Abstract:The objective of this article is to develop a validated mobile app prototype to empower the elderly and caregivers to manage falls that provides personalized and actionable educational materials at the point of care and improves the engagement of the elderly and caregiver in adopting validated fall management practices; To determine the usefulness and suitability of a fall management mobile app to the elderly and caregivers. The method used is a knowledge management approach is used to implement the app based on 2 validated models: Patient Health Engagement Model and Rockwood frailty index. A mixed method evaluation including a cognitive walk through is used to collect end-user feedback from the elderly and caregivers, on the usability, usefulness, and suitability of the app. The app was deemed easy to use, informative and understandable. Potential improvement areas include: larger print; less wordy interfaces; better navigation features; data sharing functionalities; and voice readers. These suggestions will be incorporated in the future. The conclusion of this article is that smartphones have vast potential in providing relevant and creditable fall management information to elderly and caregivers.

Abstract:With the rapid growth of the population of the elderly in the world, many countries in the world have been in the aging society. Falls are a major public health concern and one of the greatest obstacles to independent living for elder adults in terms of social services. In this paper, we present a fall detection system derived from some motion sensor via an android-based smart phone in which we utilize adaptive threshold algorithm for fall detection. Furthermore, we implement this system called E-FallD on the HTC G8 mobile phone. As demonstrated by the experimental results, E-FallD has a good detection performance and it is an efficient and smart application. In the mean time, using adaptive threshold algorithm could increase sensitivity dramatically. In a word ,our system provides a realizable, effective and portal solution to fall detection for users.

Abstract:According to World Health Organization statistics, falls are the second leading cause of unintentional injury deaths worldwide. With older people being particularly vulnerable, detecting, and reporting falls have been the focus of numerous health technology studies. We screened 267 studies and selected 15 that detailed pervasive fall detection and alerting apps that used smartphone accelerometers. The fall datasets used for the analyses included between 4 and 38 participants and contained data from young and old subjects, with the recorded falls performed exclusively by young subjects. Threshold-based detection was implemented in six cases, while machine learning approaches were implemented in the other nine, including decision trees, k-nearest neighbors, boosting, and neural networks. All methods could ultimately achieve real-time detection, with reported sensitivities ranging from 60.4 to 99.3% and specificities from 74.6 to 100.0%. However, the studies had limitations in their experimental set-ups or considered a restricted scope of daily activities—not always representative of daily life—with which to define falls during the development of their algorithms. Finally, the studies omitted some aspects of data science methodology, such as proper test sets for results evaluation, putting into question whether reported results would correspond to real-world performance. The two primary outcomes of our review are: a ranking of selected articles based on bias risk and a set of 12 impactful and actionable recommendations for future work in fall detection.

Abstract:BackgroundFall incidents are a major problem for patients and healthcare. The “Aachen Fall Prevention App” (AFPA) represents the first mobile Health (mHealth) application (app) empowering older patients (persons 50+ years) to self-assess and monitor their individual fall risk. Self-assessment is based on the “Aachen Fall Prevention Scale,” which consists of three steps. First, patients answer ten standardized yes–no questions (positive criterion ≥ 5 “Yes” responses). Second, a ten-second test of free standing without compensatory movement is performed (positive criterion: compensatory movement). Finally, during the third step, patients rate their subjective fall risk on a 10-point Likert scale, based on the results of steps one and two. The purpose of this app is (1) to offer a low-threshold service through which individuals can independently monitor their individual fall risk and (2) to collect data about how a patient-centered mHealth app for fall risk assessment is used in the field.ResultsThe results represent the first year of an ongoing field study. From December 2015 to December 2016, 197 persons downloaded the AFPA (iOS™ and Android™; free of charge). N = 111 of these persons voluntarily shared their data and thereby participated in the field study. Data from a final number of n = 79 persons were analyzed due to exclusion criteria (age, missing objective fall risk, missing self-assessment). The objective fall risk and the self-assessed subjective risk measured by the AFPA showed a significant positive relationship.ConclusionsThe “Aachen Fall Prevention App” (AFPA) is an mHealth app released for iOS and Android. This field study revealed the AFPA as a promising tool to raise older adults’ awareness of their individual fall risk by means of a low-threshold patient-driven fall risk assessment tool.

Abstract:Smartphone applications aimed at enhancing physical, cognitive, and social activities through mobile health (mHealth) technology are of increasing interest. Their feasibility and acceptability, alongside impacts on frailty phenotype scores and step counts among older adults with frailty, remain to be fully validated. This study presents a 13-week preliminary intervention trial assessing an mHealth app's feasibility in a cohort of 34 eligible older adults, including 5 frail and 29 pre-frail participants. The intervention entailed a 6-week course on app usage, followed by 7 weeks of observation, with four participants withdrawing early. Feasibility was determined by login and active use rates, with a target login rate of 60% or higher. Post-intervention, 100% session attendance and a median login rate of 88.4% were observed. Acceptability was high, with 73% affirming the app's health benefits. Notably, frailty scores and step counts improved post-intervention, underscoring the app's potential for supporting older adults with frailty.

Abstract:Fall risks to health and safety are especially dangerous for those with impairments. An automated fall detection system is necessary, especially in medical and senior care. The elderly and individuals with impairments are particularly susceptible to falls, which frequently result in severe injuries and complications, thereby presenting a considerable threat to their overall health. The early discovery and response to a fall incidence can reduce immobilization and consequent health complications, saving lives. Automatic fall detection systems quickly and reliably indicate falls and dispatch medical or emergency assistance. Researchers have introduced various automatic fall detection methods using machines or deep learning. Most fall detection systems depend on wearable or stationary sensors, which restricts the user's mobility and accessibility. Conversely, mobile sensor-based fall detection leverages the widespread presence of smartphones by obtaining motion information via their integrated accelerometers and gyroscopes. Our primary objective is to develop a reliable fall detection method using a mobile phone sensor and machine learning. This paper examines several methods employed in the identification of falls and emphasizes the significance of utilizing mobile phone sensors in the process of fall detection. It also discusses recent research in this domain and highlights research challenges. This could potentially foster further innovation in the field.

Abstract:The constant progress of technology, especially in the area of health, brings numerous benefits, one of which is the increase in human life expectancy. However, problems that occur recurrently among the elderly age group are now on the radar of studies that also seek to improve the quality of life of these people. The number of cases of falls among elderly people is worrying, even more so as this is a portion of the population that tends to live alone. In the context of smart homes, several solutions have emerged for monitoring elderly people to increase safety and provide faster assistance, if necessary. One of these solutions is the use of wearable devices responsible for identifying the person’s movements. This work presents the study and development of a wearable device capable of detecting falls and, if they occur, automatically notifying the necessary people through alert messages via the Telegram application so that they can help the person who has suffered a fall. In this work, a Wi-Fi network, MQTT protocol, accelerometer and gyroscope inertial sensors and an ESP32 board programmed using the Arduino IDE were used. Preliminary tests indicated good performance in recognizing falls, based on tilt angle analysis, gyroscope readings and accelerometer readings. The proof of concept and preliminary tests carried out demonstrate the potential for using low-cost technologies for wearable resources for application in smart homes and monitoring the health of elderly people.

Abstract:Prevention of falls requires accurate means for fall risk assessment in order to identify persons at risk. This paper introduces a novel mobile fall risk assessment solution for daily-life settings. The solution contains an Android application that uses acceleration sensor data received via Bluetooth LE connection. The application guides through a simple walk test, analyzes the acceleration data measured from the acceleration sensor attached to the lower back and gives feedback about the fall risk for the user. Preliminary user tests with 12 healthy subjects were conducted to evaluate the feasibility of the solution. Each test subject performed three walks demonstrating normal, dragging and slow gait. The results showed that the acceleration features calculated by the application distinguish normal gait from dragging and slow gaits. Further collection of comprehensive data set with older adults is needed to adjust the application parameters appropriately for the target group.

Modern technology, such as the Elderly Fall Detection System, have been created to solve this issue. Fall Perception for Elderly Care: A Fall Detection Algorithm in Smart Wristlet Mhealth System

Abstract:Mobile Health (mHealth) is expected to play a special role in today and the future healthcare delivery. Based on this trend, we design a Smart Wristlet mHealth system with mobile interface. The designed Smart Wristlet is dedicated to offer real-time alert for elderly fall, which is the most important when population ageing is becoming. In the Smart Wristlet mHealth system, fall detection is the bottleneck of the system operation. To remove this bottleneck away, we propose a fall perception solution for elderly care. In this proposal, we abstract and construct primitive-based features from raw data collected by the Smart Wristlet mHealth system, in which the most valuable features can be selected by using a TF-IDF (Term Frequency Inverse Document Frequency) metric. In reality, these selected features are the most effective to perform fall detection. Our system tests and clinical trials demonstrate that this proposal is eligible to turn the Smart Wristlet mHealth system into a real solution for elderly care. Results show that the recognition precision and recall can reach 93% and 88%, respectively. Compared with existing solutions, the gain from our proposal is an efficient prevention method for elderly fall, and can save more than 800 million dollars per year at today's socioeconomic level.

Abstract:One of the most important measures that developed countries need along with economic success is the provision of contemporary health services for the elderly. Inadequate care for the elderly can put them in danger of falls with serious injury or even death. When an older person falls due to illness, immediate lack of care may lead to death. In this paper, a wearable smart phone sensor fall event detection system is introduced. The proposed system utilizes real-time raw sensory accelerometer, gyroscope and gravity data collected from the AndroSensor App and is then processed and applied to a machine learning classifier. The smart phone is positioned at the waistline to yield the most accurate fall detection results. The system is trained and can detect common activities, like sleeping, walking, sitting, jogging, and events, such as falling in any direction. An full accuracy is almost accomplished using Support Vector Machine classifier in comparison with other classifiers as addressed by other previous research works. Nevertheless, the system requires more validation with elderly people under the supervision of caregivers in a controlled environment.

Abstract:Falls are a major health risk that diminishes the quality of life among the elderly people. The importance of fall detection increases as the elderly population surges, especially with aging “baby boomers”. However, existing commercial products and academic solutions all fall short of pervasive fall detection. In this paper, we propose utilizing mobile phones as a platform for developing pervasive fall detection system. To our knowledge, we are the first to do so. We propose PerFallD, a pervasive fall detection system tailored for mobile phones. We design two different detection algorithms based on the mobile phone platforms for scenarios with and without simple accessories. We implement a prototype system on the Android G1 phone and conduct extensive experiments to evaluate our system. In particular, we compare PerFallD’s performance with that of existing work and a commercial product. The experimental results show that PerFallD achieves superior detection performance and power efficiency.

Abstract:The elderly population is growing every year in Brazil. Consequently, health risks in elderly is a concern for public health system. During the aging process, the mobility is affected, and falls are more frequent causing injuries and even death, whose causes can be prevented, with reduction of financial costs. Therefore, a low-cost inertial sensor-based system is a tool to fulfill the need for detecting falls in elderly. In this paper, we present our system as a proof of concept for the study of fall and we propose a low cost and more accessible system for fall detection using inertial sensors. The inertial sensor collects data, identifies and detect four different “fall states”. The aim is to use this system in public health. In real-time, it will advise any person around the elder about the fall. Different machine learning classifiers are tested in the train dataset, and the best one was used for training the sensor data. Then, the model was compared with unknown sensor data (captured and from available datasets) to guess at which state the person is. We found out that there were only 15 wrong observations from all trials, thus, the system has potential to be used to detect falls.

Abstract:Timely and reliable detection of falls is a large and rapidly growing field of research due to the medical and financial demand of caring for a constantly growing elderly population. Within the past 2 decades, the availability of high-quality hardware (high-quality sensors and AI microchips) and software (machine learning algorithms) technologies has served as a catalyst for this research by giving developers the capabilities to develop such systems. This study developed multiple application components in order to investigate the development challenges and choices for fall detection systems, and provide materials for future research. The smart application developed using this methodology was validated by the results from fall detection modelling experiments and model mobile deployment. The best performing model overall was the ResNet152 on a standardised, and shuffled dataset with a 2s window size which achieved 92.8% AUC, 87.28% sensitivity, and 98.33% specificity. Given these results it is evident that accelerometer and ECG sensors are beneficial for fall detection, and allow for the discrimination between falls and other activities. This study leaves a significant amount of room for improvement due to weaknesses identified in the resultant dataset. These improvements include using a labelling protocol for the critical phase of a fall, increasing the number of dataset samples, improving the test subject representation, and experimenting with frequency domain preprocessing.

Abstract:Background: Falls in older adults are a serious public health problem that can lead to reduced quality of life or death. Patients often do not receive fall prevention guidance from primary care providers, despite evidence that falls can be prevented. Mobile health technologies may help to address this disparity and promote evidence-based fall prevention. Objective: Our main objective was to use Human-Centered Design (HCD) to develop a user-friendly, fall prevention exercise app using validated user requirements. The app features evidence-based behavior change strategies and exercise content to support older people initiating and adhering to a progressive fall prevention exercise program. Methods: We organized our multi-stage, iterative design process into three phases: Gathering User Requirements, Usability Evaluation, and Refining App Features. Our methods include focus groups, usability testing, and subject matter expert meetings. Results: Focus groups (Total n=6), usability testing (n=30) including a post-test questionnaire [Health-ITUES score: mean (SD)= 4.2 (1.1)], and subject matter expert meetings demonstrate participant satisfaction with the app concept and design. Overall, participants saw value in receiving exercise prescriptions from the app that would be recommended by their PCP and reported satisfaction with the content of the app, but several participants felt that they were not the right user for the app. Conclusions: This study demonstrates the development, refinement and usability testing of a fall prevention exercise app and corresponding tools that primary care providers may use to prescribe tailored exercise recommendations to their older patients as an evidence-based fall prevention strategy accessible in the context of busy clinical workflows.

Molecular analysis reveals somatically mutated and unmutated clonal and oligoclonal B cells in T‐cell‐rich B‐cell lymphoma

Abstract:This paper describes the immunohistology and molecular genetics of 18 cases of T‐cell‐rich B‐cell lymphoma (TCRBL). In all cases, the large B cells stained strongly for CD20, with more variable expression of CD79a, and were negative for CD30 and CD15. The majority of T cells were predominantly positive for TIA‐1 and negative for CD57; a large population of histiocytes was present in all cases. Epstein–Barr virus (EBV)‐coded RNA (EBER) was found in B blasts from four cases and in one case was present among the background lymphoid cells. IgH PCR products were generated in 16/18 cases and revealed clonal, oligoclonal and polyclonal PCR products in 12, two and two cases, respectively. In addition, TCRG clonal gene rearrangements were identified in two cases. TCRB gene rearrangements were polyclonal. Sequence analysis of seven cases with clonal/oligoclonal IgH gene rearrangements revealed functional sequences with predominant VH3 gene usage associated with various D genes and JH4 or JH6 gene segments. Four cases displayed varying degrees of replacement and silent mutations (1.8–21%), with one case exhibiting intraclonal heterogeneity; the distribution of mutations was indicative of antigen selection in three cases. The remaining three cases, including two cases with functional oligoclonal IgH rearrangements, harboured unmutated V region genes. The EBV‐positive cases were associated with clonal, oligoclonal and polyclonal PCR products and with mutated and germline clonal sequences. These data indicate that TCRBL may be a heterogeneous entity associated with clonal and oligoclonal B cells derived from both germinal centre and naïve B cells. Abstract:As we grow old, our desire for independence does not diminish; yet our health increasingly needs to be monitored. Injuries such as falling can be a serious problem for the elderly. If a person falls and is not able to get assistance within an hour, casualties arising from that fall can result in fatalities as early as 6 months later [1]. It would seem then that a choice between safety and independence must be made. Fortunately, as health care technology advances, simple devices can be made to detect or even predict falls in the elderly, which could easily save lives without too much intrusion on their independence. Much research has been done on the topic of fall detection and fall prediction. Some have attempted to detect falls using a variety of sensors such as: cameras, accelerometers, gyroscopes, microphones, or a combination of the like. This paper is aimed at reporting which existing methods have been found effective by others, as well as documenting the findings of our own experiments. The combination of which will assist in the progression towards a safe, unobtrusive monitoring system for independent seniors.

The following questions are investigated: the type of sensors and their sampling rate, the type of signal and data processing employed, the scales and tests used in the study and the type of application.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>We identified 608 studies, from which 29 were included. The accelerometer, with sampling rate 50 or 100 Hz, allocated in the waist or lumbar was the most used sensor setting. Methods comparing features or variables extracted from the accelerometry signal are the most common, and fall risk screening the most observed application.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>This review identifies the main elements to be addressed in studies on the detection of events related to falls in the elderly and may help in future studies on the subject. However, some aspects are still no reach consensus in the literature such as the size of the sample to be studied, the population under study and how to acquire data for each application.</p>

Abstract:Falldetection receives significant attention in the field of preventive medicine, wellness provision and assisted living, especially for the elderly. As a result, numerous commercial fall detection systems exist to date and most of them use accelerometers and/ or gyroscopes attached on a person's body as primary signal sources. These systems use either discrete sensors as part of a product designed specifically for this task or sensors that are embedded in mobile devices such as smartphones. The latter approach has the advantage of offering well tested and widely available communication services, e.g. for calling emergency if necessary, when someone has fallen. Apparently, automatic fall detection will continue to evolve in the following years. The aim of this work is to introduce a human activity dataset that will be helpful in testing new methods, as well as performing objective comparisons between different algorithms for fall detection and activity recognition, based on inertial-sensor data from smartphones. The dataset contains signals recorded from the accelerometer and gyroscope sensors of a latest technology smartphone for four different falls and nine different activities of daily living. Using this dataset, the results of an initial evaluation of three fall detection algorithms are finally presented.
