Abstract:BACKGROUND:Several host characteristics, including pigmentary traits (hair color, sunburn susceptibility and tanning ability), number of common nevi (moles), and family history of melanoma, have been associated with risk of melanoma. METHODS:We prospectively examined the associations between host characteristics and risk of incident melanoma by Breslow thickness (≤1 mm, thin melanoma; or >1 mm, "thicker melanoma") based on the Nurses' Health Study (NHS, n = 86,380 women), NHS II (n = 104,100 women), and Health Professionals Follow-up Study (HPFS, n = 46,934 men). RESULTS:During 22-30 years' follow-up, a total of 1,813 incident melanoma cases were identified with information on Breslow thickness, 1,392 (76.8%) of which had thin melanoma. No significant differences were observed for thin and thicker melanoma in associations with hair color, sunburn susceptibility, and tanning ability. However, we found significant differences for the association with family history of melanoma, with a higher risk estimate for thicker melanoma [HR = 2.55; 95% confidence interval (CI): 1.91-3.42] than thin melanoma (HR = 1.59; 95% CI: 1.21-2.08; P heterogeneity = 0.02). Interestingly, women and men displayed differential associations between nevi count and risk of melanoma by Breslow thickness, with the association appearing stronger for thicker melanoma than thin melanoma in men (P heterogeneity = 0.01), but not in women. CONCLUSIONS:Individuals with family history of melanoma may be more likely to develop thicker melanoma. Men with high number of common nevi may tend to develop thicker melanoma, which was not found for women. IMPACT:The findings further stress the risk of thicker melanoma for individuals with a family history of melanoma and men with a high nevi count.

Abstract:Abstract Two contrasting forms of clinically evident photoageing have been described – hypertrophic and atrophic. Recent studies postulate that hypertrophic photoageing, described as a leathery appearance with coarse wrinkles, may be protective and mitigate skin cancer. Conversely, atrophic skin ageing is described as translucent skin with telangiectasia and skin thinning and is thought to be associated with actinic damage and an increased incidence of skin cancer. Full-face photographs of 102 patients who underwent Mohs surgery for basal cell carcinomas of the cheek during 2018–2023 were selected, and their hypertrophic and atrophic photodamage scored by two dermatologists and a nonmedic independently (score of 0–8, in increasing severity). All histopathology from 102 patients was reviewed. Two of the 102 patients were omitted due to poor-quality slides. Solar elastosis was scored based on the percentage of dermis affected (grade 1 < 50%, grade 2 50–90%, grade 3 100%). Epidermal thickness was measured from an inter-rete ridge region halfway along each specimen. In total, 53% of patients were female and 47% were male; 49% had Fitzpatrick skin type I, 49% type II, and 2% type III. Overall, 89% of patients did not smoke, 9% currently smoked and 2% formerly smoked. Only 3% of patients were immunosuppressed. Five patients (5%) had an atrophic score of 2, 29% of 4, 49% of 6, and 17% of 8. Overall, 22% of patients had a hypertrophic score of 0, 58% of 2, 18% of 4, and 1% of 6. Eighty-eight patients (88%) had a solar elastosis score of 3. Of the patients with severe solar elastosis 70% had an atrophic photoageing score graded as severe (6 or 8). There was no association between sex and atrophic photoageing score. There was an association between solar elastosis grade and atrophic photoageing score. The mean epidermal thickness was lower in patients with a higher atrophic photoageing score (sum of squares 43.6, P = 0.002). The mean epidermal thickness was lower in patients with higher hypertrophic photoageing scores; however, this result was not statistically significant. There was a discrepancy between atrophic scoring and a concordance for hypertrophic scoring between the dermatologist and nonmedical personnel. Epidermal thickness was reduced regardless of sex in both atrophic and hypertrophic photodamaged skin. Most patients in our study had significant solar elastosis in their dermis, although this appeared to be more severe in those with higher atrophic photoageing scores. We postulate that photoageing changes are complex but can be depicted clinically. It is unclear whether dividing patients into atrophic and hypertrophic categories can distinguish histological changes and overall skin cancer risk.

DP23 Stage presentation of melanomas arising in the back: a 20-year retrospective study of site-specific distribution and review of the literature

Abstract:Abstract This is a study over 20 years to assess the pathological stage of presentation for melanomas arising in the back. We have reviewed studies for presentation of body-site-specific melanoma to compare against our findings and to see whether there has been any significant improvement in the rate of early detection. We extracted and analysed data from archived histopathology reports on the WinPath system by running a SNOMED code search on all melanomas reported from 2001 to 2011 and selected those affecting the back. The age of presentation varied from 29 to 94 years (mean 58.7). Twenty patients were male and seven female (male-to-female ratio 2.9 : 1). Most cases presented as American Joint Committee on Cancer 8th Edition stage pT1 (11 were 1a, 2 were 1b), pT2 (n = 2), pT3 (n = 4) and pT4 (n = 2). Six were in situ melanomas, mostly lentigo maligna or arising from severely dysplastic naevi. Breslow depth ranged from 0.1 to 6.5 mm (mean 1.6 mm). The cases were mostly from the upper back (n = 10). The melanoma types were superficial spreading (n = 14), nodular (n = 2), lentigo maligna (n = 3) and melanoma in situ from dysplastic naevi (n = 3). On reviewing the literature, we found one study suggesting that melanomas in visible areas present earlier. Another concluded that a melanoma is more often self-detected in female patients, explained by the high proportion of lesions in men presenting on the back. Further research suggests that older patients recognize melanomas at a lower rate than younger patients. Older men frequently have lesions in difficult-to-see areas such as the scalp and the back, and therefore also may be limited to self-notice new or changing lesions. The role of family members is as important in early detection as that of trained health professionals. As a delay in diagnosis may be responsible for the thicker melanomas in hidden areas, preventive and screening programmes should stress the importance of not neglecting these locations in self-examination. Special attention should be given to educating older men. To conclude, we have presented a laboratory-based study on the presenting pathological stage of melanoma of the back in patients aged > 20 years. Half of our patients presented as stage pT1, while the other half of the tumours included in situ melanoma to stage pT4. Men predominated over women (ratio 2.9 : 1). There is no screening programme for melanoma, and providing education that raises awareness of the early signs and patient self-examination with opportunistic screening is the current standard. As the back area is not routinely inspected by patients, we believe that late presentation should be avoidable by recommending whole-body examination in general practice and dermatology clinics to ensure early detection and better prognosis of melanomas arising in the back and other hidden anatomical sites.

Invasive Scalp Melanoma: Role for Enhanced Detection Through Professional Training

Abstract:Background: Scalp and neck melanomas (SNMs) have a relatively poor prognosis compared to other sites, and represent an anatomically challenging area for detection. The aim of this study was to identify the role of the hairdresser in detection of SNMs. Methods: A tertiary surgical oncology institutional database was retrospectively reviewed for all patients undergoing resection of a scalp, posterior neck, or retro auricular invasive primary melanoma between 2008 and 2014. Results: SNMs accounted for 128 melanoma patients during the study period, with median age 66 years, 88 % male, and median Breslow thickness 1.55 mm. Hairdressers detected 10 % of all SNMs, with hairdresser-detected SNMs presenting 13 years younger (53 vs. 66 years, P = 0.015), and with a trend towards lower Breslow depth (1.15 vs. 1.63) and more frequent discovery in AJCC Stage Ia or Ib (66.7 % vs. 44.8 %) than otherwise-detected SNMs. Women with SNMs were 1.8-fold more likely than men to have their SNMs detected by a hairdresser (P = 0.001), and presented at higher AJCC clinical stage than men and required wider surgical resection margins (P = 0.011). Women with hairdresser-detected SNMs were younger, with lower Breslow thickness and lower AJCC Clinical Stage than women with otherwise-detected SNM. Conclusion: This study suggests that hairdressers play a critical role in detection of invasive primary scalp and neck melanoma, accounting for 10 % of all melanomas referred to a tertiary surgical oncology center. Quality improvement initiatives aimed at increasing early detection of scalp and neck melanoma should include members of the cosmetology community.

Abstract:Background Dermoscopy is a non‐invasive, cost‐effective tool useful in the assessment of pigmented lesions. The aim of this study was to analyze the dermoscopic features in excised melanomas in a tertiary hospital in Brazil and the influence of Breslow thickness on the dermoscopic features observed. Methods An observational, cross‐sectional study in which dermoscopic images of 43 melanomas were evaluated. The patients were divided into three groups: in situ, thin invasive melanoma (Breslow thickness <1 mm), and thick invasive melanoma (Breslow thickness ≥1 mm). Results We studied 33 patients with 43 melanomas. Twenty‐one (48.8%) were melanoma in situ, 16 (37.2%) were thin melanomas, and 6 (14%) were thick melanomas. The most frequent dermoscopic criteria observed in all cases were atypical pigment network, multiple colors (3 or more), asymmetrical blotches, and peripheral asymmetric dots and globules. There was a statistically significant difference in the three groups regarding the frequency of atypical vessels, white scar areas, blue‐white veil, shiny white structures, and radial streaming. Abstract:Abstract Amelanotic melanoma (AM) is a subtype of melanoma where the lesion demonstrates little to no pigmentation. This can lead to delays in referral due to misdiagnosis, with studies showing a higher mortality rate compared with pigmented melanoma. However, there are few reports in the current literature regarding the characteristics associated with AM. In a 10-year period at a single centre in the north-west of England, patients with a histological diagnosis of AM were enrolled and data were collated retrospectively to determine the characteristics of AM lesions. Of the 16 patients identified, 69% were male, and the mean age at diagnosis was 78 years. The most common location for AM was the head (38%), which also demonstrated a higher mitotic rate (10.7 mm2) compared with the average (7.3 mm2). More than half of the lesions (56%) had been present for > 1 year. With a misdiagnosis rate of 88%, the likelihood of delays was evident. Dermoscopic findings were variable, but conspicuous vessels were noted in 63% of lesions. The most common vessel patterns were linear and irregular vessels, each identified in 19% of cases, with 13% of cases showing arborizing vessels. Our case series has demonstrated that AM is typically diagnosed in the older population, with a greater incidence rate noted in men. The age at diagnosis could be secondary to a delay in referral as AM is commonly not identified, with a misdiagnosis rate of 88% in our series. Lesions were most commonly located on the head region, whereas the back and legs were less commonly affected. This is contrary to melanoma, where the legs are most common for women and the back the most common for men. We suspect that a prolonged exposure of the head region to ultraviolet light is the reason for the highest distribution rate noted in the head. Furthermore, AM of the head was noted to have a greater mitotic rate than the average, potentially indicating more aggressive and lethal disease. The gold standard in clinical assessment is dermoscopic examination, with particular emphasis on identifying abnormal vessel features, a finding amplified due to a lack of pigmentation. We have demonstrated that AM can have a significant misdiagnosis rate with the possibility for a more aggressive cancer to form, potentially leading to unfavourable patient outcomes. We have identified key features of suspicious lesions that require assessment, notably abnormal vessels. Practitioners in primary and secondary care should be aware of and vigilant in identifying AM lesions. All suspicious lesions should undergo histopathological analysis to determine their exact nature.

Total body photography (TBP) has been widely incorporated in clinical practice, especially for follow‐up of high‐risk individuals with multiple naevi. Distinctive clinical and histological characteristics of atrophic and hypertrophic facial photoageing

Abstract:Background: Photoageing describes complex cutaneous changes which occur following chronic exposure to solar ultraviolet radiation (UVR). Amongst White Northern Europeans, facial photoageing appears as distinct clinical phenotypes: 'hypertrophic' photoageing (HP) and 'atrophic' photoageing (AP). Deep, coarse wrinkles predominate in individuals with HP, whereas those with AP have relatively smooth, unwrinkled skin with pronounced telangiectasia. AP individuals have an increased propensity for developing keratinocyte cancers. Objectives: To investigate whether histological differences underlie these distinct phenotypes of facial photoageing. Methods: Facial skin biopsies were obtained from participants with AP (10 M, 10 F; mean age: 78.7 years) or HP (10 M, 10 F; mean age: 74.5 years) and were assessed histologically and by immunohistochemistry. Results: Demographic characterization revealed 95% of AP subjects, as compared to 35% with HP, were Fitzpatrick skin type I/II; of these, 50% had a history of one or more keratinocyte cancers. There was no history of keratinocyte cancers in the HP cohort. Analysis of UVR-induced mitochondrial DNA damage confirmed that all volunteers had received similar lifetime cumulative doses of sun exposure. Histologically, male AP had a significantly thicker epidermis than did AP females or those of either sex with HP. HP facial skin exhibited severe solar elastosis, whereas in AP facial skin, solar elastosis was apparent only in females. Loss of papillary dermal fibrillin-rich microfibrils occurred in all HP and AP female subjects, but not in AP males. Furthermore, male AP had a significant reduction in collagen VII at the dermal-epidermal junction than did AP females or those of either sex with HP. Conclusions: This study provides further evidence that AP and HP represent distinct clinical and histological entities. Knowledge of these two phenotypes is clinically relevant due to the increased prevalence of keratinocyte cancers in those - particularly males - with the AP phenotype.

Abstract:The overall patterns of correlations among various melanoma risk factors have not yet been examined. The aim of this study was to assess the impact of different parameters on disease-free and melanoma-related overall survival. A retrospective cohort study was conducted encompassing all patients with a primary cutaneous melanoma diagnosed in a university referral centre. Associations were explored using semantic map analysis, which uses graph theory to find the strongest path of connections between variables. A total of 1,110 melanoma patients (median follow-up 10.6 years) were included. The analysis revealed a clustering of variables around 2 main hubs: Breslow thickness 10 mm. This semantic analysis confirmed the close link between Breslow thickness, age, sentinel lymph node biopsy findings, skin type, melanoma subtype and prognosis, and provides prognostic information useful for the further stratification and management of patients with melanoma.

Abstract:BACKGROUND: Head and neck mucosal melanoma (HNMM) is defined as a rare malignant tumor derived from melanocytes. There is no consensus regarding the treatment protocol for HNMM in elderly patients.METHODS: The National Cancer Institute's Surveillance, Epidemiology, and End Results database was used to identify elderly patients diagnosed with HNMM from 1975 to 2016. The chi-squared test was used to compare patient characteristics. The reverse Kaplan-Meier method was used to estimate the median follow-up time. The Kaplan-Meier method and log-rank test were used to estimate and compare the overall survival (OS) and disease-specific survival (DSS) of the groups. Cox regression analysis was used to evaluate the risk factors for OS and DSS of HNMM.RESULTS: Our retrospective study included 828 elderly patients with HNMM, and the 5-year OS and DSS rates were 22.4% and 27.4%, respectively. After adjusting for other variables in multivariate analysis, patients undergoing radiotherapy alone had worse OS [hazard ratio (HR) =1.449, 95% confidence interval (CI): 1.010-1.742, P=0.006] and DSS (HR =1.656, 95% CI: 1.257-2.181, P<0.001) than those undergoing surgery alone. No significant difference in OS (HR =0.892, 95% CI: 0.753-1.056, P=0.183) or DSS (HR =0.917, 95% CI: 0.764-1.101, P=0.354) was observed for patients undergoing surgery with or without radiotherapy. Our analysis of the subgroup of patients with complete clinical staging information demonstrated that the effects of surgery alone on OS (HR =0.734, 95% CI: 0.562-0.958, P=0.023) were inferior to those of surgery with radiotherapy, but no significant difference was noted compared with radiotherapy alone.CONCLUSIONS: The survival of elderly patients with HNMM is increased with the combination of surgery and radiotherapy compared with surgery alone and radiotherapy alone. In addition, the population-based analysis demonstrated that combination therapy exhibited an obviously increased usage rate from 1975 to 2016, representing a mainstream treatment modality.

Abstract:Intrinsic skin changes with advancing years include dryness, decreasing elasticity, increasing skin fragility, and more prominent vasculature. Extrinsic skin aging, caused primarily by cigarette smoking and exposure to sunlight, includes mottled pigmentation and yellow discoloration, rough leathery textural changes, and wrinkling. Major premalignant and malignant neoplasms in photodamaged skin are actinic keratosis, basal cell carcinoma, squamous cell carcinoma, and melanoma. Nonmalignant lesions include solar lentigines and seborrheic keratoses. The A, B, C, D criteria can assist in the evaluation of pigmented nevi. Physicians play an important role in educating patients about the health risks associated with excessive sun exposure and about sun protection to prevent further skin damage.

Abstract:RÉSUMÉ L’objectif de cette étude était d’examiner l’association entre la zone de statut socio-économique (SSE) et recevant une chimiothérapie chez les personnes âgées atteints de mélanome cutané. Une base de données liée à SEER-Medicare (1.329 hommes et femmes blancs âgés de ≥ 66, avec des étapes de mélanome invasif [régionaux et éloignés]; 1991-1999) a été utilisé. SSE a été mesurée par le niveau de pauvreté des secteurs de recensement (moyenne de 1990 et 2000 des données du recensement). Covariables ont été données sociodémographiques, caractéristiques de la tumeur et l’indice de comorbidité. ABSTRACT The study objective was to examine the association, among older persons with cutaneous melanoma, between areal socioeconomic status (SES) and receiving chemotherapy. SEER-Medicare-linked database (1,239 white men and women aged ≥ 66, with invasive melanoma [regional and distant stages]; 1991–1999) was used. SES was measured by census tract poverty level (average of 1990 and 2000 Census data). Covariates were sociodemographics, tumor characteristics, and comorbidity index. Residing in poorer SES areas was associated with a lower likelihood for receiving chemotherapy among patients in the overall sample (adjusted odds ratios = OR 0.97, 95% confidence interval = CI 0.95–0.99), and those with regional stage at diagnosis (OR 0.97, 95% CI 0.94–0.98). These findings reflect socioeconomic disparities in chemotherapy use for melanoma among older white patients in the United States.

Abstract:Aging is an independent prognostic factor for the diagnosis of melanoma. The extracellular matrix of the skin has well-documented changes with age, such as the loss of HAPLN1, which can lead to differential metastasis in aged individuals. The aged microenvironment can contribute to the phenotype switch, where aged individuals have smaller primary tumors but more metastases, i.e., going instead of growing. The extracellular matrix (ECM) has been shown to modulate the immune response either through direct binding of ECM proteins or through mechanotransduction, but it is unclear how aging affects this crosstalk. Our hypothesis is that the ECM can modulate immune response to melanoma and affect response to immune checkpoint blockade (ICB) therapy. Melanoma cells cultured on cell derived matrices (CDMs) from young and aged human fibroblasts show differential levels of expression of PD-L1, a ligand that binds to PD-1 on T cells to induce T cell exhaustion. Antibodies targeting PD-1/PD-L1 are one of the main ICB treatments available to melanoma. We are also investigating T cell motility on young and aged CDMs, as well as correlating T cell infiltration in murine tumors with ECM signatures as identified by second harmonic generation microscopy. Therefore, we have compiled evidence that aging affects the crosstalk between the extracellular matrix and the immune system, providing a new horizon for therapeutic improvement of response in older patients. A systematic review of factors influencing older adults' decision to accept or decline cancer treatment

Abstract:Background: Cancer is a disease that affects mostly older adults. Older adults often have other chronic health conditions in addition to cancer and may have different health priorities, both of which can impact cancer treatment decision-making. However, no systematic review of factors that influence an older cancer patient's decision to accept or decline cancer treatment has been conducted. Materials and methods: Systematic review of the literature published between inception of the databases and February 2013. Dutch, English, French or German articles reporting on qualitative studies, cross-sectional, longitudinal observational or intervention studies describing factors why older adults accepted or declined cancer treatment examining actual treatment decisions were included. Ten databases were used. Two independent reviewers reviewed manuscripts and performed data abstraction using a standardized form and the quality of studies was assessed with the Mixed Methods Appraisal Tool. Results: Of 17,343 abstracts reviewed, a total of 38 studies were included. The majority focused on breast and prostate cancer treatment decisions and most studies used a qualitative design. Important factors for accepting treatment were convenience and success rate of treatment, seeing necessity of treatment, trust in the physician and following the physician's recommendation. Factors important for declining cancer treatment included concerns about the discomfort of the treatments, fear of side effects and transportation difficulties. Conclusion: Although the reasons why older adults with cancer accepted or declined treatment varied considerably, the most consistent determinant was physician recommendation. Further studies using large, representative samples and exploring decision-making incorporating health literacy and comorbidity are needed.

Abstract:A 69-year-old man presented with a dark spot on his left malar region that had been evolving over a period of 15 years. Over the course of 3 months, the lesion progressed to a blackish papule surrounded by an erythematous halo, occasionally displaying purulent discharge and bleeding. Clinical examination also revealed the presence of a palpable lymph node on the same side as the lesion. In light of these clinical findings and a suspicion of nodular melanoma, an excisional biopsy was performed. Histopathological analysis revealed cohesive aggregates and sheets of neoplastic melanocytes in the dermis, exhibiting extensive pagetoid spread and pigmentation in some areas.
