Abstract:Imaging in clinical routine is subject to changing scanner protocols, hardware, or policies in a typically heterogeneous set of acquisition hardware. Accuracy and reliability of deep learning models suffer from those changes as data and targets become inconsistent with their initial static training set. Continual learning can adapt to a continuous data stream of a changing imaging environment. Here, we propose a method for continual active learning on a data stream of medical images. It recognizes shifts or additions of new imaging sources - domains -, adapts training accordingly, and selects optimal examples for labelling. Model training has to cope with a limited labelling budget, resembling typical real world scenarios. We demonstrate our method on T1-weighted magnetic resonance images from three different scanners with the task of brain age estimation. Results demonstrate that the proposed method outperforms naive active learning while requiring less manual labelling.

Abstract:Medical imaging analysis has witnessed remarkable advancements even surpassing human-level performance in recent years, driven by the rapid development of advanced deep-learning algorithms. However, when the inference dataset slightly differs from what the model has seen during one-time training, the model performance is greatly compromised. The situation requires restarting the training process using both the old and the new data which is computationally costly, does not align with the human learning process, and imposes storage constraints and privacy concerns. Alternatively, continual learning has emerged as a crucial approach for developing unified and sustainable deep models to deal with new classes, tasks, and the drifting nature of data in non-stationary environments for various application areas. Continual learning techniques enable models to adapt and accumulate knowledge over time, which is essential for maintaining performance on evolving datasets and novel tasks. This systematic review paper provides a comprehensive overview of the state-of-the-art in continual learning techniques applied to medical imaging analysis. We present an extensive survey of existing research, covering topics including catastrophic forgetting, data drifts, stability, and plasticity requirements. Further, an in-depth discussion of key components of a continual learning framework such as continual learning scenarios, techniques, evaluation schemes, and metrics is provided. Abstract:Machine learning in medical imaging during clinical routine is impaired by changes in scanner protocols, hardware, or policies resulting in a heterogeneous set of acquisition settings. When training a deep learning model on an initial static training set, model performance and reliability suffer from changes of acquisition characteristics as data and targets may become inconsistent. Continual learning can help to adapt models to the changing environment by training on a continuous data stream. However, continual manual expert labelling of medical imaging requires substantial effort. Thus, ways to use labelling resources efficiently on a well chosen sub-set of new examples is necessary to render this strategy feasible. Here, we propose a method for continual active learning operating on a stream of medical images in a multi-scanner setting. The approach automatically recognizes shifts in image acquisition characteristics - new domains -, selects optimal examples for labelling and adapts training accordingly. Labelling is subject to a limited budget, resembling typical real world scenarios. To demonstrate generalizability, we evaluate the effectiveness of our method on three tasks: cardiac segmentation, lung nodule detection and brain age estimation. Results show that the proposed approach outperforms other active learning methods, while effectively counteracting catastrophic forgetting.
Class-Incremental Continual Learning for General Purpose Healthcare Models

Abstract:Healthcare clinics regularly encounter dynamic data that changes due to variations in patient populations, treatment policies, medical devices, and emerging disease patterns. Deep learning models can suffer from catastrophic forgetting when fine-tuned in such scenarios, causing poor performance on previously learned tasks. Continual learning allows learning on new tasks without performance drop on previous tasks. In this work, we investigate the performance of continual learning models on four different medical imaging scenarios involving ten classification datasets from diverse modalities, clinical specialties, and hospitals. We implement various continual learning approaches and evaluate their performance in these scenarios. Our results demonstrate that a single model can sequentially learn new tasks from different specialties and achieve comparable performance to naive methods. These findings indicate the feasibility of recycling or sharing models across the same or different medical specialties, offering another step towards the development of general-purpose medical imaging AI that can be shared across institutions.

Leveraging Old Knowledge to Continually Learn New Classes in Medical Images

Abstract:Class-incremental continual learning is a core step towards developing artificial intelligence systems that can continuously adapt to changes in the environment by learning new concepts without forgetting those previously learned. This is especially needed in the medical domain where continually learning from new incoming data is required to classify an expanded set of diseases. In this work, we focus on how old knowledge can be leveraged to learn new classes without catastrophic forgetting. We propose a framework that comprises of two main components: (1) a dynamic architecture with expanding representations to preserve previously learned features and accommodate new features; and (2) a training procedure alternating between two objectives to balance the learning of new features while maintaining the model's performance on old classes. Experiment results on multiple medical datasets show that our solution is able to achieve superior performance over state-of-the-art baselines in terms of class accuracy and forgetting.

Incremental learning for an evolving stream of medical ultrasound images via counterfactual thinking

Abstract:Despite the fact that traditional deep learning (DL) approaches provide promising accuracy and efficiency in medical ultrasound image analysis, they cannot replace the physician in making a diagnosis since the DL model is only appropriate in static application scenarios. Currently, most DL-based models are incapable of learning new tasks in the dynamic clinical environments due to the catastrophic forgetting of old tasks. To address the above problem, we propose an incremental classifier that is sequentially trained on evolving tasks for medical ultrasound images by counterfactual thinking. Specifically, the proposed model consists of a feature extractor and a classifier that can add new classes at any time during training. Toward a more discriminative model in the continual learning setting, a contrastive strategy is designed to leverage fine-grained information by generating a series of counterfactual regions. For model optimization, we design a multi-task loss made up of a knowledge distillation loss, a cross-entropy loss, and a contrasting loss. This objective jointly enjoys the merits of less forgetting, better accuracy, and fine-grained information utilization. A newly collected dataset with 52 medical ultrasound classification tasks is used to demonstrate the effectiveness of our method. The proposed approach achieves 76.59%, 11.67%, and 7.93% in terms of the average incremental accuracy, forgetting rate, and feature retention, respectively.
Privacy-preserving continual learning methods for medical image classification: a comparative analysis

Abstract:Background The implementation of deep learning models for medical image classification poses significant challenges, including gradual performance degradation and limited adaptability to new diseases. However, frequent retraining of models is unfeasible and raises concerns about healthcare privacy due to the retention of prior patient data. To address these issues, this study investigated privacy-preserving continual learning methods as an alternative solution. Methods We evaluated twelve privacy-preserving non-storage continual learning algorithms based deep learning models for classifying retinal diseases from public optical coherence tomography (OCT) images, in a class-incremental learning scenario. The OCT dataset comprises 108,309 OCT images. Its classes include normal (47.21%), drusen (7.96%), choroidal neovascularization (CNV) (34.35%), and diabetic macular edema (DME) (10.48%). Each class consisted of 250 testing images. For continuous training, the first task involved CNV and normal classes, the second task focused on DME class, and the third task included drusen class. All selected algorithms were further experimented with different training sequence combinations. The final model's average class accuracy was measured. The performance of the joint model obtained through retraining and the original finetune model without continual learning algorithms were compared. Additionally, a publicly available medical dataset for colon cancer detection based on histology slides was selected as a proof of concept, while the CIFAR10 dataset was included as the continual learning benchmark. Results Among the continual learning algorithms, Brain-inspired-replay (BIR) outperformed the others in the continual learning-based classification of retinal diseases from OCT images, achieving an accuracy of 62.00% (95% confidence interval: 59.36-64.64%), with consistent top performance observed in different training sequences. For colon cancer histology classification, Efficient Feature Transformations (EFT) attained the highest accuracy of 66.82% (95% confidence interval: 64.23-69.42%). In comparison, the joint model achieved accuracies of 90.76% and 89.28%, respectively. The finetune model demonstrated catastrophic forgetting in both datasets. Conclusion Although the joint retraining model exhibited superior performance, continual learning holds promise in mitigating catastrophic forgetting and facilitating continual model updates while preserving privacy in healthcare deep learning models. Thus, it presents a highly promising solution for the long-term clinical deployment of such models.

Incremental Learning for Heterogeneous Structure Segmentation in Brain Tumor MRI

Abstract:Deep learning (DL) models for segmenting various anatomical structures have achieved great success via a static DL model that is trained in a single source domain. Yet, the static DL model is likely to perform poorly in a continually evolving environment, requiring appropriate model updates. In an incremental learning setting, we would expect that well-trained static models are updated, following continually evolving target domain data -- e.g., additional lesions or structures of interest -- collected from different sites, without catastrophic forgetting. This, however, poses challenges, due to distribution shifts, additional structures not seen during the initial model training, and the absence of training data in a source domain. To address these challenges, in this work, we seek to progressively evolve an ``off-the-shelf" trained segmentation model to diverse datasets with additional anatomical categories in a unified manner. Specifically, we first propose a divergence-aware dual-flow module with balanced rigidity and plasticity branches to decouple old and new tasks, which is guided by continuous batch renormalization. Then, a complementary pseudo-label training scheme with self-entropy regularized momentum MixUp decay is developed for adaptive network optimization. We evaluated our framework on a brain tumor segmentation task with continually changing target domains -- i.e., new MRI scanners/modalities with incremental structures. Our framework was able to well retain the discriminability of previously learned structures, hence enabling the realistic life-long segmentation model extension along with the widespread accumulation of big medical data.

Abstract:Deep Learning has shown great success in reshaping medical imaging, yet it faces numerous challenges hindering widespread application. Issues like catastrophic forgetting and distribution shifts in the continuously evolving data stream increase the gap between research and applications. Continual Learning offers promise in addressing these hurdles by enabling the sequential acquisition of new knowledge without forgetting previous learnings in neural networks. In this survey, we comprehensively review the recent literature on continual learning in the medical domain, highlight recent trends, and point out the practical issues. Specifically, we survey the continual learning studies on classification, segmentation, detection, and other tasks in the medical domain. Furthermore, we develop a taxonomy for the reviewed studies, identify the challenges, and provide insights to overcome them. We also critically discuss the current state of continual learning in medical imaging, including identifying open problems and outlining promising future directions. We hope this survey will provide researchers with a useful overview of the developments in the field and will further increase interest in the community. Abstract:In the dynamic realm of practical clinical scenarios, Continual Learning (CL) has gained increasing interest in medical image analysis due to its potential to address major challenges associated with data privacy, model adaptability, memory inefficiency, prediction robustness and detection accuracy. In general, the primary challenge in adapting and advancing CL remains catastrophic forgetting. Beyond this challenge, recent years have witnessed a growing body of work that expands our comprehension and application of continual learning in the medical domain, highlighting its practical significance and intricacy. In this paper, we present an in-depth and up-to-date review of the application of CL in medical image analysis. Our discussion delves into the strategies employed to address specific tasks within the medical domain, categorizing existing CL methods into three settings: Task-Incremental Learning, Class-Incremental Learning, and Domain-Incremental Learning. These settings are further subdivided based on representative learning strategies, allowing us to assess their strengths and weaknesses in the context of various medical scenarios. By establishing a correlation between each medical challenge and the corresponding insights provided by CL, we provide a comprehensive understanding of the potential impact of these techniques. To enhance the utility of our review, we provide an overview of the commonly used benchmark medical datasets and evaluation metrics in the field. Through a comprehensive comparison, we discuss promising future directions for the application of CL in medical image analysis. What is Wrong with Continual Learning in Medical Image Segmentation?

Abstract:Continual learning protocols are attracting increasing attention from the medical imaging community. In continual environments, datasets acquired under different conditions arrive sequentially; and each is only available for a limited period of time. Given the inherent privacy risks associated with medical data, this setup reflects the reality of deployment for deep learning diagnostic radiology systems. Many techniques exist to learn continuously for image classification, and several have been adapted to semantic segmentation. Yet most struggle to accumulate knowledge in a meaningful manner. Instead, they focus on preventing the problem of catastrophic forgetting, even when this reduces model plasticity and thereon burdens the training process. This puts into question whether the additional overhead of knowledge preservation is worth it - particularly for medical image segmentation, where computation requirements are already high - or if maintaining separate models would be a better solution. We propose UNEG, a simple and widely applicable multi-model benchmark that maintains separate segmentation and autoencoder networks for each training stage. The autoencoder is built from the same architecture as the segmentation network, which in our case is a full-resolution nnU-Net, to bypass any additional design decisions. During inference, the reconstruction error is used to select the most appropriate segmenter for each test image. Open this concept, we develop a fair evaluation scheme for different continual learning settings that moves beyond the prevention of catastrophic forgetting. Our results across three regions of interest (prostate, hippocampus, and right ventricle) show that UNEG outperforms several continual learning methods, reinforcing the need for strong baselines in continual learning research.

Abstract:While Deep Reinforcement Learning has been widely researched in medical imaging, the training and deployment of these models usually require powerful GPUs. Since imaging environments evolve rapidly and can be generated by edge devices, the algorithm is required to continually learn and adapt to changing environments, and adjust to low-compute devices. To this end, we developed three image coreset algorithms to compress and denoise medical images for selective experience replayed-based lifelong reinforcement learning. We implemented neighborhood averaging coreset, neighborhood sensitivity-based sampling coreset, and maximum entropy coreset on full-body DIXON water and DIXON fat MRI images. All three coresets produced 27x compression with excellent performance in localizing five anatomical landmarks: left knee, right trochanter, left kidney, spleen, and lung across both imaging environments. Maximum entropy coreset obtained the best performance of $11.97\pm 12.02$ average distance error, compared to the conventional lifelong learning framework's $19.24\pm 50.77$.

PILOT: Physics-Informed Learned Optimized Trajectories for Accelerated MRI

Abstract:Magnetic Resonance Imaging (MRI) has long been considered to be among "the gold standards" of diagnostic medical imaging. The long acquisition times, however, render MRI prone to motion artifacts, let alone their adverse contribution to the relative high costs of MRI examination. Over the last few decades, multiple studies have focused on the development of both physical and post-processing methods for accelerated acquisition of MRI scans. These two approaches, however, have so far been addressed separately. On the other hand, recent works in optical computational imaging have demonstrated growing success of concurrent learning-based design of data acquisition and image reconstruction schemes. Such schemes have already demonstrated substantial effectiveness, leading to considerably shorter acquisition times and improved quality of image reconstruction. Inspired by this initial success, in this work, we propose a novel approach to the learning of optimal schemes for conjoint acquisition and reconstruction of MRI scans, with the optimization carried out simultaneously with respect to the time-efficiency of data acquisition and the quality of resulting reconstructions. To be of a practical value, the schemes are encoded in the form of general k-space trajectories, whose associated magnetic gradients are constrained to obey a set of predefined hardware requirements (as defined in terms of, e.g., peak currents and maximum slew rates of magnetic gradients). With this proviso in mind, we propose a novel algorithm for the end-to-end training of a combined acquisition-reconstruction pipeline using a deep neural network with differentiable forward- and back-propagation operators. We demonstrate its effectiveness on image reconstruction and image segmentation tasks, reporting substantial improvements in terms of acceleration factors as well as the quality of these tasks.

Abstract:Addressing catastrophic forgetting is one of the key challenges in continual learning where machine learning systems are trained with sequential or streaming tasks. Despite recent remarkable progress in state-of-the-art deep learning, deep neural networks (DNNs) are still plagued with the catastrophic forgetting problem. This paper presents a conceptually simple yet general and effective framework for handling catastrophic forgetting in continual learning with DNNs. The proposed method consists of two components: a neural structure optimization component and a parameter learning and/or fine-tuning component. By separating the explicit neural structure learning and the parameter estimation, not only is the proposed method capable of evolving neural structures in an intuitively meaningful way, but also shows strong capabilities of alleviating catastrophic forgetting in experiments. Furthermore, the proposed method outperforms all other baselines on the permuted MNIST dataset, the split CIFAR100 dataset and the Visual Domain Decathlon dataset in continual learning setting.

Continual learning for seizure prediction via memory projection strategy

Abstract:Despite extensive algorithms for epilepsy prediction via machine learning, most models are tailored for offline scenarios and cannot handle actual scenarios where data changes over time. Catastrophic forgetting(CF) for learned electroencephalogram(EEG) data occurs when EEG changes dynamically in the clinical setting. This paper implements a continual learning(CL) strategy Memory Projection(MP) for epilepsy prediction, which can be combined with other algorithms to avoid CF. Such a strategy enables the model to learn EEG data from each patient in dynamic subspaces with weak correlation layer by layer to minimize interference and promote knowledge transfer. Regularization Loss Reconstruction Algorithm and Matrix Dimensionality Reduction Algorithm are introduced into the core of MP. Experimental results show that MP exhibits excellent performance and low forgetting rates in sequential learning of seizure prediction. The forgetting rate of accuracy and sensitivity under multiple experiments are below 5%. When learning from multi-center datasets, the forgetting rates for accuracy and sensitivity decrease to 0.65% and 1.86%, making it comparable to state-of-the-art CL strategies. Through ablation experiments, we have analyzed that MP can operate with minimal storage and computational cost, which demonstrates practical potential for seizure prediction in clinical scenarios.
Distribution-Aware Replay for Continual MRI Segmentation

Abstract:Medical image distributions shift constantly due to changes in patient population and discrepancies in image acquisition. These distribution changes result in performance deterioration; deterioration that continual learning aims to alleviate. However, only adaptation with data rehearsal strategies yields practically desirable performance for medical image segmentation. Such rehearsal violates patient privacy and, as most continual learning approaches, overlooks unexpected changes from out-of-distribution instances. To transcend both of these challenges, we introduce a distribution-aware replay strategy that mitigates forgetting through auto-encoding of features, while simultaneously leveraging the learned distribution of features to detect model failure. We provide empirical corroboration on hippocampus and prostate MRI segmentation.

DynaMMo: Dynamic Model Merging for Efficient Class Incremental Learning for Medical Images

Abstract:Continual learning, the ability to acquire knowledge from new data while retaining previously learned information, is a fundamental challenge in machine learning. Various approaches, including memory replay, knowledge distillation, model regularization, and dynamic network expansion, have been proposed to address this issue. Thus far, dynamic network expansion methods have achieved state-of-the-art performance at the cost of incurring significant computational overhead. This is due to the need for additional model buffers, which makes it less feasible in resource-constrained settings, particularly in the medical domain. To overcome this challenge, we propose Dynamic Model Merging, DynaMMo, a method that merges multiple networks at different stages of model training to achieve better computational efficiency. Specifically, we employ lightweight learnable modules for each task and combine them into a unified model to minimize computational overhead. DynaMMo achieves this without compromising performance, offering a cost-effective solution for continual learning in medical applications. We evaluate DynaMMo on three publicly available datasets, demonstrating its effectiveness compared to existing approaches. DynaMMo offers around 10-fold reduction in GFLOPS with a small drop of 2.76 in average accuracy when compared to state-of-the-art dynamic-based approaches. Preventing Catastrophic Forgetting through Memory Networks in Continuous Detection

Abstract:Modern pre-trained architectures struggle to retain previous information while undergoing continuous fine-tuning on new tasks. Despite notable progress in continual classification, systems designed for complex vision tasks such as detection or segmentation still struggle to attain satisfactory performance. In this work, we introduce a memory-based detection transformer architecture to adapt a pre-trained DETR-style detector to new tasks while preserving knowledge from previous tasks. We propose a novel localized query function for efficient information retrieval from memory units, aiming to minimize forgetting. Furthermore, we identify a fundamental challenge in continual detection referred to as background relegation. This arises when object categories from earlier tasks reappear in future tasks, potentially without labels, leading them to be implicitly treated as background. This is an inevitable issue in continual detection or segmentation. The introduced continual optimization technique effectively tackles this challenge. Finally, we assess the performance of our proposed system on continual detection benchmarks and demonstrate that our approach surpasses the performance of existing state-of-the-art resulting in 5-7% improvements on MS-COCO and PASCAL-VOC on the task of continual detection.

Abstract:Deep learning is increasingly used in medical imaging, improving many steps of the processing chain, from acquisition to segmentation and anomaly detection to outcome prediction. Yet significant challenges remain: (i) image-based diagnosis depends on the spatial relationships between local patterns, something convolution and pooling often do not capture adequately; (ii) data augmentation, the de facto method for learning three-dimensional pose invariance, requires exponentially many points to achieve robust improvement; (iii) labelled medical images are much less abundant than unlabelled ones, especially for heterogeneous pathological cases; and (iv) scanning technologies such as magnetic resonance imaging can be slow and costly, generally without online learning abilities to focus on regions of clinical interest. To address these challenges, novel algorithmic and hardware approaches are needed for deep learning to reach its full potential in medical imaging.
CCSI: Continual Class-Specific Impression for data-free class incremental learning

Abstract:In real-world clinical settings, traditional deep learning-based classification methods struggle with diagnosing newly introduced disease types because they require samples from all disease classes for offline training. Class incremental learning offers a promising solution by adapting a deep network trained on specific disease classes to handle new diseases. However, catastrophic forgetting occurs, decreasing the performance of earlier classes when adapting the model to new data. Prior proposed methodologies to overcome this require perpetual storage of previous samples, posing potential practical concerns regarding privacy and storage regulations in healthcare. To this end, we propose a novel data-free class incremental learning framework that utilizes data synthesis on learned classes instead of data storage from previous classes. Our key contributions include acquiring synthetic data known as Continual Class-Specific Impression ( CCSI ) for previously inaccessible trained classes and presenting a methodology to effectively utilize this data for updating networks when introducing new classes. We obtain CCSI by employing data inversion over gradients of the trained classification model on previous classes starting from the mean image of each class inspired by common landmarks shared among medical images and utilizing continual normalization layers statistics as a regularizer in this pixel-wise optimization process. Subsequently, we update the network by combining the synthesized data with new class data and incorporate several losses, including an intra-domain contrastive loss to generalize the deep network trained on the synthesized data to real data, a margin loss to increase separation among previous classes and new ones, and a cosine-normalized cross-entropy loss to alleviate the adverse effects of imbalanced distributions in training data. Extensive experiments show that the proposed framework achieves state-of-the-art performance on four of the public MedMNIST datasets and in-house echocardiography cine series, with an improvement in classification accuracy of up to 51% compared to baseline data-free methods.
