Abstract:Several publications and random observations have reported weight loss in high-altitude sojourners of both sexes. This could be a result of multiple adaptations, which hypoxia and mountaineering provoke on a cellular and organic level. Several publications have discussed the effect on appetite-regulating hormones to be one of the main contributing factors. We aimed to review the available data and show the current state of knowledge regarding nutritional aspects in high altitude with a special focus on fatty dietary forms. To reach this aim we conducted a literature search via PubMed according to the PRISMA 2020 protocol to identify relevant studies. We found that very few studies cover this field with scientifically satisfying evidence. For final analysis, reviews as well as papers that were not clearly related to the topic were excluded. Six articles were included discussing hormonal influences and the impact of exercise on appetite regulation as well as genetic factors altering metabolic processes at altitude. Leptin expression seems to be the biggest contributor to appetite reduction at altitude with an initial increase followed by a decrease in the course of time at high altitude. Its expression is greatly dependent on the amount of white adipose tissue. Since the expression of leptin is associated with an increased β-oxidation of fatty acids, a high-fat diet could be advantageous at a certain time point in the course of high-altitude sojourns.

Abstract:Endotherms at high altitude face the combined challenges of cold and hypoxia. Cold increases thermoregulatory costs, and hypoxia may limit both thermogenesis and aerobic exercise capacity. Consequently, in comparisons between closely related highland and lowland taxa, we might expect to observe consistent differences in basal metabolism (BMR), maximal metabolism (MMR), and aerobic scope. Previous broad‐scale comparative studies of birds have revealed no association between BMR and native elevation; however, altitude effects may be detectable in fine‐grained comparisons between pairs of ecologically similar, closely related species that are native to different elevations. Moreover, it remains unknown if exercise‐induced MMR and aerobic scope exhibit consistent patterns of altitude variation. To evaluate whether there is an association between altitude and metabolic rates, we tested for altitude‐related variation in aerobic metabolism in 10 Andean passerines representing five pairs of closely related species with contrasting elevational ranges. We used a paired‐lineage design such that the five pairwise comparisons were phylogenetically independent. In our small sample of Andean passerines, we found a significant effect of elevation on mass‐corrected BMR, MMR, and aerobic scope in most but not all cases. BMR and MMR were significantly higher for the high‐elevation species in three pairs, two of which also had a significantly higher aerobic scope at high‐elevation. One pair did not exhibit significant elevational differences in BMR, MMR, or aerobic scope. Overall, we could not document a uniformly consistent elevational trend in the species analyzed.

Abstract:Loss of appetite and weight are frequently observed at altitudes above 5000 m. However, the pathophysiology behind changes in body composition at extreme altitude is still not fully understood. Proper acclimatization to altitude and high caloric intake minimizes, but can not completely prevent significant weight loss under the influence of hypobaric hypoxia. The discovery of leptin in 1994 has initiated a new research area investigating molecular networks that connect peripheral organs with the central nervous system to sense and regulate energy intake as well as energy expenditure. Since then, a whole microcosm of new hormones, neurotransmitters and receptors has been discovered and studied with respect to body weight control. Those agents include neuropeptide Y (NPY), agouti-related protein (AGRP), melanocortin receptors (MC-R), cocaine-amphetamine regulated transcript (CART), pro-opiomelanocortin (POMC), orexin A and B (hypocretins), melanin-concentrating hormone (MCH) and ghrelin (endogenous ligand of the growth hormone secretagogue receptor). This overview will introduce the current concepts of the molecular control of energy homeostasis and attempt to reexamine the effects of altitude on appetite and body composition in light of these concepts. An overview of studies on changes of appetite and body composition at high altitude will be followed by the presentation of recent data on changes of endocrine parameters at hypobaric hypoxia that could be involved in the pathophysiology of weight loss.

Abstract:ABSTRACT Purpose To investigate the temporal effects of ~1,800 m altitude exposure and energy availability (EA) manipulation on resting metabolic rate (RMR). Methods Twenty elite female race walkers underwent a 3-week training camp at an altitude of ~1,800 m. During the first two weeks, athletes consumed a high EA (HEA) diet of 45 kcal·kg fat free mass (FFM) -1 ·day -1 . During the final week, half the athletes consumed a low EA (LEA) diet of 15 kcal·kg FFM -1 ·day -1 while the others continued on a HEA diet. Athletes followed individualized training plans throughout the study. To assess the effect of altitude on RMR, athletes in the HEA group had RMR measured at baseline (~580 m) prior to altitude exposure (Pre-alt), at 36-hours (36 h-alt), 2 weeks (Wk2-alt) and 3 weeks into altitude exposure (Wk3-alt), and at 36 hours post-altitude exposure at ~580 m (36 h-post). To assess the effect of LEA exposure on RMR while at altitude, athletes in the LEA group underwent RMR measurements at Pre-alt and before (Wk2-alt) and after the 7-days of LEA (Wk3-alt). Results Compared to Pre-alt, the RMR of HEA athletes was increased at 36 h-alt (+5.3 ± 3.1%; p = 0.026) and Wk2-alt (+4.9 ± 4.9%; p = 0.049), but was no longer elevated at Wk3-alt (+1.7 ± 4.2%; p = 0.850). The RMR of HEA athletes at 36 h-post was lower than all timepoints at altitude (p < 0.05) but was not different from Pre-alt (-3.9 ± 7.2%; p = 0.124). The 7-day period of LEA exposure at altitude did not affect RMR (p = 0.347). Conclusions RMR was transiently increased with ~1,800 m altitude exposure in female athletes and was unaffected by short-term LEA. However, the altitude-induced increase was small (~25-75 kcal/day) and was unlikely to have clinically significant implications for daily energy requirements.

Abstract:<p>An increasing number of people are living or working at high altitudes, which are often accompanied by cold environments. High altitude and cold exposure often result in physiological, perceptual and biochemical changes, thus affecting work efficiency and even human health. This study created different altitude conditions of 0 m, 2560 m, 3650 m, and 4290 m with an ambient temperature of -10 ℃ in a climate chamber. Six health subjects were recruited and human physiological and perceptual responses during this experiment were continuously investigated, involving objective parameters such as core temperature, local skin temperature, blood oxygen saturation, heart rate, respiratory rate and subjective parameters such as overall and local thermal sensation, overall and local thermal comfort. The statistical analysis showed that blood oxygen saturation is the most sensitive parameter to altitude changes, which is consistent with previous studies and shows a close relationship with altitude. All of the mean local skin temperatures in the face, hands and feet throughout the experimental period were lowest at 0 m altitude. The overall thermal sensation decreased by 2.83 units at 0 m, whereas it decreased by only 2.00 units at an altitude of 4290 m. The local thermal sensation correlated well with the local skin temperature (R<sup>2</sup> > 0.8) at 0 m, 2560 m and 3650 m. At an altitude of 4290 m, the local thermal sensations of the face, hands and feet were close to 0 (neutral). These findings indicate that higher altitude can compensate for some of the decreases in thermal sensation caused by a cold environment.</p>

Abstract:We have shown that hypoxia reduces plasma insulin, which correlates with corticotropin-releasing hormone (CRH) receptor 1 (CRHR1) in rats, but the mechanism remains unclear. Here, we report that hypobaric hypoxia at an altitude of 5,000 m for 8 h enhances rat plasma CRH, corticosterone, and glucose levels, whereas the plasma insulin and pancreatic ATP/ADP ratio is reduced. In islets cultured under normoxia, CRH stimulated insulin release in a glucose- and CRH-level-dependent manner by activating CRHR1 and thus the cAMP-dependent protein kinase pathway and calcium influx through L-type channels. In islets cultured under hypoxia, however, the insulinotropic effect of CRH was inactivated due to reduced ATP and cAMP and coincident loss of intracellular calcium oscillations. Serum and glucocorticoid-inducible kinase 1 (SGK1) also played an inhibitory role. In human volunteers rapidly ascended to 3,860 m, plasma CRH and glucose levels increased without a detectable change in plasma insulin. By contrast, volunteers with acute mountain sickness (AMS) exhibited a marked decrease in HOMA insulin sensitivity (HOMA-IS) and enhanced plasma CRH. In conclusion, hypoxia may attenuate the CRH-insulinotropic effect by reducing cellular ATP/ADP ratio, cAMP and calcium influx, and upregulated SGK1. Hypoxia may not affect HOMA-IS in healthy volunteers but reduces it in AMS volunteers.
Muscle Protein Turnover and the Molecular Regulation of Muscle Mass during Hypoxia

Abstract:: Effects of environmental hypoxia on fat-free mass are well studied. Negative energy balance, increased nitrogen excretion, and fat-free mass loss are commonly observed in lowlanders sojourning at high altitude. Reductions in fat-free mass can be minimized if energy consumption matches energy expenditure. However, in nonresearch settings, achieving energy balance during high-altitude sojourns is unlikely, and myofibrillar protein mass is usually lost, but the mechanisms accounting for the loss of muscle mass are not clear. At sea level, negative energy balance reduces basal and blunts postprandial muscle protein synthesis, with no relevant change in muscle protein breakdown. Downregulations in muscle protein synthesis and loss of fat-free mass during energy deficit at sea level are largely overcome by consuming at least twice the recommended dietary allowance for protein. Hypoxia may increase or not affect resting muscle protein synthesis, blunt postexercise muscle protein synthesis, and markedly increase proteolysis independent of energy status. Hypoxia-induced mTORC1 dysregulation and an upregulation in calpain- and ubiquitin proteasome-mediated proteolysis may drive catabolism in lowlanders sojourning at high altitude. However, the combined effects of energy deficit, exercise, and dietary protein manipulations on the regulation of muscle protein turnover have never been studied at high altitude. This article reviews the available literature related to the effects of high altitude on fat-free mass, highlighting contemporary studies that assessed the influence of altitude exposure (or hypoxia) on muscle protein turnover and intramuscular regulation of muscle mass. Knowledge gaps are addressed, and studies to identify effective and feasible countermeasures to hypoxia-induced muscle loss are discussed.
Seasonal Effects of High-Altitude Forest Travel on Cardiovascular Function: An Overlooked Cardiovascular Risk of Forest Activity

Abstract:Cardiovascular physiological responses involving hypoxemia in low temperature environments at high altitude have yet to be adequately investigated. This study aims to demonstrate the health effects of hypoxemia and temperature changes in cardiovascular functions (CVFs) by comparing intra-individual differences as participants ascend from low (298 m, 21.9 °C) to high altitude (2729 m, 9.5 °C). CVFs were assessed by measuring the arterial pressure waveform according to cuff sphygmomanometer of an oscillometric blood pressure (BP) device. The mean ages of participants in winter and summer were 43.6 and 41.2 years, respectively. The intra-individual brachial systolic, diastolic BP, heart rate, and cardiac output of participants significantly increased, as participants climbed uphill from low to high altitude forest. Following the altitude increase from 298 m to 2729 m, with the atmosphere gradually reducing by 0.24 atm, the measured average SpO2 of participants showed a significant reduction from 98.1% to 81.2%. Using mixed effects model, it is evident that in winter, the differences in altitude affects CVFs by significantly increases the systolic BP, heart rate, left ventricular dP/dt max and cardiac output. This study provides evidence that cardiovascular workload increased significantly among acute high-altitude travelers as they ascend from low to high altitude, particularly in winter.

Effects of altitude on performance of elite track-and-field athletes

Abstract:Purpose: Lower barometric air pressure at altitude can affect competitive performance of athletes in some sports. Reported here are the effects of various altitudes on elite track-and-field athletes' performance. Methods: Lifetime track-and-field performances of athletes placed in the top 16 in at least 1 major international competition between 2000 and 2009 were downloaded from the database at tilastopaja.org. There were 132,104 performances of 1889 athletes at 794 venues. Performances were log-transformed and analyzed using a mixed linear model with fixed effects for 6 levels of altitude and random quadratic effects to adjust for athlete age. Results: Men's and women's sprint events (100-400 m) showed marginal improvements of ~0.2% at altitudes of 500-999 m, and above 1500 m all but the 100- and 110-m hurdles showed substantial improvements of 0.3-0.7%. Some middle- and long-distance events (800-10,000 m) showed marginal impairments at altitudes above 150 m, but above 1000 m the impairments increased dramatically to ~2-4% for events >800 m. There was no consistent trend in the effects of altitude on field events up to 1000 m; above 1000 m, hammer throw showed a marginal improvement of ~1% and discus was impaired by 1-2%. Above 1500 m, triple jump and long jump showed marginal improvements of ~1%. Conclusions: In middle-and long-distance runners, altitudes as low as 150 to 299 m can impair performance. Higher altitudes (≥1000 m) are generally required before decreases in discus performance or enhancements in sprinting, triple and long jump, or hammer throw are seen.
Preconditioning by Phosphodiesterase‐5 Inhibition Improves Therapeutic Efficacy of Adipose‐Derived Stem Cells Following Myocardial Infarction in Mice

Abstract:The rationale of this article is enhancing the therapeutic potential of stem cells in ischemic microenvironments by novel preconditioning strategies is critical for improving cellular therapy. We tested the hypothesis that inhibition of phosphodiesterase‐5 (PDE‐5) with sildenafil (Viagra) or knockdown with a silencing vector in adipose‐derived stem cells (ASCs) would improve their survival and enhance cardiac function following myocardial implantation in vivo. ASCs were treated with sildenafil or PDE‐5 silencing vector short hairpin RNA (shRNAPDE‐5) and subjected to simulated ischemia/reoxygenation in vitro. Both sildenafil and shRNAPDE‐5 significantly improved viability, decreased necrosis, apoptosis, and enhanced the release of growth factors, vascular endothelial growth factor (VEGF), basic fibroblast growth factor (b‐FGF), and insulin‐like growth factor. Inhibition of protein kinase G reversed these effects. To show the beneficial effect of preconditioned ASCs in vivo, adult male CD‐1 mice underwent myocardial infarction. Preconditioned ASCs (4 × 105) were directly injected intramyocardially. Preconditioned ASC‐treated hearts showed consistently superior cardiac function when compared with nonpreconditioned ASCs after 4 weeks of treatment. This was associated with significantly reduced fibrosis, increased vascular density, and decreased resident myocyte apoptosis when compared with mice receiving nonpreconditioned ASCs. VEGF, b‐FGF, and Angiopoietin‐1 were also significantly elevated 4 weeks after cell therapy with preconditioned ASCs. We conclude that preconditioning by inhibition of PDE‐5 can be a powerful novel approach to improve stem cell therapy following myocardial infarction. STEM CELLS 2012; 30:326–335.
Effect of obesity and insulin resistance on resting and glucose-induced thermogenesis in man

Abstract:OBJECTIVE: to assess the impact of obesity and insulin sensitivity on resting (REE) and glucose-induced thermo-genesis (GIT).DESIGN: Data from 322 studies carried out in non-diabetic subjects of either gender, covering a wide range of age (18–80 y) and body mass index (BMI, 18–50 kg/m2).MEASUREMENTS: Insulin sensitivity and thermogenesis were measured by combining the euglycaemic insulin clamp technique with indirect calorimetry.RESULTS: REE was inversely related to age (P=0.001) and the respiratory quotient (P=0.03), and positively related to BMI, lean body mass (LBM), fat mass, and percentage fat mass (all P<0.0001). In a multiple regression model, LBM-adjusted REE was estimated to decline by 9% between 18 and 80 y, independently of obesity and insulin sensitivity. In contrast, GIT was strongly associated with insulin sensitivity (P<0.0001) but not with gender, age or BMI. By multiple regression analysis, GIT was linearly related to insulin sensitivity after controlling for gender, age, BMI and steady-state plasma insulin levels. Furthermore, both of the main components of insulin-mediated glucose disposal (glucose oxidation and glycogen synthesis) correlated with GIT independently of one another. In the subset of subjects (n=89) in whom waist-to-hip ratio (WHR) measurements were available, GIT was inversely associated with WHR (P<0.001 after adjustment by gender, age, BMI, insulin sensitivity and steady-state plasma insulin concentration). In this model, a significant interaction between WHR and gender indicated a stronger adverse effect on GIT of a high WHR in women than in men.CONCLUSIONS: In healthy humans, age, lean mass and respiratory quotient are the main independent determinants of resting thermogenesis. In contrast, insulin sensitivity and, to a lesser extent, abdominal obesity are the principal factors controlling glucose-induced thermogenesis.

Physiological responses to exercise at altitude : an update

Abstract:Studies performed over the past decade have yielded new information related to the physiological and metabolic adjustments made in response to both short- and long-term high-altitude exposure. These investigations have examined the potential mechanisms responsible for the alterations observed in such key variables as heart rate, stroke volume, cardiac output, muscle blood flow, substrate utilization and mitochondrial function, both at rest and during exercise of varying intensities. Additionally, the occurrence and mechanisms related to the 'lactate paradox' continues to intrigue investigators. It is apparent that exposure to high altitude is an environmental stressor that elicits a robust sympathoadrenal response that contributes to many of the critical adjustments and adaptations mentioned above. Furthermore, as some of these important physiological adaptations are known to enhance performance, it has become popular to incorporate an aspect of altitude living/training into the training regimens of endurance athletes (e.g. 'live high-train low'). Finally, it is important to note that many factors influence the extent to which individuals adjust and adapt to the stress imposed by exposure to high altitude. Included among these are (i) the degree of hypoxia; (ii) the duration of exposure to hypoxic conditions; (iii) the exercise intensity (absolute vs relative workload); and (iv) the inter-individual variability in adapting to hypoxic environments ('responders' vs 'non-responders').
Effect of Acute Altitude Exposure on Anaerobic Threshold Assessed by a Novel Electrocardiogram-Based Method

Abstract:Background: Acute altitude has a relevant impact on exercise physiology and performance. Therefore, the positive impact on the performance level is utilized as a training strategy in professional as well as recreational athletes. However, ventilatory thresholds (VTs) and lactate thresholds (LTs), as established performance measures, cannot be easily assessed at high altitudes. Therefore, a noninvasive, reliable, and cost-effective method is needed to facilitate and monitor training management at high altitudes. Methods: In a cross-sectional setting, a total of 14 healthy recreational athletes performed a graded cycling exercise test at sea level (Munich, Germany: 512 m/949 mbar) and high altitude (Zugspitze: 2,650 m/715 mbar). Anaerobic thresholds (ATs) were assessed using a novel method based on beat-to-beat repolarization instability (dT) detected by Frank-lead electrocardiogram (ECG) monitoring. The ECG-based ATs (ATdT°) were compared to routine LTs assessed according to Dickhuth and Mader. Results: After acute altitude exposure, a decrease in AT was detected using a novel ECG-based method (ATdT°: 159.80 ± 52.21 W vs. 134.66 ± 34.91 W). AtdT° levels correlated significantly with LT Dickhuth and LT Mader , at baseline (r Dickhuth/AtdT° = 0.979; p < 0.001) (r Mader/AtdT° = 0.943; p < 0.001), and at high altitude (r Dickhuth/AtdT° = 0.969; p < 0.001) (r Mader/AtdT° = 0.942; p < 0.001). Conclusion: Assessment of ATdT is a reliable method to detect performance alterations at altitude. This novel method may facilitate the training management of athletes at high altitudes.

Abstract:Purpose: The long-term development of talent in endurance sports is a topic of interest. Among various factors, the importance of total hemoglobin mass (tHbmass) and the potential benefits of being an altitude-native athlete remain unclear, particularly in young categories. This study aimed to investigate the impact of altitude and training content on hematological characteristics by comparing young male cyclists age 15–16 and 17–18 years who were born and trained at a moderate altitude (ie, greater than or equal to 2500 m; MA) and cyclists who were born and trained at low altitude (below 1000 m; LA). Methods: tHbmass (in grams and grams per kilogram), measured by using the optimized carbon monoxide rebreathing method during an incremental test on a cycle ergometer; hematocrit percentage, hemoglobin concentration; and erythrocyte, blood, and plasma volume were measured in youth male cyclists age 15–16 years and junior cyclists age 17–18 years who were born and trained at MA versus LA. All variables were analyzed with a 2-way (age [youth cyclist vs junior cyclist] × altitude level [MA vs LA]) analysis of variance with subsequent Tuckey post hoc test. Abstract:OBJECTIVE Limited information is available regarding the metabolic effects of high altitude trekking in patients with type 2 diabetes. RESEARCH DESIGN AND METHODS Thirteen individuals with type 2 diabetes took part in a 12-day expedition to the summit of Mount Toubkal (altitude, 4,167 m), Morocco, after 6 months of exercise training. Energy expenditure, body weight, blood glucose, fasting insulin, lipids, and HbA1c were assessed. RESULTS Training reduced fasting glucose (−0.7 ± 0.9 mmol/L, P = 0.026) and increased exercise capacity (+0.3 ± 0.3 W/kg, P = 0.005). High altitude trekking decreased fasting insulin concentrations (−3.8 ± 3.2 μU/L, P = 0.04), total cholesterol (−0.7 ± 0.8 mmol/L, P = 0.008), and LDL cholesterol (−0.5 ± 0.6 mmol/L, P = 0.007). CONCLUSIONS High altitude trekking preceded by exercise training is feasible for patients with type 2 diabetes. It improves blood glucose, lipids, and fasting insulin concentrations, while glucose control is maintained.

Abstract:Altitude training has become an important training application for athletes due its potential for altering physiology and enhancing performance. This practice is commonly used by athletes, with a popular choice being the live high - train low approach. This model recommends that athletes live at high altitude (1250-3000 m), but train at low altitude or sea-level (0-1200 m). Exposure to altitude often leads to hypoxic stress and in turn stimulates changes in total haemoglobin mass, erythropoietin, and soluble transferrin receptors, which alter further underlying physiology. Through enhanced physiology, improved exercise performance may arise through enhancement of the oxygen transport system which is important for endurance events. Previous investigations into the effects of altitude training on exercise performance have been completed in a range of contexts, including running, cycling, swimming, and triathlon. Often following a LHTL altitude intervention, athletes realise improvements in maximal oxygen consumption capacity, time trial performance and peak power outputs. Although heterogeneity exists among LHTL methodologies, i.e., exposure durations and altitude ranges, we synthesised this data into kilometre hours, and found that the most common hypoxic doses used in LHTL interventions ranged from ∼578-687 km h. As this narrative review demonstrates, there are potential advantages to using altitude training to enhance physiology and improve performance for endurance athletes.

Abstract:IEEE 802.15.4b is an enhanced standard to the IEEE 802.15.4, addressing the needs for high-data rate transmission using 868 or 915 MHz band in the wireless personal area networks. This paper details a modulation and code mapping scheme designed specifically for the standard's 868 MHz band that leads to efficient transmission and low implementation complexity. The code mapping scheme is called quadrature cyclic odd bit inversion length 16 or quad COBI-16. Among the scheme's desirable properties is that the total phase rotation in the I/Q plane accumulates to O degree over the complete symbol. The scheme is tested in AWGN and Rayleigh fading channels.
