Abstract:From the stripping points, the high intensity proton beam of a compact cyclotron travels through the fringe field area of the machine to the combination magnet. Starting from there the beams with various energy is transferred to the switching magnet for distribution to the beam line targets. In the design of the extraction and transport system for the compact proton cyclotron facilities, such as the 70MeV in France and the 100MeV in China, the space charge effect as the beam crosses the fringe field has not been previously considered; neither has the impact on transverse beam envelope coupled from the longitudinal direction. Those have been concerned much more with the higher beam-power because of the beam loss problem. In this paper, based on the mapping data of 70MeV cyclotron including the fringe field by BEST Cyclotron Inc (BEST) and combination magnet field by China Institute of Atomic Energy (CIAE), the beam extraction and transport are investigated for the 70MeV cyclotron used on the SPES project at Istituto Nazionale di Fisica Nucleare, Laboratori Nazionali di Legnaro (INFN–LNL). The study includes the space charge effect and longitudinal and transverse coupling mentioned above, as well as the matching of beam optics using the beam line for medical isotope production as an example. In addition, the designs of the ±45° switching magnets and the 60° bending magnet for the extracted beam with the energy from 35MeV to 70MeV have been made. Parts of the construction and field measurements of those magnets have been done as well. The current result shows that, the design considers the complexity of the compact cyclotron extraction area and fits the requirements of the extraction and transport for high intensity proton beam, especially at mA intensity levels.
Development and characterization of the first proton minibeam system for single‐gantry proton facility

Abstract:Background Minibeam represents a preclinical spatially fractionated radiotherapy modality with great translational potential. The advantage lies in its high therapeutic index (compared to GRID and LATTICE) and ability to treat at greater depth (compared to microbeam). Proton minibeam radiotherapy (pMBRT) is a synergy of proton and minibeam. While the single‐gantry proton facility has gained popularity due to its affordability and compact design, it often has limited beam time available for research purposes. Conversely, given the current requirement of pMBRT on specific minibeam hardware collimators, necessitates a reproducible and fast setup to minimize pMBRT treatment time and streamline the switching time between pMBRT and conventional treatment for clinically translation. Purpose The contribution of this work is the development and characterization of the first pMBRT system tailored for single‐gantry proton facility. The system allows for efficient and reproducible plug‐and‐play setup, achievable within minutes. Methods The single room pMBRT system is constructed based on IBA ProteusONE proton machine. The end of nozzle is attached with beam modifying accessories though an accessory drawer. A small snout is attached to the accessory drawer and used to hold apertures and range shifters. The minibeam aperture consists of two components: a fitting ring and an aperture body. Three minibeam apertures were manufactured. The first‐generation apertures underwent qualitatively analysis with film, and the second generation aperture underwent more comprehensive quantitative measurement. The reproducibility of the setup is accessed, and the film measurements are performed to characterize the pMBRT system in cross validation with Monte Carlo (MC) simulations. Results We presented initial results of large field pMBRT aperture and the film measurements indicates the effect of source‐to‐isocenter distance = 930 cm in Y proton scanning direction. Consistent with TOPAS MC simulation, the dose uniformity of pMBRT field <2 cm is demonstrated to be better than 2%, rendering its suitability for pre‐clinical studies. Subsequently, we developed the second generation of aperture with five slits and characterized the aperture with film dosimetry studies and compared the results to the benchmark MC. Comprehensive film measurements were also performed to evaluate the effect of divergence, air gap and gantry‐angle dependency and repeatability and revealing a consistent performance within 5%. Furthermore, the 2D gamma analysis indicated a passing rate exceeding 99% using 3% dose difference and 0.2 mm distance agreement criteria. We also establish the peak valley dose ratio and the depth dose profile measurements, and the results are within 10% from MC simulation. Conclusions We have developed the first pMBRT system tailored for a single‐gantry proton facility, which has demonstrated accuracy in benchmark with MC simulations, and allows for efficient plug‐and‐play setup, emphasizing efficiency.

A novel electron source for a compact x-ray tube for microbeam radiotherapy with very high dose rates

Abstract:Microbeam radiotherapy (MRT) is a novel concept in radiation oncology with arrays of alternating micrometer-wide high-dose peaks and low-dose valleys. Preclinical experiments have shown a lower normal tissue toxicity for MRT with similar tumor control rates compared to conventional radiotherapy. A promising candidate for the demanded compact radiation source is the line-focus x-ray tube. Here, we present the setup of a prototype for an electron accelerator being able to provide a suitable x-ray beam for the tube. Several beam dynamic calculations and simulations were performed concerning particle tracking, thermal and electrostatic properties of the electron source, resulting in a proper beamline, including the cathode, the pierce electrode (PE) and the focusing magnets. These parts are discussed separately. The simulations showed that a rectangular cathode with a small width of 0.4mm is mandatory. To quickly shut down the electron beam, an additional voltage of -600V must be applied to the PE. Moreover, the electric field inside the vacuum chamber stays below 10MVm-1 to minimize the risk of field emission. The thermal simulation validates a small displacement of 0.1mm of the heated cathode with respect to the PE, which must be considered during manufacturing of the cathode-PE assembly. The simulations lead to an adequate choice of cathode, electrodes and beamline to achieve the required focal spot of 0.05×20mm2 with a beam current of 0.3A and an electron energy of 300keV. With this setup first MRT experiments with high dose rates up to 10Gys-1 can be executed.
Commissioning a clinical proton pencil beam scanning beamline for pre-clinical ultra-high dose rate irradiations on a cyclotron-based system

Abstract:Background: This manuscript describes modifications to a pencil beam scanning (PBS) proton gantry that enables ultra-high dose rates (UHDR) irradiation, including treatment planning and validation. Methods: Beamline modifications consisted of opening the energy slits and setting the degrader to pass-through mode to maximize the dose rate. A range shifter was inserted upstream from the isocenter to enlarge the spot size and make it rotationally symmetric. We measured the beamline transport efficiency and investigated the variation in output due to the recombination of charge in the dose monitoring chamber. The output calibration was performed through a parallel plate chamber (PPC05), and an intercomparison was performed for various detectors. The pre-clinical field for mice irradiation consisted of different dose levels to deliver uniform doses in transmission mode. The field dose rates were determined through log files while scripting in TPS was used to estimate PBS dose rates. The survival experiments consisted of irradiating the full pelvis of the mice at UHDR and conventional dose rates. Results: The spot size was constant with beam current and had a sigma of 8.5 mm at the isocenter. The beam output increased by 35% at 720 nA compared to 5.6 nA, primarily due to recombination in the dose-monitoring ion chambers. The Faraday Cup and PPC05 agreed within 2%, while other detectors were within 3% of FC for dose rates <60 Gy/s. The pre-clinical fields' PBS dose rate is above 45 Gy/sec for all voxels within the target volume. The average and PBS dose rates decrease as field size increases and approaches 40 Gy/s for a field size of 7x7 cm 2 . All UHDR arms showed better survival than the corresponding conventional dose rate arms. Conclusions: We successfully modified a clinical system to perform UHDR pre-clinical experiments. As part of our pre-clinical experiments, we observed the FLASH effect concerning mice survival.

Abstract:Abstract Proton minibeam radiation therapy (pMBRT) is a novel dose delivery method based on spatial dose fractionation. pMBRT has been shown to be promising in terms of reduced side effects and superior tumour control in high-grade glioma-bearing rats compared to standard irradiation. These findings, together with the recent optimized implementation of pMBRT in a clinical pencil beam scanning system, have triggered reflection on the possible application to patient treatments. In this context, the present study was designed to conduct a first theoretical investigation of the clinical potential of this technique. For this purpose, a dedicated dose engine was developed and used to evaluate two clinically relevant patient treatment plans (high-grade glioma and meningioma). Treatment plans were compared with standard proton therapy plans assessed by means of a commercial treatment planning system (ECLIPSE-Varian Medical systems) and Monte Carlo simulations. A multislit brass collimator consisting of 0.4 mm wide slits separated by a centre-to-centre distance of 4 or 6 mm was placed between the nozzle and the patient to shape the planar minibeams. For each plan, spread-out Bragg peaks and homogeneous dose distributions (±7% dose variations) can be obtained in target volumes. The Peak-to-Valley Dose Ratios (PVDR) were evaluated between 9.2 and 12.8 at a depth of 20 mm for meningioma and glioma, respectively. Dose volume histograms (DVHs) for target volumes and organs at risk were quantitatively compared, resulting in a slightly better target homogeneity with standard PT than with pMBRT plans, but similar DVHs for deep-seated organs-at-risk and lower average dose for shallow organs. The proposed delivery method evaluated in this work opens the way to an effective treatment for radioresistant tumours and will support the design of future clinical research.

2.5 MeV CW 4-Vane RFQ Accelerator Design for BNCT Applications

Abstract:Boron Neutron Capture Therapy (BNCT) promises a bright future in cancer therapy for its highly selective destruction of cancer cells, using the 10B +n→7Li +4 He reaction. It offers a more satisfactory therapeutic effect than traditional methods for the treatment of malignant brain tumors, head and neck cancer, melanoma, liver cancer and so on. A CW 4-vane RFQ, operating at 162.5 MHz, provides acceleration of a 20 mA proton beam to 2.5 MeV, bombarding a liquid lithium target for neutron production with a soft neutron energy spectrum. The fast neutron yield is about 1.73×1013n∕s. We preliminarily develop and optimize a beam shaping assembly design for the 7Li(p, n)7Be reaction with a 2.5 MeV proton beam. The epithermal neutron flux simulated at the beam port will reach up to 1.575×109 n/s/cm2. The beam dynamics design, simulation and benchmark for 2.5 MeV BNCT RFQ have been performed with both ParmteqM (V3.05) and Toutatis, with a transmission efficiency higher than 99.6% at 20 mA. To ease the thermal management in the CW RFQ operation, we adopt a modest inter-vane voltage design (U=65 kV), though this does increase the accelerator length (reaching 5.2 m). Using the well-developed 3D electromagnetic codes, CST MWS and ANSYS HFSS, we are able to deal with the complexity of the BNCT RFQ, taking the contribution of each component in the RF volume into consideration. This allows us to optimize the longitudinal field distribution in a full-length model. Also, the parametric modeling technique is of great benefit to extensive modifications and simulations. In addition, the resonant frequency tuning of this RFQ is studied, giving the tuning sensitivities of vane channel and wall channel as −16.3 kHz/°C and 12.4 kHz/°C, respectively. Finally, both the multipacting level of this RFQ and multipacting suppressing in the coaxial coupler are investigated.

Abstract:S-band high-gradient accelerating structures were proposed to accelerate protons from 30 to 230 MeV for a compact proton therapy linac in Institute of Modern Physics. A backward traveling wave structure and a single-periodic magnetically coupled standing wave structure were designed, and differences between these two structures were analyzed to select a more suitable one for the proton therapy linac. In this paper, a novel single-periodic magnetically coupled standing-wave accelerating structure was studied, with an optimized nose cone that related to a smaller peak surface electric field and higher shunt impedance, with the reduced number of coupling holes and mechanical von Mises stress thus making it possible for higher duty factor and longer rf pulse operation, also with larger coupling holes and improved cell-to-cell coupling thus making this structure suitable for operating in π mode. An 11-cell high-power prototype for β=0.26 particles was developed with an active length of 143 mm. This prototype cavity is supposed to operate at an accelerating gradient of 30 MV/m with 0.1% duty factor, corresponding to a maximum surface electric field of 105 MV/m and a peak rf power of 3.43 MW. Charged Particle Radiobiology Beamline Using Tandem Accelerator-Based MeV Protons and Carbon Ions: a Pilot Study on the Track-End Radiation Quality, Variable Biological Effectiveness and Bayesian Beam Dosimetry

Abstract:For in vitro cell irradiation using tandem accelerator-based MeV protons and carbon ions, by TOPAS simulation, a pilot study of performance evaluation is presented on a collimation beamline for 3 MeV protons and 10 MeV carbon ions from a 2 × 3 MV tandem accelerator. Based on the elements and source parameters, a collimated beam of 2.8 MeV protons or 2.5 MeV carbon ions, with 5.175 mm or 5.166 mm full width tenth maximum (FWTM), respectively, can be delivered to the target cell dish. TOPAS simulations and/or deterministic algorithms present a Bragg curve of linear energy transfer (LET) (10–70 keV μm−1) along a 138 μm range of the proton beam, and a declining LET of the carbon beam (900–100 keV μm−1) within 4 μm range. Based on the biophysical models for relative biological effectiveness (RBE) of protons, TOPAS RBE scorers presents a set of depth-variation curves of the proton RBE (for V79 and DU145 cells), linearly related to the Bragg curve of the proton LET. Based on the microdosimetric-kinetic (MK) theory, in the 4 μm range for a monolayer cell thickness, the mean RBEα (V79 cells) of the carbon ion beam is estimated as 3.612 (late S phase) and 1.737 (G1/S phase) for the mean LET of 492 keV μm−1. For practical irradiations, a tunable proton RBE can be acquired by changing the thickness of the cell dish. For the low-energy high-fluence (rate) beams, indirect beam measurements are proposed to detect the proton-beam induced scattering/recoil protons from a beam-intercepting Mylar film, and the carbon-beam induced backscattered electrons from a gold-deposited Havar-foil beam window. Statistical dosimetry for the indirect measurement is established, using a Bayesian model based on the preset number of detection counts, by which the mean value of the whole-dish dose can be prescribed and the uncertainty introduced in the survival data can be corrected.
Design of a Compact Beam Transport System for Laser-Driven Proton Therapy

Abstract:Abstract We put forward a new design of a compact beam transport system for intense laser-driven proton therapy, where instead of using conventional pulsed solenoids, our design relies on a helical coil irradiated by a nanosecond laser pulse to generate strong magnetic fields for focusing protons. A pair of dipole magnets and apertures are employed to further filter protons with large divergences and low energies. Our numerical studies combine particle-in-cell simulations for laser-plasma interaction to generate high-energy monoenergetic proton beams, finite element analysis for evaluating the magnetic field distribution inside the coil, and Monte-Carlo simulations for beam transport and energy deposition. Our results show that with this design, a spread-out Bragg peak in a range of several centimeters to a deep-seated tumor with a dose of approximately 16.5 cGy and fluctuation around 2% can be achieved. The instantaneous dose rate reaches up to 109 Gy/s, holding the potential for future FLASH radiotherapy research.

Abstract:The Accelerator-based Boron Neutron Capture Therapy (AB-BNCT) is being established worldwide as a future modality to start an era of in-hospital facilities. The most popular reaction for AB-BNCT is 7Li(p, n)7Be and high-flux neutron beams can be produced by bombarding lithium targets with low-energy proton beams. We chose the combined acceleration structure of Radio Frequency Quadrupole linac (RFQ) and Cross-bar H-mode DTL (CH-DTL) considering the compactness of the structure and the adjustment of output energy. The CW proton beam could be accelerated to 1.8 MeV by one RFQ cavity, and then to the final energy by one CH-DTL cavity. The output beam energy can be adjusted in the range of 2.2 MeV to 3.0 MeV with small beam loss and high beam quality, which could be achieved by controlling the feed power and RF phase of the CH-DTL. The variation of beam energy can meet requirements of the BNCT in treating tumors of different depths without adjusting structure of beam shaping assembly (BSA). The beam dynamics of the RFQ and DTL were completed to meet all requirements and the energy stability and adjustment method of output beam were investigated. In addition, we also performed the start-to-end beam tracking and error sensitivity analysis at last.

Abstract:An S-band high-gradient accelerating structure is designed for a proton therapy linear accelerator (linac) to accommodate the new development of compact, single-room facilities and ultra-high dose rate (FLASH) radiotherapy. To optimize the design, an efficient optimization scheme is applied to improve the simulation efficiency. An S-band accelerating structure with 2856 MHz is designed with a low beta of 0.38, which is a difficult structure to achieve for a linac accelerating proton particles from 70 to 250 MeV, as a high gradient up to 50 MV/m is required. A special design involving a dual-feed coupler eliminates the dipole field effect. This paper presents all the details pertaining to the design, fabrication, and cold test results of the S-band high-gradient accelerating structure.

Abstract:A first fully domestically Accelerator-based boron neutron capture therapy demonstration device(AB-BNCT) located in Yangtze River Delta region is being build recently. The project is held by "Xi'an Jiaotong University-Huzhou Neutron Science Laboratory" joint lab established by Xi'an Jiaotong University and Huzhou Industrial Group. The AB-BNCT facility requires a proton beam of 30 mA@40 keV at RFQ entrance, with its beam duty cycle between 0.5%-100%, and its normalized root mean square emittance less than 0.2 pi .mm.mrad. Ion source group of Peking University (PKU) is in charge of the proton ion source and its low energy beam transportation section (LEBT). A PKU type compact permanent magnet 2.45 GHz ECR ion source (PKU-Type PMECR) and a two solenoid LEBT is under development for this purpose. So far, the PMECR ion source conditioning is complete and all parameters are better than required for this facility. The details will be presented in the present paper.

Abstract:Cyclotrons capable of accelerating protons up to about 20 MeV have been worldwide installed. Although their purpose is mainly positron emission tomography (PET) radioisotope production, they are equipped with several beam lines suitable for scientific research. Each beam line may typically deliver proton currents up to 150 $\mu {\bf A}\,\,(10^{15}{\rm particles/s})$. Radiobiological and radiophysiological studies using of these beam lines may contribute to further improve proton therapy results, namely by giving input to pertinent scientific questions, with some of these questions analyzed here. Once the rationale for radiobiological and radiophysiological studies is substantiated, we investigate the viability of building an experimental setup adjusted to one beam line of a proton cyclotron recently installed at the University of Coimbra (a Cyclone®18/9 HC, from IBA - Ion Beam Applications, S.A.). First GEANT4-based Monte-Carlo simulations are compared with well-estabished ion simulation codes. The precision of these results, as well as that of other published experimental data, points to experimental challenges to be addressed in order to thrive towards a high-precision dosimetric setup. Such high-precision is required by law for radioherapeutical applications, which we would like to mimic experimentally so that potential future results have impact on the medical physics community. We finally show a setup scheme, adjustable to one beamline of the cyclotron, capable of equally providing very low (1 cGy) and high dosage (100 Gy).
Stable radiation field positron acceleration in a micro-tube

Abstract:Nowadays, there is a desperate need for an ultra-acceleration-gradient method for antimatter particles, which holds great significance in exploring the origin of matter, CP violation, astrophysics, and medical physics. Compared to traditional accelerators with low gradients and a limited acceleration region for positrons in laser-driven charge separation fields, we propose an innovative high-gradient positron acceleration mechanism with implementation advantages. Injecting a relativistic electron beam into a dense plasma micro-tube generates a stable and periodic high-intensity mid-infrared radiation (mid-IR) field, reaching tens of GV/m. This field, propagating synchronously with the electron beam, achieves a 1 GeV energy gain for the positron bunch within 140 picoseconds with a minimal energy spread-approximately 1.56% during a stable phase. By utilizing continuous mid-IR, the efficiency of energy transfer from the electron beam to either a single positron bunch or three positron bunches simultaneously could reach up to 20% and 40%, respectively. This acceleration scheme can achieve cascaded acceleration for a single positron bunch and series acceleration for multiple positron bunches in a continuous, stable, and efficient manner.

Abstract:Recent advances in laser technology have made proton (light ion) acceleration possible using laser-induced plasmas. In this work, we report our work for the last few years on the investigation of a new proton therapy system for radiation oncology, which employs laser-accelerated protons. If successfully developed, the new system will be compact, cost-effective, and capable of delivering energy-and intensity-modulated proton therapy (EIMPT). We have focused our research on three major aspects: (1) target design for laser-proton acceleration, (2) system design for particle/energy selection and beam collimation, and (3) dosimetric studies on the use of laser-accelerated protons for cancer therapy. We have performed particle-in-cell (PIC) simulations to investigate optimal target configurations for proton/ion acceleration. We also performed Monte Carlo simulations to study the beam characteristics and the feasibility of using such beams for cancer treatment. Since laser-accelerated protons have broad energy and angular distributions, which are not suitable for radiotherapy applications directly, we have designed a compact particle selection and beam collimating system for EIMPT beam delivery. We also proposed a new gantry design to make the whole system compact to retrofit existing linac vaults. We have compared Monte Carlo calculated dose distributions using X-ray IMRT and laser-proton EIMPT. Our results show that EIMPT using laser protons produces superior target coverage and much reduced critical structure dose and integral dose compared to X-ray IMRT.

Abstract:Background Ultra‐high dose rate (UHDR/FLASH) irradiations, along with particle minibeam therapy (PMBT) are both emerging as promising alternatives to current radiotherapy techniques thanks to their improved healthy tissue sparing and similar tumor control. Purpose Monte Carlo (MC) modeling of a commercial machine delivering 5–7 MeV electrons at UHDR. This model was used afterward to compare measurements against simulations for an experimental setup combining both FLASH and PMBT modalities. Methods We modeled the main accelerator elements with TOPAS3.8/Geant4.10.07.p03, optimized the electron source parameters, and subsequently benchmarked this geometry against measurements. Minibeam experiments were performed by delivering 7 MeV electrons at UHDR on three different 65‐mm thick brass collimators as manufactured for protons with a 400‐μm slit width: single slit, 5 slits with a center‐to‐center (CTC) distance of 4 mm and 9 slits with CTC of 2 mm. Finally, complementary simulations were run by changing critical PMBT collimator parameters to assess their specific impact on peak‐to‐valley dose ratio (PVDR) as well as on the Bremsstrahlung photon contribution to the total dose. Results Percentage depth dose (PDD) distributions and lateral dose profiles showed a good agreement between simulations and measurements, with a maximum discrepancy of less than 4%. With the PMBT collimators in place, discrepancies between simulated and measured dose profiles, lateral and in‐depth in peaks and valleys, were within 3%. High PVDR between 5 and 26 were observed until 4 mm in the phantom. During the experiments, a mean dose rate of 167 Gy/s and an instantaneous dose rate of 1.2 × 105 Gy/s were obtained for the FLASH‐minibeam setup. PMBT collimator parameters need to be optimized to maximize PVDR while limiting Bremsstrahlung photon contribution to the total dose. Conclusions The validation of the MC model and the configuration of an electron FLASH‐minibeam setup were successfully completed, paving the way for future radiobiological investigations.

Abstract:Background: Proton minibeam radiation therapy (pMBRT) can deliver spatially fractionated dose distributions with submillimeter resolution. These dose distributions exhibit significant heterogeneity in both depth and lateral directions. Accurate characterization of pMBRT doses requires dosimetry devices with high spatial resolution and a wide dynamic range. Furthermore, the dependency of dosimetric measurements on Linear Energy Transfer (LET), as observed in conventional proton therapy, is also present in pMBRT depth dose measurements. Purpose: This work demonstrates the process of performing comprehensive dosimetric measurements to characterize the pMBRT collimator on a clinical single-gantry proton machine, utilizing commercially available dosimetry devices. Methods: The minibeam collimator is designed to be mounted on the clinical nozzle as a beam-modifying accessory. Three collimators, each with a slit opening of 0.4 mm, are thoroughly evaluated. The center-to-center (c-t-c) distances of the slits for these collimators are 2.8 mm, 3.2 mm, and 4.0 mm, respectively. High spatial resolution dosimetry devices are essential for PMBRT dose characterizations. To meet this requirement, two-dimensional (2D) dose measurement devices, Gafchromic films, are used to measure lateral profiles at various depths. Films are also used for depth dose profile measurements in solid water. Additionally, high-resolution point dose detectors, microDiamond, and Razor diode detectors are employed for lateral profile measurements at various depths. Percent depth dose (PDD) measurements of pMBRT in solid water, with various proton energies, collimators, and air gaps, are performed using Gafchromic films. The film's LET dependency for proton beams is corrected to ensure accurate pMBRT PDD measurements. The Monte Carlo simulation tool TOPAS is utilized to compare and validate all experimental measurements. Results: At depths where LET is not a concern, film dose measurements were consistent with microDiamond and Razor diode point measurements. The point detectors need to be orientated with the thin side aligned to the incoming beam. Comparison of the lateral dose profiles extracted from TOPAS simulations, films, microDiamond, and Razor diode detectors shows a passing rate exceeding 98% in 1D gamma analysis at 3% 0.1 mm criteria.However, when the microDiamond detector is orientated to face the pMBRT beam, its spatial resolution may not be sufficient to capture the peak and valley dose accurately. Nevertheless, an accuracy within 2% can still be achieved when comparing the average dose. The PDD measurements show that the peak valley dose ratio (PVDR) of pMBRT can be altered at different depths with different air gaps using the same collimator or different collimators of different c-t-c distances. Conclusion: Our study demonstrates that comprehensive dose measurements for pMBRT can be conducted using standard clinical dose measurement devices. These measurements are indispensable for guiding and ensuring accurate dose reporting in pre-clinical studies using the pMBRT technique.

Abstract:Background and purpose: Microbeam radiotherapy (MRT) is a preclinical concept in radiation oncology with arrays of alternating micrometer-wide high-dose peaks and low-dose valleys. Experiments demonstrated a superior normal tissue sparing at similar tumor control rates with MRT compared to conventional radiotherapy. Possible clinical applications are currently limited to large third-generation synchrotrons. Here, we investigated the line-focus X-ray tube as an alternative microbeam source. Materials and methods: We developed a concept for a high-voltage supply and an electron source. In Monte Carlo simulations, we assessed the influence of X-ray spectrum, focal spot size, electron incidence angle, and photon emission angle on the microbeam dose distribution. We further assessed the dose distribution of microbeam arc therapy and suggested to interpret this complex dose distribution by equivalent uniform dose. Results: An adapted modular multi-level converter can supply high-voltage powers in the megawatt range for a few seconds. The electron source with a thermionic cathode and a quadrupole can generate an eccentric, high-power electron beam of several 100 keV energy. Highest dose rates and peak-to-valley dose ratios (PVDRs) were achieved for an electron beam impinging perpendicular onto the target surface and a focal spot smaller than the microbeam cross-section. The line-focus X-ray tube simulations demonstrated PVDRs above 20. Conclusion: The line-focus X-ray tube is a suitable compact source for clinical MRT. We demonstrated its technical feasibility based on state-of-the-art high-voltage and electron-beam technology. Microbeam arc therapy is an effective concept to increase the target-to-entrance dose ratio of orthovoltage microbeams.

Abstract:The design and optimization of laser-Compton x-ray systems based on compact distributed charge accelerator structures can enable micron-scale imaging of disease and the concomitant production of beams of Very High Energy Electrons (VHEEs) capable of producing FLASH-relevant dose rates. The physics of laser-Compton x-ray scattering ensures that the scattered x-rays follow exactly the trajectory of the incident electrons, thus providing a route to image-guided, VHEE FLASH radiotherapy. The keys to a compact architecture capable of producing both laser-Compton x-rays and VHEEs are the use of X-band RF accelerator structures which have been demonstrated to operate with over 100 MeV/m acceleration gradients. The operation of these structures in a distributed charge mode in which each radiofrequency (RF) cycle of the drive RF pulse is filled with a low-charge, high-brightness electron bunch is enabled by the illumination of a high-brightness photogun with a train of UV laser pulses synchronized to the frequency of the underlying accelerator system. The UV pulse trains are created by a patented pulse synthesis approach which utilizes the RF clock of the accelerator to phase and amplitude modulate a narrow band continuous wave (CW) seed laser. In this way it is possible to produce up to 10 {\mu}A of average beam current from the accelerator. Such high current from a compact accelerator enables production of sufficient x-rays via laser-Compton scattering for clinical imaging and does so from a machine of "clinical" footprint. At the same time, the production of 1000 or greater individual micro-bunches per RF pulse enables > 10 nC of charge to be produced in a macrobunch of < 100 ns. The design, construction, and test of the 100-MeV class prototype system in Irvine, CA is also presented.

Abstract:Superconducting (SC) radio-frequency quadrupoles (RFQs) have exhibited outstanding performance in transmitting and accelerating high-current continuous-wave (CW) ion beams. They can complete beam acceleration at a much higher gradient and with much lower power consumption compared with normal conducting (NC) RFQs. In this study, we introduce a novel SC RFQ scheme operating at 162.5 MHz to accelerate 10 mA proton beams from 30 keV to 2.5 MeV. It will be used as a crucial component for a neutron source dedicated to Boron Neutron Capture Therapy (BNCT) and neutron imaging projects. For efficient transmission of proton beams, we selected a relatively high inter-vane voltage of 240 kV, and the beam dynamics design yielded satisfactory results. Subsequently, RF design and multi-physics analysis were carried out to validate the reliability of the design. A 30-centimeter-long cavity was specifically designed for the vertical test and allowed for a thorough evaluation of the performance of the SC RFQ after post-treatments. Additionally, the tuning design of the 30 cm cavity was also carried out.
