Abstract:Objective To study the correlation of HBV infection pretransplantation and posttrans-plantation and HCMV recurrence after liver transplantation (LT). Methods We reviewed historical patient medical records of LT patients in recent two years in our hospital. All the patients were divided into HBV in-fection group and a control group based on a peripheral blood HB antigen assay before LT. The HBV infec-tion group was divided into HBV reactive infection group and HBV non-relapse group. HCMV antigen pp~65 was detected by immunohistochemical methods. HB antigens and antibodies were detected by time-resolved fluorescence immunoassay, and liver enzyme levels were detected by conventional methods. Results Com-paring two groups of patients, pp65-positive rates of LT patients with HBV infection and control group pa-tients were 84.3% and 57.9% respectively (P=0.024). While in HBV recurrence infection group and non-recurrence infection group, the incidences of HCMV recurrence were 90.9% and 83.3% (P=0.843). The changes in the liver transaminases level in both groups have no statistical significance (P>0.05). Conclusion Pretransplantation HBV infection may increase the incidence of HCMV recurrence. Posttrans-plantation HBV reactive infection, however, may not increase the incidence of HCMV reactive infection. Meanwhile, compare with either HBV infection or HCMV infection alone, co-infection may not serious in liv-er enzymes levels.
Analysis of Correlative Factors in Infection of Liver Transplant Recipients.

Abstract:Objective To report the incidence, types and risk factors of infections in liver transplant recipients and to present our experience in the management of these infections. Methods A retrospective study ofall patients who presented with infections after liver transplantation from October 1991 to August 1998 in QueenMary Hospital, Hong Kong,was carried out. Results Eighty liver transplantation operations were performed in 78patients Infeetions developed in 43(54% ) cases with a total of 79 episodes (a mean of 1. 8 episodes in eachcase. Among the 79 infections, 56 were bacterial, 13 were viral, 7 were fungal and 3 were mycobacterial. Seventy-eight percent (61/79 ) of the infections occurred in the first month after liver transplantation. Two patientsdied of severe infection within the first year after transplantation. One patient died of malignant lymphoma related toEpstein-Barr virus (EBV) infection thirteen months after liver transplantation. The mortality rate related to infectionwas 4 %. Missive intraoperative blood transfusion and surgical technical errors were correlated with bacterial infection (P0. 05). No patient (0/30) in the FK506-treated group 'developed cytomegalovirus (CMV) infection,compared with 12 % (6/50 ) of the patlents who were treated with cyclosporine (P 0. 05). Conclusions Infection is a major complication after liver transplantation, generally occurring in the first month. Increased intraoperative blood transfusion requirement and surgical technical errors may be responsible for bacterial infection. There isa lower incidence of CMV infection in the patients who received FK506 treatment.
Pulmonary Infection after Liver Transplantation:A Clinical Analysis

Abstract:OBJECTIVE To analyze the risk factors of pulmonary infection after liver transplantation and improve treatment strategy.METHODS Forty six adult patients who underwent liver transplantation were divided into two groups: pulmonary infection group and non-pulmonary infection group.An analysis was performed for the commonly used variables.RESULTS The frequency of pulmonary infection after liver transplantation was 43.5%,the mortality rate in the patients who developed pulmonary infection was 60%.Intraoperative total fluid perfusion, mechanical ventilatory time,serum creatinine,albumin,abdominal bleeding,and hydrothorax after liver transplantation were risk factors of pulmonary infection(P<0.05).CONCLUSIONS The mortality of pulmonary infection after liver transplantation is high.Controlling intraoperative fluid perfusion,extubating as soon as possible,preventing renal dysfunction,improving nutrition,and dealing with hydrothorax and abdominal bleeding in time are important factors to prevent pulmonary infection after liver transplantation.

Abstract:Invasive fungal infections (IFIs) in immunocompromised patients, particularly liver transplant recipients, are the subject of increasing clinical attention. Although the overall incidence of fungal infections in liver transplant recipients has declined due to the early treatment of high-risk patients, the overall mortality rate remains high, particularly for invasive candidiasis and aspergillosis. IFIs after liver transplantation are strongly associated with negative outcomes, increasing the cost to recipients. Numerous studies have attempted to determine the independent risk factors related to IFIs and to reduce the morbidity and mortality with empirical antifungal prophylaxis after liver transplantation. Unfortunately, fungal infections are often diagnosed too late; symptoms can be mild and non-specific even with dissemination. Currently, no consensus exists on which patients should receive antifungal prophylaxis, when prophylaxis should be given, which antifungal agents should be used, and what duration is effective. This review highlights the types of IFI, risk factors, diagnosis, antifungal prophylaxis, and treatment after liver transplantation. With the early identification of patients at high risk for IFIs and the development of new molecular diagnostic techniques for early detection, the role of antifungal compounds in fungal infection prophylaxis needs to be established to improve the survival rate and quality of life in liver transplant patients.
Impact of preoperative infection on the outcomes of liver transplant recipients: a national propensity score-matched retrospective cohort study in China

Abstract:Background: Impact of preoperative infection on liver transplantation (LT) needs further investigation. Materials and Methods: From January 1, 2015 to December 31, 2022, 24,122 eligible patients receiving LT were enrolled from the China Liver Transplant Registry database. The outcomes of LT were compared after using the propensity score-matched analysis. Results: Compared to the non-infection group, patients in the infection group were more likely to have postoperative effusion, infection, abdominal bleeding and biliary complications (all P <0.01), and they had shorter 30-day, 90-day survival and overall survival (OS) (all P <0.01). Cox proportional hazards regression analysis revealed that MELD score and cold ischemia time were risk factors for the OS in the infection group (both P <0.05). Besides, compared to the non-pulmonary group, patients in the pulmonary group were more likely to have postoperative effusion and infection (both P <0.0001), and less likely to have postoperative abscess and early allograft dysfunction (both P <0.05). Patients in the non-abdominal group also had a higher proportion of postoperative infection than those in the abdominal group ( P <0.05). Furthermore, compared to the number=1 group, patients in the number≥2 group were more prone to postoperative effusion and infection (both P <0.01), and they also had shorter 30-day and 90-day survival (both P <0.05). Conclusion: Preoperative infection can result in a higher incidence of early postoperative complications and shorter survival in liver transplant recipients. The types and number of infection sites will also influence the prognosis of liver transplant recipients.

Pulmonary Infection and Its Risk Factors after Orthotopic Liver Transplantation

Abstract:OBJECTIVES:To investigate the characteristics of pulmonary infection and its risk factors after orthotopic liver transplantation (OLT).METHODS:Clinical data of 250 cases having liver transplantations from April 2001 to August 2005 were retrospectively studied in order to analyse the differences between patients with and without pulmonary infection.RESULTS:Fifty-seven (57/250, 22.8%) recipients had 72 episodes of pulmonary infection after liver transplantation. Bacterial infection was the most common followed by fungal infection (13/72, 18.1%), and cytomegalovirus infection (12/72, 16.7%). There were 36 episodes of pulmonary infection caused by one kind of bacteria, 5 episodes by two kinds of bacteria and 6 episodes by multiple kinds of bacteria. Seven episodes of fungal infection were accompanied with bacterial infection, and three episodes of cytomegalovirus infection were accompanied with bacterial infection simultaneously. The 1-, 2- and 3- year survival rates were 71.9%, 61.4%, and 53.4% of the patients with pulmonary infection and 93.1%, 75.8%, and 67.2% of those without the infection. Logistic regression analysis suggested that preoperative infection, mechanical ventilation > 12 hours, a long duration of the operation, total volume of blood transfusion during operation >1000 ml, reoperation after OLT, postoperative pleural effusion and the duration of stay in the intensive care unit were independent risk factors of pulmonary infection after OLT.CONCLUSION:Bacterial infections were the main pulmonary infection after OLT and the infections caused by multiple pathogens or multiple-antibiotic-resistant bacteria were seen more frequently. The risk factors of pulmonary infection should be controlled to decrease the infection rate after OLT. It is important to make a correct diagnosis for pulmonary infection after OLT and use appropriate antibiotics as soon as possible.

Abstract:Background: Infection is a significant risk factor that impacts for perioperative morbidity and mortality in liver transplantation (LTx) patients and is difficult to evaluate quantitatively in the early posttransplantation period. Thus, a biomarker to assess the risk of infection and the prognosis of the recipient is highly desirable.Methods: A total of 128 consecutive patients with end-stage liver diseases undergoing LTx between January 1, 2020 and December 31, 2021, at the First Affiliated Hospital of Zhejiang University School of Medicine, were screened retrospectively. Graft preservation fluid and blood samples were collected for culture, and other perioperative laboratory examination results were recorded, for assessment of infection status.Results: After a follow-up period of 30 days, the survival rate among the 128 LTx recipients was 94.5%. Multivariable regression analysis showed that the logarithmically transformed neutrophil-to-lymphocyte ratio (NLR) (HR = 3.548, 95% CI: ; p = .041) on post-LTx day 1 and graft preservation fluid culture positivity (HR = 12.032, 95% CI: ; p = .006) were independent predictive factors for early prognosis after LTx.Conclusions: Positive graft preservation fluid culture and the logarithmically transformed NLR on post-LTx day 1 were independent predictive factors for early prognosis after LTx. The logarithmically transformed NLR could provide an earlier indication than culture results in clinical practice.
Bacteriological, Drug-Resistant and Correlative Analysis of Infection after Liver Transplantation

Abstract:Objective To summarize the rules of infection after liver transplantation and provide evidence for its prevention and treatment. Methods The bacterial, antibiotic-susceptibility and clinical data of 23 patients receiving liver transplantation in our hospital were retrospectively analyzed. Meanwhile, the relationship between bacterial infection and some perioperative factors was determined. Results 1) The most common pathogens for the infection were staphylococcus aureus, Pseudomonas aeruginosa, Enterobacter cloacae, Escherichia coli and Enterococcus and the infection took place in 1 month after liver transplantation in most of the patients. 2) Most of the pathogens were resistant to broad-spectrum antibiotics. 3) The factors being related to the infection included existence of cirrhosis, albumin level before transplantation, presence of infection before transplantation, duration of operation, volume RBC transfused during the operation, duration of staying in ICU and use of ventilator. 4) The recipients of piggy-back liver transplantation seemed to be easier to suffer from intestinal bacterial sepsis than those of orthotopic liver transplantation. Conclusions Most of the bacteria responsible for postoperative infection in recipients of liver transplantation are multi-drug resistant. Cefoperazone/sulbactam, cefepime, carbopenems and glycopeptides are better antibiotics for treatment of the infection.
Analysis of High-Risk Factors for Early Pulmonary Bacterial Infection after Lung Transplantation and Their Correlation with Long-Term Mortality

Abstract:Objective: To investigate the high-risk factors for early pulmonary bacterial infection following lung transplantation and their association with long-term mortality. Methods: A retrospective analysis was conducted on 142 lung transplant recipients treated at Wuxi People's Hospital between January 2018 and July 2022. After applying predefined inclusion and exclusion criteria, 111 cases were analyzed. Univariate and multivariate logistic regression analyses were performed to identify independent risk factors for early pulmonary infection post-transplantation. Additionally, univariate and multivariate Cox regression analyses were used to identify independent prognostic facin donor lungs, and operation duration as risk factors for early pulmonary infection (all P < 0.05). Multivariate analysis was confirmatory for these as independent risk factors (all P < 0.05). Univariate analysis also showed that intraoperative blood loss and oxygenation index impacted one-year survival (P < 0.05). Multivariate analysis was confirmatory for these as independent risk factors (P < 0.05). Conclusion: Early pulmonary bacterial infection was not found to be an independent factor affecting 1-year survival. However, substantial intraoperative blood loss and a reduced oxygenation index were identified as independent risk factors associated with increased mortality within 1 year post-transplantation.
Infection within 2 weeks before liver transplantation closely related to prognosis of posttransplant infection: A single-center retrospective observational study in China

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Infections still represent the main factors influencing morbidity and mortality following liver transplantation. This study aimed to evaluate the incidence and risk factors for infection and survival after liver transplantation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We retrospectively examined medical records in 210 liver recipients who underwent liver transplantation between April 2015 and October 2017 in our hospital. Clinical manifestations and results of pathogen detection test were used to define infection. We analyzed the prevalence, risk factors and prognosis of patients with infection.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The median follow-up was 214 days; the incidence of infection after liver transplantation was 46.7% (<em>n</em>=98) which included pneumonia (43.4%), biliary tract infection (21.9%), peritonitis (21.4%) and bloodstream infection (7.6%). Among the pathogens in pneumonia, the most frequently isolated was <em>Acinetobacter baumanii</em> (23.5%) and <em>Klebsiella pneumoniae</em> (21.2%). Model for end-stage liver disease (MELD) score (OR=1.083, 95% CI: 1.045-1.123; <em>P</em><0.001), biliary complication (OR=4.725, 95% CI: 1.119-19.947; <em>P</em>=0.035) and duration of drainage tube (OR=1.040, 95% CI: 1.007-1.074; <em>P</em>=0.017) were independent risk factors for posttransplant infection. All-cause mortality was 11.0% (<em>n</em>=23). The prognostic factors for postoperative infection in liver recipients were prior-transplant infection, especially pneumonia within 2 weeks before transplantation. Kaplan-Meier curves of survival showed that recipients within 2 weeks prior infection had a significantly lower cumulative survival rate compared with those without infection (65.2% vs. 90.0%; hazard ratio: 4.480; <em>P</em><0.001).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Infection, especially pneumonia within 2 weeks before transplantation, complication with impaired renal function and MELD score after 7 days of transplantation was an independent prognostic factor for postoperative infection in liver transplant recipients.</p>

Research on the correlation between contamination of organ preservation fluid and infection after liver transplantation

Abstract:Objective: To investigate the etiology of organ preservation solution was detected by second-generation sequencing and traditional culture methods, and to study the relationship between donor liver preservation solution and post-transplant infection. Methods: A total of 40 consecutive patients with liver transplantation were enrolled from April to August 2017 in the Department of General Surgery, Huashan Hospital Fudan University. The organ preservation solution was collected, detected by second-generation sequencing technology, and cultured simultaneously. At the same time, the history of patients after transplantation was collected to diagnosis the infection after transplantation. Results: 76 pathogens were detected in 34 (85%) samples of preservation solution by next generation sequencing, including 12 (30%) pathogenic microorganisms, 22 (55%) opportunistic pathogenic microorganisms and 6 (15%) non-pathogenic microorganisms. The detected microorganisms were all bacteria except 1 virus and 1 fungus. The positive rate of preservation liquid culture was 37.5% (15 samples), and 24 pathogens were detected. Among them, 13 were fungi and 11 were bacteria. The cultures of 8 pathogens were consistent with the results of next generation sequencing. Preventive antibiotics were administered to all patients whose pathogens were detected by next generation sequencing or culture. Comparing the results of next generation sequencing and clinical history of peripheral blood before and after transplantation, 22 infections occurred in 20 patients within 30 days after transplantation, only one of which was consistent with the pathogens detected in organ preservation solution. At the same time, the detection of pathogens in donor liver preservation fluid was associated with an increased incidence of focal or systemic infection after transplantation (p = 0.026). Conclusion: Pathogens are commonly detected in organ preservation solution during liver transplantation. Targeted antibiotic therapy can partly prevent donor-derived infection after liver transplantation, but contamination of preservation fluid may increase the overall incidence of infection after transplantation.
Retrospective Study on the Postoperative Respiratory Complications in Adult Liver Transplant Recipients

Abstract:Purpose To explore the types,the incidence,and the causes of early postoperative (3 months) respiratory complications in adult liver transplant (LT) recipients. Methods We reviewed the data of 49 consecutive adult LT recipients.Pulmonary atelectasis,pleural effusion,pneumonia,respiratory failure,and pulmonary edema were the respiratory complications investigated. Results Thirty-four patients (69.4%) developed at least one respiratory complication during the early postoperative period.The most frequent complication was pleural effusion (n=26,53.1%),followed by pneumonia (n=8,16.3%),atelectasis (n=6,12.3%),acute respiratory failure (n=5,10.2%),pulmonary edema (n=2,4.1%).Compared to the patients who did not develop these problems,the affected cohort showed significantly higher mortality (P=0.048) ,had longer prothrombin time after operation (P=0.023),and was required more time to extubation(P=0.042).Only three patients who suffered pleural effusion required drainage. Conclusions Reducing the risk of early postoperative respiratory complications,early diagnosis and treatment of these problems are crucial for LT recipient survival.
DCD Liver Transplant Infection: Experience from a Single Centre in China.

Abstract:Aim: The purpose of our study was to evaluate the incidence, timing, location and risk factors for bacterial and fungal infections after donation after cardiac death (DCD) liver transplant and clearly delineate any relationship between infection and survival in DCD liver transplant recipients. Methods: We retrospectively reviewed 257 consecutive patients undergoing DCD liver transplant between October 2010 and May 2015 at our centre. Results: A total of 133 patients (51.8%) developed at least one bacterial or fungal infection episode. The predominant infection site was the respiratory tract, followed by the blood stream. Most of the infections occurred within the first week after liver transplant (61.9%). A recipient respiratory support time greater than 7 days (p = 0.041), post-transplant hospital time greater than 24 days (p = 0.002) and renal failure after DCD liver transplant (p = 0.039) were independent predictors of bacterial and fungal infection. The area under the receiver operating characteristic (ROC) curve (AUC) of the transplant infection risk assessment model was 0.788. The 1- and 3-year survival rates for recipients without infection were significantly increased compared with recipients with infection (96.1% and 89.0% vs. 81.5% and 75.9%, p = 0.007). Conclusion: This is the first study that offers detailed data revealing the timing and incidence of bacterial and fungal infection among adult DCD liver transplant recipients. Bacterial and fungal infection occurs at a high rate during the first week after DCD liver transplant, especially in patients with prolonged respiratory support time and renal failure, and infection is related to increased hospital stay.
LYMPHOCYTOPENIA AND NEUTROPHILIA DETERMINED FUNGAL INFECTION OCCURRENCE AND MORTALITY AFTER LIVER TRANSPLANTATION

Abstract:Introduction: Fungal infection in liver recipients remains a critical challenge with high mortality and morbidity. Immunosuppression regimen renders patients more vulnerable to fungal infection after liver transplantation. However, the mechanisms of immune status and its role in predicting post-transplant mortality are still undetermined. Materials and Methods: Adult patients received liver transplantation were recruited in a retrospective cohort from February 2018 to October 2019 in Beijing Tsinghua Changgung Hospital. The post-transplant immune cell subpopulations and peri-operation demographics were analyzed in patients with or without post-transplant fungal infections. Univariate and multivariate logistic regression were performed to identify risk factors for post-transplant fungal infections. The correlation between recipient immune status and post-transplant 90-day mortality was further investigated by ROC and Kaplan-Meier analysis. Results and Discussion: Thirty-six out of 164 (22%) patients were presented post-transplant fungal infections with median occurrence time of POD4 (1-287). In terms of fungi species, 27 (75%) were from Candida species, 6 (16.7%) were from Aspergillus species, and 3 (8.3%) were other fungal infections. And the infection site distributed in upper respiratory tract (17, 47.2%), lower respiratory tract (2, 5.6%), intra-abdominal cavity (11, 30.6%), bloodstream (3, 8.3%), urine (2, 5.6%) and wound (1, 2.8%). In comparison with non-infection patients, the lymphocyte population in blood was found significantly lower in fungal infection patients (5.0% vs 8.6%, p=0.007), while neutrophil increase was simultaneously observed (90.7% vs 83.3%, p=0.004). In addition, subpopulation analysis indicated the decrease of CD19+ B cell (26.2% vs 16.8%, p=0.069), but increase of CD3-CD16+CD56+ NK cell (7.0% vs 10.9%, p=0.001) and CD3+CD8+ CTL (20.9% vs 28.8%, p=0.120). By univariate and multivariate analysis, neutrophilia (percentage≥90.05%) was identified as an independent risk factor for post-transplant fungal infection (HR=3.529, 95%CI: 1.017-12.245, p=0.047, Table 1). By ROC analyses, lymphocytopenia and neutrophilia as well as the neutrophil-to-lymphocyte (N/L) ratio increase was found to have predictive value for 90-day mortality (Fig. 1A-C). And 90-day mortality divided by cut-off values of lymphocyte, neutrophil and N/L ratio was significantly different between groups (Fig. 1D-F). Conclusion: Altered immune status was the independent risk factor for fungal infections after liver transplantation. Lymphocytopenia and neutrophilia, as well as increased N/L ratio, were valuable predictors of fungal infection-related post-transplant mortality.
Risk factors of postoperative infection in liver transplantation patients

Abstract:Objective To investigate the risk factors of postoperative infection in patients with liver transplantation.Methods Clinical data and laboratory findings of 1 1 3 patients undergoing liver transplantation admitted in Beijing Chaoyang Hospital,Capital Medical University during January 201 1 and December 201 4 were retrospectively analyzed.Among 1 1 3 patients,postoperative infection occurred in 35 patients,multivariate Logistic regression analysis was performed to identify the risk factors of postoperative infection.Results Univariate analysis showed that length of hospital stay,model for end-stage liver disease (MELD)score,preoperative use of antibiotics,duration of postoperative respirator use,length of ICU stay, dosage of albumin,days of parenteral nutrition,intra-abdominal hemorrhage,fasting blood glucose,blood concentration of immunosuppressant (FK506 ),the duration of prophylactic use of antibiotics and pleural effusion were associated with postoperative infection (t =2.56,3.1 9,2.71 and 5.05;χ2 =3.87,5.75,4.66 and 5.46;Z =4.88,3.69,5.86 and 3.90;P <0.05 or <0.01 ).Multivariable logistic regression analysis showed that preoperative use of antibiotics (OR =35.03,95% CI:6.48 -94.64,P <0.01 ),duration of postoperative respirator use (OR =1 .02,95%CI:1 .01 -1 .04,P <0.01 ),days of parenteral nutrition (OR =1 .20,95%CI:1 .07 -1 .35,P <0.01 ),postoperative fasting plasma glucose(OR =1 .46,95%CI:1 .1 0 -2.1 6,P <0.05),the duration of prophylactic antibiotics use (OR =1 .1 0,95%CI:1 .33 -1 .86, P <0.05),and pleural effusion(OR =5.70,95%CI:1 .02 -31 .84,P <0.05 )were independent risk factors of postoperative infection.Conclusion Taken account of possible risk factors,effective prevention and control measures should be taken to prevent postoperative infection after liver transplantation.

Abstract:Objective To explore the microbial etiology of early onset pulmonary infection after liver transplantation.Methods The 40 episodes of early onset pulmonary infection in 75 patients receiving liver transplantation in our hospital were reviewed and analyzed retrospectively.Results The incidence rate of pulmonary infection was 53.30% in these patients.A total of 54 pathogens were isolated,including 41(75.9%)strains of bacteria and 13(24.1%)fungal isolates.Gram-negative bacillus ac- counted for 68.3%(n=28)of the bacterial isolates,11(39.3%)of which were positive for extended spectrumβ-lactamase. Gram-positive coccus accounted for 31.7%(n=13).Seven (58.3%)of the staphylococcal strains were methicillin-resistant. Conclusions The incidence of pulmonary infection is high early af- ter liver transplantation.Most of the isolated pathogens are drug resistant.Culture of sputum or secretion of lower respiratory tract and bedside X-ray examination are useful for the diagnosis.

Abstract:BACKGROUND: A bibliometric analysis was performed to reveal the current status of investigations in infectious diseases in patients with liver transplantation (LT) and to prioritize future research needs.METHODS: The present study comprehensively retrieved publications relevant to infectious diseases in LT recipients published between 2010 and 2020. The search was conducted on the Web of Science (WoS) database. A bibliometric analysis was conducted through machine learning and visualization tools, including VOSviewer, Bibliographic Item Co-Occurrence Matrix Builder, and Graphical Clustering Toolkit. Research hotspots and trends in the field were assessed, while the contributions and collaborations of countries, institutions, and authors were documented.RESULTS: A total of 691 publications were analyzed. Research output sharply increased in 2015, with a fast drop afterward. "Liver transplantation" was the most frequent keyword, with strong links to "hepatitis C virus" and "infection". Study areas included risk factors of infectious diseases in LT recipients, pathogens causing post-transplantation infections, antibacterial therapy and prophylaxis for peritransplant infection complications, living donor LT, and pediatric LT. The efficacy and safety of direct-acting antivirals (DAAs) for hepatitis C virus (HCV) infection among liver transplant recipients has attracted recent research interest. Didier Samuel was the most productive author, while Xavier Forns was the top-cited author. Shanghai Jiao Tong University was the most productive contributor, and Gilead Sciences was the most cited organization. Moreover, the USA was the greatest contributor. <i>Gastroenterology</i> was the most cited journal, while <i>Liver Transplantation</i> was the most prolific journal.CONCLUSIONS: This bibliometric analysis will better understand the research status of infectious complications in LT recipients and forecast future research trends. Priority should be given to identifying risk factors for peritransplantation infections and effective treatments against infectious complications in the coming years.

Abstract:BACKGROUND:Pulmonary complications after orthotopic liver transplantation (OLT) include high morbidity and mortality. Experimental data have suggested hepatic ischemia and reperfusion are induced by pro-inflammatory cytokines. The high level of inflammatory cytokines might additionally influence pulmonary capillary fluid filtration. The objectives of this study were to measure the concentrations of tumor necrotic factor-alpha (TNF-alpha), interleukin-6 (IL-6) and interleukin-8 (IL-8) during OLT and to investigate the relationship between these cytokines and postoperative pulmonary complications.METHODS:Twenty-two patients undergoing OLT were divided into two groups according to whether they had postoperative pulmonary complications: group A consisting of 8 patients with postoperative pulmonary complications, and group B consisting of 14 patients without postoperative pulmonary complications. Enzyme-linked immunoassay (ELISA) was used to determine serum TNF-alpha, IL-6 and IL-8. Blood samples were taken at the beginning of operation (T0), clamping and cross-clamping of the inferior cava and portal vein (T1, T2), 90 minutes and 3 hours after reperfusion (T3, T4) and 24 hours after operation (T5).RESULTS:The level of PaO2/FiO2 in group A was lower than that in group B (P<0.05). The concentrations of TNF-alpha, IL-6 and IL-8 in the two groups increased rapidly at T2, peaked at T3, decreased rapidly after T3 until 24 hours after operation. The concentrations of TNF-alpha, IL-6 and IL-8 in group A were higher than those in group B at T2, T3, and T4 (P<0.05).CONCLUSION:After un-clamping of the inferior cava and portal vein, the serum concentrations of TNF-alpha, IL-6 and IL-8 increased may be related to pulmonary injury after hepatic ischemic reperfusion.
Clinical study on pleural lavage for chest infection after thoracotomy

Abstract:OBJECTIVE To investigate the clinical results of pleural lavage for chest infection after thoracotomy in order to find the most appropriate way to improve the overall level of clinical treatment .METHODS Totally 70 cases of pleural infection in patients after thoracotomy in Apr .2010 to Apr .2013 were randomly selected for the study and divided into two groups ,the control group of 35 cases receiving traditional pleural flushing ,and the observation group of 35 cases receiving pleural lavage .Differences in the clinical effects and laboratory test results after treatment were compared .The software SPSS16 .0 was used for statistical analysis .RESULTS The cure rate was 25 .71% in the control group and 45 .71% in the observation group ,the total effective rate was 74 .29% and 91 .43% respectively ,the difference was significant (P<0 .05) .The difference in time for body temperature to re‐turn normal ,hospital stay ,time for symptom improvement ,the recovery time of atelectasis ,disappeared time of pulmonary rales ,quality of life scores and laboratory parameters WBC ,C‐reactive protein ,TNF‐α,IL‐6 between the two groups was significant (P<0 .05) .CONCLUSION The treatment of pleural lavage for chest infection after thoracotomy showed significant clinical efficacy ,which can shorten treatment time and improve the prognosis lev‐el .

Abstract:To identify the causes of respiratory complications following liver transplantation (LT) and to discuss the management of these complications.One hundred and twenty four cases with pulmonary complications in the first two weeks after LT were identified among 163 patients admitted to the First Affiliated Hospital, College of Medicine, Zhejiang University from February, 1999 to March, 2003.The incidence rate of complications was 76%(124/163) with the total cure rate of 92%(114/124). The cure rates of the various complications were as follows: pleural effusion 100%(113/113), pneumonia 92%(76/83), respiratory insufficiency 91%(59/65), pulmonary hypertension 98%(101/103), pulmonary edema 98(58/59), atelectasis 100%(4/4) and pneumothorax 100%(2/2).To drainage the pleural effusion with an unicameral venous catheter is safety and effective. To cure or prevent pneumonia and atelectasis, aseptic manipulating, aspiration of sputum and keeping respiratory channel open were the key measurements of treatment. Restrictive ventilatory functional disturbance (RVFD) and dysfunction of ventilation are two major types of respiratory insufficiency in early stage of post-transplantation. The causes of pulmonary hypertension and edema are associated with pulmonary angiotasis and blood flow volume, and the vasodilator and diuretic often introduced in the therapy.
