Abstract:Background Primary central nervous system lymphoma (PCNSL) is a rare subtype of extranodal non-Hodgkin's lymphoma with poor prognosis. High-dose methotrexate (HD-MTX) is the cornerstone for treatment. However, there is no consensus on the best combination chemotherapy regimen. This study aimed to explore the efficacy and safety study of R2-MTX (Lenalidomide, Rituximab and Methotrexate) as a first-line induction regimen in the treatment of newly diagnosed PCNSLs. Methods This is a single-arm, prospective phase II study. Immunocompetent patients (18 to 75 years of age) with newly diagnosed PCNSLs were treated with 6 cycles of R2-MTX induction chemotherapy. Lenalidomide was dosed at 25mg daily (day 1-10); rituximab was dosed at 375mg/m2 (d0), MTX was dosed at 3.5g/m2 (d1). Every 21 days were as a course of treatment. Patients who did not reach CR after 6 courses of treatment were given salvage whole-brain radiotherapy (WBRT) to 30-36 Gy followed by a limited field to gross disease to 45 Gy. Then the patients received R2 maintenance (Rituximab 375mg/m 2 d1, lenalidomide 25mg d1-14, 29-42, 56 days per cycle) for 4 cycles. The primary endpoint is the objective response rate. Results 14 patients were enrolled from Feb 2020 to June 2021. Six (42.9%) patients were male and the median age was 59 (range 44-73). Ten (83.3%) patients were mediate or high-risk group (2-5 points) according to IELSG score. The last follow-up was July 15th 2021. Twelve patients were evaluable for response (one patient withdrew the consent; another patient was found with chronic lymphocytic leukemia). The overall response rate (ORR) was 91.7% (11/12). Nine patients achieved complete remission, and two patients achieved partial remission at the last follow-up. The median follow-up time was 11.3 (1.3-16.9) months. Only one patient did not respond and died for disease progression (Figure 1). The most common grade ≥3 adverse events (AEs) included elevated alanine transaminase (ALT) (16.7%), neutropenia (16.7%), thrombocytopenia (8.3%), infection (8.3%), and acute kidney injury (8.3%) (One patient with MTX overdose suffered from all these ≥3AEs). The most common grade 1-2 AEs were neutropenia (66.7%), anemia (58.3%), thrombocytopenia (16.7%) elevated ALT (41.7%). Conclusion R2-MTX regimen is an effective and well-tolerated combination treatment for newly diagnosed PCNSLs. More clinical data will be updated. High-dose Methotrexate Plus Temozolomide As a Salvage Treatment in Metastatic Central Nervous System Lymphoma: Two Cases Report and Review of Literature.

Abstract:Temozolomide (TMZ), an alkylating agent, is widely used for treating high-grade gliomas and has emerged as a treatment option for primary central nervous system lymphoma (PCNSL) because of its good penetration of the blood-brain barrier (BBB) and mild toxicity profile. To our knowledge, the role of temozolomide in the treatment of lymphoma was confined to PCNSL and its role in lymphoma central infiltration has not been reported yet. We describe our experience using a combination of high-dose methotrexate and temozolomide (MT) in two metastatic central nervous system Lymphoma (CNSL). As far as we know, this is the first case report about temozolomide in metastatic CNSL patients and no study to date has incorporated investigation of the biology of metastatic CNSL into the treatment protocol. Temozolomide may represent a promising new drug for the treatment of metastatic CNSL.

Fourteen patients receiving rituximab plus methotrexate with cytarabine (R-MA regimen) at the same period were identified as the positive control group. The response rates, survival, toxicities, length of hospital stay (LOS), and cost were compared. Compared with the R-MA regimen, the R-M regimen showed comparable response rate and survival outcomes, but had fewer grade 3-4 hematological toxicities, shorter LOS, lower mean total hospitalization cost and lower mean total antibiotic cost. Overall response after two cycles of chemotherapy, complete remission at the end of induction chemotherapy and ECOG>3 were independent prognostic factors for overall survival. In conclusion, R-M regimen is an effective and well-tolerated combination treatment for PCNSLs, which warrants further evaluation in randomized trials.

Abstract:The optimal high-dose methotrexate (HDMTX)-based combination therapy for primary central nervous system lymphoma is unknown. We report our experience with rituximab, HDMTX, procarbazine and lomustine (R-MPL) given as first-line treatment in our center. Fifty-two patients between 2006 and 2019 were included. Eighteen patients proceeded to autologous transplant or two cycles of intermediate-dose cytarabine. The median age was 62 y (range 28–94) and the Eastern Cooperative Oncology Group performance status (ECOG-PS) was ≥2 in 62% (32/52). The overall/complete response rates were 79% (41/52) and 52% (27/52), respectively. The median progression-free/overall survival was 19 m/84m, respectively. Grade 3–4 adverse events included infections (17%) and kidney injury (13%). Ten patients (19%) discontinued therapy for toxicity. There were no treatment-related deaths. In summary, in a cohort enriched in frail patients, R-MPL achieved good responses and OS and was safe for all ages. The PFS was sub-optimal, possibly explained by a low proportion of consolidation. This regimen should be evaluated prospectively.

Abstract:Primary central nervous system lymphoma (PCNSL) is a rare form of non-Hodgkin lymphoma with poor long-term survival. This study assessed methotrexate (MTX) plus idarubicin (IDA) in treating patients of PCNSL comparing to MTX alone therapy. A total of 100 patients were retrospectively enrolled and subjected to MTX alone (n = 52) and MTX plus IDA (n = 48). The completed response (CR) rate in patients treated with MTX plus IDA was 62.50%, and overall response (OR) rate was 79.17%, which in MTX alone cohort were 42.31% and 63.46% respectively. Median progression-free survival (PFS) of patients treated with MTX plus IDA was significantly better than those treated with MTX alone (18.35 months vs.8.45months, P = 0.000). The MTX plus IDA regimen exhibited a significantly better control of PCNSL. Further studies would be needed to confirm these results.

Abstract:Objective To evaluate the effectiveness and safety of rituximab combined with highdose methotrexate,dexamethasone and dexamethasone (R-MAD regimen) for the treatment of patients with primary central nervous system lymphoma (PCNSL).Methods The clinical data of 35 patients with PCNSL treated at the Department of Hematology,Beijing Tiantan Hospital from January 2010 to June 2015 were analyzed retrospectively.All patients received rituximab (d0:375 mg/m2),high-dose methotrexate (d1:3.5g/m2),cytarabine (d2:0.5-1 g/m2),and dexamethasone (d1-3:10 mg/d) for chemotherapy.Every 21 days were as a course of treatment,and they were treated for 4-6 courses.Results The median follow-up time of 35 patients was 36 (6-66) months,20 (57.1％) of them achieved complete remission,13 (37.1％) achieved partial remission,and 2 (5.7％) had progression of disease.The total effective rate was 94.2％ (33/35).The median survival time was 38.3 months (95％ confidence interval [CI] 19.4-57.2),and the median progression-free survival was 30.0 months (95％ CI 9.4-50.6).The adverse reaction of chemotherapy was mainly bone marrow suppression.Twenty-four patients experienced grade 1 to 3 neutropenia and thrombocytopenia in the course of the disease.No grade 4 and above adverse reactions were observed.Conclusion R-MAD regimen is both safe and effective in the treatment of primary central nervous system neoplasms.
A Prospective Cohort Study of Methotrexate Plus Idarubicin in Newly Diagnosed Primary CNS Lymphoma

Abstract:Purpose High-dose methotrexate (HD-MTX)-based chemotherapy regimen is the first-line option for primary central nervous system lymphoma (PCNSL). This prospective cohort study aimed to evaluate the efficacy and adverse effects of HD-MTX plus idarubicin (IDA) in patients with newly diagnosed immunocompetent PCNSL. Methods We recruited newly diagnosed PCNSL patients from January 2017 to August 2020. Patients were assigned into two groups: HD-MTX monotherapy and HD-MTX plus IDA (HD-MTX/IDA). In the HD-MTX monotherapy group, patients were treated with MTX 8 g/m 2 alone on day 1, while the HD-MTX/IDA group received MTX 8 g/m 2 on day 1 and IDA 10 mg/m 2 on day 2. Treatments were repeated every 3 weeks for 8 cycles except for progression and/or unacceptable toxicity. Results We recruited 61 PCNSL patients, including 36 in the HD-MTX and 25 in the HD-MTX/IDA group. The CR rate was 68% in the HD-MTX/IDA group and 72.22% of patients in the HD-MTX monotherapy group (p = 0.7221), while the overall response rate was 72% vs. 77.78% (p = 0.6063). Median PFS in HD-MTX/IDA group and HD-MTX monotherapy group were 15.6 months and 18.5 months, respectively (p = 0.6374). Median OS was not reached in both groups. There were no significant differences in adverse effects between the two groups. Conclusions The combination of IDA with HD-MTX showed no obvious therapeutic advantage over HD-MTX monotherapy in newly diagnosed patients with PCNSL. HD-MTX dose of 8 g/m 2 monotherapy can still provide better therapeutic benefits in patients with acceptable adverse effects. Future studies could explore HD-MTX in combination with other chemotherapeutic agents in the first-line treatment of PCNSL.

Abstract:Purpose: Central nervous system lymphoma (CNSL) is an aggressive non-Hodgkin's lymphoma (NHL) confined to the central nervous system (CNS). Orelabrutinib is an oral second-generation Bruton tyrosine kinase (BTK) inhibitor and a novel therapeutic strategy for CNSL. The purpose of this study was to evaluate the effectiveness and safety of high-dose methotrexate (HD-MTX), thiotepa, and orelabrutinib combined with or without rituximab (MTO±R)regimen in the treatment of patients with CNSL. Methods: A total of 14 patients with CNS diffuse large B-cell lymphoma (DLBCL) were included in this retrospective study. All patients received the regimen MTO±R. Overall response rate (ORR), complete response rate(CR), partial response (PR), stable disease (SD), progressive disease (PD), progression-free survival (PFS), overall survival (OS), and the safety of MTO±R were assessed by the investigator. Results: Fourteen patients were evaluable for safety, and 13 patients were evaluable for efficacy. The overall CR rate was 69.2%, and the ORR was 92.3% for total patients. For PCNSL, the CR rate and ORR were 55.6% and 88.9%, respectively. For relapsed/refractory CNSL, the CR rate and ORR were 66.7% and 91.7%, respectively. The median follow-up time was 12.8 months. The median PFS was 11.3 months, and the median OS was not achieved. The 12-month PFS and OS rates were 60% and 70%, respectively. Adverse events occurred in 17 cycles, and Grade 3 AEs occurred in 5 patients (35.7%). Conclusion: MTO±R was an efficacious and well-tolerated regimen in patients with CNSL. A novel BTK inhibitor in combination with chemotherapy offers a new potential therapeutic strategy for patients with CNSL.

Abstract:The prognosis of patients with primary central nervous system lymphoma (PCNSL) has improved in recent years. This has partly been achieved by remission induction protocols incorporating high-dose methotrexate (HD-MTX) and rituximab. Given the high rates of relapse, consolidation therapy is usually considered in first response. Whole brain radiotherapy may prolong PFS but appears to confer no long-term survival advantage and is associated with significant neurocognitive dysfunction. Attempts to improve efficacy and reduce neurotoxicity of consolidation therapy have included thiotepa-based high-dose chemotherapy and autologous stem cell transplant (HDC-ASCT). This multi-centre, retrospective study reports the outcome of 70 patients undergoing HDC-ASCT for PCNSL in the United Kingdom. The median age at diagnosis was 56 years and all patients received HD-MTX-containing induction regimens. All patients underwent HDC-ASCT in first response. The rate of complete response increased from 50% before HDC-ASCT to 77% following HDC-ASCT. Treatment-related mortality was 6%. At a median follow-up of 12 months from HDC-ASCT, the estimated 1- and 2-year PFS rates were 71.5% and overall survival 86.4% and 83.3%, respectively. These data are comparable to published studies of HDC-ASCT for PCNSL, supporting its feasibility and efficacy.

Abstract:BACKGROUND Primary central nervous system lymphoma (PCNSL) is a rare and aggressive subtype of lymphoma with poor prognosis. One potential therapeutic approach could involve targeting the programmed death-1 (PD-1) immune checkpoint pathway, as these tumors often express PD-L1. However, the efficacy of this strategy using sintilimab in combination with immunochemotherapy as frontline therapy for patients with newly diagnosed PCNSL remains uncertain. METHODS In this phase II trial with a safety run-in, we enrolled treatment-naive PCNSL patients between the ages of 18 and 70, who had an Eastern Cooperative Oncology Group (ECOG) performance status of 0-2. All patients were given sintilimab combined with rituximab, high-dose methotrexate and temozolomide (SRMT) regimen (six cycles every 21 days): sintilimab (200 mg, i.v., Day-0), rituximab (375 mg/m 2, i.v., Day-0), methotrexate (3.0 g/m 2, i.v., Day-1 or 1.0 g/m 2 for patients aged ≥65 years, and temozolamide (150 mg/m 2/d, p.o., Days 1-5). The primary outcome was overall response rate (ORR), with secondary outcomes including duration of response (DOR), progression-free survival (PFS), overall survival (OS), safety, and potential biomarkers. RESULTS Between 3 April 2020 and 23 August 2022, a total of 28 patients were enrolled. No dose-limiting toxicities were observed during the safety run-in period. Among 27 evaluable patients, the primary outcome was met with an impressive ORR of 96.3% (95% CI: 81-99.9%). In detail, 25 patients (92.6%) achieved a complete response and one (3.7%) exhibited a partial response. As of 7 April 2023, the median follow-up was 20.8 months. The Medians for DOR, PFS, and OS were not reached. Remarkably, 2-year PFS and 2-year OS were 65.4% (48.1-88.8%) and 88.5% (74.1-100%), respectively. The most common grade 3-4 treatment-related toxicities were increased levels of alanine aminotransferase (18.5%) and aspartate aminotransferase (14.8%). Additionally, the receiver operating characteristic analysis revealed that the baseline IL10/IL6 ratio in cerebrospinal fluid exhibited promising performance in predicting PFS, achieving an area under curve of 0.91 at 2 years. CONCLUSIONS The SRMT regimen as frontline treatment for PCNSL is highly efficacious and well tolerated. (Chinese Clinical Trial Registry number: ChiCTR1900027433).

BTK inhibitors are the second-line recommended choice in relapsed of refractory cases. In this study, we described our institutional experience with the oral BTK inhibitor, ibrutinib, combined with rituximab and HD-MTX (I-RM regimen) in newly diagnosed PCNSL. Methods: The study was a phase 1, investigator-initiated, clinical trial being conducted at Xuanwu Hospital, Capital Medical University . Patients definitely diagnosed as PCNSL-DLBCL by biopsy of brain lesion, systemic imageology, bone marrow and CSF examination, et al, and be suitable for the immunochemotherapy,were enrolled the clinical trial. The next-generatioin sequencing of the lesion tissue were used to detect hot spot mutations evolved in B cell lymphoma. The ibrutinib-based I-RM regimen consists of rituximab 375mg / m 2 on day 0, MTX 3.5g/m 2 on day1, and ibrutinib 560mg once daily starting from plasma MTX concentration <0.1umol/L to day 28, every 28 days is a cycle of treatment. After 4 cycles of I-RM induction therapy, patients who achieved CR/CRu/PR received autologous peripheral blood hematopoietic stem cell transplantation (HSCT) or the extra 2 cylces of I-RM regimen as consolidation therapy. Ibrutinib or lenalidomide monotherapy was prescribed as maintenance therapy, according to the willingness of patients until the disease progresses. The primary end point was the safety and efficacy of this regimen in newly diagnosed PCNSL. Results:There were totally16 patients enrolled in this clinical trial, between April 2018 and December 2020 (Table 1). All the patients were pathologically DLBCL, including 14 non-GCB subtypes and 2 GCB subtypes. The commonest hot spot mutations were PIM1 (68.75%), MYD88 (43.75%), and CD79B (37.50%). The median follow-up time was 24.5 (6-37) months up to June 30, 2021. After 4-cycles of I-RM induction therapy, CR were achieved in 8 patients (50%), PR in 7 patients (44%), and progressive disease in one patients(6%) who quit the clinical trial and transferred to other regimen,and the ORR of induction therapy was 94%. Among 15 responsers, 5 (33%) received HSCT and 10 (67%) received extra 2 cylces of I-RM regimen as consolidation therapy. After the consolidation therapy, 93% patients (14/15) received maintenance therapy, including 6 received lenalidomide and 8 received ibrutinib. Relapse happened in 2 patients during the maintenance therapy, including 1 death due to the progressed disease (Table 2). The median PFS and OS were not achieved, and the estimated 3-year OS and PFS were 88.9% and 86.5%, respectively (Figure 1). No grade 4 adverse event was observed. The grade 3 hematologic toxicities included lymphopenia in 3 cases and neutropenia in 1 case .The grade 3 non-hematologic toxicities including hypertension in 1 case and increased serum creatinine in 1 case. Conclusions: Our data preliminarily indicates that I-MR is active and safe as a first-line regimen in newly diagnosed PCNSL and might effectively improve the survival of these patients. Abstract:Primary central nervous system lymphoma (PCNSL) remains a disease with poor outcome and high recurrence rate. We retrospectively analyzed the clinical data of 243 immunocompetent patients with PCNSL in Beijing Tiantan Hospital. The median age of PCNSL patients was 57 years (range 10-95 years). For induction therapy, 94.7% of patients received high-dose methotrexate (HD-MTX) containing regimens, and 59.3% received rituximab, which increased over time. The overall response rate was 72.8%, with 58.8% achieving complete response. With a median follow-up of 27.0 months (95% confidence interval 23.6-30.4), the median progression-free survival (PFS) time was 14.0 months (95% CI 9.45-18.55), and the 2-year PFS rate was 33.2%. The median overall survival (OS) was not reached (NR), with an estimated overall survival rate at 4 years of 61.6%. Among 95 patients who completed sequential consolidation chemotherapy with either pemetrexed or etoposide plus cytarabine, the median PFS was 28 months (95% CI 17.11-38.89), and the estimated overall survival at 4 years was 78.7%. In conclusion, HD-MTX based induction chemotherapy with non-myeloablative sequential consolidation chemotherapy is an alternative feasible treatment option.

Abstract:OBJECTIVE: This study aimed to evaluate the safety, efficacy, and feasibility of the rituximab, fotemustine, pemetrexed, and dexamethasone (R-FPD) regimen followed by whole-brain radiotherapy (WBRT) for patients with primary central nervous system lymphoma (PCNSL).METHODS: A prospective, single-center phase II clinical trial was conducted. Patients with PCNSL newly diagnosed at the First Affiliated Hospital of Zhengzhou University between July 2018 and July 2020 were studied. The R-FPD regimen consisted of rituximab (375 mg/m2 i.v. on D0), fotemustine (100 mg/m2 i.v. on D1), pemetrexed (600 mg/m2 i.v. on D1), and dexamethasone (40 mg i.v. on D1-5). Patients 60 years or younger who showed a complete response (CR) were treated with 23.4 Gy of WBRT after the end of chemotherapy; those older than 60 years with CR were treated with a wait-and-see approach; and those who did not show CR after the 4th cycle of chemotherapy were given salvage WBRT 30 Gy + local tumor field irradiation up to 45 Gy, regardless of age.RESULTS: A total of 30 patients were included. After 2 cycles, the objective response rate (ORR) was 96.5% (28/29, 1 CR, 27 PR, 0 SD, and 1 PD). After 4 cycles, the ORR was 73.1% (19/26, 11 CR, 8 PR, 4 SD, and 3 PD). After WBRT, the ORR was 90.9% (10/11, 7 CR, 3 PR, and 1 SD). The grade III and IV toxicity responses were mainly leukopenia (20.0%), thrombocytopenia (23.3%), and anemia (10.0%).CONCLUSIONS: Fotemustine-based therapy in combination with rituximab chemotherapy followed by WBRT can improve outcomes, providing ORR benefits and favorable tolerability in patients newly diagnosed with PCNSL.

Abstract:Primary central nervous system lymphoma (PCNSL) is a rare and frequently fatal lymphoma subtype. The programmed death-1 (PD-1) pathway has emerged as a potential therapeutic target, but the effectiveness of PD-1 antibody sintilimab in combination with immunochemotherapy as a frontline treatment for PCNSL remains to be determined. In this phase 2 trial (ChiCTR1900027433) with a safety run-in, we included patients aged 18–70 with newly diagnosed PCNSL. Participants underwent six 21-day cycles of a SMTR regimen, which includes sintilimab (200 mg, Day 0), rituximab (375 mg/m 2 , Day 0), methotrexate (3.0 g/m 2 , Day 1 or 1.0 g/m 2 for patients aged ≥65 years), and temozolomide (150 mg/m 2 /d, Days 1–5). Among 27 evaluable patients, the overall response rate (ORR) was 96.3% (95% confidence interval: 81–99.9%), with 25 complete responses. At a median follow-up of 24.4 months, the medians for duration of response, progression-free survival (PFS), and overall survival were not reached. The most common grade 3–4 treatment-related toxicities were increased levels of alanine aminotransferase (17.9%) and aspartate aminotransferase (14.3%). Additionally, baseline levels of interferon-α and the IL10/IL6 ratio in cerebrospinal fluid emerged as potential predictors of PFS, achieving areas under the curve of 0.88 and 0.84, respectively, at 2 years. Whole-exome sequencing revealed a higher prevalence of RTK-RAS and PI3K pathway mutations in the durable clinical benefit group, while a greater frequency of Notch and Hippo pathway mutations in the no durable benefit group. These findings suggest the SMTR regimen is highly efficacious and tolerable for newly diagnosed PCNSL, warranting further investigation.

Abstract:The optimum treatment of primary CNS lymphoma (PCNSL) is not yet determined. The objective of this study was to assess the safety and efficacy of initial methotrexate-based chemotherapy followed by high-dose chemotherapy (HDT) with autologous stem cell transplantation (ASCT) in patients with newly diagnosed PCNSL. Twenty-five patients received two courses of initial chemotherapy combining methotrexate, etoposide, carmustine and methylprednisolone, and one course of ifosfamide–cytarabine followed by peripheral stem cell collection. Seventeen responsive patients then received HDT using carmustine, etoposide, cytarabine and melphalan with autologous stem cell rescue. After ASCT for responding patients or after salvage therapy for non-responders, whole brain radiation therapy at a dose of 30 Gy was delivered. The objective response rate to the induction chemotherapy was 84%. Four of the 21 responding patients did not have ASCT because of toxicity or refusal. With a median follow-up time of 34 months, the projected event free survival rate is 46% at 4 years. Projected overall survival is 64% at 4 years. Sixteen patients are actually in continuous complete response. No evidence of late treatment-related toxicity was observed. This treatment approach appears feasible in newly diagnosed PCNSL with encouraging results.

Abstract:Introduction Treatment of PCNSL has focused on multi-agent chemotherapy designed to cross the blood brain barrier (BBB); radiation therapy (RT) has a high response rate, but responses tend to be transient and associated with cognitive toxicity. The addition of rituximab has improved treatment outcomes for virtually all CD20-positive B-cell malignancies. Although rituximab is not believed to cross the BBB, enhancement on MRI corresponds with areas of BBB breakdown that could allow penetration of the antibody. Here we report the results of E1F05, an ECOG-ACRIN multicenter phase 2 prospective trial of rituximab added to a high-dose methotrexate (HD-MTX)-based chemotherapy regimen very similar to that used in the RTOG 93-10 study, but without RT. Methods Immunocompetent patients with newly diagnosed PCNSL, ECOG PS ≤3, with adequate liver and kidney function were eligible. Subjects received HD-MTX 3.5g/m2 with vincristine 1.4mg/m2 IV (flat cap of 2.8mg) weeks 1, 3, 5, 7, and 9, with leucovorin rescue; procarbazine 100mg/m2 PO daily for 7 days in weeks 1, 5, and 9; cytarabine 3g/m2 /day IV over 2 hours for 2 days on weeks 11 and 14; dexamethasone 16mg/day week 1, tapered by 4mg/week for weeks 2 and 3, 2mg/week weeks 4-6; and rituximab 375mg/m2 IV infusion 3 times a per week for weeks 1-4. Subjects with CSF involvement received intrathecal MTX 12mg every two weeks on weeks 2, 4, 6, 8, and 10 with leucovorin rescue. The primary endpoint was complete response rate (CR). Predefined secondary endpoints were PFS, OS. A total of 39 subjects was required to be able to rule out a 50% CR rate, with 90% power to detect a 70% CR rate. An interim analysis was planned after accrual of 23 subjects, requiring >12 CR to proceed to full accrual. Results Twenty-six patients were enrolled from December 2006 to February 2010; one subject was ineligible but included in toxicity evaluation. Median age was 57 (range 30-76); 40% were male (table 1). Sixteen subjects (65%) completed treatment per protocol; the most common reason for discontinuation was adverse events (4 subjects); 2 discontinued due to progressive disease (PD). Central radiology review (per the International PCNSL Collaborative Group) revealed that CR + CRu was 16/25 (64%,.95CI(42.5%-82%); ORR= 20/25 (80%), and 4/25(16%) had PD. Although the primary endpoint was met, the study was not re-opened after the interim analysis due to slow accrual and competing trials. At a median follow-up of 60 months, median PFS is 34 months (compared to 24 months in RTOG93-10), and median OS has not been reached (versus 37 months in RTOG93-10). Four year PFS is 33%, with 4 year OS 72% (approximately 30% in RTOG 93-10). No treatment-related deaths occurred. The most common grade 3-4 toxicities were hematologic (23% anemia, 77% neutropenia, 19% thrombocytopenia). Conclusion The addition of rituximab to multiagent chemotherapy is well tolerated. Response rates, PFS and OS are comparable to or better than those seen in RTOG 93-10, which included RT. The median progression-free survival (PFS) was 20.3 months (95%CI:5.2--35.4),The median overall survival (OS) was 22.0 months (95%CI:16.1-27.9).The grade III-IV toxicities were mainly leukopenia(17.9%), thrombocypenia(25%) and anemia(10.7%). Conclusion: Fotemustine-based in combination with Rituximab chemotherapy can improve outcomes with the progress free survival and the overall survival benefits, as well as with better tolerability for newly diagnosed PCNSL patients. Abstract:ABSTRACT BACKGROUND/PURPOSE: There is currently no standard effective treatment for patients with relapsed/refractory primary central nervous system lymphoma (PCNSL). The prognosis is poor, so more studies are needed. Pemetrexed is similar to methotrexate in structure and can cross the blood-brain barrier. Compared with methotrexate, it has more targets and fewer side effects. Lenalidomide is a second generation immunomodulator with multiple functions, such as regulating immunity, anti-tumor and tumor microenvironment. A prospective observational study was conducted to explore the efficacy and safety of pemetrexed combined with lenalidomide in the treatment of relapsed/refractory primary central nervous system lymphoma. METHODS: This is a prospective observational study, we collected patients who had undergone whole brain radiotherapy and two or more chemotherapy regimens between January 2018 and December 2019 but still experienced disease progression or recurrence. each patient was treated with pemetrexed at a dose of 900mg/m2 and lenalidomide 25mg over 3 weeks as salvage chemotherapy, and one cycle consists of 4 weeks. Folic acid, vitamin B12 and dexamethasone were used to induce toxicities to pemetrexed given before chemotherapy. Oral aspirin was used to prevent thrombosis caused by lenalidomide. Adverse events were recorded in each patient during inpatients, outpatient or telephone follow-up. RESULTS: A total of 38 patients with recurrent/refractory PCNSL were enrolled in our study, including 26 males and 12 females with a median age of 57 (range from 33 to 73) years old. After treatment, the overall response rate was 68.4%(Table 1). The median progression free survival (PFS) time was 6 months and median overall survival (OS) time was 14 months (Figure1, Figure2). The adverse events mainly included fatigue, gastrointestinal reaction, myelosuppression, thrombocytopenia, fever and infection. After long-term or short-term chemotherapy, the patients had different degrees of myelosuppression symptoms presented as leukopenia (six cases in grade 1 and one case in grade 3), neutropenia (six cases in grade 1 and two cases in grade 3), anemia (6 cases in grade 1), thrombocytopenia (1 case in grade 2 and 3 cases in grade 3). Nausea and vomiting, as the common gastrointestinal reaction, appeared in two (grade 1) and one (grade 2) cases, respectively. One patient died of severe pneumonia infection. Three cases developed grade 1 cardiac disorder, including asymptomatic sinus bradycardia and asymptomatic ventricular premature beat, respectively. In addition, other reactions including fatigue (four cases in grade 1 and one case in grade 2), fever (four cases). All patients had no abnormal liver function, kidney function, constipation and leukoencephalopathy (Table 2). CONCLUSION: This study has been the first prospective clinical study of pemetrexed combined with lenalidomide in the treatment of patients with relapsed/refractory PCNSL in international. The results indicate that chemotherapy with pemetrexed combined with lenalidomide may be an effective therapy for the treatment of relapsed/refractory PCNSL with modest toxicity.
