Abstract:Amblyopia causes a contrast sensitivity deficit in the amblyopic eye. However there is also evidence of additional deficits affecting functions further along the processing stream, e.g. an impairment in global motion processing that is not specific to the amblyopic eye. Perceptual learning studies on normal observers have demonstrated significant improvements in global motion tasks. On this basis, we have applied these methods in an attempt to: i) improve global motion processing in amblyopic observers, and ii) determine whether the learning effects are specific to the trained eye. We tested 5 normals and 6 amblyopes on a motion direction discrimination task. Our stimuli were a field of isotropic log-Gabors with peak spatial frequency of 3 c/deg (spatially band-pass "dots"). In each trial we first presented a stimulus with a fixed reference motion direction, and then a test stimulus with its motion direction defined as an offset from that reference. The observer responded whether the second interval's direction was clockwise or anti-clockwise relative to the first. The difficulty of the task was varied by modifying the offset angle. We measured monocular baseline thresholds for each eye (day 1), and then conducted 10 days of monocular training for 40 minutes/day (days 2-11). Half of the amblyopes trained with their amblyopic eye, half with their fellow eye. After training we then made two retest measurements for each eye (days 12 and 13). Surprisingly, we do not find the expected training effect in either our normal or our amblyopic observers. Thresholds were generally lower following training, however this difference is not statistically significant. It is possible that the critical difference between our study and those that have found large training effects is the spatially broadband nature of the stimuli used in previous studies. Perceptual Learning Improves Adult Amblyopic Vision Through Rule-Based Cognitive Compensation

Abstract:PURPOSE. We investigated whether perceptual learning in adults with amblyopia could be enabled to transfer completely to an orthogonal orientation, which would suggest that amblyopic perceptual learning results mainly from high-level cognitive compensation, rather than plasticity in the amblyopic early visual brain.METHODS. Nineteen adults (mean age = 22.5 years) with anisometropic and/or strabismic amblyopia were trained following a training-plus-exposure (TPE) protocol. The amblyopic eyes practiced contrast, orientation, or Vernier discrimination at one orientation for six to eight sessions. Then the amblyopic or nonamblyopic eyes were exposed to an orthogonal orientation via practicing an irrelevant task. Training was first performed at a lower spatial frequency (SF), then at a higher SF near the cutoff frequency of the amblyopic eye.RESULTS. Perceptual learning was initially orientation specific. However, after exposure to the orthogonal orientation, learning transferred to an orthogonal orientation completely. Reversing the exposure and training order failed to produce transfer. Initial lower SF training led to broad improvement of contrast sensitivity, and later higher SF training led to more specific improvement at high SFs. Training improved visual acuity by 1.5 to 1.6 lines (P < 0.001) in the amblyopic eyes with computerized tests and a clinical E acuity chart. It also improved stereoacuity by 53% (P < 0.001).CONCLUSIONS. The complete transfer of learning suggests that perceptual learning in amblyopia may reflect high-level learning of rules for performing a visual discrimination task. These rules are applicable to new orientations to enable learning transfer. Therefore, perceptual learning may improve amblyopic vision mainly through rule-based cognitive compensation.
Predictive factors for the perceptual learning in stereodeficient subjects

Abstract:Background: Stereopsis is a valuable feature of human visual perception which is critically impaired in amblyopia, but can be improved through perceptual learning (PL). This article aims to determine the variables affecting the outcomes and intensity of a stereoacuity stimulation program. Methods: Re-analysis of a previous study in stereodeficient subjects with a history of amblyopia. Sixteen subjects (12 females, 4 males) aged between 7 and 14 received stereopsis stimulation through a PL program at home. A correlation analysis evaluated whether treatment intensity or percentage improvement were related to age or baseline stereoacuity measurements. Further analysis was performed to assess whether the type of amblyopia conditioned the PL treatment (Fischer Statistical Test). Results: No significant correlation was found between age and percentage improvement (rho=-0.08, p=0.749), nor was age correlated with treatment intensity (rho=0.170, p=0.544). However, a correlation did exist between baseline stereoacuity levels and treatment intensity (rho=0.734, p=0.001). Baseline stereoacuity and percentage improvement had a negative correlation (rho=-0.748, p=0.005), while treatment intensity showed only a weak association with the type of amblyopia (p=0.064). Conclusions: Present results suggest that perceptual learning in stereodeficient subjects is not influenced by either the subject's age or the type of amblyopia. Baseline stereoacuity, on the other hand, seems to be a predicting factor for perceptual learning outcomes. According to our study, subjects with poor basal stereoacuity needed more sessions to improve and their percentage improvement was lower. However, due to the reduced size of the sample, the results should be considered with caution.
Visual Cortical Function Changes After Perceptual Learning with Dichoptic Attention Tasks in Adults with Amblyopia: A Case Study Evaluated Using fMRI

Abstract:Background: Amblyopia is a neurodevelopmental disorder of vision, commonly caused by strabismus or anisometropia during early childhood. While studies demonstrated that perceptual learning improves visual acuity and stereopsis in adults with amblyopia, accompanying changes in visual cortical function remain unclear. Methods: We measured functional magnetic resonance imaging (fMRI) responses before and after perceptual learning in seven adults with amblyopia. Our learning tasks involved dichoptic high-attention-demand tasks that avoided V1 function-related tasks and required high-level cortical functions (e.g., intraparietal sulcus) to train the amblyopic eye. Results: Perceptual learning induced low-level visual cortical function changes, which were strongly associated with the etiology of amblyopia and visual function improvements. Anisometropic amblyopes showed functional improvements across all regions of interest (ROIs: V1, V2, V3, V3A, and hV4), along with improvements in visual acuity and stereoacuity. In contrast, strabismic amblyopes showed robust improvements in visual cortical functions only in individuals who experienced significant gains in visual acuity and stereoacuity. Notably, improvements in V1 functions were significantly correlated with the magnitude of visual acuity and stereoacuity improvements when combining both anisometropic and strabismic amblyopes. Conclusions: Our findings provide evidence that learning occurs in both high-level and low-level cortical processes. Our study suggests that early intervention to correct eye alignment (e.g., strabismus surgery) is critical for restoring both visual and cortical functions in strabismic amblyopia.

Dichoptic Training in Adults with Amblyopia: Additional Stereoacuity Gains over Monocular Training

Abstract:Dichoptic training is a recent focus of research on perceptual learning in adults with amblyopia, but whether and how dichoptic training is superior to traditional monocular training is unclear. Here we investigated whether dichoptic training could further boost visual acuity and stereoacuity in monocularly well-trained adult amblyopic participants. During dichoptic training the participants used the amblyopic eye to practice a contrast discrimination task, while a band-filtered noise masker was simultaneously presented in the non-amblyopic fellow eye. Dichoptic learning was indexed by the increase of maximal tolerable noise contrast for successful contrast discrimination in the amblyopic eye. The results showed that practice tripled maximal tolerable noise contrast in 13 monocularly well-trained amblyopic participants. Moreover, the training further improved stereoacuity by 27% beyond the 55% gain from previous monocular training, but unchanged visual acuity of the amblyopic eyes. Therefore our dichoptic training method may produce extra gains of stereoacuity, but not visual acuity, in adults with amblyopia after monocular training.
Interocular Shift of Visual Attention Enhances Stereopsis and Visual Acuities of Anisometropic Amblyopes Beyond the Critical Period of Visual Development: A Novel Approach

Abstract:Aims. Increasing evidence shows that imbalanced suppressive drive prior to binocular combination may be the key factor in amblyopia. We described a novel binocular approach, interocular shift of visual attention (ISVA), for treatment of amblyopia in adult patients. Methods. Visual stimuli were presented anaglyphically on a computer screen. A square target resembling Landolt C had 2 openings, one in red and one in cyan color. Through blue-red goggles, each eye could only see one of the two openings. The patient was required to report the location of the opening presented to the amblyopic eye. It started at an opening size of 800 sec of arc, went up and down in 160 sec of arc step, and stopped when reaching the 5th reversals. Ten patients with anisometropic amblyopia older than age 14 (average age: 26.7) were recruited and received ISVA treatment for 6 weeks, with 2 training sessions per day. Results. Both Titmus stereopsis (z=-2.809, P=0.005) and Random-dot stereopsis (z=-2.317, P=0.018) were significantly improved. Average improvement in best corrected visual acuity (BCVA) was 0.74 line (t=5.842, P<0.001). Conclusions. The ISVA treatment may be effective in treating amblyopia and restoring stereoscopic function.
Dichoptic De-Masking Learning in Adults with Amblyopia and Its Mechanisms

Abstract:PURPOSE. Recently, we reported that dichoptic de-masking training can further boost stereoacuity, but not visual acuity, in adults with amblyopia after extensive monocular perceptual training. Here, we investigated whether this dichoptic training targets on interocular suppression directly, or improves vision through high-level brain mechanisms. METHODS. Eleven adults with amblyopia first used amblyopic eyes (AEs) to perform contrast (n = 6) or orientation (n = 5) discrimination training, while resisting dichoptic noise masking from fellow eyes (FEs). Learning was indicated by increased maximal tolerable noise contrast (TNC) for AE contrast/orientation discrimination. After dichoptic training, six observers continued to use AEs to perform monocular training for nine sessions. RESULTS. (1) Training of dichoptic de-masking doubled maximal TNC, but learning did not transfer much to the same task at an orthogonal orientation or a different task, showing orientation/task specificities. (2) Following a training-plus-exposure (TPE) protocol, AEs then received exposure of the orthogonal orientation by performing the other orientation/contrast discrimination task at the orthogonal orientation. After this TPE training, dichoptic learning with the original discrimination task transferred to the orthogonal orientation. (3) Dichoptic training improved AE's acuity (1.2 lines), stereoacuity (60.2%), and contrast sensitivity (mainly at higher spatial frequencies). (4) Additional monocular training did not produce further acuity and stereoacuity gains. CONCLUSIONS. The initial orientation/task specificities exclude the possibility that dichoptic training reduces physiological interocular suppression. The later transfer of learning to an orthogonal orientation with TPE training suggests improvement in high-level brain processing. Dichoptic training may strengthen top-down attention to AEs to counter the impacts of attentional bias to FEs and/or physiological interocular suppression and improve stereoacuity.

Abstract:Background: The ability to merge the two retinal images to perceive depth (stereopsis) plays an important role in human vision. Its proper development requires binocular alignment and good visual acuity in both eyes during childhood. Because treatments are more effective when applied early, early diagnosis is important. Unfortunately, assessing stereo deficiencies in infants and young children remains challenging. Recently, it has been shown that ocular-following responses (OFRs; reflexive, short-latency eye movements induced by the sudden motion of a large textured pattern) are sensitive to changes in interocular correlation, making them potentially useful for stereo deficiency assessments. To test this hypothesis, we measured OFRs elicited by dichoptic stimulation in children with normal and compromised stereopsis (due to amblyopia). Methods: Two groups of six children (age- and sex-matched: 3M/3F aged 7–12 yo), one with compromised stereopsis and one with normal stereopsis, were included. OFRs were recorded using a custom high-resolution video eye-tracking system. The relative differences between eye displacement induced by correlated stimuli (up-correlated–down-correlated) and anticorrelated (up-anticorrelated–down-anticorrelated) were compared. Results: We found significant differences between OFRs induced by two dichoptic conditions (correlated and anticorrelated stimuli) in most children with normal stereopsis, whereas no differences were observed in children with compromised stereopsis, indicating a lack of disparity detectors. Conclusions: OFRs might thus be exploited as a diagnostic tool for the objective identification of stereo deficiencies in children. This might lead to improved early diagnosis and treatment outcomes for conditions like amblyopia and strabismus.

Abstract:The purpose of the experiments described here was to investigate global image processing using methods that require global processing while eliminating or compensating for low level abnormalities: visibility, shape perception and positional uncertainty. In order to accomplish this we used a closed figure made up of Gabor patches either in noise or on a blank field. The stimuli were circular or elliptical contours, formed by N equally spaced Gabor patches. We performed two separate experiments: In one experiment we fixed N and varied the aspect ratio using a staircase to determine the threshold aspect ratio; in the second experiment we held the aspect ratio constant (at twice the threshold aspect ratio) and varied N in order to measure the threshold number of elements required to judge the shape. Our results confirm and extend previous studies showing that humans with naturally occurring amblyopia show deficits in contour processing. Our results show that the deficits depend strongly on spatial scale (target size and spatial frequency). The deficit in global contour processing is substantially greater in noise (where contour-linking is required) than on a blank field. The magnitude of the deficits is modest when low-level deficits (reduced visibility, increased positional uncertainty, and abnormal shape perception) are minimized, and does not seem to depend much on acuity, crowding or stereoacuity. The residual deficits reported here cannot be simply ascribed to reduced visibility or increased positional uncertainty, and we therefore conclude that these are genuine deficits in global contour segregation and integration.

Abstract:Abstract Stereopsis is a valuable feature of human visual perception, which may be impaired or absent in amblyopia and/or strabismus but can be improved through perceptual learning (PL) and videogames. The development of consumer virtual reality (VR) may provide a useful tool for improving stereovision. We report a proof of concept study, especially useful for strabismic patients and/or those with reduced or null stereoacuity. Our novel VR PL strategy is based on a principled approach which included aligning and balancing the perceptual input to the two eyes, dichoptic tasks, exposure to large disparities, scaffolding depth cues and perception for action. We recruited ten adults with normal vision and ten with binocular impairments. Participants played two novel PL games (DartBoard and Halloween) using a VR-HMD. Each game consisted of three depth cue scaffolding conditions, starting with non-binocular and binocular cues to depth and ending with only binocular disparity. All stereo-anomalous participants improved in the game and most (9/10) showed transfer to clinical and psychophysical stereoacuity tests (mean stereoacuity changed from 569 to 296 arc seconds, P < 0.0001). Stereo-normal participants also showed in-game improvement, which transferred to psychophysical tests (mean stereoacuity changed from 23 to a ceiling value of 20 arc seconds, P = 0.001). We conclude that a VR PL approach based on depth cue scaffolding may provide a useful method for improving stereoacuity, and the in-game performance metrics may provide useful insights into principles for effective treatment of stereo anomalies. Abstract:PURPOSE. Dichoptic training is becoming a popular tool in amblyopia treatment. Here we investigated the effects of dichoptic demasking training in children with amblyopia who never received patching treatment (NPT group) or were no longer responsive to patching (PT group).METHODS. Fourteen NPT and thirteen PT amblyopes (6-16.5 years; 24 anisometropic, two strabismus, and one mixed) received dichoptic demasking training for 17 to 22 sessions. They used the amblyopic eye (AE) to practice contrast discrimination between a pair of Gabors that were dichoptically masked by a band-filtered noise pattern simultaneously presented in the fellow eye (FE). Dichoptic learning was quantified by the increase of maximal tolerable noise contrast (TNC) for AE contrast discrimination. Computerized visual acuities and contrast sensitivity functions for both eyes and the Randot stereoacuity were measured before and after training.RESULTS. Training improved maximal TNC by six to eight times in both groups, along with a boost of AE acuities by 0.15 logMAR (P < 0.001) in the NPT group and 0.06 logMAR (P < 0.001) in the PT group. This visual acuity improvement was significantly dependent on the pretraining acuity. Stereoacuity was significantly improved by 41.6% (P = 0.002) in the NPT group and 64.2% (P < 0.001) in the PT group. The stereoacuity gain was correlated to the pretraining interocular acuity difference (r = -0.49, P = 0.010), but not to the interocular acuity difference change (r = -0.28, P = 0.15). Training improved AE contrast sensitivity in the NPT group (P = 0.009) but not the PT group (P = 0.76). Moreover, the learning effects in 12 retested observers were retained for 10 to 24 months.CONCLUSIONS. Dichoptic training can improve, and sometimes even restore, the stereoacuity of amblyopic children, especially those with mild amblyopia (amblyopic VA <= 0.28 logMAR). The dissociation of stereoacuity gain and the interocular acuity difference change suggests that the stereoacuity gain may not result from a reduced interocular suppression in most amblyopes. Rather, the amblyopes may have learned to attend to, or readout, the stimulus information to improve stereopsis.
Complete Transfer of Perceptual Learning Across Retinal Locations Enabled by Double Training

Abstract:Practice improves discrimination of many basic visual features, such as contrast, orientation, and positional offset [1-7]. Perceptual learning of many of these tasks is found to be retinal location specific, in that learning transfers little to an untrained retinal location [1, 6-8]. In most perceptual learning models, this location specificity is interpreted as a pointer to a retinotopic early visual cortical locus of learning [1, 6-11]. Alternatively, an untested hypothesis is that learning could occur in a central site, but it consists of two separate aspects: learning to discriminate a specific stimulus feature ("feature learning"), and learning to deal with stimulus-nonspecific factors like local noise at the stimulus location ("location learning") [12]. Therefore, learning is not transferable to a new location that has never been location trained. To test this hypothesis, we developed a novel double-training paradigm that employed conventional feature training (e.g., contrast) at one location, and additional training with an irrelevant feature/task (e.g., orientation) at a second location, either simultaneously or at a different time. Our results showed that this additional location training enabled a complete transfer of feature learning (e.g., contrast) to the second location. This finding challenges location specificity and its inferred cortical retinotopy as central concepts to many perceptual-learning models and suggests that perceptual learning involves higher nonretinotopic brain areas that enable location transfer.
Effectiveness of binocular therapy as a complementary treatment of part-time patching in older amblyopic children: a randomized clinical trial

Abstract:Purpose: To assess the effectiveness of combined use of stereoscopic 3D video movies and part-time patching in treating older amblyopic children with poor response or compliance to traditional patching treatments and comparing this combined treatment with patching alone. Methods: Thirty-two children aged 5-12 years with amblyopia associated with anisometropia, strabismus, or both were recruited in a randomized clinical trial. Eligible participants were assigned randomly to the combined and patching groups. Here, binocular treatment refers to using the Bangerter filter to blur the fellow eye and subsequently watching a close-up 3D movie with large parallax. The primary outcome was amblyopic eye (AE) best-corrected visual acuity (BCVA) improvement at six weeks. In addition, secondary outcomes included BCVA of AE improvement at three weeks and change of stereoacuity. Results: Of 32 participants, mean (SD) age was 6.63 (1.46) years, and 19 (59%) were female. At 6 weeks, mean (SD) amblyopic eye VA improved by 0.17 ± 0.08 logMAR (2-sided 95% CI, 0.13 to 0.22; F = 57.2, p < 0.01) and 0.05 ± 0.04 logMAR (2-sided 95% CI, 0.05 to 0.09; F = 8.73, p = 0.01) in the combined and patching groups, respectively. The difference was statistically significant (mean difference, 0.13 logMAR [1.3 line]; 95% CI, 0.08-0.17 logMAR [0.8-1.7 lines]; t25 = 5.65, p < 0 .01). After treatment, only the combined group had significantly improved stereoacuity, such as binocular function score (median [interquartile range], 2.30 [2.23 to 2.68] vs. 1.69 [1.60 to 2.30] log arcsec; paired, z = -3.53, p < 0.01), and mean stereoacuity gain was 0.47 log arcsec (± 0.22). Changes in other types of stereoacuity were similar. Conclusion: Our laboratory-based binocular treatment strategy engaged a high level of compliance that led to a substantial gain in visual function after a short period of treatment for older amblyopic children having poor response or compliance to traditional patching treatments. Notably, the improving stereoacuity showed a greater advantage.

Abstract:Objective To evaluate the stereopsis in different types of ametropic amblyopic children.Methods A total of 205 children between 4-8 years old with recovered ametropic amblyopia, including 65 of astigmatic amblyopia and 30 of myopic and 110 of hypermetropic, were involved in the subject. Using Yan's stereogram random dot synptophore stereogram and synoptophore, the distance fusion range, distance stereoacuity, approximationg zero disparity of these children were observed separately. Results In mild and moderate amblyopia there was significant difference in approximationg zero disparity and distance stereoacuity among astigmatic amblyopia and myopic and hypermetropic, within which astigmatic amblyopia the worst and hypermetropic amblyopia the best (P＜0.05). No significant difference was found in the distance fusion range (P＞0.05). Different type of ametropic amblyopic led no significant difference in severe amblyopia (P＞0.05) which caused worse influence to the stereopsis than mild and moderate degree (P ＜0.05). Conclusions Amblyopia decrease children's stereopsis by degrees. Mild and moderate amblyopia, astigmatic amblyopia effect most severely,however hypermetropic effect the least. No significant difference is found among them in severe amblyopia.

Abstract:Amblyopia is characterized by poor visual acuity in the amblyopic eyes (AEs) and degraded stereoacuity. Previously we found that dichoptic de-masking training aiming at reducing the impact of interocular suppression could further boost stereoacuity, but not visual acuity, in adult amblyopes who had received extensive monocular perceptual training (Zhang et al., VSS2013). Here we further investigated whether dichoptic de-masking learning is a low- or high-level process. Eleven new adult amblyopes used AEs to practice contrast or orientation discrimination with two aligned vertical/horizontal Gabors (80% contrast). The stimuli were dichoptically masked by a band-filtered noise masker simultaneously presented in NAEs. A 1-interval 2AFC staircase procedure estimated the maximal tolerable noise contrast (TNC). Training of dichoptic de-masking for contrast discrimination (n=6) doubled maximal TNC, but maximal TNC was unchanged at an orthogonal orientation, showing orientation specificity. AEs were then exposed to the orthogonal orientation through irrelevant orientation discrimination also under dichoptic masking, which improved maximal TNC for contrast discrimination at the orthogonal orientation as much as at the trained orientation. A control experiment confirmed that improved maximal TNC for contrast discrimination did not result from de-masking training for orientation discrimination. (2) Reversely, orientation specificity in dichoptic de-masking learning for orientation discrimination (n=5) was also eliminated by exposure of the transfer orientation through contrast discrimination under dichoptic masking. (3) Training (15-hrs) improved stereoacuity by 61.3%, similar to previous 66% improvements after two-stage monocular and dichoptic training. The complete orientation transfer of dichoptic de-masking learning and the task specificity suggest that dichoptic de-masking learning is mainly a high-level process. The brain may learn to readout orientation or contrast signals from dichoptically presented noise, and this learning is rule-based to allow transfer to untrained orientations. Vernier Perceptual Learning Transfers to Completely Untrained Retinal Locations after Double Training: A "piggybacking'' Effect

Abstract:Perceptual learning, a process in which training improves visual discrimination, is often specific to the trained retinal location, and this location specificity is frequently regarded as an indication of neural plasticity in the retinotopic visual cortex. However, our previous studies have shown that "double training'' enables location-specific perceptual learning, such as Vernier learning, to completely transfer to a new location where an irrelevant task is practiced. Here we show that Vernier learning can be actuated by less location-specific orientation or motion-direction learning to transfer to completely untrained retinal locations. This "piggybacking'' effect occurs even if both tasks are trained at the same retinal location. However, piggybacking does not occur when the Vernier task is paired with a more location-specific contrast-discrimination task. This previously unknown complexity challenges the current understanding of perceptual learning and its specificity/transfer. Orientation and motion-direction learning, but not contrast and Vernier learning, appears to activate a global process that allows learning transfer to untrained locations. Moreover, when paired with orientation or motion-direction learning, Vernier learning may be "piggybacked'' by the activated global process to transfer to other untrained retinal locations. How this task-specific global activation process is achieved is as yet unknown.
Comparison of Three Clinical Stereoscopic Methods for Measuring Binocular Visual Function During Amblyopic Treatment in Unilateral Amblyopia

Abstract:This study aimed to compare the measuring stereopsis results of unilateral amblyopia during amblyopic treatment by employing some of the most widely used clinical tests. Thirty-four individuals with previously untreated unilateral amblyopia, aged 8.4 ± 2.7 years, were included in the study. Monocular (best corrected visual acuity [BCVA] at distance) and binocular (including Titmus, Random-dot, and Frisby stereopsis) visual functions were measured at baseline and 2-month and 6-month visits after synthetical treatment. We found that Titmus stereopsis was always significantly better than Random-dot stereopsis (p < 0.001). Frisby stereopsis was also always significantly better than Random-dot stereopsis (p < 0.001). However, there was no significant difference between Titmus stereopsis and Frisby stereopsis (p = 0.562). However, interestingly, there was no significant difference in the mean improvement of the three stereopsis methods from baseline to the 2-month visit, F = 1.158, p = 0.318. Similarly, a significant difference was also lacking in the mean improvement of the three stereopsis methods from baseline to the 6-month visit, F = 0.302, p = 0.740. We conclude that there will be different results obtained from different stereopsis measuring methods used to measure amblyopia in patients. We recommend performing at least two types of stereoscopic measurements to evaluate each case of amblyopia. However, for observing therapeutic effects, each measurement method has the same performance for clinical results during amblyopic treatment.
Abnormal Bone Quality in Adolescent Idiopathic Scoliosis: A Case-Control Study on 635 Subjects and 269 Normal Controls With Bone Densitometry and Quantitative Ultrasound

Abstract:Study Design. A case-control study comparing bone quality in Adolescent Idiopathic Scoliosis (AIS) with normal controls. Objective. To evaluate bone quality with quantitative ultrasound (QUS) in AIS and normal controls so as to detect any derangement in bone quality among AIS subjects. Summary of Background Data. AIS is characterized by complex spinal deformities. Despite its high prevalence and clinical impact in adolescents, etiology of AIS remains unknown but one possible mechanism is related to derangement of bony mechanical stability, as quantified by bone mineral density (BMD) and bone quality. AIS is known for its association with osteopenia, but little is known about the bone quality in AIS. With technological advancement, QUS can provide objective measurement of bone quality. In this study, we sought to compare bone quality in AIS with normal controls using QUS in addition to the conventional BMD measurement. Methods. Six hundred thirty-five AIS girls and 269 age-matched normal girls were investigated. Broadband ultrasound attenuation (BUA), velocity of sound (VOS), and stiffness index (SI) were measured over the nondominant calcaneus using QUS. The results were correlated with anthropometric measurement, radiologic assessment, and BMD of both hips. Results. The z-score of BMD at the femoral neck of AIS subjects (−0.47 ± 0.97) was significantly lower than that of normal controls (−0.12 ± 1.01, P < 0.001). Crude comparison showed that BUA, VOS, and SI of AIS group were 3.8% (P < 0.01), 0.5% (P = 0.042), and 6.9% (P < 0.01) lower than controls, respectively. After controlling confounding from maturity, body weight, body height, and BMD with multiple linear regression analysis for both mild (Cobb's angle ⩽ 25°) and severe (Cobb's angle > 25°) curves, BUA and SI were found to be statistically significantly lower in AIS as compared with controls (P < 0.05). Conclusion. In addition to higher prevalence of osteopenia, AIS patients were also found to have deranged bone quality. These might contribute to the etiopathogenesis of spinal deformities in AIS.

Abstract:Abstract Long-term and chronic visual suppression to the non-preferred eye in early childhood is a key factor in developing amblyopia, as well as a critical barrier to treat amblyopia. To explore the relationship between selective visual attention and amblyopic suppression and its role in the success of amblyopic training, we used EEG source-imaging to show that training human adults with strabismic and anisometropic amblyopia with dichoptic attention tasks improved attentional modulation of neural populations in the primary visual cortex (V1) and intraparietal sulcus (IPS). We also used psychophysics to show that training reduced interocular suppression along with visual acuity and stereoacuity improvements. Importantly, our results revealed that the reduction of interocular suppression by training was significantly correlated with the improvement of selective visual attention in both training-related and -unrelated tasks in the amblyopic eye, relative to the fellow eye. These findings suggest a relation between interocular suppression and selective visual attention bias between eyes in amblyopic vision, and that dichoptic training with high-attention demand tasks in the amblyopic eye might be an effective way to treat amblyopia.
