Abstract:BACKGROUND Pulmonary stretch reflex plays an important role in regulation of respiratory movement. This study aimed to evaluate the effect of pulmonary stretch reflex on lung injury in rabbits with acute respiratory distress syndrome (ARDS). METHODS ARDS rabbits were given intratracheal infusion of hydrochloric acid and ventilated with neurally adjusted ventilatory assistance (NAVA) with a tidal volume (VT) of 6 mL/kg and the electrical activity of diaphragm (EAdi)-determined positive end expiratory pressure. After isolation of the bilateral vagus nerve trunk, the rabbits were randomized into two groups: sham operation (SHAM) group (n=5) and bilateral vagotomy (VAG) group (n=5). Gas exchange and respiratory mechanics were detected at baseline, after lung injury and 1, 2, and 3 hours after ventilation respectively. Pulmonary permeability index, pathological changes and inflammatory response were also measured. RESULTS Compared with the SHAM group, PaO2/FiO2 in the VAG group decreased significantly 2 and 3 hours after ventilation (P<0.05). There was no significant difference in PaCO2 between the SHAM and VAG groups (P>0.05), and the VAG group had a high VT, peak pressure (Ppeak), and mean pressure (Pm) compared with the SHAM group 1, 2, 3 hours after ventilation (P<0.05). Compared to the SHAM group, dead space fraction (VD/VT) and respiratory system elastance (Ers) in the VAG group increased (P<0.05) and static pulmonary compliance (Cst) decreased markedly (P<0.05) after ventilation for 3 hours. Lung wet/dry weight ratio (W/D) (8.4±1.2 vs. 6.6±1.0), lung injury score (6.3±1.8 vs. 3.8±1.3), tumor necrosis factor-α (TNF-α) (779±372 pg/mL vs. 355±130 pg/mL) and interleukin-8 (IL-8) (169±21 pg/mL vs. 118±17 pg/mL) increased significantly in the VAG group compared with the SHAM group (P<0.05). CONCLUSION Lung injury is aggravated after bilateral vagotomy, demonstrating that pulmonary stretch reflex may have protective effect on the lung.
The Effect of Final Irrigation Agitation Techniques on Postoperative Pain after Single Visit Root Canal Treatment of Symptomatic Irreversible Pulpitis: A Randomised Clinical Trial

Abstract:Objective: To evaluate the degree of postoperative pain and rate of analgesic intake in patients with symptomatic irreversible pulpitis in mandibular first molar teeth at 6 h, 12 h, 24 h, 48 h, and 72 h after using different irrigation activation techniques in single-visit endodontic treatment. Methods: A total of 78 patients with symptomatic irreversible pulpitis with no signs of periapical pathology were randomly divided into 3 groups according to the final irrigation activation technique; Group XP-endo Finisher, Group Ultra X ultrasonic device, and Group side-vented needle. The teeth underwent standardised single-visit root canal treatment procedures using 2.5% sodium hypochlorite for irrigation. Each patient was given a chart to record postoperative pain at 6, 12, 24, 48 and 72 hours intervals. Ibuprofen, 400 mg tablets, was prescribed to be taken when the pain was unbearable. The incidence and number of analgesic tablets taken were recorded. Data were analysed using a Kruskal-Wallis test followed by a pairwise Mann-Whitney U test with Bonferroni correction for intergroup comparisons and Freidman's test followed by Dunn's post hoc test for intragroup comparisons. Results: No statistically significant difference was found between all groups regarding the incidence and intensity of pain at different time intervals (p>0.05). There was no significant difference in analgesic intake between different groups, with most cases in all groups not taking analgesics (p>0.05). Conclusion: Adding XP-endo Finisher or passive ultrasonic irrigation to the final irrigation protocol in singlevisit endodontic treatment had no significant effect on postoperative pain or analgesic intake. Antibacterial Effect of Silver Nanoparticle Gel as an Intracanal Medicament in Combination with Other Medicaments against Enterococcus faecalis: An In vitro Study

Abstract:Background: Bacteria and their by-products are responsible for various pulpal and periapical infections which can be classified as biofilm-mediated infections. Recently, nanoparticles have been introduced to decrease the bacterial load in endodontic infections. Aim and Objectives: The aim of the study was to compare and evaluate the antibacterial effect of silver nanoparticles alone and combination of silver nanoparticles with calcium hydroxide and chlorhexidine against Enterococcus faecalis. Materials and Methods: A pure culture of E. faecalis was used as the test microorganism. After 24 h of incubation the bacterial colonies were isolated and suspended in 5 ml of infusion broth followed by incubation at 37°C for 4 h. 0.5 McFarland of the bacterial suspension was prepared and then cultured on Mueller–Hinton agar culture medium with the help of a sterile swab. In each culture plate, five wells were created with a sterile pipette for placement of the samples. Results: Statistically significant difference in the antibacterial effect of the combination of silver nanocure gel with various medicaments was observed when compared to the effect of silver nanocure gel alone. Silver nanocure gel in combination with calcium hydroxide (Group B) was more efficacious as compared to silver nanocure gel alone (Group A) (P < 0.001). Antibacterial activity of silver nanocure gel in combination with chlorhexidine (Group C) was also significantly higher than silver nanocure gel alone (P < 0.001). Conclusion: The present study is an in vitro study, in which we concluded that the combination of all the intracanal medicaments is the best among for elimination of E. faecalis biofilm from the root canal. The above findings need to be tested in vivo also.

Abstract:Aims: This clinical study aimed to evaluate the effect of different clinical practices with endodontic emergency treatment on postoperative pain in mandibular permanent molar teeth with symptomatic apical periodontitis. Methods: In the present study, root canal treatment was applied to 100 patients who met the inclusion criteria. Patients were randomly assigned to 5 groups (control, nonsteroidal anti-inflammatory drug [NSAID], NSAID and antihistaminic drug combination, placebo drug, and intracanal cryotherapy) using a web program. In the first visit, after preparation with the WaveOne Gold file system, calcium hydroxide (Ca(OH)2) medicament was placed on the root canals. However, after the treatment of the patients was completed, six patients patients were excluded from the study because they did not come to the control appointment 7 days later. After the removal of Ca(OH)2 medicament in the second session, the root canals were obturated according to cold lateral condensation technique and final restorations of the teeth were performed. Patients' preoperative and postoperative pain values (4-8-12-24-48 and 72 hours) determined by visual analog scale (VAS) were recorded. The data were analyzed statistically using analysis of variance, Kruskal-Wallis test, Friedman test, Wilcoxon test, and Chi-square test. Results: According to the results of our study; there was no significant difference between the groups in terms of age, gender, preoperative pain, and postoperative analgesic requirement (P > 0.05). In all groups, there were significant differences between the values of different times (P < 0.05). While initial pain values were higher than the hours after treatment (P < 0.05), there was no significant difference between postoperative pain values (P > 0.05). When the groups in terms of pain scales measured at different times compared with each other; in all times there was no significant difference between the groups (P > 0.05). Conclusion: It was observed that all clinical practices significantly reduced preoperative pain.

Abstract:Aim: The aim of this study is to evaluate the effect of different intracanal medicaments used during endodontic treatment on periodontal attachment level in concurrent endodontic-periodontal lesions without communication. Materials and methods: Seventy-five teeth with combined endodontic-periodontal lesions were randomly assigned to 3 groups: Test Group I (calcium hydroxide [Ca(OH)2]), Test Group II (2% chlorhexidine + Ca(OH)2), and control group (no intracanal medicament). Root canal treatment (RCT) was performed using standard methodology. Teeth of test groups were medicated with respective medicaments for 10 days before obturation. Scaling and root planning was done 1 month after completion of RCT in all the groups. Clinical measurements, including probing depth (PD), clinical attachment level (CAL), and bleeding on probing (BOP) were recorded at baseline, 1, 3, and 6 months. Radiographic parameters, including periapical index (PAI) score and alveolar bone level, were measured at baseline and 6 months. Statistical analysis: Data were analyzed using Friedman test, Kruskal-Wallis, and Wilcoxon signed-rank test. Results: Significant improvement in PD, CAL, %BOP, and PAI score (P < 0.05) was found in all the three groups from baseline to 6 months. However, there was no significant intergroup difference. No significant difference was observed from baseline to 6 months in marginal bone level within the three groups. Conclusion: Endodontic treatment before periodontal therapy results in attachment gain. Intracanal medicaments may not affect the healing of concurrent endodontic-periodontal lesions without communication.

Abstract:Aim: Evaluate the degree of postoperative pain in patients with necrotic teeth with symptomatic apical periodontitis in mandibular first molar teeth at 6, 12, 24, 48 and 72 hrs time interval after using different irrigation activation techniques in single-visit endodontic treatment. Material and Methods: Seventy-eight patients diagnosed with necrotic mandibular first molar with symptomatic apical periodontitis were randomly allocated into 1 of 3 separate groups (n=26); Navitip group (control), EndoUltra group, or Eddy group. After a single visit root canal treatment and a specific method of agitation, depending on each group, the patients were given a questionnaire on which the patient would mark the degree of pain in a scale from 0 to 10 at 6, 12, 24, 48, 72 hours and 7 days post-obturation. Data were statistically analyzed with a significance level of P≤0.05. Results: EndoUltra ultrasonic agitation and Eddy sonic agitation as a final irrigation protocol showed significantly lower pain values than the control group, but there was no significant difference in pain values between the experimental groups. Conclusion: There was significantly less pain associated with EndoUltra ultrasonic and Eddy sonic agitation compared to Navitip irrigation

Abstract:Background: Many challenges encounter the endodontist, especially when dealing with necrotic teeth with chronic periapical lesions. Postoperative pain may be induced following conventional endodontic therapy and total eradication of the bacteria is almost unachievable even with recently available techniques. In recent years, diode laser usage in the endodontic field has gained acceptance. Thus, this study aimed to investigate the ability of the diode laser (DL) to decrease postoperative pain and achieve root canal sterility. Methods: 56 patients with anterior teeth with chronic periapical lesions in upper anterior teeth were randomly divided into two groups (n = 28). All patients were treated with two visits of conventional root canal treatment with ProTaper Universal. The DL group: root canals were irradiated with 200 μm fiber optic at both visits; the control group (Endo): the DL fiber was placed in root canal with no activation. Bacterial samples were collected from all the cases at each step of the treatment. Pain levels were evaluated using a numerical rating scale preoperatively, and after 6, 12, 24, 48 hours and 7 days. Bacterial count was used to detect both aerobic and anaerobic bacterial load. Results: The qualitative pain scores revealed statistically significant lower pain levels in the DL group compared with the Endo group at all time intervals (P<0.001), except preoperatively where there was no significant difference. There was a statistically significant lower bacterial count for both aerobic and anaerobic bacteria in the DL group compared with the Endo group in both S3 samples (after laser application) and S4 samples (bacterial colonization) (P<0.001). Conclusion : The 980 nm diode laser may be a successful adjunct to conventional endodontic treatment of necrotic cases with chronic periapical lesions in terms of postoperative pain and root canal disinfection. Abstract:This study of the fine structure of the neuroglia in the spinal cord of the newt, Triturus viridescens, has permitted the identification of three types of glial elements. One type is the ependymal cell whose processes are rich in filaments and glycogen and end on the pia in the deeper portion of the fissures of the cord. The two other types of glial cells display some morphological similarities to typical mammalian neuroglia although they appear to be more primitive. These are the astrocytes and oligodendrocytes. The astrocytes contain glial filaments, glycogen, specialized mitochondria, few ribosomes, and probably send end feet to the surface of the cord and to the intramedullary vessels. The oligodendrocytes present some variation in morphology but always exhibit an abundance of ribosomes. They are located either near the neurons, the intramedullary vessels, the pia, or the myelin sheath.
Intracanal antibacterial effects of a bioceramic medication compared with calcium hydroxide pastes in different vehicles

Abstract:Introduction The intracanal antibacterial effectiveness of a bioceramic medication was compared with calcium hydroxide pastes in different vehicles. Methods Extracted mandibular incisors with a single long oval canal were selected and distributed into 5 groups based on anatomically paired micro-computed tomographic analyses. The root canals were prepared up to an instrument size 35/04 and contaminated for 30 days with a mixed bacterial culture from subgingival biofilm added with Enterococcus faecalis. The canals were medicated with Bio-C Temp (Angelus Indústria de Produtos Odontológicos, Londrina, PR, Brazil) or a calcium hydroxide paste in glycerin (CHG), camphorated paramonochlorophenol/ glycerin (CHPG), or 2% chlorhexidine (CHCX). Control specimens were filled with sterile saline. After 7 days, the medication was removed by using rotary instruments and saline irrigation. A supplementary approach was conducted, which consisted of 2.5% NaOCl irrigation and agitation with XP-endo Finisher. Bacteriological samples were taken before application (S1) and after removal (S2) of the intracanal medication, and after the supplementary approach (S3). Bacterial DNA extracted from the samples was quantified using real-time polymerase chain reaction. Results Bacterial counts were significantly reduced from S1 to S2 in all groups, except for the control group. As for S1-to-S3 changes, all groups (control included) showed significant bacterial reduction. S2-to-S3 changes were only significant in the CHPG and CHCX groups (p 0.05), with all medications significantly better than the control (p 0.05). Conclusion All tested medications significantly reduced the intracanal bacterial counts, with no significant differences between them. Final NaOCl irrigation and agitation using XP-endo Finisher further enhanced disinfection in the CHCX and CHPG groups.

Antibacterial and Antibiofilm Efficacy of Antimicrobial Photodynamic Therapy Against Intracanal Enterococcus faecalis: An In Vitro Comparative Study with Traditional Endodontic Irrigation Solutions

Abstract:Objectives: Enterococcus faecalis (E. faecalis), an infecting microorganism of the root canals, is difficult to eliminate during endodontic therapy. In this study, the effect of root canal disinfection with sodium hypochlorite (NaOCl) and chlorhexidine (CHX) was evaluated on planktonic and biofilm forms of E. faecalis in comparison with antimicrobial photodynamic therapy (aPDT) as an alternative strategy for root canal disinfection. Materials and Methods: In this study, E. faecalis (ATCC 29212) was used. The experimental procedures included aPDT with curcumin (CUR) and indocyanine green (ICG) as photosensitizers, irrigation with 5.25% NaOCl, 0.2% and 2.0% CHX solutions as traditional endodontic irrigating solutions, and the control group. The antibacterial and anti-biofilm potentials were assessed by counting the colony forming units and also using the crystal violet assay, respectively. Results: According to the results, E. faecalis biofilm was disrupted by 65.3%, 81.0% and 92.6% using 0.2% CHX, 2.0% CHX, and 5.25% NaOCl, respectively (P<0.05). In addition, CUR- and ICG-mediated aPDT displayed a significant reduction in E. faecalis count (90.2% and 82.5%, respectively) and its biofilm (83.6% and 75.2%, respectively) in comparison to the control group (P<0.05). Conclusions: APDT has a high potential for elimination of E. faecalis and is almost equivalent to NaOCl and CHX. It can be used as an adjucnt to conventional endodontic irrigating solutions.

Abstract:Background: In root canal disinfection, intracanal medicaments play a vital role. Typical intracanal medicament, calcium hydroxide (CH), has the lowest efficacy against the biofilm of Enterococcus faecalis. Nanoparticles and their combinations may increase antimicrobial activity, making them better than conventional CH. Aim: The aim is to determine the efficacy of nanoparticle CH (NPCH) and silver nanoparticles (AgNP) against E. faecalis. Materials and Methods: A total of 60 premolars were chosen. The samples were cleaned and decoronated. Cleaning and shaping were done. The apices were tightly sealed with modeling wax before being injected with 5 μl of E. faecalis (ATCC 29212) bacterial suspension, and then left to incubate. Samples were categorized into CH (G-I), NPCH (G-II), AgNP (G-III), and a combination of NPCH + AgNP (G-IV) according to the intracanal medication used. Eppendorf tubes were used to transfer dentin chips after they had been incubated for 7 days. The process of obtaining bacterial colony-forming units (CFUs) involves inoculating sediments onto blood agar. Results: The findings are statistically significant, number of CFUs seen with the Combination of NPCH + AgNP is lower than the other experimental groups, the group treated with CH had the largest number of CFUs. Conclusions: The highest antibacterial effect was seen in combination of NPCH + AgNP.

Abstract:Introduction The present study evaluated the effect of two different back pressure-based supplemental anesthesia on postoperative pain in patients receiving endodontic treatment for a mandibular molar with symptomatic irreversible pulpitis. Methods One-hundred-thirty-five adult patients with symptomatic irreversible pulpits in a mandibular first or second molar, received an initial inferior alveolar nerve block (IANB) injection with 2% lidocaine with 1:80,000 epinephrine. Ten minutes following the injection, access cavity preparation began. Lip numbness was a must for all patients. The Heft-Parker visual analogue scale (HP-VAS) was used to measure pain during endodontic therapy. Success of primary injections was defined as no or mild pain (less than 55 mm on HP-VAS) during access preparation. The patients with initial successful anesthesia served as control and received endodontic treatment. Ninety-five patients with unsuccessful primary anesthesia randomly received either intraligamentary injections of 2% lidocaine with 1:80,000 epinephrine or intrapulpal injections with similar anesthetic solution. Endodontic treatment was re-initiated and canals were instrumented till working length under copious irrigation. Intracanal medicament of calcium hydroxide was placed and teeth received a temporary restoration. Postoperative pain was measured at 2 hours, 4 hours, 6 hours, 24 hours, and 3 days. Data were analyzed using the Pearson chi-square test, one-way analysis of variance, and one-way repeated measures analysis of variance. Results The initial IANB was successful in 40 cases (out of 135). The intraligamentary injections were successful in 33 out of 47 cases (70%) and intrapulpal injections were successful in all cases (45/45). The patients receiving intraligamentary injections reported significantly higher pain scores at all intervals till 24 hours. After 3 days, the pain significantly reduced in all the groups with no significant difference between them. Conclusions Patients receiving supplementary intraligamentary injections can experience increased postoperative pain till 24 hours after the endodontic treatment. The pain scores reduced to the level of the control group after 3 days.

Abstract:Objective: The objective is to evaluate and compare the antibacterial efficacy of nitrofurantoin, chitosan, and calcium hydroxide (Ca(OH)2) in combination with propylene glycol (PG) as an intracanal medicament against endodontic pathogens. Materials and methods: Fifty-two extracted single-rooted maxillary and mandibular anterior teeth and premolars were selected. The root canals were enlarged using Protaper universal rotary files. Clinical isolates of microorganisms collected from retreatment cases were used. Bacterial isolates obtained from infected root canals were introduced into brain-heart infusion (BHI) broth. Incubation of samples for 14 days was carried out to facilitate the development of mature biofilms. Intracanal medicaments were divided into four groups: Group 1 - Nirofurantoin+20% PG, Group 2 - Chitosan+20% PG, Group 3 - Ca(OH)2 + 20% PG, and Group 4 - 20% PG. The prepared root samples were incubated for 7 days. After collecting dentin samples, they were placed in a phosphate-buffered saline solution. Serial dilutions were then performed, and each dilution was plated on BHI agar. The plates were incubated for 24 h at 37°C. The antibacterial efficacy was assessed by calculating the percentage of remaining colony-forming units. Results: Antibacterial efficacy of chitosan paste was significantly higher followed by nitrofurantoin as compared to other groups when used as an intracanal medicament. Conclusions: In the root-canal biofilm model, the combination of chitosan and PG demonstrated a significant reduction in the viability of endodontic pathogens when employed as intracanal medication for 7 days. This suggests its potential as an effective intracanal medicament for endodontic retreatment.

Abstract:The novel pyroelectric IR detectors have been fabricated using the Polyvinylidene Fluoride (PVDF)/Lead Titanate (PT) pyroelectric bilayer thin films, which were deposited onto Pt(111)/SiO2/Si(100) substrates by a sol-gel process. The ceramic/polymer structure was constructed of the randomly oriented polycrystalline PT film (approximately 1 microm) heated at 700 degrees C for 1 h and the beta-phase PVDF film crystallized at 65 degrees C for 2 h. The effects of PVDF thin film thickness (100 approximately 580 nm) on the pyroelectric response of IR detectors were studied. The results show that the depositions of PVDF thin films onto the PT films will cause the leakage current (J) of the detectors decrease from 6.37 x 10(-7) A/cm2 to 3.86 x 10(-7) A/cm2. The specific detectivity (D*) measured at 100 Hz decreased from 2.72 x 10(7) cm x Hz(1/2)/W for detector without PVDF to 1.71 x 10(7) cm x Hz(1/2)/W for detector with PVDF thickness of 580 nm. By optimizing the ratio of the specific detectivity (D*) to leakage current, D*/J, the detector with PVDF thickness of 295 nm exhibits the best performance.
The Impact of Silver Nanoparticles' Size on Biofilm Eradication

Abstract:Introduction: Efficient intracanal disinfection is required for a successful regenerative endodontic treatment. Thus, this study aimed to identify the silver nanoparticles' (NPs) size (AgNPs) with the highest antibiofilm efficacy when mixed with calcium hydroxide [Ca(OH)2] to eradicate an in vitro endodontic biofilm. Methods: The various sizes of AgNPs and mixtures were characterized by scanning electron microscopy, transmission electron microscopy, and ultraviolet-visible spectroscopy. A total of 168 dentin root segments were prepared, sterilized, and inoculated for 3 weeks with Actinomyces naeslundii and Fusobacterium nucleatum. Samples were randomly allocated to 4 experimental groups (n = 28/group): 2 nm AgNPs + 35% Ca(OH)2, 5 nm AgNPs + 35% Ca(OH)2, 10 nm AgNPs + 35% Ca(OH)2, and 35% Ca(OH)2 alone. Samples exposed to saline and triple antibiotic paste (TAP) acted as negative and positive control groups, respectively. After 1 and 2 weeks, samples were stained with LIVE/DEAD BacLight dye and examined under a confocal laser scanning microscope to determine the proportion of dead bacteria. Results: The characterization procedure revealed a spherical NP's structure with minor aggregations. Except for Ca(OH)2 group, all groups had significantly higher antibiofilm efficacy at 2 weeks. Both the 10 nm mixture (99.5%) and TAP (99.2%) exhibited the highest antibiofilm efficacy at 2 weeks and were not significantly different from one another (P > .05). No significant difference was noted between the 2 and 5 nm mixtures at 1 week (81% and 84%) and 2 weeks (89% and 91%). Conclusion: The 10 nm AgNPs (0.02%) + 35% Ca(OH)2 mixture exhibited the highest antibiofilm efficacy at 2 weeks compared to all other mixtures at both observation periods. Interestingly, the 10 nm mixture performed similarly to TAP at 2 weeks. Excluding Ca(OH)2 group, longer application significantly improved the antibiofilm efficacy of all tested medicaments. Clinical relevance: The 10 nm AgNPs + 35% Ca(OH)2 mixture revealed promising results as an intracanal medicament in the regenerative endodontic treatment protocol.
Effectiveness of chitosan-propolis nanoparticle against Enterococcus faecalis biofilms in the root canal

Abstract:Abstract Background The successful outcome of endodontic treatment depends on controlling the intra-radicular microbial biofilm by effective instrumentation and disinfection using various irrigants and intracanal medicaments. Instrumentation alone cannot effectively debride the root canals specially due to the complex morphology of the root canal system. A number of antibiotics and surfactants are being widely used in the treatment of biofilms however, the current trend is towards identification of natural products in disinfection. The aim of the study was to determine the antibacterial effect of chitosan-propolis nanoparticle (CPN) as an intracanal medicament against Enterococcus faecalis biofilm in root canal. Methods 240 extracted human teeth were sectioned to obtain 6 mm of the middle third of the root. The root canal was enlarged to an internal diameter of 0.9 mm. The specimens were inoculated with E. faecalis for 21 days. Following this, specimens were randomly divided into eight groups ( n = 30 ) according to the intracanal medicament placed: group I: saline, group II: chitosan, group III: propolis100 μg/ml (P100), group IV: propolis 250 μg/ml (P250), group V: chitosan-propolis nanoparticle 100 μg/ml (CPN100), group VI: chitosan-propolis nanoparticle 250 μg/ml (CPN250), group VII: calcium hydroxide(CH) and group VIII: 2% chlorhexidine (CHX) gel. Dentine shavings were collected at 200 and 400 μm depths, and total numbers of CFUs were determined at the end of day one, three and seven. The non-parametric Kruskal Wallis and Mann–Whitney tests were used to compare the differences in reduction of CFUs between all groups and probability values of p < 0.05 were set as the reference for statistically significant results. The scanning electron microscope (SEM) and confocal laser scanning microscopy (CLSM) were also performed after exposure to CPNs. The effectiveness of CPNs were also evaluated against E. faecalis isolated obtained from patients having failed root canal treatment. Results The treatments of chitosan, P100, P250, CPN100, CPN250, CH and 2% CHX reduced the CFUs significantly compared to saline (p < .05). On day one and three, at 200 and 400-μm, CPN250 showed significant reduction of CFUs compared to all other groups (p < .05), while CPN100 was significantly better than other groups (p < .05) except CPN250 and 2% CHX. On day seven, at 200-μm CPN250 showed significant reduction of CFUs compared to all other groups (p < .05) except CPN100 and CHX, while at 400 μm CPN250 showed similar effectiveness as CPN100, CH and 2% CHX. SEM images showed root canal dentin treated with CPN250 had less coverage with E. faecalis bacteria similarly, CLSM images also showed higher percentage of dead E. faecalis bacteria with CPN250 than to CPN100. Conclusion CPN250 was the most effective in reducing E. faecalis colonies on day one, three at both depths and at day seven CPN250 was equally effective as CPN100 and 2% CHX.

Abstract:Aim: To assess the antimicrobial efficacy of Silver diamine fluoride (SDF), Sodium hypochlorite (NaOCl), Chlorhexidine (CHX) and Photodynamic therapy (PDT) in radicular dentin and subsequent bond strength of Fiber reinforced composite post (FRCP) to radicular dentin. Material and methods: Forty extracted premolars were disinfected and decoronated up-to cement-o-enamel junction (CEJ). Canals were instrumented using crown-down technique, dried and filled with gutta percha. Post space was prepared with the help of peso reamers and inoculated with E. faecalis. Samples were randomly allocated into four groups based on chemical irrigation (n = 10). Group 1, canal disinfected with (PDT, MB), group 2 (5.25 % NaOCl +17 % EDTA), group 3 (3.8 % SDF) and group 4 (CHX). Specimens were tested with confocal laser scanning microscopy analysis (CLSM) to confirm the presence of E. faecalis. Fibre post were luted in root canal and sectioned at three levels (apical, middle and coronal). Push-out test was performed and failure analysis of debonded surfaces was evaluated using stereomicroscope. Live and dead cells after exposure were calculated in percentage. One-way analysis of variance (ANOVA) assessed the means and standard deviations of extrusion bond strength (EBS). Tukey multiple comparison tests was used to compare means of bond strength (p = 0.05). Results: The maximum antimicrobial efficacy against E. faecalis was observed in group 2 which included canal disinfected with 5.25 % NaOCl and 17 % EDTA (64.41 ± 1.22). Similarly, CHX showed lowest effectiveness as canal irrigant against E.faecalis (58.29 ± 2.41). Intragroup comparison revealed highest bond strength for group 2 at all three level. Similarly, the lowest EBS was found in CHX group. Inter-group comparison confirmed comparable EBS at all root levels (apical, middle and coronal) for group 2 and group 3 (p > 0.05). Conclusion: Radicular dentin treated with 5.25 % NaOCl and 17 % EDTA displayed highest antimicrobial and bond strength scores. SDF and PDT can potentially be used in canal irrigation.
