Abstract:This study investigated hypoxia related changes in the human brain function through EEG analysis with the method of Lempel-Ziv complexity (LZC) and approximate entropy (ApEn). Meanwhile, multichannel human EEG signals were studied topographically in 3 healthy subjects under normal and different hypoxia conditions respectively. It was found that when subjects were at the simulated altitude of 3000 m, the measure of LZC and ApEn increased in almost all channels. The results also show that after 15 days hypoxic training, the LZC measure and ApEn decreased obviously and almost the same as those in normal condition. This suggests that after hypoxic training, the subjects were almost adapted to the hypoxia environment. In addition, the complexity measure was calculated and compared in each sub frequency band. We propose that LZC and ApEn can be used to evaluate the effect of brain functional changes at different levels of hypoxia and hypoxic training is an effective means to prevent hypoxic fatigue
Electrophysiological correlates of hyperoxia during resting-state EEG in awake human subjects

Abstract:Abstract Recreational use of concentrated oxygen has increased. Claims have been made that hyperoxic breathing can help reduce fatigue, increase alertness, and improve attentional capacities; however, few systematic studies of these potential benefits exist. Here we examined the effects of short-term (15 minute) hyperoxia on resting-states in awake human subjects by measuring spontaneous EEG activity between normoxic and hyperoxic situations, using a within-subjects design for both eyes-opened and eyes-closed conditions. We also measured respiration rate, heart rate, and blood oxygen saturation levels to correlate basic physiological changes due to the hyperoxic challenge with any brain activity changes. Our results show that breathing short term 100% oxygen led to increased blood-oxygen saturation levels, decreased heart rate, and a slight, but non-significant, decrease in breathing rate. Changes of brain activity were apparent, including decreases in low-alpha (7-10 Hz), high-alpha (10-14 Hz), beta (14-30 Hz), and gamma (30-50 Hz) frequency ranges during eyes-opened hyperoxic conditions. During eyes-closed hyperoxia, increases in the delta (0.5-3.5 Hz) and theta (3.5-7 Hz) frequency range were apparent together with decreases in the beta range. Hyperoxia appeared to accentuate the decrease of low alpha and gamma ranges across the eyes-opened and closed conditions suggesting that it modulated brain state itself. As decreased alpha during eyes-opened conditions has been associated with increased attentional processing and selective attention, and increased delta and theta during eyes-closed condition are typically associated with the initiation of sleep, our results suggest a state-specific and perhaps opposing influence of short-term hyperoxia.

Abstract:A single session of aerobic or resistance training transiently enhances cognitive function, making it a valuable strategy for dementia prevention in the older people. Despite its acknowledged benefits, the precise mechanism behind exercise-induced cognitive improvement remains controversial. In the present study, we investigated the impact of altered cerebral blood flow (CBF) on brain neural activity originating from motor executive and inhibitory processing using electroencephalographic event-related potentials (EEG-ERPs). Sixteen healthy subjects participated in four sessions, with EEG-ERPs measured during somatosensory Go/No-go tasks. The sessions were conducted under four distinct respiratory conditions presented in random order: normal breathing (NB) and rapid breathing (RB) with room air, normal breathing with hypercapnic gas (5% CO 2 , 21% O 2 , and balanced N 2 ) (NB+Gas), and rapid breathing with the same gas (RB+Gas). Changes in CBF were evaluated based on the middle cerebral artery mean blood velocity (MCA V mean ) using transcranial Doppler. P ET CO 2 was decreased under the RB condition but increased under the NB+Gas condition, thereby decreasing and increasing MCA V mean , respectively. Under the NB+Gas condition, MCA V mean significantly increased, but it had no effect on either the executive or inhibitory function. In contrast, the reduction in MCA V mean induced by RB decreased the peak amplitudes of Go-P300 and No-go-P300. However, even under the RB+Gas condition while MCA V mean increased, the peak amplitudes of both also decreased. These findings suggest that neither increases nor decreases in CBF affected cognitive function.

Exposure to Non-Steady-State Oxygen Is Reflected in Changes to Arterial Blood Gas Values, Prefrontal Cortical Activity, and Systemic Cytokine Levels

Abstract:Onboard oxygen-generating systems (OBOGSs) provide increased inspired oxygen (FiO2) to mitigate the risk of neurologic injury in high altitude aviators. OBOGSs can deliver highly variable oxygen concentrations oscillating around a predetermined FiO2 set point, even when the aircraft cabin altitude is relatively stable. Steady-state exposure to 100% FiO2 evokes neurovascular vasoconstriction, diminished cerebral perfusion, and altered electroencephalographic activity. Whether non-steady-state FiO2 exposure leads to similar outcomes is unknown. This study characterized the physiologic responses to steady-state and non-steady-state FiO2 during normobaric and hypobaric environmental pressures emulating cockpit pressures within tactical aircraft. The participants received an indwelling radial arterial catheter while exposed to steady-state or non-steady-state FiO2 levels oscillating ± 15% of prescribed set points in a hypobaric chamber. Steady-state exposure to 21% FiO2 during normobaria produced arterial blood gas values within the anticipated ranges. Exposure to non-steady-state FiO2 led to PaO2 levels higher upon cessation of non-steady-state FiO2 than when measured during steady-state exposure. This pattern was consistent across all FiO2 ranges, at each barometric condition. Prefrontal cortical activation during cognitive testing was lower following exposure to non-steady-state FiO2 >50% and 50% reduced prefrontal cortical brain activation during the cognitive challenge, consistent with an evoked pattern of neurovascular constriction and dilation.

The influence of basal CO2 on neurofluid dynamics measured using resting-state BOLD fMRI

Abstract:An understanding of neurofluid dynamics has been gaining importance, in part given the link between neurofluid dynamics and glymphatic flow. As carbon dioxide (CO2) strongly modulates cerebral blood volume (CBV) and cerebrospinal fluid (CSF) flow, modulation of basal CO2 through different capnic conditions may offer insight into the mechanisms through which neurofluid dynamics are influenced. In this work, we report observations of neurofluid dynamics at normocapnia as well as during short-term hyper- and hypocapnia. We use the resting-state BOLD fMRI signal in neurofluid regions of interest (i.e. blood vessels, CSF compartments) as a surrogate of neurofluid dynamics. From these BOLD signals, we extract the power and central frequency of signal oscillations. We found that 1) Relative to a normocapnic steady state, both hypocapnic and hypercapnic steady states are associated with increased BOLD signal power and shifts in BOLD signal frequency in vascular ROIs in a band-dependent manner; 2) Relative to hypocapnia, hypercapnia is associated with both increased and decreased BOLD signal power in vascular ROIs, depending on the frequency band; 3) these trends are largely reproduced in the CSF ROIs and in the CSF velocity dynamics; 4) these power and frequency variations across capnic conditions are mostly driven by respiratory and heart-rate differences rather than by steady-state CO2 and associated vascular-tone variations; 5) the cardiac and respiratory response functions differ substantially across capnic conditions. This work contributes to the establishment of the BOLD signal as a surrogate for neurofluid flow, and highlight the role of the autonomic nervous system (ANS) in linking vascular and CSF dynamics in the brain. The findings suggest that the ANS is also instrumental in the regulation of neurofluid flow in response to alterations of cerebral hemodynamic homeostasis. Furthermore, our findings suggest that this mechanism of ANS regulation differs across capnic states, or more broadly, across individuals with different basal capnic states.

Abstract:ABSTRACT Hypercapnia increases cerebral blood flow. The effects on cerebral metabolism remain incompletely understood although studies show an oxidation of cytochrome c oxidase, Complex IV of the mitochondrial respiratory chain. Systems modelling was combined with previously published non-invasive measurements of cerebral tissue oxygenation, cerebral blood flow, and cytochrome c oxidase redox state to evaluate any metabolic effects of hypercapnia. Cerebral tissue oxygen saturation and cytochrome oxidase redox state were measured with broadband near infrared spectroscopy and cerebral blood flow velocity with transcranial Doppler ultrasound. Data collected during 5-min hypercapnia in awake human volunteers were analysed using a Fick model to determine changes in brain oxygen consumption and a mathematical model of cerebral hemodynamics and metabolism (BrainSignals) to inform on mechanisms. Either a decrease in metabolic substrate supply or an increase in metabolic demand modelled the cytochrome oxidation in hypercapnia. However, only the decrease in substrate supply explained both the enzyme redox state changes and the Fick-calculated drop in brain oxygen consumption. These modelled outputs are consistent with previous reports of CO2 inhibition of mitochondrial succinate dehydrogenase and isocitrate dehydrogenase. Hypercapnia may have physiologically significant effects suppressing oxidative metabolism in humans and perturbing mitochondrial signalling pathways in health and disease.

Regulation of cerebral blood flow in mammals during chronic hypoxia: a matter of balance

Abstract:Respiratory-induced changes in the partial pressures of arterial carbon dioxide (PaCO2) and oxygen (PaO2) play a major role in cerebral blood flow (CBF) regulation. Elevations in PaCO2 (hypercapnia) lead to vasodilatation and increases in CBF, whereas reductions in PaCO2 (hypocapnia) lead to vasoconstriction and decreases in CBF. A fall in PaO2 (hypoxia) below a certain threshold (<40-45 mmHg) also produces cerebral vasodilatation. Upon initial exposure to hypoxia, CBF is elevated via a greater relative degree of hypoxia compared with hypocapnia. At this point, hypoxia-induced elevations in blood pressure and loss of cerebral autoregulation, stimulation of neuronal pathways, angiogenesis, release of adenosine, endothelium-derived NO and a variety of autocoids and cytokines are additional factors acting to increase CBF. Following 2-3 days, however, the process of ventilatory acclimatization results in a progressive rise in ventilation, which increases PaO2 and reduces PaCO2, collectively acting to attenuate the initial rise in CBF. Other factors acting to lower CBF include elevations in haematocrit, sympathetic nerve activity and local and endothelium-derived vasoconstrictors. Hypoxia-induced alterations of cerebrovascular reactivity, autoregulation and pulmonary vascular tone may also affect CBF. Thus, the extent of change in CBF during exposure to hypoxia is dependent on the balance between the myriad of vasodilators and constrictors derived from the endothelium, neuronal innervations and perfusion pressure. This review examines the extent and mechanisms by which hypoxia regulates CBF. Particular focus will be given to the marked influence of hypoxia associated with exposure to high altitude and chronic lung disease. The associated implications of these hypoxia-induced integrative alterations for the regulation of CBF are discussed, and future avenues for research are proposed.

Simple exercises that significantly increase cerebral blood flow and cerebral oxygenation

Abstract:We tested the hypothesis that simple exercises may significantly increase cerebral blood flow (CBF) and/or cerebral oxygenation. Eighteen subjects ranging in age from nineteen to thirty nine participated in a four-stage study during which measurements of end tidal CO_2 (EtCO2 - by capnometer) and local brain oxygenation (by near-infrared spectroscopy (NIRS) sensor) were taken. The four stages were 1) baseline, 2) breathing exercises, 3) solving an arithmetic problem, and 4) biofeedback. During the breathing exercises there was a significant increase in EtCO2 indicating a significant increase in global CBF. The increase in global CBF was estimated on the basis of a theoretical model. During the arithmetic and biofeedback tasks there was a significant increase in the local (Fp1) oxygenation, but it varied between the different participants. The results may lead to new clinical applications of CBF and brain oxygenation monitoring and behavioral control. We foresee future more detailed investigations in the control of CO2 in brain circulation in specific regions of the brain involved in cognition and memory.
Electroencephalographic (EEG) Changes Accompanying Normal Breathing of Concentrated Oxygen (Hyperoxic Ventilation) by Healthy Adults: A Systematic Review

Abstract:Divers often increase their fraction of inspired oxygen (FiO ) to decrease their risk of decompression sickness. However, breathing concentrated oxygen can cause hyperoxia, and central nervous system oxygen toxicity (CNS-OT). This study aims to review the literature describing hyperoxic ventilation's effect on the electroencephalogram (EEG), thus exploring the potential for real-time detection of impending CNS-OT seizure. We searched Medline, Embase, Scopus, and Web of Science for articles that reported EEG measures accompanying hyperoxic ventilation (FiO = 1.0) in healthy participants. We included peer-reviewed journal articles, books, and government reports with no language or date restrictions. Randomised controlled trials and cross-over studies were included; case reports were excluded. We used the Newcastle-Ottawa scale to evaluate evidence quality. Our search strategy returned 1025 unique abstracts; we analysed the full text of 40 articles; 22 articles (16 studies) were included for review. Study cohorts were typically small, and comprised of male non-divers. We discovered a variety of EEG analysis methods: studies performed spectral analysis (n = 12), the analysis of sensory-evoked potentials (n = 4), connectivity/complexity analysis (n = 3), source localization (n = 1), and expert qualitative analyses (n = 4). Studies of severe exposures (long duration at hyperbaric pressure) typically reported qualitative measures, and studies of mild exposures typically reported quantitative measures. There is a need for a large randomised controlled trial (RCT) reporting quantitative measures to better understand hyperoxic ventilation's effect on EEG, thus enabling the development of real-time monitoring of CNS-OT risk.
Effects of Changes in Arterial Carbon Dioxide and Oxygen Partial Pressures on Cerebral Oximeter Performance

Abstract:Abstract Background Cerebral oximetry (cerebral oxygen saturation; ScO2) is used to noninvasively monitor cerebral oxygenation. ScO2 readings are based on the fraction of reduced and oxidized hemoglobin as an indirect estimate of brain tissue oxygenation and assume a static ratio of arterial to venous intracranial blood. Conditions that alter cerebral blood flow, such as acute changes in Paco2, may decrease accuracy. We assessed the performance of two commercial cerebral oximeters across a range of oxygen concentrations during normocapnia and hypocapnia. Methods Casmed FORE-SIGHT Elite (CAS Medical Systems, Inc., USA) and Covidien INVOS 5100C (Covidien, USA) oximeter sensors were placed on 12 healthy volunteers. The fractional inspired oxygen tension was varied to achieve seven steady-state levels including hypoxic and hyperoxic Pao2 values. ScO2 and simultaneous arterial and jugular venous blood gas measurements were obtained with both normocapnia and hypocapnia. Oximeter bias was calculated as the difference between the ScO2 and reference saturation using manufacturer-specified weighting ratios from the arterial and venous samples. Results FORE-SIGHT Elite bias was greater during hypocapnia as compared with normocapnia (4 ± 9% vs. 0 ± 6%; P < 0.001). The INVOS 5100C bias was also lower during normocapnia (5 ± 15% vs. 3 ± 12%; P = 0.01). Hypocapnia resulted in a significant decrease in mixed venous oxygen saturation and mixed venous oxygen tension, as well as increased oxygen extraction across fractional inspired oxygen tension levels (P < 0.0001). Bias increased significantly with increasing oxygen extraction (P < 0.0001). Conclusions Changes in Paco2 affect cerebral oximeter accuracy, and increased bias occurs with hypocapnia. Decreased accuracy may represent an incorrect assumption of a static arterial–venous blood fraction. Understanding cerebral oximetry limitations is especially important in patients at risk for hypoxia-induced brain injury, where Paco2 may be purposefully altered.

[Investigation of canine leishmaniosis by nested-PCR in Kayseri and vicinity].

Abstract:Canine leishmaniosis (CanL) caused by Leishmania spp. is a zoonotic protozoon disease. It is widespread in most parts of the world including the Mediterranean basin. The present study was carried out in order to determine the prevalence of CanL in dogs in Kayseri and vicinity by nested-PCR. A total of 300 asymptomatic dogs were sampled randomly. Blood samples taken from the vena cephalica antebrachii were collected into tubes containing EDTA. Furthermore, lymph samples were taken from 14 dogs while bone marrow, spleen and liver biopsies were taken from three dogs. The DNA's obtained from these samples were examined for the presence of Leishmania spp. by nested-PCR which amplified the small subunit ribosomal RNA (ssr RNA) gene. According to the results of the nested-PCR, none of the 300 dogs were Leishmania spp. DNA positive.

Abstract:Electroencephalography (EEG) data simultaneously acquired with functional magnetic resonance imaging (fMRI) data are preprocessed to remove gradient artifacts (GAs) and ballistocardiographic artifacts (BCAs). Nonetheless, these data, especially in the gamma frequency range, can be contaminated by residual artifacts produced by mechanical vibrations in the MRI system, in particular the cryogenic pump that compresses and transports the helium that chills the magnet (the helium-pump). However, few options are available for the removal of helium-pump artifacts. In this study, we propose a recursive approach of EEG-segment-based principal component analysis (rsPCA) that enables the removal of these helium-pump artifacts. Using the rsPCA method, feature vectors representing helium-pump artifacts were successfully extracted as eigenvectors, and the reconstructed signals of the feature vectors were subsequently removed. A test using simultaneous EEG–fMRI data acquired from left-hand (LH) and right-hand (RH) clenching tasks performed by volunteers found that the proposed rsPCA method substantially reduced helium-pump artifacts in the EEG data and significantly enhanced task-related gamma band activity levels (p = 0.0038 and 0.0363 for LH and RH tasks, respectively) in EEG data that have had GAs and BCAs removed. The spatial patterns of the fMRI data were estimated using a hemodynamic response function (HRF) modeled from the estimated gamma band activity in a general linear model (GLM) framework. Active voxel clusters were identified in the post-/pre-central gyri of motor area, only from the rsPCA method (uncorrected p < 0.001 for both LH/RH tasks). In addition, the superior temporal pole areas were consistently observed (uncorrected p < 0.001 for the LH task and uncorrected p < 0.05 for the RH task) in the spatial patterns of the HRF model for gamma band activity when the task paradigm and movement were also included in the GLM.
Cognitive activity significantly affects the dynamic cerebral autoregulation, but not the dynamic vasoreactivity, in healthy adults

Abstract:Introduction: Neurovascular coupling (NVC) is an important mechanism for the regulation of cerebral perfusion during intensive cognitive activity. Thus, it should be examined in terms of its effects on the regulation dynamics of cerebral perfusion and its possible alterations during cognitive impairment. The dynamic dependence of continuous changes in cerebral blood velocity (CBv), which can be measured noninvasively using transcranial Doppler upon fluctuations in arterial blood pressure (ABP) and CO2 tension, using end-tidal CO2 (EtCO2) as a proxy, can be quantified via data-based dynamic modeling to yield insights into two key regulatory mechanisms: the dynamic cerebral autoregulation (dCA) and dynamic vasomotor reactivity (DVR), respectively. Methods: Using the Laguerre Expansion Technique (LET), this study extracted such models from data in supine resting vs cognitively active conditions (during attention, fluency, and memory tasks from the Addenbrooke's Cognitive Examination III, ACE-III) to elucidate possible changes in dCA and DVR due to cognitive stimulation of NVC. Healthy volunteers (n = 39) were recruited at the University of Leicester and continuous measurements of CBv, ABP, and EtCO2 were recorded. Results: Modeling analysis of the dynamic ABP-to-CBv and CO2-to-CBv relationships showed significant changes in dCA, but not DVR, under cognitively active conditions compared to resting state. Discussion: Interpretation of these changes through Principal Dynamic Mode (PDM) analysis is discussed in terms of possible associations between stronger NVC stimulation during cognitive tasks and enhanced sympathetic activation.

Abstract:s Significance Cerebral blood flow is influenced by respiration, primarily through changes in the CO2 concentration of arterial blood. Aim The objective of this study was to investigate the effect of changes in arterial CO2 concentration induced by respiratory changes on oxygenation and hemodynamics in the cerebral and extracerebral tissue layers of the forehead. Approach We used systemic physiology augmented functional near-infrared spectroscopy (SPA-fNIRS) to assess changes in forehead hemodynamics and oxygenation as well as systemic physiology in 20 healthy subjects. All participants performed two respiratory tasks (breath-holding and hyperventilation). Results In our SPA-fNIRS study we found that changes in breathing affected hemodynamics and oxygenation in both the extracerebral and cerebral tissue layers of the forehead in characteristic ways, depending on the two respiratory tasks. Conclusion The results show that extracerebrovascular reactivity (ECVR) exists in parallel with the well-known cerebrovascular reactivity (CVR): CVR and ECVR must be considered when performing fNIRS neuroimaging studies involving changes in respiration.

Abstract:The fundamentals of near infrared spectroscopy (NIRS) are reviewed. Among the major factors controlling the cerebral blood flow (CBF), the effect of PaCO2 is peculiar in that it violates autoregulatory CBF mechanisms and allows to explore the full range of the CBF. A simple physical model, with a four parameter formula, relating the CBF to PaCO2 is presented. It can be used to transform the fits of one animal to the fits of another one. It enable the use of rats data as monkeys data simply by rescaling the PaCO2 values and the CBF data. Controlled breathing can change the PaCO2. Experiments on human subjects relating the PaCO2 to rSO2, measured with brain oximeters, are presented. A simple model relating the mean blood pressure to CBF is worked out.

Determining the effects of elevated partial pressure of oxygen on hypercapnia-induced cerebrovascular reactivity

Abstract:Evaluation of cerebrovascular reactivity (CVR) to hypo- and hypercapnia is a valuable test for the assessment of vasodilatory reserve. While hypercapnia-induced CVR testing is usually performed at normoxia, mild hyperoxia may increase tolerability of hypercapnia by reducing the ventilatory distress. However, the effects of mild hyperoxia on CVR was unknown. We therefore recruited 21 patients with a range of steno-occlusive diseases and 12 healthy participants who underwent a standardized 13-minute step plus ramp CVR test with a carbon dioxide gas challenge at the subject's resting end-tidal partial pressure of oxygen or at mild hyperoxia (PetO2 = 150 mmHg) depending on to which group they were assigned. In 11 patients, the second CVR test was at normoxia to examine test-retest differences. CVR was defined as % Δ Signal/ΔPetCO2. We found that there was no significant difference between CVR test results conducted at normoxia and at mild hyperoxia for participants in Groups 1 and 2 for the step and ramp portion. We also found no difference between test and retest CVR at normoxia for patients with cerebrovascular pathology (Group 3) for step and ramp portion. We concluded normoxic CVR is repeatable, and that mild hyperoxia does not affect CVR.
Effects of clamping end-tidal CO2 on neurofluidic low-frequency oscillations

Abstract:In recent years, low-frequency oscillations (LFOs) (0.01-0.1 Hz) have been a subject of interest in resting-state functional magnetic resonance imaging research. They are believed to have many possible driving mechanisms, from both regional and global sources. Internal fluctuations in the partial pressure of CO2 (PCO2) has long been thought of as one of these major driving forces, but its exact contributions compared with other mechanisms have yet to be fully understood. This study examined the effects of end-tidal PCO2 (PetCO2) oscillations on LF cerebral hemodynamics and cerebrospinal fluid (CSF) dynamics under "clamped PetCO2" and "free-breathing" conditions. Under clamped PetCO2, a participant's PetCO2 levels were fixed to their baseline average, whereas PetCO2 was not controlled in free breathing. Under clamped PetCO2, the fractional amplitude of hemodynamic LFOs in the occipital and sensorimotor cortex and temporal lobes were found to be significantly reduced. Additionally, the fractional amplitude of CSF LFOs, measured at the fourth ventricle, was found to be reduced by almost one-half. However, the spatiotemporal distributions of blood and CSF delay times, as measured by cross-correlation in the LF domain, were not significantly altered between conditions. This study demonstrates that, while PCO2 oscillations significantly mediate LFOs, especially those observed in the CSF, other mechanisms are able to maintain LFOs, with high correlation, even in their absence.
The effect of CO2 on the age dependence of neurovascular coupling

Abstract:Prior studies have identified variable effects of aging on neurovascular coupling (NVC). Carbon dioxide (CO2) affects both cerebral blood velocity (CBv) and NVC, but the effects of age on NVC under different CO2 conditions are unknown. Therefore, we investigated the effects of aging on NVC in different CO2 states during cognitive paradigms. Seventy-eight participants (18-78 yr), with well-controlled comorbidities, underwent continuous recordings of CBv by bilateral insonation of middle (MCA) and posterior (PCA) cerebral arteries (transcranial Doppler), blood pressure, end-tidal CO2, and heart rate during poikilocapnia, hypercapnia (5% CO2 inhalation), and hypocapnia (paced hyperventilation). Neuroactivation via visuospatial (VS) and attention tasks (AT) was used to stimulate NVC. Peak percentage and absolute change in MCAv/PCAv, were compared between CO2 conditions and age groups (≤30, 31-60, and >60 yr). For the VS task, in poikilocapnia, younger adults had a lower NVC response compared with older adults [mean difference (MD): -7.92% (standard deviation (SD): 2.37), P = 0.004], but comparable between younger and middle-aged groups. In hypercapnia, both younger [MD: -4.75% (SD: 1.56), P = 0.009] and middle [MD: -4.58% (SD: 1.69), P = 0.023] age groups had lower NVC responses compared with older adults. Finally, in hypocapnia, both older [MD: 5.92% (SD: 2.21), P = 0.025] and middle [MD: 5.44% (SD: 2.27), P = 0.049] age groups had greater NVC responses, compared with younger adults. In conclusion, the magnitude of NVC response suppression from baseline during hyper- and hypocapnia, did not differ significantly between age groups. However, the middle age group demonstrated a different NVC response while under hypercapnic conditions, compared with hypocapnia.NEW & NOTEWORTHY This study describes the effects of age on neurovascular coupling under altered CO2 conditions. We demonstrated that both hypercapnia and hypocapnia suppress neurovascular coupling (NVC) responses. Furthermore, that middle age exhibits an NVC response comparable with younger adults under hypercapnia, and older adults under hypocapnia.
Effects of Chronic Hypoxic Environment on Cognitive Function and Neuroimaging Measures in a High-Altitude Population

Abstract:ObjectiveThis study aimed to investigate the effects of long-term hypoxic environment exposure on cognitive ability and neuroimaging characteristics in a highland population in China. MethodsHealth system workers in Maduo County (4,300 m above sea level) and Minhe County (1,700 m above sea level) were selected as research participants and divided into a high-altitude (HA) group and low-altitude (LA) group, respectively. Cognitive ability was assessed using the Montreal Cognitive Assessment (MoCA), Verbal Fluency Test (VFT), Symbol Digit Modalities Test (SDMT), Trail Making Test A and B (TMT), Digit Span Test (DST), and Rey Auditory Verbal Learning Test (RAVLT). All participants underwent a magnetic resonance imaging (MRI) scan, resting state functional MRI scan, and diffusion tensor imaging to clarify changes in regional gray matter (GM) volume, anisotropy index (FA), local consistency (ReHo), and low-frequency oscillation amplitude (ALFF). ResultsThe HA group had significantly lower MoCA, DST, VFT, RAVLT, and TMT scores compared to the control group. No significant differences were found in SDMT score. Furthermore, compared to the LA group, the HA group had significantly lower GM density of the left olfactory cortex, right medial orbital superior frontal gyrus, bilateral insula, left globus pallidus, and temporal lobe (left superior temporal gyrus temporal pole, bilateral middle temporal gyrus temporal pole, and right middle temporal gyrus). In terms of FA, compared with the LA group, the HA group had lower values for the corpus callosum, corpus callosum knee, bilateral radiative corona, and left internal capsule. The HA group had lower ALFF values of the left cerebellum, left putamen, left orbital inferior frontal gyrus, and left precuneus, but higher ALFF values of the left fusiform gyrus, bilateral inferior temporal gyrus, left orbital superior frontal gyrus and medial superior frontal gyrus, compared to the LA group. There was no significant group difference in ReHo values. ConclusionOur findings suggest that a chronic hypoxic environment can induce extensive cognitive impairment. Decreased GM density in multiple brain regions, damaged nerve fibers, and unbalanced neuronal activity intensity in different brain regions may be the structural and functional basis of cognitive impairment due to hypoxia.

Abstract:Previous studies reported enhanced cerebrovascular CO2 reactivity upon ascent to high altitude using linear models. However, there is evidence that this response may be sigmoidal in nature. Moreover, it was speculated that these changes at high altitude are mediated by alterations in acid-base buffering. Accordingly, we reanalyzed previously published data to assess middle cerebral blood flow velocity (MCAv) responses to modified rebreathing at sea level (SL), upon ascent (ALT1) and following 16 days of acclimatization (ALT16) to 5260 m in 21 lowlanders. Using sigmoid curve fitting of the MCAv responses to CO2, we found the amplitude (95 vs. 129%, SL vs. ALT1, 95% confidence intervals (CI) [77, 112], [111, 145], respectively, P = 0.024) and the slope of the sigmoid response (4.5 vs. 7.5%/mmHg, SL vs. ALT1, 95% CIs [3.1, 5.9], [6.0, 9.0], respectively, P = 0.026) to be enhanced at ALT1, which persisted with acclimatization at ALT16 (amplitude: 177, 95% CI [139, 215], P < 0.001; slope: 10.3%/mmHg, 95% CI [8.2, 12.5], P = 0.003) compared to SL. Meanwhile, the sigmoidal response midpoint was unchanged at ALT1 (SL: 36.5 mmHg; ALT1: 35.4 mmHg, 95% CIs [34.0, 39.0], [33.1, 37.7], respectively, P = 0.982), while it was reduced by ~7 mmHg at ALT16 (28.6 mmHg, 95% CI [26.4, 30.8], P = 0.001 vs. SL), indicating leftward shift of the cerebrovascular CO2 response to a lower arterial partial pressure of CO2 (PaCO2) following acclimatization to altitude. Sigmoid fitting revealed a leftward shift in the midpoint of the cerebrovascular response curve which could not be observed with linear fitting. These findings demonstrate that there is resetting of the cerebrovascular CO2 reactivity operating point to a lower PaCO2 following acclimatization to high altitude. This cerebrovascular resetting is likely the result of an altered acid-base buffer status resulting from prolonged exposure to the severe hypocapnia associated with ventilatory acclimatization to high altitude.
