Abstract:AIM: To assess neovascularization within human carotid atherosclerotic soft plaques in patients with ischemic stroke.METHODS: Eighty-one patients with ischemic stroke and 95 patients without stroke who had soft atherosclerotic plaques in the internal carotid artery were studied. The thickest soft plaque in each patient was examined using contrast-enhanced ultrasound. Time-intensity curves were collected from 5 s to 3 min after contrast injection. The neovascularization within the plaques in the internal carotid artery was evaluated using the ACQ software built into the scanner by 2 of the experienced investigators who were blinded to the clinical history of the patients.RESULTS: Ischemic stroke was present in 7 of 33 patients (21%) with grade. plaque, in 14 of 51 patients (28%) with grade. plaque, in 26 of 43 patients (61%) with grade. plaque, and in 34 of 49 patients (69%) with grade. plaque (P < 0.001 comparing grade. plaque with grade I plaque and with grade. plaque and P = 0.001 comparing grade. plaque with grade. plaque and with grade. plaque). Analysis of the time intensity curves revealed that patients with ischemic stroke had a significantly higher intensity of enhancement (IE) than those without ischemic stroke (P < 0.01). The wash-in time (WT) of plaque was significantly shorter in stroke patients (P < 0.05). The sensitivity and specificity for IE in the plaque were 82% and 80%, respectively, and for WT were 68% and 74%, respectively. There was no significant difference in the peak intensity or time to peak between the 2 groups.CONCLUSION: This study shows that the higher the grade of plaque enhancement, the higher the risk of ischemic stroke. Ipsilateral Plaques Display Higher T1 Signals Than Contralateral Plaques in Recently Symptomatic Patients with Bilateral Carotid Intraplaque Hemorrhage.

Abstract:Background and aims: Prospective studies have shown a strong association between carotid intraplaque hemorrhage (IPH), detected by magnetic resonance imaging (MRI), and cerebrovascular ischemic events. However, IPH is also observed in a substantial number of asymptomatic patients. We hypothesized that there are differences in the characteristics of IPH+ plaques associated with recent symptoms, compared to IPH+ plaques not associated with recent symptoms.Methods: Patients with recent (<= 2 weeks) anterior circulation ischemic events were scanned using a standardized multisequence protocol. Those showing IPH bilaterally were included and analyzed for differences in T1/T2 signals, plaque morphology, and coexisting plaque characteristics between the ipsilateral symptomatic and contralateral asymptomatic sides.Results: Thirty-one subjects (67 +/- 9 years, 97% males) with bilateral IPH were studied. Despite comparable luminal stenosis (53 +/- 42% vs. 53 +/- 39%, p = 0.99), T1 signal of IPH measured as signal-intensity-ratio compared to muscle was stronger (SIRIPH-to-muscle: 5.8 +/- 2.4 vs. 4.7 +/- 1.8, p = 0.004) and tended to be more extensively distributed (IPH volume: 150 +/- 199 vs. 88 +/- 106 mm(3), p = 0.071) on the symptomatic side. IPH+ plaques on the symptomatic side were longer (24 +/- 6 vs. 21 +/- 7 mm, p = 0.026) and associated with larger necrotic core volume (406 +/- 354 vs. 291 +/- 293 mm(3), p = 0.039) than those on the asymptomatic side.Conclusions: In recently symptomatic patients with bilateral carotid IPH, the symptomatic side showed stronger T1 signals, larger necrotic cores, and longer plaque length than the asymptomatic side. Serial studies on the temporal relationship between these imaging features and clinical events will eventually establish their diagnostic and prognostic value beyond the mere presence of IPH. Characteristics of Symptomatic Intra- and Extracranial Atherosclerotic Plaques in Anterior Circulation Based on Multi-contrast Magnetic Resonance Vascular Wall Imaging

Abstract:Objective To compare the characteristics of intra- and extracranial plaques using 3D multi-contrast magnetic resonance vessel wall imaging in patients with symptomatic intra- and extracranial atherosclerotic diseases. Methods The data of patients with symptomatic intra-and extracranial carotid artery culprit plaques identified by MR vessel wall imaging were retrospectively analyzed. The distribution, burden and components of plaques were analyzed. The plaque burden including the maximum wall thickness (Max WT), plaque length and vessel stenosis were measured. Plaque components such as lipid-rich necrotic core (LRNC), intraplaque hemorrhage (IPH) and calcification on MRI were evaluated. The imaging characteristics of plaques between intracranial and extracranial arteries were compared. Results In total, 45 patients were recruited and 203 atherosclerotic plaques were detected, including 156 extracranial plaques (76.8%) and 47 intracranial plaques (23.2%). Max WT [(3.2±1.0) mm vs (1.9±0.4) mm, P<0.001] and plaque length [10.7 (7.5-13.7) mm vs 4.3 (2.6-6.3) mm, P<0.001] of extracranial plaques were significantly larger than that of intracranial plaques. Stenosis degree [28.0 (20.0-38.5)% vs 18.8 (10.8-30.5)%, P<0.001] of intracranial plaques were significantly larger than that of extracranial plaques. Compared with intracranial plaques, extracranial plaques showed more higher rate of LRNC (66.0% vs 21.3%, P<0.001) and lower rate of IPH (13.5% vs 27.7%, P=0.027). Conclusions For patients with atherosclerotic culprit plaques and symptomatic intracranial and extracranial artery diseases, extracranial plaques commonly have greater Max WT, higher rate of LRNC, less severe stenosis and lower rate of IPH than intracranial plaques.

Abstract:A partial resummation of perturbation theory is described for field theories containing spin-$\frac{1}{2}$ particles in states that may be far from thermal equilibrium. This allows the nonequilibrium state to be characterized in terms of quasiparticles that approximate its true elementary excitations. In particular, the quasiparticles have dispersion relations that differ from those of free particles, finite thermal widths and occupation numbers which, in contrast with those of standard perturbation theory, evolve with the changing nonequilibrium environment. A description of this kind is essential for estimating the evolution of the system over extended periods of time. In contrast with the corresponding description of scalar particles, the structure of nonequilibrium fermion propagators exhibits features which have no counterpart in the equilibrium theory.

Abstract:Background One of the main features of vulnerability in atherosclerotic carotid plaques is intraplaque hemorrhage (IPH), which is related to a higher incidence of cerebrovascular events in patients with carotid artery disease. Additionally, inflammation plays a pivotal role in the progression of atherosclerotic disease. Purpose We aimed to determine plasma and tissue levels of inflammatory cytokines in patients with carotid artery disease hypothesizing that plaques with IPH are characterized by the presence of a more inflamed environment. Methods We retrospectively enrolled 104 consecutive patients undergoing carotid endarterectomy (CEA) with symptomatic (ischemic stroke, TIA, amaurosis fugax) or asymptomatic ipsilateral carotid artery disease. Plasma and carotid plaque specimens were collected at CEA. The tissue samples were stained with H&E to identify IPH and classified as specimens with and without IPH. Frozen homogenates of both plasma and tissue were analyzed using a customized 10-plex Luminex assay to determine the levels of inflammatory cytokines such as MCP-1/CCL2, IL-8/CXCL8, IFN-gamma, IL-10, IL-1beta/IL-1F2, IL-6, PDGF-AA, PDGF-AB/BB, TNF-alpha, and VEGF. Results IPH was identified in 53 patients (51.0 %). Cytokine levels in plasma and tissue from patients with and without IPH are reported in Table 1. In the univariate logistic regression MCP-1/CCL2, IL-8/CXCL8, IFN-gamma, IL-10, IL6, PDGF-AA, and PDGF-AB/BB were associated with IPH (Table 2). In the multivariate logistic regression, IL-6 was an independent predictor of IPH (HR 1.026 [95% CI 1.005-1.047]; p=0.014) after adjusting for cardiovascular risk factors such as age, BMI, sex, hypertension, diabetes mellitus, hyperlipidemia, ipsilateral carotid stenosis %. Conclusions Carotid plaques with IPH have an elevated inflammatory burden, potentially impacting adverse outcomes in this patient cohort. The local inflammatory cytokines in carotid plaques with IPH might serve as biomarkers and therapeutic targets in patients with carotid artery disease.

Association of carotid atherosclerotic plaque features with acute ischemic stroke: a magnetic resonance imaging study.

Abstract:It remains unclear whether direct vessel wall imaging can identify carotid high-risk lesions in symptomatic subjects and whether carotid plaque characteristics are more effective indicators for cerebral infarct severity than stenosis. This study sought to determine the associations of carotid plaque characteristics by MR imaging with stenosis and acute cerebral infarct (ACI) sizes on diffusion weighted imaging (DWI).One hundred and fourteen symptomatic patients underwent carotid and brain MRI. ACI volume was determined from symptomatic internal carotid artery territory on DWI images. Ipsilateral carotid plaque morphological and compositional characteristics, and stenosis were also determined. The relationships between carotid plaque characteristics, stenosis and ACIs size were then evaluated.In carotid arteries with 30-49% stenosis, 86.7% and 26.7% were found to have lipid-rich necrotic core (LRNC) and intraplaque hemorrhage, respectively. Furthermore, 45.8% of carotid arteries with 0-29% stenosis developed LRNCs. Carotid morphological measurements, such as % wall volume, and the LRNC size were significantly associated with ipsilateral ACIs volume before and after adjustment for significant demographic factors (age and LDL) or stenosis in patients with carotid plaque (all p<0.05).A substantial number of high-risk plaques characterized by vessel wall imaging exist in carotid arteries with lower grade stenosis. In addition, carotid plaque characteristics, particularly the % wall volume and LRNC size, are independently associated with cerebral infarction as measured by DWI lesions. Our findings indicate that characterizing atherosclerotic plaque by MR vessel wall imaging might be useful for stratification of plaque risk and infarction severity.
Major carotid plaque surface irregularities correlate with neurologic symptoms

Abstract:Purpose: Many studies have linked carotid plaque surface irregularities with stroke risk, but this relationship has been obscured by the limited ability of available imaging modalities to resolve plaque surface morphology. To address this issue, we performed a prospective study correlating the presenting neurologic symptoms of patients with high-resolution magnetic resonance imaging (MRI; 200 microm) studies of ipsilateral plaque surface invaginations and ledges, lumen shape, and the location of the plaque bulk creating the stenosis. Methods: One hundred patients, 17 women and 83 men, 45 to 81 years old (mean, 68 years) underwent surgery. Forty-five patients had a transient ischemic attack (TIA) or stroke as the indication for surgery, and 55 patients had no symptoms. Angiograms were obtained in 50 patients. Carotid plaques were removed "en bloc" and placed in gadolinium doped saline for imaging in a Siemens Symphony, 1.5T scanner with a custom-built transmit-receive radiofrequency coil. The resulting slice thickness was 200 microm, with 200 microm by 200 microm in plane resolution. The MRI data and angiograms were reviewed by using National Institutes of Health Image software and read by consensus. A surface irregularity was categorized as a ledge or ulcer and measured by using electronic calipers. Luminal shape was determined at the point of maximal stenosis with a "slice" set at 90 degrees to the lumen axis. The location of the maximal stenosis was recorded. In the internal carotid artery, plaque bulk was designated to be on the flow divider wall or non-flow divider wall. Results: The mean maximal stenosis was 81.5% +/- 12.0%. Surface contour irregularities were found in 80 plaques. Thirty-five plaques were graded as having major surface contour irregularities, and 45 plaques were graded as having minor irregularities. There was a significant correlation between major surface irregularity and TIA or stroke (P <.01). Irregular plaques were identified with angiography, but the irregularity in size was underestimated (P <.01). Only 28% of plaques had circular lumens; 50% had elliptical lumens, and 22% had either crescentic or multi-lobular lumens. The maximal stenosis was located in the internal carotid artery in 82 plaques, the bifurcation in 17 plaques, and the common carotid artery in one plaque. Conclusion: Surface irregularities were revealed by means of submillimeter resolution of the carotid plaques with MRI to be common, but only the presence of major irregularities correlated with the patient having TIA or stroke. Lumen shape and plaque location did not appear to predict stroke risk, but may effect imaging accuracy in determining the degree of stenosis. These data further define the relationship of plaque irregularity and cerebrovascular symptoms caused by atheroemboli.

Abstract:Successful application of active immunotherapy to the treatment of cancer will require stimulation of potent antigen-specific T-cell responses. It is not known how numerous or how potent these T cells must be in order to abrogate tumors, but the levels of immunity needed to control chronic viral infections may provide estimates for comparison. Evaluation of the efficacy of a vaccine strategy in attaining these levels of immunity will depend on the use of assays that create a picture of T-cell number and function that correlates with clinical outcomes. We discuss the currently available in vivo and in vitro T-cell assays and their relevance for detecting therapeutic levels of T-cell activity. We also propose a strategy for efficiently evaluating the immunologic efficacy of cancer vaccines so that the most promising candidates can be brought more rapidly into definitive clinical trials.
Discriminating Carotid Atherosclerotic Lesion Severity By Luminal Stenosis And Plaque Burden A Comparison Utilizing High-Resolution Magnetic Resonance Imaging At 3.0 Tesla

Patients with either carotid stenosis >50% by duplex ultrasound or suspected coronary artery disease underwent multi-contrast carotid MRI at 3.0 T. For each artery, stenosis, percent wall volume (PWV=100% x wall volume/total vessel volume), and mean wall thickness (MWT) were measured. Presence or absence of a lipid-rich necrotic core, calcification, IPH, and surface disruption were recorded.Results-One hundred eighty-one patients were included in the final analysis. The area under the curve (AUC) calculated from receiver-operating-characteristics analysis found the presence of IPH was similarly classified by stenosis (AUC=0.82), PWV (AUC=0.88), and MWT (AUC=0.88). Notably, IPH was present in the lowest category of each parameter. Prevalence of IPH in arteries with 0% stenosis was 4.4%. In arteries with PWV <40%, prevalence was 3.2%; in arteries with MWT <1.0 mm, prevalence was 2.3%. Strength of classification for surface disruption was similarly classified by stenosis (AUC=0.87), PWV (AUC=0.93), and MWT (AUC=0.94).Conclusions-Measures of plaque burden do not substantially improve disease assessment compared to stenosis. The finding of IPH in all categories of stenosis and plaque burden suggests that direct characterization of plaque composition and surface status is necessary to fully discriminate disease severity. Size of Carotid Artery Intraplaque Hemorrhage and Acute Ischemic Stroke: a Cardiovascular Magnetic Resonance Chinese Atherosclerosis Risk Evaluation Study

Abstract:To determine the usefulness of the size of carotid artery intraplaque hemorrhage (IPH) in discriminating the risk of acute ischemic stroke using cardiovascular magnetic resonance (CMR) vessel wall imaging. Symptomatic patients with carotid atherosclerotic plaque who participated in a cross-sectional, multicenter study of CARE-II (NCT02017756) were included. All patients underwent carotid and brain CMR imaging. Carotid plaque burden and the size of plaque compositions including calcification, lipid-rich necrotic core (LRNC), and IPH were measured. Presence of acute cerebral infarct (ACI) in ipsilateral hemisphere of carotid plaque was determined. The relationship between carotid plaque features and presence of ipsilateral ACI was then analyzed. Of 687 recruited patients (62.7 ± 10.1 years; 69.4% males) with carotid plaque, 28.5% had ACI in ipsilateral hemispheres. Logistic regression revealed that carotid plaque burden was significantly associated with the presence of ACI before and after adjusted for clinical confounding factors. The volume of LRNC, %LRNC volume, volume of IPH, and %IPH volume were significantly associated with ACI before (volume of LRNC: OR = 1.297, p = 0.005; %LRNC volume: OR = 1.119, p = 0.018; volume of IPH: OR = 2.514, p = 0.003; %IPH volume: OR = 2.202, p = 0.003) and after (volume of LRNC: OR = 1.312, p = 0.006; %LRNC volume: OR = 1.90, p = 0.034; volume of IPH: OR = 2.907, p = 0.007; % IPH volume: OR = 2.374, p = 0.004) adjusted for clinical confounding factors. The association between volume of IPH and ACI remained statistically significant after further adjusted for plaque volume (OR = 2.813, p = 0.016) or both plaque volume and volume of LRNC (OR = 4.044, p = 0.024). In symptomatic patients with carotid atherosclerotic plaques, the size of IPH is independently associated with ipsilateral ACI, suggesting the size of IPH might be a useful indicator for the risk of ACI. Comparison of carotid atherosclerotic plaque characteristics between symptomatic patients with transient ischemic attack and stroke using high-resolution magnetic resonance imaging

Abstract:Background: This study aimed to compare the characteristics of carotid plaques between patients with transient ischemic attack (TIA) and ischemic stroke using magnetic resonance (MR) imaging. Methods: Patients with a recent ischemic stroke or TIA who exhibited atherosclerotic plaques of carotid arteries in the symptomatic sides determined by MR vessel wall imaging were recruited. The plaque morphology and compositions including intraplaque hemorrhage (IPH), lipid-rich necrotic-core (LRNC) and calcification were compared between TIA and stroke patients. Logistic regression was performed to relate the plaque characteristics to the types of ischemic events. Results: A total of 270 patients with TIA or ischemic stroke were recruited. Stroke patients had a significantly higher prevalence of diabetes (42.2% vs. 28.2%, p = 0.021), greater mean wall area (35.1 ± 10.1 mm2 vs. 32.0 ± 7.7 mm2, p = 0.004), mean wall thickness (1.3 ± 0.2 mm vs. 1.2 ± 0.2 mm, p = 0.001), maximum normalized wall index (NWI)(63.9% ± 6.0% vs. 62.2% ± 5.9%, p = 0.023) and %volume of LRNC (9.7% ± 8.2% vs. 7.4% ± 7.9%, p = 0.025) in the carotid arteries compared to those with TIA. After adjustment for clinical factors, above characteristics of carotid arteries were significantly associated with the type of ischemic events. After further adjustment for maximum NWI, this association remained statistically significant (OR, 1.41; CI, 1.01-1.96; p = 0.041). Conclusions: Ischemic stroke patients had larger plaque burden and greater proportion of LRNC in carotid plaques compared to those with TIA. This study suggests that ischemic stroke patients had more vulnerable plaques compared to those with TIA.

Abstract:BACKGROUND AND PURPOSE: CT is considered the standard reference both for quantification and characterization of carotid artery calcifications. Our aim was to investigate the relationship among different types of calcium configurations detected with CT within the plaque with a novel classification and to investigate the prevalence of cerebrovascular events. MATERIALS AND METHODS: Seven hundred ninety patients (men = 332; mean age, 69.7 [SD, 13] years; 508 symptomatic for cerebrovascular symptoms and 282 asymptomatic) who underwent computed tomography of the carotid arteries were retrospectively included in this institutional review board–approved study. The plaque was classified into 6 types according to the different types of calcium configurations as the following: type 1, complete absence of calcification within the plaque; type 2, intimal or superficial calcifications; type 3, deep or bulky calcifications; type 4, adventitial calcifications with internal soft plaque of <2 mm thickness; type 5, mixed patterns with intimal and bulky calcifications; and type 6, positive rim sign. RESULTS: The highest prevalence of cerebrovascular events was observed for type 6, for which 89 of the 99 cases were symptomatic. Type 6 plaque had the highest degree of correlation with TIA, stroke, symptoms, and ipsilateral infarct for both sides with a higher prevalence in younger patients. The frequency of symptoms observed by configuration type significantly differed between right and left plaques, with symptoms observed more frequently in type 6 calcification on the right side (50/53; 94%) than on the left side (39/46; 85%, P < .001). CONCLUSIONS: We propose a novel carotid artery plaque configuration classification that is associated with the prevalence of cerebrovascular events. If confirmed in longitudinal analysis, this classification could be used to stratify the risk of occurrence of ischemic events.

Comparison of Carotid Atherosclerotic Plaque Characteristics between Symptomatic Patients with Transient Ischemic Attack and Stroke using Multi-Contrast Magnetic Resonance Imaging

Abstract:Abstract Purpose:This study aimed to compare the characteristics of carotid plaques between patients with transient ischemic attack (TIA) and ischemic stroke using magnetic resonance (MR) imaging.Methods：Symptomatic patients with carotid atherosclerotic plaque who had a recent ischemic stroke or TIA were recruited and underwent carotid MR imaging. The characteristics of plaque morphology and compositions including intraplaque hemorrhage (IPH), lipid-rich necrotic-core (LRNC) and calcification were compared between TIA and stroke patients. Logistic regression was performed to relate the plaque characteristics to the types of ischemic events.Results:A total of 270 patients with TIA or ischemic stroke were recruited. Stroke patients had significantly higher prevalence of diabetes (42.2% vs. 28.2%, p=0.021), greater mean wall area (35.1 ± 10.1 mm2 vs. 32.0 ± 7.7mm2, p = 0.004), mean wall thickness (1.3 ± 0.2 mm vs. 1.2 ± 0.2 mm, p=0.001), maximum normalized wall index (NWI)(63.9% ± 6.0% vs. 62.2% ± 5.9%, p=0.023)and %volume of LRNC(9.7%± 8.2% vs. 7.4% ±7.9%, p=0.025)in carotid arteries compared to those with TIA. After adjusted for clinical factors, above characteristics of carotid arteries were significantly associated with the type of ischemic events. After further adjusting for maximum NWI, the association of %volume of LRNC and stroke subtype remained statistically significant (OR, 1.41; CI, 1.01-1.96; p=0.041).Conclusions:Ischemic stroke patients had larger plaque burden and greater proportion of LRNC in symptomatic carotid plaques compared to patients with TIA. This study suggests that ischemic stroke patients had more vulnerable plaques compared to those with TIA.

Abstract:Aim To analyze the correlation of carotid stenosis severity, the Plaque Reporting and Data System (RADS) score, arterial transit artifacts (ATAs), and cerebral blood flow (CBF) with clinical cerebral ischemic symptoms in patients with carotid artery stenosis (CAS). Materials and methods Sixty-one patients with unilateral internal carotid artery stenosis or occlusion (≥50% stenosis) diagnosed by ultrasound, Computed Tomography(CT) angiography, or Magnetic Resonance(MR) angiography in Yichang City Central People's Hospital from January 2022 to February 2024 were retrospectively enrolled and divided into two groups according to the presence or absence of symptoms. Both groups underwent MR plaque imaging and arterial spin labeling (ASL)-based 3.0 T MRI to compare the differences in stenosis degree, Plaque-RADS score, ATA grade, and CBF between the two groups. Binary regression analysis was used to identify the parameters with statistically significant differences between the two groups and to evaluate their diagnostic efficacy using the area under the workup curve of the subjects. Results The Plaque-RADS score, ATA grade, and CBF differences in the anterior cerebral artery( ACA )blood supply region were correlated with symptoms, and the areas under the ROC curves for the CBF differences in the ACA blood supply region, Plaque-RADS score, ATA grade and a joint model that combines all three to predict symptoms in CAS patients were 0.672, 0.796, 0.788 and 0.919, respectively. Conclusions CBF, Plaque-RADS and ATAs were identified as independent risk factors for symptoms in patients with CAS and have a certain predictive value for symptoms, and the combined predictive value is greater, potentially providing a more effective imaging modality for clinical treatment and evaluation.

Investigating the Association of Carotid Atherosclerotic Plaque MRI Features and Silent Stroke After Carotid Endarterectomy

Abstract:Background: The predictive value of carotid plaque characteristics for silent stroke (SS) after carotid endarterectomy (CEA) is unclear. Objective: To investigate the associations between carotid plaque characteristics and postoperative SS in patients undergoing CEA. Study type: Prospective. Population: One hundred fifty-three patients (mean age: 65.4 ± 7.9 years; 126 males) with unilateral moderate-to-severe carotid stenosis (evaluated by CT angiography) referred for CEA. Field strength/sequence: 3 T, brain-MRI:T2-PROPELLER, T1-/T2-FLAIR, diffusion weighted imaging (DWI) and T2*, carotid-MRI:black-blood T1-/T2W, 3D TOF, Simultaneous Non-contrast Angiography intraplaque hemorrhage. Assessment: Patients underwent carotid-MRI within 1-week before CEA, and brain-MRI within 48-hours pre-/post-CEA. The presence and size (volume, maximum-area-percentage) of carotid lipid-rich necrotic core (LRNC), intraplaque hemorrhage (Type-I/Type-II IPH) and calcification were evaluated on carotid-MR images. Postoperative SS was assessed from pre-/post-CEA brain DWI. Patients were divided into moderate-carotid-stenosis (50%-69%) and severe-carotid-stenosis (70%-99%) groups and the associations between carotid plaque characteristics and SS were analyzed. Statistical tests: Independent t test, Mann-Whitney U-test, chi-square test and logistic regressions (OR: odds ratio, CI: confidence interval). P value <0.05 was considered statistically significant. Results: SS was found in 8 (16.3%) of the 49 patients with moderate-carotid-stenosis and 21 (20.2%) of the 104 patients with severe-carotid-stenosis. In patients with severe-carotid-stenosis, those with SS had significantly higher IPH (66.7% vs. 39.8%) and Type-I IPH (66.7% vs. 38.6%) than those without. The presence of IPH (OR 3.030, 95% CI 1.106-8.305) and Type-I IPH (OR 3.187, 95% CI 1.162-8.745) was significantly associated with SS. After adjustment, the associations of SS with presence of IPH (OR 3.294, 95% CI 1.122-9.669) and Type-I IPH (OR 3.633, 95% CI 1.216-10.859) remained significant. Moreover, the volume of Type-II IPH (OR 1.014, 95% CI 1.001-1.028), and maximum-area-percentage of Type-II IPH (OR 1.070, 95% CI 1.002-1.142) and LRNC (OR 1.030, 95% CI 1.000-1.061) were significantly associated with SS after adjustment. No significant (P range: 0.203-0.980) associations were found between carotid plaque characteristics and SS in patients with moderate-carotid-stenosis. Data conclusions: In patients with unilateral severe-carotid-stenosis, carotid vulnerable plaque MR features, particularly presence and size of IPH, might be effective predictors for SS after CEA. Carotid Artery Plaque Burden as Measured by Magnetic Resonance Imaging : A Potential Imaging Indicator for Acute Cerebral Ischemic Lesion Volume

Abstract:Introduction: The heterogeneity origin of stroke makes its risk prediction clinically challenging. Recently, the volume of infarction and the ischemic lesion patterns have been increasingly assumed to play an important role in investigation of stroke mechanisms and outcomes . Atherosclerotic diseases occurring in carotid arteries have been demonstrated to be associated with transient ischemic attack (TIA) and stroke . Thus, a comprehensive assessment of carotid atherosclerosis is believed to be helpful for predicting neurovascular events and potentially improving the prognosis. Black-blood magnetic resonance imaging (MRI) has been proven to be an effective tool for the quantification of carotid atherosclerotic plaque burden and compositions . However, the correlation between carotid artery plaque burden as measured by black-blood MRI and cerebral ischemic lesion volume is unknown. Purpose: To determine the association between carotid plaque burden and the size of cerebral ischemic lesion volume by MR imaging. Methods: Forty-three patients (mean age 64.5 years, 36 males) with TIA or stroke underwent carotid artery and brain MR imaging on a 3.0T whole-body scanner ( Philips Achieva, Best, the Netherlands) within 1 week after neurovascular symptoms onset. Imaging protocol: Brain MRI protocol: T1w: TR/TE 151/2.3ms;T2w TR/TE 2123/80ms; T2w FLAIR TR/TE 11000/120ms; DWI TR/TE 1598/46ms; acquisition matrix 256×256; slice thickness 6mm; FOV 24cm×24cm. A multicontrast protocol was used to acquire cross-sectional carotid MR images with the following parameters: 3D TOF: TR/TE 20/4ms, flip angle 20°; T1w: quadruple inversion-recovery (QIR) , black-blood, 2D TSE, TR/TE 800/10ms; T2w: Multi-slice Double IR (MDIR), TR/TE 4000/50ms; MP-RAGE: IR TFE, TR/TE 13.4ms, flip angle 15°; FOV 14cm x 14cm; acquisition matrix 256x256; in-plane resolution 0.55mmx0.55mm. A longitudinal coverage of 32 mm was used for bilateral carotid MR images acquisition. Image interpretation: Acute ischemic lesions on DWI were diagnosed when these lesions were shown to be hyperintense on the DWI integrated for the 3 diffusion sensitivity directions and hypointense on the apparent diffusion coefficient map. The cerebral hemisphere (CH) ischemic lesion volume (ILV) was measured by an experienced radiologist blinded to carotid MR images. The ischemic lesions in internal carotid artery (ICA) blood supplying territories (frontal lobe, temporal lobe, parietal lobe, occipital lobe, and basal ganglia) were also evaluated. Carotid MR images were interpreted separately by two trained reviewers with consensus using custom designed soft ware (CASCADE , Seattle, WA, USA). The lumen area (LA), wall area (WA), total vessel area (TVA), mean and maximum wall thickness (Mean/MaxWT), and normalized wall index (NWI=WA/TVA) were measured for each axial location. Carotid plaque burden measurements including mean LA, WA, MeanWT, MaxWT, and Mean NWI for each artery were determined. Results: Of 43 subjects, 21 had ischemic lesions in left cerebral hemisphere, 14 of them were located in the left ICA supplying territories, 19 had ischemic lesions in right cerebral hemisphere, and 15 of them were located in the right ICA supplying territories. No significant difference of carotid plaque burden was found between both sides (P>0.05, Table 1). The left cerebral hemisphere showed larger but not statistically significant acute DWI lesion volume than that of the right side (Table 1). For subjects with left cerebral ischemic lesions, strong positive correlations were found between left carotid plaque burden measurements and the volume of left cerebral ischemic lesions (r ranging from 0.641 to 0.807, P<0.05, Table 2 and Fig). In addition, left carotid LA was negatively correlated with left cerebral ischemic lesion volume P<0.05, Table 2). In contrast, there were no significant correlations of right carotid plaque burden with ischemic lesion volume in right cerebral hemisphere (P>0.05, Table 2). Discussion and conclusions: In this study, we found that left carotid artery plaque burden was significantly correlated with the volumes of ipsilateral cerebral hemisphere ischemic lesions. This finding suggests that the carotid plaque burden may be an effective predictor for the severity of ischemic stroke. We also noticed the unbalanced correlation of bilateral carotid arteries plaque burden with brain ischemic lesions and the asymmetry of ischemic lesion volume in this group, which is hypothesized to be caused by small sample size or affected by other risk factors. Upon the availability of larger patient pool, we are planning to further study the correlation between carotid plaque compositions and intracranial ischemic lesion patterns in this population.
Symptomatic vs. Asymptomatic 20–40% Internal Carotid Artery Stenosis: Does the Plaque Size Matter?

Abstract:Background: Around 9–15% of ischemic strokes are related to internal carotid artery (ICA)-stenosis ≥50%. However, the extent to which ICA-stenosis <50% causes ischemic cerebrovascular events is uncertain. We examined the relation between plaque cross-sectional area and length and the risk of ischemic stroke or TIA among patients with ICA-stenosis of 20–40%.Methods: We retrospectively identified patients admitted to the Department of Neurology, University Hospital of Würzburg, from January 2011 until September 2016 with ischemic stroke or TIA and concomitant ICA-stenosis of 20–40%, either symptomatic or asymptomatic. Plaque length and cross-sectional area were assessed on ultrasound scans.Results: We identified 41 patients with ischemic stroke or TIA and ICA-stenosis of 20–40%; 14 symptomatic and 27 asymptomatic. The plaque cross-sectional area was significantly larger among symptomatic than asymptomatic ICA-stenosis; median values (IQR) were 0.45 (0.21–0.69) cm2 and 0.27 (0.21–0.38) cm2, p = 0.03, respectively. A plaque cross-sectional area ≥0.36 cm2 had a sensitivity of 71% and a specificity of 76% for symptomatic compared with asymptomatic ICA-stenosis. In a sex-adjusted multivariate logistic regression, a plaque cross-sectional area ≥0.36 cm2 and a plaque length ≥1.65 cm were associated with an OR (95% CI) of 5.54 (1.2–25.6), p = 0.028 and 1.78 (0.36–8.73), p = 0.48, respectively, for symptomatic ICA-stenosis.Conclusion: Large plaques might increase the risk of ischemic stroke or TIA among patients with low-grade ICA-stenosis of 20–40%. Sufficiently powered prospective longitudinal cohort studies are needed to definitively test the stroke risk stratification value of carotid plaque length and cross-sectional area in the setting of current optimal medical treatment.

Abstract:Introduction: Prospective studies have showed a strong association between intraplaque hemorrhage (IPH) on carotid MRI and increased risk of cerebrovascular ischemic events. However, it is also seen in a substantial number of asymptomatic patients. Serial MRI studies indicate that IPH can persist for years before causing clinical symptoms. Hypothesis: We hypothesized that there are differences in the characteristics of IPH-positive (IPH+) plaques associated with recent symptoms, compared to IPH+ plaques not associated with recent symptoms. Methods: Patients with recent (≤2 weeks) cerebrovascular ischemic events were scanned with a standardized multicontrast protocol that included time-of-flight, black-blood fast spin echoes, and magnetization prepared rapid acquisition gradient echo (MPRAGE). Abstract:OBJECTIVES The Hypertension Optimal Treatment (HOT) Study had two objectives: (1) to define the optimal target blood pressure when treating hypertensive patients (i.e. the level of blood pressure associated with the lowest incidence of major cardiovascular events such as fatal and non-fatal stroke and myocardial infarction and other cardiovascular mortality); and (2) to assess the effect of a low dose of acetylsalicylic acid (aspirin) compared with placebo on major cardiovascular events. METHODS The HOT study recruited 18 790 hypertensive subjects aged 50 to 80 years with diastolic blood pressure (DBP) between 100 and 115 mmHg (mean DBP 105 mmHg). Subjects were randomly assigned to be treated with the goal of reaching one of three target DBPs: < or = 90 mmHg (n = 6264), < or = 85 mmHg (n = 6264), or < or = 80 mmHg (n = 6262) using felodipine as initial therapy with the addition of other agents according to a 5-step regimen. In addition, subjects were randomly assigned to receive either 75 mg/day aspirin (n = 9399) or placebo (n = 9391). RESULTS DBP was reduced by 20.3, 22.3, and 24.3 mmHg in the < or = 90, < or = 85, and < or = 80 mmHg target groups, respectively. The incidence of all major cardiovascular events (fatal and non-fatal stroke, myocardial infarction, and other cardiovascular mortality) decreased, although not significantly, in relation to the randomized target blood pressure. There were 84, 64, and 61 fatal and non-fatal myocardial infarctions in the < or = 90 mmHg, < or = 85 mmHg, and < or = 80 mmHg groups, respectively (P = 0.05). The overall effect of aspirin on cardiovascular events was positive, most noticeably on fatal and non-fatal myocardial infarctions; aspirin reduced events by 36%. There were seven fatal episodes of bleeding (two cerebral) in the aspirin group compared with eight (three cerebral) in the placebo group, and 129 versus 70 non-fatal major episodes of bleeding in the two groups, respectively. The lowest incidence of all major cardiovascular events combined was found at an achieved DBP of 82.6 mmHg and at an achieved systolic blood pressure (SBP) of 138.5 mmHg. In the subgroup with diabetes mellitus at baseline (n = 1501), the effect of intensive lowering of blood pressure was especially noticeable. CONCLUSIONS Most cardiovascular end-points showed a declining frequency in relation to target blood pressure. The optimal protection against combined major cardiovascular end-points was observed in the range 80-85 mmHg for DBP and in the range 130-140 mmHg for SBP. It is worth noting that 92% of all subjects reached a DBP of < or = 90 mmHg and that side-effects gradually declined from 16.9% at 3 months to 2.2% at the end of the study. A substudy showed that quality of life was linked to the level of blood pressure obtained: the lower the blood pressure, the better the quality of life.
