Abstract:Nanomaterial-based multi-modal synergistic therapies are emerging as highly promising strategies for the treatment of malignant tumors. Herein, a novel nanoplatform modified with the peptide of cyclo(Arg-Gly-Asp-D-Phe-Lys) (cRGD) was designed for synergistic treatment of hypoxic tumors by the combination of chemotherapy (CT)/photodynamic therapy (PDT)/photothermal therapy (PTT). Firstly, Mn@APDA nanoparticles were synthesized for PTT and oxygen (O2) supply, consisting of an L-arginine-doped polydopamine (APDA) core and a manganese dioxide (MnO2) shell. Subsequently, the liposome nanoplatform (cRGD-Lipo-MART), loaded with Mn@APDA (MA), chemotherapy drug of deoxybouvardin dodecyl ester (RA, “R” for short), and mitochondrial targeted photosensitizer of triphenylphosphine-grafted protoporphyrin IX (TPPP, “T” for short), was developed using a film dispersion method. Upon targeted delivery to tumors, the nanoplatform disintegrated in response to the acidic and hydrogen peroxide (H2O2)-rich tumor microenvironment (TME) and near-infrared (NIR) laser irradiation, thereby facilitating controlled drug release and O2 supply to overcome hypoxia. The released RA could cause DNA damage and suppress the HIF-1α expression to sensitize PDT. Simultaneously, TPPP could generate singlet oxygen (1O2) and convert L-arginine (L-Arg) into nitric oxide (NO) for PDT, while APDA could induce hyperthermia for PTT. The combination of PDT and PTT resulted in significant mitochondrial dysfunction and lysosomal damage, ultimately leading to apoptosis. Furthermore, the chemotherapeutic effect of RA could be potentiated by the enhancement of cytoplasmic delivery caused by lysosomal disruption and the blocking of DNA damage repair pathway caused by mitochondrial dysfunction. Overall, cRGD-Lipo-MART, designed with a synergistic CT/PDT/PTT strategy, showed minimal systemic toxicity and could be used as an effective treatment for hypoxic tumors.

Abstract:Photodynamic therapy (PDT) as emerging phototherapy has attracted extensive attention in cancer therapy. However, the overexpressed glutathione (GSH) and hypoxic conditions of the tumor microenvironment (TME) sevely limit the therapeutic efficiency of PDT. Herein, we design a multifunctional nanoplatform (CBDM) for cancer therapy consisting of photosensitizer (Ce6), bovine serum albumin (BSA), and polydopamine-manganese dioxide (MnO 2 ) nanocomplex, which is simultaneously modified by nucleolin-targeting AS1411 aptamer. This MnO 2 -based nanoplatform ameliorates tumor hypoxia and promotes oxygen production via the reaction between MnO 2 and endogenous H 2 O 2 and acid H + in the TME. Besides, this biocompatibility nanoplatform exhibits an augmented PDT effect and high photothermal conversion efficiency under 808 nm laser irradiation. Importantly, the MnO 2 -based nanoplatform achieves a significant in vivo synergistic therapeutic effect and fights against liver metastasis of breast cancer. As a result, the excellent biocompatibility, especially the superior anticancer efficacy, and suppression of tumor metastasis make the MnO 2 -based nanoplatform a promising candidate for cancer therapy.

Abstract:Hypoxia plays a pivotal role in malignant tumor invasion and metastasis, affecting tumor therapeutic effect and tumor immune microenvironment (TME). Previous studies have proved that manganese dioxide (MnO2) nanoparticles could significantly improve the hypoxic tumor microenvironment. Yet the fatal disadvantage of agglomeration upon collection by centrifugation or other treatments limit their uses in hypoxia therapies. In this study, we avoided the agglomeration of MnO2 by a controlled etching method via the unique weak acidity of PEG-PLGA aqueous solution. Then, the photosensitizer chlorin e6 (Ce6) was loaded into the PEG-PLGA core and 4T1 cancer cell membrane fragments acting as a shell (PLGA-Ce6-MnO2@4T1M, ). After intravenous injection into the 4T1-tumor bearing mice, showed homogeneous targeting to 4T1 tumor and greatly enhanced the effect of photodynamic therapy (PDT) in vivo. In addition, also alleviate tumor hypoxia and polarize macrophages in the TME. More interestingly, activated T-cells and maturated dendritic cells in vivo upon laser irradiation. All data showed that can reshape the immunosuppressive TME and be promising in cancer treatment.

Cancer-Responsive Multifunctional Nanoplatform Based on Peptide Self-Assembly for Highly Efficient Combined Cancer Therapy by Alleviating Hypoxia and Improving the Immunosuppressive Microenvironment

Abstract:Hypoxia, as a main feature of the tumor microenvironment, has greatly limited the efficacy of photodynamic therapy (PDT), as well as its clinical application. Here, a multifunctional composite nanoplatform, the peptide/Ce6/MnO2 nanocomposite (RKCM), has been constructed to alleviate tumor hypoxia and increase the efficacy of PDT using rationally designed peptide fibrils to encapsulate chlorin e6 (Ce6) inside and to mineralize MnO2 nanoparticles on the surface. As a result, RKCM significantly improved the PDT efficacy by increasing reactive oxygen species (ROS) generation, decreasing tumor cell viability, and inhibiting tumor growth and metastasis. Besides, decreased HIF-1α expression and increased immune-activated cell infiltration were also observed in RKCM/laser treatment xenograft. Mechanically, (1) Ce6 can induce singlet oxygen (1O2) generation under laser irradiation to give photodynamic therapy (PDT); (2) MnO2 can react with H2O2 in situ to supply additional O2 to alleviate tumor hypoxia; and (3) the released Mn2+ ions can induce a Fenton-like reaction to generate •OH for chemical dynamic therapy (CDT). Moreover, RKCM/laser treatment also presented with an abscopal effect to block the occurrence of lung metastasis by remolding the pre-metastasis immune microenvironment. With these several aspects working together, the peptide/Ce6/MnO2 nanoplatform can achieve highly efficient tumor therapy. Such a strategy based on peptide self-assembly provides a promising way to rationally design a cancer-responsive multifunctional nanoplatform for highly efficient combined cancer therapy by alleviating hypoxia and improving the immune microenvironment.

Abstract:The trade-off in nanomaterials-based cancer therapy between safety and therapeutic efficacy remains a significant challenge to overcome in the long run. In this context, we have designed a tumor microenvironment (pH, H 2 O 2 )-responsive multifunctional nanoplatform with a core–shell structure (C/B@M) for magnetic resonance imaging (MRI)-guided photothermal (PTT) /photodynamic (PDT) /nitric oxide (NO) combined cancer therapy. The pH/H 2 O 2 -responsive MnO 2 coating not only shields from light, preventing the premature release of singlet oxygen, which is non-specifically toxic, but also generates O 2 to alleviate tumor hypoxia, enhancing PDT efficacy. The degradation of MnO 2 to Mn 2+ aids in precise tumor localization for MRI-guided irradiation, thereby reducing the risk of inadvertent damage to normal cells. Once the shell is degraded within the tumor, the covalent organic framework (COF) core, crafted from porphyrin, produces singlet oxygen and thermal effects upon exposure to infrared light irradiation (660 nm). Concurrently, BNN6 decomposes under photothermal stimulation, releasing NO for gas therapy, which further ameliorates the tumor's hypoxic microenvironment, enabling MRI imaging-guided multimodal combined tumor treatments. The introduced C/B@M, with its selective toxicity, offers a novel approach to designing multifunctional nanoplatforms, addressing the intrinsic balance between safety and tumor-killing efficacy.

Tumor microenvironment activated mussel-inspired hollow mesoporous nanotheranostic for enhanced synergistic photodynamic/chemodynamic therapy

Abstract:Anti-tumor therapies reliant on reactive oxygen species (ROS) as primary therapeutic agents face challenges due to a limited oxygen substrate. Photodynamic therapy (PDT) is particularly hindered by inherent hypoxia, while chemodynamic therapy (CDT) encounters obstacles from insufficient endogenous hydrogen peroxide (H2O2) levels. In this study, we engineered biodegradable tumor microenvironment (TME)-activated hollow mesoporous MnO2-based nanotheranostic agents, designated as HAMnO2A. This construct entails loading artemisinin (ART) into the cavity and surface modification with a mussel-inspired polymer ligand, namely hyaluronic acid-linked poly(ethylene glycol)-diethylenetriamine-conjugated (3,4-dihydroxyphenyl) acetic acid, and the photosensitizer Chlorin e6 (mPEG-HA-Dien-(Dhpa/Ce6)), facilitating dual-modal imaging-guided PDT/CDT synergistic therapy. In vitro experimentation revealed that HAMnO2A exhibited ideal physiological stability and enhanced cellular uptake capability via CD44-mediated endocytosis. Additionally, it was demonstrated that accelerated endo-lysosomal escape through the pH-dependent protonation of Dien. Within the acidic and highly glutathione (GSH)-rich TME, the active component of HAMnO2A, MnO2, underwent decomposition, liberating oxygen and releasing both Mn2+ and ART. This process alleviates hypoxia within the tumor region and initiates a Fenton-like reaction through the combination of ART and Mn2+, thereby enhancing the effectiveness of PDT and CDT by generating increased singlet oxygen (1O2) and hydroxyl radicals (•OH). Moreover, the presence of Mn2+ ions enabled the activation of T1-weighted magnetic resonance imaging. In vivo findings further validated that HAMnO2A displayed meaningful tumor-targeting capabilities, prolonged circulation time in the bloodstream, and outstanding efficacy in restraining tumor growth while inducing minimal damage to normal tissues. Hence, this nanoplatform serves as an efficient all-in-one solution by facilitating the integration of multiple functions, ultimately enhancing the effectiveness of tumor theranostics.

Abstract:The integration of reactive oxygen species (ROS)-involved molecular dynamic therapy (MDT) and photodynamic therapy (PDT) holds great promise for enhanced anticancer effects. Herein, we report a biodegradable tumor microenvironment-responsive nanoplatform composed of sinoporphyrin sodium (SPS) photosensitizer-loaded zinc peroxide nanoparticles (SPS@ZnO2 NPs), which can enhance the action of ROS through the production of hydrogen peroxide (H2O2) and singlet oxygen (1O2) for MDT and PDT, respectively, and the depletion of glutathione (GSH). Under these conditions, SPS@ZnO2 NPs show excellent MDT/PDT synergistic therapeutic effects. We demonstrate that the SPS@ZnO2 NPs quickly degrade to H2O2 and endogenous Zn2+ in an acidic tumor environment and produce toxic 1O2 with 630 nm laser irradiation both in vitro and in vivo. Anticancer mechanistic studies show that excessive production of ROS damages lysosomes and mitochondria and induces cellular apoptosis. We show that SPS@ZnO2 NPs increase the uptake and penetration depth of photosensitizers in cells. In addition, the fluorescence of SPS is a powerful diagnostic tool for the treatment of tumors. The depletion of intracellular GSH through H2O2 production and the release of cathepsin B enhance the effectiveness of PDT. This theranostic nanoplatform provides a new avenue for tumor microenvironment-responsive and ROS-involved therapeutic strategies with synergistic enhancement of antitumor activity.

Abstract:Photodynamic therapy (PDT) has attracted extensive attention in the clinical treatment of malignant tumor. However, the acidic and hypoxic conditions of the tumor microenvironment (TME) limit the further application of PDT in the clinic. Herein, we fabricate a new nanoplatform─HPDA@MnO2@Ce6/DOX@PEG-RGD (HPMRCD)─by means of coating hollow polydopamine nanoparticles (HPDA) with manganese dioxide (MnO2), which is modified by cyclic RGD functionalized poly(ethylene glycol) (PEG) and further co-loaded with a photosensitizer, Chlorin e6 (Ce6), and a chemotherapy drug, doxorubicin (DOX). This nanoplatform could be enriched in tumor tissues, then instantly dissociated under an acidic and H2O2-rich TME. The dual-responsive release of Mn2+ ions and oxygen (O2) can relieve tumor hypoxia, which can be used as a magnetic resonance contrast agent and the latter can enhance the PDT effect. Furthermore, the degradation of HPMRCD leads to an efficient loaded therapeutic molecule release, thus yielding a potential therapy to enhance tumor suppression by adopting the combined chemo-photodynamic therapy.

Abstract:Chemodynamic therapy (CDT), designed to trigger a tumor-specific hydrogen peroxide (H 2 O 2 ) reaction generating highly toxic hydroxyl radicals (·OH), has been investigated for cancer treatment. Unfortunately, the limited Fenton or Fenton-like reaction rate and the significant impact of excessive reducing glutathione (GSH) in the tumor microenvironment (TME) have severely compromised the effectiveness of CDT. To address this issue, we designed a dual-responsive nanoplatform utilizing a metal-polyphenol network (MPN) -coated multi-caged IrO x for efficient anti-tumor therapy in response to the acidic TME and intracellular excess of GSH, in which MPN composed of Fe 3+ and tannic acid (TA). Initially, the acidic TME and intracellular excess of GSH lead to the degradation of the MPN shell, resulting in the release of Fe 3+ and exposure of the IrO x core, facilitating the efficient dual-pathway CDT. Subsequently, the nanoplatform can mitigate the attenuation of CDT by consuming the excessive GSH within the tumor. Finally, the multi-caged structure of IrO x is advantageous for effectively implementing photothermal therapy (PTT) in coordination with CDT, further enhancing the therapeutic efficacy of tumors. Moreover, the outstanding Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) (T 1 /T 2 ) multimodal imaging capabilities of IrO x @MPN enable early diagnosis and timely treatment. This work provides a typical example of the construction of a novel multifunctional platform for dual-responsive treatment of tumors.

Abstract:STUDY DESIGN Stresses in vertebral endplates and discs were calculated using the three-dimensional nonlinear finite-element model of a lumbar spine with an internal spinal fixation device. OBJECTIVE To determine the influence of fixator stiffness on stresses in the adjacent discs. SUMMARY OF BACKGROUND DATA There are few computer models of the lumbar spine with a fixator. Most of these models neglect the muscle forces. Fixator stiffness is assumed to influence greatly the stresses in the adjacent discs. METHODS Two three-dimensional nonlinear finite-element models were used to determine stresses in the lumbar spine for standing and 60 degrees flexion of the upper body. One model had an internal spinal fixator, the other did not. In a parameter study, the diameters of the longitudinal rod of the fixator were assumed to be 3, 5, 7, and 10 mm. In the computer model, the forces of the trunk muscles were simulated. RESULTS The diameter of the longitudinal rod strongly affected the fixator loads but hardly influenced the stresses in the vertebral endplates. The stresses in the bridged discs were strongly reduced. However, the internal fixator had only a minor influence on the stresses in the anulus fibrosus and the pressure in the nucleus pulposus of the adjacent discs. CONCLUSIONS The stiffness of an internal spinal fixation device has only a minor influence on stresses in the adjacent discs.

Abstract:Photodynamic therapy (PDT) is an effective noninvasive therapeutic strategy that can convert oxygen to highly cytotoxic singlet oxygen (O-1(2)) through the co-localization of excitation light and photosen-sitizers. However, compromised by the hypoxic tumor microenvironment, the therapeutic efficacy of PDT is reduced seriously. Herein, to overcome tumor-associated hypoxia, and further achieve tumor-targeted synergistic chemotherapy/PDT/photothermal therapy (PTT), we have constructed a biodegrad-able oxygen-producing nanoplatform (named Ini@PM-HP), which was composed of the porous metal-organic framework (PCN-224(Mn)), the poly (ADP-ribose) polymerase (PARP) inhibitor (Iniparib), and the polydopamine-modified hyaluronic acid (HA-PDA). Since HA can specifically bind to the overexpressed HA receptors (cluster determinant 44, CD44) on tumor cell, Ini@PM-HP prefers to accumulate at the tumor site once injected intravenously. Then iniparib can be released in tumor environment (TME), thereby dysfunctioning DNA damage repair and promoting cell apoptosis. At the same time, the chelating of Mn and tetrakis(4-carboxyphenyl) porphyrin (Mn-TCPP) can generate O-2 in situ by reacting with endogenous H2O2, relieving the hypoxic TME and achieving enhanced PDT. Moreover, owing to the high photother-mal conversion efficiency of PDA, PTT can be driven by the 808 nm laser irradiation. As systematically demonstrated in vitro and in vivo, this nanotherapeutic approach enables the combined therapy with great inhibition on tumor. Overall, the as-prepared nanoplatform provide a promising strategy to over-come tumor-associated hypoxia, and shows great potential for combination tumor therapy. Statement of significance A delicately designed biodegradable oxygen-producing nanoplatform Ini@PM-HP is constructed to achieve combination therapy of solid tumors. Taking advantage of the active-targeting, PTT, enhanced PDT and PARPi, this nanotherapeutic approach successfully enables the combined chemo/photothermal/photodynamic therapy with great inhibition of solid tumors. Abstract:Cytopathology is a subspecialty of pathology in which pathologists frequently interact directly with patients. Often this interaction is in the context of fine needle aspiration (FNA) procedures performed at the bedside by the cytopathologist or by another clinician with the cytopathologist present. Patient requests for preliminary results in such settings raise fundamental questions about professional scope of practice and communication of uncertainty that apply not merely to pathologists but to all clinicians. In certain settings, cytopathologists may share preliminary diagnostic impressions directly with patients. Essential to these conversations is the need to articulate potential uncertainty about both the diagnosis and next steps. In addition, the involvement and notification of the referring physician is obligatory, both for care coordination and to ensure that patients receive a consistent message.
Tumor acidity activating multifunctional nanoplatform for NIR-mediated multiple enhanced photodynamic and photothermal tumor therapy

Abstract:The study reports a multifunctional nanoplatform based on mesoporous silica coated gold nanorod (AuNR@MSN) to overcome biological barriers associating with nanocarrier for multiple enhanced photodynamic therapy (PDT) and photothermal therapy (PPT). Indocyanine green (ICG) was loaded into AuNR@MSN and end-capped with β-cyclodextrin (β-CD). Then, a peptide RLA ([RLARLAR]2) with plasma membrane permeability and mitochondria-targeting capacity was anchored to AuNR@MSN via host-gust interaction. Subsequently, a charge-reversible polymer was introduced to endow stealth property. When the nanoplatform extravasates to tumor tissue, the weak acidity in tumor microenvironment could induce the dissociation of charge-reversible polymer and re-exposure of RLA peptide. Such a pH-mediated transition could facilitate the targeted accumulation of the nanoplatform in mitochondria. Upon singular 808 nm laser irradiation, the nanoplatform displayed enhanced PDT effect through the generation of reactive oxygen species (ROS) mediated by the local electric field of AuNR, plasmonic photothermal effect, and leakage of endogenous ROS by mitochondrion-targeted PDT. Meanwhile, local hyperthermia was generated by both ICG and AuNR for PPT. The in vitro and in vivo experiments demonstrated that the composite nanoplatform had good antitumor effect with minimal side effect. This work provides new insight into the development of new phototherapeutics for oncotherapy.

Abstract:Phototherapy, including photothermal therapy (PTT) and photodynamic therapy (PDT), has been considered as a noninvasive option for cancer therapy. However, insufficient penetration depth, tumor hypoxia, and a single treatment method severely limit the effectiveness of treatment. Methods: In this study, a multifunctional theranostic nanoplatform has been fabricated based on Au/Ag-MnO2 hollow nanospheres (AAM HNSs). The Au/Ag alloy HNSs were first synthesized by galvanic replacement reaction and then the MnO2 nanoparticles were deposited on the Au/Ag alloy HNSs by the reaction between Ag and permanganate (KMnO4), finally obtained the AAM HNSs. Then, SH-PEG was modified on the surface of AAM HNSs by the interaction of sulfhydryl and Au/Ag alloy, which improved the dispersibility and biocompatibility of the HNS. Next, the PDT photosensitizer Ce6 was loaded into AAM HNSs, benefiting from the hollow interior of the structure, and the AAM-Ce6 HNSs were obtained. Results: The AAM HNSs exhibit broad absorption at the near infrared (NIR) biological window and remarkable photothermal conversion ability in the NIR-II window. The MnO2 nanoparticles can catalyze endogenous H2O2 to generate O2 and enhance the therapeutic effect of PDT on tumor tissue. Simultaneously, MnO2 nanoparticles intelligently respond to the tumor microenvironment and degrade to release massive Mn2+ ions, which introduce magnetic resonance imaging (MRI) properties. When AAM-Ce6 HNSs are loaded with Ce6, the AAM-Ce6 HNSs can be used for triple-modal imaging (fluorescence/photoacoustic/magnetic resonance imaging, FL/PAI/MRI) guided combination tumor phototherapy (PTT/PDT). Conclusion: This multifunctional nanoplatform shows synergistic therapeutic efficacy better than any single therapy by achieving multimodal imaging guided cancer combination phototherapy, which are promising for the diagnosis and treatment of cancer.
A HMCuS@MnO2 nanocomplex responsive to multiple tumor environmental clues for photoacoustic/fluorescence/magnetic resonance trimodal imaging-guided and enhanced photothermal/photodynamic therapy

Abstract:Hollow mesoporous copper sulfide nanoparticles (HMCuS NPs) are advantageous for loading small-molecule therapeutic drugs coupled with photothermal ablation for synergistic tumor therapy. However, treatment efficacy mediated by HMCuS NPs is not always satisfactory owing to their insensitivity toward the tumor microenvironment (TME), and unpredictable drug leakage may also result in deleterious systemic toxicity. Here, a novel HMCuS@MnO2-based core-shell nanoplatform was developed as a highly efficient TME modulator, which could alleviate tumor hypoxia, deplete the level of intracellular glutathione (GSH) and trigger the dissolution of Mn2+. Moreover, MnO2, in situ grown on the surface of HMCuS, may act as a gatekeeper by forming a stimulus-responsive plug within the mesoporous structure, which effectively prevented the premature release of encapsulated photosensitizer chlorin e6 (Ce6) and was responsive to the acidic TME for demand-based drug release. Under the condition of 660/808 nm dual-wavelength laser irradiation, hyperthermia-mediated photothermal therapy (PTT) and reactive oxygen species (ROS)-mediated photodynamic therapy (PDT) can be triggered for tumor eradication, which were further enhanced upon the modification of the TME. In the meantime, splendid photoacoustic (PA)/fluorescence (FL)/magnetic resonance (MR) imaging properties of HMCuS@MnO2/Ce6 (CMC) NPs could enable the realization of more precise, reliable and on-demand combination therapy. In a word, this study illustrated a promising approach to strengthen the efficacy of HMCuS-based nanotherapeutics, which would definitely promote the further exploitation of smarter nanoplatforms for synergistic disease management.

Abstract:Manganese has recently been exploited for cancer immunotherapy, fenton-like reaction-mediated chemo-dynamic therapy, and magnetic resonance imaging. The integration of multiple roles of manganese into one platform is of great significance for cancer theranostics and tumor inhibition. Here, we designed a multifunctional nanoplatform based on manganese, which consisted of a manganese-containing inner core and a phospholipid bilayer shell co-loaded with glucose oxidase (GOx), paclitaxel (PTX), and a NIR fluorescent dye (NanoMn-GOx-PTX). In a pH-dependent manner, the nanoplatform released manganese ions and payloads inside the tumor cells. In vitro characterization and cellular experiments indicated that NanoMn-GOx-PTX could catalyze the conversion of glucose into reactive oxygen species (ROS) through a cascade Fenton-like reaction as well as release free PTX. The consumption of glucose, ROS production, and the chemotherapeutic effect of PTX contributed to the superior cytotoxicity and apoptosis of 4T1 cancer cells. Moreover, NanoMn-GOx-PTX effectively induced the production of large amounts of type I interferon and pro-inflammatory cytokines in vivo, activating the innate immune response. Through the synergistic functions of the above components, NanoMn-GOx-PTX exerted the strongest anti-tumor effect in 4T1 tumor-bearing models. Therefore, the manganese-based nanoplatform could serve as a promising theranostic tool for breast cancer therapy. STATEMENT OF SIGNIFICANCE: 1) This nanoplatform can be used as a universal tool for delivering proteins and anticancer drugs into cells; 2) The PEG-modified phospholipid bilayer shell plays a significant role in retarding the release of overloaded manganese ions and drugs in a pH-sensitive manner; 3) The released Mn2+ has the ability to enhance T1 contrast in magnetic resonance imaging; 4) The released Mn2+ can function as nanoadjuvants to activate the cGAS-STING pathway and effectively induce the natural immune response;5) The overloaded manganese ions are combined with glucose oxidase to form a cascade reaction system, indirectly converting glucose into ROS to induce oxidative damage of tumor tissue.

Abstract:The development of smart theranostic nanoplatforms has gained great interest in effective cancer treatment in facing the complex tumor microenvironment (TME), including weak acidity, hypoxia, and glutathione (GSH) overexpression. Herein, a TME‐responsive nanoplatform named PMICApt/ICG, based on PB:Mn&Ir@CaCO3Aptamer/ICG, was designed for the competent synergistic photothermal therapy (PTT) and photodynamic therapy (PDT) under the guidance of photothermal (PT) and magnetic resonance (MR) imaging. The nanoplatform's aptamer modification targeting the transferrin receptor (TfR) and the epithelial cell adhesion molecule (EpCAM) on breast cancer cells, and the acid degradable CaCO3 shell allowed for effective tumor accumulation and TME‐responsive payload release in situ. The nanoplatform also exhibited excellent PDT properties due to its ability to generate O2 and consume antioxidant GSH in tumors. Additionally, the synergistic therapy was achieved by a single wavelength of near‐infrared (NIR) laser. RNA sequencing was performed to identify differentially expressed genes (DEGs), which showed that the expressions of proliferation and migration‐associated genes were inhibited, while the apoptosis and immune response gene expressions were upregulated after the synergistic treatments. This multifunctional nanoplatform that responds to the TME to realize the on‐demand payload release and enhancs PDT induced by TME modulation holds great promise for clinical applications in tumor therapy. Supramolecular Nanoplatform for Imaging-Guided Phototherapies via Hypoxia Tumour Microenvironment Remodeling

Abstract:Phototherapies including photodynamic therapy (PDT) and photothermal therapy (PTT), have emerged as an invasive and promising antitumour treatment. However, the hypoxia in deep tumour tissues and the poor water-solubility of photosensitizers as bottlenecks greatly hinder efficiency of phototherapies. Herein, a tumour microenvironment (TME) activated supramolecular nanoplatform consisting of the pillar[5]arene-based amphiphilic polymer POPD, the phototherapeutic agent Cy7-CN, respiratory medication atovaquone (ATO) and chemotherapeutic drug pyridinyl camptothecin (CPT-Py), was constructed for imaging-guided hypoxia-ameliorated phototherapies. Owing to host-guest interaction, the photochemical and photophysical properties of cyanine were improved exceedingly due to the suppression of π-π stacking. Triggered by the acidic microenvironment in tumour sites, the supramolecular nanoplatform would dissociate and release CPT-Py and ATO which inhibits mitochondria-associated oxidative phosphorylation (OXPHOS) and encourages more oxygen to be used in enhanced phototherapies. In vitro and in vivo studies verified that the rational combination of ATO-enhanced PDT and PTT complemented the disadvantages of single phototherapy and formed mutual promotion, and simutaneously sensitized the chemotherapeutic drugs, which resulted to the high tumour inhibition. It is hoped that the supramolecular nanoplatform could shed light on the development of phototherapeutic agents.

Abstract:Photodynamic therapy (PDT) induces aggravation of tumor hypoxia, which leads to worsening tumor invasiveness and metastasis. Combining hypoxia-responsive prodrugs with photosensitizers could transform "enemy" (tumor hypoxia) into "friend" (positive amplifier) to facilitate efficient activation of prodrugs. However, it remains challenging to develop a highly effective co-delivery nanoplatform due to the poor compatibility between vehicles and cargoes. Herein, we selected podophyllotoxin (PPT) as a model drug to synthesize hypoxia-responsive polyphenol prodrug (PAG). By introducing a chemical modification strategy, PAG and Chlorin e6 (Ce6) could be remotely co-encapsulated into liposomes (PAG/Ce6 LPs) at a synergistic ratio utilizing metal ions gradient. In vitro experiments demonstrated that PAG/Ce6 LPs could kill tumor cells through multiple pathways; i) generating reactive oxygen species (ROS) for direct cell killing; ii) consuming intracellular glutathione (GSH) to disturb intracellular redox homeostasis to induce tumor cell apoptosis. iii) aggravating hypoxia micro-environment to promote PPT release for chemotherapy. Furthermore, the excellent stability of PAG/Ce6 LPs guaranteed extended drug blood circulation time, enhanced tumor accumulation and improved anticancer efficacy. Additionally, PAG/Ce6 LPs kept fluorescence quench in circulation and specifically recovered at the tumor site, avoiding the obstacle of phototoxicity. Such a co-delivery nanoplatform, integrating hypoxia-responsive polyphenol prodrugs and photosensitizer into one nanocarrier, provides new insight into solving the challenges of chemo-photodynamic therapy.
