Roentgenograms of the skull revealed no calcification. However, a CAT scan showed densities in the region of the basal ganglia bilaterally that were believed to represent calcium deposits (Figure).

Abstract:BACKGROUND:To establish a stable ex vivo lung perfusion (EVLP) model and determine the preservation effect of the EVLP technique on donor lungs in vitro.METHODS:EVLP was performed on nonacceptable human donor and porcine lungs, and during perfusion, the oxygenation index was assessed and blood gas analysis was performed.RESULTS:After 4 h of EVLP of nonacceptable human donor and porcine lungs, lung function parameters remained stable, and lung energy metabolism was improved to a certain extent.CONCLUSIONS:EVLP can suitably maintain donor lungs in vitro for transplantation and is an effective method for ex vivo preservation of donor lungs.

Abstract:With the ongoing shortage of donor lungs, ex vivo lung perfusion (EVLP) offers the opportunity for objective assessment and potential therapeutic repair of marginal organs. There is a need for robust research on EVLP interventions to increase the number of transplantable organs. The use of human lungs, which have been declined for transplant, for these studies is preferable to animal organs and is indeed essential if clinical translation is to be achieved. However, experimental human EVLP is time-consuming and expensive, limiting the rate at which promising interventions can be assessed. A split-lung EVLP model, which allows stable perfusion and ventilation of two single lungs from the same donor, offers advantages scientifically, financially and in time to yield results. Identical parallel circuits allow one to receive an intervention and the other to act as a control, removing inter-donor variation between study groups. Continuous hemodynamic and airway parameters are recorded and blood gas, perfusate, and tissue sampling are facilitated. Pulmonary edema is assessed directly using ultrasound, and indirectly using the lung tissue wet:dry ratio. Evans blue dye leaks into the tissue and can quantify vascular endothelial permeability. The split-lung ex vivo perfusion model offers a cost-effective, reliable platform for testing therapeutic interventions with relatively small sample sizes.

Cost-effective Donor Lung Preservation with High-Volume Continuous Perfusate Purification in Ex Vivo Lung Perfusion

Abstract:Background: Ex vivo lung perfusion is an ideal platform for donor lung evaluation and preservation.Its application in Asia was limited due to the high running costs and potential lung reperfusion injury.Previous research showed that perfusate purification could maintain perfusate homeostasis during ex vivo lung perfusion and improve the perfuse outcome.However, the optimal purification rate remains unclear.Methods: A home-designed EVLP system (LuPS-1) which provides the capability of continuous perfusate purification (CPP), was used in this study.Seventeen human donor lungs experienced extended cold ischemia preservation and were perfused by the LuPS-1 system for 12 hours.Five of them were set as the control group.The rest 12 lungs were separated into the low-volume CPP group (LV-CPP, n=6) and the high-volume CPP group (HV-CPP, n=6), the hyperfiltration and replacement volume targets were set at 500 mL/hr for the LV group, and 1500 mL/hr for the HV group.Perfusate electrolyte and lactate profiles were analyzed.Lung function parameters, radiographic, histologic assessment, and weight gain were used for donor graft evaluation.Cell apoptosis and tight junction protein levels in the donor graft were investigated.Results: All the donor lungs from the two CPP groups and two lungs from the control group accomplished 12 hours of EVLP.HV-CPP group showed stable perfusate electrolyte, lactate profile, and significantly decreased lung weight gain compared with the other two groups.High-volume CPP reduced lung reperfusion injury characterized by significantly reduced cell apoptosis and lower microvascular permeability.Conclusions: High volume CPP improved donor lung function and reduced lung injury during EVLP.Meanwhile, it significantly reduced running costs.

Abstract:Background: Ex vivo lung perfusion (EVLP) is a relatively new technique that can be used to assess and repair the donor lungs, increasing the utilization of high-risk lungs. However, its effect on outcomes of lung transplantation patients is uncertainty. This meta-analysis is conducted to assess the impact of EVLP on donor lungs and outcomes of recipients compared with the standard lung transplantation. Material/Methods: We systematically searched for studies comparatively analyzing the efficacy of EVLP and standard cold storage in lung transplantation. The hazard ratio (HR), relative risk (RR), and weighted mean difference (WMD) were used as the effect size (ES) to evaluate the survival outcomes, categorical variables, and continuous variables respectively. Results: A total of 20 published articles (including 2574 donors and 2567 recipients) were eligible. The chest x-ray manifestations and PaO2/FiO(2) 100% were more deficient in the EVLP group than the standard group. EVLP improved the function of high-risk donor lungs with the conversion rate ranging from 34% to 100%. The EVLP group had a lower incidence of primary graft dysfunction 3, but longer intensive care unit stay. Other clinical outcomes between the 2 groups were similar. Conclusions: The pooled results indicated that EVLP could be used to assess and improve high-risk donor lungs and had non-inferior postoperative outcomes compared with the standard cold storage. EVLP not only increased the utilization of marginal donors, but also could extend preservation time and reduce the total ischemia time of donors.

Abstract:Objective To evaluate pulmonary vascular permeability by using an improved situ-isolated lung perfusion rat model.Methods Based on traditional rat isolated lung perfusion methods,we established the improved situ-isolated lung perfusion rat model and Lipopolysaccharide(LPS)induced lung injury model.The content of Evans blue(EB)in lung was tested and the albumin permeability-surface area(PS)was caculated to quantitatively evaluate pulmonary vascular permeability.Results PS values of normal control and lung injury group were 5.37±0.94,25.00±8.8×10-2 cm3(/min.g),respectively.Conclusion The present evaluation method of pulmonary vascular permeability is simple,accurate positioning and reliable.

Perfusate Exchange Does Not Improve Outcomes in 24-hour Ex Situ Lung Perfusion

Abstract:Background: Reliable 24-hour preservation is required to optimize the rehabilitation potential of Ex Situ Lung Perfusion (ESLP). Other ESLP protocols include fresh perfusate replacement to counteract an accumulation of deleterious by-products. We describe the results of our reliable 24-hour negative pressure ventilation (NPV)-ESLP protocol with satisfactory acute post-transplant outcomes and investigate perfusate exchange (PE) as a modification to enhance prolonged ESLP. Methods: Twelve pig lungs underwent 24 hours of NPV-ESLP using 1.5L of cellular perfusate (500 mL packed red blood cells and 1 L buffered perfusate). The Control (n = 6) had no PE; the PE (n = 6) had 500 mL replaced after 12 hours of NPV-ESLP with 1000 mL fresh perfusate. Three left lungs per group were transplanted. Results: Results are reported as Control vs PE (mean ± SEM). Both groups demonstrated stable and acceptable oxygenation during 24 hours of ESLP with final PF ratios of 527.5 ± 42.19 and 488.4 ± 35.38 (P = .25). Final compliance measurements were 20.52 ± 3.59 and 18.55 ± 2.91 (P = .34). There were no significant differences in pulmonary artery pressure after 24 hours of ESLP (10.02 ± 2.69 vs 14.34 ± 1.64, P = .10), and pulmonary vascular resistance only differed significantly at T12 (417.6 ± 53.06 vs 685.4 ± 81.19, P = .02). Percentage weight gain between groups was similar (24.32 ± 8.4 and 45.33 ± 7.76, P = .07). Post-transplant left lung oxygenation was excellent (327.3 ± 14.62 and 313.3 ± 15.38, P = .28). There was no significant difference in % weight gain of lungs post-transplant (22.20 ± 7.22 vs 14.36 ± 9.96, P = .28). Conclusion: Acceptable lung function was maintained during 24-hour NPV-ESLP and post-transplant regardless of PE.
Biochemical and morphological changes in the liver during isolated liver perfusion with double bypass using automatic blood pumps

Abstract:Isolated organ perfusion is used in clinical practice for chemotherapy in adults with malignant tumors. However, it has not been performed in children because of the size mismatch with the adult circuits. The authors have previously studied isolated liver perfusion in small animals using the self-regulating extracorporeal membrane oxygenation circuit. The present study was designed to investigate the biochemical and morphological changes in the liver during isolated liver perfusion with double bypass using automatic blood pumps. Isolated liver perfusion was performed with bypass between the hepatic and portal veins in seven weanling Yorkshire swine weighing 8.2 to 12.2 kg, at a flow rate of 20 mL/min/kg for up to 4 hours. Venous blood from the intestine and lower body was bypassed to the superior vena cava. As a result, perfusate glutamic pyruvic transaminase and lactate concentrations did not change during liver perfusion. On gross inspection, the surface of the liver was mottled. Microscopically, normal histology of the hepatic parenchyma and portal tract structures was preserved. Transmission electron microscopy showed no gross structural abnormalities in most of the hepatocytes for up to 4 hours. However, swelling of the mitochondria and smooth endoplasmic reticulum was seen occasionally in a very small number of the hepatocytes after more than 3 hours of perfusion. Glycogen granules decreased with time in some animals. Isolated liver perfusion at 20 mL/min/kg of perfusion flow can be performed safely for up to 4 hours with nearly intact hepatocellular function and morphology.

Abstract:BACKGROUND:Minimally invasive cardiac surgery (MICS) is increasingly performed due to faster recovery time and lower postoperative complications when compared with the traditional open surgery. However, hypoxemia in lung isolation duration after cardiopulmonary bypass (CPB) surgery has been the focus of anesthesiologists' attention. In the present study, we designed a novel lung isolation strategy to improve oxygenation using a bronchial blocker (BB) to isolate the right middle and lower lobes and preserve the ventilated right upper lobe without affecting the surgical field.METHODS:Patients who had undergone right lateral mini-thoracotomy, a MICS, between August 2018 and February 2019, were enrolled in this randomized controlled study. Patients were randomly divided into a modified lung isolation group (group M) and a conventional lung isolation group (group C). In group M, BBs were used to block the bronchus intermedius, while left-sided double lumen endotracheal tubes were used in group C to isolate the right lung. The primary outcome was to determine the number of patients who required an increase in ventilation volume due to hypoxemia during lung isolation after CPB.RESULTS:Sixty-one patients (30 in group C and 31 in group M) were enrolled. Five patients in group M were converted to right lung isolation due to poor surgical field exposure. During lung isolation after CPB, the number of patients with hypoxemia was lower in group M than group C (5/31 vs. 15/30, P=0.005).CONCLUSIONS:The novel modified lung isolation strategy reduced the incidence of hypoxemia after CPB.
Technical Advances Targeting Multiday Preservation of Isolated Ex Vivo Lung Perfusion

Abstract:Indications for ex vivo lung perfusion (EVLP) have evolved from assessment of questionable donor lungs to treatment of some pathologies and the logistics. Yet up to 3 quarters of donor lungs remain discarded across the globe. Multiday preservation of discarded human lungs on EVLP platforms would improve donor lung utilization rates via application of sophisticated treatment modalities, which could eventually result in zero waitlist mortality. The purpose of this article is to summarize advances made on the technical aspects of the protocols in achieving a stable multiday preservation of isolated EVLP. Based on the evidence derived from large animal and/or human studies, the following advances have been considered important in achieving this goal: ability to reposition donor lungs during EVLP; perfusate adsorption/filtration modalities; perfusate enrichment with plasma and/or donor whole blood, nutrients, vitamins, and amino acids; low-flow, pulsatile, and subnormothermic perfusion; positive outflow pressure; injury specific personalized ventilation strategies; and negative pressure ventilation. Combination of some of these advances in an automatized EVLP device capable of managing perfusate biochemistry and ventilation would likely speed up the processes of achieving multiday preservation of isolated EVLP.

Abstract:ObjectiveTo investigate the feasibility and isolation efficiency of percutaneous selective isolated hepatic perfusion chemotherapy(PSIHP). MethodsSix pigs underwent the procedure of routine transhepatic arterial infusion(HAI) and 6 underwent PSIHP.5-FU was used in this study. The drug(5-FU) (concentration) of blood from hepatic and systemic veins of both groups was observed. Liver tissue was (investigated) for pathologic changes. ResultsThe peak level of 5-FU concentration in blood from right (hepatic) vein and systemic vein in HAI group was(4082.530415.213)mg/L and (1682.230216.834)mg/L respectively.In PSIHP group, the peak level(5-FU) was(5321.711517.318)mg/L and(510.83452.518)mg/L, respectively.There was a statistically significant difference between HAI group an PSIHP group(P0.01). The level of 5-FU concentration in blood from right hepatic vein of PSIHP group(postfiltration) was(1192.063114.864)mg/L and the filtration rate of charcoal (hemoperfusion) was(77.60.9%). The damage of perfused area of liver tissue was more serious in PSIHP group compared with HAI group. ConclusionsPSIHP is a simple and feasible method for isolated hepatic perfusion. Compared with HAI, PSIHP can increase the concentration level of chemotherapeutic agents in (regional) liver, and can even more markedly decrease the drug concentration in systemic circulation and (decrease) toxic side effcts.

Abstract:OBJECTIVE To establish and modify of the isolated lung transplantation and reperfusion model in rats with a relatively less-invasive way.METHODS Angiocatheters were inserted into femoral artery and jugular vein of the recipient rat to establish the controlled cross-circulatory.The venous cannula was connected to the pulmonary artery of the ex vivo lung block after preservation and cold storage to provide inflow,and Pulmonary effluent blood from the lung block was collected and returned to the support rat via the femoral artery for detoxification and deoxygenation.Each support rat was used to perfuse one lung block for at least one hour.The inflow sample to each lung block was analyzed for pH,pO2,pCO2,and Hb to follow alterations in support rat physiology.RESULTS There were no statistical differences in pH,PvO2,PvCO2 or Hb(P>0.05).CONCLUSION The support rat can deliver constant systemic venous blood at stable physiologic parameters to the ex vivo lung block and this animal model can be used in research of lung transplantation and ischemic-reperfusion injury.
Study on the effects of percutaneaus selective isolated hepatic perfusion with new double balloon catheter

Abstract:Objective To investigate the isolated effects on the percutaneous selective isolated hepatic perfusion chemotherapy with self-made double balloon catheted. Methods From July 2005 to March 2004,12 adult pigs were randomly divided into 2 groups:The group HAI of 6 pigs underwent routine trans-hepatic artery perfusion chemotherapy, and group PSIHP of 6 pigs underwent percutaneous selective isolated hepatic perfusion chemotherapy combined with blood perfusion used self-made double balloon catherter through interventional radiologic method. 5 Fu were selected as chemotherapeutic drug. Peak value of blood-drug concentration was detected in portal vein and peripheral blood respectively. And effect on balloon isolated liver was understood. Results In the group HAI,peak value of blood-drug concentration of portal vein and peripheral vein were 4658.420±433.204mg/L and 1676.140±226.93mg/L respectively. In the group PSIHP, they were 5321.711±517.318mg/L and 65.735±6.425mg/L respectively. The pesk value of peripheral venous blood-drug concentration had significant difference between two groups(P0.01). Conclusion Self-made new double ballon catheter can perform effective isolated hepatic perfusion and ti is a desirable balloon catherter to isolate hepatic perfusion chemotherapy.
Cost effectiveness of commercial portable ex vivo lung perfusion at a low‐volume US lung transplant center

Abstract:This study found that commercial portable ex‐vivo lung perfusion is not cost effective at low‐volume U.S. lung transplant centers. Background Portable ex vivo lung perfusion during lung transplantation is a resource‐intensive technology. In light of its increasing use, we evaluated the cost‐effectiveness of ex vivo lung perfusion at a low‐volume lung transplant center in the USA. Methods Patients listed for lung transplantation (2015–2021) in the United Network for Organ Sharing database were included. Quality‐of‐life was approximated by Karnofsky Performance Status scores 1‐year post‐transplant. Total transplantation encounter and 1‐year follow‐up costs accrued by our academic center for patients listed from 2018 to 2021 were obtained. Cost‐effectiveness was calculated by evaluating the number of patients attaining various Karnofsky scores relative to cost. Results Of the 13 930 adult patients who underwent lung transplant in the United Network for Organ Sharing database, 13 477 (96.7%) used static cold storage and 453 (3.3%) used ex vivo lung perfusion, compared to 30/58 (51.7%) and 28/58 (48.3%), respectively, at our center. Compared to static cold storage, median total costs at 1 year were higher for ex vivo lung perfusion ( 516 000; p = 0.007) along with the cost of living 1 year with a Karnofsky functional status of 100 after transplant ( 841 000). In simulated scenarios, each Karnofsky‐adjusted life year gained by ex vivo lung perfusion was 1.00–1.72 times more expensive. Conclusions Portable ex vivo lung perfusion is not currently cost‐effective at a low‐volume transplant centers in the USA, being 1.53 times more expensive per Karnofsky‐adjusted life year. Improving donor lung and/or recipient biology during ex vivo lung perfusion may improve its utility for routine transplantation.

Prone ex vivo lung perfusion protects human lungs from reperfusion injury

Abstract:Background: We previously reported beneficial effects of prone positioning during ex vivo lung perfusion (EVLP) using porcine lungs. In this study, we sought to determine if prone positioning during EVLP was beneficial in human donor lungs rejected for clinical use. Methods: Human double lung blocs were randomized to prone EVLP (n = 5) or supine EVLP (n = 5). Following 16 h of cold storage at 4°C and 2 h of cellular EVLP in either the prone or supine position. Lung function, compliance, and weight were evaluated and transplant suitability determined after 2 h of EVLP. Results: Human lungs treated with prone EVLP had significantly higher partial pressure of oxygen/fraction of inspired oxygen (P/F) ratio [348 (291-402) vs. 199 (191-257) mm Hg, p = 0.022] and significantly lower lung weight [926(864-1078) vs. 1277(1029-1483) g, p = 0.037] after EVLP. 3/5 cases in the prone group were judged suitable for transplant after EVLP, while 0/5 cases in the supine group were suitable. When function of upper vs. lower lobes was evaluated, prone EVLP lungs showed similar P/F ratios and inflammatory cytokine levels in lower vs. upper lobes. In contrast, supine EVLP lungs showed significantly lower P/F ratios [68(59-150) vs. 467(407-515) mm Hg, p = 0.012] and higher tissue tumor necrosis factor alpha levels [100.5 (46.9-108.3) vs. 39.9 (17.0-61.0) ng/ml, p = 0.036] in lower vs. upper lobes. Conclusions: Prone lung positioning during EVLP may optimize the outcome of EVLP in human donor lungs, possibly by improving lower lobe function.

Abstract:Objective To evaluate the security and effectiveness of raffinose-low potassium dextran solution (RLPDs) for donor lung preservation.Methods From Sep.2002 to Dec.2011,131patients underwent lung transplantation in our institution,including 81cases of single lung transplantation and 50 cases of bilateral lung transplantation.There were 95 males and 36 females.A total of 129 donors were used for organ harvest.Two donors denoted their lungs to four patients receiving single lung transplantation. All the donors were flushed, perfused antegradely and retrogradely,and preserved with hypothermal RLPDs.The repeated reperfusion was necessary when the cold ischemic time was fairly long and the function of donor was poor.During operation,the blood gas analysis,mean pulmonary artery pressure (mPAP) and breathing mechanics were monitored.Postoperatively,all the recipients received re-examination of cardiac ultrasonography,pulmonary function,blood gas analysis and chest imaging.Results All lung transplantations were performed successfully,with a mean cold ischemic time of (193±21) min (range,65-630 min).There was a significant relationship between postoperative pneumonedema and donor lung cold ischernic time (P＜0.05),which was also observed between postoperative pneumonedema and primary graft dysfunction (P＜0.01). The blood gas analysis and oxygenation index were significantly improved postoperatively,with the mPAP and parameters of breathing mechanics being descended to a normal level.There were 24 early deaths in the first month post-transplant,with a mortality of 18.3％ (24/131),including 11cases of sepsis,7 cases of primary graft dysfunction,3 cases of heart failure,1case of bronchial stomal leak,1case of acute rejection,and 1case of pulmonary infarction.The pulmonary function was improved significantly 3 months postoperatively.Conclusion RLPDs is efficient in the preservation of donor lung,with the ability to alleviate ischemic reperfusion injury and improve pulmonary function.
Moderate-Flow Perfusion is Superior to Low-Flow Perfusion in Ex Situ Lung Perfusion

Abstract:Background: Full-flow perfusion during prolonged ex situ lung perfusion (ESLP) results in unacceptable pulmonary edema formation. Clinical ESLP at 30% to 50% predicted cardiac output (CO) supports acceptable physiologic outcomes; however, progressive pulmonary edema still develops. Lower flow rates may provide equivalent physiologic preservation with less edema formation due to reduced hydrostatic pressures. We report our results of moderate-flow (MF; 30% CO) vs low-flow (LF; 10% CO) negative pressure ventilation (NPV)-ESLP with transplantation. Methods: Twelve pig lungs underwent 12-hours of NPV-ESLP with 30% or 10% CO (n = 6/group). Three left lungs per group were transplanted post-ESLP and assessed in vivo over 4 hours. Lung function was assessed by physiologic parameters, weight-gain, and pro-inflammatory cytokine profiles. Results: Results are MF vs LF (mean ± SEM). All lungs demonstrated acceptable oxygenation post-ESLP (454.2 ± 40.85 vs 422.7 ± 31.68, P = .28); however, after transplantation, the MF lungs demonstrated significantly better oxygenation (300.7 ± 52.26 vs 141.9 ± 36.75, P = .03). There was no significant difference in compliance after ESLP (21.38 ± 2.28 vs 16.48 ± 2.34, P = .08); however, pulmonary artery pressure (PAP; 10.89 ± 2.28 vs 21.11 ± 0.93, P = .06) and pulmonary vascular resistance (PVR; 438.60 ± 97.97 vs 782.20 ± 162.20, P = .05) were significantly higher in the LF group. Weight gain (%) post-ESLP and post-transplant was similar between groups (29.42 ± 5.72 vs 24.17 ± 4.42, P = .24; and 29.63 ± 7.23 vs 57.04 ± 15.78, P = .09). TNF-α and IL-6 were significantly greater throughout LF ESLP. Conclusions: The MF NPV-ESLP results in superior lung function with less inflammation compared to LF NPV-ESLP.
The impact of ex vivo lung perfusion location on lung transplant outcomes

Abstract:Background: Ex vivo lung perfusion (EVLP) conducted outside of the transplant center has increased in recent years to mitigate its limitation by resources and expertise. We sought to evaluate EVLP performed at transplant centers and externally. Methods: Lung transplant recipients were identified from the United Network for Organ Sharing Database. Recipients were then stratified into two groups based where they were perfused: Transplant Program (TP) or External Perfusion Centers (EPC). The groups were assessed with comparative statistics and long-term survival was assessed by Kaplan-Meier method. The groups were then 1:1 propensity and this process was repeated. Results: EPC use was generally restricted to the Southern United States. Following matching, there were no significant differences in post-operative outcomes to include post-operative stroke, dialysis, airway dehiscence, ECMO use, ventilator use or incidence of primary graft dysfunction Grade 3. Adjusted 3-year survival was 68.9% (95% Confidence Interval [CI]: 60.9%-77.9%) for the TP group and 67.6% (95% CI: 61.0%-74.9%) for the EPC group (p = 0.69). In allografts with extended ischemia (14+ h), those in the TP group had significantly longer length of stay, prolonged ventilation and treated rejection in the 1st year, though no significant difference in mid-term survival (p = 0.66). Conclusion: EVLP performed at an EPC can be carried out with results and survival similar to allografts undergoing EVLP at a TP. EPCs will extend the valuable resource of EVLP to lung transplant programs without the resources to perform EVLP.
