Abstract:Objective To explore the common viral composition of acute diarrhea from Zhejiang province.Methods 811 stool specimens were collected from outpatient children(≤5 years) with acute diarrhea.They were tested by ELISA for group A rotavirus and by multiplex PCR for Group B/C rotavirus,Norovirus,Sapovirus,Adenovirus,and Astrovirus.For group A rotavirus-positive specimens,a RT-PCR was conducted to determine the G and P genotypes.Results In 811 cases,the overall proportions for rotavirus,norovirus,sapovirus,adenovirus,and astrovirus were 25.52% 、18.37% 、4.44% 、2.71% and 1.23%,respectively.Mixed infections were detected in 65(8.0%) of the 811 cases and rotavirus/norovirus dual infections took the largest percentage(56.92%).G3(37.93%),G1(32.75%) and P [8] were the most prevalent genotypes and G3/P[8](27.59%) was the predominant combination in 116 RV-positive samples.Conclusion The results indicated the prevalence of the common agents of viral diarrhea(including mono-infection and mixed infection) in Zhejiang province,which would provide valuable reference for clinical treatment and disease surveillance.

Abstract:Background Multiplex-PCR is a valuable tool for diagnosing viral acute gastroenteritis (AGE), enabling the detection of multiple pathogens. However, distinguishing between active disease and shedding poses challenges. This study aimed to evaluate viral AGE epidemiology and compare clinical characteristics among the five most common viruses. Methods Rotavirus vaccine was introduced in 2010, with 70% coverage achieved in southern Israel in two years. All rectal swabs for multiplex-PCR targeting rotavirus, norovirus, adenovirus, astrovirus and sapovirus from hospitalized diarrheic children <5 years were included, from December 2017 through March 2022. Detection of the same virus within two months was considered a single episode. Clinical analysis included episodes with single-virus detection and negative bacterial PCR. Results Among 5,879 rectal swabs, 2,662 (45.3%) tested positive for at least one virus, with 245 (9.2%) showing multiple virus detection. Rotavirus was the most prevalent. While rotavirus exhibited typical winter-spring seasonality in 2018-19, an unusual off-season surge was observed during the second year of the COVID-19 pandemic. Among negative bacterial PCR episodes, 34.6% had mucus stool, 5.9% had bloody stool, and 29.3% received antibiotics. Astrovirus or sapovirus infections were associated with higher rates of hospital-acquired AGE and immunodeficiency ( P <0.05), whereas rotavirus infections had higher rates of dehydration severity and acute kidney injury ( P <0.05). Discussion Enteric viruses were detected in 45.3% of rectal swabs from hospitalized children with diarrhea. Despite vaccination efforts, rotavirus remained prevalent and caused more severe disease. Continuous surveillance using multiplex-PCR is crucial for accurate management and future prevention strategies for viral AGE.

Abstract:Utilizing multiplex real time polymerase chain reaction (RT-PCR) for rapid diagnosis of gastroenteritis, enables simultaneous detection of multiple pathogens. A comparative analysis of disease characteristics was conducted between cases with single and multiple viruses. Rotavirus vaccine was introduced in 2010, reaching a 70% coverage in 2 years. All rectal swabs collected from diarrheic children (<5 years) between December 2017 and March 2022 were included. Detection of the same viruses within 2 months was considered a single episode. Episodes with positive stool bacterial PCR were excluded. A total of 5879 samples were collected, revealing 86.9% (1509) with single virus detection and 13.1% (227) with multiple viruses. The most frequent combination was rotavirus and norovirus (27.8%), these infections followed a winter-spring seasonality akin to rotavirus. Children with multivirus infections exhibited higher immunodeficiency (OR 2.06) rates, but lower food allergy (OR 0.45) and prematurity rates (OR 0.55) compared to single infections. Greater disease severity, evaluated by the Vesikari score, was observed in multivirus episodes (p < 0.001, OR 1.12). Multivirus infections accounted for 13.1% of symptomatic cases in hospitalized young children. Despite vaccination efforts, rotavirus remained prominent, frequently in co-infections with norovirus. Overall, multivirus infections were linked to more severe diseases than single virus cases.

Abstract:Rotavirus (RV), norovirus (NoV), sapovirus (SaV), human astrovirus (HAstV) and human adenovirus (HAdV) are significant because they are the most common pathogens that cause diarrhea in young children. The aim of this study was to investigate the genetic characteristics and compare the roles of these five viruses in outpatient children with diarrhea in Shanghai. A total of 436 fecal samples were collected from pediatric patients with acute gastroenteritis from January 2010 to December 2011. The selected samples were subjected to reverse transcription PCR (RT-PCR) or PCR to detect and genotype RV, NoV, SaV, HAstV and HAdV. RV (43.3 %, 189/436) was the most prevalent virus, followed by NoV (28.9 %, 126/436), HAdV (7.1 %, 31/436). HAstV (1.8 %, 8/436) and SaV (0.5 %, 2/436). The percentage of multiple infection cases was 14.9 % (65/436), and RV + NoV was the predominant mixed infection. The RV genotype combinations of P[8]G3 (52/189, 27.5 %), P[8]G1 (51/189, 26.9 %) and P[8]G9 (48/189, 25.4 %) occurred most frequently. The predominant NoV genotype was GII.4 (73.0 %, 92/126), and the majority of GII.4 clustered as GII.4-2006b (65.2 %, 60/92). Two of the SaV cases were identified as GI.2 and GII.1. All HAstV-positive samples belonged to HAstV-1. The predominant HAdV type was HAdV-41 (45.2 %, 14/31). This study clearly shows the diversity of the viral causative agents of acute gastroenteritis in outpatient children in Shanghai, which will provide baseline information for future vaccination strategies and development in this area.

Diagnosing viral gastro-enteritis using the fully automated sample-in, result-out STARlet All in one system (AIOS)

Abstract:The STARlet All-In-One system is a modular platform that integrates the complete molecular diagnostic workflow from nucleic acid extraction of clinical samples to PCR set-up and amplification. The platform was evaluated in comparison with laboratory developed tests (LDT) on fecal samples from patients with suspected viral gastro-enteritis. In a retrospective study, 72 positive samples were analysed, including all pathogens detected by the Seegene Allplex™ GI-virus assay, adenovirus, astrovirus, norovirus GI and GII, sapovirus, and rotavirus. Concordant results were obtained for 69 samples (96 %). Three discordant results were observed, one norovirus GII positive that gave an invalid result in the AIOS and two samples that were negative in the AIOS. One adenovirus positive that was subtyped as a genotype 2 virus, which is not associated with gastro-enteritis, and a sapovirus. In the prospective part of the study, 661 fecal samples were included. A total of 61 positive samples were detected, of which 60 were also detected by the AIOS. One norovirus GII positive sample (CT 35.2) was tested negative in the AIOS. Two additional sapovirus positive samples, CT 37 and 38, were detected by the AIOS but not by the LDT. The STARlet All-In-One platforms results in an automated molecular workflow with reduced hands-on time and enables running assays during out of office hours. Application of the Seegene Allplex™ GI-virus assay showed excellent concordance to the current diagnostic LDT. In a prospective comparison, only three discordant results were observed, all with CT values over 35 and therefore unlikely of clinical relevance.
Changes in Glycosaminoglycans During the Neuritogenesis in PC12 Pheochromocytoma Cells Induced by Nerve Growth Factor

Abstract:Abstract: Previously, we had suggested that heparan sulfate (HS) makes some contribution to a flat‐shaped morphology of PC12D cells. Therefore, we carried out quantitative and qualitative analyses of glycosaminoglycans (GAGs), the polysaccharide moiety of proteoglycans, during neuritogenesis in PC12 cells that is induced by nerve growth factor (NGF). (a) In PC12 cells, NGF induced a flat‐shaped morphology with a few short processes after 3 days of culture, and then it elicited short and long neurites after 6 (in ∼30% of cells) and 9 (in 60–70%) days of culture, respectively, (b) HS and chondroitin sulfate (CS) were detected in the cell layer at all times. Only CS was found in the medium at 3 and 6 days, whereas a low level of HS, in addition to CS, was detectable on day 9. (c) In the NGF‐treated cultures, the amounts of cell‐associated HS per cell were two to three times as high as those in the respective nontreated cultures at all times, whereas the amount based on phospholipid was about twofold higher after 3 days of culture. (d) The levels of HS labeled with [35S]sulfate during the last 48 h of the culture were 1.5‐to twofold higher in the NGF‐treated cultures than in the respective controls at any time. (e) The amount of cell‐associated CS per cell (or per unit of phospholipid), but not of labeled CS per cell, was transiently enhanced at 3 days in culture with or without NGF. At all times, NGF treatment caused an increase in the levels of total and [35S]sulfate‐labeled CS associated with the cells and released into the medium, (f) NGF enhanced the amount of N‐sulfation of glucosamine residues of HS at all times, but it did not change the ratio of 4‐sulfate units to 6‐sulfate units in CS. (g) At 3 days in culture, the uptake of [35S]sulfate by PC12 cells was lower in the NGF‐treated culture than in the nontreated control. (h) In chase experiments, the percentage of unrecovered CS was about twofold higher in the NGF‐treated culture than in the non‐treated control. These results suggest that the enhanced synthetic activity and the accumulation of GAGs as well as the structural change of HS induced by NGF occur preceding the neurite elongation from PC12 cells. Also, it is suggested that the increase in content of HS is closely correlated with the morphological change from round to flat in PC12 cells.

Abstract:Background: Gastroenteritis is a major cause of childhood morbidity and mortality worldwide. Rotavirus, human caliciviruses (HucV), adenovirus, and astrovirus are recognized as common etiologies of acute gastroenteritis.Objectives: To use antigen detection and molecular methods to determine the viral etiology of childhood diarrhea in Lanzhou, China, 2005-2007.Study design: 544 stool specimens were collected from children hospitalized with diarrhea. ELISA, RT-PCR, or PCR were used to detect viruses commonly causing diarrhea.Results: Group A rotavirus, norovirus, sapovirus, astrovirus, and adenovirus, were detected in 54.0%, 9.2%, 1.1%, 3.3%, and 4.4%, respectively. No group B or group C rotaviruses were detected. The relative contribution of these viruses changed greatly over 2 years. The percentage of rotavirus and adenovirus dropped from 61.2% and 5.4% to 47.6% and 1.4%, whereas HucV increased from 5.0% to 15.0%. G1 and P[8] were the predominant rotavirus strains, and P[6] was detected for the first time in this area. The predominant norovirus strain changed from GII3 to GII4, and the subtypes of GII4 changed from the Hunter strain to the variant 2006b strain.Conclusions: The distribution of viruses and genotypes of individual viruses causing gastroenteritis in Lanzhou, China changed greatly during 2005-2007. Abstract:BACKGROUND: Noroviruses are recognized as the most common cause of outbreaks of acute gastroenteritis. Yet, diagnostic testing for norovirus is based mostly on RNA detection by RT-PCR, which is not widely available. While antigen detection tests (ELISAs) are easier to perform, they are in general less sensitive.OBJECTIVES: Our aim was to provide a scientific basis for declaring norovirus as the causative agent of an outbreak of acute gastroenteritis.STUDY DESIGN: Statistical analysis used binomial distribution to determine the minimal number of positive samples, and the probability of detecting the required number of positive samples, for different tests, required to assign norovirus as the causative agent of an outbreak of acute gastroenteritis.RESULTS: For either a standard RT-PCR or a commercially available ELISA, finding only 1 sample positive out of 2, 3 or 4 samples is sufficient to assign norovirus as the causative agent of an outbreak of acute gastroenteritis. However, when ELISA is used, the probability of detecting this required minimum number of positive samples is low when small numbers of samples are tested (57% when 2 samples are tested; 72% when 3 samples are tested). In order to reach a 90% probability of detecting a norovirus outbreak (false negativity at outbreak level <10%), at least 3 samples should be tested using RT-PCR, and 6 samples when using an ELISA.CONCLUSIONS: The sensitivity for NoV outbreak diagnosis will increase from 57% to 92%, or from 84% to 96%, for ELISA or RT-PCR respectively, when sample size increases from 2 to 6. Thus, using ELISA instead of RT-PCR for the detection of norovirus in stool samples will result in considerable numbers of false negative outbreaks unless a minimum of 6 samples are tested per outbreak.

Abstract:Quantitative molecular assays are increasingly used for detection of enteric viruses. We compared the clinical severity using modified Vesikari score (mVS) of enteric viruses detected by conventional assays (enzyme immunoassays [EIA] for rotavirus and adenovirus 40/41 and conventional polymerase chain reaction for astrovirus, sapovirus, and norovirus) and a quantitative molecular assay (TaqMan Array Card [TAC]) among children aged 0-59 months in the Global Enteric Multicenter Study. For rotavirus and adenovirus 40/41, we compared severity between EIA-positive and TAC-positive cases assigned etiologies using different cycle threshold (CT) cutoffs. Using conventional assays, the median (interquartile range) mVS was 10 (8, 11) for rotavirus, 9 (7, 11) for adenovirus 40/41, 8 (6, 10) for astrovirus, sapovirus, and norovirus GII, and 7 (6, 9) for norovirus GI. Compared to rotavirus EIA-positive cases, the median mVS was 2 and 3 points lower for EIA-negative/TAC-positive cases with CT<32.6 and 32.6≤CT<35, respectively (p-value<.0001). Adenovirus 40/41 EIA-positive and EIA-negative/TAC-positive cases were similar, regardless of CT cutoff. Quantitative molecular assays compared to conventional assays, such as EIA, may influence severity of identified cases, especially for rotavirus. Cutoffs to assign etiology for quantitative assays should be considered in the design and interpretation of enteric virus studies.

Abstract:ABSTRACT Laboratory diagnosis of orthohantavirus infection is primarily based on serology. However, for a confirmed serological diagnosis, evaluation of a follow-up serum sample is essential, which is time consuming and causes delay. Real-time reverse transcription polymerase chain reaction (RT-PCR) tests, if positive, provide an immediate and definitive diagnosis, and accurately identify the causative agent, where the discriminative nature of serology is suboptimal. We re-evaluated sera from orthohantavirus-suspected clinical cases in the Dutch regions of Twente and Achterhoek from July 2014 to April 2016 for the presence of Puumala orthohantavirus (PUUV), Tula orthohantavirus (TULV), and Seoul orthohantavirus (SEOV) RNA. PUUV RNA was detected in 11% of the total number ( n = 85) of sera tested, in 50% of sera positive for anti-PUUV/TULV IgM ( n = 16), and in 1.4% of sera negative or indeterminate for anti-PUUV/TULV IgM ( n = 69). No evidence was found for the presence of TULV or SEOV viral RNA. Based on these findings, we propose two algorithms to implement real-time RT-PCR testing in routine orthohantavirus diagnostics, which optimally provide clinicians with early confirmed diagnoses and could prevent possible further invasive testing and treatment. IMPORTANCE The addition of a real-time reverse transcription polymerase chain reaction test to routine orthohantavirus diagnostics may better aid clinical decision making than the use of standard serology tests alone. Awareness by clinicians and clinical microbiologists of this advantage may ultimately lead to a reduction in over-hospitalization and unnecessary invasive diagnostic procedures.

Abstract:Objective:To investigate the epidemiogical characteristics of rotavirus and human calicivirus gastroenteritis in pediatric diarrheal patients.Methods:The stool samples of 228 children with diarrhea were collected from April 2010 to March 2011,and the rotavirus and human calicivirus were determined by enzyme linked immunosorbent assay(ELISA) and reverse transcription-polymerase reaction(RT-PCR) respectively.Results:The positive rate of rotavirus and human calicivirus were 22.81%(52/228) and 32.89%(75/228) respectively.and difference of the positive rates had significant difference in different month(P<0.05).Difference of rotavirus positive rate in different clinical features(vomiting,stool character and stool routine test of white blood cells) were statistically significant(P<0.05),and difference of human calicivirus positive rate in different clinical feature(vomiting) was statistically significant(P<0.05).The erroneous diagnosis rates in rotavirus and human calicivirus positive cases were 78.85%,77.33% respectively.Conclusion:The infection rates of rotavirus and human calicivirus in children were high,and erroneous diagnosis rates in viral diarrhea was also high.

Abstract:Abstract Background As of 2017 (WHO, 2017), diarrheal disease ranked second as a cause of worldwide mortality for children under five years of age. Approximately 50-70% of acute gastroenteritis (AGE) is viral in etiology, with commonly detected viruses including norovirus, rotavirus, and adenovirus. However, the epidemiology of less commonly detected viruses, specifically enterovirus (EV) and parechovirus A (PeV-A), associated AGE in the United States is not well described. The purpose of our study was to determine the prevalence of EV and PeV-A in children with AGE vs. healthy controls (HC) over a 7-year period. Methods From December 2011 – November 2018, we screened stool samples from children less than 18 years of age prospectively enrolled in Children’s Mercy-Kansas City’s (CM-KC) site for the CDC’s New Vaccine Surveillance Network; 3005 samples from subjects presenting with AGE and 1097 from HC. Samples from 2011 – 2016 (AGE: 2453; HC:752) and 2017 – 2018 (AGE:552; HC:344) were tested at CDC and CM-KC respectively by a real-time-PCR assay using specific EV and PeV-A primers targeting the highly conserved 5’ untranslated region. Demographic data were collected from EMR. Results Among 3005 AGE samples, EV was detected in 12.5% (n=386/3004), and PeV-A in 10.3% (n=252/3005). Among 1097 HC samples, EV was detected in 9.0% (99/1096), and PeV-A in 11.9% (130/1097). In 2014-2015 EV detection in AGE was highest (17.9%) among all years and significantly higher (p=0.004) than in HC samples (9.1%), whereas PeV-A detection in AGE was 9.5% vs. 15.6% in HC samples, p=0.008 (Table 1). Co-infections with EV and PeV-A were seen in 55 AGE and 21 HC. Most EV detections (45.1%) were in 1- to 2-year-olds, whereas PeV-A detections (47.3%) were in children < 1 year old (Table 2). Both EV (58.3%) and PeV-A (48.4%) detections were significantly more frequent in male children (p=0.006). The highest frequency of EV detections was in summer to fall months, and for PeV-A in late summer through early winter. Conclusion We report a higher prevalence of EV infections in AGE, and PeV-A infections in HC during 2011 to 2018, plus their seasonal and age distributions. Although our data do not currently demonstrate an association between detection of EV or PeV-A types and AGE, additional data could provide more clarity into a potential association. Clinical Profiles of Childhood Astrovirus-, Sapovirus-, and Norovirus-Associated Acute Gastroenteritis in Pediatric Emergency Departments in Alberta, 2014-2018

Abstract:Background: Infections by previously underdiagnosed viruses astrovirus and sapovirus are poorly characterized compared with norovirus, the most common cause of acute gastroenteritis. Methods: Children <18 years old with acute gastroenteritis were recruited from pediatric emergency departments in Alberta, Canada between 2014 and 2018. We described and compared the clinical course of acute gastroenteritis in children with astrovirus, sapovirus, and norovirus. Results: Astrovirus was detected in 56 of 2688 (2.1%) children, sapovirus was detected in 146 of 2688 (5.4%) children, and norovirus was detected in 486 of 2688 (18.1%) children. At illness onset, ~60% of astrovirus cases experienced both diarrhea and vomiting. Among sapovirus and norovirus cases, 35% experienced diarrhea at onset and 80% of 91% (sapovirus/norovirus) vomited; however, diarrhea became more prevalent than vomiting at approximately day 4 of illness. Over the full course of illness, diarrhea was 18% (95% confidence interval [CI], 8%- 29%) more prevalent among children with astrovirus than norovirus infections and had longer duration with greater maximal events; there were a median of 4.0 fewer maximal vomiting events (95% CI, 2.0-5.0). Vomiting continued for a median of 24.8 hours longer (95% CI, 9.6-31.7) among children with sapovirus versus norovirus. Differences between these viruses were otherwise minimal. Conclusions: Sapovirus infections attended in the emergency department are more similar to norovirus than previously reported, whereas astrovirus infections have several distinguishable characteristics.
Multiplex PCR Pathogen Detection in Acute Gastroenteritis Among Hospitalized US Children Compared With Healthy Controls During 2011–2016 in the Post–Rotavirus Vaccine Era

Abstract:Abstract Background Despite vaccine-induced decreases in US rotavirus (RV) disease, acute gastroenteritis (AGE) remains relatively common. We evaluated AGE pathogen distribution in hospitalized US children in the post–RV vaccine era. Methods From December 2011 to June 2016, the New Vaccine Surveillance Network (NVSN) conducted prospective, active, population-based surveillance in hospitalized children with AGE. We tested stools from 2 NVSN sites (Kansas City, Houston) with Luminex x-TAG Gastrointestinal Pathogen Panels (Luminex GPP) and analyzed selected signs and symptoms. Results For 660 pediatric AGE inpatients and 624 age-matched healthy controls (HCs), overall organism detection was 51.2% and 20.6%, respectively (P < .001). Among AGE subjects, GPP polymerase chain reaction detected >1 virus in 39% and >1 bacterium in 14% of specimens. Detection frequencies for AGE subjects vs HCs were norovirus (NoV) 18.5% vs 6.6%, RV 16.1% vs 9.8%, adenovirus 7.7% vs 1.4%, Shigella 4.8% vs 1.0%, Salmonella 3.1% vs 0.1%, and Clostridioides difficile in ≥2-year-olds 4.4% vs 2.4%. More co-detections occurred among AGE patients (37/660, 5.6%) than HCs (14/624, 2.2%; P = .0024). Per logistic regression analysis, ill contacts increased risk for NoV, RV, and Shigella (P < .001). More vomiting episodes occurred with NoV and RV, and more diarrheal episodes with Shigella and Salmonella. Modified Vesikari scores were highest for Shigella and lowest for C. difficile. Conclusions NoV detection was most frequent; however, RV remained important in hospitalized AGE in the post–RV vaccine era. Continued active surveillance is important to document ongoing vaccine effects, pathogen emergence, and baseline disease burden for new vaccines.

Abstract:We report a 5 year prospective study of episodes of rotavirus, subgenus F adenovirus, and astrovirus gastroenteritis diagnosed by electron or immune electron microscopy in a single regional virology laboratory. Of 1426 total infections, the numbers in each category were 1117 (78.3%), 254 (17.8%), and 20 (7.9%), respectively. Using restriction endonuclease analysis or immune electron microscopy, all but 20 of the subgenus F adenovirus strains were classified as type 40 (n = 50) or type 41 (n = 184). Rotavirus and astrovirus infections were more prevalent in winter than summer, whereas subgenus F (either type 40 and 41) adenoviruses showed no seasonal variation in prevalence. The ratio of type 40 to all typable subgenus F adenoviruses declined between 1984 and 1986 and then increased again. Adenoviruses were relatively more important as causes of viral gastroenteritis in infants aged less than 6 months than in toddlers aged 12 months or more, but even in young infants more rotavirus than adenovirus infections were diagnosed. Our data confirmed the epidemiological differences between rotavirus, subgenus F adenovirus and rotavirus gastroenteritis and documented the shared epidemiological characteristics of type 40 or 41 adenovirus infections.

Abstract:Objective A molecular epidemiological study on rotavirus(HRV),human calicivirus(HuCV),astrovirus(AstV) and adenovirus(AdV) was conducted in stool specimen from patients with diarrhea under 5 years of age in Nanjing Children Hospital of Nanjing Medical University from July 2009 to June 2010,to provide the basic data and theory evidence for prevention and treatment of viral diarrhea.Methods In Nanjing Children's Hospital of Nanjing Medical University from July 2009 to June 2010,300 stool specimens were collected from outpatients under 5 years of age diagnosed as diarrhea.HRV was detected by ELISA,strain characterization was carried out by nested PCR.HuCV (including norovirus and sapovirus),AstV and AdV were detected by RT-multiplex PCR or PCR.Results In the 300 children whose samples were collected,188 patients were boys,112 were girls.The age of children ranged from 1 to 59 months (mean±SD age:10.5±9.2 months).At least one of the four viral agents was found in 60.0% (180/300) of the specimens.HRV,HuCV,AstV and AdV were identified in 37.7%(113/300),23.7% (71/300),4.0%(12/300),and 2.7%(8/300) in 300 specimens respectively.①HRV G3 (38.9%) was the most prevailing serotype,followed by G2 (8.9%).Based on P typing,P[8] (33.6%) was the most common genotype.The most common G-P combination was G3P[8] (15.9%).The incidence of HRV infections peaked from October 2009 to January 2010,HRV was mostly found in children aged from 7 to 12 months.②Of 71 HuCV positive samples,norovirus was detected in 67 cases and sapovirus was detected in 4 cases.HuCV infection peaked in August 2009,and was mostly found in children aged from 7 to 12 months.95.8%(68/71) children aged under 24 months.③Ten of 12 AstV infections occurred from October 2009 to January 2010,and AstV infected children were aged from 2 to 16 months.④Of 8 AdV positive samples,there were seven serotypes:Ad31(2),Ad2(1),Ad3(1),Ad5(1),Ad7(1),Ad12(1) and Ad41(1).AdV infection peaked from January to June in 2010 and was mostly found in children aged from 7 to 12 months.⑤Twenty-three of 300 samples were found positive to more than one viral agent,in which 12 samples contained both HRV and HuCV,and 5 samples contained both HRV and AstV,1 sample contained both HRV and AdV,2 samples contained both HuCV and AstV,1 sample contained both HuCV and AdV,1 sample contained both AstV and AdV,and 1 sample contained HRV,AstV and HuCV.Conclusions The results indicated clearly the impact of viral agents in causing diarrhea in children and the importance of long-term systematic surveillance.HRV was also the most important pathogen,but HuCV,AstV and AdV should be given more attention.

Abstract:OBJECTIVE:To study molecular epidemiology of four major etiological viruses among children with acute diarrhea in Lanzhou, Gansu province.METHODS:Stool specimens were collected from all 400 inpatients less than 5 years old with acute diarrhea admitted in Department of Pediatrics, the First Hospital, Lanzhou University from Jul. 2004 through Jun. 2005. Dako IDEIA kits were used for detection of rotavirus, adenovirus and astrovirus. Calicivirus detection was done by both reverse transcription-polymerase chain reactions (RT-PCR) and ELISA. Further strain characterization of rotavirus and astrovirus was carried out with RT-PCR.RESULTS:A total of 400 stool samples were collected and tested. Rotavirus was present in 47.3% of the cases: calicivirus in 15.5%, astrovirus in 9.5% and adenoviruses in 7.5%. Mixed infections with multiple enteric viruses were present in 13.5% of all samples. Among 189 rotavirus positive samples, serotype G2 (34.4%) was the predominant strain followed by G3 (32.8%), G1 (1.1%) and mixed-G infection (5.8%). 25.9% of strains remained to be non-typeable. P genotyping showed P [4] (45%) was most common followed by P [8] (22.1%), and non-typeable (32.9%). Strain P [4] G2 (43.6%) was the most common combination followed by P [8] G3 (25.6%), P [4] G3 (13.8%), and P [8] G2, P [4] G1 and P [8] G1. Among 38 astrovirus strains, serotypes 1 (57.8%) was the predominant. Serotype 3 and 8 were found only in one case respectively, 14 strains remained to be non-typeable. A peak admission of rotavirus diarrhea was observed from October through December. More than 95.0% of viral diarrhea patients under hospitalization occurred among children younger than 2 years. The incidence rates of rotavirus were highest in infants aged 6 - 23 months.CONCLUSION:Rotavirus was the most important pathogen for viral diarrhea among children hospitalized in Lanzhou followed by calicivirus, astrovirus, and adenovirus. The predominant rotavirus strain circulated was P [4] G2, which was different from the finding in the previous years. The high rate of mixed infection with different viral agents was notable.
