Abstract:Introduction Transcatheter aortic valve replacement (TAVR) has gained steam within the last decade and widened the patient population eligible for aortic valve replacement. When compared to the surgical aortic valve replacement approach (SAVR), TAVR approach has been documented with the least incidence of complications and shorter length of hospital stay in the appropriate patient demographic. Objective We aim to estimate the five-year readmission trends, burden, and outcomes on heart failure patients who had TAVR procedure. Methods We retrospectively analyzed the readmissions tracked in the National Readmission database over a five year period from January 1, 2016, to December 31, 2020. Patients with an index admission and readmission corresponding ICD codes for TAVR and heart failure were used. Parameters of statistical interest included fatality rate in index admission, readmission rate, etiologies of readmission, and predictor of readmission, all these outcomes were stratified by the index year. Results About 315,937 index admissions for TAVR were recorded with an estimated 0.04% (106) all-cause readmission within 30-days of discharge. The average index admission fatality rate within the five-year period was 0.99%. Valvular stenosis (27.80%), valve leakage (12.27%), and other mechanical valve complications (18.72%) were the most common causes of readmission. A 30-day readmission due to heart failure were seldom and figured among the top three in 2016 (2.09%). Despite almost a twofold increase in the number of TAVR performed from 2016 to 2020, the 30-day readmission rate has remained steady, and the index fatality rate trended downwards. Conclusion About 1 in 100 patients who had TAVR procedure suffered mortality during their index admission. The major cause of readmission in the five year period studied was valvular stenosis and valve leakage. Heart failure readmissions in the TAVR population were already on a downward trend based on earlier data. More information is needed to understand the role played by improvement in procedures and the vulgarization of SGLT2 inhibitors around 2014.
