Abstract:Journal of Health and Social Behavior, Ahead of Print. Less than 1% of studies on racialized health inequities have empirically examined their root cause: structural racism. Moreover, there has been a disconnect between the conceptualization and measurement of structural racism. This study advances the field by (1) distilling central tenets of theories of structural racism to inform measurement approaches, (2) conceptualizing U.S. states as racializing institutional actors shaping health, (3) developing a novel latent measure of structural racism in states, (4) using multilevel models to quantify the association between structural racism and five individual-level health outcomes among respondents from the Health and Retirement Study (N = 9,020) and the Behavioral Risk Factor Surveillance System (N = 308,029), and (5) making our measure of structural racism publicly available to catalyze research. Results show that structural racism is consistently associated with worse health for Black people but not White people. We conclude by highlighting this study's contributions (theoretical, methodological, and substantive) and important avenues for future research on the topic.

Abstract:Introduction: Black Americans have greater coronary heart disease (CHD) burden than White Americans, disparities that are largely socially determined. Discriminatory societal practices that systematically disadvantage Black Americans are forms of structural racism but few studies have examined structural racism and incident CHD. We sought to determine associations between three validated measures of structural racism and incident CHD, hypothesizing that greater state-level structural racism is associated with incident CHD for Black but not White individuals. Methods: We used data from the national REasons for Geographic And Racial Differences in Stroke (REGARDS) cohort, which enrolled 30,239 Black and White community dwelling adults between 2003-7 who were contacted every 6 months with retrieval of medical records and expert adjudication of myocardial infarction and cause of death. Incident CHD was defined as myocardial infarction or death due to CHD. Structural racism variables included Black:White % living below the Federal poverty line, Black:White % uninsured, and the Dissimilarity Index (DI), a measure of residential racial segregation. Structural racism variables were dichotomized at the median. Separate race-stratified Cox proportional hazards models examined associations between each measure of structural racism and incident CHD. Results: The 24,099 participants free of CHD at baseline included 10,286 Black and 13,813 White participants. Mean age at baseline was 64 years, 58% were women, and 47% had annual household income <$35,000. Greater structural racism was significantly associated with incident CHD for Black but not White participants. For high Black:White poverty, Black HR=1.17 (95% CI 1.01-1.35), White HR=0.93 (0.83-1.06); for high Black:White uninsurance, Black=HR 1.34 (1.06-1.70), White HR=1.20 (0.98-1.47); for high DI, Black HR=1.17 (1.01-1.35), White HR=0.99 (0.88-1.12). Findings suggest that structural racism variables indirectly influence CHD via individual-level income and education. Results were similar for men and women and for older and younger individuals. Significant associations were observed for fatal but not nonfatal CHD events. Conclusions: Structural racism was associated with higher incidence of CHD for Black but not White individuals. If these associations are causal, changing state level laws to combat poverty in Black communities, expand Medicaid, and reduce segregation could potentially lessen Black:White disparities in CHD.

Abstract:Abstract Progress toward racial health equity cannot be made if we cannot measure its fundamental driver – structural racism. As in other epidemiological studies, the first step is to measure the exposure. But how to measure structural racism is an ongoing debate. To characterize the approaches epidemiologists and other health researchers use to quantitatively measure structural racism, highlight methodological innovations, and identify gaps in the literature, we conducted a scoping review of the peer-reviewed and grey literature published during 2019-2021 to accompany the work of Groos et al. (J Health Dispar Res Pract. 2018;11(2):Article 13), which surveys the scope of structural racism measurement up to 2017. We identified several themes from the recent literature: the current predominant focus on measuring anti-Black racism, using residential segregation as well as other segregation-driven measures as proxies of structural racism, measuring structural racism as spatial exposures, an increasing call by epidemiologists and other health researchers to measure structural racism as a multidimensional, multi-level determinant of health and related innovations, the development of policy databases, the utility of simulated counterfactual approaches in the understanding of how structural racism drive racial health inequities, and the lack of measures of antiracism and limited work on later life effects. Our findings sketch out several future steps to improve the science around structural racism measurements, which is the key to advancing antiracism policies.

Historical Structural Racism in the Built Environment and Physical Health among Residents of Allegheny County, Pennsylvania

Abstract:Historical structural racism in the built environment contributes to health inequities, yet to date, research has almost exclusively focused on racist policy of redlining. We expand upon this conceptualization of historical structural racism by examining the potential associations of probable blockbusting, urban renewal, and proximity to displacement from freeway construction, along with redlining, to multiple contemporary health measures. Analyses linked historical structural racism, measured continuously at the census-tract level using archival data sources, to present-day residents' physical health measures drawn from publicly accessible records for Allegheny County, Pennsylvania. Outcome measures included average life expectancy and the percentage of residents reporting hypertension, stroke, coronary heart disease, smoking, insufficient sleep, sedentary behavior, and no health insurance coverage. Multiple regression analyses were conducted to examine separate and additive associations between structural racism and physical health measures. Redlining, probable blockbusting, and urban renewal were associated with shorter life expectancy and a higher prevalence of cardiovascular conditions, risky health behaviors, and residents lacking health insurance coverage. Probable blockbusting and urban renewal had the most consistent correlations with all 8 health measures, while freeway displacement was not reliably associated with health. Additive models explained a greater proportion of variance in health than any individual structural racism measure alone. Moreover, probable blockbusting and urban renewal accounted for relatively more variance in health compared to redlining, suggesting that research should consider these other measures in addition to redlining. These preliminary correlational findings underscore the importance of considering multiple aspects of historical structural racism in relation to current health inequities and serve as a starting point for additional research.

Self-Rated Health and Structural Racism Indicated by County-Level Racial Inequalities in Socioeconomic Status: The Role of Urban-Rural Classification

Abstract:Recent attention to the interrelationship between racism, socioeconomic status (SES) and health has led to a small, but growing literature of empirical work on the role of structural racism in population health. Area-level racial inequities in SES are an indicator of structural racism, and the associations between structural racism indicators and self-rated health are unknown. Further, because urban-rural differences have been observed in population health and are associated with different manifestations of structural racism, explicating the role of urban-rural classification is warranted. This study examined the associations between racial inequities in SES and self-rated health by county urban-rural classification. Using data from County Health Rankings and American Communities Surveys, black-white ratios of SES were regressed on rates of fair/poor health in U.S. counties. Racial inequities in homeownership were negatively associated with fair/poor health (<i>β</i> = −0.87, s.e. = 0.18), but racial inequities in unemployment were positively associated with fair/poor health (<i>β</i> = 0.03, s.e. = 0.01). The associations between structural racism and fair/poor health varied by county urban-rural classification. Potential mechanisms include the concentration of resources in racially segregated counties with high racial inequities that lead to better health outcomes, but are associated with extreme black SES disadvantage. Racial inequities in SES are a social justice imperative with implications for population health that can be targeted by urban-rural classification and other social contextual characteristics.

Abstract:Abstract Background: Although some racial disparities in US cancer mortality rates have narrowed over time, many persist; driven by racialized systems of oppression. Few studies have used multidimensional indices to explore the influence of structural racism on cancer outcomes while also accounting for ambient exposures to environmental hazards or opportunities. To add to research on the impact of structural racism on health, we examined associations between county level measures of structural racism and cancer mortality rates, while accounting for factors associated with cancer mortality, and county level measures of environmental burden. Methods: Data for our analysis come from a previously developed index of county level structural racism, and publicly available data on 2015-2019 cancer mortality rates from the United States Cancer Statistics Data Visualization Tool, 2019 County Health Rankings and Roadmaps, EPA’s 2006-2010 Environmental Quality Index, and 2015-2019 estimates from the US Census American Community Survey. Our outcome of interest was the number of deaths for all cancer types per 100,000 people. Our primary independent variable was the county standardized factor score for structural racism. To account for counties’ level of environmental burden we utilized the overall environmental quality index score. We also accounted for factors associated with cancer mortality rates including race (i.e., Non-Hispanic Black, White, Asian/Pacific Islander, American Indian Alaskan Native), adult smoking, obesity, excessive drinking, uninsured, sexually transmitted infection, and mammography screening rates, and the primary care physician to population ratio. Multiple linear regression analyses were used to examine associations between cancer mortality rates and structural racism standardized factor scores, overall and between race groups, while accounting for the aforementioned covariates and county geographic mobility and percent of the population considered rural. Results: Our analyses of 1,026 US counties indicated cancer mortality rates increased by 3.3 deaths per 100,000 people (95% CI: 1.2, 5.3) for every standard deviation increase in structural racism factor score, while controlling for all other covariates. Additionally, we found that race modified the relationship between structural racism and cancer mortality. Compared to the White population, Black cancer mortality rates increased by 11.9 deaths per 100,000 people (95% CI: 8.8, 14.8), Asian/Pacific Islander rates increased by 4.7 deaths per 100,00 people (95% CI: 1.3, 8.1), and American Indian/Alaskan Native mortality rates increased by 17.4 deaths per 100,000 people (95% CI: 4.2, 30.6) for every standard deviation increase in county structural racism factor score. Conclusion: This study demonstrates how race groups are differentially impacted by the social and physical characteristics of their environments. Abstract:Structural gendered racism – the "totality of interconnectedness between structural racism and sexism" – is conceptualized as a fundamental cause of the persistent preterm birth inequities experienced by Black and Indigenous people in the United States. Our objective was to develop a state-level latent class measure of structural gendered racism and examine its association with preterm birth among all singleton live births in the US in 2019. Using previously-validated inequity indicators between White men and Black women across 9 domains (education, employment, poverty, homeownership, health insurance, segregation, voting, political representation, incarceration), we conducted a latent profile analysis to identify a latent categorical variable with k number of classes that have similar values on the observed continuous input variables. Racialized group-stratified multilevel modified Poisson regression models with robust variance and random effects for state assessed the association between state-level classes and preterm birth. We found four distinct latent classes that were all characterized by higher levels of disadvantage for Black women and advantages for White men, but the magnitude of that difference varied by latent class. We found preterm birth risk among Black birthing people was higher across all state-level latent classes compared to White birthing people, and there was some variation of preterm birth risk across classes among Black but not White birthing people. These findings further emphasize the importance of understanding and interrogating the whole system and the need for multifaceted policy solutions.

Abstract:Purpose Methods for assessing the structural mechanisms of health inequity are not well established. This study applies a phased approach to modeling racial, occupational, and rural disparities on the county level. Methods Rural counties with disparately high rates of COVID-19 incidence or mortality were randomly paired with in-state control counties with the same rural-urban continuum code. Analysis was restricted to the first six months of the pandemic to represent the baseline structural reserves for each county and reduce biases related to the disruption of these reserves over time. Conditional logistic regression was applied in two phases—first, to examine the demographic distribution of disparities and then, to examine the relationships between these disparities and county-level social and structural reserves. Results In over 200 rural county pairs (205 for incidence, 209 for mortality), disparities were associated with structural variables representing economic factors, healthcare infrastructure, and local industry. Modeling results were sensitive to assumptions about the relationships between race and other social and structural variables measured at the county level, particularly in models intended to reflect effect modification or mediation. Conclusions Multivariable modeling of health disparities should reflect the social and structural mechanisms of inequity and anticipate interventions that can advance equity.

Abstract:Introduction: Racial differences in socioeconomic characteristics are believed to be secondary to structural racism. While socioeconomic factors explain some of the racial disparity in stroke incidence at an individual level, little is known about the association between societal-level structural racism and the incidence of acute ischemic stroke (AIS). Methods: We conducted a national cross-sectional analysis of 71,078,619 adults aged >65 years enrolled in Medicare from January 1, 2016 to December 31, 2019. The primary exposure was a county-level composite structural racism score that was calculated from constituent measures of structural racism: segregation indices (delta index, dissimilarity index, isolation index), Gini index, housing discrimination, educational attainment ratio, employment ratio, income ratio. The primary outcome was AIS incidence. Results: The composite structural racism score identified significant geographic variation in structural racism across the U.S. (mean 0.818, SD 2.874, IQR 3.02). Among 844,406 Medicare beneficiaries with incident AIS, Black individuals had a 19% increased hazard of AIS compared to White individuals (HR 1.19, 95% CI 1.14-1.25, p<0.0001). Increasing segregation conferred increased incidence of AIS for both Black and White populations. Each standard deviation increase in the composite structural racism score was associated with an 18% increased incidence in AIS (HR 1.18, 95% CI 1.12-1.24) in the total population. This association interacted with race (p=0.03), with a greater magnitude of association for White (HR 1.19, 95% CI 1.13-1.25, p<0.0001) versus Black (HR 1.09 ,95% CI 1.03-1.16, p=0.0073) individuals. Conclusions: There is significant geographic variation in structural racism at the county level across the US, and increasing levels of structural racism are associated with increased incidence of AIS, regardless of race.

Abstract:This article advances the field by integrating insights from intersectionality perspectives with the emerging literatures on structural racism and structural sexism—which point to promising new ways to measure systems of inequality at a macro level—to introduce a structural intersectionality approach to population health. We demonstrate an application of structural intersectionality using administrative data representing macrolevel structural racism, structural sexism, and income inequality in U.S. states linked to individual data from the Behavioral Risk Factor Surveillance System to estimate multilevel models (N = 420,644 individuals nested in 76 state-years) investigating how intersecting dimensions of structural oppression shape health. Analyses show that these structural inequalities: (1) vary considerably across U.S. states, (2) intersect in numerous ways but do not strongly or positively covary, (3) individually and jointly shape health, and (4) are most consistently associated with poor health for black women. We conclude by outlining an agenda for future research on structural intersectionality and health.

Abstract:Quantum entanglement, a key resource for quantum information science, is inherent in a solid. It has been recently shown that entanglement between a single optical photon and a single spin qubit in a solid is generated via spontaneous emission. However, entanglement generation by measurement is rather essential for quantum operations. We here show that the physics behind the entangled emission can be time reversed to demonstrate entangled absorption mediated by an inherent spin-orbit entanglement in a single nitrogen vacancy center in diamond. Optical arbitrary spin state preparation and complete spin state tomography reveal the fidelity of the entangled absorption to be 95%. With the entangled emission and absorption of a photon, materials can be spontaneously entangled or swap their quantum state based on the quantum teleportation scheme.
The odds grow longer. Doctors had hoped by now to have eliminated the AIDS virus from some patients. No such luck.

Abstract:In the U.S., inequities by race/ethnicity in health outcomes, such as in the HIV epidemic, are long standing but have come to the forefront during the COVID-19 pandemic. There is growing recognition of the role of structural racism in racialized health inequities, yet the conceptualization and operationalization of structural racism in HIV research lags. We conducted a scoping review of existing published literature, between 1999-April 2024, conceptualizing and measuring structural racism's impact among people living with or at risk for HIV in the U.S. Our initial search yielded 236 unique articles, which after title and abstract screening yielded ten articles meeting full text review criteria. We then extracted key parameters, such as conceptualization, method of measurement of structural racism, study aims, design, and findings. Three of the articles were qualitative studies that conceptualized structural racism using (1) the social network model, (2) individual and structural intersectionality and (3) critical race theory. Operationalization of structural racism within the seven quantitative studies fell into three categories: (1) structural level, (2) a scale of experiences of racism, including structural racism, and (3) using explanatory demographic factors as downstream measures of the effects of structural racism. The variance in the conceptualization and operationalization of structural racism highlights the different interpretations of structural racism in its applications to the field of HIV research. Given the vast racial/ethnic inequities in HIV, we propose three overarching suggestions for next steps in improving the conduct of research on structural racism in HIV: (1) we must prioritize measuring racism past the individual and interpersonal levels to consider systemic factors at a societal level that manifest as structural racism to improve HIV outcomes in the U.S., (2) consider intergenerational effects of structural racism through the use of longitudinal data, and (3) broaden the agenda of structural racism to incorporate other systems of oppression. Additionally, broadening the scope of funding and inclusion of more researchers and individuals with lived experiences to support structural racism research to drive the scientific agenda and design of structural-level interventions will not only bolster achieving the U.S. Ending the HIV Epidemic goals but will do so by addressing inequities.

Abstract:BackgroundLimited literature exists on structural racism measures on health outcomes for Asian Americans, Native Hawaiians, and Pacific Islanders (AAs and NH/PIs). AAs and NH/PIs make up approximately 6.2% of the U.S. population and consist of diverse ethnic subgroups with distinct languages, cultures, religions, socioeconomic statuses, and historical backgrounds. The lack of disaggregated data collection and contextualized measures hinders our understanding of how structural racism affects health outcomes in these populations.MethodsWe conducted a scoping review to assess the extent to which measures of structural racism are used in research with AAs and NH/PIs. Databases, including CINAHL, EBSCO, PsychINFO, PubMed, Scopus, and Social Science Citation Index, were searched for peer-reviewed articles on the measures of and empirical impacts of structural racism on AA and NH/PI health. We identified 23 full-text articles from a pool of 11,660 screened articles. Four articles were included in the final analysis.ResultsAmong the selected studies, two studies identified an association between racial segregation and mental and behavioral health outcomes within AAs and NH/PIs. The other two studies found redlining on chronic health outcomes in these communities. These studies uncovered associations between government systems and policies and AA and NH/PI health outcomes.DiscussionExisting measures may not adequately capture the complex relationships between structural racism and health outcomes in AAs and NH/PIs. Future research should contextualize and operationalize the multifaceted manifestations of structural racism unique to AAs and NH/PIs to achieve health equity.

The Impact of Structural Racism on Continuity of Care at Pediatric Academic Primary Care Clinics

Abstract:Objective: Using a structural racism framework, we assessed racial inequities in continuity of care, using the Usual Provider Continuity Index (UPC - the proportion of visits with the provider the patient saw most frequently out of all visits), in a set of large pediatric academic clinics. Methods: We conducted a retrospective cohort study. Patients 12-24 months seen at three pediatric academic primary care clinics for any visit during October 1-31, 2021 were included. We then reviewed continuity for these patients in the preceding 12 months. Outcomes included each patient's UPC for all visits, and a modified UPC for well child checks only (UPC Well). Covariates included race, ethnicity, insurance, clinic site, age, sex, care management, or seeing a social worker. We evaluated for differences in outcomes using bivariate analyses and multivariable regression models. Results: Our cohort included 356 patients (74% Black, 5% Hispanic, 85% Medicaid, 52% female, median age 15.8 months). The median UPC was 0.33 and median UPC Well was 0.40. Black patients had significantly lower median values for UPC (0.33 Black vs 0.40 non-Black, P < .01) and UPC Well (0.33 Black vs 0.50 non-Black, P < .01). There were similar inequities in continuity rates by insurance and clinic site. In multivariable models, clinic site was the only variable significantly associated with continuity. Conclusions: Clinic sites serving higher percentages of Black patients had lower rates of continuity. The main driver of racial inequities in continuity rates was at the institutional level.

Abstract:It's about time. Since 2020, new work on structural racism and health has erupted in the United States, with 93% of the 1655 PubMed articles indexed by "structural racism" as of April 4, 2024 having been published during or since 2020. Among the 310 review articles included, most have focused either on improving conceptual, substantive, and methodologic clarity and precision about measuring and modeling structural racism, or reviewing evidence about its impacts on specific exposures or outcomes. However, only 2.5% of these 1655 articles are also indexed by the term "lifecourse" and its variants, and among the reviews considered here, none explicitly discuss issues involving etiologic period. Informed by ecosocial theory's temporal theorizing about pathways of embodiment, and also Latin American social medicine-collective health framings of "health-illness-disease processes," lifecourse models, and the construct of the "cancer control continuum," in this brief commentary I consider how time matters in relation to concrete examples involving structural racism and cancer, and also inconsistent results reported by several studies using latent measures of structural racism. When it comes to structural racism and health, it truly is about time - and it is time for this work to tackle issues of time.

Abstract:BACKGROUND AND OBJECTIVES There are well-documented links between structural racism and inequities in children’s opportunities. Yet, when it comes to understanding the role of the built environment, a disproportionate focus on redlining obscures other historical policies and practices such as blockbusting, freeway displacement, and urban renewal that may impact contemporary child development. We hypothesized that historical structural racism in Allegheny County, Pennsylvania’s, built environment would be associated with fewer contemporary educational, socioeconomic, and health opportunities. We also hypothesized that these measures would explain more collective variance in children’s opportunities than redlining alone. METHODS We used geospatial data from the US Census, Mapping Inequality Project, and other archival sources to construct historical measures of redlining, blockbusting, freeway displacement, and urban renewal in ArcGIS at the census tract level. These were linked with data from the Child Opportunity Index 2.0 to measure children’s opportunities across domains of education, socioeconomic status, and health. We ran spatial regression analyses in Stata 18.0 to examine individual and collective associations between structural racism and children’s opportunities. RESULTS Historical redlining, blockbusting, and urban renewal were largely associated with fewer contemporary educational, socioeconomic, and health opportunities, and explained up to 47.4% of the variance in children’s opportunities. The measures collectively explained more variance in children’s opportunities than redlining alone. CONCLUSIONS In support of our hypotheses, novel measures of structural racism were related to present-day differences in children’s opportunities. Findings lay the groundwork for future research focused on repairing longstanding harm perpetuated by structural racism.

Abstract:Despite growing interest in the health-related consequences of racially discriminatory institutional policies and practices, public health scholars have yet to reach a consensus on how to measure and analyze exposure to institutional racism. The purpose of this paper is to provide an overview of the conceptualization, measurement, and analysis of institutional racism in the context of quantitative research on minority health and health disparities in the United States. We begin by providing definitions of key concepts (e.g., racialization, racism, racial inequity) and describing linkages between these ideas. Next, we discuss the hypothesized mechanisms that link exposure to institutional racism with health. We then provide a framework to advance empirical research on institutional racism and health, informed by a literature review that summarizes measures and analytic approaches used in previous studies. The framework addresses six considerations: (1) policy identification, (2) population of interest, (3) exposure measurement, (4) outcome measurement, (5) study design, and (6) analytic approach. Research utilizing the proposed framework will help inform structural interventions to promote minority health and reduce racial and ethnic health disparities.

Abstract:Objectives This paper aims to examine the psychometric properties of social capital indicators, comparing Black and White respondents to identify the extent of measurement invariance in social capital by race. Study design We used data from the longitudinal study Midlife in the United States (MIDUS), waves 1 through 3 (1995-2016). Methods Data were from 6513 respondents (5604 White and 909 Black respondents). Social capital indicators were social cohesion, contributions to community, and community involvement. We used Structural Equation Modeling and Item Response Theory methods to test for measurement invariance in social capital by race. Results We observed violations of longitudinal and multi-group measurement invariance (MI) at configural and metric levels on two scales. Factor structures and indicator loadings were inconsistent over time. In IRT analysis, 'Many people come for advice' exhibited Differential Item Functioning (DIF), indicating a consistent advantage for White respondents on the contributions to community scale. Despite similar social capital levels (P(χ2,2) = 0.00), DIF was found in all contributions to community items and some community involvement items when examining race and education interaction. Conclusions Invariance issues in social capital items suggest potential biases in comparing Black and White respondents. Recognizing these biases is essential. Future social capital research should assess existing data assumptions and involve stakeholders from diverse communities in creating new items.

Abstract:Background: Racial disparities in COVID-19 incidence and outcomes have been widely reported. Non-Hispanic Black patients endured worse outcomes disproportionately compared with non-Hispanic White patients, but the epidemiological basis for these observations was complex and multifaceted. Objective: This study aimed to elucidate the potential reasons behind the worse outcomes of COVID-19 experienced by non-Hispanic Black patients compared with non-Hispanic White patients and how these variables interact using an explainable machine learning approach. Methods: In this retrospective cohort study, we examined 28,943 laboratory-confirmed COVID-19 cases from the OneFlorida Research Consortium's data trust of health care recipients in Florida through April 28, 2021. We assessed the prevalence of pre-existing comorbid conditions, geo-socioeconomic factors, and health outcomes in the structured electronic health records of COVID-19 cases. The primary outcome was a composite of hospitalization, intensive care unit admission, and mortality at index admission. We developed and validated a machine learning model using Extreme Gradient Boosting to evaluate predictors of worse outcomes of COVID-19 and rank them by importance. Results: Compared to non-Hispanic White patients, non-Hispanic Blacks patients were younger, more likely to be uninsured, had a higher prevalence of emergency department and inpatient visits, and were in regions with higher area deprivation index rankings and pollutant concentrations. Non-Hispanic Black patients had the highest burden of comorbidities and rates of the primary outcome. Age was a key predictor in all models, ranking highest in non-Hispanic White patients. However, for non-Hispanic Black patients, congestive heart failure was a primary predictor. Other variables, such as food environment measures and air pollution indicators, also ranked high. By consolidating comorbidities into the Elixhauser Comorbidity Index, this became the top predictor, providing a comprehensive risk measure. Conclusions: The study reveals that individual and geo-socioeconomic factors significantly influence the outcomes of COVID-19. It also highlights varying risk profiles among different racial groups. While these findings suggest potential disparities, further causal inference and statistical testing are needed to fully substantiate these observations. Recognizing these relationships is vital for creating effective, tailored interventions that reduce disparities and enhance health outcomes across all racial and socioeconomic groups.
