Abstract:Purpose of review: This paper reviews recent surgical contributions to the multidisciplinary approach to head and neck cancer treatment as well as published data on pertinent questions in the field. Recent findings: The role of surgery in the strategy of organ preservation via chemoradiation, including treatments of complications and locoregional recurrences, such as when N1-N3 necks should be operated on and the role of CT and positron emission tomography (PET) scanning. Also covered are the role of sentinel lymph node biopsy for the detection of occult micrometastases in the N0 neck and the value of fluorodeoxyglucose PET scanning combined with the former. Transoral laser surgery for tumors of the larynx or of the hypopharynx is discussed, as is a new technique of reconstruction involving the anterolateral thigh free flap. Also included are quality of swallowing after resections and reconstruction of the oral cavity and orpharyngeal tumors. Summary: Surgery must define its role in the multidisciplinary treatment of advanced cancers, which currently often favors (chemo)radiotherapy protocols. Organ sparing by transoral laser surgery for laryngeal and hypopharyngeal cancer has established itself within our armamentarium and its exact role will be further refined. Larger and well-executed studies on sentinel lymph node biopsy in the clinically negative neck are needed before this procedure can be implemented in our routines. The latest addition, the anterolateral thigh flap, to further decrease patients' morbidity is unlikely to completely replace the currently favored radial forearm flap in soft tissue replacement of the head and neck. Outcome analysis, including swallowing, is a major step that surgeons should further investigate.

Abstract:Salvage surgeries of head and neck cancer are often complicated and do not always show decent results. This type of procedure is tough on the patient, as many crucial organs can be affected. A long period of reeducation usually follows the surgery because of the need to rehabilitate functions such as speech or swallowing. In order to lighten the journey of the patients, it is important to develop new technologies and techniques to ease the surgery and limit its damages. This seems even more crucial since progress has been made in the past years, allowing more salvage therapy to take place. This article aims at showing the available tools and procedures for salvage surgeries, such as transoral robotic surgery, free-flap surgery, sentinel node mapping, and many others, that help the work of the medical team to operate or obtain a better understanding of the status of the cancer when taken in charge. Yet, the surgical procedure is not the only thing determining the outcome of the operation. The patient themself and their cancer history also play an important part in the care and must be acknowledged.

Abstract:Oropharyngeal squamous cell carcinoma (OPSCC) related to human papillomavirus (HPV) infection has better survival outcomes compared to non-HPV-related OPSCC, leading to efforts to de-escalate the intensity of treatment to reduce associated morbidity. This article reviews recent clinical efforts to explore different de-escalation frameworks with a particular emphasis on the emergence of transoral robotic surgery and surgically driven de-escalation approaches. It discusses the current evidence for incorporating surgery into an evolving treatment paradigm for HPV-related OPSCC.

Inflammatory changes with features of chronic leukocytoclastic vasculitis in a Spitz nevus

Abstract:An 11-year old boy presented with a 5 mm slightly elevated, erythematous papule of the right lateral calf (Figure 1). He was unsure how long the lesion had been present. It was surgically removed. Histology revealed a well-circumscribed, rather symmetrical melanocytic proliferation with nests of spindled melanocytes at the dermal-epidermal junction and in the underlying dermis with maturation of deeper melanocytes. Some melanocytes, however, showed larger spindled, to some extent hyperchromatic nuclei with prominent nucleoli. A few mitotic figures were noticeable down to the middle part of the lesion and Ki67 showed an increased proliferation rate. Further characterization in a histological reference center revealed normal expression of the cell cycle regulation proteins p16 and p21 which have been found down-regulated in malignant melanoma in contrast to Spitz nevi and can be additionally helpful in differentiating the two entities [1, 2]. Based on the unusual finding of deep mitotic figures and the increased proliferation rate seen, the lesion was diagnosed as atypical Spitz nevus. The melanocytic tumor was accompanied by an inflammatory infiltrate. Lymphocytes, neutrophils and eosinophils were distributed in a perivascular and interstitial fashion accompanied by neutrophilic nuclear dust and some endothelial swelling (Figures 2a, b). Features were that of a chronic leukocytoclastic vasculitis (LcV). Recently chronic LcV was reported in association with a basal cell carcinoma [3]. The authors discussed an interferon gamma (INFγ)-mediated up-regulation of the inflammation as pathogenic factor of LcV [4]. IFNγ has a known role in tumor immune surveillance and induces the expression of antigen-presenting molecules on endothelial cells, macrophages and keratinocytes. Furthermore it enhances leukocyte recruitment and macrophage activation. Clinically and histopathologically LcV has a broad spectrum of presentation dependent on acuity of the disease. Whereas early acute LcV may have only a sparse infiltration of neutrophils and nuclear debris around postcapillary venules, mature acute LcV shows denser inflammation and fibrin in the wall of venules [5]. By contrast, in fully developed chronic LcV, infiltrates of neutrophils with nuclear dust and eosinophils may contain larger amounts of lymphocytes, plasma cells, and sometimes macrophages [5]. At this stage, there is no fibrin in the wall of venules [5]. Clinical Letter Clinically, chronic LcV of the skin most often presents as granuloma faciale (GF) or erythema elevatum diutinum (EED) [6, 7]. The cause of development of chronic LcV is unknown. An antigen stimulus persisting in the sites of lesions for years may be the pathogenetic explanation. Our patient showed no clinical signs of GF or an extrafacial variant of an EED. His medical history was normal; specifically there was no evidence for a generalized leukocytoclastic vasculitis or an ‘interstitial neutrophilic dermatitis’ in the context of an underlying rheumatoid disease. As our case shows, chronic LcV can be an accompanying histopathological feature without recognizable clinical features. Even though our young patient had no history of preceding local trauma, folliculitis or insect sting, chronic irritation of further kind cannot be ruled out. The accompanying inflammatory reaction could account for the increased proliferation rate that alone should not be over-interpreted as indicator for malignancy.

Abstract:Head and neck microvascular reconstruction is a complex but powerful tool following immediate cancer ablation or delayed sequalae after cancer treatment. Virtual surgical planning (VSP), using computer-aided design (CAD)/computer-aided manufacturing (CAM) has emerged as a potentially beneficial adjunct to head and neck reconstruction. The aim of this study was to assess the outcomes using VSP technology using KLS Martin custom-made cutting guides and low-profile plates in head and neck microvascular reconstruction within our centre.A retrospective study was conducted at our regional head and neck centre in the U.K, from March 2017 until June 2023.A total of 127 patients were included. 77.8% of patients had an immediate reconstruction following a cancer ablative procedure, with 22.2% being treated for ORN. Median flap ischemia time was 130 mins (45-360 mins). Median number of osteotomies was 1(0-3). Median operative time was 622 mins (302-962 mins). Procedural complications occurred in 16.2%. Total flap failure rate was 6.35%. This study found that patients who were operated on with palliative intent had significantly worse flap survival rates (p<0.033). Length of hospital stay was significantly impacted by longer operative time (p<0.002) and overall length of ITU stay was significantly increased in diabetic and palliative patients.Three-dimensional planning software has become a standard of care within our centre, leading to shorter operative time, reliable and accurate reconstruction and a short learning curve for skill acquisition. Extensive surgery in palliative patients requires a robust consenting process in the context of a multi-disciplinary team discussion.

Abstract:Résumé La prise en charge des cancers de la tête et du cou est multidisciplinaire, reposant souvent sur l'utilisation de traitements combinés dans le parcours de soin pour maximiser les chances de guérison, est aussi responsable d'effets secondaires cumulatifs. La chirurgie reconstructrice avec lambeau a pour but de restaurer une fonction perdue avec la perte de substance de la résection tumorale. Elle pose cependant quelques questions dans ses articulations avec la radiothérapie postopératoire. L'optimisation des protocoles d'imagerie pour la planification de radiothérapie doit permettre d'identifier les remaniements postopératoires et de distinguer lambeaux des tissus natifs environnants pour délinéer les lambeaux et documenter l'évolution spontanée de ces lambeaux et les relations dose–effets de la radiothérapie, notamment sur atrophie, fibrose des lambeaux de tissus mous et ostéoradionécrose des lambeaux osseux. Il s'agit aussi de standardiser la définition des volumes cibles en situation de lambeau, une situation de plus en plus fréquente en soins courants. Cette évolution des pratiques, au-delà des incontournables réunions de concertation pluridisciplinaire définissant les indications de traitement, nécessite une confrontation technique radio chirurgicale pour anticiper les profils de rechute et de toxicités et éventuellement proposer des stratégies de désescalade personnalisée des traitements multimodaux à travers des essais interdisciplinaires spécialisés. Summary The management of head and neck cancers is multidisciplinary, often relying on the use of combined treatments to maximize the chances of cure. Combined treatments are however also responsible for cumulative side effects. The aim of reconstructive surgery with a flap is to restore a function lost with the loss of substance from the tumor resection. However, changes in reconstructive surgery have impact of postoperative radiotherapy planning. The optimization of imaging protocols for radiotherapy planning should make it possible to identify postoperative changes and to distinguish flaps from surrounding native tissues to delineate the flaps and document the spontaneous evolution of these flaps or dose-effect relationships in case of radiotherapy. Such changes include atrophy, fibrosis of soft tissue flaps and osteoradionecrosis of bone flaps. Radiotherapy optimization also involves standardization of the definition of target volumes in situations where a flap is present, a situation that is increasingly common in routine care. This evolution of practice, beyond the essential multidisciplinary consultation meetings defining treatment indications, requires a close radio surgical collaboration with respect to technical aspects of the two disciplines. Doing so, anticipation of relapse and toxicity profiles could possibly lead to propose strategies for personalized de-escalation of multimodal treatments through interdisciplinary trials.

Abstract:When the term "chronic paroxysmal hemicrania" (CPH) was coined, all three major characteristics of the headache were taken into consideration; this purely descriptive term did not allude to any etiological factors. In the headache cases that we examined at the time, the headache was chronic in the sense that attacks occurred every day. The attacks were clear-cut and short-lasting. The pain was not constant, hence the term paroxysmal, the pain was hemicranial (1).

Abstract:Head and neck cancer (HNC) is the 6th most common cancer across the world, with a particular increase in HNC associated with human papilloma virus (HPV) among younger populations. Historically, the standard treatment for this disease consisted of combined surgery and radiotherapy or curative platinum-based concurrent chemoradiotherapy, with associated long term and late toxicities. However, HPV-positive HNC is recognized as a unique cancer subtype, typically with improved clinical outcomes. As such, treatment de-escalation strategies have been widely researched to mitigate the adverse effects associated with the current standard of care without compromising efficacy. These strategies include treatment de-escalation, such as novel surgical techniques, alternative radiation technologies, radiation dose and volume reduction, as well as neoadjuvant chemotherapies, immunotherapies, and combined therapies. Although these therapies show great promise, many of them are still under investigation due to hesitation surrounding their widespread implementation. The objective of this review is to summarize the most recent progress in de-escalation strategies and neoadjuvant therapies designed for HPV-positive HNC. While specific treatments may require additional research before being widely adopted, encouraging results from recent studies have highlighted the advantages of neoadjuvant chemotherapy and immunotherapy, as well as radiation and surgical de-escalation approaches in managing HPV-positive HNC.

Abstract:A large extraoral fungus, frequently seen in late head and neck cancers, poses serious difficulties for the management of anesthesia and surgery. Essential factors include preoperative optimization, airway assessment, intraoperative monitoring, and postoperative care. Risk mitigation and outcome optimization strategies are discussed, including appropriate airway management and hemodynamic monitoring. Ideal patient outcomes in situations of extensive extraoral fungation can be attained by a complete plan that integrates surgical expertise and anesthetic care. This case discusses the successful anesthetic management of a 55-year-old man undergoing composite resection with segmental mandibulectomy, appropriate neck dissection, free fibular flap, and scalp flap for squamous cell carcinoma of the lower labial mucosa with significant extraoral fungation.

Porter (1963) by summarizing the very limited knowledge of plant cell ultrastructure that we had prior to that publication, and, by way of our three retrospective accounts, show how this paper stimulated and influenced subsequent research on plant microtubules. Micrographs of historical interest are presented that are either previously unpublished or from primary publications.
Reconstructive flap surgery in head and neck cancer patients: an interdisciplinary view of the challenges encountered by radiation oncologists in postoperative radiotherapy

Abstract:Background: Major advances have been made in reconstructive surgery in the last decades to reduce morbidity in head and neck cancer. Flaps are now present in 80% of patients with oral cavity cancer to cover anatomic, functional, and cosmetic needs. However, gaps in interdisciplinary innovation transfer from surgery to postoperative radiotherapy (poRT) remain challenging. We aimed to provide an interdisciplinary view of the challenges encountered by radiation oncologists in planning head and neck postoperative radiotherapy. Methods: A systematic and critical review was conducted to address areas of optimization in surgery and radiology that may be relevant to poRT. Results: Despite extensive surgical literature on flap techniques and salvage surgery, 13 retrospective series were identified, where flap outcomes were indirectly compared between surgery alone or poRT. These low-evidence studies suggest that radiotherapy accelerates flap atrophy, fibrosis, and osteoradionecrosis and deteriorates functional outcomes. Preliminary evidence suggests that tumor spread occurs at the flap–tissue junction rather than in the flaps. One prospective 15-patient study showed 31.3% vs. 39.2% flap volume reduction without or with poRT. In an international consensus, experts recognized the needs for optimized flap-sparing poRT against flap-related functional deterioration and bone damage. CT, MRI, and PET-CT modalities show potential for the delineation of the junction area between native tissues and flap for flap segmentation and to characterize flap-specific changes quantitatively and correlate them with patterns of relapse or complications. Conclusion: Flap management in poRT is insufficiently documented, but poRT seems to damage flaps. Current gaps in knowledge underscore the need for prospective flap assessment and interdisciplinary trials investigating flap morbidity minimization by flap-sparing poRT planning.

Abstract:Human papillomavirus (HPV)-positive oropharyngeal cancer (OPC) is increasing rapidly. The younger age, significantly improved prognosis, and relative morbidity of the standard-of-care cisplatin and radiotherapy in this population have led to the popularization of the concept of treatment de-escalation. The recent results of the first 3 randomized de-escalation trials, however, have shown a clear detriment in survival when cisplatin is omitted or substituted. In view of these results, the Head and Neck Cancer International Group identified the need to issue guidance regarding future de-escalation studies for patients with HPV-positive head and neck cancer to avoid the possibility of patients being harmed. We review the current state of the literature regarding HPV de-escalation trials and present a framework and guidance on future and existing clinical trials for treatment de-escalation of HPV-positive OPC. De-escalation paradigms of HPV-positive OPC should be evaluated in phase II studies, and results should be awaited before proceeding to phase III studies. Implementation into clinical practice before high-level evidence is available should not be undertaken in this context. Finally, harm-minimization techniques should also be evaluated as an alternative to de-escalation of treatment in these patient groups.

Abstract:Head and neck squamous cell carcinoma (HNSCC) constitutes a significant global cancer burden, given its high prevalence and associated mortality. Despite substantial progress in survival rates due to the enhanced multidisciplinary approach to treatment, these methods often lead to severe tissue damage, compromised function, and potential toxicity. Thus, there is an imperative need for novel, effective, and minimally damaging treatment modalities. Neoadjuvant treatment, an emerging therapeutic strategy, is designed to reduce tumor size and curtail distant metastasis prior to definitive intervention. Currently, neoadjuvant chemotherapy (NACT) has optimized the treatment approach for a subset of HNSCC patients, yet it has not produced a noticeable enhancement in overall survival (OS). In the contemporary cancer therapeutics landscape, immunotherapy is gaining traction at an accelerated pace. Notably, neoadjuvant immunotherapy (NAIT) has shown promising radiological and pathological responses, coupled with encouraging efficacy in several clinical trials. This potentially paves the way for a myriad of possibilities in treatment de-escalation of HNSCC, which warrants further exploration. This paper reviews the existing strategies and efficacies of neoadjuvant immune checkpoint inhibitors (ICIs), along with potential de-escalation strategies. Furthermore, the challenges encountered in the context of the de-escalation strategies of NAIT are explored. The aim is to inform future research directions that strive to improve the quality of life (QoL) for patients battling HNSCC.

Abstract:Cancer is emerging globally and remains the second common cause of mortality. It is estimated that up to 50% of inpatient admissions worldwide are for a diagnosis of cancer. With increasing cancer prevalence over time, it is expected that patients requiring surgical intervention under anesthesia shall also increase. Despite significant immunological advances in cancer care, surgery will continue to be a mainstay strategy for reducing tumor burden, particularly for solid tumors. Frequently, chemo-radiation therapies are administered before the surgical resection as neoadjuvant therapy or after the surgical resection as adjuvant therapies to provide optimal surgical resection and increase overall cancer-free survival. The routine presurgical evaluation and optimization remain an important parameter for better patient care. These relate to evaluating and optimizing the anatomic, physiological paraneoplastic, and treatment-related effects of cancer on different organ function. Anesthesia providers should therefore be cognizant of immediate, and long-term systemic effects of cancer therapies (organ toxicities), and the effects of chemo-radiation on nutrition, fatigue, anemia, and physical deconditioning all of which could influence the recovery profile after major surgery.

Abstract:Taking into account its phenotypical and genetic characteristics, Acetomicrobium faecale was first recognized as a member of the genus Acetomicrobium, family Bacteroidaceae, order Bacteroidales, phylum Bacteroidetes, with Acetomicrobium flavidum the type species of the genus. However, it was found that A. faecale had 95.8 %, 97.6 % and 98.4 % similarity, respectively, with Caldicoprobacter guelmensis, Caldicoprobacter algeriensis and Caldicoprobacter oshimai and only 82 % similarity with A. flavidum. The DNA G+C content of A. faecale is 45 mol , which is of the same order as the DNA G+C content of the three strains of species of the genus Caldicoprobacter and its main fatty acid is C16 : 0, with its second most prominent fatty acid, iso-C17 : 0, also common to strains of species of the genus Caldicoprobacter. On the basis of further phylogenetic, genetic and chemotaxonomic studies, we propose that A. faecale (type strain DSM 20678T = JCM 30420T) be reclassified as Caldicoprobacter faecalis comb. nov.

Summary: Virtual planning and rapid prototype modeling are increasingly used in head and neck reconstruction with the aim of achieving superior surgical outcomes in functionally and aesthetically critical areas of the head and neck compared with conventional reconstruction. The reconstructive surgeon must be able to understand this rapidly-advancing technology, along with its advantages and disadvantages. There is no limit to the degree to which patient-specific data may be integrated into the virtual planning process. For example, vascular mapping can be incorporated into virtual planning of mandible or midface reconstruction. Representative mandible and midface cases are presented to illustrate the process of virtual planning. Although virtual planning has become helpful in head and neck reconstruction, its routine use may be limited by logistic challenges, increased acquisition costs, and limited flexibility for intraoperative modifications. Nevertheless, the authors believe that the superior functional and aesthetic results realized with virtual planning outweigh the limitations.
Early assessment of feasibility and technical specificities of transoral robotic surgery using the da Vinci Xi

Abstract:The latest generation Da Vinci® Xi™ Surgical System Robot released has not been evaluated to date in transoral surgery for head and neck cancers. We report here the 1-year results of a non-randomized phase II multicentric prospective trial aimed at assessing its feasibility and technical specificities. Our primary objective was to evaluate the feasibility of transoral robotic surgery using the da Vinci® Xi™ Surgical System Robot. The secondary objective was to assess peroperative outcomes. Twenty-seven patients, mean age 62.7 years, were included between May 2015 and June 2016 with tumors affecting the following sites: oropharynx (n = 21), larynx (n = 4), hypopharynx (n = 1), parapharyngeal space (n = 1). Eighteen patients were included for primary treatment, three for a local recurrence, and six for cancer in a previously irradiated field. Three were reconstructed with a FAMM flap and 6 with a free ALT flap. The mean docking time was 12 min. "Chopsticking" of surgical instruments was very rare. During hospitalization following surgery, 3 patients experienced significant bleeding between day 8 and 9 that required surgical transoral hemostasis (n = 1) or endovascular embolization (n = 2). Transoral robotic surgery using the da Vinci® Xi™ Surgical System Robot proved feasible with technological improvements compared to previous generation surgical system robots and with a similar postoperative course. Further technological progress is expected to be of significant benefit to the patients.
Robotic Neck Dissection in Head and Neck Cancer via Modified BABA Technique

Abstract:Introduction Neck dissection forms an important component in the surgical management of head and neck cancers. By using the conventional techniques of neck dissection, a conspicuous scar is inevitable for the patients. The development of robotic assisted neck dissection provides for a scar‐free neck along with good oncological and functional outcomes. Methods A prospective observational study was conducted in our institute from 2020 March to 2022 March, where robotic‐assisted neck dissections performed via the modified bilateral axillo‐breast insufflation technique. Results Eighty‐two patients underwent robotic neck dissections in our institute. Notably, 79 patients were treatment‐naïve. The average docking time was 12 min and console time was 160 ± 15 min. The mean lymph node yield was 28.2. The average post‐operative stay was 5.6 days. The average follow‐up was noted to be 6.4 months. The mean cosmetic satisfaction score in our patients was 4.45. Only one patient presented with nodal recurrence, who was identified as a defaulter for adjuvant treatment. Robotic neck dissection gives similar functional and oncological outcomes as compared with conventional neck dissection. Patients had excellent cosmetic satisfaction following the procedure. The limitations of these techniques include high cost of procedure and longer operating time. This is a level IV evidence study. Conclusion Although good oncological, functional, and cosmetic outcomes have been attained in robotic assisted neck dissection, further randomized controlled studies need to be conducted to justify the added costs, cosmetic advantage, and the time taken.
