Abstract:Abstract Introduction: The resin infiltration technique, a minimally invasive method, involves the saturation, strengthening, and stabilization of demineralized enamel by a mixture of polymer resins without the need to use rotary tools or the risk of losing healthy tooth structures. Aim of the study: 1. To design and synthesize an experimental infiltrant with potential bacteriostatic properties. 2. To compare the depth of infiltration of the designed experimental preparation with the infiltrant available in the market using a scanning electron microscope. Material and Methods: Composition of the experimental infiltrant was established after analysis of 1H NMR spectra of the commercially available compounds that can penetrate pores of demineralized enamel. As the infiltrant should have bacteriostatic features by definition, an addition of 1% of monomer containing metronidazole was made. Thirty extracted human teeth were soaked in an acidic solution, which was to provide appropriate conditions for demineralization of enamel. Afterward, each tooth was divided along the coronal-root axis into two zones. One zone had experimental preparation applied to it (the test group), while the other had commercially available Icon (the control group). The teeth were dissected along the long axis and described above underwent initial observation with use of a Hitachi S-4200 scanning electron microscope. Results: It was found that all samples contained only oxygen and carbon, regardless of the concentration of additions introduced into them. The occurrence of carbon is partially because it is a component of the preparation in question and partially because of sputtering of the sample with it. Hydrogen is also a component of the preparation, as a result of its phase composition; however, it cannot be detected by the EDS method. Conclusions: 1. SEM, in combination with X-ray microanalysis, does not allow one to explicitly assess the depth of penetration of infiltration preparations into enamel. 2. In order to assess the depth of penetration of infiltration preparations with use of X-ray microanalysis, it is recommended to introduce a contrast agent that is approved for use in dental materials, such as ytterbium III fluoride.

[A Study on the Penetration Abilities of Natural Initial Caries Lesions with Resin Infiltration].

Abstract:OBJECTIVE:To compare the penetration abilities of resin infiltration into natural initial caries lesions with those of adhesive in vitro.METHODS:Thirty-six extracted human molars and premolars showing proximal white spot lesions were selected. Teeth roots were removed, and the crowns were cut across the caries lesions perpendicular to the surface. Corresponding lesion halves were etched for 2 min with 15% hydrochloric acid gel and were subsequently treated with either adhesive or resin infiltration. Specimens were observed with confocal laser scanning microscopy (CLSM) in dual fluorescence mode. In confocal microscopic images, penetration depth (PD) and lesion depth (LD) were defined as the distance from the surface to the deepest point of red and green fluorescence, respectively. The penetration percentages (PP) were calculated.RESULTS:At the same level of caries, mean maximum lesion LD were comparable for both lesion halves (P > 0.05). But mean maximum PD and PP of the resin infiltration were significantly higher than those of the adhesive (P < 0.01).CONCLUSION:Penetration of enamel caries lesions is observed in the adhesive and the resin infiltration. But the resin infiltration is capable of penetrating almost completely into enamel parts of natural caries lesions.
Influence of different pre-treatments on the resin infiltration depth into enamel of teeth affected by molar-incisor hypomineralization (MIH)

Abstract:Highlights • Lesion depths of creamy-white and yellow-brown demarcated opacities are increased compared to initial caries. • Resin infiltration into MIH enamel is more heterogeneous than into initial caries. • Various pre-treatments influence resin infiltration into MIH-affected enamel variably. • Further studies are needed on how to improve resin infiltration into MIH enamel. Objectives This in vitro pilot study aimed to evaluate whether different pre-treatments (demineralization, deproteinization, (chemo-)mechanical reduction of the surface layer) influence the penetration depth of a resin infiltrant into MIH-affected enamel compared to initial carious lesions. Methods Thirty extracted human permanent molars with non-cavitated initial carious lesions ( n = 5) or MIH ( n = 25) were chosen and randomly assigned to six experimental groups: IC: initial caries; M: MIH; MN: MIH, 5.25% sodium hypochlorite; MM: MIH, microabrasion; MA: MIH, air abrasion; MAN: MIH, air abrasion and 5.25% sodium hypochlorite. A modified indirect dual fluorescence staining method was adopted to assess the penetration depth (PD) of the resin infiltrant and the lesion depth (LD) by confocal laser scanning microscopy (CLSM). Exemplarily, scanning electron microscopic (SEM) images were captured. The relationship between group assignment and penetration/lesion depth was estimated using a linear mixed model incorporating the tooth as random effect (two observations/tooth). The significance level was set at p < 0.05. Results For MIH-affected molars, the mean PD (in μm; median, [minimum-maximum]) were M (178.2 [32.5–748.9]), MN (275.6 [105.3–1131.0]), MM (48.7 [0.0–334.4]), MA (287.7 [239.4–491.7]), and MAN (245.4 [76.1–313.5]). Despite the observed differences in PD between the groups, these could not be statistically verified (Bonferroni, p = 0.322). The percentage penetration was significantly higher for IC than for MIH groups (Bonferroni, p < 0.05). Significance Compared to IC, resin infiltration into MIH-affected enamel ist more variable. Different pre-treatments influence the resin penetration into developmentally hypomineralized enamel to a fluctuating level.

In Vitro Evaluation of Antibacterial Properties and Smear Layer Removal/Sealer Penetration of a Novel Silver-Citrate Root Canal Irrigant

Abstract:This study aimed at evaluating the efficacy of a novel silver-citrate root canal irrigation solution (BioAKT) on smear layer removal, sealer penetration after root canal instrumentation and antibacterial activity. Single-root teeth were endodontically treated, sealed with an epoxi-amine resin sealer and irrigated using: Group I: 5.25% sodium hypochlorite (NaOCl); Group II: silver-citrate solution (BioAKT); Group III: phosphate buffer solution (PBS); Group IV: 17% ethylenediaminetetraacetic acid (EDTA). Smear layer removal and silver deposition at the coronal, middle and apical portion of each canal were analyzed using scanning electron microscopy (SEM) and energy-dispersive x-ray spectroscopy (EDS). Sealer penetration into dentinal tubules at coronal, middle and apical portion was assessed through dye-assisted confocal microscopy (CSM). Both SEM and CSM micrographs were evaluated by two examiners (κ = 0.86), who were blind to the irrigation regimens; scores were given according to the degree of penetration of the sealer. Data analysis included Pearson's x2 and Sidak's multiple comparisons. Dentin discs were polished and sterilized. Enterococcus faecalis biofilms were grown using a continuous-flow bioreactor under anaerobic conditions for 72 h. Specimens were irrigated with the tested solutions, and bacterial viability was assessed using a tetrazolium salt assay (MTT). Statistical analysis included one-way ANOVA and Student's post-hoc t-test (p < 0.05). BioAKT and EDTA were the most efficient solutions both in removing the smear layer and allowing sealer penetration. However, at the apical portion BioAKT performed significantly better compared to EDTA both in smear layer removal and sealer penetration (p < 0.05). BioAKT and NaOCl showed comparable antibacterial effect (p = 0.53). In conclusion, BioAKT represents a suitable smear layer removal agent, which allows for reliable sealer penetration at the apical portion of the root canal system and offers significant antibacterial properties.
Assessment of microleakage in root canals sealed with an experimental endodontic composite

Abstract:Research in endodontics has for the most part been primarily focused on the development of the tools and materials most effective in facilitating root canal treatment. The majority of endodontic treatment principles originally evolved on the basis of trial and error; only recently have scientific methods been adopted that identify appropriate clinical strategies. The biological basis for the therapy has received comparatively little attention, and the advancement of biologically-based knowledge significant to clinical endodontics has been slow. Several controversial and as-yet unresolved biological issues relevant to endodontics were discussed in a recent review article (1) which specifically deals with disagreements regarding the management of pulpal exposures by caries in adult dentition, as well as the causes of and measures to control infections of the root canal system. The primary objective of root canal therapy is to introduce the appropriate placement of a seal between the root canal system and the periodontium. An ideal root-end filling This study compares the apical microleakage (ML) of an experimental amorphous calcium phosphate (ACP) endodontic sealer with the ML of a commercial material with similar resin composition (Resilon). The penetration of the methylene blue dye solution following 72 h immersion at 37 °C was measured in root canals of single-rooted human teeth treated with: 1) experimental primers or Resilon primer, 2) experimental ACP selaer or Resilon sealer, and 3) Gutta-perha or Resilon filler. The results contradict the reported ability of Resilon to provide an almost hermetic seal or perform better than gutta-percha. The ML values obtained with ACP sealant (equal to or up to 16 % lower than the ML values in Resilon groups) merit further evaluation of this experimental material.

In vitro bacterial penetration and dissemination through dentinal tubules in roots filled with calcium silicate‐based cements

Abstract:The aim of this study was to assess whether calcium silicate root fillings prevent bacterial penetration and to determine how bacteria penetrate roots. Extracted single‐rooted, single‐canal human teeth were decoronated, prepared and filled with ProRootMTA or Biodentine (n = 12 each). Positive and negative (n = 2 each) controls were not filled. A two‐chamber model was used with Streptococcus gordonii. The lower compartment was evaluated for turbidity over 150 days. Roots were split and examined for bacteria via SEM. The chi‐squared test was used for comparisons (α = 0.05). Experimental groups had bacteria in their coronal thirds. Tubules contained bacteria in 90.9% and 91.7% of areas examined in the Biodentine and ProRootMTA groups, respectively, with no significant difference (p = 0.914). Experimental and negative roots had no turbidity with no significant difference between Biodentine and ProRootMTA (p = 1.000). Positive controls had turbidity. Bacteria penetrate roots via dentine tubules of root‐filled teeth. Biodentine was comparable to ProRoot MTA.

Remineralization of Artificial Dentin Caries Using Dentin and Enamel Matrix Proteins

Abstract:To assess the remineralizing potential of dentin matrix proteins and enamel matrix derivatives (DMPs and EMDs) after application on artificially induced dentin lesions, given the hypothesis that these materials increase the mineral uptake, binding, and mineralization. Forty-eight caries-free human premolars were used. Teeth were cut, polished, and embedded, leaving an open window on the root surface, of which one-third was covered with a flowable composite to preserve the healthy untreated dentin. Then, samples were demineralized in Buskes solution for 33 days. A micro-CT scan prior to treatment was performed. Next, the samples were randomly allocated into four groups: (A) An untreated negative control (CON), (B) application of porcine dentin matrix proteins (DMP), (C) treatment with enamel matrix derivatives (EMD, Emdogain, Straumann), and (D) amine fluoride application (AMF, Elmex fluid, GABA). All samples were placed in artificial saliva for 21 days. A second micro-CT scan was performed, after which the change in gray scaling within a defined region of interest (0.25 mm3) was analyzed. ANCOVA was applied to discover statistical differences between the different treatments. Both, treatment with AMF; (P = 0.011 versus CON) as well as with DMP (P = 0.043 versus CON) yielded a statistically significant difference compared to the control treatment. EMD treatment was not found to differ (P > 0.05). Mainly the top layer of the defects showed clear signs of remineralization, which was also evident in CON. This study was able to visually confirm the remineralization potential of demineralized dentin especially after DMP application, which, however, did not outperform AMF. Based on this, additional studies combining proteins and fluorides are now warranted and ongoing.

Abstract:Background: The main factors that affect the success of an endodontic infection are effective cleaning and shaping of the root canal system including complete disinfection by using chemical irrigating solutions and obturation with an endodontic sealer to provide a fluid-tight seal. Using rotary and hand instruments for shaping and cleaning the root canal along with irrigants produces a smear layer on the surface of the root dentin affecting the penetration of the endodontic sealer into the dentinal tubules. This smear is difficult to remove with the use of only endodontic irrigants, hence, chelating agents were introduced in adjunct with irrigating solutions for irrigation protocol for effective removal of smear layer which affect the penetration of endodontic sealers into the dentinal tubules. Methods: 32 mandibular premolar teeth were used. The biomechanical preparation was done till Protaper F3 size. Irrigation was done with 2.5 mL sodium hypochlorite (NaOCl) solution after each instrumentation change for 1 min. Samples were then divided into 4 groups according to the irrigating solution used as the final rinse used with passive ultrasonic agitation. The groups were: Group I: 5 ml of saline, Group II: 5ml of 17% ethylenediaminetetraacetic acid, Group III: 5 ml of 10% citric acid, Group IV: 5 ml of 7% maleic acid each for one minute. All the canals were obturated with BioRoot tmRCS with gutta-percha using the ultrasonic condensation technique. For staining the samples for Confocal LASER microscopy, BioRoot tmRCS was mixed with Rhodamine B dye. Results: The maximum penetration of bio-ceramic sealer was observed in the coronal region. At the apical third, the maximum sealer penetration was seen with 7% maleic acid. Conclusions: Maximum sealer penetration was seen in the coronal section followed by the middle and apical section. Maximum sealer penetration was seen with 7 % maleic acid at the apical third.
[Comparative Study on the Penetration Abilities of Resin Infiltration into Proximal Initial Caries Lesions in Primary Molars and Permanent Posterior Teeth].

Abstract:To compare the penetration abilities of resin infiltration into proximal lesions in primary molars and permanent posterior teeth in vitro.Extracted or exfoliated primary molars (n = 32) and permanent posterior teeth (n = 36) with proximal white spot lesions were selected. Roots of the teeth were removed, and the crowns were cut across the caries lesions perpendicular to the surface. Cut surfaces were examined with stereo microscopy and classified with respect to histological lesion extension (C1-C4): Lesions confined to the outer half on enamel (C1), lesions confined to the inner half on enamel(C2), lesions confined to the outer half on dentin(C3), lesions extending into the inner half of dentin (C4). C4 were excluded. Caries lesions were etched for 2 min with 15% hydrochloric acid gel and were subsequently treated for 5 min with resin infiltration. Thin sections from each lesion were prepared and specimens were observed with confocal laser scanning microscopy (CLSM) through a dual-fluorescence staining technique. Lesion depth (LD) and penetration depth (PD) of resin infiltration were measured in primary molars and permanent posterior teeth in confocal microscopic images, and percentage penetration (PP) were calculated respectively. The date were collected and analyzed.There were no significant differences for overall PP in primary molars and permanent posterior teeth groups (P > 0.05). However, the penetration abilities in primary molars were slight higher than those in permanent posterior teeth in C1 group (P < 0.05). The penetration abilities had no significant difference in primary molars and permanent posterior teeth in both C2 and C3 groups (P > 0.05).Resin infiltration have satisfied penetration abilities in primary molars and permanent posterior teeth with proximal initial lesions. The better penetration abilities of resin infiltration in primary molars are shown in Cl lesions than those in permanent posterior teeth.
Evaluation of resin sealer penetration of dentin following different final rinses for endodontic irrigation using confocal laser scanning microscopy

Abstract:The aim of the study was to assess the depth of sealer penetration into dentinal tubules following different final rinses and indirectly evaluate precipitation of irrigating solutions. Methods: 52 extracted maxillary incisors were prepared to size 40/04 with 6% sodium hypochlorite irrigating solution. Following power analysis, teeth were randomly divided into 4 groups (n = 13) according to final rinse. Group 1 - 17% EDTA, Saline and 2% Chlorhexidine solution; Group 2 - SmearOFF™; Group 3 - Qmix® ; and Group 4 - saline control. Obturation was performed with gutta percha and resin sealer mixed with rhodamine-B-isothiocyanate dye using warm vertical compaction. Teeth were sectioned into apical, middle, and coronal thirds using Buehler diamond saw at 3, 6 and 9 mm from the apex. Sections were examined under confocal laser scanning microscopy, and images were exported to Image J software to measure the maximum depth of sealer penetration circumferentially. Data were analysed using a generalised linear model and post hoc multiple comparisons with significance set at P < 0.05. Group 3 had the highest depth of penetration. There were statistically significant differences between Groups 2 and 3 and control for all sections, Group 1 and control in apical section only. Conclusion: Recently developed final rinses produced higher sealer penetration and more patent dentinal tubules than using EDTA and chlorhexidine sequentially.
Analysis of the depth of penetration of an epoxy resin-based sealer following a final rinse of irrigants and use of activation systems: An in vitro study

Abstract:Objective: The objective of the study was to compare and evaluate the depth of penetration of an epoxy resin-based sealer following a final rinse of 17% ethylenediaminetetraacetic acid (EDTA) and 18% 1-hydroxyethylidene 1, 1-diphosphonate (HEDP), with diode laser and passive ultrasonic activation (PUI): an in vitro confocal laser scanning microscopy study. Materials and methods: Fifty-two extracted human mandibular premolar teeth with single root and single canal were selected. They were disinfected in 0.1% thymol solution, cleaned of calculus and soft tissues, and stored in 0.1% thymol solution till use. All teeth were radiographed and selected as per the inclusion and exclusion criteria. The teeth were decoronated using a diamond disk under copious water spray to acquire a standardized root length of 14 mm. Working length was established by inserting a size 10-K file into each root canal until it is visible at the apical foramen and by subtracting 1 mm from the recorded length. Instrumentation of the root canal was done till master apical file size of F3 using ProTaper universal, rotary instruments. The canals were irrigated with 2 mL of 3% sodium hypochlorite between successive files. Teeth were randomly divided into four subgroups n = 12 according to the intervention. Passive ultrasonic irrigation and diode laser were used to activate the irrigants. Final irrigation was performed with distilled water. These specimens were examined using confocal laser scanning microscope (OLYMPUS FLUOVIEW FV 3000) for dentinal tubule penetration of the sealer. Two-way ANOVA test and Tukey's multiple post hoc test were used for statistical analysis. Results: Highly significant difference was seen between the groups with EDTA and HEDP, with HEDP demonstrating the highest penetration. Among the activation techniques used in this study, PUI showed the highest penetration of the sealer. The least penetration was seen with diode laser activation and EDTA. Conclusions: The irrigation activation techniques significantly influence the penetration of sealer into root dentinal tubules. When penetration of sealer with different irrigation techniques and irrigants was evaluated, significant greater level of sealer penetration was attained with PUI activation of HEDP.
Effectiveness of XP-Endo Finisher, Endoactivator, and PUI agitation in the penetration of intracanal medicaments into dentinal tubules: A confocal laser scanning microscope analysis

Abstract:Background: This investigation assessed the impact of irrigation activation systems on the depth of penetration of intracanal medicaments into dentinal tubules. Methods: Ninety-six mandibular premolar teeth were prepared using ProTaper Next up to size X3. The teeth were randomly divided into four groups based on the final irrigation activation systems (n=24): group 1: EndoActivator, group 2: XP-Endo Finisher, group 3: Passive ultrasonic irrigation (PUI), and group 4: control. After the final irrigation, all the samples were divided into two subgroups (n=12): subgroup A: calcium hydroxide (Ca(OH)2 ) and subgroup B: modified triple antibiotic paste (mTAP). Statistical analysis involved three-way analysis of variance (ANOVA). Results: EndoActivator, XP-Endo Finisher, and PUI activation methods significantly increased the penetration of intracanal medicaments compared to conventional needle irrigation (P<0.05). The XP-Endo Finisher group exhibited the highest penetration percentage and maximum penetration depth, showing a statistically significant difference from the EndoActivator group (P<0.001). No significant difference was observed between XP-Endo Finisher and PUI, nor between PUI and EndoActivator (P>0.05). mTAP showed a higher percentage of dentinal tubule penetration than Ca(OH)2, although no significant difference was found in maximum penetration depth. The coronal region demonstrated the highest penetration percentage and depth, while the apical region showed the lowest. Conclusion: This study showed the effectiveness of XP-Endo Finisher in improving the penetration of intracanal medicaments into dentinal tubules. The findings emphasize the importance of selecting appropriate irrigation activation systems to improve treatment outcomes in endodontics. By demonstrating the effectiveness of advanced systems like XP-Endo Finisher, this research supports their integration into routine clinical practice for better endodontic success.
Dentinal tubule penetration of AH Plus, iRoot SP, MTA fillapex, and guttaflow bioseal root canal sealers after different final irrigation procedures: A confocal microscopic study

Abstract:Background and objective: Varied physical and chemical characteristic of root canal sealers and different irrigant agitation systems can influence the depth of penetration. The aim of this in vitro study was to use a laser scanning confocal microscope in order to assess the dentinal tubules penetration of various sealers after the application of different final irrigation techniques. Study design/materials and methods: A total of 156 single-rooted extracted mandibular premolars were prepared up to size 40 and randomly distributed into four groups according to the sealer type (n = 39): AH Plus, iRoot SP, MTA Fillapex, and GF Bioseal. Each group was randomly subdivided into three groups according to the final irrigation protocol (n = 13): conventional needle irrigation (CI), photon-induced-photoacoustic streaming activation (PIPS), and passive ultrasonic irrigation (PUI). After the final irrigation procedures, the root canals were obturated with single gutta-percha and labeled sealer mixed with 0.1% fluorescent rhodamine B isothiocyanate. Specimens were sectioned at 2, 5, and 8 mm from the apex, and all the sections were examined under confocal microscope to calculate the dentinal tubule penetration area. Data were analyzed using three-way analysis of variance and Tukey's post hoc tests (P = 0.05). Results: iRoot SP exhibited a significantly higher penetration area than the other groups (P < 0.001), although there were no statistically significant differences between AH Plus, MTA Fillapex, and GF Bioseal (P > 0.05). Er:YAG laser activation with PIPS and PUI had significantly higher penetration than CI (P < 0.001). Statistically significant differences were also determined at each root canal third (coronal > middle > apical; P < 0.001). Conclusions: The dentinal tubule penetration area was significantly affected by the selection of root canal sealer, final irrigation procedure, and root canal third. Use of iRoot with PIPS tip or PUI seems advantageous in dentinal tubule penetration.

Abstract:Aim: Aim of the present research was to investigate the effectiveness of various fluoride-releasing dental restorative agents in preventing demineralization of enamel. Materials and methods: Eighty human mandibular permanent molar teeth constituted the study group. All samples were subjected to storage in thymol, after which they were taken out to prepare alike proximal box in each. Inductions of artificial enamel surface lesions were done by placing the teeth in demineralizing solution for 96 hours. Subsequently, all 80 molars were randomly assigned to any of the four groups (i.e., 20 in every individual group) according to the restoration as group A: giomer (composite resin containing surface pre-reacted glass-ionomer fillers), group B: compomer (polyacid-modified composite resin), group C: resin-modified glass-ionomer cement (RMGIC), group D: fluoride-releasing composite. After this, the pH cycling was performed, and the samples were subjected to examination beneath scanning electron microscope (SEM). Results: Higher mean areas of remineralization were noted when RMGIC (96.34 ± 0.06) was used followed by the compomer (109.52 ± 0.17), giomer (118.39 ± 0.82), and the fluoride-releasing composite group (129.27 ± 0.31) in that order. A statistically significant difference was seen amid the investigational groups that utilized different restorative agents (p <0.001). A pairwise evaluation that was performed revealed that except for the giomer group and the compomer group, a statistically significant difference (p <0.001) was found among the experimental groups. Conclusion: This research infers that the RMGIC-treated samples exhibited significantly superior performance in preventing enamel demineralization in comparison to compomer, giomer as well as fluoride-releasing composites. Clinical significance: One among the highly frequently employed anticariogenic materials is fluorides. Owing to this characteristic, they are integrated into numerous restorative substances. Nevertheless, the quantity and speed of fluoride release differ in different agents, which translates to the efficacy of the restorative agent in avoiding demineralization about the restoration.

Abstract:The ideal root end filling material should form a tight seal in the root canal by adhering to the cavity walls. Several materials have been used for root end filling. The present study aims to find out and compare the bioactivity of Neo MTA Plus, Pro Root MTA White, BIODENTINE & glass ionomer cement as root end filling materials using 1% methylene blue as tracer. Materials and methods: 80 extracted human permanent maxillary anterior teeth were used in the study. They were divided into four groups. Specimens were sectioned transversely in the cervical area to separate the crown from the root. The root canal was obturated with gutta percha and zinc oxide eugenol sealers. Thereafter, each sample was resected apically by removing 3 mm of the apex and filled with different materials. Samples were kept in buffering solution at 37 °C until the recommended evaluation periods. The specimens were then suspended in 1% methylene blue for 24 h, prior to the analysis. The teeth were then sectioned, and dye penetration was examined, photographed, and evaluated under a stereomicroscope. Results: Vertical dye penetration showed significant differences across different groups. The minimum dye penetration was seen in Neo MTA plus followed by BIODENTINE, Pro Root MTA and maximum in GIC. There was no significant difference in dye penetration between Neo MTA plus and BIODENTINE both at fifteen days and one-month intervals. Conclusion: The present study suggests Neo MTA plus and BIODENTINE should be the preferred material for root end filling.

Abstract:This preregistered ex vivo investigation examined the dentinal hybrid layer formation of a resinous infiltrant (Icon), with reference to both thickness (HLT) and homogeneity when combined with modified tunnel preparation (occlusal cavity only) and internal/external caries infiltration. The adhesives Syntac and Scotchbond MP were used as controls (Groups 1 and 3) or in combination with Icon (Groups 2 and 4). A split-tooth design using healthy third molars from 20 donors resulted in 20 prepared dentine cavities per experimental group. The cavity surfaces (n = 80) were etched (37% H 3 PO 4 ), rinsed, and air-dried. Rewetting with ethanol was followed by application of the respective primers. After labeling with fluorescent dyes, either Syntac Adhesive/Heliobond or Scotchbond MP Adhesive was used alone or supplemented with Icon. HLT, as evaluated by scanning electron microscopy, did not significantly differ (P > 0.05), and confocal laser scanning microscopy revealed homogeneously mixed/polymerized resin-dentine interdiffusion zones in all groups. Icon can be successfully integrated into an ethanol-wet dentine bonding strategy, and will result in compact and homogeneous hybrid layers of comparable thickness considered equivalent to the non-Icon controls, thus allowing for preservation of the tooth's marginal ridge and interdental space in the case of internal/external infiltration of proximal caries.

Abstract:This study aimed to evaluate the dentinal tubule penetration and filling quality of a bioceramic sealer (iRoot SP). Forty-two roots of extracted adult lower incisors were selected. After instrumentation with Protaper Universal to F3, 40 roots were chosen and randomly divided into 4 groups, as follows: iRoot SP single cone group, iRoot SP warm vertical group, AH Plus single cone group, and AH Plus warm vertical group. Before root canal filling, sealers were mixed with Rhodamine B dye for visualization under confocal laser scanning microscope. All samples were sectioned at 2, 4, and 6 mm to apex. Then, the percentages of void areas, gap regions, and segments of sealer that penetrated into dentinal tubules in each section were calculated. Non-parametric test was used for statistical analysis (α = 0.05). We found that filling techniques and types of sealer had no statistically significant effects on the occurrence of voids and gaps. The segments of iRoot SP penetrated into dentinal tubules were statistically more than that of AH Plus in both single cone and warm vertical techniques at 2 mm to apex (P < 0.05). Regardless of the filling technique used, iRoot SP can achieve comparable filling quality and better dentinal tubules penetration than AH Plus. Considering the good bioactivity of iRoot SP, it may help improve the seal of root canal system.

Abstract:The mineral content of dental hard tissues has traditionally been measured by destructive tests such as transverse microradiography. Microfocus X-ray computed tomography (micro CT) has enabled non-destructive 3D assessment of tooth demineralization. This study compared the preventive effects of silver diammine fluoride (SDF) and potassium iodide (KI) in comparison with fluoride varnish. SDF has been known to arrest caries but darkens the tooth. KI creates a precipitate with SDF that reduces the discoloration, but its effects on SDF efficacy in terms of preventing demineralization of at-risk root dentin surfaces is unknown. Bovine root dentin blocks were randomly distributed into four groups and subjected to a pretreatment in each group (n = 8); Control: deionized water (DIW); F-Varnish: 5% sodium fluoride varnish: 38% Saforide; SDF+KI: SDF followed by saturated solution of KI in DIW. The treated dentin was subjected to 8 cycles of demineralization (pH 5) for 14 h and remineralization in artificial saliva (pH 7) for 10 h. Specimens were then scanned for 12 min using micro CT at 73 KV and 1012 μA with 8.3 μm resolution. The 3D images were analyzed in Amira software to calculate lesion depth (LD), surface layer mineral density (SL) and mineral loss (ΔZ) for each specimen. One-way ANOVA with Bonferroni posthoc showed that there was a statistically significant difference between Control and all three other groups for all parameters (P 0.05). Single application of F-Varnish, SDF and SDF+KI showed comparable preventive effects against root dentin demineralization. Application of KI did not affect anti-demineralization properties of SDF in this study. Micro CT is a quick and effective method for objective and high-resolution characterization of dentin caries lesions.

Abstract:The adaptation of a sealer along with the periphery of the dentinal tubules of the root canal is studied. Various techniques have been used for the application of these sealers onto the canal wall for better adaptation but have not been compared to date. The purpose of the study was to comparatively evaluate the sealing ability of a bioceramic sealer with AH plus sealer with root canal dentin using three different techniques for the application of sealer. One hundred twenty extracted maxillary and mandibular anterior teeth were collected, disinfected, and decoronated at the cemento-enamel junction to maintain a standard working length for all samples. The establishment of the working length (40.10) and instrumentation was performed using a rotary instrument, along with a standard irrigation regimen. The teeth were then divided into two main groups according to the sealer used, i.e., Group A (AH Plus) and Group B (iRoot SP). These two main groups were categorized into three sub-groups depending on the technique of sealer placement, i.e., Subgroup 1 (master cone gutta-percha), Subgroup 2 (bidirectional spiral), and Subgroup 3 (passive ultrasonic activation). Out of the 20 samples, 15 samples were randomly allocated for the assessment of sealing ability using the routine dye extraction method, and to verify the results of the dye extraction method, a more advanced evaluation method, i.e., SEM evaluation, was utilized further. To this end, five random samples from each subgroup were allocated for SEM analysis. The obtained scores were then statistically analyzed using an ANOVA test and Post Hoc Tukey’s test. In the current study, statistical significance was seen among the three main groups and six subgroups with p-values < 0.005. Subgroup B3 performed significantly better than the other subgroups in both the dye extraction method as well as in SEM analysis. The highest microleakage was shown by subgroup A1; it also exhibited poor penetration of sealer in SEM evaluation. The bioceramic sealer (iRoot SP), when applied using passive ultrasonic activation, showed the best results in both the dye extraction method and the SEM evaluation.
