrhythmias (VA) are the leading cause of sudden cardiac death (SCD), which is the most significant cause of natural death in the US. The implantable cardioverter defibrillator (ICD) is a small device implanted to patients under high risk of SCD as a preventive treatment. The ICD continuously monitors the intracardiac rhythm and delivers shock when detecting the life-threatening VA. Traditional methods detect VA by setting criteria on the detected rhythm. However, those methods suffer from a high inappropriate shock rate and require a regular follow-up to optimize criteria parameters for each ICD recipient. To ameliorate the challenges, we propose the personalized computing framework for deep learning based VA detection on medical IoT systems. The system consists of intracardiac and surface rhythm monitors, and the cloud platform for data uploading, diagnosis, and CNN model personalization. We equip the system with real-time inference on both intracardiac and surface rhythm monitors. To improve the detection accuracy, we enable the monitors to detect VA collaboratively by proposing the cooperative inference. We also introduce the CNN personalization for each patient based on the computing framework to tackle the unlabeled and limited rhythm data problem. When compared with the traditional detection algorithm, the proposed method achieves comparable accuracy on VA rhythm detection and 6.6% reduction in inappropriate shock rate, while the average inference latency is kept at 71ms.

Abstract:An electrocardiogram (ECG) is an essential piece of medical equipment that helps diagnose various heart-related conditions in patients. An automated diagnostic tool is required to detect significant episodes in long-term ECG records. It is a very challenging task for cardiologists to analyze long-term ECG records in a short time. Therefore, a computer-based diagnosis tool is required to identify crucial episodes. Myocardial infarction (MI) and conduction disorders (CDs), sometimes known as heart blocks, are medical diseases that occur when a coronary artery becomes fully or suddenly stopped or when blood flow in these arteries slows dramatically. As a result, several researchers have utilized deep learning methods for MI and CD detection. However, there are one or more of the following challenges when using deep learning algorithms: (i) struggles with real-life data, (ii) the time after the training phase also requires high processing power, (iii) they are very computationally expensive, requiring large amounts of memory and computational resources, and it is not easy to transfer them to other problems, (iv) they are hard to describe and are not completely understood (black box), and (v) most of the literature is based on the MIT-BIH or PTB databases, which do not cover most of the crucial arrhythmias. This paper proposes a new deep learning approach based on machine learning for detecting MI and CDs using large PTB-XL ECG data. First, all challenging issues of these heart signals have been considered, as the signal data are from different datasets and the data are filtered. After that, the MI and CD signals are fed to the deep learning model to extract the deep features. In addition, a new custom activation function is proposed, which has fast convergence to the regular activation functions. Later, these features are fed to an external classifier, such as a support vector machine (SVM), for detection. The efficiency of the proposed method is demonstrated by the experimental findings, which show that it improves satisfactorily with an overall accuracy of 99.20% when using a CNN for extracting the features with an SVM classifier.

Abstract:Early automatic detection of cardiac arrhythmia in an ambulatory health monitoring setup can reduce the mortality rates. In the existing approaches on multiclass arrhythmia detection (MCAD) techniques, adopting either time-domain morphological features (TDMFs), fails to extract hidden pattern from the single-lead electrocardiogram (ECG) resulting in poor accuracy (Ac), or adopting deep-learning (DL) techniques becomes too complex or heavy in architecture, thus hindering their easy implementation in low resource architecture for wearable applications. This work presents a judicious hybridization of both approaches in an end-to-end MCAD using two-stage classifier to achieve higher Ac and low latency while detecting 15 classes of cardiac rhythms [14 types of arrhythmia beats or normal sinus rhythms (NSRs)]. The memory resource saving is achieved through the beat skeletonization process and two-stage feature minimization. The stage#1 classifier uses 14 TDMF features fed to a random forest (RF) regressor and provides probability values for NSR, atrial, or ventricular conduction problem. The stage#2 classifier, another RF, accepts stacked autoencoder (SAE) features, TDMF features, and output of stage#1 to achieve a blind test Ac of 99.21% using only 13 features while evaluated with full Massachusetts Institute of Technology-Beth Israel (MIT-BIH) arrhythmia database (mitdb) records. A stand-alone on-board application of the two-stage MCAD using ARMv6 single-core microcontroller yields a detection latency of 782 ms and consumes 390 kB of memory per cardiac cycle, thus proving a feasible solution for wearable cardiac monitors. It also provides the competitive performance with the State of the art (SOTA) methods and outperforms the limited available research on hardware implementation in terms of number of classes handled and Ac.

Abstract:Wearable and implantable medical devices (IMDs) are increasingly deployed to diagnose, monitor, and provide therapy for critical medical conditions. Such medical devices are safety-critical cyber-physical systems (CPSs). These systems support wireless features introducing potential security vulnerabilities. Although these devices undergo rigorous safety certification processes, runtime security attacks are inevitable. Based on published literature, IMDs such as pacemakers and insulin infusion systems can be remotely controlled to inject deadly electric shocks and excess insulin, posing a threat to a patient’s life. While prior works based on formal methods have been proposed to detect potential attack vectors using different forms of static analysis, these have limitations in preventing attacks at runtime. This paper discusses a formal framework for detecting cyber-physical attacks on a pacemaker by monitoring its security policies at runtime. We propose a wearable device that senses the Electrocardiogram (ECG) and Photoplethysmogram (PPG) of the body to detect attacks in a pacemaker. To facilitate the design of this device, we map the security policies of a pacemaker w.r.t ECG and PPG, paving the way for designing formal verification monitors for pacemakers for the first time using multiple physiological signals. The proposed monitoring framework allows the synthesis of parallel monitors from a given set of desired security policies, where all the monitors execute concurrently and generate an alarm to the user in the case of policy violation. Our implementation and the performance evaluation results demonstrate the technical feasibility of designing such a wearable device for attack detection in pacemakers. This device is separate from the pacemaker, ensuring no need for re-certification of pacemakers. Our approach is amenable to the application of security patches when new attack vectors are detected, making the approach ideal for runtime monitoring of medical CPSs.

Real-World Data Analysis of Implantable Cardioverter Defibrillator (ICD) in Patients with Hypertrophic Cardiomyopathy (HCM)

Abstract:Background: One of the common causes of sudden cardiac death (SCD) in young people is hypertrophic cardiomyopathy (HCM) and the primary prevention of SCD is with an implantable cardioverter defibrillators (ICD). Concerning the incidence of appropriate ICD therapy and the complications associated with ICD implantation and discharge, patients with implanted ICDs are closely monitored and interrogation reports are generated from clinical consultations. Methods: In this study, we compared the performance of structured device data and unstructured interrogation reports for extracting information of ICD therapy and heart rhythm. We sampled 687 reports with a gold standard generated through manual chart review. A rule-based natural language processing (NLP) system was developed using 480 reports and the information in the corresponding device data was aggregated for the task. We compared the performance of the NLP system with information aggregated from structured device data using the remaining 207 reports. Results: The rule-based NLP system achieved F-measure of 0.92 and 0.98 for ICD therapy and heart rhythm while the performance of aggregating device data was significantly lower with F-measure of 0.78 and 0.45 respectively. Limitations of using only structured device data include no differentiation of real events from management events, data availability, and disparate perspectives of vendor and data granularity while using interrogation reports needs to overcome non-representative keyword/pattern and contextual errors. Conclusions: Extracting phenotyping information from data generated in real-world requires the incorporation of medical knowledge. It is essential to analyze, compare, and harmonize multiple data sources for real-world evidence generation.

Abstract:Automatic cardiac arrhythmia detection (CAD) from electrocardiogram (ECG) is a challenging topic for biomedical research community. In recent times, machine learning (ML) and deep learning (DL) models have been widely adopted to achieve good prediction accuracy. However, the primary obstacles toward their standalone implementation often have high complexity and memory requirement, preventing low resource embedding. The other one is low prediction accuracy toward minority classes. This work presents a new data augmentation model, a combination of borderline majority class undersampling (BMU) and chaos-based minority class oversampling (CMO). In BMU, the undersampling was done with priority to the majority class instances which are far away from the boundary. In CMO, the beat synthesis was done by adaptive chaos addition to the parent beats of minority classes so that they remain clinically acceptable. A simple classification flow using an autoencoder with random forest (RF) was used with the synthesized data for multiclass CAD. For performance evaluation three benchmark datasets, viz., MITDB, INCART, and SVDB datasets were used. It was found that the score of the minority classes have increased, with reduction in false positive and negative rates as compared to few published works employing complex architectures. Also, the simple CAD model could achieve equally good accuracy with much lower (70%) latency, and lower (90%) memory requirement than few reported works using MITDB dataset in a standalone hardware implementation (ARM v6 controller). This research shows great promise toward developing low-complexity wearable CAD monitors.

This paper presents a Public Key Infrastructure (PKI) secured IoT enabled framework entitled Cardiac Diagnostic Feature and Demographic Identification (CDF-DI) systems with significant Models that recognize several Cardiac disease features related to HF. To achieve this goal, we used statistical and machine learning techniques to analyze the Cardiac secondary dataset. The Elevated Serum Creatinine (SC) levels and Serum Sodium (SS) could cause renal problems and are well established in HF patients. The Mann Whitney U test found that SC and SS levels affected the survival status of patients (p < 0.05). Anemia, diabetes, and BP features had no significant impact on the SS and SC level in the patient (p > 0.05). The Cox regression model also found a significant association of age group with the survival status using follow-up months. Furthermore, the present study also proposed important features of Cardiac disease that identified the patient's survival status, age group, and gender. The most prominent algorithm was the Random Forest (RF) suggesting five key features to determine the survival status of the patient with an accuracy of 96%: Follow-up months, SC, Ejection Fraction (EF), Creatinine Phosphokinase (CPK), and platelets. Additionally, the RF selected five prominent features (smoking habits, CPK, platelets, follow-up month, and SC) in recognition of gender with an accuracy of 94%. Moreover, the five vital features such as CPK, SC, follow-up month, platelets, and EF were found to be significant predictors for the patient's age group with an accuracy of 96%. The Kaplan Meier plot revealed that mortality was high in the extremely old age group (χ2 (1) = 8.565). The recommended features have possible effects on clinical practice and would be supportive aid to the existing medical support system to identify the possibility of the survival status of the heart patient. The doctor should primarily concentrate on the follow-up month, SC, EF, CPK, and platelet count for the patient's survival in the situation.

Abstract:Objective. Prompt identification and recognization of myocardial ischemia/infarction (MI) is the most important goal in the management of acute coronary syndrome. The 12-lead electrocardiogram (ECG) is widely used as the initial screening tool for patients with chest pain but its diagnostic accuracy remains limited. There is early evidence that machine learning (ML) algorithms applied to ECG waveforms can improve performance. Most studies are designed to classify MI from healthy controls and thus are limited due to the lack of consideration of ECG abnormalities from other cardiac conditions, leading to false positives. Moreover, clinical information beyond ECG has not yet been well leveraged in existing ML models.Approach.The present study considered downstream clinical implementation scenarios in the initial model design by dichotomizing study recordings from a public large-scale ECG dataset into a MI class and a non-MI class with the inclusion of MI-confounding conditions. Two experiments were conducted to systematically investigate the impact of two important factors entrained in the modeling process, including the duration of ECG, and the value of multimodal information for model training. A novel multimodal deep learning architecture was proposed to learn joint features from both ECG and patient demographics.Main results.The multimodal model achieved better performance than the ECG-only model, with a mean area under the receiver operating characteristic curve of 92.1% and a mean accuracy of 87.4%, which is on par with existing studies despite the increased task difficulty due to the new class definition. By investigation of model explainability, it revealed the contribution of patient information in model performance and clinical concordance of the model's attention with existing clinical insights.Significance.The findings in this study help guide the development of ML solutions for prompt MI detection and move the models one step closer to real-world clinical applications.
Multimodal explainable artificial intelligence identifies patients with non-ischaemic cardiomyopathy at risk of lethal ventricular arrhythmias

Abstract:The efficacy of an implantable cardioverter-defibrillator (ICD) in patients with a non-ischaemic cardiomyopathy for primary prevention of sudden cardiac death is increasingly debated. We developed a multimodal deep learning model for arrhythmic risk prediction that integrated late gadolinium enhanced (LGE) cardiac magnetic resonance imaging (MRI), electrocardiography (ECG) and clinical data. Short-axis LGE-MRI scans and 12-lead ECGs were retrospectively collected from a cohort of 289 patients prior to ICD implantation, across two tertiary hospitals. A residual variational autoencoder was developed to extract physiological features from LGE-MRI and ECG, and used as inputs for a machine learning model (DEEP RISK) to predict malignant ventricular arrhythmia onset. In the validation cohort, the multimodal DEEP RISK model predicted malignant ventricular arrhythmias with an area under the receiver operating characteristic curve (AUROC) of 0.84 (95% confidence interval (CI) 0.71-0.96), a sensitivity of 0.98 (95% CI 0.75-1.00) and a specificity of 0.73 (95% CI 0.58-0.97). The models trained on individual modalities exhibited lower AUROC values compared to DEEP RISK [MRI branch: 0.80 (95% CI 0.65-0.94), ECG branch: 0.54 (95% CI 0.26-0.82), Clinical branch: 0.64 (95% CI 0.39-0.87)]. These results suggest that a multimodal model achieves high prognostic accuracy in predicting ventricular arrhythmias in a cohort of patients with non-ischaemic systolic heart failure, using data collected prior to ICD implantation.

Abstract:Implantable medical devices, such as cardiac pacemakers, must be designed and programmed to the highest levels of safety and reliability. Recently, errors in embedded software have led to a substantial increase in safety alerts, costly device recalls or even patient death. To address such issues, we propose a model-based framework for quantitative, automated verification of pacemaker software. We adapt the electrocardiogram model of Clifford et al, which generates realistic normal and abnormal heart beat behaviours, with probabilistic transitions between them, to produce a timed sequence of action potential signals that serve as pacemaker input. Working with the timed automata model of the pacemaker by Jiang et al, we develop a methodology for deriving the composition of the heart and the pacemaker, based on discretisation. The main correctness properties we consider include checking that the pacemaker corrects Bradycardia (slow heart beat) and does not induce Tachycardia (fast heart beat), for a range of realistic heart behaviours. We also analyse under sensing, through considering noise on sensor readings, and energy usage. We implement the framework using the probabilistic model checker PRISM and MATLAB and demonstrate encouraging experimental results. Our approach can be adapted to individual patients and is applicable to other pacemaker models.
Medical MLLM is Vulnerable: Cross-Modality Jailbreak and Mismatched Attacks on Medical Multimodal Large Language Models

Abstract:Security concerns related to Large Language Models (LLMs) have been extensively explored, yet the safety implications for Multimodal Large Language Models (MLLMs), particularly in medical contexts (MedMLLMs), remain insufficiently studied. This paper delves into the underexplored security vulnerabilities of MedMLLMs, especially when deployed in clinical environments where the accuracy and relevance of question-and-answer interactions are critically tested against complex medical challenges. By combining existing clinical medical data with atypical natural phenomena, we define the mismatched malicious attack (2M-attack) and introduce its optimized version, known as the optimized mismatched malicious attack (O2M-attack or 2M-optimization). Using the voluminous 3MAD dataset that we construct, which covers a wide range of medical image modalities and harmful medical scenarios, we conduct a comprehensive analysis and propose the MCM optimization method, which significantly enhances the attack success rate on MedMLLMs. Evaluations with this dataset and attack methods, including white-box attacks on LLaVA-Med and transfer attacks (black-box) on four other SOTA models, indicate that even MedMLLMs designed with enhanced security features remain vulnerable to security breaches. Our work underscores the urgent need for a concerted effort to implement robust security measures and enhance the safety and efficacy of open-source MedMLLMs, particularly given the potential severity of jailbreak attacks and other malicious or clinically significant exploits in medical settings.
