Abstract:Background Colleges and universities across the country are struggling to develop strategies for effective control of COVID-19 transmission as students return to campus. Methods and findings We conducted a prospective cohort study with students living on or near the UC Berkeley campus from June 1st through August 18th, 2020 with the goal of providing guidance for campus reopening in the safest possible manner. In this cohort, we piloted an alternative testing model to provide access to low-barrier, high-touch testing and augment student-driven testing with data-driven adaptive surveillance that targets higher-risk students and triggers testing notifications based on reported symptoms, exposures, or other relevant information. A total of 2,180 students enrolled in the study, 51% of them undergraduates. Overall, 6,247 PCR tests were administered to 2,178 students over the two-month period. Overall test positivity rate was 0.9%; 2.6% of students tested positive. Uptake and acceptability of regular symptom and exposure surveys was high; 98% of students completed at least one survey, and average completion rate was 67% (Median: 74%, IQR: 39%) for daily reporting of symptoms and 68% (Median: 75%, IQR: 40%) for weekly reporting of exposures. Of symptom-triggered tests, 5% were PCR-positive; of exposure-triggered tests, 10% were PCR-positive. The integrated study database augmented traditional contact tracing during an outbreak; 17 potentially exposed students were contacted the following day and sent testing notifications. At study end, 81% of students selected their desire “to contribute to UC Berkeley’s response to COVID-19” as a reason for their participation in the Safe Campus study. Conclusions Our results illustrate the synergy created by bringing together a student-friendly, harm reduction approach to COVID-19 testing with an integrated data system and analytics. We recommend the use of a confidential, consequence-free, incentive-based daily symptom and exposure reporting system, coupled with low-barrier, easy access, no stigma testing. Testing should be implemented alongside a system that integrates multiple data sources to effectively trigger testing notifications to those at higher risk of infection and encourages students to come in for low-barrier testing when needed.

Abstract:Objective: A small percentage of universities and colleges conducted mass SARS-CoV-2 testing. However, universal testing is resource-intensive, strains national testing capacity, and false negative tests can encourage unsafe behaviors. Participants: A large urban university campus. Methods: Virus control centered on three pillars: mitigation, containment, and communication, with testing of symptomatic and a random subset of asymptomatic students. Results: Random surveillance testing demonstrated a prevalence among asymptomatic students of 0.4% throughout the term. There were two surges in cases that were contained by enhanced mitigation and communication combined with targeted testing. Cumulative cases totaled 445 for the term, most resulting from unsafe undergraduate student behavior and among students living off-campus. A case rate of 232/10,000 undergraduates equaled or surpassed several peer institutions that conducted mass testing. Conclusions: An emphasis on behavioral mitigation and communication can control virus transmission on a large urban campus combined with a limited and targeted testing strategy.

Abstract:Residential colleges and universities face unique challenges in providing in-person instruction during the COVID-19 pandemic. Administrators are currently faced with decisions about whether to open during the pandemic and what modifications of their normal operations might be necessary to protect students, faculty and staff. There is little information, however, on what measures are likely to be most effective and whether existing interventions could contain the spread of an outbreak on campus. We develop a full-scale stochastic agent-based model to determine whether in-person instruction could safely continue during the pandemic and evaluate the necessity of various interventions. Simulation results indicate that large scale randomized testing, contact-tracing, and quarantining are important components of a successful strategy for containing campus outbreaks. High test specificity is critical for keeping the size of the quarantine population manageable. Moving the largest classes online is also crucial for controlling both the size of outbreaks and the number of students in quarantine. Increased residential exposure can significantly impact the size of an outbreak, but it is likely more important to control non-residential social exposure among students. Finally, necessarily high quarantine rates even in controlled outbreaks imply significant absenteeism, indicating a need to plan for remote instruction of quarantined students.

Abstract:Background: Universities are vulnerable to infectious disease outbreaks, making them ideal environments to study transmission dynamics and evaluate mitigation and surveillance measures. Here, we analyze multimodal COVID-19-associated data collected during the 2020-2021 academic year at Colorado Mesa University and introduce a SARS-CoV-2 surveillance and response framework. Methods: We analyzed epidemiological and sociobehavioral data (demographics, contact tracing, and WiFi-based co-location data) alongside pathogen surveillance data (wastewater and diagnostic testing, and viral genomic sequencing of wastewater and clinical specimens) to characterize outbreak dynamics and inform policy. We applied relative risk, multiple linear regression, and social network assortativity to identify attributes or behaviors associated with contracting SARS-CoV-2. To characterize SARS-CoV-2 transmission, we used viral sequencing, phylogenomic tools, and functional assays. Findings: Athletes, particularly those on high-contact teams, had the highest risk of testing positive. On average, individuals who tested positive had more contacts and longer interaction durations than individuals who never tested positive. The distribution of contacts per individual was overdispersed, although not as overdispersed as the distribution of phylogenomic descendants. Corroboration via technical replicates was essential for identification of wastewater mutations. Conclusions: Based on our findings, we formulate a framework that combines tools into an integrated disease surveillance program that can be implemented in other congregate settings with limited resources. Comparing intervention measures in a model of a disease outbreak on a university campus

Abstract:A number of theoretical models have been developed in recent years modelling epidemic spread in educational settings such as universities to help inform re-opening strategies during the Covid-19 pandemic. However, these studies have had differing conclusions as to the most effective non-pharmaceutical interventions. They also largely assumed permanent acquired immunity, meaning we have less understanding of how disease dynamics will play out when immunity wanes. Here we complement these studies by developing and analysing a stochastic simulation model of disease spread on a university campus where we allow immunity to wane, expoloring the effectiveness of different interventions. We find that the two most effective interventions to limit the severity of a disease outbreak are reducing extra-household mixing and surveillance testing backed-up by a moderate isolation period. We find that contact tracing only has a limited effect, while reducing class sizes only has much effect if extra-household mixing is already low. We identify a range of measures that can not only limit an outbreak but prevent it entirely, and also comment on the variation in measures of severity that emerge from our stochastic simulations. We hope that our model may help in designing effective strategies for universities in future disease outbreaks.

Abstract:We develop an agent-based model on a network meant to capture features unique to COVID-19 spread through a small residential college. We find that a safe reopening requires strong policy from administrators combined with cautious behavior from students. Strong policy includes weekly screening tests with quick turnaround and halving the campus population. Cautious behavior from students means wearing facemasks, socializing less, and showing up for COVID-19 testing. We also find that comprehensive testing and facemasks are the most effective single interventions, building closures can lead to infection spikes in other areas depending on student behavior, and faster return of test results significantly reduces total infections.

Abstract:This study introduces a stochastic model of COVID-19 transmission tailored to the Colorado School of Mines campus and evaluates surveillance testing strategies within a university context. Enhancing the conventional SEIR framework with stochastic transitions, our model accounts for the unique characteristics of disease spread in a residential college, including specific states for testing, quarantine, and isolation. Employing an approximate Bayesian computation (ABC) method for parameter estimation, we navigate the complexities inherent in stochastic models, enabling an accurate fit of the model with the campus case data. We then studied our model under the estimated parameters to evaluate the efficacy of different testing policies that could be implemented on a university campus. This framework not only advances understanding of COVID-19 dynamics on the Mines campus but serves as a blueprint for comparable settings, providing insights for informed strategies against infectious diseases.

Abstract:Modeling infection spread during pandemics is not new, with models using past data to tune simulation parameters for predictions. These help in understanding of the healthcare burden posed by a pandemic and responding accordingly. However, the problem of how college/university campuses should function during a pandemic is new for the following reasons: (i) social contact in colleges are structured and can be engineered for chosen objectives; (ii) the last pandemic to cause such societal disruption was more than 100 years ago, when higher education was not a critical part of society; (iii) not much was known about causes of pandemics, and hence effective ways of safe operations were not known; and (iv) today with distance learning, remote operation of an academic institution is possible. As one of the first to address this problem, our approach is unique in presenting a flexible simulation system, containing a suite of model libraries, one for each major component. The system integrates agent-based modeling and the stochastic network approach, and models the interactions among individual entities (e.g., students, instructors, classrooms, residences) in great detail. For each decision to be made, the system can be used to predict the impact of various choices, and thus enables the administrator to make informed decisions. Although current approaches are good for infection modeling, they lack accuracy in social contact modeling. Our agent-based modeling approach, combined with ideas from Network Science, presents a novel approach to contact modeling. A detailed case study of the University of Minnesota’s Sunrise Plan is presented. For each decision made, its impact was assessed, and results were used to get a measure of confidence. We believe that this flexible tool can be a valuable asset for various kinds of organizations to assess their infection risks in pandemic-time operations, including middle and high schools, factories, warehouses, and small/medium-sized businesses.

Abstract:The number of coronavirus disease 2019 (COVID-19) cases continues to surge, overwhelming healthcare systems and causing excess mortality in many countries. Testing of infectious populations remains a key strategy to contain the COVID-19 outbreak, delay the exponential spread of the disease, and flatten the epidemic curve. Using the Omicron variant outbreak as a background, this study aimed to evaluate the effectiveness of testing strategies with different test combinations and frequencies, analyze the factors associated with testing effectiveness, and optimize testing strategies based on these influencing factors. We developed a stochastic, agent-based, discrete-time susceptible–latent–infectious–recovered model simulating a community to estimate the association between three levels of testing strategies and COVID-19 transmission. Antigen testing and its combination strategies were more efficient than polymerase chain reaction (PCR)-related strategies. Antigen testing also showed better performance in reducing the demand for hospital beds and intensive care unit beds. The delay in the turnaround time of test results had a more significant impact on the efficiency of the testing strategy compared to the detection limit of viral load and detection-related contacts. The main advantage of antigen testing strategies is the short turnaround time, which is also a critical factor to be optimized to improve PCR strategies. After modifying the turnaround time, the strategies with less frequent testing were comparable to daily testing. The choice of testing strategy requires consideration of containment goals, test efficacy, community prevalence, and economic factors. This study provides evidence for the selection and optimization of testing strategies in the post-pandemic era and provides guidance for optimizing healthcare resources.

Modeling the Spread of COVID-19 in University Communities

Abstract:Mathematical and simulation models are often used to predict the spread of a disease and estimate the impact of public health interventions, and many such models have been developed and used during the COVID-19 pandemic. This paper describes a study that systematically compared models for a university community, which has a much smaller but more connected population than a state or nation. We developed a stochastic agent-based model, a deterministic compartment model, and a model based on ordinary differential equations. All three models represented the disease progression with the same susceptible-exposed-infectious-recovered (SEIR) model. We created a baseline scenario for a population of 14,000 students and faculty and eleven other scenarios for combinations of interventions such as regular testing, contact tracing, quarantine, isolation, moving courses online, mask wearing, improving ventilation, and vaccination. We used parameter values from other epidemiological studies and incorporated data about COVID-19 testing in College Park, Maryland, but the study was designed to compare modeling approaches to each other using a synthetic population. For each scenario we used the models to estimate the number of persons who become infected over a semester of 119 days. We evaluated the models by comparing their predictions and evaluating their parsimony and computational effort. The agent-based model (ABM) and the deterministic compartment model (DCM) had similar results with cyclic flow of persons to and from quarantine, but the model based on ordinary differential equations failed to capture these dynamics. The ABM's computation time was much greater than the other two models' computation time. The DCM captured some of the dynamics that were present in the ABM's predictions and, like those from the ABM, clearly showed the importance of testing and moving classes on-line.

Modeling of Network Based Digital Contact Tracing and Testing Strategies for the COVID-19 Pandemic

Abstract:With more than 1.7 million COVID-19 deaths, identifying effective measures to prevent COVID-19 is a top priority. We developed a mathematical model to simulate the COVID-19 pandemic with digital contact tracing and testing strategies. The model uses a real-world social network generated from a high-resolution contact data set of 180 students. This model incorporates infectivity variations, test sensitivities, incubation period, and asymptomatic cases. We present a method to extend the weighted temporal social network and present simulations on a network of 5000 students. The purpose of this work is to investigate optimal quarantine rules and testing strategies with digital contact tracing. The results show that the traditional strategy of quarantining direct contacts reduces infections by less than 20% without sufficient testing. Periodic testing every 2 weeks without contact tracing reduces infections by less than 3%. A variety of strategies are discussed including testing second and third degree contacts and the pre-exposure notification system, which acts as a social radar warning users how far they are from COVID-19. The most effective strategy discussed in this work was combined the pre-exposure notification system with testing second and third degree contacts. This strategy reduces infections by 18.3% when 30% of the population uses the app, 45.2% when 50% of the population uses the app, 72.1% when 70% of the population uses the app, and 86.8% when 95% of the population uses the app. When simulating the model on an extended network of 5000 students, the results are similar with the contact tracing app reducing infections by up to 79%.

Abstract:Testing for infection with SARS-CoV-2 is an important intervention in reducing onwards transmission of COVID-19, particularly when combined with the isolation and contact-tracing of positive cases. Many countries with the capacity to do so have made use of lab-processed Polymerase Chain Reaction (PCR) testing targeted at individuals with symptoms and the contacts of confirmed cases. Alternatively, Lateral Flow Tests (LFTs) are able to deliver a result quickly, without lab-processing and at a relatively low cost. Their adoption can support regular mass asymptomatic testing, allowing earlier detection of infection and isolation of infectious individuals. In this paper we extend and apply the agent-based epidemic modelling framework Covasim to explore the impact of regular asymptomatic testing on the peak and total number of infections in an emerging COVID-19 wave. We explore testing with LFTs at different frequency levels within a population with high levels of immunity and with background symptomatic PCR testing, case isolation and contact tracing for testing. The effectiveness of regular asymptomatic testing was compared with `lockdown' interventions seeking to reduce the number of non-household contacts across the whole population through measures such as mandating working from home and restrictions on gatherings. Since regular asymptomatic testing requires only those with a positive result to reduce contact, while lockdown measures require the whole population to reduce contact, any policy decision that seeks to trade off harms from infection against other harms will not automatically favour one over the other. Our results demonstrate that, where such a trade off is being made, at moderate rates of early exponential growth regular asymptomatic testing has the potential to achieve significant infection control without the wider harms associated with additional lockdown measures.

Abstract:Returning universities to full on-campus operations while the COVID-19 pandemic is ongoing has been a controversial discussion in many countries. The risk of large outbreaks in dense course settings is contrasted by the benefits of in-person teaching. Transmission risk depends on a range of parameters, such as vaccination coverage and efficacy, number of contacts and adoption of non-pharmaceutical intervention measures (NPIs). Due to the generalised academic freedom in Europe, many universities are asked to autonomously decide on and implement intervention measures and regulate on-campus operations. In the context of rapidly changing vaccination coverage and parameters of the virus, universities often lack sufficient scientific insight to base these decisions on. To address this problem, we analyse a calibrated, data-driven agent-based simulation of transmission dynamics of 10755 students and 974 faculty members in a medium-sized European university. We use a co-location network reconstructed from student enrollment data and calibrate transmission risk based on outbreak size distributions in education institutions. We focus on actionable interventions that are part of the already existing decision-making process of universities to provide guidance for concrete policy decisions. Here we show that, with the Omicron variant of the SARS-CoV-2 virus, even a reduction to 25% occupancy and universal mask mandates are not enough to prevent large outbreaks given the vaccination coverage of about 80% recently reported for students in Austria. Our results show that controlling the spread of the virus with available vaccines in combination with NPIs is not feasible in the university setting if presence of students and faculty on campus is required.

Abstract:Mechanistic models fit to streaming surveillance data are critical to understanding the transmission dynamics of an outbreak as it unfolds in real-time. However, transmission model parameter estimation can be imprecise, and sometimes even impossible, because surveillance data are noisy and not informative about all aspects of the mechanistic model. To partially overcome this obstacle, Bayesian models have been proposed to integrate multiple surveillance data streams. We devised a modeling framework for integrating SARS-CoV-2 diagnostics test and mortality time series data, as well as seroprevalence data from cross-sectional studies, and tested the importance of individual data streams for both inference and forecasting. Importantly, our model for incidence data accounts for changes in the total number of tests performed. We model the transmission rate, infection-to-fatality ratio, and a parameter controlling a functional relationship between the true case incidence and the fraction of positive tests as time-varying quantities and estimate changes of these parameters nonparametrically. We compare our base model against modified versions which do not use diagnostics test counts or seroprevalence data to demonstrate the utility of including these often unused data streams. We apply our Bayesian data integration method to COVID-19 surveillance data collected in Orange County, California between March 2020 and February 2021 and find that 32--72\% of the Orange County residents experienced SARS-CoV-2 infection by mid-January, 2021. Despite this high number of infections, our results suggest that the abrupt end of the winter surge in January 2021 was due to both behavioral changes and a high level of accumulated natural immunity.

Abstract:Non-pharmaceutical public health measures (PHMs) were central to pre-vaccination eﬀorts to reduce Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) exposure risk; heterogeneity in adherence placed bounds on their potential eﬀectiveness, and correlation in their adoption makes assessing the impact attributable to an individual PHM diﬃcult. During the Fall 2020 semester, we used a longitudinal cohort design in a university student population to conduct a behavioral survey of intention to adhere to PHMs, paired with an IgG serosurvey to quantify SARS- CoV-2 exposure at the end of the semester. Using Latent Class Analysis on behavioral survey responses, we identiﬁed three distinct groups among the 673 students with IgG samples: 256 (38.04%) students were in the most adherent group, intending to follow all guidelines, 306 (46.21%) in the moderately-adherent group, and 111 (15.75%) in the least-adherent group, rarely intending to follow any measure, with adherence negatively correlated with seropositivity of 25.4%, 32.2% and 37.7%, respectively. Moving all individuals in an SIR model into the most adherent group resulted in a 76-93% reduction in seroprevalence, dependent on assumed assortativity. The potential impact of increasing PHM adherence was limited by the substantial exposure risk in the large proportion of students already following all PHMs.

Modeling the Risk of In-Person Instruction during the COVID-19 Pandemic

Abstract:During the COVID-19 pandemic, safely implementing in-person indoor instruction was a high priority for universities nationwide. To support this effort at the University, we developed a mathematical model for estimating the risk of SARS-CoV-2 transmission in university classrooms. This model was used to evaluate combinations of feasible interventions for classrooms at the University during the pandemic and optimize the set of interventions that would allow higher occupancy levels, matching the pre-pandemic numbers of in-person courses. Importantly, we determined that requiring masking in dense classrooms with unrestricted seating with more than 90% of students vaccinated was easy to implement, incurred little logistical or financial cost, and allowed classes to be held at full capacity. A retrospective analysis at the end of the semester confirmed the model's assessment that the proposed classroom configuration would be safe. Our framework is generalizable and was used to support reopening decisions at Stanford University. In addition, our framework is flexible and applies to a wide range of indoor settings. It was repurposed for large university events and gatherings and could be used to support planning indoor space use to avoid transmission of infectious diseases across various industries, from secondary schools to movie theaters and restaurants.

Cost-effective proactive testing strategies during COVID-19 mass vaccination: A modelling study

Abstract:BACKGROUND: As SARS-CoV-2 vaccines are administered worldwide, the COVID-19 pandemic continues to exact significant human and economic costs. Mass testing of unvaccinated individuals followed by isolation of positive cases can substantially mitigate risks and be tailored to local epidemiological conditions to ensure cost effectiveness.METHODS: Using a multi-scale model that incorporates population-level SARS-CoV-2 transmission and individual-level viral load kinetics, we identify the optimal frequency of proactive SARS-CoV-2 testing, depending on the local transmission rate and proportion immunized.FINDINGS: Assuming a willingness-to-pay of US$100,000 per averted year of life lost (YLL) and a price of $10 per test, the optimal strategy under a rapid transmission scenario (Re ∼ 2.5) is daily testing until one third of the population is immunized and then weekly testing until half the population is immunized, combined with a 10-day isolation period of positive cases and their households. Under a low transmission scenario (Re ∼ 1.2), the optimal sequence is weekly testing until the population reaches 10% partial immunity, followed by monthly testing until 20% partial immunity, and no testing thereafter.INTERPRETATION: Mass proactive testing and case isolation is a cost effective strategy for mitigating the COVID-19 pandemic in the initial stages of the global SARS-CoV-2 vaccination campaign and in response to resurgences of vaccine-evasive variants.FUNDING: US National Institutes of Health, US Centers for Disease Control and Prevention, HK Innovation and Technology Commission, China National Natural Science Foundation, European Research Council, and EPSRC Impact Acceleration Grant.

Abstract:Testing individuals for pathogens can affect the spread of epidemics. Understanding how individual-level processes of sampling and reporting test results can affect community- or population-level spread is a dynamical modeling question. The effect of testing processes on epidemic dynamics depends on factors underlying implementation, particularly testing intensity and on whom testing is focused. Here, we use a simple model to explore how the individual-level effects of testing might directly impact population-level spread. Our model development was motivated by the COVID-19 epidemic, but has generic epidemiological and testing structures. To the classic SIR framework we have added a per capita testing intensity, and compartment-specific testing weights, which can be adjusted to reflect different testing emphases -- surveillance, diagnosis, or control. We derive an analytic expression for the relative reduction in the basic reproductive number due to testing, test-reporting and related isolation behaviours. Intensive testing and fast test reporting are expected to be beneficial at the community level because they can provide a rapid assessment of the situation, identify hot spots, and may enable rapid contact-tracing. Direct effects of fast testing at the individual level are less clear, and may depend on how individuals' behaviour is affected by testing information. Our simple model shows that under some circumstances both increased testing intensity and faster test reporting can reduce the effectiveness of control, and allows us to explore the conditions under which this occurs. Conversely, we find that focusing testing on infected individuals always acts to increase effectiveness of control.

Abstract:Early case detection is critical to preventing onward transmission of COVID-19 by enabling prompt isolation of index infections, and identification and quarantining of contacts. Timeliness and completeness of ascertainment depend on the surveillance strategy employed. This paper presents modelling used to inform workplace testing strategies for the Australian government in early 2021. We use rapid prototype modelling to quickly investigate the effectiveness of testing strategies to aid decision making. Models are developed with a focus on providing relevant results to policy makers, and these models are continually updated and improved as new questions are posed. Developed to support the implementation of testing strategies in high risk workplace settings in Australia, our modelling explores the effects of test frequency and sensitivity on outbreak detection. We start with an exponential growth model, which demonstrates how outbreak detection changes depending on growth rate, test frequency and sensitivity. From the exponential model, we learn that low sensitivity tests can produce high probabilities of detection when testing occurs frequently. We then develop a more complex Agent Based Model, which was used to test the robustness of the results from the exponential model, and extend it to include intermittent workplace scheduling. These models help our fundamental understanding of disease detectability through routine surveillance in workplaces and evaluate the impact of testing strategies and workplace characteristics on the effectiveness of surveillance. This analysis highlights the risks of particular work patterns while also identifying key testing strategies to best improve outbreak detection in high risk workplaces.
