Abstract:Long-term Electrocardiogram (ECG) analysis has become a common means of diagnosing cardiovascular diseases. In order to reduce the workload of cardiologists and accelerate diagnosis, an automated patient-specific heartbeat classification method based on a customized convolutional neural network (CNN) is proposed in this paper. The parallel convolutional layers with kernels of different receptive fields in the network are responsible for extracting multi-spatial deep features of the heartbeats, and the channel-wise attention module is adopted to selectively emphasize the informative features, which are beneficial to distinguish different classes of beats. To facilitate the extraction and emphasis of the important features, each heartbeat is segmented and stacked to form the multi-channel network input according to the basic temporal characteristics of the main components (P wave, QRS complex, and T wave) in the ECG. Besides, to further improve the network generalization and achieve better performance on various ECG of new patients, a method of intra-record sample clustering is proposed to select the representative heartbeats to construct the training set. The proposed method classifies heartbeats into five classes (normal beat (N), supraventricular ectopic beat (SVEB), ventricular ectopic beat (VEB), fusion beat (F), and unclassifiable beat (Q)). Validated on the MIT-BIH arrhythmia database, our approach demonstrates performance superior to several state-of-the-art methods. In addition to an average accuracy and specificity of over 99%, this method achieves a sensitivity of 95.4% and a positive predictivity of 97.1% for VEB class, and a sensitivity of 81.1% and a positive predictivity of 90.0% for SVEB class. With high classification performance and pathological heartbeat detection accuracy, the proposed method is promising for clinical device applications.

Abstract:Accurate reconstruction and imaging of cardiac transmembrane potential through body surface ECG signals can provide great help for the diagnosis of heart disease. In this paper, a cardiac transmembrane potential reconstruction method (GISTA-Net) based on graph convolutional neural network and iterative soft threshold algorithm is proposed. It fully combines the rigor of mathematical derivation of traditional iterative threshold shrinkage algorithm and the powerful expression ability of deep learning method, as well as the characterization ability of graph convolutional neural network for non-Euclidean space data. We used this algorithm to simulate ectopic pacing data and simulated myocardial infarction data. The experimental results show that this algorithm can not only accurately locate the ectopic pacing point, but also accurately reconstruct the edge details of myocardial infarction scar while graph convolution makes full use of the connection information between the nodes on the heart surface.
Patient-Specific ECG Classification with Integrated Long Short-Term Memory and Convolutional Neural Networks

Abstract:This paper presents an automated patient-specific ECG classification algorithm, which integrates long short-term memory (LSTM) and convolutional neural networks (CNN). While LSTM extracts the temporal features, such as the heart rate variance (HRV) and beat-to-beat correlation from sequential heartbeats, CNN captures detailed morphological characteristics of the current heartbeat. To further improve the classification performance, adaptive segmentation and re-sampling are applied to align the heartbeats of different patients with various heart rates. In addition, a novel clustering method is proposed to identify the most representative patterns from the common training data. Evaluated on the MIT-BIH arrhythmia database, our algorithm shows the superior accuracy for both ventricular ectopic beats (VEB) and supraventricular ectopic beats (SVEB) recognition. In particular, the sensitivity and positive predictive rate for SVEB increase by more than 8.2% and 8.8%, respectively, compared with the prior works. Since our patient-specific classification does not require manual feature extraction, it is potentially applicable to embedded devices for automatic and accurate arrhythmia monitoring.
EGCNet: a Hierarchical Graph Convolutional Neural Network for Improved Classification of Electrocardiograms

Abstract:The automatic classification of electrocardiograms (ECGs) plays a crucial role in the early diagnosis of cardiovascular diseases. In recent research, deep neural network (DNN)-based methods have garnered significant attention due to their exceptional feature extraction capabilities. However, these methods face challenges in dealing with the complex inter- and intra-class dependencies inherent in different arrhythmias. We propose a classification model for electrocardiograms based on the Graph Convolutional Neural Network (EGCNet) to address this. In our EGCNet framework, we first employ the row and column 1-dimensional residual convolution to learn intra-class dependency. Then, we construct a hierarchical graph structure to model the inter-class dependency. In such a graph, each node represents one type of disease, and the edges describe the relationship between diseases. To reduce the complexity of inter-class dependency, we build a hierarchical graph structure by treating similar nodes as the same type. Thus, we can learn the discriminant dependency in the higher-level graph space and feed this high-level knowledge back to the lower-level graph space. Finally, we evaluated our method using publicly available datasets from the PhysioNet/Computing Challenge 2020 ECG data. For instance, our model achieved robust performance in the 24-label experiment with a precision of 0.753 and an F1 score of 0.722.
A coordinated adaptive multiscale enhanced spatio-temporal fusion network for multi-lead electrocardiogram arrhythmia detection

Abstract:The multi-lead electrocardiogram (ECG) is widely utilized in clinical diagnosis and monitoring of cardiac conditions. The advancement of deep learning has led to the emergence of automated multi-lead ECG diagnostic networks, which have become essential in the fields of biomedical engineering and clinical cardiac disease diagnosis. Intelligent ECG diagnosis techniques encompass Recurrent Neural Networks (RNN), Transformers, and Convolutional Neural Networks (CNN). While CNN is capable of extracting local spatial information from images, it lacks the ability to learn global spatial features and temporal memory features. Conversely, RNN relies on time and can retain significant sequential features. However, they are not proficient in extracting lengthy dependencies of sequence data in practical scenarios. The self-attention mechanism in the Transformer model has the capability of global feature extraction, but it does not adequately prioritize local features and cannot extract spatial and channel features. This paper proposes STFAC-ECGNet, a model that incorporates CAMV-RNN block, CBMV-CNN block, and TSEF block to enhance the performance of the model by integrating the strengths of CNN, RNN, and Transformer. The CAMV-RNN block incorporates a coordinated adaptive simplified self-attention module that adaptively carries out global sequence feature retention and enhances spatial-temporal information. The CBMV-CNN block integrates spatial and channel attentional mechanism modules in a skip connection, enabling the fusion of spatial and channel information. The TSEF block implements enhanced multi-scale fusion of image spatial and sequence temporal features. In this study, comprehensive experiments were conducted using the PTB-XL large publicly available ECG dataset and the China Physiological Signal Challenge 2018 (CPSC2018) database. The results indicate that STFAC-ECGNet surpasses other cutting-edge techniques in multiple tasks, showcasing robustness and generalization.
Automatic Recognition of Coronary Artery Disease and Congestive Heart Failure Using a Multi-Granularity Cascaded Hybrid Network

Abstract:Numerous researchers have developed electrocardiogram (ECG) classification systems to automatically diagnose coronary artery disease (CAD) and congestive heart failure (CHF). However, they have failed to assess the applicability of their proposed methods on inter-patient, noisy, imbalanced and small-scale data, limiting their use in real-world situations. To address these issues, a multi-granularity cascaded hybrid network (MGCH-Net) is proposed to automatically identify abnormal ECGs. MGCH-Net mainly consists of a multi-granularity cascaded task-related component analysis (TRCA)-principal component analysis (PCA) network (MGC-TPNet), a multi-granularity cascaded independent component analysis (ICA)-PCA network (MGC-IPNet), and a cascaded weighted averaging and Dempster–Shafer (CWA-DS) method. Among them, MGC-TPNet extracts multi-scale inter-lead correlation features from multi-lead ECGs, by which the information from the sagittal and horizontal planes of the heart is comprehensively utilized by maximizing the reproducibility under the same task. Then, MGC-IPNet mines multi-scale intra-lead specificity features from more-valuable-lead ECGs. Finally, CWA-DS is employed to fuse linear support vector machine (SVM)- and softmax-based decision probabilities for the above features to obtain the final results. Since the convolutional kernels of MGCH-Net can be directly calculated through TRCA, PCA, and FastICA algorithms, multiple iterative processes are not required. Hence, MGCH-Net has a faster training speed than most convolutional neural networks. In this work, accuracies of 99.92% and 97.46% were achieved in recognizing normal, CAD and CHF heartbeats in intra- and inter-patient experiments, respectively. In addition, MGCH-Net performed well on imbalanced, noisy, and small-scale data. These results show that our method outperforms the state-of-the-art methods.

Abstract:In recent years, there has been an increase in the number of deaths caused by heart disease. The design of computer-aided systems for the diagnosis of heart diseases, especially myocardial infarction (MI), has become a hot topic. The diagnosis of MI is a classification task. Electrocardiogram (ECG) is one of the most important tools for diagnosing heart disease. ECG signals are classified into corresponding MI categories based on their extracted features. Thus, the diagnosis of MI depends on the feature extraction of the ECG signal. Because signal processing methods, such as wavelet transform, may not effectively extract hidden features, researchers nowadays turn to using convolutional neural networks to solve the problem of hidden feature extraction. However, this approach ignores the role of temporal information. we proposed a new network called Multi-scale SE-Residual Network with Transformer encoder (MRTNet) to extract hidden features and temporal information features better. The data processed are localized to heartbeat units based on the way cardiologists diagnose myocardial infarction. Inspired by multi-scale learning, the sampling module was designed to acquire heartbeat units at different scales. We provided a processing model for multi-scale data, called the feature extraction module. The residual network with SE block and Customized Pooling Component (CPC) was used to extract global and local features of the heartbeat units. A Transformer encoder was used to extract the common features of the previous module’s output. The common features at different scales were fused to provide the basis for the classification task. The experiment was conducted on the public PTB-XL dataset. MRTNet achieved a higher accuracy and F1 score than other models by approximately 2%. Furthermore, MRTNet demonstrated a superior AUC of approximately 0.77. It is a superior classifier when compared to other models. The results demonstrate the effectiveness of the designed multi-scale architecture. And it provides a way into exploring further potential information from ECG signals.
12-Lead ECG signal classification for detecting ECG arrhythmia via an information bottleneck-based multi-scale network

Abstract:The 12-lead electrocardiogram (ECG) is a reliable diagnostic tool for detecting and treating severe cardiovascular conditions like arrhythmia and heart attack. Deep neural networks (DNNs) have achieved higher accuracy in recent years than traditional ECG signal classification task methods. Convolutional neural network (CNN) and Transformer are the two mainstream architectures of DNN, respectively good at extracting local and global features from input data. This paper proposes the multi-scale convolutional Transformer network (MCTnet), an efficient combination of Transformer encoder and CNN for ECG signal classification. MCTnet utilizes the advantages of CNN and self-attention mechanisms to capture potential features in ECG signal accurately. The dual-branch Transformer encoder extracts different-scale feature representations, enabling the capture of both local and global information. Additionally, an information bottleneck method eliminates redundant information and enhances task-relevant information in the learned representations. To evaluate the performance of MCTnet, comprehensive experiments are conducted on three commonly used ECG datasets. The results demonstrate that MCTnet outperforms current deep learning-based models, highlighting its effectiveness in ECG signal classification. It also shows that the performance of the model can be effectively improved by utilizing multi-scale representation learning and information bottleneck.

Abstract:The timely detection of abnormal electrocardiogram (ECG) signals is vital for preventing heart disease. However, traditional automated cardiology diagnostic methods have the limitation of being unable to simultaneously identify multiple diseases in a segment of ECG signals, and do not consider the potential correlations between the 12-lead ECG signals. To address these issues, this paper presents a novel network architecture, denoted as Branched Convolution and Channel Fusion Network (BCCF-Net), designed for the multi-label diagnosis of ECG cardiology to achieve simultaneous identification of multiple diseases. Among them, the BCCF-Net incorporates the Channel-wise Recurrent Fusion (CRF) network, which is designed to enhance the ability to explore potential correlation information between 12 leads. Furthermore, the utilization of the squeeze and excitation (SE) attention mechanism maximizes the potential of the convolutional neural network (CNN). In order to efficiently capture complex patterns in space and time across various scales, the multi branch convolution (MBC) module has been developed. Through extensive experiments on two public datasets with seven subtasks, the efficacy and robustness of the proposed ECG multi-label classification framework have been comprehensively evaluated. The results demonstrate the superior performance of the BCCF-Net compared to other state-of-the-art algorithms. The developed framework holds practical application in clinical settings, allowing for the refined diagnosis of cardiac arrhythmias through ECG signal analysis.

An Electrocardiogram Classification Using a Multiscale Convolutional Causal Attention Network

Abstract:Electrocardiograms (ECGs) play a pivotal role in the diagnosis and prediction of cardiovascular diseases (CVDs). However, traditional methods for ECG classification involve intricate signal processing steps, leading to high design costs. Addressing this concern, this study introduces the Multiscale Convolutional Causal Attention network (MSCANet), which utilizes a multiscale convolutional neural network combined with causal convolutional attention mechanisms for ECG signal classification from the PhysioNet MIT-BIH Arrhythmia database. Simultaneously, the dataset is balanced by downsampling the majority class and oversampling the minority class using the Synthetic Minority Oversampling Technique (SMOTE), effectively categorizing the five heartbeat types in the test dataset. The experimental results showcase the classifier's performance, evaluated through accuracy, precision, sensitivity, and F1-score and culminating in an overall accuracy of 99.35%, precision of 96.55%, sensitivity of 96.73%, and an F1-recall of 96.63%, surpassing existing methods. Simultaneously, the application of this innovative data balancing technique significantly addresses the issue of data imbalance. Compared to the data before balancing, there was a significant improvement in accuracy for the S-class and the F-class, with increases of approximately 8% and 13%, respectively.

Abstract:Myocardial infarction (MI) is a life-threatening cardiac disease. Electrocardiogram (ECG) is crucial in the clinical diagnosis of MI, facilitating early localisation and diagnosis, guiding timely intervention, and improving patient outcomes. However, most previous studies required additional denoising operations and did not propose an effective method to reduce differences between patients. Therefore, this paper presents a multi-channel dense attention neural network (MCDANN) for MI detection and localisation using 12-lead ECG signals without denoising preprocessing. MCDANN is composed of 12 parallel channels dedicated to the automatic extraction of heartbeat features. The extracted features from all channels are then aggregated for MI detection and localisation. Each channel consists primarily of the dense layers and the squeeze-and-excitation (SE) module. The dense layers enhance the reuse of feature information within the leads, whereas the SE module weakens less critical information in the ECG classification task. We conduct inter-patient and intra-patient experiments to evaluate the MCDANN model based on the PTB diagnostic database. For MI detection, MCDANN achieved 99.94% accuracy and 99.92% F1 score in intra-patient experiments and 98.27% accuracy and 96.84% F1 score in inter-patient experiments. For MI localisation, MCDANN achieved 99.85% accuracy and 99.84% F1 score in intra-patient experiments, and 81.70% accuracy and 80.73% F1 score in inter-patient experiments. Based on the experimental findings, it is clear that the MCDANN model exhibits remarkable performance in MI detection and localisation, both for intra-patient and inter-patient cases. Notably, it substantially enhances the performance of inter-patient MI localisation compared to other existing methods.

Abstract:A novel structure based on channel-wise attention mechanism is presented in this paper. Embedding with the proposed structure, an efficient classification model that accepts multi-lead electrocardiogram (ECG) as input is constructed. One-dimensional convolutional neural networks (CNNs) have proven to be effective in pervasive classification tasks, enabling the automatic extraction of features while classifying targets. We implement the residual connection and design a structure which can learn the weights from the information contained in different channels in the input feature map during the training process. An indicator named mean square deviation is introduced to monitor the performance of a particular model segment in the classification task on the two out of the five ECG classes. The data in the MIT-BIH arrhythmia database is used and a series of control experiments is conducted. Utilizing both leads of the ECG signals as input to the neural network classifier can achieve better classification results than those from using single channel inputs in different application scenarios. Models embedded with the channel-wise attention structure always achieve better scores on sensitivity and precision than the plain Resnet models. The proposed model exceeds the performance of most of the state-of-the-art models in ventricular ectopic beats (VEB) classification and achieves competitive scores for supraventricular ectopic beats (SVEB). Adopting more lead ECG signals as input can increase the dimensions of the input feature maps, helping to improve both the performance and generalization of the network model. Due to its end-to-end characteristics, and the extensible intrinsic for multi-lead heart diseases diagnosing, the proposed model can be used for the realtime ECG tracking of ECG waveforms for Holter or wearable devices.
CRT-Net: A Generalized and Scalable Framework for the Computer-Aided Diagnosis of Electrocardiogram Signals

Abstract:Electrocardiogram (ECG) signals play critical roles in the clinical screening and diagnosis of many types of cardiovascular diseases. Despite deep neural networks that have been greatly facilitated computer-aided diagnosis (CAD) in many clinical tasks, the variability and complexity of ECG in the clinic still pose significant challenges in both diagnostic performance and clinical applications. In this paper, we develop a robust and scalable framework for the clinical recognition of ECG. Considering the fact that hospitals generally record ECG signals in the form of graphic waves of 2-D images, we first extract the graphic waves of 12-lead images into numerical 1-D ECG signals by a proposed bi-directional connectivity method. Subsequently, a novel deep neural network, namely CRT-Net, is designed for the fine-grained and comprehensive representation and recognition of 1-D ECG signals. The CRT-Net can well explore waveform features, morphological characteristics and time domain features of ECG by embedding convolution neural network(CNN), recurrent neural network(RNN), and transformer module in a scalable deep model, which is especially suitable in clinical scenarios with different lengths of ECG signals captured from different devices. The proposed framework is first evaluated on two widely investigated public repositories, demonstrating the superior performance of ECG recognition in comparison with state-of-the-art. Moreover, we validate the effectiveness of our proposed bi-directional connectivity and CRT-Net on clinical ECG images collected from the local hospital, including 258 patients with chronic kidney disease (CKD), 351 patients with Type-2 Diabetes (T2DM), and around 300 patients in the control group. In the experiments, our methods can achieve excellent performance in the recognition of these two types of disease.

Abstract:Electrocardiogram (ECG) is an efficient and simple method for the diagnosis of cardiovascular diseases and has been widely used in clinical practice. Because of the shortage of professional cardiologists and the popularity of electrocardiograms, accurate and efficient arrhythmia detection has become a hot research topic. In this paper, we propose a new multi-task deep neural network, which includes a shared low-level feature extraction module (i.e., SE-ResNet) and a task-specific classification module. Contextual Transformer (CoT) block is introduced in the classification module to dynamically model the local and global information of ECG feature sequence. The proposed method was evaluated on public CPSC2018 and PTB-XL datasets and achieved an average F1 score of 0.827 on the CPSC2018 dataset and an average F1 score of 0.833 on the PTB-XL dataset.

Abstract:In clinical neurology practice, the classification of intracranial electroencephalography (iEEG) recordings into artifacts, pathological, and physiological activities has traditionally been performed by expert visual review, which is a difficult, time-consuming, and subjective process. Recently, deep learning methods have shown remarkable success in automatic iEEG recognition. However, the current convolutional neural network (CNN)-based methods for iEEG signal analysis only focus on extracting local features, while largely disregarding the global context information. To address this limitation, we propose a novel hierarchical CNN-Transformer combined network for iEEG classification, named IEEG-HCT. The proposed model first employs a CNN Stem to extract preliminary local features, which are subsequently fed into a hierarchical alternating structure comprising of convolutional embedding (CE) blocks and Transformer blocks. Our explorations of combining CNN and Transformer models are conducted at both the macro and micro levels, allowing the resulting model to effectively capture both local features and long-distance dependencies. At the macro level, we utilize CE blocks to extract local features and reduce intermediate feature size, while at the micro level, we employ appropriate convolutions to enhance Transformer blocks. In addition, the hierarchical architecture allows for the extraction of multiscale features. We evaluated the proposed IEEG-HCT model on the multicenter iEEG dataset using out-of-institution and cross-subject validations. Experimental results demonstrate that the proposed model outperforms most existing models in all experimental settings.

Automatic detection of arrhythmia based on ECG plays a critical role in the early prevention and diagnosis of CVDs. In recent years, numerous studies have focused on using deep learning methods to address arrhythmia classification problems. However, the transformer-based neural network in current research still has a limited performance in detecting arrhythmias for the multi-lead ECG. In this study, we propose an end-to-end multi-label arrhythmia classification model for the 12-lead ECG with varied-length recordings. Our model, called CNN-DVIT, is based on a combination of convolutional neural networks (CNNs) with depthwise separable convolution, and a vision transformer structure with deformable attention. Specifically, we introduce the spatial pyramid pooling layer to accept varied-length ECG signals. Experimental results show that our model achieved an F1 score of 82.9% in CPSC-2018. Notably, our CNN-DVIT outperforms the latest transformer-based ECG classification algorithms. Furthermore, ablation experiments reveal that the deformable multi-head attention and depthwise separable convolution are both efficient in extracting features from multi-lead ECG signals for diagnosis. The CNN-DVIT achieved good performance for the automatic arrhythmia detection of ECG signals. This indicates that our research can assist doctors in clinical ECG analysis, providing important support for the diagnosis of arrhythmia and contributing to the development of computer-aided diagnosis technology.

Abstract:Although convolutional neural networks (CNNs) can be used to classify electrocardiogram (ECG) beats in the diagnosis of cardiovascular disease, ECG signals are typically processed as one-dimensional signals while CNNs are better suited to multidimensional pattern or image recognition applications. In this study, the morphology and rhythm of heartbeats are fused into a two-dimensional information vector for subsequent processing by CNNs that include adaptive learning rate and biased dropout methods. The results demonstrate that the proposed CNN model is effective for detecting irregular heartbeats or arrhythmias via automatic feature extraction. When the proposed model was tested on the MIT-BIH arrhythmia database, the model achieved higher performance than other state-of-the-art methods for five and eight heartbeat categories (the average accuracy was 99.1% and 97%). In particular, the proposed system had better performance in terms of the sensitivity and positive predictive rate for V beats by more than 4.3% and 5.4%, respectively, and also for S beats by more than 22.6% and 25.9%, respectively, when compared to existing algorithms. It is anticipated that the proposed method will be suitable for implementation on portable devices for the e-home health monitoring of cardiovascular disease.

Enhancing Cardiac MRI Segmentation via Classifier-Guided Two-Stage Network and All-Slice Information Fusion Transformer

Abstract:Cardiac Magnetic Resonance imaging (CMR) is the gold standard for assessing cardiac function. Segmenting the left ventricle (LV), right ventricle (RV), and LV myocardium (MYO) in CMR images is crucial but time-consuming. Deep learning-based segmentation methods have emerged as effective tools for automating this process. However, CMR images present additional challenges due to irregular and varying heart shapes, particularly in basal and apical slices. In this study, we propose a classifier-guided two-stage network with an all-slice fusion transformer to enhance CMR segmentation accuracy, particularly in basal and apical slices. Our method was evaluated on extensive clinical datasets and demonstrated better performance in terms of Dice score compared to previous CNN-based and transformer-based models. Moreover, our method produces visually appealing segmentation shapes resembling human annotations and avoids common issues like holes or fragments in other models' segmentations.

Abstract:Abstract Background Heart disease diagnosis is a challenging task and it is important to explore useful information from the massive amount of electrocardiogram (ECG) records of patients. The high-precision diagnostic identification of ECG can save clinicians and cardiologists considerable time while helping reduce the possibility of misdiagnosis at the same time.Currently, some deep learning-based methods can effectively perform feature selection and classification prediction, reducing the consumption of manpower. Methods In this work, an end-to-end deep learning framework based on convolutional neural network (CNN) is proposed for ECG signal processing and arrhythmia classification. In the framework, a transformer network is embedded in CNN to capture the temporal information of ECG signals and a new link constraint is introduced to the loss function to enhance the classification ability of the embedding vector. Results To evaluate the proposed method, extensive experiments based on real-world data were conducted. Experimental results show that the proposed model achieve better performance than most baselines. The experiment results also proved that the transformer network pays more attention to the temporal continuity of the data and captures the hidden deep features of the data well. The link constraint strengthens the constraint on the embedded features and effectively suppresses the effect of data imbalance on the results. Conclusions In this paper, an end-to-end model is used to process ECG signal and classify arrhythmia. The model combine CNN and Transformer network to extract temporal information in ECG signal and is capable of performing arrhythmia classification with acceptable accuracy. The model can help cardiologists perform assisted diagnosis of heart disease and improve the efficiency of healthcare delivery.
